ヒト抗IL-34抗体のパネルを、完全ヒト酵母ディスプレイライブラリーを使用して取得し、効果的なヒトIL-34中和抗体であり得る試薬を特定するためにスクリーニングされる。親和性が改善されたクローンを単離するために、変異を各抗体の個々の相補性決定領域(CDR)に体系的に導入し、得られたライブラリーを、抗原濃度を低下させ、かつ/又は解離の時間の増やしながら、複数ラウンドの選択にかける。個々のバリアントの配列を決定し、使用して、コンビナトリアルライブラリーを構築し、それを、ストリンジェンシーを高めながら追加ラウンドの選択にかけて、個々のCDR領域間の相加的又は相乗的な変異のペアリングを特定する。個々のコンビナトリアルクローンを、配列決定し、結合特性を決定する。IL-34に対する親和性を更に高めるために、これらのコンビナトリアルクローンを追加ラウンドの単一及びコンビナトリアル突然変異誘発にかけてもよい。このスクリーニングをヒト又はカニクイザルIL-34に対して行い、選択した種に対する親和性を高めることができる。選択した抗体を変異誘発して、IL-34に対する結合親和性を維持しながら、異性化などの翻訳後改変を修復することもできる。追加的に、潜在的な免疫原性リスクを低減するために、フレームワーク(FW)又はCDR置換を抗体に対して行って、配列をそれらの生殖系列状態に戻すことができる。
ヒト及びカニクイザルIL-34に対する結合親和性
ヒト及び/又はカニクイザル(cyno)IL-34に対する本開示の抗IL-34モノクローナル抗体の結合親和性は、当技術分野で公知の方法によって決定することができる。手短に言うと、抗体の結合親和性及び動態学は、BIAcore(商標)8K(Cytiva)を37℃で使用して表面プラズモン共鳴によって評価される。結合親和性は、BIAcore(商標)Sensor Chip Protein A(Cytiva)に抗IL-34抗体を固定化し、HBS-EP+緩衝液(Teknova)で2倍連続希釈した25nM又は12.5nMから開始して、ヒト又はカニクイザルIL-34を流すことによって測定される。各サイクルで、200μLのIL-34を固定化抗体上に100μL/分で流し、次いで20分間解離させる。チップ表面は、pH1.5のグリシン緩衝液50μLで流速100μL/分で再生される。データを1:1のLangmiur結合モデルに適合させてkon、koffを導き出して、KDを計算する。表3は、例示される抗体1についてヒト及びカニクイザルIL-34のための少なくとも3回の実験の平均を示す。
本開示の抗体を、IL-34結合及び/又は活性を中和する能力について試験する。本開示の抗体によるIL-34結合及び/又は活性の中和は、例えば、以下に記載のように、1つ以上のIL-34/CSF1R受容体結合アッセイフォーマット、並びにIL-34細胞ベースの活性アッセイによって評価され得る。
IL-34/CSF1R結合の中和抗体のアッセイは、酵素アッセイを使用して行うことができる。そのようなアッセイは、IL-34に結合することができる組換え発現CSF1R細胞外ドメインタンパク質を使用することができる。可溶性IL-34を捕捉するために、これらのタンパク質をELISAプレートに結合させることができる。次いで、IL-34は、抗原のビオチン化、及びストレプトアビジン/ニュートラアビジン結合ペルオキシダーゼ又はホスファターゼ酵素による検出のいずれかによって検出されることができる。そのような中和アッセイは、結合アッセイに添加する前に、標識されたIL-34(並びにIL-34を標的とする抗体が関与しない対照試料)で評価される抗体のプレインキュベーション(例えば1時間)を含む。
ヒトIL-34中和は、U2OS CSF1R/CSF1R細胞(Path Hunter(登録商標)eXpress Dimerization Assay、DiscoverX)を96ウェルプレートに播種して、抗IL-34抗体がCSF1Rの二量体化を阻害する能力を評価することによって更に評価できる。これらのアッセイは、b-ガラクトシダーゼ(b-gal)酵素がProLink(PK)とEnzyme Acceptor(EA)との2つのフラグメントに分割されるEnzyme Fragment Complementation(EFC)技術を利用している。独立的に、これらのフラグメントにはb-gal活性がないが、しかしタンパク質間相互作用によって補完を強いられると、それらは活性なb-gal酵素を形成する。PathHunter(登録商標)eXpress Dimerizationアッセイは、CSF1R受容体-二量体対の2つのサブユニットのリガンド誘導性二量体化を検出する。細胞は、酵素ドナー(ED)に融合した1つのCSF1R受容体サブユニットと、酵素アクセプター(EA)に融合した第2の二量体パートナーを共発現するように設計されている。