Abstract
Description
本開示は、腫瘍の増殖を処置もしくは阻害するか、または子宮頸がん患者の全生存を改善するための方法であって、子宮頸がんを伴う患者を選択する工程と、それを必要とする患者へと、PD-1、PD-L1、および/もしくはPD-L2、または本明細書で記載される、他の任意の「PD-1阻害剤」に特異的に結合する抗体またはその抗原結合性断片を投与する工程とを含む方法を含む。本開示で、特定の抗PD-1抗体への言及がなされるのは、代表的なPD-1阻害剤を例示するためであり、本開示の範囲を限定するためではない。
本明細書で開示される方法は、治療有効量のPD-1阻害剤を投与する工程を含む。本明細書で使用される、「PD-1阻害剤」とは、PD-1の活性または発現を阻害するか、遮断するか、妨げるか、またはこれに干渉することが可能な、任意の分子を指す。一部の実施形態では、PD-1阻害剤は、抗体、低分子化合物、核酸、ポリペプチド、またはこれらの機能的断片もしくは変異体でありうる。適切なPD-1阻害剤抗体の非限定例は、抗PD-1抗体およびその抗原結合性断片、抗PD-L1抗体およびその抗原結合性断片、ならびに抗PD-L2抗体およびその抗原結合性断片を含む。適切なPD-1阻害剤の、他の非限定例は、抗PD-1 RNAi分子、抗PD-L1 RNAi、および抗PD-L2 RNAiのようなRNAi分子、抗PD-1アンチセンスRNA、抗PD-L1アンチセンスRNA、および抗PD-L2アンチセンスRNAのようなアンチセンス分子、ならびにPD-1ドミナントネガティブタンパク質、PD-L1ドミナントネガティブタンパク質、およびPD-L2ドミナントネガティブタンパク質のようなドミナントネガティブタンパク質を含む。前出のPD-1阻害剤の一部の例については、PD-1阻害剤を同定するそれらの一部を、参照によって本明細書に組み入れる、例えば、US9308236、US10011656、およびUS20170290808において記載されている。
一部の実施形態では、本明細書で開示される方法において使用されるPD-1阻害剤は、PD-1に特異的に結合する、抗体またはその抗原結合性断片である。「~に特異的に結合する」などという用語は、抗体またはその抗原結合性断片が、抗原と共に、生理学的条件下で、比較的安定な複合体を形成することを意味する。当技術分野では、抗体が、抗原に特異的に結合するのかどうかを決定するための方法が周知であり、例えば、平衡透析、表面プラズモン共鳴などを含む。例えば、本開示の文脈で使用される、PD-1「に特異的に結合する」抗体は、PD-1またはその一部に、表面プラズモン共鳴アッセイで測定される通り、約500nM未満、約300nM未満、約200nM未満、約100nM未満、約90nM未満、約80nM未満、約70nM未満、約60nM未満、約50nM未満、約40nM未満、約30nM未満、約20nM未満、約10nM未満、約5nM未満、約4nM未満、約3nM未満、約2nM未満、約1nM未満、または約0.5nM未満のKDで結合する抗体を含む。しかし、ヒトPD-1に特異的に結合する単離抗体は、他の(非ヒト)種に由来するPD-1分子のような、他の抗原との交差反応性を有する場合がある。

本明細書で開示される方法に従い、対象へと投与されるPD-1阻害剤(例えば、抗PD-1抗体またはその抗原結合性断片)の量は、一般に、治療有効量である。本明細書で使用される、「治療有効量」という用語は、(a)子宮頸がんの症状または徴候(例えば、腫瘍病変)の重症度の軽減または持続期間の短縮;(b)腫瘍の増殖の阻害、または腫瘍壊死、腫瘍縮小、および/もしくは腫瘍消失の増大;(c)腫瘍の増殖および発症の遅延;(d)腫瘍転移の阻害;(e)腫瘍増殖の再発の防止;(f)がんを伴う対象の生存の延長;ならびに/または(g)従来の抗がん治療の使用またはこれに対する必要(例えば、手術に対する必要の消失、または化学療法剤もしくは細胞傷害剤の使用の低減もしくは消失)の、非処置対象、または白金ベースの化学療法、もしくは本明細書で開示されるSOCのような、他のSOCで処置された対象と比較した低減のうちの1つまたはそれ以上を結果としてもたらす、PD-1阻害剤の量を意味する。
