Description
【産業上の利用分野】
本発明は、脊柱から離れた椎骨もしくは椎部分のためのスペース保持部材として脊柱の椎体間に装入するための生体埋め込み部材で、隣接する椎骨に据え付けるために配置される2つの埋め込み部材末端部分と、該部分の間にある埋め込み部材中央部分とより成り、該中央部分は、末端部分にそれぞれ1つのネジ山によって接続されてるものに関する。ここで、両ネジ山は脊柱の縦方向において共軸に配設されていて、互いに逆方向のネジ山方向を有していて、その結果、埋め込み部材中央部分の回転によって、生体埋め込み部材の長さ全体が、変更可能である。

【従来の技術】
前記の如き生体埋め込み部材が米国特許明細書第4657550号より公知であり、埋め込み部材中央部分の回転によって、生体埋め込み部材を含む脊柱領域の伸延を可能にする。

公知の生体埋め込み部材においては、埋め込み部材の両方の末端部分がそれぞれ2部分に構成されていて、それぞれは、隣接する椎骨に据え付けられ、椎骨へと向かって針部を有する押圧プレート、および、ネジ山ボルトから成っている。該ボルトは、多角頭部でもって、ボルト軸周りの回転を防ぎ、押圧プレート中の対応する受入部中に差し込まれている。埋め込み部材中央部分は、ネジ山ボルトの受け入れのためのネジ山穴ぐりを備えて構成されている。前記の公知の生体埋め込み部材は、生体埋め込み部材の装入後の骨形成及び骨再成を望まれるというままに残しておくという欠点を有している。ドイツ特許DE3435771A1号からは、前記欠点を回避する生体埋め込み部材が公知であり、該生体埋め込み部材は、多くの穴を備えているが、前記の如き生体埋め込み部材は、1つの椎骨もしくは椎部分のためのスペース保持部材としては不適である。

ヨーロッパ特許EP0268115B1号において、1つの椎骨のためのスペース保持部材として働く生体埋め込み部材が記されていて、該生体埋め込み部材は、壁部中に穴を備えたカバー形成部材として構成されていて、その際、上方及び下方のカバー縁部は、接続されるべき椎骨間の回転不能の接続を生じるために、少なくとも部分的に歯状に構成されている。前記部材は、穴を備えた金属シートより、一層もしくは二層に巻いて作られている。穴を通って、所望の場合、骨セメントもしくは骨断片の如き、カバー内部に装入されるべき物質が充填されることができ、このことによって、生体埋め込み部材との、より迅速な血管接続が達成される。前記の如き生体埋め込み部材において不都合であるのは、該部材が、隣接する椎骨の伸延を全く可能としない点である。

生体埋め込み部材が骨形成の誘導及び骨の刺激にとって特に適していて、従って埋め込み後に特に容易に、かつ、迅速に根付くことができるように冒頭に述べた如き生体埋め込み部材を構成するという課題が本発明の根底を成している。

【課題を解決するための手段】
本発明によると前記課題は、冒頭に述べた如き構成要件を備えた生体埋め込み部材において、埋め込み部材の3つの部分全てが、少なくともネジ山の領域が円形断面の管状スリーブとして構成されていて、互いに共軸にネジ締めされていること、及び、スリーブ壁が開口によって穴をあけられていることによって解決される。

3つのスリーブより成る生体埋め込み部材の、本発明に則した発展は、生体埋め込み部材内部に、通り抜ける中空空間を生じさせ、該空間は、スリーブ壁中にある開口を通り抜けて至る所で生体埋め込み部材外側につながっている。このことは、骨セメントまたは、患者自身もしくは他人の骨くずを前記中空空間に満たす可能性と併せて、生体埋め込み部材を迅速に、かつ、確実に、根付かせる。それにもかかわらず、手術の間の生体埋め込み部材の伸延の可能性は充分に維持される。

