本発明は、股関節症、特に脱臼性股関節症による障害を緩和するための保存的治療或いはその予防のための装具の構造に関するものである。

【従来の技術】
我が国における変形性股関節症は、基礎疾患に起因する2次性のものが90%に達すると言われており、しかもその70%以上が脱臼性股関節症である。これは、歩行や立居振舞で生じる運動時痛、可動制限、跛行を症状とし、関節軟骨の退行変性と反応性の骨増殖が特徴となっている。中でも患者にとって最大の問題は「疼痛」であると言って良く、股関節に発生する痛みを回避しようとして、身体各部位に支障が波及してしまい、臀部痛や大腿部痛、時には膝上部痛を訴えることも稀ではない。また、股関節の運動制限や跛行なども、この疼痛の増強と共に強まる傾向がある。そして治療には保存的治療と手術的治療とがあり、年齢、症状の進行度、両側罹患の有無、或いは職業や性等を勘案して決定される。

このうち保存的治療は、高齢その他により手術的治療が好ましくない又は進行度が鈍化しているという場合にも行なわれるが、概ね軽度の障害に対してなされる治療である。基本的には、疼痛を和らげるための装具装用を続けながら、適当なリハビリテーション(例えば側臥位及び背臥位での下肢挙上訓練等)を施して筋力の増強を図ることが多い。

【発明が解決しようとする課題】
ところが現在、股関節に装用する装具には効果的なものがなく、治療効果(障害の改善効果)はおろか、その第一の目的である疼痛の軽減ですら満足に果たせない装具が多かった。これは、股関節は立位と座位とで形状が大きく異なるがその形状変化に追従しつつ適切な保持力を維持するのが困難であること(後述)、用便の際に邪魔になることが多いこと、炎症性疾患ではないので疼痛は安静時には発生せず動きはじめた時に強く出現することが多くそうした者にとっては装着動作それ自体が苦痛であること、金属や硬質プラスチックを用いた装具を装用すると嵩張り過ぎて装用感も見た目も悪いこと、等々が原因であると考えられる。従って、装具装用は忌避されがちとなり、歩行時には杖(場合によっては松葉杖)を使用し疼痛出現時には鎮痛剤を投与するだけといった、およそ関節症自体の改善にはほとんど寄与しない方法で対処することが多かった。

股関節は立位と座位とで形状が大きく異なるがその形状変化に追従しつつ適切な保持力を維持するのが困難である、という点について詳述する。股関節痛用装具は基本的には腰痛におけるサポータやコルセットと同様の形態、即ち体躯を周回するベルト状のものであり、左右の大転子を締めつければ関節痛は軽減されることになる。但し、腰痛におけるサポータやコルセットの締めつけ箇所が股関節の伸展屈曲に際して大きく形状が変わらないのに対し、左右の大転子を覆いながら体躯を周回する股関節用装具は、前方は恥骨下部付近、後方は臀部後端付近を通過することになり、股関節屈曲時、最も折曲するのがこの装具の前部、最も伸長するのがこの装具の後部ということになる。従って、腰痛用装具と同様の構造で股関節用装具を製作しそれを装用した場合には、椅子に腰掛けるような姿勢をとる度に前部が折曲され劣悪な装用感を味わうことになる。しかしこの欠点を、柔軟で薄く伸縮性に優れた材質のものを採用することで解消しようとすると、当然ながら締めつけ力が弱くなり、効果を上げることはむずかしい。

【課題を解決するための手段】
そこで本発明者は、上記諸点に鑑み鋭意研究の結果遂に本発明を成したものでありその特徴とするところは、尻当て部とベルト部とにより成る伸縮性のあるシート状軟性装具であり、該尻当て部は下側面が張り出しており、且つ該張り出し部分の少なくとも下端付近が湾曲面を形成しており、また該ベルト部は該尻当て部の左右端からそれぞれ二本ずつ延出するベルト本体と、該各ベルト本体の先端に取設される掛止具とにより形成される点にある。

即ち本発明装具は、尻当て部を臀部に当てて両端のベルト部を引張しながら体躯前方側に周回させて固定する股関節症用装具である。ベルトは左右それぞれに二本ずつ存在しているので、股関節が屈曲位にある時に最も折曲する部分である前側腿つけ根付近が開放されることになる。従って、装用した状態で椅子に腰掛けても窮屈感はさほど感じなくて済む。また、尻当て部の下側面が張り出した上で湾曲面を形成しているので下着の上に本発明装具を装着し、その上から上着を着た場合に、身体のラインに沿って装着されているので余り目立たず、また椅子から立ち上がった時に、伸長していた尻当て部が元に戻ろうとする際この湾曲部が存在することによって装具の上方へのズレはほとんどない。従って、一旦装着した後には「締め直し」がほぼ不必要になる。

例えば、平坦な布を裁断して尻当て部をする場合、該尻当て部の下側面を湾曲させるために、下端を一乃至数箇所V字状に切除したのちその切欠部分を縫合すること、周回させたベルトを固定するために面ファスナーを取設すること、等がこの加工に相当する。その他、日常動作で装具がずれて治療効果が損なわれたり着け心地が悪くなるのを防止するために防滑手段を付与するための加工を施しても良い。この防滑手段は大転子先端に当たる箇所付近に設けるのが良く、ここにシリコーン樹脂を防滑パッドとして設けるのが効果的である。その場合、この防滑パッドは、溶融シリコーン樹脂を塗布して硬化させるという手法を採用しても良いし、別部材として形や厚さの違うパッドを幾種類か準備しておき適当なサイズのものを掛止して症状に合わせることができるようにしても良い。

【発明の実施の形態】
以下図面に基づき本発明を更に詳細に説明する。

図1は、本発明に係る股関節症用装具1(以下本発明装具1という)の一例を示すものであり、本発明装具1は、尻当て部2と呼ぶ部分と、その両端よりそれぞれ長手方向に延出する一対ずつ(即ち4本)のベルト3とにより構成される概ね偏平なシート状体である。4本の中の2本のベルト3の先端付近には面ファスナー4が縫合されている。なお本例の場合尻当て部2とベルト3とは別部材ではなく、一枚の布地を裁断縫合して成したものである。使用した布は、少なくとも一面を起毛させたポリプロピレン繊維糸のニット製品(商品名「パイレン」:東レ株式会社製)であるが、これはわずかな伸縮性があって、強靱であり、且つ、市販されている通常の面ファスナーの雄側を起毛面に接触させると簡単確実に係止できるので本発明にとって好適な材料だと言える。
