Abstract
本発明は、バイオテクノロジー及び医学の分野に関する。上皮起源の腫瘍、特にヒトKRASタンパク質の天然型を発現する腫瘍の処置における、上皮成長因子を遮断する化合物及びPD-1/PD-1リガンドシグナル伝達経路を遮断する抗体を含む治療用組成物の使用が記載されている。当該治療用組成物で処置した天然のKRASを発現する上皮起源のがんを有する患者では、その生存率の増加が観察された。
Description
本発明は、バイオテクノロジー及び医学の分野に関する。特に、上皮成長因子及びPD1/PD1リガンドのシグナル伝達経路の同時遮断による上皮起源の腫瘍を有する患者の処置のための治療用組成物の使用が記載される。
PD1受容体及びそのPDL1リガンドが目立つ「チェックポイント」と呼ばれる分子は、免疫系によって生成される抗腫瘍応答を負に調節する。これらのチェックポイントでの阻害性シグナルの伝達を遮断するモノクローナル抗体(mAb)(抗CPI)は、より効率的な抗腫瘍応答を誘導する。2020年12月現在、黒色腫、肺がん、頭頸部扁平上皮腫瘍、肝細胞がん腫、尿路上皮がん、胃がん、乳房及び結腸直腸の腫瘍などの多数の「免疫感受性」悪性腫瘍の処置には、7つの抗CPI mAbが承認されている(Ravindranathan D et al.(2021)Biology,10:325)。
最も成功した抗CPI mAbの中には、PD1特異的(抗PD1)ニボルマブ及びペンブロリズマブ並びにPDL1に対するアテゾリズマブ及びデュルバルマブがある。これらの治療法は、PDL1分子を発現し、高頻度の突然変異を有し、阻害シグナルが遮断された場合に腫瘍を攻撃することが潜在的に可能な免疫細胞の浸潤を有する腫瘍においてより効果的であることが示されている(Gibney GT y cols.(2016)Lancet Oncol.17(12):e542-e551)。PD1/PDL1に対して低分子量阻害剤も開発されており、免疫系に関連する有害作用を示すことなくこのシグナル伝達経路を遮断するという点で有望な結果が得られている(Liu Ch y cols.(2021)Cancer Cell Int.21:e1-e17)。
上皮成長因子受容体(EGFR)はチロシンキナーゼ受容体であり、7つの天然リガンドによって活性化される既知のがん遺伝子(Yarden Y.(2001)European Journal of Cancer.37:S3-S8)である。いくつかの研究では、EGFRに結合するリガンドに応じて、EGFRシグナル伝達カスケードの示差的活性化が報告されている。EGFなどのリガンドは、腫瘍増殖に有利なシグナル伝達を誘導し、アンフィレグリンなどの低親和性リガンドは分化を促進する(Freed DM y cols.(2017)Cell 171:1-13)。リガンド依存性シグナル伝達カスケードの示差的活性化は、例えば抗EGFR mAb又はTKIシグナル伝達カスケードの阻害剤による直接的な受容体遮断とは対照的に、これらの分子に対する標的化療法において重要であり得る。そのような違いは、処置の忍容性/安全性、並びに異なる治療状況でのより大きい又はより小さい臨床的有効性に影響を及ぼし得る。この意味で、EGFRリガンドを標的とする治療様式は、リガンドに対する特異的抗体(Ab)を誘導し、その結果、EGFRとのその相互作用を遮断するワクチン組成物の使用である。この治療戦略の一例は、ヒト上皮成長因子(EGF)に対する特異的mAbを生成し、この重要なリガンドを腫瘍から奪うCIMAvax-EGFワクチンである。CIMAvax-EGFで免疫した非小細胞肺がん(NSCLC)患者における多数の臨床試験は、ワクチンが安全で免疫原性であることを示している。臨床応答の観点から、ワクチンの投与は患者の生存を有意に増加させた(Rodriguez PC y cols.(2016)Clin Cancer Res.22(15):3782-90)。
EGFRシグナル伝達に関与する最も重要な分子の1つは、GTPアーゼKRASである。この分子はまた、腫瘍形成に関与する複数のシグナル伝達カスケードを活性化する。すべてのヒト腫瘍の約30%が、KRASをコードする遺伝子に突然変異を有し、これにより、従来の膜受容体活性化とは無関係に、EGF-Rカスケードの何らかの構成的活性化が誘導される(Fernandez-Medarde,A y cols.(2011)Genes and Cancer.2:344-358)。
