本発明は、乳酸菌発酵代謝物を含有する安定な眼科局所用組成物に関するものである。
本発明は、眼科分野及び点眼薬の分野に由来する。
特に、本発明は、ポストバイオティクスを有効成分として含み、かつポストバイオティクスが水溶液中に安定するように保持する特定の製剤を有する、眼への投与のための局所使用組成物(composition for topical use)に関する。
プロバイオティクスは、生物学的に活性な量で投与される場合に宿主に有益な効果を発揮する生きた微生物である。ポストバイオティクスは、プロバイオティクスによって行われる発酵過程の副産物である。
ポストバイオティクスとは、酵素、ペプチド、テイコ酸、ムロペプチド、多糖類、細胞表面のタンパク質、ペプチドグリカン由来の有機酸などの、生きた細菌により分泌されるか又は細菌溶解後に放出された可溶性因子、すなわち代謝産物又は副産物を指す。
ポストバイオティクスは、生物学的活性を持ち、安全性の高い製品であると考えられているため、栄養学および薬学領域での利用が常に増加している。
点眼薬の形態でのプロバイオティクスの使用も、例えば、Bonniらにより知られている。
ポストバイオティクスは、水溶液中での安定性が低いため、眼科分野での使用が困難である。
したがって、安定性を保つポストバイオティクスを含む利用可能な眼科用製剤を提供する必要がある。
本発明の目的の1つは、生物学的活性成分としてポストバイオティクスを含有する眼科局所用組成物を提供するということにある。
生物学的活性成分として特定のポストバイオティクスを含有する高い安定性を有する眼科用組成物が現在同定されており、本発明の主題を形成する。
そのような組成物は、眼科用液剤中のポストバイオティクスを安定に維持する製剤を有する。
特に、本発明の組成物は、ポストバイオティクスのタンパク質成分が、タンパク質の三次及び四次構造の変化又はその変性に由来するタンパク質凝集などの不安定現象を引き起こすことを防止し、実質的に低減するものである。
第1の態様によれば、以下を含む眼局所用組成物が提供される。
ラクトバチルス・カゼイ種又はラクトバチルス・パラカゼイ種(Lactobacillus casei or paracasei species)の発酵産物であるポストバイオティクスと、
特に一水和物であるリン酸水素ナトリウム、及び特に十二水和物であるリン酸二ナトリウムを含む緩衝系(buffer system)と、
塩化マグネシウム、塩化カルシウム及びそれらの混合物を含む等張化剤と、
生理学的に許容される水性担体と、を含み、
前記組成物は、pHが6.7~6.9、好ましくは6.8~6.9である。本発明に係る組成物は、以下に詳細に説明するように、任意の緩衝剤、等張化剤及び防腐剤を更に含んでもよい。

前記組成物は、pHが6.7~6.9、好ましくは6.8~6.9である。本発明に係る組成物は、以下に詳細に説明するように、任意の緩衝剤、等張化剤及び防腐剤を更に含んでもよい。
このようにして得られた眼科用組成物は、眼の表面レベルで作用する驚くべき能力を示すため、この眼の部分(ocular segment、眼体節)の疾患を予防又は治癒するように適合された、有効性が高く安全マージンが広い局所治療法を実行することを可能にする。この新しい形態の治療法は、同じ疾患に対する他の既知の治療法と組み合わせて使用することができる。
本発明は、本明細書で定義されるポストバイオティクスを含む組成物/水溶液がその安定を保持し、ポストバイオティクスのタンパク質成分の変性を防止するか又は大幅に低減することを可能にする特定の条件を見出したことに由来する。
これらの条件は、a)適切な緩衝剤又は緩衝系を用いて、6.7~6.9の範囲、好ましくは6.8~6.9の範囲の溶液のpHの選択と、b)塩化マグネシウム及び塩化カルシウムなどの二価カルシウム及びマグネシウム塩の溶液中の存在などである。
出願人は、また、リン酸水素ナトリウム及びリン酸二ナトリウムの緩衝剤を使用して、所望の範囲におけるpHの効果的な調節がどのように得られるかも観察した。
