Abstract
Description
第一選択の抗リーシュマニア薬の大部分は、複数の薬物耐性の増加によりすでにこれらの有用性を失っている(4)。抗リーシュマニア薬の現在の創薬パイプラインも、かなり減少している。新しい抗リーシュマニア薬の創薬パイプラインを豊かにするためには持続した取り組みが必要とされる。
抗マラリア処置に対するより高い有効性、ならびに生理学的環境下でのバイオアベイラビリティーおよび安定性に関係した問題を克服するためのより良い化学的プロファイルを有するアーテミシニンのいくつかの誘導体が開発されてきた(11、12)。アーテミシニン誘導体はまた、がん、炎症性疾患、ウイルス性疾患、リーシュマニア症および他の感染性疾患を含む、いくつかの非抗マラリア状態の処置におけるこれらの潜在的用途についても調査されてきた(13、14)。
アーテミシニンおよびその誘導体は、皮膚リーシュマニア症に対する原因物質である大形リーシュマニアに対する抗リーシュマニア活性が初めて報告されている(15)。続いて、いくつかの他のグループが、大形リーシュマニアにおけるアーテミシニンの抗リーシュマニア活性を報告した(16、17)。ドノバン・リーシュマニア(L.donovani)に対するアーテミシニンの活性に関係した研究は非常に限定されている(18~20)。アーテミシニンは、非常に高濃度で、ドノバン・リーシュマニアプロマスチゴートに対するその抗リーシュマニア活性が報告されており(21)、その活性は、寄生生物の生物学的関連形態であるアマスチゴートに対しては明白に存在しない。アーテミシニンはまた、ドノバン・リーシュマニアに感染したBALB/cマウスにおいて抗リーシュマニア活性が報告されている(17)。
アーテミシニンおよびその誘導体のモノマーの大部分は、経口バイオアベイラビリティーの欠如に悩まされている(22、23)。本発明のアーテミシニンダイマーは、これら製剤の特徴を改善するように設計されている。
並行して、本発明のダイマーを、異なる形態のドノバン・リーシュマニア寄生生物に対してスクリーニングした。本発明のアーテミシニンダイマーは強力な抗リーシュマニア活性を有することが判明した。
さらに、これらの抗リーシュマニア作用の分子モードについて本発明のアーテミシニンダイマーを評価した。アーテミシニンおよびその誘導体は、ドノバン・リーシュマニアプロマスチゴート寄生生物に対するそのアポトーシス効果について報告されている(20、24)。この関連で、アネキシンV結合アッセイの助けを借りて、プロマスチゴート形態の寄生生物に対するアポトーシス応答を評価するため、現在最も強力なアーテミシニンダイマー、ダイマーモルホリン、およびダイマーガンマ-アミノ酪酸(GABA)が選択された。
本発明のダイマーの投与は、従来の投与経路、例えば、経口、皮下、腹腔内、筋肉内、静脈内または直腸内のうちのいずれかによるものであってよい。好ましい実施形態では、化合物は、選択および投与経路に応じて、固体または液体であってよい薬学的に許容される担体と組み合わせて投与される。許容される担体の例として、これらに限定されないが、デンプン、ブドウ糖、スクロース、ラクトース、ゼラチン、寒天、ステアリン酸、ステアリン酸マグネシウム、アカシア、および類似の担体が挙げられる。液体の例として、生理食塩水、水、緩衝液、および食用油、例えば、ピーナッツ油およびトウモロコシ油が挙げられる。
これらの検討材料を考慮に入れて、特定の患者に対する用量単位は、医学的技術において公知の技術に従い医師により容易に決定することができる。
内臓リーシュマニア症による死亡の原因となる要素はいくつかある。第1に、影響を受ける集団の大部分は貧困層であり、生活水準が低い。現在、影響を受ける集団は食料調達の余裕がなく、大部分の処置は非常に高価である(25、26)。また、利用可能な処置の大部分は重度の毒性を有する(27)。
高い有効性、より良い安全性プロファイルおよび低コストを有し、耐性が発生しにくいより良い薬物を開発することが非常に必要となっている。アーテミシニンはクソニンジン(Artemisia annua)から単離した天然産物であり、より良い、より安価の代替物をここにもたらすが、強力な抗リーシュマニア活性を有するアーテミシニンは報告されていない(5)。
他のいくつかのアーテミシニン誘導体が調査された。これらの大部分は、対照薬物(シタマキン、ミルテホシン、アムホテリシンBまたはペンタミジン)より高いまたは同等の活性を有する(14、19、32)。
本発明では、アーテミシニンダイマーは、対照薬物アムホテリシンBおよびペンタミジンより高い活性およびより高い選択性を有すると報告されている。ダイマーモルホリンおよびダイマーGABAはそれぞれSI>2056および>1086を有する。ここで初めて、一部の抗リーシュマニア薬物リード(ダイマーモルホリンおよびダイマーGABA)がこのような高い選択性プロファイルを有すると報告されている。