この遺伝子の突然変異は、これらの新生物の一部、例えば膵臓腺がんにおいて優勢であり、腫瘍の90%において突然変異を示す。結腸などの他の腺がんでは、それは、それらの40%に存在する。肺の場合、KRAS突然変異は、すべての非小細胞肺腫瘍の30%、主に腺がん組織学に存在する(Moore AR y cols.(2020)Nat Rev Drug Discov.19(8):533-552)。
膵臓及び結腸などの腫瘍では、KRAS突然変異の存在は、腫瘍増殖及び治療に対する耐性に有利である(Haigis KM et al.(2008)Nat Genet.40:600-8.;Bournet B et al.(2016)Clin Transl Gastroenterol.7:e157)。しかしながら、肺腺がんなどの他のものでは、この遺伝子における突然変異の予後値に関する議論の余地のあるデータがある(Shepherd FA et al.(2013)J Clin.Oncol.31(17):2173-81;Zer A et al.(2016)J Thoracic.Oncol.11(13):312-23)。進行した結腸腫瘍において、天然のKRASの存在は、EGFR特異的mAbであるパニツムマブ及びセツキシマブによる処置の臨床的利益の予測因子である。さらに、ゲニシタビンと組み合わせたニモツズマブ(別の抗EGFR)の使用は、腫瘍がKRASに対して天然である進行膵臓がんを有する患者において臨床的利益を提供する(Schultheis B et al.(2017)Ann.Oncol.28:2429-35)。しかし、NSCLCの処置のためのセツキシマブの使用は、特にKRASを含むEGFRシグナル伝達カスケードに突然変異を有する患者のサブグループのいずれにおいても臨床的利点を示していない。一方、腫瘍におけるKRASの突然変異又は非変異は、それ自体では、抗PD1/PDL1 mAbに対する患者の臨床応答に影響を及ぼさないか、又は調整しない。肺がんにおける抗PD1/PDL1 mAbを用いた第III相臨床試験では、突然変異KRAS腫瘍を有する患者について有意な臨床的利益が観察された(Borghaei,H et al.(2015)N Engl J Med.373(17):1627-1639;Socinski May cols.(2018)N Engl J Med.378(24):2288-2301)。この知見と一致して、最近の臨床試験は、KRAS突然変異(同時突然変異)と組み合わせた他の突然変異の存在が、抗PD1/PDL1 mAbで処置された患者においてより良好な又はより悪い応答を調整し得ることを示している。特に、KRASがTP53遺伝子と同時突然変異する肺腺がんは、CPIに対する感受性がより高い免疫原性腫瘍をもたらす。対照的に、STK11における同時突然変異は、炎症を起こさず、このタイプの治療に対してより抵抗性である腫瘍をもたらす(Dong ZY et al.(2017)Clin Cancer Res 23:3012-3024;Skoulidis Fy cols.(2018)Cancer Discov;8:822-835)。
EGFRシグナル伝達遮断療法及びCPIの成功を考慮して、いくつかの研究では、異なる処置適所における両方のアプローチの併用処置の利益が評価されており、結果は様々である。いくつかの著者らは、抗PD1/PDL1 mAbを、肺がん患者における突然変異EGFRバリアントによる異常なシグナル伝達を遮断するTKI療法と組み合わせることの簡便さの有無を調査した。これらの研究は、組み合わせの利点がなく、毒性が有意に増加したことを示した(Yang JC et al.(2019)J Thorac Oncol.;14(3):553-9;Schoenfeld AJ et al.(2019)Ann Oncol.30(5):839-44)。他の著者らは、肺がん患者においてネシツムマブ(別の抗EGFR mAb)との組み合わせを評価した。この試験では、毒性は管理可能であったが、臨床的に関連する効果の証拠はなかった(Besse B et al.(2020)Lung Cancer.142:63-69)。これらの試験のいずれにおいても、KRAS分子の状態との関連は報告されていない。