本発明の主題は、請求項1に定義されるような眼科用組成物である。
組成物の更なる実施形態は、添付の従属請求項2~9に定義される。
組成物の生物学的活性成分は、ポストバイオティクスを含み、特にラクトバチルス・カゼイ種又はラクトバチルス・パラカゼイ種、好ましくはラクトバチルス・パラカゼイに属する細菌株からの発酵産物を含む。
好ましい実施形態において、プレバイオティクス(プロバイオティクス)は、CNCM(Collection Nationale de Cultures de Microorganismes、Institut Pasteur、Paris、FR)番号I-5220でブダペスト条約に従って、2017年7月26日に寄託されたラクトバチルス・カゼイCNCMI-1390の株である。
いくつかの実施形態において、組成物は、0.05~5%、0.1~1.9%、0.2~1%の重量/体積(g/100ml)の量のポストバイオティクスを含む。
本発明の組成物は、特に一水和物であるリン酸水素ナトリウム、及び特に十二水和物であるリン酸二ナトリウムを含む緩衝剤又は緩衝系を含む。
好ましくは、リン酸水素ナトリウム一水和物/リン酸二ナトリウム十二水和物の比が16/41=0.39の比率である。
いくつかの実施形態において、組成物は、クエン酸ナトリウムを含む更なる緩衝剤を含む。
本発明の組成物は、塩化マグネシウム、塩化カルシウム及びそれらの混合物から選択される少なくとも1つの等張化剤を含む。好ましい実施形態において、等張化剤は、塩化マグネシウム及び塩化カルシウムを含む。好ましくは、塩化マグネシウムは、塩化マグネシウム六水和物であり、塩化カルシウムは二水和物である。好ましくは、塩化マグネシウム六水和物/塩化カルシウム二水和物の比が10/7=1.42の比率である。
いくつかの実施形態において、本発明の組成物は、グリセロールを含む更なる等張化剤を含む。
典型的には、本発明の組成物は、本明細書に記載されているポストバイオティクスが分散される生理学的に許容される水ベースの担体を含む。好ましくは、水は精製されたものである。
本発明の組成物は、pHが6.7~6.9、好ましくは6.8~6.9である。したがって、組成物の成分である緩衝剤、等張化剤は、pHを6.7~6.9、好ましくは6.8~6.9にするような量で本発明の組成物中に存在する。
一実施形態において、本発明の組成物は、以下の製剤を有する:
好ましい形態によれば、組成物は、実施例3の製剤を有する。本発明の組成物は、本明細書で定義されるように、安定である。
更に、組成物において、pHを6.7~6.9にするような量のリン酸水素ナトリウム緩衝液、好ましくは一水和物、及び好ましくは十二水和物であるリン酸二ナトリウムと、塩化マグネシウム及び塩化カルシウムに基づく等張化剤との組み合わせにより、組成物は、安定なものになる。
本明細書において、「安定」という用語は、組成物が、特にポストバイオティクスに関して、40℃及び25℃で6ヶ月で、平均粒子分散(サイズ)≦200nmであることを意味する。したがって、試験条件下で、ポストバイオティクスに由来する物質の凝集体の形成に起因する有意な変動、すなわち、25℃又は40℃で200nmを超えるサイズの増加は、発生しない。
本発明の範囲内で、ポストバイオティクスという用語は、ラクトバチルス・カゼイ種、ラクトバチルス・パラカゼイ種又はそれらの混合物の発酵産物、特にラクトバチルス・パラカゼイCNCMI-5220の発酵産物を意味する。
前の実施形態のいずれかに係る組成物が上皮細胞に対してバリア効果を有する場合、ポストバイオティクスに生物界面活性剤、糖タンパク質、有機酸及びペプチドの混合物が存在するため、ポストバイオティクスのこれらの成分は、病原性細菌の眼粘膜への結合を防止することができることが観察された。その結果、組成物は、眼感染症に対して予防作用又は治療を果たす。