細胞毒性および抗リーシュマニア活性
分化THP1細胞に対して細胞毒性を評価した。ダイマートリプタミン(IC50 7.388μM)およびダイマーオキシム(IC50 11.383μM)を除いて、化合物のいずれも分化THP1細胞に対して毒性を有さなかった。すべてのアーテミシニンダイマー、親アーテミシニン、および対照薬物アムホテリシンBならびにペンタミジンの抗リーシュマニア活性をプロマスチゴートおよび細胞内アマスチゴート形態のドノバン・リーシュマニア寄生生物に対して評価した(表1)。アラマーブルーアッセイにより、すべてのアーテミシニンダイマーは、プロマスチゴート形態の寄生生物に対して強力な活性を示す。寄生生物レスキューおよび変換アッセイ(transformation assay)において、すべてのアーテミシニンダイマーは、細胞内アマスチゴート形態の寄生生物に対して強力な活性を示す(表1)。親アーテミシニンは、非常に高濃度(IC50 99.175μM)でプロマスチゴート形態の寄生生物に対して活性を示し、細胞内アマスチゴートでは、濃度35.419μMまでいかなる活性も有さない(THP1細胞に対するDMSOの許容限度が0.5%であるという事実により、寄生生物レスキューおよび変換アッセイにおける濃度は限定される)。寄生生物レスキューおよび変換アッセイにおける細胞毒性のIC50値を抗リーシュマニア活性のIC50値で割ることにより、各アーテミシニンダイマーに対する選択指数(SI)を計算した。ダイマーモルホリン(0.007μM、SI>2052)およびダイマーGABA(0.013μM、SI>1086)は、最も高い選択性インデックスを有する最も活性のあるアーテミシニンダイマーであった。デジタル画像分析アッセイでは、寄生生物レスキューおよび変換アッセイにおいて以前に強力な活性を示した、選択されたアーテミシニンダイマーの抗リーシュマニア活性を再確認した(表2)。アマスチゴート核の数を、THP1細胞核の数で割ることにより、分化THP1細胞の感染性を計算した。選択されたアーテミシニンダイマーは、デジタル画像分析アッセイにおいても細胞内アマスチゴートに対して強力な活性を示している(図3/表2)。
テューキー比較検定を用いた二元配置ANOVA(図2):

THP-1細胞(ヒト単球性白血病細胞)は、American Type Culture Collection(ATCC)より入手可能となり、ピルビン酸ナトリウム、グルタミン、HEPES(Life-81 Technologies)および10%加熱不活性化したFBS(Sigma)を補充したRPMI 1640培地(Life-Technologies)内で維持した。培養物を5%CO2インキュベーター内で、37℃で維持した。THP1細胞の継代培養を3~4日ごとに行った。pH7.4で10%ウシ胎児血清を補充したRPMI 1640培地内で、プロマスチゴート形態のドノバン・リーシュマニア(S1株)を増殖させた。培養物をインキュベーターにより26℃で維持した。継代培養を3~4日ごとに行った。
プロマスチゴートアッセイは、アラマーブルーベースの成長分析に基づいた(33)。3~4日経過した、指数増殖期のプロマスチゴート培養物をRPMI 1640培地で1×106細胞/mlに希釈した。アーテミシニンダイマー試料をストック濃度2mg/mlに希釈した。アーテミシニンダイマー試料を培養プレートに分注し、試料の最も高い最終濃度が10μg/mlとなり、親アーテミシニンの最も高い濃度が40μg/mlとなるよう、プロマスチゴート培養物で希釈した。最も高い濃度の各アーテミシニンダイマーを有する培養物を6つの異なる濃度へと1:5比で連続希釈した。すべてのアーテミシニンダイマーを3連で試験した。プロマスチゴート寄生生物およびアーテミシニンダイマーを有する培養プレートを26℃で48時間インキュベートした。48時間後、アラマーブルーを各ウェルに加え、プレートをさらに24時間インキュベートした。標準蛍光を544nm ex、590nm emで、Fluostar Galaxy蛍光光度計(BMG LabTechnologies)により測定した。XLfit5.2.2ソフトウエアを使用して、用量反応曲線を調製した(34)。

このアッセイはJainらにより以前に公開されたプロトコールに基づく(35)。3日経過した、指数増殖期にあるTHP1細胞の培養物をRPMI培地で2.5×105細胞/mlに希釈した。PMAを最終濃度25ng/mlまで加えた。PMAで処理した培養物を透明な平底培養物プレートに分注し、5%CO2インキュベーター内で、37℃で終夜インキュベートした。分化THP1細胞を有するプレートを、Molecular device AquaMax 4000の補助を用いて、血清を含まない培地で洗浄した。5×106細胞/mlに希釈した、5~6日経過したドノバン・リーシュマニアプロマスチゴート培養物を分化THP1細胞上に加えた。感染に対して、マクロファージ細胞の寄生生物に対する比は1:10であった。