【発明の属する技術分野】
本発明は、内視鏡の挿入部に着脱可能に装着されて体腔内等に挿入されるカバー式内視鏡に関するものである。

【従来の技術】
内視鏡は人体等の体腔内に挿入される関係から、使用の都度洗浄及び消毒を行なう必要があり、しかも完全に滅菌できるようにしなければならない。この洗浄,消毒は多大の時間と労力とを要することから、内視鏡、特にその挿入部をカバーで覆った状態で、体腔内等に挿入するようにした、所謂カバー式内視鏡が開発されている。カバー式内視鏡は、カバーを交換することによって、内視鏡を洗浄及び消毒をせずに再使用できる点で大きな利点がある。

内視鏡は、基本的には、体腔内の検査を行なうものであるから、最低限内視鏡観察機構として、体腔内を照明する照明部と、この照明部からの照明下で体腔内を観察する観察部とが挿入部の先端部分に装着される。また、体腔内を観察した結果、必要に応じて適宜の処置を施す場合もある。このために、内視鏡には鉗子等の処置具を挿通する処置具挿通路を備えているのが一般的である。さらに、吸引通路を設けて、体腔内の汚物や体液等を吸引除去するようにしたものもある。そして、これら処置具挿通路と吸引通路とを兼用すること、つまり処置具挿通路の基端部に吸引チューブを接続する構成としたものが多い。さらに、体腔内壁が汚れている等の場合には、生理食塩水等を供給して灌流を行なうことも必要となってくる。さらにまた、観察部が体液等で汚損された時には、この観察部に付着する汚損物を洗い流さなければならず、このために洗浄用流体の供給経路が設けられる。

以上のように、内視鏡には1乃至複数の通路が設けられるが、内視鏡検査時にはこの通路も汚損されることから、カバー式内視鏡として構成する場合には、前述した各通路は内視鏡側ではなく、カバー側に設けられる。このように、1乃至複数の通路を形成したカバーを有するカバー式内視鏡は、例えば、実開平6−66608号公報等に開示されている。この公知のカバー式内視鏡のカバーは、カバー外皮の内部に、内視鏡挿通チャンネルを設けた挿入部カバーと、複数のチャンネルを有するマルチルーメンチューブとを挿入することにより構成され、マルチルーメンチューブに設けた通路は鉗子チャンネルや送気経路及び送水経路として利用される。

ここで、これら挿入部カバー及びマルチルーメンチューブは、その断面形状が概略半円となっており、従って挿入部カバーに形成される内視鏡挿通チャンネルの断面も概略半円形状となっている。内視鏡挿通チャンネル内には内視鏡観察機構のみを備えた内視鏡の挿入部が挿入されるが、この内視鏡の挿入部も、内視鏡挿通チャンネルの断面形状に倣う形状となっている。また、挿入部カバーの先端は気密状態に保持されるようになっており、このために外皮カバーの先端にはカバー先端構成部が連結して設けられている。このカバー先端構成部には、内視鏡の挿入部の先端部分を固定的に保持する内視鏡先端部挿入孔が穿設されており、この内視鏡先端部挿入孔にはカバーガラスが装着されて、このカバーガラスにより内部が気密状態に保たれている。また、カバー先端構成部には、送気送水ノズルと、チャンネル開口部とが形成されている。

【発明が解決しようとする課題】
前述した従来技術の構成にあっては、内視鏡そのものは、使用の都度洗浄しなくても再使用できるが、内視鏡以外の機構は、当然、そのままでは再使用できない。しかも、内視鏡観察機構以外の機構は全てカバー側にあるので、その構成が極めて複雑になってしまう。このために、それを1回の使用の都度廃棄する、所謂使い捨てとするには、極めて高価なものとなってしまうという問題点がある。

