Abstract
本発明は、ポリメラーゼ連鎖反応(PCR)との関連における、核酸、特に腫瘍核酸における対立遺伝子頻度および/または変異率を測定するための新たな方法、および、少なくとも1つの参照核酸(RN)と、この参照核酸に対する1つの変異配列とが用いられる、この目的のための診断方法に関する。この参照核酸と変異配列とにより、特に機器のパラメータおよび試料調製に基づいてポリメラーゼ連鎖反応(PCR)法を検証することが可能になる。本発明は、さらに、特に液体生検の過程での腫瘍診断のための、関連する診断方法および予後判定方法に関する。【選択図】図3
Description
本発明は、ポリメラーゼ連鎖反応(PCR)との関連における、核酸、特に腫瘍核酸における対立遺伝子頻度および/または変異率を測定するための新たな方法、および、少なくとも1つの参照核酸(RN)と、この参照核酸に対する1つの変異配列とが用いられる、この目的のための診断方法に関する。この参照核酸と変異配列とにより、特に機器のパラメータおよび試料調製に基づいてポリメラーゼ連鎖反応(PCR)法を検証することが可能になる。本発明は、さらに、特に液体生検の過程での腫瘍診断のための、関連する診断方法および予後判定方法に関する。
体液および組織試料における臨床化学検査またはイメージング法などの、腫瘍を検出するための現在の方法は、近年のすべての進歩にもかかわらず、悪性変化の検出が遅すぎる場合が多い。腫瘍患者における高い死亡率の主な原因は、原発腫瘍の発症ではなく、転移の発症である。腫瘍の転移を防ぐためには、できるだけ早く悪性変化を検出する必要がある。さらに、悪性細胞と正常細胞とを正しく区別することができる方法が必要である。偽陽性結果と偽陰性結果は、いずれも罹患した人間に致命的な結果をもたらす。腫瘍患者に、正確な予後判定を提供し、それに従って各患者に個別に合わせた治療を可能にする方法も必要である。
これまで、いわゆる腫瘍マーカ、特に癌遺伝子は、血液、尿、痰または組織試料などの体液で測定されてきた。腫瘍マーカは腫瘍細胞の成分であり、それらの形成は増加または減少し、それらの変化は血液、血漿、または血清などの体液で検出することができる。しかしながら、これらの腫瘍マーカは、診断の特異度と感度がしばしば低すぎるため、腫瘍疾患の一般的スクリーニングには使用できない。これらの腫瘍マーカは、多くの場合、イメージング法よりも早期の段階で悪性変化を示すことができるが、現在のところ、悪性変化の適切な検出を実現していない。
細胞内成分、主に核酸(リボ核酸(RNA)およびデオキシリボ核酸(DNA))の分離、特徴付けおよび分析は、現代の微生物学において極めて重要である。1983年のポリメラーゼ連鎖反応(PCR)の発明以来、多数の核酸診断法が開発されており、それらは疾患および病原体などを検出するために使用されている。
従来技術では、このような細胞物質は、試料調製、抽出、濃縮、分離、精製、逆転写、増幅、および検出などのステップにおける診断方法のために提供されている。
PCR、特に「qPCR」(リアルタイム定量ポリメラーゼ連鎖反応)、「デジタル液滴PCR」(ddPCR)、または「次世代シーケンシング」(NGS、または「超並列シーケンシング」などの並列シーケンシング)による腫瘍DNAの極めて有効な増幅によって、試料中の循環遊離DNAが検査される、いわゆる非侵襲的液体生検が、近年、腫瘍診断において確立されている。このような循環遊離DNA(cfDNA)は、癌/腫瘍細胞に由来する可能性がある(いわゆるctDNA)。
癌患者では、健常被験者よりも平均cfDNA量が高く、特に、進行期の癌患者のcfDNA血漿中濃度は、軽症期/初期の癌患者よりも高い。ctDNAはたとえば、cfDNAの1%を構成する(腫瘍の病期および部位に応じて、正常なcfDNAの0.01~90%)。
しかしながら、相対的な診断情報だけでなく、絶対的な診断情報を得ることができる標準試料の必要性は高い。このような標準試料の使用は、たとえば特許文献1に記載されている。同様に、関連する診断方法が開示されている(請求項183)。
現在、腫瘍診断のために検出が必要な変異については、市販検査薬と自家調製検査薬(LDT:lab‐developed test)の両方が利用可能である。従来技術を構成する、プラスミドDNAまたは合成オリゴヌクレオチドなどの検査室内標準試料(in‐laboratory standard)が、現在、検証または校正の精度管理用に使われている。
しかしながら、これらの検査室内標準試料は、機器または方法の検証または校正のために希釈する必要があり、これを行うための自動処理が提供されておらず、つまり、通常、標準化に誤差が生じやすいのが欠点である。さらに、このような希釈液の安定性は保証されておらず、大腸菌(E.coli)由来のプラスミドDNAなどの起源に基づいて、RNA汚染のリスクがある。したがって、PCRによって測定されたコピー数は、適切な標準化が行われていないために、相対的および絶対的に不正確な場合がある。
したがって、ヒト参照核酸に基づく標準化には、さらなる変異配列を提供することによる、さらなる改善が必要である。このヒト参照核酸と変異配列は、標準試料を構成する。
驚くべきことに、本発明者らは、このような標準試料を用いれば、DNAを含まないヒト血清または血漿試料の使用によって、検証および診断の改善が可能になることを立証することができた。
したがって、本発明が取り組む課題は、試料核酸中の対立遺伝子頻度および/または変異率を検出するための改善された方法とともに、適切な標準試料が使用される関連する診断を提供することである。
課題は、核酸における対立遺伝子頻度および/または変異率を測定することによってポリメラーゼ連鎖反応(PCR)法を検証するための方法であって、

