Abstract

レット症候群(RTT)は、主に、X連鎖遺伝子、メチルCpG結合タンパク質2(MECP2)の変異によって引き起こされる神経発達障害である。レット症候群は、主に、女児に影響し、ダウン症候群に次いで2番目に多い重度知的障害の原因であり、出生10,000例当たり1例という発生率を有する。患者は、認知能力障害および身体能力障害に苦しみ、その大部分が車椅子および常時介護を必要とする。現在のところ、治療法はないが、生後6~18ヶ月からの遅発は、治療的介入の貴重な好機を提供する。実験モデルであるMecp2欠損マウスが、前臨床研究のために利用可能である。RTTの研究への重要な貢献は、Mecp2欠損マウスにおいて、アストロサイトにおいて優先的に、MeCP2を再発現させることによって、包括的ヌルマウス(globally null mice)と比較して、自発運動および不安のレベルが有意に改善され、呼吸異常が正常パターンに回復し、寿命が大幅に延長されるという概念実証である。これらの研究は、ニューロンと同様に、グリアも、RTTの神経病理の不可欠な成分であることを示した(Lioy et al.,Nature,475(7357):497-500(2012)(非特許文献1))。変異マウスにおいてMecp2発現を回復させたところ、「レット様表現型」からの劇的な回復がもたらされたという画期的な成果は、治療法を見出す意欲を増加させた。しかしながら、障害に関連する症状を緩和するレット症候群の処置は、必要とされたままである。

I. 定義

科学者は、一般に、レット症候群の4つの病期を記載している。発症初期と呼ばれる第1期は、典型的には、6~18ヶ月齢で始まる。障害の症状がやや曖昧であり、発達の軽微な遅延が最初は気付かれない場合があるため、この病期は、しばしば、見過ごされる。乳児は、より少ないアイコンタクトを示し、玩具への低下した関心を有するようになり始めることがある。座位または腹這いなどの粗大運動技能の遅延が起こることがある。手もみおよび頭部成長の減少が起こることがあるが、注意を引くのに十分ではない。この病期は、一般的には、数ヶ月にわたるが、1年より長く続くこともある。
アストロサイトは、健常CNSにおいて、血流の調節、体液/イオン/pH/神経伝達物質のホメオスタシス、シナプス形成/機能、エネルギーおよび代謝、ならびに血液脳関門の維持を含む、多くの必須の複雑な機能を実行する特殊なグリア細胞である(Barres B.A.(2008)Neuron 60:430-440)。ニューロンが、最も高レベルのMeCP2発現を有するが、アストロサイトおよびその他の細胞型も、検出可能なレベルのMeCP2を発現する。最近の研究は、アストロサイトが、レット症候群の進行に寄与する可能性が高いことを示唆している(McGann et al.,Curr Opin Neurobiol.2012 Oct;22(5):850-858を参照すること)。多くの研究が、MeCP2の欠陥によってアストロサイトの機能が損なわれることを示唆している(Maezawa et al.,J Neurosci.2009 Apr 22;29(16):5051-5061)。アストロサイトにおけるMeCP2レベルは、ニューロンよりおよそ5倍低いが(Skene et al.Mol Cell(2010)37:457-468;Maezawa et al.,Neurosci(2009)29:5051-5061)、最近の研究は、アストロサイトにおけるMeCP2の喪失が、レット様症状に寄与し、MeCP2の回復が、これらの欠陥のうちのいくつかを救済し得ることを示唆している(Lioy et al.,Nature(2011)475:497-500)。レット症候群における中心的な役割を考えると、障害の進行を効果的に遅延させ、さらには逆転させるため、正常なアストロサイト機能を回復させる新たな分子標的を同定し、精密に試験することが、大いに必要とされている。アストロサイトがレット症候群の病理に影響し得る、いくつかの可能性のある経路が存在する。
Mecp2欠損マウスにおける生後のMeCP2の包括的な再発現は、正常な寿命をもたらし、運動行動を救済し、全体的な健康状態を改善する(Guy et al.,Science.(2007)315:1143-1147)。発生初期におけるニューロンtau遺伝子座からのMeCP2の発現は、いくつかのレット様症状の出現を防止するため(Luikenhuis et al.,Proc.Nat.Acad.Sci.(2004)101:6033-6038)、ニューロンは救済において重大な役割を果たす可能性が高い。しかしながら、インビトロ研究は、アストロサイトMeCP2が、正常なニューロン形態を支持することを示している(Maezawa et al.,J.(2009)29:505105056;Ballas et al.,Nat.Neurosci.(2009)12:311-317)。従って、アストロサイトも、インビボのレット神経病理の救済において役割を果たす可能性が高い。