ヒトIL-34が受容体サブユニットの1つに結合すると、その二量体パートナーとの相互作用が誘導され、2つの酵素フラグメントの補完が強いられる。これにより、基質を加水分解して化学発光シグナルを生成する機能酵素が形成される。表6に示す相対蛍光単位(RFU)の低減は、抗体1がヒトIL-34を中和し、化学発光を低減する能力を反映している。抗体1の半最大阻害濃度(IC50)値は、1.035nMである。ヒトCSF1R-Fcは、このアッセイにおいて陽性対照として使用され、1.025nMのIC50でRFU単位を阻害する。表6のデータは、抗体1がヒトIL-34とCSF1Rとの相互作用をブロックする能力を支持し、その能力によりこのアッセイにおいてCSF1Rの二量体化が阻害される。このデータは、ヒトIL-34を中和するための本開示の抗体の使用を支持する。
本開示の抗体によるIL-34活性の中和は、例えば以下に記載するように、1つ以上のIL-34細胞ベースのアッセイによって評価することができる。ヒトIL-34誘導性ルシフェラーゼレポーター活性を中和する本開示の抗体の能力は、ヒトCSF1Rを発現するcDNAでトランスフェクトされた293 hCSF1R SRE細胞で評価することができる(受入:NP_001275634.1)。例えば、ヒトCSF1R(hCSF1R)を安定的に過剰発現する293/SRE細胞を0.05%トリプシン-PBS中で解離させ、組織培養処理した96ウェルプレートに100ul当たり70,000細胞で播種する。翌日、増殖培地を除去し、熱不活化1%FBS(ウシ胎児血清)を添加したDMEM-F12(ダルベッコ改変イーグル培地:栄養混合物F-12)で細胞を飢餓状態にする。飢餓の24時間後、細胞を100ng/mlのヒトIL-34及び複数濃度のhCSF1R-Fc又は抗体1のいずれかで6時間処理する。インキュベーション後、細胞を50ulのPromega(商標)Glo(商標)溶解緩衝液(Promega(商標)E266A)で5分間穏やかに撹拌しながら溶解する。50mlのBrightGlo(商標)発光試薬(Promega(商標)E2620)を添加し、溶解細胞上で2分間インキュベートする。発光を、Perkin Elmer Wallac 1420 Victor2(商標)マイクロプレートリーダーで読み取る。表7及び図1に示す相対蛍光単位(RFU)の低減は、ヒトIL-34誘導ルシフェラーゼ活性を中和する抗体1の能力を反映している。hIL-34の中和について、抗体1の半最大阻害濃度(IC50)値は、0.05037ug/mlである。ヒトCSF1R-Fcは、このアッセイにおいて陽性対照として使用され、0.09603ug/mlのIC50でルシフェラーゼ活性を阻害する。
IL-34の中和は、NIH-3T3/CSF1Rにおける細胞外シグナル調節キナーゼ(ERK)リン酸化を阻害する抗IL-34抗体の能力を評価することによって決定し得る。このアッセイでは、1日目に10%FBSを補充したDMEMに細胞を播種し、37℃で一晩インキュベートする。2日目に、培地を除去し、細胞を無血清DMEMで洗浄し、更に24時間インキュベートする。3日目に、培地を、抗IL-34抗体を含む無血清DMEMに交換する。ヒト又はカニクイザルIL-34を5分間添加して、最終濃度1ug/mlにする。ヒト又はカニクイザルIL-34及びアイソタイプ対照抗体のいずれかが、それぞれ陽性及び陰性対照として機能する。リン/全ERK1/2レベルは、Whole Cell Lysate Kit(Meso Scale Discovery、カタログ番号K15107D)を使用して電気化学発光シグナルを測定することによって評価する。データを、リン-ERK1/2対全ERK1/2タンパク質の電気化学発光シグナルの比として計算される。表8及び/又は図2に示されるシグナル比の低下は、IL-34活性を中和する抗体1の能力を反映している。ヒトIL-34に対する抗体1の半最大阻害濃度(IC50)値は26nMであり、カニクイザルIL-34に対しては53nMである。
IL-34の中和は、フローサイトメトリーによってIL-34で処理した後のヒト単球における細胞表面抗原CD163の発現を測定することによっても評価し得る(例えば、Boulakirba,S.,et al.による、IL-34及びCSF-1は、同等のマクロファージ分化能力を示すが、分極電位は異なる。Sci Rep 8、256(2018を参照されたい)。CD14陽性単球をIL-34で6日間処理し、CD163について抗体を染色した後、フローサイトメトリーでCD163の発現を評価する。実験では、CD163を発現する細胞数の変化は、IL-34処理が単球におけるこの抗原の発現を増加させることを示している。抗体1の添加により、CD163の発現上昇が抑制される。この実験では、アイソタイプが一致したIgG4抗体を陰性対照として使用する。結果を、表10に示す。