本発明は、天然のKRASを有する患者における、PD1とPDL1との間の相互作用を遮断するmAbと自己ヒトEGFに対するAbを誘導するワクチン組成物とを組み合わせた治療用組成物の使用を初めて報告する。得られた結果は、治療が十分に忍容され、天然のKRASを示す腫瘍を有する患者に特異的に利益をもたらすことを示している。さらに、この治療法は、抗PD1/PDL1 mAb治療に対してあまり感受性ではない、PD-L1発現が低い腫瘍を有する患者に利益をもたらす。
1つの実施形態において、本発明は、上皮起源の腫瘍を処置するための治療用組成物の使用に関する。これらの組成物は、EGFに対する特異的Abの産生を誘導するワクチン組成物及びPD1/PD1リガンドシグナル伝達経路を遮断する化合物の使用を特徴とする。特に、上述の治療用組成物の使用は、KRASタンパク質の天然型を発現する上皮起源の腫瘍、好ましくは配列番号1及び配列番号2に記載される配列を有する腫瘍において記載される。
特に、EGFに対するAbの産生を誘導するワクチン組成物は、有効成分として、組換えヒトEGFと担体タンパク質との間のコンジュゲートを含む。当該担体タンパク質は、コレラ毒素B、破傷風トキソイド、KLH及び髄膜炎菌(Neisseria meningitidis)p64kタンパク質を含む群から選択される。
さらに、EGFに対するAbの産生を誘導するワクチン組成物は、不完全なフロイントのアジュバント、スクアレンに基づくもの、合成起源のもの、鉱物起源のもの、植物起源のもの、動物起源のもの、粒子状タンパク質及びリポソームを含む群から選択されるアジュバントを有する。
PD1/PD1リガンドシグナル伝達経路を遮断する化合物は、その中でもニボルマブ、ペンブロリズマブ、MEDI0608及びピジリズマブである抗PD1 Abと、アテゾリズマブ、デュルバルマブ、アベルマブ及びMDX-1105であり得る抗PDL1 Abとを含む群から選択される。
本発明の一実施形態では、患者は、それらの腫瘍細胞の試料中の天然KRASの存在又は非存在によってスクリーニングされる。天然のKRASが存在するものを処置のために選択する。処置される腫瘍の種類には、非小細胞肺がん、頭頸部扁平上皮がん、尿路上皮がん腫、結腸直腸がん、胃がん、食道がん、子宮頸がん、肝細胞がん腫、メルケル細胞がん腫、腎細胞がん腫、子宮内膜がん腫、乳がん及び皮膚がんがある。
さらなる実施形態では、本発明は、患者を本明細書に記載の治療用組成物による処置に対して応答者又は非応答者に層別化する方法に関する。患者は、腫瘍細胞の試料中の天然のKRASの存在又は非存在を決定することによって層別化され、天然のKRASの存在が検出された患者は、処置に対する応答者と見なされる。好ましくは、腫瘍試料のPDL1レベルが1%未満である患者が応答者と見なされる。
発明の詳細な説明
KRAS
本発明の方法及び使用は、上皮性腫瘍が野生型タンパク質KRASを発現する患者の処置及び/又は層別化のために想定される。用語「野生型」KRASは、天然に存在するKRASアイソフォームを指す(Uniprot Acc.No.P01116、2021年4月7日のバージョン246)。野生型KRASは、配列番号1又は配列番号2に対応する配列を含むことが想定される。特に、野生型KRASは、配列番号1又は配列番号2に対応する配列からなり得る。
KRAS
本発明の方法及び使用は、上皮性腫瘍が野生型タンパク質KRASを発現する患者の処置及び/又は層別化のために想定される。用語「野生型」KRASは、天然に存在するKRASアイソフォームを指す(Uniprot Acc.No.P01116、2021年4月7日のバージョン246)。野生型KRASは、配列番号1又は配列番号2に対応する配列を含むことが想定される。特に、野生型KRASは、配列番号1又は配列番号2に対応する配列からなり得る。