本開示の方法は、レット症候群を有する対象を処置するための、抗SEMA4D結合分子、例えば、抗体(その抗原結合断片、バリアント、および誘導体を含む)の使用に関する。ある特定の態様において、内皮細胞がSEMA4D受容体を発現し、他の態様において、ニューロン細胞がSEMA4D受容体を発現し、他の態様において、内皮細胞およびニューロン細胞の両方がSEMA4D受容体を発現する。ある特定の態様において、受容体はプレキシンB1である。以下の記述は抗SEMA4D抗体の投与に言及するが、本明細書に記載される方法は、本開示の抗SEMA4D抗体の所望の特性を保持する、例えば、SEMA4D、例えば、ヒトSEMA4D、マウスSEMA4D、もしくはヒトSEMA4DおよびマウスSEMA4Dと特異的に結合し、SEMA4D中和活性を有し、かつ/またはSEMA-4Dとその受容体、例えば、プレキシンB1との相互作用を阻止する、これらの抗SEMA4D抗体の抗原結合断片、バリアント、および誘導体、またはその他の生物製剤もしくは低分子にも適用可能である。もう1つの態様において、方法は、抗SEMA4D抗体の投与を含むが、本明細書に記載される方法は、プレキシンB1および/またはプレキシンB2と特異的に結合し、SEMA-4Dとそのプレキシン受容体、例えば、プレキシンB1および/またはプレキシンB2の一方または両方との相互作用を阻止することができる抗プレキシンB1結合分子または抗プレキシンB2結合分子の投与も含み得る。
抗SEMA4D結合分子、例えば、抗体、またはその抗原結合断片、変異体、もしくは誘導体を調製する方法、およびこれらを必要とする対象に投与する方法は、当業者に周知であるか、または当業者により容易に決定される。抗SEMA4D結合分子、例えば、抗体、またはその抗原結合断片、変異体、もしくは誘導体の投与の経路は、例えば、経口であるか、非経口であるか、くも膜下腔内であるか、脳室内注射であるか、または吸入によるか、または局所投与であることができる。本明細書において使用される非経口という用語は、例えば、静脈内投与、動脈内投与、腹腔内投与、筋肉内投与、皮下投与、直腸投与、または膣投与を含む。投与のこれらの形態のすべてが、本開示の範囲内であると明らかに企図されるが、投与のための形態の例は、注射用、特に、静脈内または動脈内の注射または点滴用の溶液であろう。注射に適した薬学的組成物は、緩衝剤(例えば、酢酸緩衝剤、リン酸緩衝剤、またはクエン酸緩衝剤)、界面活性剤(例えばポリソルベート)、任意で安定剤(例えばヒトアルブミン)などを含むことができる。しかしながら、本明細書における教示と適合する他の方法において、抗SEMA4D結合分子、例えば、抗体、またはその抗原結合断片、変異体、もしくは誘導体を、有害な細胞集団の部位に直接送達し、それにより患部組織の治療剤への曝露を増加させることができる。
実験設計
Mecp2T158Aレット症候群マウスモデルは、Goffinらによって作製されたものであり(Nature Neuroscience volume 15,pages274-283(2012))、ヒト疾患の表現型特色を密接に再現する。トレオニン158における変異は、レット症候群患者において観察される最も多いMecp2変異であり、トランスジェニックマウスにおいて高度に可変性の表現型を生ずる。Mecp2T158A/yヘミ接合性雄マウス(以後、Mecp2マウスと呼ぶ)は、異なる齢で一連の症状を示し、一般的には、4~6週齢で症状を発症し始める。前臨床試験を、ヘミ接合性Mecp2T158A/y雄マウスおよび野生型C57BL/6雄マウスにおいて実施した。
抗SEMA4Dは、発症前マウスにおいて、後肢抱え込み表現型を改善し、身体振戦を減少させる
Mecp2マウスの後肢抱え込みは、レット症候群患者に存在する特徴的な手もみ常同症に類似した姿勢反応である。Mecp2マウスの表現型後肢抱え込みを図4Aに示す。表2に概説される0から2までの範囲のスコア化基準を使用して、後肢抱え込みの重症度を査定した。アイソタイプ対照抗体を受容した発症前マウスの後肢抱え込みは、10週研究の全体を通して次第に悪化し、およそ1.8のスコアに達した。対照的に、抗SEMA4Dによって処置されたマウスの後肢抱え込みは、3週目にピークに達し(およそ1.1のスコア)、2週間、衰退し、最終的には、プラトーに達し、研究の残りの期間、およそ0.75のスコアであった。野生型マウスは、研究全体を通して後肢抱え込みを示さなかった(図4B)。
時間の経過と共に、レット症候群を有する子供においては、動作、協調運動、コミュニケーションを制御する筋肉の使用に関する問題が増加する。Mecp2マウスは、協調運動および自発運動に関する類似した問題を示す。
高架式十字迷路(EPM)試験は、2つのオープンアームおよび2つの囲われたアームを有する高架式の十字(+)形の装置を使用する。行動モデルは、オープンスペースに対するマウスの一般的な嫌悪に基づく。この嫌悪は、囲われた空間の中、または境界のある空間の端の近くに留まることへの嗜好、即ち、装置の囲われたアームへの動物の移動の制限として表される。高架式十字迷路においては、オープンアームにおいて費やされた時間の割合(オープンアームにいた時間/オープンアームまたはクローズアームにいた時間の合計)の増加、およびオープンアームへの進入回数の割合(オープンアームへの進入回数/オープンアームまたはクローズアームへの進入回数の合計)の増加によって、不安の低減が示される。アーム進入回数の合計およびクローズアーム進入回数は、一般的な活動性の尺度として使用される。