樹状細胞(DC)内在化アッセイ
単球由来DC培養(MDDC)
標準プロトコルに従って、CD14+単球を末梢血単核細胞(PBMC)から分離させ、培養し、DCに分化させる。手短に言うと、LRS-WBCからFicoll(#17-1440-02、GE Healthcare)及びSepmate 50(#15450、STEMCELL Technologies)による密度勾配遠心分離を使用してPBMCを分離する。製造元のマニュアルに従って、CD14+マイクロビーズキット(#130-050-201、Miltenyi Biotec)による陽性対照を使用してCD14+単球を分離する。次いで、1000単位/mlのGM-CSF及び600単位/mlのIL-4とともに、細胞を100万/mlで6日間培養して、L-グルタミン及び25mMのHEPESを添加し、10%FBS、1mMのピルビン酸ナトリウム、1×ペニシリン-ストレプトマイシン、1×非必須アミノ酸、及び55μMの2-メルカプトエタノールを補充したRPMI培地(以下、Life Technologiesから購入した完全RPMI培地又は培地と呼ぶ)中で未成熟樹状細胞(MDDC)に誘導する。培地を2日目及び5日目の2回変える。6日目に、セルスクレーパーで細胞を静かに収集し、実験に使用する。MDDCを、顕微鏡による樹状形態、及びフローサイトメトリーによるCD14、CD11c、及びHLA-DRの発現について視覚的に特徴付ける。LPS治療に応答する能力は、フローサイトメトリーを使用してCD80、CD83、及びCD86の増加を測定することによって確認される。
F(ab’)2フラグメントヤギ抗ヒトIgG(Jackson ImmunoResearch)をQSY7-NHS及びTAMRA-SE(Molecular Probes)で二重標識して、試験品の内在化を追跡するためのユニバーサルプローブとして使用するFab-TAMRA-QSY7を取得する。F(ab’)2(1.3mg/mlで約1ml)の各バイアルを、Amico Ultra-0.5遠心フィルター装置(#UFC501096、Millipore)を使用して14,000rcfで2分間遠心分離することにより、約2mg/mlに濃縮する。pHを10%(v/v)1M重炭酸ナトリウムで塩基性(>pH8)に調整し、DMSO中10mMのQSY-NHS保存液6.8μlを加えて混合する。反応バイアルを室温で30分間暗所に保つ。中間生成物であるFab-QSY7を、Zeba Spin脱塩カラム(#89890、Thermo Scientific)を用いて、相対遠心力(RCF)1000で2分間遠心分離することにより精製する。NanoDrop(ThermoFisher)で280nm及び560nmの吸光度を測定することにより、濃度及び標識度(DOL)を計算する。次いで、再びAmico Ultra-0.5遠心フィルター装置を用いて14,000rcfで2分間遠心分離することにより、Fab-QSY7を約2mg/mlに濃縮する。10%(v/v)1M重炭酸ナトリウムでpHを調整した後、DMSO中の15mMのTAMRA-SE保存液4.3μlを加えて混合する。暗所で室温で30分後に、最終生成物Fab-TAMRA-QSY7を精製し、1000rcfで2分間遠心分離することによりZeba Spin脱塩カラムを使用して収集する。NanoDrop分光光度計で280nm、555nm、及び560nmの吸光度を読み取ることにより、濃度及びDOLを再度定量化する。このプロトコルを使用すると、F(ab’)2当たり約2つのQSY7と2つのTAMRAと、を含む約1.5mg/mlのFab-TAMRA-QSY7を約300μlが得られる。
個々の試験分子をPBSで1mg/mlに正規化し、次いで完全RPMI培地で更に8μg/mlに希釈する。Fab-TAMRA-QSY7を完全RPMI培地で5.33μg/mlに希釈する。抗体とFab-TAMRA-QSY7とを等量で混合し、複合体形成のために暗所で、4℃で30分間インキュベートする。MDDCを完全RPMI培地に400万/mlで再懸濁し、抗体/プローブ複合体50μlを加えた96ウェル丸底プレートにウェル当たり50μlで播種する。細胞をCO2インキュベーターで、37℃で24時間インキュベートする。細胞を2%FBS PBSで洗浄し、Cytox Greenのlive/dead色素を含む100μlの2%FBS PBSに再懸濁する。BD LSR Fortessa X-20でデータを収集し、FlowJoで分析する。生単一細胞をゲーティングし、TAMRA蛍光陽性細胞の割合を読み出しとして記録する。