「野生型KRAS」という用語は、一般に、配列番号1(KRAS4A)又は配列番号2(KRAS4B)に示される参照配列に対して突然変異を有するKRASポリペプチドも包含し、本明細書に記載の配列番号1(KRAS4A,Uniprot Acc.No.P01116-1)又は配列番号2(KRAS4B,Uniprot Acc.No.P01116-2)に示されるアミノ酸配列とある程度の同一性として共有するアミノ酸配列を有するポリペプチドも包含する。より具体的には、「野生型KRAS」には、配列番号1(KRAS4A)又は配列番号2(KRAS4B)と少なくとも80%、85%、90%、95%又は100%の同一性を有するKRASポリペプチドが含まれる。特に、KRASアイソフォーム4A及び4Bは、「野生型KRAS」という用語に包含される。好ましくは、「野生型KRAS」は、それぞれ配列番号1又は配列番号2と比較して、そのアミノ酸配列に突然変異を含まない。
治療用組成物
本発明は、特に標的EGF及びシグナル伝達経路PD1/PD1リガンドを遮断することを目的とする、がんを処置するための治療用組成物に関する。本発明は、上皮起源の腫瘍、特に典型的には免疫療法に応答する腫瘍を有する患者の亜集団における、PD1又はそのリガンドPDL1に対する化合物と、EGFの濃度を低下させる薬剤との組み合わせの有効な使用を記載する。
本発明は、特に標的EGF及びシグナル伝達経路PD1/PD1リガンドを遮断することを目的とする、がんを処置するための治療用組成物に関する。本発明は、上皮起源の腫瘍、特に典型的には免疫療法に応答する腫瘍を有する患者の亜集団における、PD1又はそのリガンドPDL1に対する化合物と、EGFの濃度を低下させる薬剤との組み合わせの有効な使用を記載する。
PD1又はそのリガンドに対する化合物の中には、本発明において使用されるMAb抗PD1がある。そのような化合物はすべて、細胞表面受容体PD1に特異的に結合し、阻害経路PD1/PDL1を遮断するものである。これらの抗PD1 MAbの中には、米国特許第8,008,449号に記載されているニボルマブ、米国特許第8,354,509号及び米国特許第8,900,587号に記載されているペンブロリズマブ、MEDI0608(米国特許第8,609,089号)、ピジリズマブ(米国特許第8,686,119号)及びセミプリマブがある。
本発明で使用される抗PDL1 MAbはすべて、PDL1に特異的に結合し、阻害経路PD1/PDL1を遮断するものである。これらの抗PDL1 MAbの中には、米国特許第8,217,149号に記載されているアテゾリズマブ、デュルバルマブ(米国特許第8,779,108号)、米国特許第9,624,298号に記載されているアベルマブ及びMDX-1105(米国特許第7943743号)がある。
さらに、相互作用PD1/PDL1を抑制する低分子量阻害剤を、本発明によるEGF濃度を低下させる薬剤と組み合わせることができる。これらの阻害剤の中には、BMS1166、BMS202及びCA-170がある。
本発明において提供されるEGFの濃度を低下させる薬剤はすべて、自己ヒトEGFに対する特異的Abの産生を誘導するワクチン組成物である。そのようなAbは、EGF受容体とのEGFの相互作用を遮断し、このことは血清EGFのレベルの低下及び/又は排除に寄与する。これらのワクチン組成物の例は、組換えヒトEGF(EGFhr)と輸送体タンパク質との間のコンジュゲートを有効成分として含むすべてのものである。この輸送タンパク質は、コレラ毒素B、破傷風トキソイド、KLH及び髄膜炎菌(Neisseria meningitidis)由来のP64kであり得るが、これらに限定されない。さらに、これらのワクチン組成物は、フロイント不完全アジュバント、スクアレン系アジュバント、合成起源、鉱物起源、植物起源、動物起源、粒子状タンパク質アジュバント及びリポソームから選択されるアジュバントを含む。
患者の特定及び/又は選択の方法
特定の患者が本発明の方法によって処置される患者の集団に属するか否かは、当技術分野で公知の日常的な実験を用いて評価することができる。例えば、上皮性腫瘍が野生型KRASを発現するかどうかを決定するために、腫瘍試料を典型的には評価される患者から得て、KRAS核酸配列をそれぞれ試料から得て増幅し、シーケンシングに供する。或いは、ポリメラーゼ連鎖反応は、KRAS遺伝子の存在を検出することができる。