好ましい実施形態では、参照核酸含有量は、血清または血漿中に好ましくは20~600ngのDNAであり、特に400ngのDNAであり、たとえば血清または血漿中に400ng/5mlのDNAは、80ng/mlのDNAに相当し、これは0.08ng/μlのDNAに相当する。
さらに、参照核酸は、50bp~500bpのサイズを有することが好ましい。
本発明の意味において、「1つまたは複数の変異を有するヒト核酸」または「変異配列または変異核酸」は、参照核酸としての対立遺伝子のヒト野生型配列と比較して1つまたは複数の変異を有する配列を意味する。ここで、ヒト野生型配列に関して公知の変異配列を用いることもできるし、または人工変異を導入することもできる。このような人工変異を作製するための方法は、従来技術に記載されている。本発明の方法において、この参照配列および変異配列が公知である(たとえば核酸配列に基づいて記載されている)ことが不可欠である。さらに、変異配列が、公知の変異を有する少なくとも1つの腫瘍マーカ、たとえば癌遺伝子を有することが好ましい。さらに、このような腫瘍マーカ配列または癌遺伝子は、さらなる人工変異を有することができる。
好ましい実施形態では、変異配列の含有量は、参照配列の含有量よりも少ない。さらに、変異配列は、50bp~500bpのサイズを有することが好ましい。
さらに、参照核酸および変異配列は、それぞれ、特定の濃度で存在することが好ましい。
本発明によれば、対立遺伝子頻度または変異率は、たとえば、変異配列に対する参照核酸の存在コピー数、または変異配列に対する参照核酸の濃度に基づく比による、変異配列に対する参照核酸の比(RN/MS)を意味する。
たとえば、参照核酸(「野生型対立遺伝子」)4,000コピーに対して変異配列(「変異対立遺伝子」)200コピー(この場合:4.76%=(200x100%)/(200+4000))が共に存在する場合、対立遺伝子頻度または変異率は4.76%である。対立遺伝子頻度の意味においてこれは4.76%であり、すなわち対立遺伝子変異または変異配列の頻度は4.76%であり、または変異率は4.76%である。
このようにして、特定のまたは設定された対立遺伝子頻度または変異率を有する、本発明の検証試料を提供することができる。
本発明の意味において、検証とは、所定の検証試料に基づいて、特定のまたは設定された対立遺伝子頻度または変異率が、ポリメラーゼ連鎖反応(PCR)の過程で検出できることを意味する。これは、機器のパラメータおよび実施されるポリメラーゼ連鎖反応(PCR)の両方に依存する可能性があり、特に試料調製に依存する可能性がある。検証試料は、キットによって提供することができる。
ポリメラーゼ連鎖反応(PCR)を実施するときに対立遺伝子頻度/変異率に関して検証試料が抽出できない場合は、感度が得られないかまたは検出限界が高すぎて検出できないことになる。さらに、PCR用検証試料を調製するときに、抽出されるDNA物質が少な過ぎる可能性がある。したがって、抽出効率が不十分であり、試料核酸の並行した患者試料または被験者試料を用いる試料調製に派生的な影響をもたらす可能性がある。
例示的な検証は、実施例ならびに図2-1、図2-2および図3に記載されている。
他の好ましい実施形態において、検量線または検証曲線を作成することができる。
例示的な検量線は、実施例および図3に記載されている。
驚くべきことに、このようにして、有利には、適切な特異度および感度で少ない量の試料核酸を検出することができる。これにより、腫瘍活性、特に転移の可能性についての早期の情報を提供することができる。好ましい試料核酸は、患者または被験者のcfDNAまたはctDNAである。
したがって、本発明は、本発明の検証方法が実施されるPCR法によって少なくとも1つの試料核酸の対立遺伝子頻度および/または変異率を測定するための方法に関する。特に有利には、試料核酸は、校正または検証に基づいて定量的に測定することができる。
本発明の他の特定の実施形態は、腫瘍疾患の診断または予後判定のための方法であって、第1試料からの第2試料へおよび/またはさらなる試料への試料核酸の対立遺伝子頻度および/または変異率の変化が、早期検出および検出、重症度の評価および治療に伴う経過評価を可能にする方法において、本発明の検証方法によって校正が実施される、方法に関する。
特に、第2試料またはさらなる試料は、後の時点で患者から採取することができる。
特に有利には、DNAを含まない血清または血漿試料を提供することによって、標準核酸および試料核酸の検出限界を検証することが可能になる。したがって、特に有利な用途は、本発明の方法が実施される、ポリメラーゼ連鎖反応(PCR)、特に次世代シーケンシング(NGS)を実施するための機器の検証に関する。
特に有利には、このようにして、本発明の標準試料の予め正確に定められたDNA物質の量または濃度を試料に導入することができ、それによってこの試料は、患者の血清または血漿試料をほぼ模擬し、腫瘍診断の実施に非常に適したものになる。
さらに、好ましくは変異核酸の濃度は4x10E-05fg/μl~0.03fg/μlであることが好ましい。
本発明の変異配列は、好ましくは腫瘍マーカ、特にMTOR、MPL、NRAS、PARP1、AKT3、DNMT3A、MSH2、IDH1、VHL、MLH1、MYD88、CTNNB1、ATR、PIK3CA、FGFR3、PDGFRA、KIT、FBXW7、APC、GABRG2、NPM1、EGFR、MET、BRAF、EZH2、JAK2、GNAQ、RET、PTEN、ATM、KRAS、PTPNII、FLT3、RB1、PARP2、ARHGAP5、AKT1、RAD51、IDH2、TP53、NF1、SMAD4、AKT2、ERCC1、GNAS、ERBB2、FOXL2、NOTCH1、またはNTKRの群から選択される癌遺伝子を含む。
本発明の意味において、「診断」は、変異率から腫瘍活性についての結論を得ることができることを意味する。変異率が高いほど、腫瘍活性、特に転移の可能性が高くなる。したがって、本発明は癌または腫瘍の診断に関する。「診断」という用語には、医療診断および関連する検査、特にin vitro診断および検査室診断が含まれる。好ましくは、この情報は、患者の疾患または状態に関する。
本発明の文脈において、「患者」は任意の被験者と理解される。

