本発明は、ノルビキシンの誘導体であり、かつ眼に対する指向性を有する、化学化合物Cに関し、特に網膜色素上皮の変化の文脈、より詳細には加齢性黄斑変性症(AMD)及びシュタルガルト病の文脈で、哺乳類の眼疾患の治療に使用することを目的としたものである。【選択図】図4
Description
本発明は、経口投与後に眼に対する指向性を有する、新規に合成された化学化合物に関する。また本発明は、治療への応用の分野にも関し、また哺乳類の視覚の改善のための上記化学化合物の使用にも関する。
より詳細には、本発明は、網膜色素上皮(retinal pigment epithelium:RPE)の細胞の保護のため、特に哺乳類の加齢性黄斑変性症(age‐related macular degeneration:AMD)又はシュタルガルト病及び網膜色素変性症の治療のための、化合物の使用に関する。本発明の目的は、これらの疾患に苦しめられている個体の視覚を改善すること、又は少なくともこれらの疾患の進行を安定させることである。
加齢性黄斑変性症(AMD)は、特に欧州及び北米において、60歳以上の人々における法的失明の最も一般的な原因である(非特許文献1)。AMDは黄斑と呼ばれる網膜の中心部に影響を及ぼし、重度の視覚障害及び不可逆的な中心視の喪失をもたらす。
黄斑の機能は中心視及び視覚の起源であり、その高い解像度は、特に日中の視覚及び色の知覚に関与する錐体と呼ばれる光受容体における、その高い濃度に関連している。AMDの初期段階は、ドルーゼンと呼ばれる沈着物の出現と、日中の視覚にはわずかしか影響しない、夜間の視覚に特化された桿体と呼ばれる光受容体の漸進的喪失とを特徴とする。しかしながら、対象は自身の視覚の変化を少しずつ観察し、それがその後で眼科医によって診断され、これが中期又は加齢性黄斑変性症(AMD)である。AMDの後期には、滲出性若しくは新生血管性とも呼ばれる湿性形態(網膜下腔での脈絡膜新生血管の成長を特徴とする)、又は網膜色素上皮(RPE)の細胞と、視覚サイクルに必要な細胞である光受容体との喪失を特徴とする地図状萎縮(乾性形態)の、2つの形態があり得る。中期AMD又は乾性形態に苦しめられている患者の数は、湿性形態のAMDに苦しめられている対象の数よりはるかに多い(非特許文献1)。AMDの後期段階は、その形態(湿性又は乾性)にかかわらず、黄斑の不可逆的な破壊につながる。滲出性AMD(湿性形態)の進行は、数週間で完全な失明につながる場合があるが、乾性AMDの進行は一般に遅い。
AMDの発症に関与する具体的な機序は複数存在するものの、酸化ストレス及び炎症がその生理病理学に寄与する重要な要素であることが示されている。AMDの病因論には、脂質を含む細胞外物質の全身的蓄積によって引き起こされるブルッフ膜の流体力学的変化、及び光受容体の生存にその活性が不可欠であるRPEの老化が含まれる。AMDの第1のリスク因子である加齢は、RPEの細胞の機能不全及び代謝の機能不全、並びに食作用の機能不全を引き起こす。光受容体の外部セグメントの不完全な消化は、ドルーゼンの形成につながる可能性があり、これはブルッフ膜を介した拡散を減少させる。年齢と共にRPEはますます多量のリポフスチンを蓄積させる。リポフスチンは脂質及びタンパク質で構成され、これらはRPE中に存在するリソソームであるファゴリソソームに由来し、また光受容体に直接由来する。リポフスチンはまた、N‐レチニル‐N‐レチニリデンエタノールアミン(A2E)も含有し、これは、2つのレチンアルデヒド分子と1つのエタノールアミンとの縮合によって形成される。