更に、組成物は、免疫調節活性を有し、眼アレルギー及び/又は炎症の予防及び/又は治療に適用することができる。
組成物は、単回投与又は複数回投与の形態とすることができる。
特に、緩衝成分は、眼組織および担持された活性成分と適合する5~8の間のpHを得る/維持するような濃度で組成物中に存在する。
好ましくは、組成物は、涙液と等張であり、270~310mOsm/Kgの値を有する。
好ましくは、本発明の組成物は、洗眼剤又は眼科用ゲルである。複数回投与で処方された本発明に係る組成物は、例えば、パラベン、第四級アンモニウム塩、ポリヘキサメチレンビグアニド(PHMB)、及び眼科用の組成物に使用できる成分に含まれる可能性のある他の成分などの抗菌防腐剤を含んでもよい。組成物に使用される溶媒は、好ましくは、水、又は局所眼科使用に適合性のある1つ以上の成分の水溶液である。
投与の目的で、前述の組成物は、点眼薬として、或いは液体又はゲルの形態で示すことができる。
本発明の更なる主題は、眼の炎症及び/又はアレルギーの治療又は予防のための薬物の製造における上記定義された組成物の局所眼科使用である。
本発明の組成物は、眼アレルギー、春季カタル、乾性角結膜炎及び眼瞼炎の治療に使用することができる。
本発明の組成物は、また、以下の実施例1において実験的に示されるように、目の表面への異物又は微生物の付着の防止に適用することができる。
以下の実施例は、本発明を限定することなく本発明を更に説明する。
本実験研究では、以下でEyeLac(登録商標)と呼ばれる、ラクトバチルス・パラカゼイCNCMI-5220の発酵プロセスに由来するポストバイオティクスを含む実施例3の製剤は、使用された。2つのバッチのEyeLac(登録商標)(R&D1907、R&D1908)組成物の付着防止効果、安全性及び安定性を評価するために、様々なアッセイが開発された。
方法:
担体(carrier、VHC)に関する製剤R&D1907及びR&D1908の付着防止特性を、付着防止アッセイを使用して黄色ブドウ球菌(S.a.)ATCC43300の株に対して評価した。特に、200μlの各製剤を各ウェルに添加し、プレートを4℃で18時間インキュベートした。その期間後、試験物質を除去し、3×108ufc/mlの密度でS.a.ATCC43300の細菌懸濁液と同じ量で交換し、プレートを4℃で4時間インキュベートして、微生物の付着を促進した。4時間後、リン酸緩衝生理食塩水(PBS)で2回洗浄し、ウェルに付着しない細菌懸濁液を除去し、一方、ウェルあたり200μlの99%メタノールを添加し、プレートを室温(RT)で15分間インキュベートすることにより、付着した微生物を固定した。次に、メタノールを除去し、プレートを37℃で5分間インキュベートすることにより、残留メタノールを蒸発させた。最後に、各ウェルに200μlの2%クリスタル・バイオレットを添加することにより、付着した微生物の着色をRTで5分間行った。その後、過剰な色素を除去し、ウェルあたり200μlの33%氷酢酸を添加することにより、付着した微生物に結合した色素を溶解した。分光光度計で595nmで読み取ることにより、ウェルあたりの光学密度を測定した。ゼロ時間(T0)と6ヶ月(T6)に、25℃及び40℃で全てのバッチに対して付着防止アッセイを実行した。データは、3回の繰り返しの平均±SEMを表し、統計分析を一元配置ANOVAとそれに続くボンフェローニ検定により実行した。

短期間の試験項目への反復曝露(STRE)を想定した標準プロトコルに対して改良された細胞毒性プロトコルを使用して、ヒト角膜上皮細胞株HCEに対して、製剤の安全性プロフィールを評価した。細胞を、異なる濃度(2.5、5、10、20、100%)の両方の製剤R&D1907及びR&D1908で5分間繰り返し(6X)処理した。各処理の後、プロトコルは、2つの異なる条件に従った。
洗浄:HCEに5分間接触した後、PBSで洗浄することにより処理物を除去した。この条件は、瞼を閉じたときに洗眼剤の一部が涙液膜で洗い流される、生体内で起こることを表している。