また、内視鏡の挿入部はカバーにおける挿入部カバーの形状に依存することから、その断面形状を円形にすることができず、従って内視鏡とカバーとは対としてしか使用できず、つまり他のタイプ、例えば異なる通路形態を有するカバーに装着できるようになってはおらず、また内視鏡単独で体腔内に挿入するのに適したものとなっていない。さらに、内視鏡の挿入部は挿入部カバーからカバー先端構成部の内視鏡先端部挿入孔に固定されることになるが、挿入部の先端を安定的に保持するには、内視鏡先端部挿入孔にほぼ密嵌状態に嵌合させなければならない。内視鏡先端部挿入孔には、カバーガラスが装着されて、外部に対して気密状態に保たれているので、内視鏡先端部挿入孔内に挿入部の先端部分を挿入する際に、内部の空気を円滑に排除できないことがあり、挿入部の先端を確実に内視鏡先端部挿入孔の所定の位置にまで挿入する操作は極めて困難になり、かなりの手間と時間とが必要になる。内視鏡のカバーへの組み込み時に、挿入部の先端がカバーガラスに当接する位置まで完全に挿入されていないと、挿入部に設けた照明部及び観察部からカバーガラスまでの間に空気間隙が生じることになり、照明光がカバーガラスの表面に反射して、有害光として観察部に取り込まれることになり、鮮明な観察像が得られない。

ところで、内視鏡の検査時に必要な通路として、処置具を挿通する通路、吸引通路、灌流通路、観察部の洗浄通路等があるが、内視鏡検査時にこれら各通路は必ずしも全て必要とするとは限らず、検査の態様によっては通路を必要としない場合があり、また必要とする通路も異なってくる。例えば、処置具を使用しない場合には、処置具を挿通するための通路を設けなくても良いこともある。

本発明は以上の点に鑑みてなされたものであって、その目的とするところは、極めて簡単な構成で、必要に応じて所望の機能を持たせたカバーを容易に装着できるようにしたカバー式内視鏡を提供することにある。

【課題を解決するための手段】
前述した目的を達成するための第1の発明としては、照明部及び観察部を設けた先端硬質部を有する挿入部の基端部に本体操作部を連結して設けた内視鏡と、この内視鏡の挿入部に着脱可能に被挿される内視鏡カバーとを備えたカバー式内視鏡であって、前記内視鏡カバーは、硬質筒体に透明窓を装着した先端本体と、この先端本体に連結され、前記挿入部を覆う可撓性スリーブと、この可撓性スリーブの基端部に連結して設けた弾性部材からなる止着部とを備えたカバー本体部と、このカバー本体部の可撓性スリーブに装着した可撓性を有する通路形成部材とから構成され、前記内視鏡には、その挿入部と本体操作部との連結部に前記止着部が着脱可能に係止されるカバー係止部が設けられ、前記通路形成部材は可撓性チューブから構成され、その先端が処置具導出用開口として前記可撓性スリーブの先端に開口し、基端部を前記止着部に着脱可能に固定した処置具導入部材に接続され、前記処置具導入部材の側面に吸引チューブが接続され、またこの吸引チューブには吸引バルブが接続され、この吸引バルブは吸引機からの配管を、前記吸引チューブと接続する状態と、大気と連通する状態に切り換え操作が可能となし、前記処置具導入部材には、吸引時に着脱可能にシールする栓部材を装着する構成としたことをその特徴とするものである。

ここで、内視鏡の挿入部の断面形状は特に規制されないが、それ自体で単独に体腔内に挿入できるようにするには、断面円形のものが好適である。また、挿入部の全体を硬質の部材で形成しても良いが、軟性部及びアングル部を備えた軟性鏡として構成する方が望ましい。挿入部には照明部及び観察部が設けられるが、これら以外の機構は設けられていない。従って、その挿入部の細径化が図られる。この挿入部に装着される内視鏡カバーのカバー本体部は、基本的には、硬質筒体に透明窓を設けた先端本体に可撓性スリーブを装着し、この可撓性スリーブの基端部には、カバーを内視鏡に着脱可能に固定するための止着部を設けた簡単な構成のものであるから、この内視鏡カバーは1回限りの使用で、使い捨て用として好適である。

内視鏡カバーは、このカバー本体部を含み、かつ少なくとも1つの通路が設けられている。基本形態となるカバー本体部に他の通路が通路形成部材として付加的に設けられるものである。従って、カバー本体部における可撓性スリーブの内部は内視鏡の挿入部のみが位置し、通路形成部材は可撓性チューブとして、その外部に通設けられ、この通路形成部材の先端を先端本体に連結する。

