Description

【発明の詳細な説明】 〔産業上の利用分野〕 本発明は人の関節、殊に人工股関節に関するものであ
る。
〔従来の技術とその問題点〕
従来から用いられている人工股関節はステンレス鋼、
コバルトクロム系合金などの金属を用い骨頭球とステム
が一体的もしくはテーパー嵌合によって固定されてお
り、ステムを大腿骨中に挿入し、骨とステムとの隙間を
セメントを用いて固定している。一方の臼蓋側は骨頭を
受座する合成樹脂より成るソケットをこれも骨盤にセメ
ントを用いて固定するものである。特に大腿骨側におい
てはステムが長管骨中に長く挿入されるが、骨と金属と
のヤング率の違いにより、荷重を受けた場合の変形量が
大きく異なりセメントと骨、セメントと金属ステム間で
の弛みが生じやすく特にステムの弛みに伴う沈み込みが
大きな問題となっており人工股関節が生体から離脱した
り、部材間にガタが生ずるなどにより関節機能を喪失し
てしまう結果となる。近年このセメントと骨との間、又
はセメントと金属材料との間の弛み無くすために多くの
試みがなされてきた。ステムの材質については、純チタ
ンやチタン合金が従来のコバルトクロム合金やステンレ
ス鋼に代って用いられるようになり、より骨に近いヤン
グ率のものを用い骨の撓みに追従しようとする試みが行
われるようになってきたが、それだけでは完全に問題を
解決するには至っていない。
補綴物を、複雑な形状をした骨にできるだけ適合さ
せ、骨と補綴物との隙間をできるだけ少なくし、かつセ
メントを用いないでステムを固定しようとするデザイン
的な改良の試みも一つの方法としてなされてきている。
しかしセメントを用いない全形状を骨の形状に似せる
ことは個体差の問題もあり、極めて困難である。
また、その反対に骨を補綴物の形状に加工して挿入す
ることも行われているが、加工の際に多くの健康な骨を
削り取ることとなり骨全体の強度低下をまねくなど、こ
れも多くの問題がある。
〔問題点を解決するための手段〕
これらの問題を解決するために、大きさ、形状の異な
った別体の支持部材の中から骨髄腔の大きさにフィット
し強固に固定可能な支持部材を選択し、これを直接、骨
に固定する。この支持部材は骨に強固に固定でき、骨と
の親和性の優れた材料で構成され、かつこの骨に強固に
固定された支持部材にステムをテーパー嵌合等の手段を
用いて強固に連結、支持させる。
〔作用〕
このような人工股関節の作用として 1.骨髄腔の大きさの個体差を支持部材の大きさの選択に
よって追従せしめ、ステムと骨髄鋼の大きさのギャップ
による固定力の差をなくす事が可能とする。
2.ステムの形状は単純な形状で良いために製造が非常に
容易である。
3.セメントを必ずしも使用しなくても良く、セメントを
使用しない場合にはセメントの重合熱による生体への悪
影響をなくする事が可能となる。
4.術式上非常に容易で強固な固定、支持が得られる。
〔実施例〕
本発明の実施例では第１図に示す通り、１はアルミナ
もしくはジルコニアなどから成るボールで、チタン合金
製のステム２の先端部に嵌合されている。また、３はネ
ジ構造をもつ支持部材で大腿骨髄腔中に強固にネジ込ま
れており、その支持部材３の内面に備えられたテーパー
４によって上記ステム２と強固に嵌合しており固定され
ている。この支持部材３は、チタン合金製でステム２と
同様表面にアパタイトや骨形成因子（BMP）がコーティ
ングされており骨とのなじみを良くしてある。このよう
な人工股関節を成すステム２を装着するには、支持部材
３は骨の髄腔の大きさに合わせて多くの種類が準備され
たなかから術者が最も適当な大きさの支持部材３を選ん
で最適な深さまで髄腔中に挿入し強固に骨に螺入し、固
定する。この場合内ネジ５は挿入する際に深さを調節で
きる治具を取り付けるのに用い外ネジ3aとは逆方向のネ
ジ溝が形成してある。このように挿入された支持部材３
に対してステム２を打ち込んで嵌合させ組立てを完了す
る。なお、第２図に示したように表面に付着したビーズ
状の凸起６による凹凸でもって骨と強固に固定されるよ
うにした支持部材３′を用いてもよい。
〔発明の効果〕
叙上のように、本発明による人工股関節によれば、セ
メントを使用しなくともステムの強固な固定、支持が得
られ従来から指摘されているセメントを使用する事によ
る問題点はすべて解決する事が可能となり、歩行回復に
絶大なる威力を発揮し、長期の安定性を保証することが
できる。

    (57)【特許請求の範囲】
  1. 【請求項１】大腿骨の骨髄腔中に挿入されるステムと、
    該ステムの先端部に一体的もしくは別体で作られた骨頭
    球を具備した人工股関節において、大腿骨髄腔内にて上
    記ステムの下端部に当接し固定支持すべく、外周に骨と
    係合せしめる外ネジ又は凹凸を形成してなる支持部材を
    備えたことを特徴とする人工股関節。