非洗浄:HCEに対して5分間処理した後、洗眼剤を洗浄せずに除去した。この条件は、前の(洗浄)の「悪化した(exasperated)」状況を表し、涙液膜の体積が変化する状況と比較することができる。
洗浄:HCEに5分間接触した後、PBSで洗浄することにより処理物を除去した。この条件は、瞼を閉じたときに洗眼剤の一部が涙液膜で洗い流される、生体内で起こることを表している。
非洗浄:HCEに対して5分間処理した後、洗眼剤を洗浄せずに除去した。この条件は、前の(洗浄)の「悪化した(exasperated)」状況を表し、涙液膜の体積が変化する状況と比較することができる。
説明した両方のプロトコルにおいて、1つの処理と次の処理の間に、37℃の完全培養培地中での90分間の「回復」期間がある。示された時間が経過した後、細胞生存率をMTTアッセイにより評価した。
50%未満の細胞生存率の値は、細胞毒性を示す(DB-ALM:プロトコル番号17)。製剤の安定性プロファイルを評価するために、25±2℃/60±5%RHの長期安定試験条件及び40±2℃/75±5%RHの増殖安定条件という2つの異なる温度条件を考慮した。安定性試験中に、様々な時間間隔(0、1、2、3及び6ヶ月)におけるpH値、浸透圧、平均粒子分布(サイズ)、ゼータ電位(pZ)及び多分散度指数(PDI)という化学的/物理的パラメータを評価した。
結果:
付着防止試験:試験した全てのバッチの製剤は、試験した両方の温度で、S.a.ATCC43300に対して付着防止活性を示し、VHCに関して最大32.3%の微生物阻害値を示した(図1)。更に、付着防止効果は、6ヶ月まで変化しないように保持されるようである。実際、いずれかの製剤のT0とT6月の間、又は製剤自体(R&D1907、R&D1908)の間で統計的に有意な差は、検出されなかった。
付着防止試験:試験した全てのバッチの製剤は、試験した両方の温度で、S.a.ATCC43300に対して付着防止活性を示し、VHCに関して最大32.3%の微生物阻害値を示した(図1)。更に、付着防止効果は、6ヶ月まで変化しないように保持されるようである。実際、いずれかの製剤のT0とT6月の間、又は製剤自体(R&D1907、R&D1908)の間で統計的に有意な差は、検出されなかった。
安全性評価:HCEに対して実行された細胞生存率アッセイに関連する結果は、STREプロトコルに従って実行された処理後のバッチR&D1907及びR&D1908が、洗浄条件又は非洗浄条件のいずれかで、試験濃度のいずれかで細胞毒性がないことを示した(図2のA~B)。
安定性試験:2つのバッチの製剤の安定性試験データは、それぞれ図3(A~E)及び図4(A~E)で、試験時間の関数としてグラフに示される。増殖安定条件又は長期安定性条件のいずれでも、安定性試験中に化学的物理的パラメータ(pH、サイズ、PDI、pZ及び浸透圧)に顕著な違いは見られなかった。本製品は安定であると考えることができる。
結論:
この結果は、眼の局所用製剤が安全で安定し、生物界面活性剤、タンパク質及び有機酸の混合物の存在による付着防止活性を特徴とすることを示唆する。これらの結果は、そのような製剤が、眼感染症、アレルギー、角結膜炎などの、結膜上皮のバリア機能が損なわれる疾患を予防するために使用されてもよいことを示す。
この結果は、眼の局所用製剤が安全で安定し、生物界面活性剤、タンパク質及び有機酸の混合物の存在による付着防止活性を特徴とすることを示唆する。これらの結果は、そのような製剤が、眼感染症、アレルギー、角結膜炎などの、結膜上皮のバリア機能が損なわれる疾患を予防するために使用されてもよいことを示す。
6.7~6.9、特に6.8~6.9の範囲でのpHの選択が、組成物が安定するように保持し、タンパク質画分が生物学的に活性なポストバイオティクス成分から沈殿することを防止するという目的にとって非常に重要であることを確認する目的で、試験を実行した。