本研究は、進行充実性腫瘍を伴う患者における、抗PD-1抗体(セミプリマブ)についての、フェーズI、オープンラベル、多施設研究であった。セミプリマブは、PD-1の、PD-L1およびPD-L2との相互作用を強力に遮断する、PD-1受容体に対する、高アフィニティーの、ヒンジ安定化IgG4ヒトモノクローナル抗体である。セミプリマブは、本明細書で記載される、配列番号9のアミノ酸配列を有する重鎖、および配列番号10のアミノ酸配列を有する軽鎖;配列番号1/2を含む、HCVR/LCVRアミノ酸配列対;ならびに配列番号3~8を含む、重鎖CDR配列および軽鎖CDR配列を含む。また、US9987500も参照されたい。研究は、年齢を、≧18歳とし、組織学的に、または細胞学的に確認された、再発性子宮頸がんまたは転移性子宮頸がんを伴い、白金およびタキサンの二重化学療法に対して耐性であるか、または忍容性でない患者を組み入れた。
患者:10例の患者を、各拡大コホートに登録した。年齢中央値は、単剤療法コホートおよび組合せ療法コホートにおいて、それぞれ、55.0歳(範囲:31.0~76.0)および51.5歳(範囲:29.0~65.0)であった。ベースラインの特徴は、2つのコホートにわたり同様であり、表2にまとめられる。10例の患者は、扁平上皮癌の組織像を有し、8例の患者は、腺癌を有し、1例の患者は、腺扁平上皮癌の組織像を有し、1例の患者は、粘液性癌の組織像を有した。データカットオフ時に、セミプリマブ単剤療法を施される患者の中で、1例の患者は、計画された処置(48週間)を完了し、9例の患者は、主に、疾患の進行または再発に起因して、処置を中断した(n=8)。セミプリマブ+hfRTを施される患者では、10例全ての患者は、主に、疾患の進行または再発に起因して、処置を中断した(n=8)。
これらの拡大コホートの臨床活性結果は、セミプリマブによる処置が、再発性子宮頸がんまたは転移性子宮頸がん患者の間で、応答および臨床利益を誘導することを裏付ける。これは、3例中2例の子宮頸がん患者が、恒常的な応答を有した、セミプリマブFIH(first-in-human)研究の用量漸増コホートにおいて観察された、臨床活性シグナルを裏書きする(Papadopoulosら、Clin Cancer Res(2019年12月)、Epub)。全合計は、フェーズI研究(23例中4例:組合せにおける応答患者;3例中2例:用量漸増における応答患者+20例中2例:拡大コホートにおける応答患者)に登録された子宮頸がん患者中、17%の目的奏効率に等しかった。全てのレスポンダーは、扁平上皮癌の組織像を有した。本研究で観察された安全性結果は、セミプリマブについての他の研究、およびPD-1/PD-L1軸の他の阻害剤において観察された安全性結果と符合する。セミプリマブ処置の開始後における、hfRTの付加による、目的奏効率(ORR)の明らかな改善は、見られなかった。実際、用量漸増におけるセミプリマブ+hfRTに対する目的の応答を示した、子宮頸部扁平上皮癌患者のうちの1例は、その後、計画された処置の完了の後に、疾患の再発を示し、セミプリマブ単剤療法による再処置に対して、完全奏効を経た。