年齢と共に、AMDに罹患した患者の網膜中のA2Eの蓄積の増加が観察される(非特許文献2)。青色光の作用下、かつ酸素の存在下において、A2Eは反応性種を生成し、これはタンパク質、脂質及びDNAに損傷を与え、従ってRPEの老化細胞に相当な酸化ストレスを引き起こす(非特許文献3)。この損傷は、RPEの細胞のリソソーム活性を阻害し、老廃物の蓄積を引き起こし、これはあちこちでRPEの細胞の死を引き起こし、その後、それらが関連する光受容体の死を引き起こす。
シュタルガルト病及び網膜色素変性症は、多数の突然変異又は遺伝子多型によって引き起こされ得る、遺伝的起源の網膜変性である。これらの2つの病状は、シュタルガルト病の場合はA2Eの漸進的蓄積に関連する、RPE及び光受容体の細胞の漸進的喪失を特徴とするため、AMDの乾性形態と同等と考えることができる。AMDの乾性形態と同様に、光受容体及びRPEの変性は、これらの2つの遺伝的病状に苦しめられている患者による、夜間の視覚の喪失、そして中心視の低下につながる。
既存の治療の中で、抗VEGF(Vascular Endothelial Growth Factor:血管内皮増殖因子)抗体の硝子体内注射が知られており、これは、新生血管の形成を部分的に遮断できるため、AMDの湿性形態のための治療の選択肢を提供できる。現在、AMDの乾性形態(AMD及び地図状萎縮)に対する治療法は市場に存在しない。同様に、シュタルガルト病又は網膜色素変性症の治療のための薬剤も入手できない。乾性AMDの枠組みの中では、食品サプリメントが、一般的な抗酸化化合物,即ち抗酸化剤特性を有するミネラル及びビタミン、例えば亜鉛、ビタミンA、C、Eを用いて処方されている。これらの治療有効性は実際にあるが、AMDに限定されている。AREDS栄養補助食品処方1、2(非特許文献4)は、米国においてAMDの治療のための治療標準とみなされており、5年間で湿性AMDの進行のリスクを25%低減し、視覚の喪失を19%低減する。多くの製品が共通の製剤ベース、即ち亜鉛及びビタミンC、Eを提案しており、これらにルテイン、レスベラトロール、オメガ3脂肪酸といった様々な成分が加えられるが、これらの追加の成分、又はこれらの様々な分子に対して好ましい応答を示すことができる患者のカテゴリーに関して、説得力のある有効性のデータは存在しない(非特許文献5)。
カロテノイド(食物摂取のみに由来する分子)は、そのうちのいくつか(ルテイン、ゼアキサンチン=キサントフィル)が黄斑中に自然に存在するため、より具体的に研究され(非特許文献6)、これらの化合物が強い抗酸化力を有し、青色光を吸収することによってその毒性作用を制限することが分かっている。従って、これらの化合物をAREDS処方において(単独で又は組み合わせて)試験したのは、論理的である。得られた結果は残念なものであり、その補給は、これらの化合物が不足している患者のサブセットにのみ有効であった(非特許文献7)。他のキサントフィルも、単独での、又はルテイン及び/若しくはゼアキサンチンと組み合わせての経口補給による研究の対象であった(例えばアスタキサンチン、非特許文献8)。ジアポカロテノイド(=両端が切除されたカロテノイド、IUPAC化学命名法)、特にクロセチン(=8,8’‐ジアポカロテン‐8,8’‐ジオエート)及びその配糖体(クロシン)を、インビトロ及びインビボで試験した。クロシンは、ウシ又は霊長類の光受容体の初代培養物に対して、インビトロで光防御効果を有し(非特許文献9)、クロセチンは神経節細胞を酸化ストレスから保護する(非特許文献10)。また、別のアポカルテノイドであるビキシン(=6‐メチル水素[9Z]6,6’‐ジアポカロテン‐6,6’‐ジオエート)又はその誘導体のうちのいくつかを用いて、神経節細胞に対してインビトロで、及び小胞体のストレスの影響を相殺するために硝子体内注射によってインビボで、実験を実施した(非特許文献11)。