Abstract
Description
本出願は、2019年2月15日に出願された米国特許仮出願第62/806,432号および2019年12月17日に出願された米国特許仮出願第62/949,181号の恩典およびそれに対する優先権を主張し、どちらの出願も、その全体が参照により本明細書に組み入れられる。
ヒトアストロウイルスは、エンベロープを持たない正二十面体RNAウイルスの科に属し、ヒト幼児において急性胃腸炎を引き起こす世界中に広まった風土病病原体である[1]。重度の急性疾患を引き起こすカリシウイルスおよびロタウイルスとは違って、アストロウイルス胃腸炎は非炎症性である[2]。アストロウイルスに対する炎症反応が弱いのは、アストロウイルスカプシドを形成する787アミノ酸残基のアストロウイルスコートタンパク質に起因する可能性がある。このコートタンパク質は、補体の古典経路の活性化を阻害することが判明した[3]。補体は、病原体に対する強い炎症反応を特徴とする、ヒトの先天性免疫応答である[4]。アストロウイルスコートタンパク質のアミノ酸配列についての解析により、ヒト好中球ディフェンシン1型(HNP-1)に対して緩い相同性を有する領域が同定され、補体の古典経路を阻害する能力を保持した短いペプチドの開発につながった[5]。
他に規定されない限り、本明細書で使用される技術用語および科学用語はすべて、当業者によって一般に理解されるのと同じ意味を有する。本明細書に記載されているものと同様または等価な方法および材料を、本発明の実践または試験において使用することができるが、適切な方法および材料を後述する。
Eastern Virginia Medical School IRB承認済みのプロトコール02-06-EX 0216のもと、文書による同意を得て健常ドナーの血液を採取した。血液を反応物、すなわち精製した血小板、赤血球、および好中球の調製のために使用した。
PA-dPEG24(IALILEPICCQERAA-dPEG24またはPIC1)は、HPLCおよび質量スペクトル解析によって確認される純度が95%以上になるようにPolyPeptide Group(San Diego, CA)によって製造された。凍結乾燥されたPA-dPEG24を0.05Mヒスチジン緩衝液中で可溶化し、pHを6.7に調整した。サルコシン置換誘導体ペプチドおよびベースとなるペプチドIALILEPICCQERAA(PA)は、純度が90%を上回るようにNew England Peptide(Gardner, MA)によって合成された。サルコシン変異体およびPEGを水に溶解し、NaOHを用いてpHを調整した。PAをDMSOに溶解し、次いで、水を加えて最終濃度が30%DMSOになるようにし、pHを調整した。抗体感作したヒツジ赤血球(EA)、精製したC1q、およびB因子を枯渇させたヒト血清は、Complement Technology(Tyler, TX)から購入した。精製ミエロペルオキシダーゼはLee BioSolutions(Maryland Heights, MO)から購入し、テトラメチルベンジジン(TMB)およびPicoGreenはThermo Fisher(Waltham MA)から購入した。
ベロナール緩衝生理食塩水、0.1%ゼラチン、0.15mM CaCl2、および1mM MgCl2を含む補体許容性GVBS++緩衝液を調製した[12]。ベロナール緩衝生理食塩水ならびに0.1%ゼラチンおよび10mM EDTAを含む補体阻害性緩衝液GVBS--を調製した。
以前に説明されているようにしてプール正常ヒト血清(NHS)を調製した[12]。少なくとも4名の健常ヒトドナーの血液を、添加剤を入れていない)バキュテナーチューブ(赤トップ)中に集めた。血液を凝固させ血清を分離するために、血液を室温で30分間、氷上で2時間放置した。次いで、血清を集め、等分し、-80℃で凍結した。
溶血補体アッセイのために、以前に説明されているようにして、AB型ドナーに由来するヒト赤血球(RBC)を精製し、洗浄し、1×109細胞/mlに標準化した[13]。最終濃度20%のO型ドナー由来のヒト血清を1mM PIC1またはサルコシン変異体ペプチドと混合し、体積が0.15mlになるまでGVBS++を加え、0.5ml RBCを加えた。B因子枯渇血清の溶血アッセイの場合、最終0.005%のB因子枯渇血清を、1mM PIC1またはサルコシン変異体ペプチドおよび0.1mlの抗体感作ヒツジ赤血球(EA)とともに最終体積0.75mlのGVBS++中でインキュベーションした。試料は、37℃で1時間インキュベーションした。次いで、1.0mlのGVBS--をB因子枯渇試料に添加して反応を停止させた。試料を3,000rpmで5分間、遠心した。次いで、上清を採取し、412nmで読み取った。解析を実施して、GVBS++緩衝液中のヒトO型血清およびAB型赤血球からなる陽性対照に対する比率(%)として値を表した。
