Description
織、特に(と言って限定するつもりはないが)、たとえ
ば、偶発的な傷害、外科手術その他の傷害で生じる創傷
によって損傷を受けた皮膚その他の上皮組織の修復、治
療を容易にする薬剤および技術に関する。本発明は、特
に、人間その他の脊椎動物の創傷の治療に関係する。
は、一般に、少なくとも成体の人間その他の哺乳動物で
は、細胞間基質(ECM)生合成・転換・器質化法で行
う。この方法は、普通、線維性結合組織の瘢痕を生じさ
せ、その結果、正常組織の機能喪失を招く。
題であるが、現在のところ完全に満足できる解決策はな
い。同様に、事故による火傷その他の傷害による瘢痕形
成、線維化は、特に子供の場合、重大な結果となること
が多く、機能障害、成長阻害、醜い傷跡を招き、ここで
も重大な問題である。
外観を改善するために美容措置や美容手術を試みる必要
性が生じる。さらに、入れ墨その他の皮膚の欠陥を隠す
にも美容措置の必要性が同様にしばしば生じる。しかし
ながら、現在のところ、このような美容措置、手術を満
足な状態で実施することは難しく、不可能ですらある。
というのは、一般的に或る程度の外科手術を伴い、それ
自体が新たに醜い瘢痕組織を生じさせる創傷の原因にな
りがちだからである。
組織での創傷治療は、一般的には、胎児組織および胚組
織の創傷で生じると思われる再生過程と逆に、補綴法で
ある。創傷修復過程の転帰は、内因性パラメータ(たと
えば、組織酸素化)および外因性パラメータ(たとえ
ば、創傷ドレッシング)の両方を含む種々の要因によっ
て影響を受けると思われる。しかしながら、必要な細胞
間連絡を含む創傷損傷組織の治療および修復の全過程
は、成体の人間その他の哺乳動物では、多数の特殊な可
溶性成長因子の協調によって調整されることを示す重大
な証拠がある。可溶性成長因子は、創傷環境内へ(特に
脱顆粒血小板および到来マクロファージによって)放出
され、とりわけ、創傷内で血管新生、白血球走化、線維
芽細胞増殖、コラーゲンその他の細胞外基質分子の移動
および付着を生じさせると思われる。識別され、分類さ
れているこのような成長因子は、一般的に、特殊化した
可溶性タンパク質またはポリペプチドであり、形質転換
成長因子アルファ(TGF−α)、形質転換成長因子ベー
タ(TGF−β1、TGF−β2、TGF−β3等)、血小板誘
導成長因子(PDGF)、上皮細胞成長因子(EGF)、イン
スリン様成長因子I、II(IGF I、IGF II)および酸
性、塩基性線維芽細胞成長因子(酸性FGFおよび塩基性F
GF)を含む。これらの成長因子の多くは、DNA再結合技
術を用いる遺伝子工学によってすでに作られている。

照されたい)。これが、これら成長因子について獲得さ
れた知識の治療への応用を発展させる努力における主た
る趨勢であった。

際、PCT/US90/05566が、TGF−βに抗体を用いてラット
の腎臓ネフローゼ誘導モデルで線維増多を低下させるこ
とを開示している。しかしながら、TGF−β成長因子の
すべてが線維性であるわけではなく、特にTGF−β3の
活性を抑制すると逆効果であることが新たにわかった。
胞外基質産生量を増加させる機能を有すると記載してい
るが、なんらかの特定のTGFβがこのような効果を持た
ないということは示唆していない。

いていの予備実験検査の場合に行われている。望むなら
ば、完全抗体の代わりに、特定の抗原結合特性を保持し
ているその断片(Fab's)も使用できる。これらの断片
は、本明細書において用いる「抗体」なる用語の範囲内
に含まれると考えられる。
成長因子は、最初は、大型のタンパク質分子の一部とし
てあるいは大型タンパク質分子に結合したリガンドとし
て不活性状態で存在し、たとえば、酵素作用によって活
性形態で放出されるときにタンパク質分子から分離する
ことは公知である。したがって、抗体のような中和剤の
このような不活性タンパク質前駆物質への結合が、タン
パク分解作用、したがって、活性成長因子の放出を阻止
または抑制することができ、これが、阻害剤の活性成長