ビキシンで強化された、過去に開発されたUrucum種子抽出物(Bixa orellana)(Bixilia(登録商標)は、UVに曝露されたヒトの皮膚に対する(特許文献1;非特許文献12)、及び光酸化ストレスを受けたRPE細胞に対する(特許文献2;非特許文献13)、光防御効果を示した。最後に、ノルビキシン(6,6’‐ジアポカロテン‐6,6’‐ジオエート)及び特にその9’‐シス形態は、N‐レチニル‐N‐レチニリデンエタノールアミン(A2E)で予備処理されたRPEの細胞の、青色放射による照明によって引き起こされる細胞死を、大幅に削減できる(特許文献3;非特許文献14、非特許文献15)。9’‐シスノルビキシンによる経口慢性治療は、ダブルKOマウス(ABCA4-/-、RDH8-/-)の網膜におけるA2Eの蓄積を減少させることもできる(非特許文献15)。ノルビキシンは良好なバイオアベイラビリティを有するが、眼内でのその蓄積量は非常に低いままであるため、網膜の保護活性は制限される。

本発明の枠内では、「薬学的に許容可能な(pharmaceutically acceptable)」は、一般に安全で毒性がなく、生物学的に又は他の面で望ましくないものではなく、獣医学における使用及びヒトの医薬品にとって許容可能な、医薬組成物の調製に有用であるものを意味する。
本発明の枠内では、用語「化合物の薬学的に許容可能な塩(pharmaceutically acceptable salts of a compound)」 は、本明細書で定義されているように薬学的に許容可能な、そして親化合物の所望の薬理学的活性を有する、塩を意味する。このような塩としては、以下が挙げられる:

実施例1(ダイヤグラムA)
化合物C21の調製
ステップ1(アミンとカルボニルジイミダゾール(CDI)との典型的なカップリング法):C21のメチルエステルの調製
化学反応をアルゴン雰囲気下で実施した。反応の全てのステップの間、並びに抽出及び溶媒除去のステップの間、試薬及び産物をアルミニウム箔で光から保護した。産物を4℃で貯蔵した。ビキシン(1g;2.54mmol)を10mLの乾燥ジメチルホルムアミド(DMF)中に溶解させ、トリエチルアミン(1.06mL;7.61mmol)、続いてCDI(0.823g;5.07mmol)を加える。2時間後、2‐アミノエチルフラン(845mg;7.61mmol)を加え、反応混合物を20℃で18時間撹拌した。塩酸(HCl、1N;20mL)を加え、得られた懸濁液を遠心分離する。上清の除去後、メチルエステルを含有するペレットを、水の存在下で2回再懸濁させた。再び遠心分離した後、ペレット(1.24g)をそのまま加水分解ステップで使用した。

化合物C19の調製
ステップ1(HBTUを用いたアミンのカップリングのための典型的な手順):C19のメチルエステルの調製(CDIを用いた反応はこの化合物には遅すぎるため、C19のメチルエステルの調製のみに使用される方法)
化学反応をアルゴン雰囲気下で実施した。反応の全てのステップの間、並びに抽出及び溶媒除去のステップの間、試薬及び産物をアルミニウム箔で光から保護した。産物を4℃で貯蔵した。ビキシン(1.17g;2.97mmol)を20mLの無水DMF及びジイソプロピルアミン(1.23mL;7.42mmol)中に溶解させ、続いてHBTU((1H‐ベンゾトリアゾール‐1‐イルオキシ)‐ジメチルアミノヘキサフルオロリン酸)‐N、N’‐ジメチルメタンイミニウム;1.69g;4.45mmol)を加える。1時間後、トランス‐4‐アミノシクロヘキサノール(680mg;4.45mmol)を加え、反応混合物を20℃で18時間撹拌する。60mLの水を加え、沈殿物を濾過して60mLの水で2回洗浄し、1.4gの紫色の固体の化合物を得て、これをそのまま加水分解ステップで使用する。