実施例1:
キットの提供による、本発明の方法の実施:
1)QIAamp ccfDNA/RNA Kit(Qiagen(登録商標))、PME Free‐Circulating DNA Extraction Kit(AnalytikJena(登録商標))、またはMagMAX(登録商標)Cell‐Free DNA Isolation Kit(ThermoFisher(登録商標))などの市販キットを用いる、本発明の血清または血漿参照物質からの核酸(DNA)、特にcfDNAの抽出。
実施例1:
キットの提供による、本発明の方法の実施:
1)QIAamp ccfDNA/RNA Kit(Qiagen(登録商標))、PME Free‐Circulating DNA Extraction Kit(AnalytikJena(登録商標))、またはMagMAX(登録商標)Cell‐Free DNA Isolation Kit(ThermoFisher(登録商標))などの市販キットを用いる、本発明の血清または血漿参照物質からの核酸(DNA)、特にcfDNAの抽出。
2)たとえばQubit(登録商標)(ThermoFisher(登録商標))を用いる蛍光測定法、またはNanoDrop(登録商標)または他のスペクトロメータを用いる分光測定法での溶出液中の核酸、特にcfDNAの定量化。
3)任意選択で、バイオアナライザー(Agilent(登録商標))、フラグメントアナライザー(Agilent(登録商標))、またはパルスフィールドゲル電気泳動を用いて、フラグメント長分析などの、核酸、特にcfDNAの定性分析を実施することができる。
4)次いで、得られた核酸、特にcfDNAをPCRにかける。
同時に、患者からの対応する血清または血漿試料を調製することができる。

ddPCRによって、検査される規定量の核酸が、数千の個別の反応空間に分配される。このプロセスにおいて、検査されるDNA配列は、これらの反応空間にポアソン分布に従って分布する。核酸が存在する場合、反応空間内で核酸の増幅が起きる。変異配列は、参照配列(野生型配列)と色で区別できるPCR反応を引き起こす。

ddPCRによって、検査される規定量の核酸が、数千の個別の反応空間に分配される。このプロセスにおいて、検査されるDNA配列は、これらの反応空間にポアソン分布に従って分布する。核酸が存在する場合、反応空間内で核酸の増幅が起きる。変異配列は、参照配列(野生型配列)と色で区別できるPCR反応を引き起こす。
結果は、関連するソフトウェアを使用して評価し、プロット、特に2D振幅プロットに表示することができる:
FAMチャネル(青色蛍光)(チャネル1振幅)で検出された結果はy軸にプロットされる。
FAMチャネル(青色蛍光)(チャネル1振幅)で検出された結果はy軸にプロットされる。