1種類または複数種類の成長因子の遮断結合によって成
長因子活性度に拮抗するかあるいはそれを阻害するよう
に作用し得る1種類または複数種類の合成ペプチドから
なるものであってもよい。このようなペプチド「阻害
剤」は、潜在的な免疫原性副作用を持たないという利点
を有し、抗体よりも容易に膜バリヤを通過することがで
き、局所適用の製薬製剤または組成物を構成するのに最
も適している。これら「阻害」ペプチドは、当該成長因
子のアミノ酸配列または、この配列の、細胞レセプタに
結合する際に伴う部分についての知識から容易に設計す
ることができる。これは配列のこの結合部分を「模擬す
る」必要があるからである。たとえば、TGF−β1の場
合、レセプタ結合に関係するのは分子のc末端であると
いうことは公知である。同様に、TGF−αの場合のレセ
プタ結合に関係する領域は、システイン33とシステイン
42の間の領域であり、EGFの場合には、システイン20と
システイン31の間、チロシン14とシステイン31の間、ロ
イシン15とアルギニン53の間のそれぞれの領域が関係す
る。FGFの場合には、重要なレセプタ結合領域はアミノ
酸105、115の間である。
複数種類の成長因子中和剤は、1種類または複数種類の
の成長因子またはその前駆物質に直接結合してそれを不
活性にすることによって作用する他の分子単位からなっ
てもよい。この種の中和剤または阻害剤の一例は、Deco
rinであり、これは、Nature(1990)、346、281−284で
Yamaguchi等によって報告されているように、TGF−βと
強く結合すると知られている小クロドロイチン−デルマ
タン硫酸プロテオグリカンである。
長因子中和剤は、分子レベルでは活性であってもよく、
成長因子のmRNAに対して活性である分子からなるもので
あってもよい。このような分子としては、トランスレー
ションを防ぐように1種類または複数種類の成長因子mR
NA配列に対して合成された1種類または複数種類のアン
チセンス・オリゴヌクレオチドがあり、あるいは、mRNA
を破壊し、再びそのトランスレーションを防ぐように1
種類または複数種類の成長因子mRNA配列の特殊な配列に
的を絞った1種類または複数種類のribosymeであっても
よい。

にだけ中和効果を有する抗体または他の薬剤は多少とも
瘢痕組織が生じるのを防ぐにまったく充分なものである

上の種類に対して中和効果を有する2つまたはそれ以上
の異なった抗体その他の阻害剤を組み合わせて投与すれ
ば、特に比較的大きな切除創傷にとって、さらにまた有
効であることがわかった。この場合、これらの阻害剤は
個別に投与してもよいし、同時にあるいは順次に投与し
てもよいし、あるいは、単一の製薬製剤内で混合物また
は「カクテル」として構成してもよい。
成長因子は、通常は、互いに協調してオーケストラのよ
うに作用して瘢痕組織の生成に通じる諸段階を含めて全
創傷治療過程を調節すると考えられるが、瘢痕組織の生
成に作用して任意の1つの成長因子の活性度を低下ある
いは中和する効果は、その成長因子の性質または同一性
および結果として生じる活性成長因子プロファイルの形
態に依存して変わると思われる。したがって、TGF−β
またはPDGFあるいはこれら両方の活性度の抑制はこの点
については非常に有効であり得るが、他の成長因子のう
ち或る種の成長因子の活性度の抑制は、たとえこの成長
因子が少なくとも創傷治癒を促進することに関してまだ
必要であるかも知れないか、あるいは、少なくとも或る
種の有利な効果を持つかも知れないにしても、瘢痕組織
形成を低下させるという同様の条件の下では、少なくと
もそれ自体については、効果は低い。

の活性度を低下させるのに有効な1種類または複数種類
の阻害剤または中和剤を、多少とも独立して瘢痕組織形

を促進するかあるいは治癒程度に関して有利な効果を奏
することのできる別の外生的成長因子と組み合わせて用
いる別の可能性があり得る。少なくとも或る種の場合、
たとえば1種類または複数種類のTGF−βまたはPDGF中
和剤と組み合わせても用いるためのこのような他の付加
的な外生的成長因子は、線維芽細胞成長因子(FGF)に
よって提供され得る。したがって、活性cyktokine FGF
対TGF−βまたはPDGFあるいはこれら両方の中和剤の或
る種の比率を有する製薬製剤を提供することによって、
創傷の瘢痕生成を防ぐばかりでなく、創傷治療の全過程
を促進する薬剤を得ることができる。

