Description

本発明は、長手の軸部を備え、穿孔部を軸部の前端に、回転式ねじ込み具との連結用の固定部を軸部の後端に備え、ガイドワイヤを受け入れるための軸心貫通孔を形成した骨用、特に近位大腿骨用の穿孔具に関する。
大腿骨、特に身体の中心に近い近位大腿骨の骨折の修復に、大腿骨頸部ピン、特に大腿骨頸部ねじを使用することが知られている。それらは、いわゆるポール（Ｐｏｈｌ）当て板あるいは固定くぎのいずれかとの組合せで導入される。前者の場合、骨に髄外固定される当て板は大腿骨頸部ねじを受け入れるスリーブを含む。後者の場合、固定くぎは大腿骨頸部ねじを受け入れる斜めの孔を持つ。
骨に大腿骨頸部ねじをしっかりと取り付けるために、大腿骨頸部ねじをねじ込む前に、通常、骨にコアねじ山径の程度の穿孔を行う。
そのような穿孔工程の前に、骨の穿孔軸心、したがって大腿骨頸部ねじの位置をＸ線操作で最初に決定する必要がある。これは通常、キルシュナーねじワイヤを用いて行う。正確な位置を決定した後、キルシュナーねじワイヤは骨内に残され、穿孔具用の、そしてその後で大腿骨頸部ねじのねじ込みのガイドとして用いられる。このために、穿孔具と大腿骨頸部ねじの両方に軸心孔が設けられる。
穿孔具は、適当な穿孔部を長手の軸部の前端に、回転式ねじ込み具との連結用の固定部を後端に備える。
しかしながら、このような構成を有する従来例の場合には、次のような問題がある。
すなわち、不利な状況の下では、比較的細いキルシュナーねじワイヤが、大腿骨頸部に導入される際に曲がってしまうことがある。したがって、キルシュナーねじワイヤの位置をＸ線操作でモニターすることが不可欠である。これによって、ねじれた部分が穿孔具または大腿骨頸部ねじの孔内で行き詰まって、ワイヤが股関節に不測に進入することを防ぐ。いずれにしても、これを回避する必要がある。
本発明の目的は、穿孔具を進めているときに、キルシュナーねじワイヤが孔内で行き詰まったかを、Ｘ線操作を使用せずに術者が判断することを可能にする、骨用、特に近位大腿骨用の穿孔具を提供することにある。
上記目的はクレーム１の特徴により達成される。
すなわち、本発明の穿孔具では、軸部が、軸心孔につながる開口を後部に備えている。好ましくは、軸心孔につながる開口が径方向に対向して２つ設けられる。
発明の他の観点によれば、開口は長手の形状で、軸部の軸心方向に平行に延びる。好ましくは、開口は軸部の固定部に近接して形成される。
骨の外にあって、穿孔工程時にチェックできる開口によって、術者は、穿孔具の前進中に穿孔具とキルシュナーねじワイヤとの間に相対移動が生じたかを確認することができる。相対移動がない場合、術者は、キルシュナーねじワイヤが穿孔具の孔内で行き詰まり、したがって別のものと取り替える必要があることがわかる。したがって、その少なくとも１つの開口は、ガイドワイヤまたはピンの位置を外科医が知ることができる観察窓である。
さらに、通常、穿孔具の骨内への進入深さを確認することから、長手の開口は、大腿骨頸部にねじ込まれたキルシュナーねじワイヤの長さを調べることを可能にする。穿孔具に対するキルシュナーねじワイヤの位置が限界まで変化すれば、穿孔工程を中止し、別のキルシュナーねじワイヤを導入する必要がある。
本発明に係る近位大腿骨用穿孔具によれば、穿孔工程時に軸部後部に備わる開口を介して、術者は、穿孔具の前進中に穿孔具とキルシュナーねじワイヤとの間に相対移動が生じたかを確認することができる。相対移動がない場合、術者は、キルシュナーねじワイヤが穿孔具の孔内で行き詰まり、したがって別のものと取り替える必要があることがわかる。
以下、図示の実施例を参照して発明をより詳細に説明する。
図１および図２で、穿孔具１０は長手の軸部１２を持ち、その前端には、詳細な説明を省略する２段階穿孔部１４が設けられている。穿孔具１０は、骨折した近位大腿骨の大腿骨頸部に穿孔するためのものである。軸部１２の後端には固定部１６が設けられている。軸部１２上で移動可能で、パチンとはまって位置決め可能なスライド１８が、所望の穿孔深さの設定に役立つ。スライド１８がはまり、穿孔深さを示す複数個の周溝２０が形成されている。スライド１８は図２では省略されている。
穿孔具１０は軸心貫通孔２２を有する。それは、図２中で部分的に示されるキルシュナーねじワイヤ２４を受け入れるためのものである。軸部１２の固定部１６付近の後部２６に、軸心方向に平行で径方向に対向する２つのスロットが形成され、そのうちの一方が２８で示されている。スロット２８は軸心孔２２と連通している。これによって、軸心孔２２内のキルシュナーねじワイヤ２４の端部３０がよく見える。したがって、穿孔工程の間、術者はキルシュナーねじワイヤ２４、つまりその自由端３０の相対位置をよく見ることができ、軸部１２がキルシュナーねじワイヤ２４に対して、あるいはそれと共に進められているかを確認できる。
１つの開口の代わりにいくつかの開口を設け、それらを順次、あるいは周方向にオフセット配置させることができる。

Claims (4)

  1. 長手の軸部を備え、穿孔部を軸部の前端に、回転式ねじ込み具との連結用の固定部を軸部の後端に備え、ガイドワイヤを受け入れるための軸心貫通孔を形成した骨用、特に近位大腿骨用の穿孔具であって、
    軸部（１２）が、軸心貫通孔（２２）につながる少なくとも１つの開口（２８）を後部（２６）に備え、
    前記ガイドワイヤが前記軸心貫通孔に受け入れられている場合に、前記開口（２８）を介して前記ガイドワイヤを見ることができることを特徴とする近位大腿骨用穿孔具。
  2. 軸心孔（２２）につながる開口（２８）が軸部（１２）の径方向に対向する側部にそれぞれ形成されていることを特徴とする請求項１に記載の近位大腿骨用穿孔具。
  3. 長手の、軸心方向に平行な１つの開口（２８）が設けられていることを特徴とする請求項１または２に記載の近位大腿骨用穿孔具。
  4. 開口（２８）が固定部（１６）に近接して形成されていることを特徴とする請求項１から３のいずれかに記載の近位大腿骨用穿孔具。