プロットされた各点は、参照配列(野生型配列)との反応(Q4)、変異配列との反応(Q1)、両方の配列との反応(Q2)または、標的核酸が存在しないために配列なしとの反応(Q4)による反応空間(図1a)を表し、得られた点群Q1~Q4は図1bのグラフに示されている。
ソフトウェアによって測定された生データ(プロットされた点)が、参照配列および変異配列の絶対的に定量化された数値になるように評価が行われる。
対立遺伝子頻度は、これらの値から、たとえば手動またはソフトウェアで測定することができる。すなわち、対立遺伝子頻度について、参照配列と変異配列とのコピー数は、次式に従って計算される:

このようにして、キットのチューブ(図2-1および図2-2における0.0、0.1、1、および5%などのプリセット対立遺伝子頻度を有する該当する標準試料)で本発明の方法を行って、有利には以下の情報が得られる:
1)定性的情報:変異は、変異配列で検出可能か? はい/いいえ
2)参照配列と変異配列とについての絶対的に定量化されたコピー数(「CN」)
3)対立遺伝子頻度の測定。

計算値(図2-1、図2-2、表)から検量線を作成し、検証を完了することができる(図3)。
各コピーが「読み取り」に対応し、参照配列と変異配列もシンプルに区別できるため、NGSによってシンプルに評価することもできる。
検量線に基づいて、患者または被験者試料(前出)を、同時にまたは異なる時間に測定することができ、診断を行うことができる。

- 核酸における対立遺伝子頻度および/または変異率を測定することによってポリメラーゼ連鎖反応(PCR)法を検証するための方法であって、
a.)DNAを含まない血清または血漿の試料中の、少なくとも1つのヒト参照核酸と、該参照核酸に対する1つまたは複数の変異を有する1つのヒト核酸とを提供するステップであって、該ヒト参照核酸および該ヒト核酸の特定の対立遺伝子頻度および/または変異率が所定である、提供するステップと、
任意選択で、

- 前記変異核酸が、少なくとも1つの腫瘍マーカを含むことを特徴とする、請求項1~4のいずれか1項に記載の、核酸における対立遺伝子頻度および/または変異率を測定するためのPCR法を検証するための方法。
- qPCR(リアルタイム定量ポリメラーゼ連鎖反応)、デジタル液滴PCR(ddPCR)、または「次世代シーケンシング」(NGS)が実施されることを特徴とする、請求項1~5のいずれか1項に記載の、核酸における対立遺伝子頻度および/または変異率を測定するためのPCR法を検証するための方法。
- PCR法によって少なくとも1つの試料核酸の対立遺伝子頻度および/または変異率を測定する方法であって、請求項1に記載の方法によって校正が実施される、方法。
- PCR法によって少なくとも1つの試料核酸の対立遺伝子頻度および/または変異率を測定する方法であって、前記試料核酸が定量的に測定される、方法。
- 患者の試料核酸がcfDNAまたはctDNAであることを特徴とする、請求項7または請求項8に記載の、PCR法によって前記少なくとも1つの試料核酸の対立遺伝子頻度および/または変異率を測定する方法。
- 腫瘍疾患の診断または予後判定のための方法であって、第1試料からの第2試料へおよび/またはさらなる試料への前記試料核酸の対立遺伝子頻度および/または変異率の変化が、早期検出および検出、重症度の評価および治療に伴う経過評価を可能にする方法において、請求項1に記載の方法によって校正が実施される、方法。
- 前記第2試料またはさらなる試料が後の時点で患者から採取される、請求項10に記載の腫瘍疾患の診断または予後判定のための方法。
- a.)DNAを含まない血清または血漿の試料中の、少なくとも1つのヒト参照核酸と、該参照配列に対して1つまたは複数の変異を有する1つの核酸とを有する部分であって、該ヒト参照核酸および該ヒト核酸の所定の対立遺伝子頻度が設定されている、部分と、
任意選択で、
b.)DNAを含まない血清または血漿の試料中の、ヒト参照核酸を有する部分と、
任意選択で、
c.)DNAを含まない血清または血漿の試料を有する部分と、
を含む、請求項1または請求項7または請求項10に記載の方法を実施するための、キット。 - 請求項1または請求項7または請求項10の方法を実施するための、請求項12に記載のキットの使用。