化学反応をアルゴン下で実施し、アルミニウム箔で光から保護した。産物を4℃で貯蔵した。1.4gのC21のメチルエステルを、10mLのテトラヒドロフラン(THF)及び10mLのMeOH中に溶解させる(暗赤色の溶液)。5当量の水酸化ナトリウム(NaOH、10N;1.43mL)を加え、反応混合物を周囲温度で48時間撹拌する。HPLC‐MSでは、単一のピーク及び開始材料の不在が観察される。塩酸(HCl 12N;1.18mL)を加えた。得られた沈殿物を60mLの水で希釈する。溶液を遠心分離して、水性上清を除去する。続いて固体のペレットを得て、水(30mL)と混合してから再び遠心分離し、上清を除去する。このステップを、(水の存在下で懸濁液を形成する)橙色の固体ペーストが得られるまで2回繰り返す。この固体ペーストを50mLの水に入れて橙色の懸濁液を得て、これを冷凍して直接凍結乾燥し、橙色の粉末(1.02g)を得る。

ノルビキシン由来の化学化合物(C)の生物学的効果のカスケードスクリーニング及び特性決定
スクリーニング試験の開発は、乾性AMDの病理の特徴に基づいて、文献の研究から開始された。生理病理学的レベルでは、この疾患は、光受容体及びRPEの細胞の変性に続く、視覚の漸進的喪失の誘発を特徴とする。RPEの細胞は、光受容体に必要な栄養素を供給すること、視覚サイクルに関与すること、及び光受容体の外部セグメントから来る、このサイクルに由来する破片を除去することによって、光受容体の生存及び正しい動作に重要な役割を果たす。患者に外傷を与える、及び局所的感染のリスクを有する眼内投与を回避しながら、好ましくは眼を標的とする能力に関して、開発中の薬剤をスクリーニングすることが重要である。これを実施するために、本発明の目的であるノルビキシン由来の化学化合物(C)の、血漿及び眼内レベルでの薬物動態研究を実施して、眼に関するAUC(area under the curve:曲線下面積)がノルビキシンに関して改善された化合物を選択した。

標的組織中でのより良好な分布に加えて、新たな化合物の選択も、RPEの細胞の喪失を制限することによって、AMD並びに網膜色素変性症及びシュタルガルト病等の他の変性疾患の間に観察される網膜変性を低減する、良好な光防御活性に基づくものである必要がある。細胞レベルでは、ブタの網膜に由来するRPEの細胞の培養物について、非特許文献15は、ノルビキシン(BIO201)による処置が、RPEの細胞を、A2Eの存在下での照明後のアポトーシスから保護する(曝露後24時間で80%の生存率)ことを示している。光防御のパーセンテージに従った同じスクリーニング試験を用いて、本発明の目的であるノルビキシン由来の化学化合物(C)によるその変調を、ノルビキシンの光防御効果と比較して決定し、これらの変調を統計学的観点から特性決定した。
プロトコル
マウスにおける分子の経口投与による薬物動態研究
経口投与後の化学化合物(C)の薬物動態研究を、マウスC57BL/6(Janvier、53940、ル・ジュネスト=サンティスル、フランス)を用いて実施した。ノルビキシン由来の化学化合物(C)を、50mg/kg体重の用量で投与した。投与後、t=0.25時間;0.5時間;1時間;3時間;6時間;8時間において尾から血液を採取した。血液サンプルを遠心分離して血漿を得た。血漿サンプルの投与量によって、薬物動態パラメータ、即ち分子の投与後に観察される最大濃度に相当するCmax、分子の投与後に最大濃度に到達するために必要な時間であるTmax、及び血漿曝露に対応する曲線の下の面積であるAUCの決定が可能となった(図2)。

