Claims (21)

複数の呼吸サイクルを含む治療送達期間全体にわたって、患者の舌の突き出し状態を持続させるように、前記第1の複数の電極を介して、前記突き出し筋組織を活性化するための電気刺激パルスを送達することと、を含む、方法。 - 前記治療送達期間中に、前記第1の複数の電極から複数の異なる双極電極対を順次選択することをさらに含む、請求項1に記載の方法。
- 前記突き出し筋組織の左部分を刺激するための、前記第1の複数の電極のうちの少なくとも第1の双極電極対、および前記突き出し筋組織の右部分を刺激するための、前記第1の複数の電極のうちの少なくとも第2の双極対を位置決めするように、前記突き出し筋組織の正中面を横切って前記第1の遠位部分を前進させることと、
前記治療送達期間全体にわたって、前記舌の前記突き出し状態を持続させるために、左突き出し筋および右突き出し筋を順次活性化するための前記第1および第2の双極電極を順次選択することと、をさらに含む、請求項2に記載の方法。 - 前記正中面を横切って前記第1の遠位部分を前進させることが、前記正中面を横切って前記第1の遠位部分を斜め方向に前進させることを含む、請求項3に記載の方法。
- 前記突き出し筋組織の後方部分を刺激するための、前記第1の複数の電極のうちの少なくとも第1の双極電極対、および前記突き出し筋組織の前方部分を刺激するための、前記第1の複数の電極のうちの少なくとも第2の双極電極対を位置決めするように、前記突き出し筋の正中面に沿って前記第1の遠位部分を前進させることと、
前記舌の突き出し筋の前方部分の筋組織を活性化するための前記第1および第2の双極電極対、および前記舌の突き出し筋の後方部分の筋組織を活性化するための前記第1の対とは異なる前記複数の電極の第2の対を順次選択することと、をさらに含む、請求項1に記載の方法。 - 前記電気刺激パルスを送達することが、前記治療送達期間の開始時間から前記治療送達期間の終了時間まで、
前記第1の複数の電極から第1の双極対を選択することと、
前記治療送達期間の第1のデューティーサイクルの間、前記第1の双極電極対を介して前記電気刺激パルスの第1の部分を送達することと、
前記第1の複数の電極から第2の双極対を選択することと、
前記治療送達期間の第2のデューティーサイクルの間、前記第2の双極電極対を介して、前記電気刺激パルスの第2の部分を送達することと、によって、前記電気刺激パルスを継続的に送達することを含む、請求項1に記載の方法。 - 前記複数の呼吸サイクルの吸気および呼気段階とは無関係に、前記第1のデューティーサイクルの間、前記電気刺激パルスの前記第1の部分を送達することと、前記電気刺激パルス前記第2のデューティーサイクルの前記第2の部分を送達することと、をさらに含む、請求項6に記載の方法。
- 前記治療送達期間中に突き出し筋緊張と相関する信号を感知することと、
前記センサ信号に基づいて、前記突き出し筋緊張の状態を判定することと、
前記突き出し筋緊張の前記判定された状態に基づいて、前記治療送達期間中に前記電気刺激パルスを調整することと、をさらに含む、請求項1に記載の方法。 - 前記中空送達ツールを顎下挿入部位において、さらに前記突き出し筋組織の中に経皮的に前進させることをさらに含む、請求項1に記載の方法。
- 前記中空送達ツールを前進させることが、顎下挿入部位から、前方経路に向かう後方経路に沿って、前記突き出し筋組織の中に経皮的に前記中空送達ツールを前進させることと、
前方経路に向かう後方経路に沿って、前記突き出し筋組織の中に前記第1の遠位部分を前進させることと、を含む、請求項1に記載の方法。 - 前記突き出し筋組織の第1の側に沿って、前記第1の複数の電極を位置決めするように、前記第1の遠位部分を、前記突き出し筋組織の正中面に横方向に隣接する前記突き出し筋組織の中に前進させることと、
前記突き出し筋組織の前記第2の側に沿って、前記第2の遠位部分によって支持される第2の複数の電極を位置決めするように、第2の医療用電気リードの第2の遠位部分を、前記口腔の下方にあり、かつ前記突き出し筋組織の前記第1の側の反対側の第2の側での前記突き出し筋組織の前記正中面の横方向に隣接する前記突き出し筋組織の中に経皮的に前進させることと、