白金およびタキサンの二重化学療法に対して耐性であるか、または忍容性でない、再発性子宮頸がんまたは転移性子宮頸がんを伴う患者において、セミプリマブは、他のPD-1阻害剤について観察される臨床利益および安全性プロファイルと同様の、臨床利益および安全性プロファイルを裏付けた。他の抗PD-1剤によるデータと併せた、セミプリマブ試験による結果は、効能が、子宮頸がんにおける組織像と関連することを示唆する。さらに、組織像と、PD-1阻害の効能との潜在的関連は、PD-L1のタンパク質およびmRNAの発現が、扁平上皮癌の組織像において、扁平上皮癌以外の組織像より高度である他の供給源に由来する子宮頸がん検体についての解析により、間接的に裏書きされる。セミプリマブを、治験責任医師選定化学療法と対比する、フェーズIII無作為化試験は、進行中であり、全生存についての階層的一次解析は、まず、扁平上皮癌の組織像を伴う患者においてなされる。
本研究は、白金含有化学療法の後で、再発性子宮頸がんまたは転移性子宮頸がんが進行した患者における、治験責任医師選定(IC)化学療法と対比した、セミプリマブについての、オープンラベル、無作為化、フェーズ3試験である。
研究の主要目的は、セミプリマブまたは治験責任医師選定(IC)化学療法で処置された、再発性子宮頸がんまたは転移性子宮頸がんを伴う患者であって、扁平上皮癌(SCC)の組織像を有する患者、および任意の適格な組織像を有する患者について、OSを比較することである。SCC患者、および全ての適格な組織像(SCC、腺癌、または腺扁平上皮癌)の間で実施された研究の副次的目的は、(1)セミプリマブによる無進行生存(PFS)を、IC化学療法と対比して比較すること;(2)RECIST(Response Evaluation Criteria in Solid Tumors)1.1に従う、セミプリマブによる目的の奏効率(ORR)(部分奏効[PR]+CR)を、IC化学療法と対比して比較すること;(3)セミプリマブによる応答持続期間(DOR)を、IC化学療法と対比して比較すること;(4)有害事象(AE)について記載することにより、セミプリマブによる安全性プロファイルを、IC化学療法と対比して比較すること;および(5)EORTC QLQ-C30(European Organization for Research and Treatment of Cancer Quality of Life Questionnaire-Core 30)を使用して、セミプリマブで処置された患者についての生活の質(QOL)を、IC化学療法と対比して比較することを含む。
研究は、再発性子宮頸がんまたは転移性子宮頸がんを処置するのに施された白金含有化学療法の後で、再発性子宮頸がん、遷延性子宮頸がん、および/または転移性子宮頸がんが進行した、≧18歳の女性を登録している。限局性疾患のための放射線療法と併用して、白金ベースの先行治療を施されたのに過ぎない患者は、適格ではない。
本研究の主要評価項目は、無作為化~死亡日の時間として規定されるOSである。死亡していない患者は、わかっている最終接触日に打ち切る。
本研究では、約534例の患者(これらのうち、436例は、扁平上皮癌(SCC)の組織像を有する)を、被験セミプリマブ処置アーム、または化学療法対照処置アームであるICアームへと無作為化する。436例のSCC患者を、1:1(218が、処置アームに従う)に無作為化する。被験群では、セミプリマブを、毎週3回、350mgのフラット用量として投与する。対照群では、IC化学療法選択肢は、4つのクラス:(1)抗葉酸剤であるペメトレキセド、(2)トポイソメラーゼ1阻害剤(トポテカンまたはイリノテカン)、(3)ヌクレオシド類似体(ゲムシタビン)、および(4)ビンカアルカロイド(ビノレルビン)にある。対照アームでは、IC選択肢として列挙される、5つの薬物のうちのいずれかの化学療法処置だけが許容される。本研究では、これらのクラス内の他の薬剤は許容されない。図2は、本研究デザインの概略を提示する。
腫瘍イメージング(コンピュータ断層法[CT]または磁気共鳴イメージング[MRI])を実施して、腫瘍量を測定し、応答基準を使用して、研究処置の効能プロファイルを特徴付けた。任意の理由で、研究から離脱しているが、生存情報の収集に対する同意は取り下げていない患者を含む、全ての患者についての生存データを収集するために、あらゆる努力を払う。