本発明による生体埋め込み部材の好ましい実施態様は、埋め込み部材末端部分が埋め込み部材中央部分に外側から軸方向に重ねられ、その際、ネジ山がそれぞれ、埋め込み部材中央部分上では雄ネジとして埋め込み部材末端部分中では雌ネジとして構成されていることを特徴としている。前記配設の結果、ネジ山の外側は十分に埋め込み部材末端部分によって覆われている。埋め込まれた状態において埋め込み部材の部分の相互の回転を阻止するために、埋め込み部材末端部分の少なくとも1つがネジ山領域において、半径方向のネジ穴を有していてよく、該ネジ穴中を、埋め込み部材中央部分に対して締付可能な締付ネジが案内されていて、該締付ネジは締め付けられた状態において埋め込み部材の両方の部分を互いに固定する。締付ネジの領域においてスリーブ壁を固定させるためには、締付ネジを備えた埋め込み部分が軸方向において内側に位置するスリーブ縁に、残りのスリーブ部分に比べるとより大きな壁厚のリングフランジを有していて、該リングフランジ中に締付ネジ用ネジ穴が配設されていることが好ましい。ネジ穴中の締付ネジの十分に大きな案内長を得るために、リングフランジがネジ穴の領域において、半径方向に外側に突出する突起を備えているのが合目的である。

主に、埋め込み部材中央部分が自身の両ネジ山の間に、ネジ山のないスリーブ部分を有していて、該スリーブ部分中で開口がスリーブ周囲に渡って均一に分配されていて、埋め込み部材中央部分の回転に役立つスパナの差し込みのためのスパナ開口として構成されている。

埋め込み部材中央部分のネジ山のないスリーブ部分の外側での開口の構成及び配設に関して、本発明の枠内で、ある選択の自由がある。一方で、開口全体の横断面形状ができる限り大であり、他方では、開口は、埋め込み部分の間のネジ山接続の確実性が損われないよう、ネジ山領域を許容できないくらいに小さくしてはならない。前記全ての要求を特に好都合に満たしていて従って本発明の枠内で好ましい実施態様は、開口が埋め込み部分中該部分のネジ山領域中に備えられていて、スリーブ周囲方向に向けられた開口の長軸をもつ長穴として構成されていることを特徴としている。合目的には、開口は、自身の開口の長軸の方向に、スリーブ軸に垂直な平面に並べられている。たいていの場合、各ネジ山領域毎に、1つより多くの穴の列が備えられる。そこで、同一のネジ山領域中の幾つかの穴の列において、隣接する開口が周方向において合目的には互いに位置をずらされている。前記の如くに位置をずらされた開口においては、一つの列の開口は自身の開口の中心が、主に、隣接する列のそれぞれ連続する2つの開口の間の中央に位置している。更に、開口の長軸が、それぞれの列において連続する開口の間の縁の距離よりも長く、特に2倍より長いように、かつ、隣接する列に配設された開口の間の軸方向の縁の距離が、開口の短軸の長さより小さく、特に約半分であるように、配設がなされていることが好都合である。前記の如き開口配設の場合、隣接する開口の間に、十分に大きくて、しっかりとつながっているネジ山領域が残っており、該領域は、埋め込み部分間の荷重伝達を確実に保証する。

一般に、生体埋め込み部材と、隣接する椎骨との間の力の伝達は、埋め込み部材端部の、椎骨に据え付けるスリーブ縁を介してのみ可能である。しかしながらその際、椎骨における応力が余りに大きくなるならば、スリーブ縁を拡大するか、もしくは、開口によってできるだけ穴をあけられている横板で全体的に覆う可能性が存在する。更に、椎骨に据え付けるためのスリーブ縁は基本的には一平面中に配置され、その平面は個々の場合の必要に応じてスリーブ軸にほぼ垂直に、もしくは、スリーブ軸に傾斜した方向に配されていてよい。しかしながらもちろん、スリーブ縁が、1つの平面からそれた面に配置されることも可能である。