分子は、2連又は3連で3人以上のドナーで試験する。各ドナーについて、TAMRA陽性集団の割合を考慮する。異なるドナーから生成されたデータと分子の比較を可能にするために、正規化された内在化指数(NII)が使用される。内在化シグナルは、次の式を使用してIgG1アイソタイプ(NII=0)及び内部陽性対照PC(NII=100)に対して正規化する。
10人の正常なヒトドナーからの初代ヒト樹状細胞は、以下に記載のように、CD-14陽性細胞の分離によるバフィーコートから調製され、5%血清代替物(Thermo Fisher Scientific、カタログ番号A2596101)を含む完全RPMI培地で20ng/ml IL-4及び40ng/ml GM-CSFとともに37℃、5% CO2で3日間インキュベートすることによって未成熟樹状細胞に分化させた(Knierman et al.,“The Human Leukocyte Antigen Class II Immunopeptidome of the SARS-CoV-2 Spike Glycoprotein”,Cell Reports,33,108454(2020))。4日目に3マイクロモルの試験抗体を約5×106細胞に添加し、細胞を成熟樹状細胞に形質転換するために5μg/mlのLPSを含む新鮮な培地を5時間のインキュベーション後に交換する。翌日、成熟細胞をプロテアーゼ阻害剤及びDNAseを含む1mlのRIPA緩衝液中で溶解する。ライセートを、サンプル分析まで-80℃で保存する。
このアッセイは、試験候補又は試験候補のMAPPs由来ペプチドクラスターの、以下に記載のように細胞増殖を誘導することによるCD4+T細胞を活性化する能力を評価する(Walsh et al.,“Post-hoc assessment of the immunogenicity of three antibodies reveals distinct immune stimulatory mechanisms”,mAbs,12,1764829(2020))。10人の健康なドナーからの凍結保存したPBMCを使用し、PBMCからCD8+T細胞を枯渇させ、1μMカルボキシフルオレセインジアセテートスクシンイミジルエステル(CFSE)で標識した。PBMCを、5%CTS(商標)Immune Cell SR(Gibco、カタログ番号A2596101)を含むAIM-V培地(Life Technologies、カタログ番号12055-083)に4×106細胞/ml/ウェルで播種し、様々な試験物質、DMSO対照、培地対照、及びキーホールリンペットヘモシアニン(KLH;陽性対照)を含む2.0mLで、3連で試験した。細胞を培養し、CO25%、37℃で7日間インキュベートした。7日目に、ハイスループットサンプラー(HTS)を装備したBD LSRFortessa(商標)を使用したフローサイトメトリーによる生存率検出のために、試料を次の細胞表面マーカーで染色した:抗CD3、抗CD4、抗CD14、抗CD19、及びDAPI。FlowJo(登録商標)ソフトウェア(FlowJo、LLC、TreeStar)を使用してデータを分析し、細胞分裂指数(CDI)を計算した。手短に言うと、各試験分子のCDIは、刺激されたウェルで増殖しているCFSEdimCD4+T細胞のパーセントを、刺激されていないウェルで増殖しているCFSEdimCD4+T細胞のパーセントで割ることによって計算した。2.5以上のCDIは、陽性反応を表すとみなした。すべてのドナーにわたるドナー頻度の割合を評価した。抗体1の結果を表13に示す。
カニクイザルに、1mL/kgの容量のPBS(pH7.4)中の抗体1の3mg/kgの単回静脈内(IV)用量を投与する。薬物動態学的特徴付けのために、投与後1、3、6、24、48、72、96、120、168、240、336、408、504及び672時間後に2匹の動物/時点から血液を採取し、血清に処理する。抗体1の血清濃度は、適格な免疫親和性液体クロマトグラフィー質量分析法によって決定される。抗体1及びヒト抗体内部標準(安定同位体標識ヒトIgG)を、ビオチン化ヤギ抗ヒトIgG抗体を使用して100%カニクイザル血清から抽出し、続いてQ-Exactive(商標)Orbitrap(登録商標)質量分析計を使用してトリプシンサロゲートペプチドを定量化する。薬物動態パラメータを、各動物(N=2)について非コンパートメント分析(NCA)を使用して計算し、パラメータを平均値によって要約する。NCA及び要約統計計算は、Phoenixを使用して行う。表14に示すように、抗体1は、カニクイザルにおいて薬物動態プロファイルの拡張を示す。

- 前記抗N3pGlu Aβ抗体が、最長72週間の一連の治療期間にわたって、又はアミロイドの正常レベルが達成されるまで、前記ヒト対象に投与される、請求項31~35のいずれか一項に記載の方法。