プレートを24時間インキュベートした。24時間後、AquaMax 4000の補助を用いて、プレートを再度洗浄し、血清を含まない培地を、希釈した試験試料を有する培地に置き換えた。プレートを再度CO2インキュベーター内に、37℃で48時間配置した。48時間後、マクロファージアマスチゴートプレートを洗浄し、0.05%SDSで30秒間処理し、希釈したSDS培地を、完全なRPMI培地に直ちに置き換えた。アマスチゴートからプロマスチゴートへの変換のためにプレートをインキュベートし、プロマスチゴートを26℃で48時間成長させた。48時間後、5μlのアラマーブルーをマクロファージアマスチゴートアッセイプレートの各ウェルに加え、プレートをさらに24時間インキュベートした。標準蛍光を、544nm ex、590nm emで、Fluostar Galaxy蛍光光度計(BMG LabTechnologies)により測定した。Xlfit5.3.1ソフトウエアですべての用量反応曲線を生成した。
3日経過した、指数増殖期のTHP1細胞の培養物をRPMI培地で2.5×105細胞/mlに希釈した。PMAを最終濃度25ng/mlまで加えた。PMA処理した培養物を16ウェルチャンバースライドに分注し、5%CO2インキュベーター内で、37℃で終夜インキュベートした。分化THP1細胞を有する16ウェルチャンバースライドを、血清を含まない培地で洗浄した。5×106細胞/mlに希釈した、5~6日経過したドノバン・リーシュマニアプロマスチゴート培養物を分化THP1細胞上に加えた。感染に対して、マクロファージ細胞の寄生生物に対する比は1:10であった。16ウェルチャンバースライドを24時間インキュベートした。24時間後、チャンバースライドを再度洗浄し、残留する血清を含まない培地を、希釈した試験試料を有する完全培地に置き換えた。チャンバースライドを再度CO2インキュベーター内に、37℃で48時間配置した。48時間後、マクロファージアマスチゴートプレートを洗浄し、100%メタノールで30秒間固定した。固定したチャンバースライドを5×CybrGreen Iで15分間染色した。ドノバン・リーシュマニアアマスチゴート寄生生物に感染した分化THP1細胞のすべての画像をNIKON 90i Eclipse蛍光顕微鏡により収集した。THP1細胞の数およびTHP1細胞中のアマスチゴートの数の鑑別計算をImageJソフトウエアで行った(35)。
3日経過した、指数増殖期のTHP1細胞の培養物を、RPMI培地で2.5×105細胞/mlに希釈した。PMAを最終濃度25ng/mlまで加えた。PMAで処理した培養物を透明な平底培養物プレートに分注し、5%CO2インキュベーター内で、37℃で終夜インキュベートした。分化THP1細胞を有するプレートを、Molecular device AquaMax4000による補助を用いて、血清を含まない培地で洗浄し、血清を含まない培地を、希釈した試験試料を有する培地に置き換えた。プレートを再度CO2インキュベーター内に、37℃で48時間配置した。48時間後、2.5ulのalamar-blueを細胞毒性プレートの各ウェルに加え、プレートをさらに24時間インキュベートした。544nm ex、590nm emにおいて、標準蛍光をFluostar Galaxy蛍光光度計(BMG LabTechnologies)により測定した。すべての用量反応曲線はXlfit5.3.1ソフトウエアで生成された(36)。
プログラム細胞死としても公知のアポトーシスは、望ましくない細胞を排除する生理的プロセスである(37)。アポトーシスのプロセスには、早期事象の1つに、細胞膜の内側から表面への膜ホスファチジルセリンの転位が含まれる(38)。アネキシンVは、ホスファチジルセリンに対して高親和性を有するCa++依存性リン脂質結合タンパク質である(39)。よって、FITC標識したアネキシンVを、フローサイトメーターを使用して、曝露されたPSの分析に使用することができる(40)。アネキシンV/ヨウ化プロピジウム染色法を使用して、アポトーシス分析をフローサイトメトリーにより行った。高い選択指数を有する最も活性のあるアーテミシニンダイマーをアポトーシス実験に対して選択した(41)。1×107プロマスチゴート細胞/mlをアーテミシニン(35μM)、ダイマーモルホリン(14μM)およびダイマーGABA(14μM)で処理した。アリコートを異なる時間間隔で回収し、FITC-アネキシンVおよびヨウ化プロピジウムで染色した。フローサイトメトリーで細胞を分析した。図2に示されている通り、FFC/SSCドットプロットにおいて、ゲーティングした集団を健康なプロマスチゴート細胞に対して選択した。50,000回のゲート事象(gated event)をFL2(ヨウ化プロピジウム)/FL1(アネキシンFITC)で実行し、四角形描画法(quadrangle plot)により分析した。