更に、本発明の枠内で、埋め込み部材端部の少なくとも1方の軸方向に外側の縁が、1つもしくは幾つかの、隣接する椎骨中に入り込むために配置された刃部もしくは尖端を備えていることによって、隣接する椎骨における埋め込み部材末端部分の望ましくない移動を防ぐ可能性が存在する。個々において、種々の構成が可能である。そこで、刃部は、周方向において、縁に沿って、連続してもしくは断続して配置されることができる。しかしながら、刃部は、半径方向においても、縁に沿って次々に設けられた突起の端部として構成されていてよい。最後に、尖端は、縁に沿って配設され軸方向に整列されたピンとして形成されていてよい。刃部もしくは尖端が、隣接する椎骨中に深く入り込みすぎることを回避するために、埋め込み部分に、刃部もしくは尖端の入り込む深さを制限し、半径方向に内側もしくは外側へと突出するリングウエブが備えられていることが好ましく、該ウエブは、隣接する椎体に対するストッパーとして働くことができる。該リングウエブの半径方向において外側の縁部面は、ほぼ軸方向に向けられていて凸面の稜部を備えていてよく、該縁部は生体埋め込み部材の確実なセッティングに寄与する。

【実施例】
以下において、図面に描かれた実施例を用いて本発明が詳細に説明される。

図示された生体埋め込み部材は、図面中にそれ自体は描かれていない脊柱の椎体の間に装入するために、脊柱から離れた椎骨もしくは椎部分のためのスペース保持部材として働く。生体埋め込み部材は、隣接する椎骨に設置するために配置される2つの埋め込み部材末端部分1と、該部分の間にある埋め込み部材中央部分2とより成っている。該埋め込み部材中央部分はそれぞれ1つのネジ山3,4によって埋め込み部材末端部分に接続されていて、その際、両ネジ山は脊柱の縦方向において共軸に配設されていて、特に図2より明らかなように互いに反対のネジ方向を有している。このことによって、両方の埋め込み部材末端部分1に対する埋め込み部材中央部分2の相対的な回転の際に、生体埋め込み部材全体の長さが変えられることができ、その際、特に、生体埋め込み部材を備えた脊柱領域が埋め込みの際に伸延されることができることが達成される。

埋め込み部材末端部分1が、軸方向において埋め込み部材中央部分2の外側上を覆い、その際、ネジ山3,4はそれぞれ、埋め込み部材中央部分2上で雄ネジとして、埋め込み部材末端部分1中で雌ネジとして構成されている。埋め込み部材の両方の末端部分1は更に、ネジ山領域に、半径方向のネジ穴5を有していて、該ネジ穴中を、埋め込み部材中央部分に対して締付可能で、図面においては見通しを良くするためにそれ自体は不図示の締付ネジが案内されている。該締付ネジのそれぞれの埋め込み部材中央部分2に対する締付によって埋め込み部材の部分1,2は相互に回転しないよう固定される。その際、埋め込み部材末端部分1が、それぞれ軸方向において内側に位置するスリーブ縁に、残りのスリーブ部分に比べて大きな壁厚のリングフランジ6を有していてよい。そこで締付ネジのためのネジ穴5が前記リングフランジ6中に配設されていて、該リングフランジは、埋め込み部材端部を、締付ネジの締付の際に生じる力に対して安定なものとする。又、前記リングフランジ6は、ネジ穴5の領域において、半径方向において外へと突出する突起7を与えられてもよく、その結果、ネジ穴5中において、締付ネジのための十分な大きさの案内長が使われる。

埋め込み部材中央部分2が自身の両方のネジ山3,4の間にネジ山のないスリーブ部分8を有している。該ネジ山のないスリーブ部分8中において、開口9がスリーブ周に渡って均一に分配されていて、かつ、埋め込み部材中央部分2の回転に役立つ不図示のスパナの差し込みのためのスパナ開口として構成されている。実施例においては、該スパナ開口が、円形シリンダ状の穴の形を有していて、その結果、スパナは特に簡単な実施態様において、単に、差込端部において円形シリンダ状ロッドによって形成されていてよい。