長因子中和剤での処置は、早期治療段階で実施してこそ
効果的となり得るということがわかった。一般的に言っ
て、この処置の最良の実施は、フィブリンがまだ存在し
ている肉芽位相前またはその最中あるいはこの両方で、
すなわち、フィブリンが完全に肉芽組織と入れ換わって
しまう前である。これは、通常、創傷の初期発生後約14
日の期間内である。しかしながら、処置はより早くに、
7日以内あるいはできるならば創傷を受けた後3日以内
またはもっと早くに開始すると好ましい。実際、創傷を
受けた同じ日あるいは少なくとも翌日に開始すると最良
であることが多く、外科手術創傷の場合、この処置の開
始は、すなわち、創傷領域へ適用された製薬薬剤での1
種類または複数種類の成長因子中和剤の局所的すなわち
非経口的な適用による処置の開始は、外科手術の一部に
組み込まれ、外科手術の前または主手術の完了直後、縫
合の前後に適用される。

すべきかつさらに驚くべき特徴によれば、全治癒時間に
は現実に多少とも延長が観察されず、治癒時になんら創
傷強度の障害もなかった。実際、この後者の点に関し
て、少なくとも、創傷が通常瘢痕組織の形成を伴って治
癒するときに普通に見られるように皮膚外面に対して大
きな角度、一般的には直角になることなく、皮膚外面に
対してほぼ平行に位置する、未損傷組織のそれに非常に
類似した切開皮膚創傷の場合には、治療中に形成される
新しいコラーゲン線維すなわちフィブリルの配向が観察
されたという点で創傷強度さえ改善され得ることがわか
った。

し、1つ(実験創傷)には1種類または複数種類の適切
な成長因子中和抗体の薬剤を注射した。実験をこれらの
動物の群について行い、該当群に従って、創傷を受けた
日あるいは次の日から数えて中断なしに3日間あるいは
7日間にわたって毎日注射(100μ毎)を実施し、
2、3の群では、かなり遅い段階、たとえば、創傷受傷
後7日または19日で注射を実施した。

および生化学技術によって組織分析を行った。

均質化してグリコースアミノグリカンを抽出した。次
に、ホモジネートを1時間18,000gの遠心力にかけた。
ペレットを0.5mの水で2回洗浄し、洗液を上澄み液に
加えた。上澄み液はpH6.5の5mM EDTAで100mMリン酸緩
衝液に対して透析し、パパイン2.5mg/mで消化した。

いずれでも対照創傷と見かけ対照創傷の間に差異はなん
ら見出せなかった。このことは、外部タンパク質の導入
によってなんら主要な影響がないことを示している。ま
た、どの創傷も欠陥のある治癒を示さず、上皮化の率も
すべての処置で同じであった。
中和抗体で処理した実験創傷の場合には、創傷がまだ新
鮮なうち、好ましくは、創傷を受けた時点あるいはその
直後で肉芽組織形成位相前またはその最中に治療を開始
したときには大きな影響が生じた。したがって、創傷受
傷後19日までに治療を始めなかったときに対照創傷と実
験創傷との間には大きな差異は観察されなかったが、他
の場合、特に創傷を与えたと同じ日または次の日に治療
を開始したとき、実験創傷は、いかなる時点でも、同じ
動物の他の3つの創傷に比べて、実験創傷の含むコラー
ゲンI、IIIはかなり少なかった。コラーゲン・フィブ
リルの間隔はかなり大きかったが、この配向は正常な皮
膚のものとほとんど同じである。実際、中和抗体処置創
傷においては、(外胚葉毛嚢の喪失を除いて)この創傷
の部位を検出することはしばしば難しかった。このこと
は、垂直に配向し、平行に稠密に詰まったコラーゲン・
フィブリルを持つ特異な瘢痕を示す他の創傷とは大きく
異なる。これらの影響は、乳頭状真皮組織および皮下組
織で最も顕著であった。TGF−βまたはPDGFに対する中
和抗体で処置した創傷は、特に網様真皮でフィブロネク
チンの顕著な低下を示し、その配向パターンはコラーゲ
ン・フィブリルのそれに類似したものであった。フィブ
ロネクチン染色は創傷全体で顕著に低下したが、皮膚/
表皮接合部ではまだ光沢が最大であった。TGF−βおよ
びPDGFに対する中和抗体での処置は、また、治療してい
る創傷内の血管、単球およびマクロファージの数も減じ
る。これと逆に、TGF−βおよびPDGFで処置した正対照
創傷は細胞外基質集積、細胞外基質充填密度および血
管、単球およびマクロファージの数の顕著な増加を示し
た。これら成長因子処置創傷における瘢痕形成は、対照
と比べて、より顕著であった。