並行して、複数の眼内濃度の、ノルビキシン由来の化学化合物(C)を、以下の方法で投薬した(図1):各マウスの両眼をPrecellysチューブに取り、投薬時まで-80℃で貯蔵した。次に眼を、ベンチトップホモジナイザFast‐prep(Fischer Scientific、ハンプトン、米国)を用いて、最初のステップでは500μLのクロロホルム/メタノール(1/1、v/v)、続いて500μLのクロロホルム/ジクロロメタン(1/1、v/v)である有機溶媒の混合物中で、眼を粉砕する。各ステップにおいて上清を回収して、2mLの96ウェルプレートに移す。
化学化合物Cの定量化のために、同一の有機溶媒混合物で8つの標準液(5~5,000ng/mL)を用いて検量線を作成し、2mLの96ウェルプレートに移す(100μL)。
上清及び標準液をEZ2(Genevac、イプスウィッチ、英国)内で加熱せずに揮発させ、100μLのDMSO/メタノール(20:80、v/v)に取り込んだ後、200μLの96ウェルプレートに移す。
LC‐MSMS分析を、HPLC 1200 Infinity chain(Agilent Technologies、サンタクララ、米国)、UV検出器、及び質量分析計QQQ6420(Agilent Technologies、サンタクララ、米国)を用いて実施する。注入体積は5μLである。化学化合物Cを、逆相カラムC18(2.1×50mm、粒子3μm;Ace‐C18‐Excel、AIT)上で、アセトニトリル及び水(0.1%のギ酸を含有)のグラジエントと0.3mL/分の流量とを用いて溶出させる。グラジエントの条件は、分析される化学化合物Cに応じて変化し得る。UV検出器は460nmで分析を行い、質量分析計はモードMRM‐Positiveで分析を行う。
RPEの細胞の光防御
様々な化学化合物Cによる、A2Eの存在下で照明されたRPEの細胞の光防御のインビトロ試験
上述の、ノルビキシンの光防御効果の研究を目的としたインビトロ試験を用いて、ノルビキシン由来の様々な化学化合物(C)の、A2Eの存在下で青色光で照明されたRPEの細胞に対する光防御効果を定量化した(図3)。上記分子の光防御効果を、A2Eでの処置及びこれに続く青色光での照明によって誘導された光毒性の細胞モデルで評価した。用語「青色放射(blue radiation)」は、可視光スペクトルの青色帯域に対応する、即ち435~490nmの波長の放射を意味する。このモデルは、成ブタのRPEの初代培養物を使用する。細胞生存率を、細胞生存率試験によって定量化する。-48時間において、(DMSO中の5mMの溶液である)試験対象の化合物を加えて1~20μMとし、続いて-19時間においてA2Eを加え(最終濃度30μM)、細胞を照明する(時点:0時間)。24時間後に細胞の生存率を測定する。画像の取得及びその処理は、Metamorphソフトウェア及び専用の定量化プログラムで制御された蛍光顕微鏡を用いて実施する。実験は、4連の96ウェルマイクロプレート上で実施され、実験は少なくとも4回再現される。結果は、試験対象の分子で処置されたウェル内の生存細胞の個数を、(A2Eを含まない希釈媒体で処置された)対照ウェル中の生存細胞の個数で除算したものを表す比の形式で表される。この試験は以前に、ノルビキシンの光防御活性を明らかにすることができた(非特許文献15)。

- 哺乳類の網膜色素上皮の細胞の光防御のために使用するための、請求項1若しくは2に記載の一般式(I)を有する化学化合物、又は請求項3~5のいずれか1項に記載の組成物。
- 可視光スペクトルの青色帯域に対応する435nm~490nmの波長の青色光への曝露によって引き起こされる、哺乳類の網膜への損傷の治療及び/又は予防のために使用するための、請求項1若しくは2に記載の一般式(I)を有する化学化合物、又は請求項3~5のいずれか1項に記載の組成物。
- 網膜変性によって引き起こされる哺乳類の網膜への損傷の治療及び/又は予防のために使用するための、請求項1若しくは2に記載の一般式(I)を有する化学化合物、又は請求項3~5のいずれか1項に記載の組成物。