埋め込み部材の3つの部分1,2中の他の全ての開口10は該埋め込み部材のネジ山3,4の領域中に備えられていて、スリーブ周方向に向けられた開口の長軸11を備えた長穴として構成されている。開口10はその長軸11の方向において、スリーブ軸に垂直な平面に並べられている。図1及び図2の実施例においては、それぞれ同一のネジ山領域3,4中に、2つの前記の如き穴の列があり、他方、図4〜図12の実施例においては、少なくとも3つの穴の列が備えられている。その際、隣接する列の開口10が周方向において互いに位置をずらされていて、その際、詳細には、それぞれ1つの列の開口は、自身の開口の中央が、隣接する列のそれぞれ2つの連続する開口の間の中央に位置するように配設がなされている。開口10の開口の長軸11は、それぞれの列中で連続する開口の間の縁の距離の2倍よりも大である。隣接する列中に配設された開口の間の軸方向の縁の距離13は、開口の短軸14の長さの約半分である。

図1、図4、及び、図10の実施例において、埋め込み部材末端部分1を隣接する椎骨に据え付けるために配置される、軸方向において外側の縁15が、スリーブ軸に垂直な平面中に配置される。それに対して図6の実施例においては、前記の縁15は、スリーブ軸に対して傾斜させられた平面中に配置され、その際、実施例において傾斜角は10度である。前記の軸方向において外側の縁15は、図1、図10、及び、図12の実施例における如く、刃部を備えているか、もしくは、図4及び図6における如く、縁15に沿って、次々に設けられた突起16を備えていてよく、その際、刃部ないしは突起は、該刃部ないしは突起が隣接する椎骨中に程度の差はあれ深く入り込むことによって、埋め込み部材末端部分1の、隣接する椎骨に据え付けた状態で横への位置のずれもしくは移動を防ぐという目的を有している。突起16も、自身に刃部を備えていてよく、該刃部は、椎骨中への突起の入り込みを容易にさせるために周方向、もしくは、周方向に交差した方向に配される。その際、入り込みの深さを制限するために、図1の実施例における如く、下方の埋め込み部材末端部分1に半径方向において外側へと突出するリングウエブ17が備えられていてよく、該リングウエブは、隣接する椎骨に対するストッパーとして働く。リングウエブ17の外側の縁の面は軸方向に向けられており、外側へと突出する、即ち、凸面の稜部18を備えている。

【発明の効果】
3つのスリーブより成る生体埋め込み部材の、本発明に則した発展は、生体埋め込み部材内部に、通り抜ける中空空間を生じさせ、該空間を、スリーブ壁中にある開口を通り抜けて至る所で生体埋め込み部材外側につながる様にしている。このことは、骨セメントまたは、患者自身もしくは他人の骨くずを前記中空空間に満たす可能性と併せて、生体埋め込み部材を迅速に、かつ、確実に生体に根付かせる。それにもかかわらず、手術の間の生体埋め込み部材の伸延の可能性は充分に維持される。

Claims (17)
- 脊柱から離れ出た椎骨もしくは椎骨部分に代わるスペース保持部材として、脊柱の椎体間に装入するための生体埋め込み部材であって、隣接する椎骨に据え付けるために配置される2つの埋め込み部材末端部分(1)と、該埋め込み部材末端部分の間にある1つの埋め込み部材中央部分(2)とより成っており、該埋め込み部材中央部分は、埋め込み部材末端部分(1)とそれぞれ1つのネジ山(3,4)で接続され、両ネジ山(3,4)は脊柱の長手方向に対して共軸に配され、互いに逆のネジ山方向を有しており、その結果、埋め込み部材中央部分(2)の回転によって、生体埋め込み部材全体の長さが変更可能であるというものであって、
埋め込み部材の3つの部分(1,2)全てが少なくともネジ山(3,4)の領域において円形の横断面を有する管状のスリーブとして構成されていて、互いに共軸にネジ締めされており、