PIC1およびサルコシン変異体を25mg/mlに希釈し、次いで、96ウェルプレート中、0.02mlの体積で段階的に濃度設定した。ミエロペルオキシダーゼ(MPO)を20μg/mlに希釈し、0.02mlを該濃度設定したペプチドに添加した。TMB(3,3′,5,5′-テトラメチルベンジジン)(0.1ml)を各ウェルに2分間添加し、続いて0.1mlの2.5N H2SO4をさらに2分間添加し、次いで、96ウェルプレートリーダー(BioTek)を用いて450nmで読み取った。
重炭酸緩衝液に溶かした1μg/mlのC1qまたはMPOで、Immunlon-2HB ELISAプレートを4℃で一晩コーティングした。プレートをPBST(phosphate buffered saline + 0.1% Tween)で洗浄し、次いで、室温で2時間、1%ゼラチン/PBSによってブロックした。洗浄後、プレートを、2.5mg/mlから始め、次いで1%ゼラチン/PBS中で段階的に希釈したPAまたはサルコシン変異体ペプチドとともに、室温で1時間インキュベーションした。洗浄後、プレートを、1%ゼラチン/PBS中で1:1000希釈したウサギ抗PA(Cocalico Biologicals, Reamstown, PAと共に開発した)を用いて室温で1時間プローブし、続いて、1%ゼラチン/PBS中で1:1000希釈したヤギ抗ウサギHRP(Sigma Aldrich, St Louis, MO)を用いて室温で1時間プローブした。このとき、各プローブの間に洗浄段階を含めた。TMB基質溶液を用いてウェルを発色させ、1N H2SO4を用いて停止し、プレートリーダーにおいて450nmで読み取った。
96ウェルプレートにおいて、0.025mlの1.7mg/ml MPOを、合計体積0.125mlのPBS中で、0.00125mlの0.5% H2O2および3mMのPIC1またはサルコシン変異体ペプチドと混合し、室温で2分間、放置してインキュベーションした。以前に報告されているように、96ウェルプレートリーダーを用いて各ウェルの300~550nmの吸光度をスキャンして、MPOヘム環の鉄の状態を反映する曲線を作成した[14]。
TAC(Total Antioxidant Capacity)アッセイ(Cell Biolabs, Inc, San Diego, CA)を用いて、銅(II)から銅(I)への還元に基づいてサルコシン変異体の抗酸化能を測定した。このアッセイは、推奨されるキットプロトコールに従って実施した。
以前に説明されているようにして、免疫複合体を用いるNETosisアッセイを実施した[9]。簡単に説明すると、GVBS++に溶かしたオボアルブミン-抗オボアルブミン免疫複合体で、37℃で30分間、正常ヒト血清を刺激した。次いで、この混合物を0.05%H2O2と共に、サルコシン変異体(2mM)を含むまたは含まないRPMIに再懸濁した精製ヒト好中球に添加して、NETosisを生じさせた。PicoGreenを用いて、好中球から放出された遊離DNAの定量を行った。スライドをDAPI(Southern Biotech, Birmingham, AL)で染色した。以下の抗体を用いてNET形成を可視化した:
(i)一次抗体としてのマウス抗エラスターゼ(Invitrogen, Carlsbad, CA)およびヤギ抗マウス二次抗体Alexa Fluor 568(Novus Biologicals, Centennial, CO)ならびに
(ii)ウサギ抗ヒストンH3(Abcam, Cambridge MA)およびヤギ抗ウサギ抗体Alexa Fluor 488(Novus Biologicals)。
Olympus BX53顕微鏡を用いた蛍光顕微鏡検査法を実施してNETを可視化した。
Excel(Microsoft, Redmond, WA)を用いて定量データを解析して、平均値、標準誤差(SEM)、およびスチューデントのt検定[15]を明らかにした。