通路形成部材の一例としては、先端本体に連結した可撓性チューブを備える構成としたものがある。そして、可撓性チューブの基端部には処置具導入部材を連結することによって、通路形成部材は処置具挿通路として使用できる。この処置具挿通路を設ける場合には、先端本体の硬質筒部には、処置具導出用の開口を設ける。また、処置具導入部材に吸引チューブを接続し、かつこの処置具導入部材の処置具導入口を着脱可能にシールする栓部材を装着すると共に、吸引チューブには吸引バルブを装着する。このように構成すると、可撓性チューブは処置具挿通路としての機能と吸引機能とを発揮する構成となる。可撓性チューブは少なくとも1本設けられるが、複数本の可撓性チューブを設けるようにしても良い。また、通路形成部材は、先端本体の硬質筒部に連結され、可撓性スリーブを囲繞するように設けた可撓性部材からなる外装スリーブで構成することもできる。この場合には、外装スリーブ自体またはそれと可撓性スリーブとの間を流体の供給通路となし、先端本体にこの供給通路に通じる流出用開口を形成する。これによって、灌流用や観察部の洗浄用流体等を流通させる通路とすることができる。

内視鏡カバーは、先端本体と、可撓性スリーブと、カバー本体部とを有する構成としたものが基本構成であるが、この基本構成となる内視鏡カバーに通路形成部材を設けたものとする。そして、通路形成部材は、処置具導入部材及び吸引チューブが接続されたものがあるが、これ以外にも液注入部材を接続したものもある。そこで、通路形成部材を設けない基本構成となる内視鏡カバーを第1の内視鏡カバーとし、処置具導入部材及び吸引チューブを接続した通路形成部材を有する内視鏡カバーを第2の内視鏡カバー、液注入部材を接続した通路形成部材を有する内視鏡カバーを第3の内視鏡カバーとして、これらを適宜交換して使用することができる。

このように構成することによって、検査態様に応じて必要な機能を有する内視鏡であって、当該の検査にとって不必要な機能を省略した内視鏡として構成される。つまり、内視鏡である以上、内視鏡観察機構は必須のものであるが、検査のみを行う場合には、最も簡単な構成を有する第1の内視鏡カバーを装着するようになし、処置具を用い、また吸引を行う必要があれば、処置具挿通路を備えた第2の内視鏡カバーを使用し、観察部の洗浄や灌流等の必要があれば、供給通路を設けた第3の内視鏡カバーを装着する。さらに、内視鏡単独でも体腔内に挿入することができる。ただし、このような使用態様を採ると、洗浄及び消毒を行なわなければ、再使用できないことから、この使用態様は緊急時のものに限定される。

【発明の実施の形態】
以下、図面に基づいて本発明の実施の形態について説明する。まず、図1に本発明のカバー式内視鏡を構成する内視鏡と、基本形態となる第1の内視鏡カバーとの全体構成を示し、また図2に内視鏡の挿入部の先端部分を示す。

図1(A)と図2とから明らかなように、内視鏡1は本体操作部2に体腔内への挿入部3を連設し、また本体操作部2からはユニバーサルコード4を引き出したものから構成される。挿入部3は先端側から順に先端硬質部3a,アングル部3b及び軟性部3cとで構成され、アングル部3bは本体操作部2に設けた操作ノブ5を操作することによって、少なくとも上下方向(または上下及び左右の4方向)に湾曲させることができる。

内視鏡1は、その挿入部3の先端硬質部3aの先端面に一対からなる照明部6及び観察部7が設けられている。照明部6にはライトガイドの出射端が臨んでおり、また観察部7には対物レンズが装着され、またこの対物レンズの結像位置には固体撮像素子(またはイメージガイド)が設けられている。これらにより内視鏡観察機構が構成されるが、内視鏡1の挿入部3としては、この内視鏡観察機構のみを装着した細径のものである。なお、この内視鏡観察機構及び挿入部全体の内部構成そのものは従来から周知であるので、その図示及び詳細な説明は省略する。