特定の患者が本発明の方法によって処置される患者の集団に属するか否かは、当技術分野で公知の日常的な実験を用いて評価することができる。例えば、上皮性腫瘍が野生型KRASを発現するかどうかを決定するために、腫瘍試料を典型的には評価される患者から得て、KRAS核酸配列をそれぞれ試料から得て増幅し、シーケンシングに供する。或いは、ポリメラーゼ連鎖反応は、KRAS遺伝子の存在を検出することができる。
別の方法は、KRASがん遺伝子を有する循環腫瘍細胞を膜マイクロアレイを用いて検出することができる血漿又は血清試料に関する。これに関して、血漿又は血清中の陽性KRAS當津善変異は腫瘍中のKRAS突然変異を示唆するが、血漿又は血清中にKRAS突然変異が存在しないことは、膵臓腫瘍組織における同様の突然変異の欠如を必ずしも証明しない。
追加的又は代替的に、野生型KRASの発現は、腫瘍試料中の突然変異及び/又は野生型KRASポリペプチドを検出することによって、例えば、それぞれ野生型又は突然変異KRASに特異的なエピトープに結合する特異的抗体を使用することによって決定することができる。野生型KRASを発現する上皮性腫瘍を有する患者は処置のために選択されるが、発現突然変異KRASの患者は処置のために選択されないことが想定される。
本明細書の他の箇所に記載されているように、突然変異KRAS、特に1つ又は複数の突然変異を有するKRASを発現する上皮性腫瘍を有する患者は、本発明の方法で使用される治療用組成物による処置に対して応答性が低いか又は非応答性であると考えられる。したがって、本発明はまた、上皮性腫瘍を有する患者を層別化するための方法を提供する。「層別化」は、本明細書で使用される場合、上皮性腫瘍を有する患者を、本発明の治療用組成物による処置から利益を得る可能性のある患者と、利益を得ない可能性のある患者に分類することを意味する。上皮性腫瘍が野生型KRASを発現しない患者、特に上皮性腫瘍が突然変異KRASを発現する患者は、本明細書に記載の治療用組成物による処置から利益を得る(応答する)可能性が低いことが想定される。
そうでなければ、PDAC腫瘍が本明細書の他の箇所で定義される野生型KRAS、HRAS又はNRASを発現する患者は、本明細書に記載される処置から利益を得る(応答性である)可能性が高い。当該「野生型」KRASは、好ましくは配列番号1又は配列番号2に対応する配列を有する。
本明細書で提供される処置方法の文脈における「応答性である」という用語は、患者又は腫瘍が、臨床試験における応答評価基準(iRECIST)に従って、本明細書で定義される治療用組成物を投与した後に完全奏効、部分奏効又は疾患安定化を示すことを意味する。本明細書で使用される「非応答性」という用語は、iRECISTに従って本明細書で定義される治療用組成物を投与した後、患者又は腫瘍が安定疾患又は進行性疾患を示すことを意味する。iRECISTは、Seymour L y cols.RECIST working group(2017)Lancet Oncol.18(3):e143-e152に記載されている。
したがって、処置される患者又は患者の集団を選択する方法も本明細書で提供される。これは、各患者の上皮性腫瘍試料中の突然変異及び/又は野生型KRASの存在又は非存在を決定することによって達成される。腫瘍試料において突然変異KRASが検出され、及び/又は野生型KRASが検出されない患者は、処置に適格であると見なされないが、腫瘍試料において野生型KRASが検出され、及び/又は突然変異KRASが検出されない患者は、本発明による処置に適格であると見なされ、したがって、本発明による処置のために選択される。
さらに、前述のように、本発明は、PDAC又は上皮性腫瘍に罹患している患者が本明細書に記載の治療用組成物による処置に対して応答性又は非応答性であるかどうかの予後診断のための方法を提供することを可能にする。前述のように、患者の腫瘍試料中の突然変異及び/又は野生型KRASの非存在又は存在は、当技術分野で公知であり、本明細書の他の箇所に記載されている日常的な方法を用いて評価することができる。上皮性腫瘍における突然変異KRASの発現は、患者又は上皮性腫瘍が処置に反応しないことを示すが、上皮性腫瘍における野生型KRASの発現は、患者が本明細書に記載の治療用組成物による処置に反応することを示す。
本発明のさらに好ましい実施形態では、処置に応答する患者は、天然のKRASを発現する患者であり、腫瘍試料中の免疫組織化学的技術によって測定したPDL1レベルが1%未満である。