RTT患者は、過換気、無呼吸、バルサルバ法によって終了する息こらえ(breath holds terminated by Valsalva maneuvers)、強制呼吸および深呼吸、持続性吸息、ならびに心拍数制御および心肺統合の異常を含み得る、複雑な呼吸表現型を呈する。重度不整呼吸は、レット症候群の最大の特徴であり、患者およびその家族の生活の質に深刻に影響する。
抗SEMA4Dは、有症状マウスにおいて、後肢抱え込み表現型を改善し、身体振戦を減少させる
表2に概説される0から2までの範囲のスコア化基準を使用して、野生型マウス、および対照によって処置された8週齢有症状Mecp2マウス、および抗SEMA4Dによって処置された8週齢有症状Mecp2マウスの後肢抱え込みの重症度を10週にわたって査定した。アイソタイプ対照抗体を受容した有症状マウスの後肢抱え込みは、10週研究の全体を通して次第に悪化し、アイソタイプ対照発症前マウス(図5B)に類似した、およそ1.8のスコアに達した。対照的に、抗SEMA4Dによって処置されたマウスの後肢抱え込みは、試験開始時に最も高く(およそ1.4のスコア)、試験期間中に減少し、そのレベルより低く維持された。野生型マウスは、研究全体を通して後肢抱え込みを示さなかった(図9)。
抗SEMA4D抗体による処置が、筋肉制御の問題が発生した後に投与された場合に、協調運動および自発運動に影響するか否かを決定するため、有症状マウスおよび対照を、加速式ロータロッド装置において試験した。有症状マウス(8週齢)を、前記のように、トライアルの間に少なくとも15分休息させて、3回のトライアルを行うため、2週毎に、加速式ロータロッド装置に置いた。結果を図11に示す。見て分かるように、アイソタイプ抗体によって処置されたMecp2マウスは、第1週の後に改善を示すことができず、試験期間中に自発運動および協調運動が悪化した。対照的に、抗SEMA4Dによって処置されたMecp2マウスは、研究の過程で有意な改善を示し、さらには、試験全体を通して比較的安定したレベルの技能を保持していた野生型マウスの技能レベルを上回った。
抗SEMA4Dによる有症状マウス(8週齢)の処置が、障害マウスにおいて認知に影響するか否かを決定するため、EPMを使用した。野生型コホート、アイソタイプ抗体によって処置された対照コホート、および抗SEMA4Dによって処置されたコホートの各マウスを、5分間、高架式十字迷路装置(図2)の交差部分に置いた。クローズアームおよびオープンアームにおいて費やされた時間、ならびにクローズアームおよびオープンアームへの進入回数を記録した。野生型マウスは、オープンアームの嫌悪特性の認識を示し、抗SEMA4Dによって処置されたMecp2マウスも同様であったが、アイソタイプ抗体によって処置された対照Mecp2マウスは、EPMのオープンアームの嫌悪特性を学習することができなかった。発症前マウスによって示されたように、野生型マウスおよび抗SEMA4Dによって処置されたMecp2マウスは、アイソタイプ抗体によって処置されたMecp2マウスより、有意に少なくオープンアームに滞在し、オープンアームにいた時間量が減少し(図12A)、アイソタイプ抗体によって処置されたマウスと比較して、オープンアームへの進入回数が少なかったことから、オープンアームの危険性の認識も示した(図12B)。これらの結果は、発症前マウスにおいて抗SEMA4D処置が認知衰退を防止するのと全く同様に、抗SEMA4D抗体による有症状マウスの処置が、認知を増加させることを示している。
SEMA4Dシグナル伝達の遮断が有症状対象の呼吸パターンに対して正の効果を及ぼすか否かを決定するため、抗SEMA4Dによって処置されたMecp2マウス、対照Mecp2マウス、および野生型マウスを、前記のように全身プレチスモグラフィに供した。簡単に説明すると、野生型マウス、抗SEMA4Dによって処置されたMecp2マウス、およびアイソタイプ抗体によって処置されたMecp2マウスを、干渉することなく、3時間、暗室において、図3に示される全身プレチスモグラフィ装置のチャンバーに置いた。各動物の呼吸パターン(表3)を記録し、EMKA ioxソフトウェアによって分析した。
およそ8ヶ月齢の雌マウス(Mecp2T158Aおよび野生型同腹仔対照C57BL/6)に由来する脳のホルマリン固定パラフィン包埋切片を、SEMA4D(CD100ポリクローナル抗体-Invitrogen-PA5-47711)およびニューロンマーカーHuC/HuD(モノクローナル抗体(16A11)-Invitrogen:A-21271)について染色した。海馬領域の代表的な画像を図14に示す。見て分かるように、SEMA4Dは、レットマウスのニューロンにおいて有意にアップレギュレートされているが、野生型対照と比較した、レットマウスにおけるニューロンマーカーHuC/HuDの発現には差がない。