- 前記抗N3pGlu Aβ抗体は、患者におけるアミロイドプラークレベルが約25センチロイド以下になるまで、前記ヒト対象に投与される、請求項31~36のいずれか一項に記載の方法。
- 前記抗N3pGlu Aβ抗体は、前記ヒト対象におけるアミロイドプラークレベルが2回の連続PETイメージングスキャンで約25センチロイド以下になるまで、前記ヒト対象への一連の治療のために投与され、適宜、前記2回の連続PETイメージングスキャンが、少なくとも6ヶ月間隔があるか、又は1回のPETイメージングスキャンで約11センチロイド以下となる、請求項31~36のいずれか一項に記載の方法。
- 前記ヒト対象が、3回の700mgの第1の用量のドナネマブを4週間ごとに1回、次いで1400mgの第2の用量を4週間ごとに1回、最長72週間の一連の治療期間にわたって投与される、請求項31~36のいずれか一項に記の載の方法。
- 前記ヒト対象は、前記対象におけるアミロイドプラークレベルが約25センチロイド以下になるまで、3回の700mgの第1の用量を4週間ごとに1回、次いで1400mgの第2の用量を4週間ごとに1回投与される、請求項31~36のいずれか一項に記載の方法。
- 前記ヒト対象は、前記対象におけるアミロイドプラークレベルが2回の連続PETイメージングスキャンで約25センチロイド以下になるまで、3回の700mgの第1の用量のドナネマブを4週間ごとに1回、次いで1400mgの第2の用量を4週間ごとに1回投与され、適宜、前記2回の連続PETイメージングスキャンが少なくとも6ヶ月間隔があるか、又は1回のPETイメージングスキャンで約11センチロイド以下となる、請求項31~36のいずれか一項に記載の方法。
- 前記ヒト対象が、前記疾患を治療又は予防するのに十分な一連の治療期間にわたって、前記第2の用量のドナネマブを投与される、請求項31~41のいずれか一項に記載の方法。
- 前記疾患の治療又は予防が、i)前記ヒト対象の脳内のAβ沈着物の低減、及び/又はii)前記ヒト対象の認知若しくは機能低下の遅延を引き起こす、請求項31~42のいずれか一項に記載の方法。
- 前記ヒト対象の脳内のAβ沈着物の低減が、アミロイドPET脳イメージング又はAβのバイオマーカーを検出する診断法によって決定される、請求項43に記載の方法。
- 前記第2の用量は、前記ヒト対象の脳内のAβ沈着物が約20~100%低減するまで、前記ヒト対象に投与される、請求項43又は44に記載の方法。
- 前記ヒト対象の脳内のAβ沈着物が、約20%、約25%、約30%、約35%、約40%、約45%、約50%、約75%、又は約100%低減する、請求項45に記載の方法。
- 前記第2の用量のドナネマブは、前記ヒト対象の脳内のAβ沈着物が、i)およその平均で約25センチロイド~約100センチロイド、ii)およその平均で約50センチロイド~約100センチロイド、iii)約100センチロイド、又はiv)約84センチロイド低減するまで、前記ヒト対象に投与される、請求項31~44のいずれか一項に記載の方法。
- 前記ヒト対象の脳内のAβ沈着を特徴とする疾患が、前臨床アルツハイマー病(AD)、臨床AD、前駆期AD、軽度AD、中等度AD、重度AD、ダウン症候群、臨床的脳アミロイド血管症、又は前臨床的脳アミロイド血管症から選択される、請求項31~47のいずれか一項に記載の方法。
- 前記ヒト対象が、早期症候性AD患者である、請求項31~48のいずれか一項に記載の方法。
- 前記ヒト対象が、前駆期AD及びADによる軽度の認知症を有する、請求項49に記載の方法。
- 前記ヒト対象が、i)非常に低いから中等度のタウ負荷を有するか、若しくは非常に低いから中等度のタウ負荷を有することが決定されている、ii)低いから中等度のタウ負荷を有するか、若しくは低いから中等度のタウ負荷を有することが決定されている、iii)非常に低いから中等度のタウ負荷を有するか、若しくは非常に低いから中等度のタウ負荷及びAPOE e4の1つ又は2つの対立遺伝子を有することが決定されている、iv)低いから中等度のタウ負荷を有するか、若しくは低いから中等度のタウ負荷及びAPOE e4の1つ又は2つの対立遺伝子を有することが決定されている、又はv)APOE e4の1つ又は2つの対立遺伝子を有する、請求項26~50のいずれか一項に記載の方法。
- 前記ヒト対象は、i)PET脳イメージングによって測定されるタウ負荷が1.46SUVr以下である場合、非常に低いから中等度のタウ負荷を有するか、又はii)PET脳イメージングによって測定されるタウ負荷が1.10SUVr~1.46SUVrである場合、低いから中等度のタウ負荷を有する、請求項51に記載の方法。