処置方法
本明細書中に記載される治療用組成物により処置され得るがんのタイプの中には、限定されないが、NSCLC、頭頸部の扁平上皮がん、尿路上皮がん腫、結腸直腸がん、胃がん、食道がん、子宮頸がん、肝細胞がん腫、メルケル細胞がん腫、腎細胞がん腫、子宮内膜がん腫、乳がん、扁平上皮皮膚がん腫がある。さらに、本発明は、本発明の組み合わせを使用して増殖を阻害することができる難治性又は再発性新生物を含む。
本明細書中に記載される治療用組成物により処置され得るがんのタイプの中には、限定されないが、NSCLC、頭頸部の扁平上皮がん、尿路上皮がん腫、結腸直腸がん、胃がん、食道がん、子宮頸がん、肝細胞がん腫、メルケル細胞がん腫、腎細胞がん腫、子宮内膜がん腫、乳がん、扁平上皮皮膚がん腫がある。さらに、本発明は、本発明の組み合わせを使用して増殖を阻害することができる難治性又は再発性新生物を含む。
有効成分としてEGFを含むワクチン組成物の投与は、好ましくは筋肉内で行われ;最初の4回の投与は14日ごとに、残りは28日ごとに、3日間の許容時間範囲で行われる。これらの組成物が使用される用量範囲は、重量1キログラム当たり20~70μL/kgの重量又は20~70μgの総タンパク質、又は最大5mgの総タンパク質を含み、より推奨されるのは30~60μg/kgである。処置段階は、6ヶ月の最小期間を有し、その後、得られた結果に応じて頻度及び用量を変えることができる維持段階が続く。この維持期間は、血清EGF濃度の低下が保証されるならば、生成されたAbs力価並びに臨床徴候の改善及び/又は安定化を考慮して最適化することができる。このような血清EGF濃度は、この目的のために市販されている診断セットのいずれかによって測定することができる。
抗PD1及び抗PDL1 MAbは、それぞれの場合について推奨される用量及びスケジュールで投与される。抗PD1 MAbの場合、100~500mgの総タンパク質の用量範囲で静脈内に2~6週間の頻度で投与される。抗PDL1 MAbは、2~5週間の頻度で600~1800mgの総タンパク質の範囲で静脈内投与される。ワクチン及びMAbの投与は、PD1/PD1Lシグナル伝達の遮断作用及びEGF媒介シグナル伝達の阻害と一致するように調整される。この原理に従って、投与は付随的又は連続的であり得る。特に、ワクチン組成物の投与は、MAbの投与スキームと重複し得る。
本発明を、以下の例及び図面を用いてさらに詳述する。しかしながら、これらの例は、本発明の範囲の限定として解釈されるべきではない。
例1.CIMAvax-EGFと抗PD1 mAbニボルマブとの併用投与は安全であり、強力な抗ヒトEGF抗体応答を誘導する。
ニューヨーク州バッファローのロズウェルパーク総合がんセンター(Roswell Park Comprehensive Cancer Center)における第I/II相臨床試験(NCT02955290)では、抗PD1 mAbニボルマブとともに、ワクチン組成物CIMAvax-EGFを含む治療用組成物を、進行したNSCLC患者の処置に使用した。
ニューヨーク州バッファローのロズウェルパーク総合がんセンター(Roswell Park Comprehensive Cancer Center)における第I/II相臨床試験(NCT02955290)では、抗PD1 mAbニボルマブとともに、ワクチン組成物CIMAvax-EGFを含む治療用組成物を、進行したNSCLC患者の処置に使用した。
この研究では、第I相用量漸増及び第II相有効性評価を行った。合計で29人の転移性NSCLC患者を含めた。ニボルマブは、2週間ごとに240mgの用量で静脈内に使用した。CIMAvax-EGFワクチン組成物を、誘導期中に2週間ごとに2.4mgの用量で筋肉内投与し(4用量)、続いて維持期に毎月注射して使用した。最初の6人の患者は、半分の用量のCIMAvax-EGF(1.2mg)を受けた。
安全性プロファイルは良好であり、適用された治療に関連する重篤な有害反応はなかった。CIMAvax-EGFは、1:4000以上の抗EGF抗体(Ab)力価によって定義されるように(血清希釈(図1))、すべての患者において良好な応答を誘導した。