内視鏡1の挿入部3は、断面が円形であり、かつ内視鏡観察機構を設けた先端硬質部3aをアングル操作により所望とする方向に向けることができるようになっているので、それ自体単独で体腔内に挿入して検査を行うことができるものである。ただし、挿入部3を直接体内に挿入すると、その表面が汚損されることになるので、内視鏡カバーで挿入部3を覆うこといによって、この挿入部3が体腔内で汚損されないようにしている。

内視鏡カバーとして、図1(B)及び図3乃至図5に示した内視鏡カバー10は基本形態となるものであり、第1の内視鏡カバーである。この内視鏡カバー10の他にも、他の形態の内視鏡カバーとして、後述するように、第2の内視鏡カバー20及び第3の内視鏡カバー60を内視鏡1に装着できるようになっている。これら第2,第3の内視鏡カバー20,60は、実質的に第1の内視鏡カバー10に加えて通路形成体を有するものであり、従って第2,第3の内視鏡カバー20,60では、この第1の内視鏡カバー10に相当する構成は、カバー本体部となる。

而して、内視鏡カバー10は、図1に示したように、先端本体11と、可撓性スリーブ12と、止着部13とから構成され、図3に示したように内視鏡1の挿入部3に装着されるようになっている。先端本体11は、図4から明らかなように、硬質筒体14と、この硬質筒体14に気密構造となるように装着した透明窓としてのカバーガラス15とを含むものである。硬質筒体14は、金属等で構成することもできるが、材料の安価性、電気絶縁性等の観点から、プラスチック材で形成するのが望ましい。

可撓性スリーブ12は、曲げ方向の可撓性に優れ、また全体にわたって多少の伸縮性があるウレタン樹脂等の樹脂材またはゴム材等で構成される。なお、伸縮性は必ず全長にわたって必要とするのではなく、少なくとも基端側の部分が伸縮性のある部材で形成される。そして、内視鏡1の挿入部3を覆うためのものであることから、可撓性スリーブ12は比較的薄い膜状またはシート状のものでも構成できる。また、可撓性スリーブ12の内径は挿入部3の外径より多少大きくなっており、従って挿入部3は可撓性スリーブ12内に容易に挿入できるようになっている。

可撓性スリーブ12の基端部に設けた止着部13は、内視鏡1における本体操作部2と挿入部3との連結部分に固定するためのものであり、従ってこの止着部13は可撓性スリーブ12と比較すれば、その肉厚が十分大きい部材で形成される。止着部13は内視鏡1の本体操作部2と挿入部3との連結部に固定されるものであり、このために連結部において、本体操作部2側若しくは挿入部3側、好ましくは軟性部3cの本体操作部2への連結部を覆うカバーゴム3dの先端部に当接させるようにして硬質リング部材からなるカバー係止部8が装着されている。

止着部13とカバー係止部8との係止構造については、例えば、図5に示したように、内視鏡1側に設けたカバー係止部8の外面に形成した円環状の係止突条8aと、止着部13に設けた係止溝13aとから構成され、係止溝13aは係止突条8aにスナップアクション作用により着脱可能に連結されるようになっている。従って、止着部13は弾性のあるプラスチックやゴム等の部材で形成される。なお、この止着部13の内視鏡1への係止機構の構成はこれ以外にも、例えばバイオネット方式、ルアーロック方式等種々のものを採用することができる。要は、止着部13は内視鏡1の本体操作部2と挿入部3との連結部乃至その近傍に着脱可能に連結され、かつ連結状態で固定的に保持できる構成であれば良い。

ここで、図1に示したように、内視鏡1の挿入部3における先端硬質部3aの先端面からカバー係止部8の係止突条8aまでの長さをL1とし、また内視鏡カバー10の全長、つまりカバーガラス15の内面から止着部13における係止溝13aまでの長さをL2としたときに、L1の方がL2より長くなっている。ただし、この寸法差は数%程度とする。