前記治療送達期間全体にわたって前記患者の舌の突き出し状態を持続させるように、前記第1の複数の電極および前記第2の複数の電極を介して、前記突き出し筋組織を活性化するための前記電気刺激パルスを送達することと、をさらに含む、請求項1に記載の方法。 - 前記治療送達期間全体にわたって前記患者の舌の前記突き出し状態を持続させるように、前記第1の複数の電極および前記第2の複数の電極から、少なくとも第1のデューティーサイクルのための第1の双極電極対、および第2のデューティーサイクルのための第2の双極電極対を順次選択することによって、前記電気刺激パルスを送達すること、をさらに含む、請求項11に記載の方法。
- 前記第1の複数の電極のみから前記第1の双極対を選択することと、前記第2の複数の電極のみから前記第2の双極対を選択することと、をさらに含む、請求項12に記載の方法。
- 前記第2の複数の電極のうちの少なくとも1つと共に対にされた前記第1の複数の電極のうちの少なくとも1つを選択することによって、前記第1の双極対を選択すること、をさらに含む、請求項12に記載の方法。
- 前記患者の頸部に沿って、前記第1の医療用電気リードの近位端を皮下ポケットにトンネリングすることと、
前記近位端を、前記頸部に沿って前記皮下ポケット内に移植されたパルス発生器に結合することと、
前記パルス発生器によって前記電気刺激パルスを発生させることと、をさらに含む、請求項13に記載の方法。 - 医療機器システムであって、
第1の医療用電気リードであって、
第1の近位端から第1の遠位端に、前記第1の遠位端に近接する第1の遠位部分と共に延在する第1の細長いリード本体であって、前記第1の遠位部分が、前記口腔中に貫入することなく、口腔の下方の突き出し筋組織内に中空送達ツールを介して経皮的に前進するように構成される、第1の細長いリード本体と、
前記口腔の下方の前記突き出し筋組織内の移植のための、前記第1の遠位部分に沿って離間された第1の複数の電極と、を含む、第1の医療用電気リードと、
治療送達期間全体にわたって患者の舌の突き出し状態を持続させるように、前記第1の近位端を受容し、かつ前記第1の複数の電極を介して前記突き出し筋組織を活性化するための電気刺激パルスを送達するように構成されたパルス発生器と、を備える、医療機器システム。 - 前記パルス発生器が、前記治療送達期間全体にわたって前記舌の前記突き出し状態を持続させるように、少なくとも前記突き出し筋組織の第1の部分および前記突き出し筋組織の第2の部分をそれぞれ順次活性化するように、前記治療送達期間中に前記第1の複数の電極から、少なくとも第1の双極対および第2の双極対”を順次選択するように構成されている、請求項15に記載のシステム。
- 前記パルス発生器が、前記治療送達期間の開始時間から前記治療送達期間の終了時間まで、前記複数の呼吸サイクルの吸気および呼気段階とは無関係に、

前記センサ信号に基づいて、前記突き出し筋緊張の状態を判定することと、
前記突き出し筋緊張の前記判定された状態に基づいて、前記治療送達期間中に前記電気刺激パルスを調整することと、を行うように構成されている、請求項15に記載のシステム。 - 第2の医療用電気リードをさらに備え、前記第2の医療用電気リードが、
第2の近位端から第2の遠位端まで、前記第2の遠位端に近接する第2の遠位部分と共に延在する第2の細長いリード本体と、
前記第2の遠位部分に沿って離間された第2の複数の電極と、を含み、
前記第2の遠位部分が、前記第1の複数の電極よりも前記正中面の反対側の前記突き出し筋組織に沿って、前記第2の遠位部分によって支持される前記第2の複数の電極を位置決めするように、前記口腔中に貫入せず、かつ前記第1の遠位部分の反対側の前記突き出し筋組織の正中面の横方向に隣接する、前記口腔の下方の前記突き出し筋組織内の中空送達ツールを介して、経皮的に前進するように構成されており、
前記パルス発生器が、前記治療送達期間全体にわたって前記患者の舌の前記突き出し状態を持続させるように、前記第2の近位端を受容し、かつ前記第1の複数の電極および前記第2の複数の電極から、少なくとも第1のデューティーサイクルのための第1の双極電極対、および第2のデューティーサイクルのための第2の双極電極対を順次選択することによって、前記電気刺激パルスを送達するようにさらに構成されている、請求項15に記載のシステム。 - 前記第1の細長いリード本体は、前記パルス発生器が前記患者の頸部に沿って移植されるときに、前記遠位部分から前記パルス発生器までの最大距離を拡張する長さを有する、請求項15に記載のシステム。