DGFに対しても同様な結果が達成され得ると考えられ
る。

われる少なくともTGF−βの場合、通常、初期創傷後、
創傷の環境でのこの成長因子のレベルは自己触媒カスケ
ード効果によってかなり急速に増大する可能性がある。
したがって、血小板劣化から初期創傷内に存在するTGF
−βが増大濃度で単球、マクロファージおよび線維芽細
胞に対する化学誘因物質として作用するばかりでなく、
それ自体のプロモータにフィードバックしてそれ自体の
合成を刺激し、その結果、高レベルが直ちに現れる。炎
症細胞、特に、マクロファージは、TGF−βを放出し、T
GF−β合成についてこの自己誘導効果を示す。TGF−β
は、また、他の成長因子、たとえば、TGF−α、PDGF、E
GF、TGF−βの合成および放出も刺激し、これら他の成
長因子が、創傷線維芽細胞によって細胞外基質分子、た
とえば、コラーゲンおよびグリコースアミノグリカンの
合成を刺激し、また、これら細胞外基質分子のタンパク
分解性転換、構築の程度にも影響を与える。初期フィブ
リン凝塊が稠密なので、隣接する正常皮膚からの線維芽
細胞は、最初、基底膜に対してほぼ直角の方向で凝塊と
創傷縁との間で上下に移動する。コラーゲンその他の細
胞外基質分子もこの異常な配向で沈着し、最終的には瘢
痕を生じさせる。

下する。しかしながら、中和剤は、この成長因子のすべ
てを中和するに充分な量で適用されて、厳しい抑制なし
に創傷治療を処理するに充分に残ることはない。これは
少なくともPDGFにも当てはまる。

での瘢痕組織形成を制御するために臨床に直接応用でき
ることは明らかである。実際の用途のためには、一般的
には、創傷の治療における瘢痕組織形成の原因となる活
性のある関連成長因子のプロファイルを中和または改質
あるいはこれら両方を行う際に有効な材料を構成する適
正量の成長因子抗体(単数または複数)または他の成長
因子抑制剤(単数または複数)は、創傷治療の必要な患
者へ任意適当な要領で投与するための製薬製剤として薬
剤の分野では周知の方法のいずれかによって構成される
ことになる。このような製薬製剤は、さらに、臨床で個
別に供給されるばかりでなく、たとえば臨床外緊急時使
用のための救急キットの構成要素としても提供され得
る。
一般には、1種類または複数種類の抗体または1種類ま
たは複数種類の他の中和剤は、1投与あたり1直線切開
cmあたり1pg−1μgのTGF−β(1および2)および/
またはPDGF(好ましくは、100pg−10ng)を効果的に中
和するように投与されなければならない(少なくとも切
開性創傷について)。先に指摘したように、創傷治療過
程で早期の適用が必須である。通常、この時期として
は、肉芽組織形成の段階前、その最中またはその前から
その最中まで、創傷を受けたときから約14日以内、好ま
しくは、7日または3日以内あるいはそれより短い期間
となる。
あまり遅くならずに、液体、ゲル、エアロゾル、クリー
ムまたは粉末の形態で、もしくは、ドレッシング、生物
分解性パッチまたは制御放出移植装置の形で創傷まわり
の表面に適用できると便利である。非経口投与、特に、

は複数種類の中和抗体または1種類または複数種類の他
の薬剤を最大効率で創傷環境へ直接導入することができ
る。この目的のために、調製した製薬製剤は、密閉アン
プル内に単位投与形態で収容された所定量の活性材料、
たとえば、関連した1種類または複数種類の抗体の滅菌
液体製剤(たとえば、リン酸緩衝生理食塩水内にある)
を包含するとよい。しかしながら、或る種の場合に好ま
しい別の局所投与モードの場合、製薬上許容できるキャ
リヤ、稀釈剤または添加剤を構成する少なくとも1つの
他の成分と密接に組み合わせるかあるいは混ぜ合わせた
活性材料で構成して、局所適用に最も適した組成物(た
とえば、クリーム、ゲル、軟膏等)としてもよい。この
製剤は、局所適用のために滅菌ドレッシング、生物分解
/吸収パッチまたはドレッシングに適用してもよいし、
初期に放出量が大きくてその後にゆっくり放出する遅速
放出移植システムに適用してもよい。