- 前記ヒト対象が、i)高いタウ負荷を有していないか、若しくは高いタウ負荷を有していないと決定されているか、又はii)APOE e4の1つ又は2つの対立遺伝子を保有し、高いタウ負荷を有していないか、若しくは有していないと決定されている、請求項26~50のいずれか一項に記載の方法。
- 前記ヒト対象は、PET脳イメージングによって測定されるタウ負荷が1.46SUVrを超える場合、高いタウ負荷を有する、請求項53に記載の方法。
- 前記ヒト対象の前記タウ負荷が、PET脳イメージング又はタウのバイオマーカーを検出する診断法を使用して決定される、請求項51又は53に記載の方法。
- ヒト対象の脳内のAβ沈着物を特徴とする疾患の治療又は予防のための薬剤の製造における、抗体1と同時、別々、若しくは順次組み合わせた、抗N3pGlu Aβ抗体の使用であって、
1回以上の約100mg~約700mgの第1の用量の前記抗N3pGlu Aβ抗体が投与され、各第1の用量が、約4週間ごとに1回投与され、続いて1回以上の700mg超~約1400mgの第2の用量が、前記1回以上の第1の用量の投与の4週間後に投与され、各第2の用量の抗N3pGluAβ抗体が、約4週間ごとに1回投与され、
前記抗N3pGlu Aβ抗体が、ドナネマブである、使用。 - 前記ヒト対象が、ドナネマブの前記第2の用量を投与する前に、ドナネマブの前記第1の用量を1回、2回、又は3回投与される、請求項56に記載の使用。
- 前記ヒト対象が、約700mgのドナネマブの第1の用量を3回投与される、請求項56又は57に記載の使用。
- 前記ヒト対象が、1回以上の約800mg、約900mg、約1000mg、約1100mg、約1200mg、約1300mg、又は約1400mgの第2の用量のドナネマブを投与される、請求項56~58のいずれか一項に記載の方法。
- 前記ヒト対象が、1回以上の約1400mgの第2の用量のドナネマブを投与される、請求項56~59のいずれか一項に記載の使用。
- 前記抗N3pGlu Aβ抗体が、最長72週間の一連の治療期間にわたって、又はアミロイドの正常レベルが達成されるまで、前記ヒト対象に投与される、請求項56~60のいずれか一項に記載の使用。
- 患者におけるアミロイドプラークレベルが、約25センチロイド以下になるまで、前記抗N3pGlu Aβ抗体が、前記ヒト対象に投与される、請求項56~61のいずれか一項に記載の使用。
- 前記抗N3pGlu Aβ抗体は、患者における前記アミロイドプラークレベルが2回の連続PETイメージングスキャンで約25センチロイド以下になるまで、前記ヒト対象に投与され、適宜、前記2回の連続PETイメージングスキャンが、少なくとも6ヶ月間隔があるか、又は1回のPETイメージングスキャンで約11センチロイド以下となる、請求項56~61のいずれか一項に記載の使用。
- 前記ヒト対象が、3回の700mgの第1の用量のドナネマブを4週間ごとに1回、次いで1400mgの第2の用量のドナネマブを4週間ごとに1回、最長72週間の期間にわたって投与される、請求項56~61のいずれか一項に記載の使用。
- 患者における前記アミロイドプラークレベルが、約25センチロイド以下になるまで、前記ヒト対象が、700mgのドナネマブの第1の用量を4週間ごとに1回で3回投与され、次いで、1400mgのドナネマブの第2の用量を4週間ごとに1回投与される、請求項56~61のいずれか一項に記載の使用。
- 前記ヒト対象は、患者におけるアミロイドプラークレベルが2回の連続PETイメージングスキャンで約25センチロイド以下になるまで、3回の700mgの第1の用量のドナネマブを4週間ごとに1回、次いで1400mgの第2の用量のドナネマブを4週間ごとに1回投与され、適宜、前記2回の連続PETイメージングスキャンが少なくとも6ヶ月間隔があるか、又は1回のPETイメージングスキャンで約11センチロイド以下となる、請求項56~61のいずれか一項に記載の使用。
- 前記ヒト対象が、前記疾患を治療又は予防するのに十分な一連の治療期間にわたってドナネマブの前記第2の用量を投与される、請求項56~66のいずれか一項に記載の使用。
- 前記疾患の治療又は予防が、i)前記ヒト対象の脳内のAβ沈着物の低減、及び/又はii)前記ヒト対象における認知若しくは機能低下の遅延を引き起こす、請求項56~67のいずれか一項に記載の使用。
- 前記ヒト対象の脳内のAβ沈着物の低減が、アミロイドPET脳イメージング又はAβのバイオマーカーを検出する診断法によって決定される、請求項68に記載の使用。
- 前記ヒト対象の脳内のAβ沈着物が、約20~100%低減するまで、ドナネマブの前記第2の用量が、前記ヒト対象に投与される、請求項68又は69に記載の使用。
- 前記ヒト対象の脳内のAβ沈着物が、約20%、約25%、約30%、約35%、約40%、約45%、約50%、約75%又は約100%低減される、請求項70に記載の使用。