説明同意時~最終研究処置の90日後までに投与される、抗がん治療以外の、任意の処置は、併用処置と考えられる。これは、投与が、研究の前に始まり、研究中に持続する、薬物および他の治療の他、研究薬関連AEを処置するために、追跡期間中に始まった、任意の治療を含む。
セミプリマブの投与は、SCCの組織像を伴う、子宮頸がんを伴う患者における、腫瘍退縮の増強をもたらすことが予測される。このような患者は、部分奏効および完全奏効の増大の他、全生存および全奏効率の著明な増大を呈することが予測される。また、このような利益は、SCCの組織像を伴う子宮頸がん患者において、腺癌の組織像を伴う子宮頸がん患者において観察される利益と比較して、より大きな程度で達成されることも予測される。
本実施例は、実施例2に記載した臨床試験から得られる結果を提示する。本実施例において記載される研究は、ECOG(Eastern Cooperative Oncology Group)パフォーマンスステータスが、0または1である、白金ベースの化学療法に対して耐性である、合計608例の転移性子宮頸がん患者(扁平上皮癌(SCC)を伴う477例;腺癌(AC)を伴う131例)を含む。本研究の主要評価項目は、全生存(OS)であった。副次的評価項目は、無進行生存(PFS)、全奏効率(ORR)/応答持続期間(DOR)、安全性、および生活の質を含んだ。
本実施例は、実施例3に示された結果の全体にわたり更新された結果を提示するが、これらは、実施例2で記載された臨床試験から得られた。本実施例において記載される研究は、第一選択の白金ベースの化学療法の後で進行した、再発性子宮頸がんまたは転移性子宮頸がんを伴う、合計608例の女性を含む。患者を、PD-L1の発現状態にかかわらず登録し、96週間後まで、3週間ごとに、350mgのセミプリマブを、静脈内に施すか、または治験責任医師選定(IC)化学療法を施した。IC化学療法の選択肢は、ペメトレキセド、ビノレルビン、ゲムシタビン、イリノテカン、またはトポテカンであった。患者を、組織像(扁平上皮癌(SCC)/腺癌、または腺扁平上皮癌(AC))により層別化した。患者の人口学的特徴を、表13に示す。
本実施例は、第一選択の白金ベースの処置の後で進行した、再発性子宮頸がんまたは転移性子宮頸がんにおいて、抗PD-1セミプリマブを、治験責任医師選定(IC)単剤化学療法と対比する、オープンラベル、無作為化(1:1)、多施設、フェーズ3臨床試験である、実施例2に記載した臨床試験から得られる結果を提示する。治験責任医師により選択された単剤化学療法は、ゲムシタビン、ペメトレキセド、ビノレルビン、トポテカン、またはイリノテカンを含んだ。成人女性(年齢≧18歳)を、PD-L1の発現にかかわらず登録し、最長で96週間にわたり、3週間ごと、350mgのセミプリマブを、静脈内に施すか、またはIC化学療法を施し、組織像(扁平上皮癌[SCC]/腺癌、または腺扁平上皮癌[AC])、地理的地域(北米/アジア/その他の地域)、先行ベバシズマブ、およびECOGパフォーマンスステータス(0/1)により層別化した。主要評価項目は、OSであった。さらなる評価項目は、無進行生存(PFS)、目的の奏効率(ORR)、応答持続期間、生活の質、および安全性を含んだ。

- 腫瘍の増殖を処置もしくは阻害するか、または子宮頸がん患者の全生存を改善する方法であって、
(a)子宮頸がんを伴う患者を選択する工程と;
(b)患者へと、治療有効量のプログラム死1(PD-1)阻害剤を投与する工程と
を含む前記方法。 - 子宮頸がんは、扁平上皮癌、腺癌、および腺扁平上皮癌からなる群から選択される、請求項1に記載の方法。
- 子宮頸がんは、進行性、再発性、遷延性、および/または転移性である、請求項1または2に記載の方法。