- 請求項2に記載の生体埋め込み部材において、埋め込み部材末端部分(1)における締付ネジを備えた部分が、軸方向内側に位置するスリーブ縁に、残りのスリーブ部分に比べるとより大きな壁厚のリング状カラー部(6)を有していて、締付ネジのためのネジ穴(5)が該リング状カラー部(6)中に配設されていることを特徴とする生体埋め込み部材。
- 請求項3に記載の生体埋め込み部材において、リング状カラー部(6)がネジ穴(5)の領域に、半径方向に外側へと突出する突起(7)を有していることを特徴とする生体埋め込み部材。
- 請求項1〜4のいずれかに記載の生体埋め込み部材において、埋め込み部材中央部分(2)が自身の両方のネジ山(3,4)の間に、ネジ山のないスリーブ部分(8)を有していて、該スリーブ部分中には、開口(9)が、スリーブ周囲に渡って均一に分布していて、埋め込み部材中央部分(2)の回転のために供する、スパナの差し込みのためのスパナ開口として構成されていることを特徴とする生体埋め込み部材。
- 請求項1〜5のいずれかに記載の生体埋め込み部材において、開口(10)が埋め込み部分(1,2)中、該部分のネジ山(3,4)の領域に備えられていて、スリーブ周方向に開口の長軸(11)をもつ長穴として構成されていることを特徴とする生体埋め込み部材。
- 請求項6に記載の生体埋め込み部材において、開口(10)が開口の長軸(11)の方向に、スリーブ軸に垂直な平面に、並べられていることを特徴とする生体埋め込み部材。
- 請求項7に記載の生体埋め込み部材において、同一のネジ山領域(3,4)中に幾つかの穴の列がある場合、隣接する列の開口(10)が周方向において互いに位置をずらされていることを特徴とする生体埋め込み部材。
- 請求項8に記載の生体埋め込み部材において、位置をずらされた開口(10)においては、一つの列の開口は自身の開口の中央が、隣接する列のそれぞれ連続する2つの開口の間の中央に位置することを特徴とする生体埋め込み部材。
- 請求項7〜9のいずれかに記載の生体埋め込み部材において、開口(10)の長軸(11)が、それぞれの列中で連続する開口の間の縁の距離(12)より長く、特に2倍より長いことを特徴とする生体埋め込み部材。
- 請求項7〜10のいずれかに記載の生体埋め込み部材において、隣接する列に配設された開口(10)の間の軸方向の縁の距離(13)が、開口の短軸(14)の長さより短く、特に約半分であることを特徴とする生体埋め込み部材。
- 請求項1〜11のいずれかに記載の生体埋め込み部材において、少なくとも一方の埋め込み部材末端部分(1)の、軸方向の端縁(15)が、スリーブ軸に対して傾斜させられた平面中に配置されることを特徴とする生体埋め込み部材。
- 請求項1〜12のいずれかに記載の生体埋め込み部材において、少なくとも一方の埋め込み部材末端部分(1)の、軸方向の端縁(15)が、隣接する椎骨中への進入のために配置される、1つもしくは幾つかの刃部もしくは尖端を備えていることを特徴とする生体埋め込み部材。
- 請求項13に記載の生体埋め込み部材において、刃部が、縁(15)の周方向に、連続してもしくは断続して配置されることを特徴とする生体埋め込み部材。
- 請求項13に記載の生体埋め込み部材において、刃部が半径方向に、縁(15)に沿って、次々に設けられた突起(16)の端として形成されていることを特徴とする生体埋め込み部材。
- 請求項13に記載の生体埋め込み部材において、尖端が、縁(15)に沿って配設されていて軸方向を向いたピンとして形成されていることを特徴とする生体埋め込み部材。
- 請求項13〜16のいずれかに記載の生体埋め込み部材において、埋め込み部材末端部分(1)に、刃部もしくは尖端が入り込む深さを制限する、半径方向に内側もしくは外側に突出するリングフランジ(17)が備えられていることを特徴とする生体埋め込み部材。