- 前記患者における脳内のAβ沈着物が、100%低減する、請求項70又は71に記載の使用。
- 前記ヒト対象の脳内のAβ沈着物が、i)およその平均で約25センチロイド~約100センチロイド、ii)およその平均で約50センチロイド~約100センチロイド、iii)約100センチロイド、又はiv)約84センチロイド低減されるまで、ドナネマブの前記第2の用量が、前記ヒト対象に投与される、請求項56~72のいずれか一項に記載の使用。
- 前記ヒト対象の脳内のAβ沈着物を特徴とする疾患が、前臨床アルツハイマー病、臨床的AD、前駆期AD、軽度AD、中等度AD、重度AD、ダウン症候群、臨床的脳アミロイド血管症、又は前臨床的脳アミロイド血管症から選択される、請求項56~73のいずれか一項に記載の使用。
- 前記ヒト対象が、早期症候性AD患者であるか、又は前記ヒト対象が、前駆期AD若しくはADによる軽度の認知症を有する、請求項56~74のいずれか一項に記載の使用。
- 前記ヒト対象が、i)非常に低いから中等度のタウ負担を有するか、若しくは非常に低いから中等度のタウ負荷を有することが決定されている、ii)低いから中等度のタウ負荷を有するか、若しくは低いから中等度のタウ負荷を有することが決定されている、iii)非常に低いから中等度のタウ負荷を有するか、若しくは非常に低いから中等度のタウ負荷及びAPOE e4の1つ又は2つの対立遺伝子を有することが決定されている、iv)低いから中等度のタウ負荷を有するか、若しくは低いから中等度のタウ負荷及びAPOE e4の1つ又は2つの対立遺伝子を有することが決定されている、又はv)APOE e4の1つ又は2つの対立遺伝子を有する、請求項56~75のいずれか一項に記載の使用。
- 前記ヒト対象が、i)PET脳イメージングによって測定されるタウ負荷が1.46SUVr以下である場合、非常に低いから中等度のタウ負荷を有するか、又はii)PET脳イメージングによって測定されるタウ負荷が1.10SUVr~1.46SUVrである場合、低いから中等度のタウ負荷を有する、請求項76に記載の使用。
- 前記ヒト対象が、i)高いタウ負荷を有していないか、若しくは高いタウ負荷を有していないと決定されているか、又はii)APOE e4の1つ又は2つの対立遺伝子を保有し、高いタウ負荷を有していないか、若しくは高いタウ負荷を有していないと決定されている、請求項56~75のいずれか一項に記載の使用。
- PET脳イメージングによって測定されるタウ負荷が1.46SUVrを超える場合、前記ヒト対象が、高いタウ負荷を有する、請求項78に記載の使用。
- 前記ヒト対象のタウ負荷が、タウPET脳イメージング又はタウのバイオマーカーを検出する診断法を使用して決定される、請求項76又は78に記載の使用。
- i)非常に低いから中等度のタウ負荷、若しくは低いから中等度のタウ負荷を有するか、又はii)非常に低いから中等度のタウ負荷、若しくは低いから中等度のタウ負荷及びAPOE e4の1つ又は2つの対立遺伝子を有することが決定されているヒト対象の脳内のアミロイドベータ(Aβ)沈着物を特徴とする疾患を治療又は予防する方法であって、
i)前記ヒト対象に、1回以上の約100mg~約700mgの第1の用量のドナネマブを投与すること、ここで、ドナネマブの各第1の用量が約4週間ごとに1回投与され、
ii)前記1回以上の第1の用量を投与した4週間後に、前記ヒト対象に、1回以上の700mg超~約1400mgの第2の用量のドナネマブを投与することを含み、ここで、各第2の用量が、約4週間ごとに1回投与され、
有効量の抗体1と同時に、別々に、又は順次組み合わせる、方法。 - ヒト対象の脳内のアミロイドベータ(Aβ)沈着物を特徴とする疾患を治療又は予防する方法であって、
前記ヒト対象が脳の側頭葉、後頭葉、頭頂葉、又は前頭葉にタウ負荷を有する場合、前記ヒト対象が、脳の側頭葉、後頭葉、頭頂葉、又は前頭葉にタウ負荷を有するかどうかを決定し、次いで
i)前記ヒト対象に、1回以上の約100mg~約700mgの第1の用量の抗N3pGlu Aβ抗体を投与すること、ここで、各第1の用量が、約4週間ごとに1回投与され、
ii)前記1回以上の第1の用量を投与した約4週間後に、1回以上の700mg超~約1400mgの第2の用量の抗N3pGlu Aβ抗体を前記ヒト対象に投与することを含み、ここで、各第2の用量が、約4週間ごとに1回投与され、
有効量の抗体1と同時に、別々に、又は順次組み合わせる、方法。 - 前記ヒト対象が、脳の後外側側頭葉又は側頭葉にタウ負荷を有する、請求項82に記載の方法。
- 前記ヒト対象が、脳の後頭葉にタウ負荷を有する、請求項82に記載の方法。
- 前記ヒト対象が、脳の頭頂葉にタウ負荷を有する、請求項82に記載の方法。