ELISA(ヒトEGF Quantikine ELISAキット、R&D Systems)によって測定した患者の血清中のEGF濃度の急速な減少も、CIMAvax-EGF及びmAbニボルマブで処置した患者で観察された(図2)。
処置された29人の患者の全生存期間の中央値は10.36ヶ月であった。1年全生存率は44%であった。
例2.CIMAvax-EGFと抗PD1 mAbニボルマブとの併用投与は、天然のKRASを有する患者に有意に利益をもたらす。
例1に記載の試験では、腫瘍におけるKRAS遺伝子の突然変異の有無により患者を層別化して解析を行った。突然変異の有無及びKRAS遺伝子のコピー数は、マルチパラメトリックPCRベースのDNAシーケンシングを使用する新世代シーケンシングアッセイを使用して検証した。驚くべきことに、天然のKRASを有する患者の生存率は、突然変異KRASを有する患者の生存率よりも有意に高かった(図3)。
例1に記載の試験では、腫瘍におけるKRAS遺伝子の突然変異の有無により患者を層別化して解析を行った。突然変異の有無及びKRAS遺伝子のコピー数は、マルチパラメトリックPCRベースのDNAシーケンシングを使用する新世代シーケンシングアッセイを使用して検証した。驚くべきことに、天然のKRASを有する患者の生存率は、突然変異KRASを有する患者の生存率よりも有意に高かった(図3)。
生存期間中央値は、天然KRAS患者では22.06ヶ月であり、突然変異KRAS患者では10.26ヶ月であった。1年生存率は、天然KRAS患者では69%、突然変異KRAS患者では37%であった。
さらに、併用療法後、天然のKRASを有する患者は、疾患制御率の有意な改善を有した(irRECIST基準により少なくとも疾患が安定している患者(Seymour L et al.RECIST working group(2017)Lancet Oncol.18(3):e143-e152))。天然KRASを有する患者では、CIMAvax-EGFとニボルマブとの組み合わせ後の疾患制御率は56.3%であったのに対して、KRAS突然変異を含む腫瘍を有する患者では12.5%であった。
文献によれば、KRAS突然変異による層別化は、ニボルマブmAbのみで処置された患者の生存率に影響を及ぼさないので、天然KRASを有する患者において併用療法で観察された生存率は臨床的に重要である。進行したNSCLCを有するこれらの患者において、ニボルマブによる単独療法は、突然変異KRAS又は天然KRASを有する患者においてそれぞれ11.2ヶ月及び10ヶ月の生存期間中央値をもたらした(Passiglia F et al.(2019)Br J Cancer 120(1):57-62)。
例3.CIMAvax-EGFと抗PD1 Abニボルマブとの併用投与は、天然のKRAS腫瘍及びPDL1<1%を有する患者に有意に利益をもたらす。
腫瘍におけるPDL1の発現が低い患者において抗PD1 mAbによる単剤療法に対するより低い応答に関する文献における既知の報告を考慮して、腫瘍がPDL1を発現しない患者(PDL1<1%)について例2の分析を繰り返した。PDL1発現を、PDL1決定のためにpharmDxアッセイ28-8を使用して決定した。
腫瘍におけるPDL1の発現が低い患者において抗PD1 mAbによる単剤療法に対するより低い応答に関する文献における既知の報告を考慮して、腫瘍がPDL1を発現しない患者(PDL1<1%)について例2の分析を繰り返した。PDL1発現を、PDL1決定のためにpharmDxアッセイ28-8を使用して決定した。
驚くべきことに、KRASの突然変異による患者の層別化は、患者の生存を再び区別することが観察された。生存期間中央値は、天然KRAS患者では22.06ヶ月であり、突然変異KRAS患者では10.26ヶ月であった。1年生存率は非常に高く、天然KRAS患者では80%であった(図4)。

- 上皮成長因子(EGF)に対する特異的抗体(Ab)の産生を誘導するワクチン組成物と、PD1/PD1リガンド(PD-1L)シグナル伝達経路を遮断する化合物とを含む、上皮起源の腫瘍を処置するための治療用組成物の使用。
- 上皮起源の腫瘍が野生型形態のヒトKRASタンパク質を発現する、請求項1に記載の使用。
- 野生型KRASが、配列番号1及び配列番号2からなる群から選択される配列を有することを特徴とする、請求項2に記載の使用。