試験した製剤の好ましいpHは、6.8~6.9(実施例3の製剤1)に等しく、一方、ラクトバチルス・パラカゼイCNCM I-5220の発酵から得られたポストバイオティクス(以下、ポストバイオティクスと呼ばれる)を含む溶液のpHは、緩衝液がない場合に6.3に等しい。
本発明の組成物は、ポストバイオティクスのタンパク質の三次及び四次三次元構造が変性して凝集体を形成しないように維持することに寄与するpHを有する。平均粒子分散(サイズ)データは、バッチR&D1907及び1908のサイズ≦200nmであるため、バッチR&D1907及び1908に最適であり、40℃及び25℃で6ヶ月までの安定性研究中に、凝集体の形成による有意な変動、すなわち、サイズの増加は、見出されなかった。
カルシウム、マグネシウム塩の製剤における存在は、安定化に寄与し、グリセロールは、等張化機能を有するだけでなく、増粘剤/懸濁剤機能を有する。二価カルシウム塩とマグネシウム塩の存在は、タンパク質に存在する(-COO-)基との弱い相互作用を形成し、タンパク質構造を安定化させることができるため、非常に重要である。
図5Bに示すように、必要に応じて水和物の形態のリン酸水素ナトリウム及びリン酸二ナトリウム(例えば、リン酸水素ナトリウムが一水和物であり、リン酸二ナトリウムが十二水和物である)の緩衝系が存在しないが、等張化剤が存在する場合の製剤に対する試験では、40℃で1週間の研究を行った後に沈殿の形成が製剤中に観察されているため、安定した製剤は得られなかった。
他の試験は、pH値が5.6~5.8に等しい以下の緩衝剤(製剤3及び製剤4)に対して実行されたが、低濃度のポストバイオティクス(0.1%)で満たす結果が得られず、得られた平均粒子分散(サイズ)値は、どちらの場合もサイズ>200nmである。サンプルのサイズが大きく、あまり均一ではない。これらの因子は、タンパク質の凝集及び製剤の不安定性の指標である。
Claims (11)
- ラクトバチルス・カゼイ種、ラクトバチルス・パラカゼイ種又はそれらの混合物の発酵産物であるポストバイオティクスと、
好ましくは水である液体担体と、
pHを6.7~6.9、好ましくは6.8~6.9にするような量の、好ましくは一水和物であるリン酸水素ナトリウム、及び好ましくは十二水和物であるリン酸二ナトリウムを含む緩衝系と、
特に六水和物である塩化マグネシウム塩、及び特に二水和物である塩化カルシウムを含む等張化剤とを含む、安定した眼科局所用組成物。 - ポストバイオティクスは、2017年7月26日にCNCM(Collection Nationale de Cultures de Microorganismes、Institut Pasteur、Paris、France)番号I-5220で寄託されたラクトバチルス・カゼイ株CNCMI-1390の発酵産物である、請求項1に記載の組成物。
- 塩化ナトリウム、塩化カリウム及びそれらの混合物から選択される更なる等張化剤を含む、請求項1又は2に記載の組成物。
- グリセロールベースの懸濁等張化剤を更に含む、請求項1~4のいずれか1項に記載の組成物。
- クエン酸ナトリウムである更なる緩衝剤を含む、請求項1~5のいずれか1項に記載の組成物。
- 塩化マグネシウム六水和物/塩化カルシウム二水和物=10/7=1.42の比率、及び/又は、リン酸水素ナトリウム一水和物/リン酸二ナトリウム十二水和物=16/41=0.39の比率を特徴とする、請求項1~7のいずれか1項に記載の組成物。
- 6ヶ月後に40℃で粒子平均分散(サイズ)≦200nmであることを特徴とする、請求項1~8のいずれか1項に記載の組成物。
- 微生物性眼感染症の予防又は治療に使用され、又は、眼アレルギー、春季カタル、乾性角結膜炎、眼瞼炎の治療に使用される、請求項1~9のいずれか1項に記載の組成物。
- 外部体又は微生物が眼の表面に付着することを防止するために使用される、請求項1~9のいずれか1項に記載の組成物。