- ヒト対象が脳の前頭葉にタウ負荷を有する、請求項82に記載の方法。
- 前記ヒト対象が、脳の後外側側頭葉(PLT)及び/又は後頭葉にタウ負荷を有する、請求項82に記載の方法。
- 前記ヒト対象が、脳のPLT若しくは後頭領域におけるタウ負荷とともに、i)頭頂若しくは楔前領域、又はii)前頭領域にタウ負荷を有する、請求項82~87のいずれか一項に記載の方法。
- 前記ヒト対象が、脳のi)前頭葉に隔離されたタウ負荷、又はii)後外側側頭領域(PLT)を含まない側頭葉の領域にタウ負荷を有する、請求項82~86のいずれか一項に記載の方法。
- 前記ヒト対象が、脳の後外側側頭葉、後頭葉、及び頭頂葉にタウ負荷を有する、請求項82~88のいずれか一項に記載の方法。
- 前記ヒト対象が、脳の後外側側頭葉、後頭葉、頭頂葉、及び前頭葉にタウ負荷を有する、請求項82~88のいずれか一項に記載の方法。
- 前記ヒト対象が、脳の後外側側頭葉、後頭葉、頭頂葉及び/又は前頭葉にタウ負荷を有する、請求項82~88のいずれか一項に記載の方法。
- 前記ヒト対象が、前記第2の用量を投与される前に、前記第1の用量を1回、2回、又は3回投与される、請求項82~92のいずれか一項に記載の方法。
- 前記ヒト対象が、約700mgの第1の用量を投与される、請求項82~93のいずれか一項に記載の方法。
- 前記ヒト対象が、1回以上の約800mg、約900mg、約1000mg、約1100mg、約1200mg、約1300mg、又は約1400mgの第2の用量を投与される、請求項82~94のいずれか一項に記載の方法。
- 前記ヒト対象が、1回以上の約1400mgの第2の用量を投与される、請求項82~95のいずれか一項に記載の方法。

- 疾患の治療又は予防が、i)前記ヒト対象の脳内のAβ沈着物の低減、及び/又はii)前記ヒト対象の認知若しくは機能低下の遅延を引き起こす、請求項82~103のいずれか一項に記載の方法。
- 前記ヒト対象における脳内のAβ沈着物の低減が、アミロイドPET脳イメージング又はAβのバイオマーカーを検出する診断法によって決定される、請求項97に記載の方法。
- 前記第2の用量は、前記ヒト対象の脳内のAβ沈着物が約20~100%低減するまで、前記ヒト対象に投与される、請求項97又は98に記載の方法。
- 前記ヒト対象の脳内のAβ沈着物が、約20%、約25%、約30%、約35%、約40%、約45%、約50%、約75%、又は約100%低減される、請求項106に記載の方法。
- 前記第2の用量は、前記ヒト対象の脳内のAβ沈着物が、i)およその平均で約25センチロイド~約100センチロイド、ii)およその平均で約50センチロイド~約100センチロイド、iii)約100センチロイド、又はiv)約84センチロイド低減されるまで、前記ヒト対象に投与される、請求項82~107のいずれか一項に記載の方法。
- 前記ヒト対象の脳内のAβ沈着物を特徴とする疾患が、前臨床アルツハイマー病(AD)、臨床AD、前駆期AD、軽度AD、中等度AD、重度AD、ダウン症候群、臨床的脳アミロイド血管症、又は前臨床的脳アミロイド血管症から選択される、請求項82~108のいずれか一項に記載の方法。
- 前記ヒト対象が、早期症候性AD患者である、請求項82~109のいずれか一項に記載の方法。

- 前記ヒト対象が、高いタウ負荷を有していないか、又は高いタウ負荷を有していないと決定されている、請求項82~113のいずれか一項に記載の方法。
- 前記ヒト対象は、PET脳イメージングによって測定されるタウ負荷が1.46SUVrを超える場合、高いタウ負荷を有する、請求項114に記載の方法。
- 前記ヒト対象のタウ負荷が、PET脳イメージング又はタウのバイオマーカーを検出する診断法を使用して決定される、請求項114又は115に記載の方法。
- 前記抗N3pGlu Aβ抗体が、ドナネマブを含む、請求項82~116のいずれか一項に記載の方法。
- 前記患者が、APOE e4の1つ又は2つの対立遺伝子を有する、請求項82~117のいずれか一項に記載の方法。
- タウ負荷の更なる増加を減少させる/予防する、又はヒト脳の側頭葉、後頭葉、頭頂葉、又は前頭葉におけるタウ蓄積速度を遅らせる方法であって、ヒト対象に、抗N3pGlu Aβ抗体を、有効量の抗体1と同時に、別々に、又は順次組み合わせて投与することを含む、方法。
- 必要とする対象においてARIAを治療する方法であって、前記対象に、治療有効量の請求項1~3若しくは12のいずれか一項に記載の抗体又は請求項13に記載の医薬組成物を投与することを含む、方法。
- 必要とする対象においてARIAを予防する方法であって、前記対象に、治療有効量の請求項1~3若しくは12のいずれか一項に記載の抗体又は請求項13に記載の医薬組成物を投与することを含む、方法。