これによって、内視鏡カバー10を内視鏡1の挿入部3に被着させると、この内視鏡カバー10の可撓性スリーブ12が引っ張られた状態となり、そのカバーガラス15は挿入部3の先端硬質部3aの先端面に圧接されて、その間に空気間隙が生じない状態となる。その結果、照明部6からの照明光がカバーガラス15の内面で反射して、観察部7に有害光として入り込むことはない。また、前述した寸法差は数%程度であるので、可撓性スリーブ12は、先端硬質部3aとカバーガラス15とを密着させるのに必要最小限の張力が作用するが、挿入部3における曲げ方向の可撓性が大きく低下することはなく、また特にアングル部3bを湾曲操作する際に、格別の支障を来たすことはない。

内視鏡1により体腔内の検査を行なうに当って、特に治療等を施さず、単に体腔内の観察をする場合には、この内視鏡1の挿入部3にはこの通路を有さない簡単な構成の内視鏡カバー10が被着される。

内視鏡カバー10を構成する止着部13はカバー係止部8に係止されるものであり、このカバー係止部8の外径は挿入部3の外径より大きくなっている。また、可撓性スリーブ12及びその先端に連結した先端本体11の内径は、挿入部3の外径より多少大きくしている。従って、内視鏡1の挿入部3は止着部13側から内視鏡カバー10内に挿入して、この挿入部3の先端硬質部3aの先端面をカバーガラス15の内面に当接させることができる。この状態で、可撓性スリーブ12を止着部13側に手繰り寄せるようにして、止着部13の先端部をカバー係止部8における係止突条8aを乗り越えさせる。ここで、止着部13は弾性部材で形成されているので、この止着部13を強制的に拡開させれば、係止突条8aを乗り越えることになる。そして、止着部13の内面に形成した係止溝13aを係止突条8aに嵌合させる。これによって、内視鏡カバー10を容易に、しかも迅速に内視鏡1に組み込むことができる。

この状態で、内視鏡1の挿入部3を体腔内に挿入するが、この挿入部3は内視鏡カバー10で完全に覆われており、かつこの内視鏡カバー10は気密構造となっているので、挿入部3は体液等により汚損されるおそれはない。また、挿入部3と可撓性スリーブ12との間に空気層が介在しているが、その間の径差は挿入部3が可撓性スリーブ12内に挿入する操作を円滑に行なうのに必要最小限のものに抑制できるので、また止着部13がカバー係止部8に係止されると、可撓性スリーブ12に張力が作用することになるので、その間の空気層は極めて僅かなものとなり、挿入部3の挿入操作性に支障を来たさない。また、可撓性スリーブ12に張力が作用しているものの、この張力は比較的弱いものであるから、挿入部3における軟性部3cの可撓性及びアングル部3bの湾曲性を実質的に損なうおそれはない。

挿入部3の先端硬質部3aはカバーガラス15に接合されているが、この先端硬質部3aの外径は硬質筒体14の内径より小さいことから、硬質筒体14内で先端硬質部3aが動き得る余地がある。しかしながら、可撓性スリーブ12の張力の作用で先端硬質部3aとカバーガラス15とを密着させているので、先端硬質部3aが硬質筒体14内でみだりに動くことはない。特に、先端硬質部3aとカバーガラス15との間に水等の液膜を形成しておけば、この液膜が実質的な接着機能を発揮するようになる結果、先端硬質部3aの固定性がさらに向上する。

内視鏡カバー10を装着した状態で、内視鏡1による体腔内の検査が終了すると、内視鏡1の挿入部3が体腔内から取り出されるが、勿論内視鏡カバー10で覆われた状態のまま体外に取り出される。その後、内視鏡カバー10の止着部13をカバー係止部8から脱着して、この内視鏡カバー10を引き出すことによって、使用済みで、体液等で汚損されている内視鏡カバー10が挿入部3から分離される。この状態で、体腔内に挿入したにも拘らず、内視鏡1は汚損されていない。従って、使用後に内視鏡1を洗浄したり、消毒したりすることなく、再度の使用が可能となる。

ところで、内視鏡検査の種類や態様によっては、内視鏡観察機構だけではなく、通路を必要とすることがある。この通路の代表的なものとしては、処置具を挿通するための処置具挿通路、吸引通路や、洗浄液や薬液等の流体供給用の通路である。一般的な内視鏡においては、処置具挿通路と吸引通路とは共用されるものである。そこで、図6乃至図12に処置具の挿通及び体液等の吸引を行なうための通路を供えた他の内視鏡カバーとして、内視鏡カバー20の構成について説明する。この内視鏡カバー20が第2の内視鏡カバーである。

第2の内視鏡カバー20は、基本構造、つまり内視鏡1の挿入部3が挿入されるカバー本体部として、第1の内視鏡カバー10の実質的に全ての構成を備えている。即ち、内視鏡カバー20は、先端本体21と、可撓性スリーブ22及び止着部23を備え、これらによりカバー本体部が構成される。そして、先端本体21は硬質筒体24とカバーガラス25とを含み、止着部23には係止溝23aが形成されている。また、カバーガラス25の内面から止着部23の係止溝23aまでの長さは第1の内視鏡カバー10と同様の寸法、つまりL2となっている。

第2の内視鏡カバー20の先端本体21における硬質筒体24は、図7及び図8に示したように、カバーガラス固定部24aと通路支持部24bとから構成される。通路支持部24bには、処置具導出用開口として機能する透孔26が穿設されており、かつチューブ嵌合用筒部27が基端側に向けて延在されている。そして、チューブ嵌合用筒部27には通路を形成する可撓性チューブ28の先端が嵌合固定されている。これらにより通路形成体が形成され、可撓性チューブ28は鉗子等の処置具を挿通する処置具挿通路として機能するものである。このために、曲げ方向に可撓性を有し、かつ耐潰性に優れ、かつ内面が滑りの良いチューブ材から構成される。この可撓性チューブ28は可撓性スリーブ22に沿って基端側に延在されており、その基端部には処置具導入部材29が連結して設けられている。処置具導入部材29は、例えば図9に示したように構成される。

即ち、処置具導入部材29は本体ケーシング30を有し、この本体ケーシング30の先端にはチューブ連結部31が設けられており、可撓性チューブ28の基端部はこのチューブ連結部31に嵌合・固定されている。本体ケーシング30の先端部は開口しており、この先端部にはゴム等の弾性部材からなる処置具ガイド筒32が取り付けられている。この処置具ガイド筒32の途中には、逆止弁33が設けられて、処置具を挿通させたときにおける体内から、体液等の逆流を防止している。

処置具導入部材29の本体ケーシング30には連結パイプ34が接続されており、この連結パイプ34には吸引チューブ35が接続されている。従って、可撓性チューブ28及び処置具導入部材29は処置具の挿通路に加えて吸引通路としても機能するものである。そして、吸引通路として機能させる場合には、処置具ガイド筒32の先端を閉鎖する必要がある。このために、処置具ガイド筒32に一体に設けた連結帯36の先端に栓部材37を連結して設けている。従って、この栓部材37を処置具ガイド筒32の先端に嵌着することによって、処置具導入部材29の内部が密閉される。

処置具導入部材29は、止着部23に固定的に保持される。このために、図10に示したように、止着部23にはホルダ38が一体的に設けられている。止着部23は弾性部材であり、従ってホルダ38も弾性を備えている。このホルダ38は概略C字形状をしたものであり、処置具導入部材29を弾性的に抱持することにより固定的に保持される。ホルダ38はC字形状をしており、そのスリット部は横向きとなっており、処置具導入部材29をホルダ38に装着する際には、連結パイプ34に嵌合させた吸引チューブ35がスリット部に配置されるようになっている。従って、スリット部の間隔は少なくとも吸引チューブ35の外径より広くなっている。さらに、好ましくは可撓性チューブ28の外径より大きくする。

以上説明したように、本発明は、内視鏡と、この内視鏡の挿入部に装着可能で、内視鏡観察機構以外で、検査に必要であり、かつ十分な機能を有する通路を持った内視鏡カバーとから構成したので、極めて簡単な構成で、必要に応じて所望の機能を持たせたカバーを容易に装着できる等の効果を奏する。