ベースとなるペプチドIALILEPICCQERAA(PA)の溶解性および生物学的機能に対するサルコシン残基の影響を評価するために、15箇所すべてをサルコシン残基で置換したペプチド誘導体を合成した。また、9位および10位の隣接するシステイン(C9、C10)が単一のサルコシン残基で置換されたペプチドも含まれる。これらのペプチドを表2に示している。各ペプチドの水溶性についてのアッセイを実施し、結果を表3に示した。A2位、L3位、I4位、L5位、I8位、C9位、およびC9,10位のサルコシン置換により、水溶性のペプチドが生じた。PEG化の非存在下で溶解性が向上したことから、様々な生物活性をさらに評価するためにこれらのペプチドを選択した。
抗体によって開始される補体活性化をペプチド変異体が阻害する程度の評価を行った。次の2つの溶血アッセイにおいてペプチド変異体を試験した:(i)ABO不適合性エクスビボアッセイおよび(ii)B因子枯渇血清中での古典経路CH50式アッセイ。
次いで、PA-dPEG24について以前に説明されているようにして、TMBベースのインビトロアッセイにおいてミエロペルオキシダーゼ(MPO)活性の阻害を試験した[7]。このアッセイでは、ある範囲の濃度にわたって変異体をMPO阻害について試験した。このデータを図2Aに示す。システイン無しの変異体(C9,10)を除いて、すべての変異体において強いMPO阻害が見出された。図2Bに示すデータを用いて、各変異体の用量反応曲線から最大半減阻害値を算出した。MPO阻害には測定できるほどの相違があり、この場合もやはり、変異体I8が、様々な変異体のなかでも最大の効力を示した。
次いで、変異体がMPO分子中のヘム環の分解を防止する能力を評価した。PA-dPEG24について以前に示されているように、MPOは塩素イオンおよび過酸化水素の存在下で次亜塩素酸を生成するため、次亜塩素酸がヘム環を分解すると考えられる[8]。ヘム環の分解は、波長300~550nmの吸光度を分光計で測定することにより評価することができる。このスペクトル範囲ですべての変異体について吸光度測定を行い、各変異体のデータを図3A~3Hに示した。どのペプチドもヘム環分解をいくらか阻害し、図3Fおよび図3Gに示すように、I8変異体およびC9変異体の場合にヘム環の吸収スペクトルがほぼ完全に維持されていた。これら2種の変異体の場合のヘム環維持は、PA-dPEG24ペプチドの場合のヘム環維持を上回っていた(図3A)。C9,10変異体もまたヘム環分解の抑制を示したことから(図3H)、MPOペルオキシダーゼ活性の阻害以外の保護メカニズムがあることが示唆された。(図2Aに示されるように)C9,10変異体はMPOペルオキシダーゼ活性を阻害する性質を持たない。
PA-dPEG24について以前に説明されているようにして、総抗酸化能(TAC)アッセイにおいて各変異体の抗酸化能を試験した[10]。抗酸化活性を、銅還元当量(CRE)として測定した。0.03~0.25mMの濃度範囲でペプチドを試験し、次いで標準物質と比較した。これらの結果を図4に示す。変異体は異なる総抗酸化能を示し、L3、I4、およびI8は、PA-dPEG24(PIC1)の抗酸化能に迫る抗酸化能を示した。システイン無しの変異体C9,10は、抗酸化能を示さなかった。
ペプチド変異体をNETosis阻害について試験した。このアッセイでは、精製したヒト好中球を、オボアルブミン-抗オボアルブミン免疫複合体および過酸化水素を用いて活性化した正常ヒト血清によって刺激した。次いで、それらの好中球から出された遊離DNAをPicoGreenアッセイにおいて測定した。これらの結果を図5Aに示す。図5Aのグラフに示すように、このアッセイにおいて、A2、L3、L5、I8、およびC9を含む多数の変異体が、PA-dPEG24と比べて同様のNETosis阻害能力を示した。I4は、PA-dPEG24と比べて低いNETosis阻害能力を示し、C9,10は、陰性対照(緩衝液)と比べてごくわずかな阻害を示した。これらの結果は、サルコシン変異体ペプチドの多くが、刺激されていない好中球に由来する遊離DNAのベースライン値までNETosisを阻害できることを示す。蛍光顕微鏡検査法による好中球細胞外トラップ(NET)形成の直接的可視化を利用して、I8変異体による上記のNETosis結果を確認した。以前に説明されているようにして、ヒト好中球を精製し、スライドガラス上で刺激し、次いでDAPIで染色してDNAを可視化し、抗好中球エラスターゼ(αNE)および抗ヒストンH3(αヒストン)でプローブした[9]。刺激されていない好中球は、NETの証拠を示さなかった(図5B、1行目)。オボアルブミン-抗オボアルブミン免疫複合体および過酸化水素で刺激した好中球は、多くのNETを示し(図5B、2行目)、PA-dPEG24の存在下で免疫複合体によって刺激した好中球は、NETの著しい減少を示した(図5B、3行目)。蛍光顕微鏡検査法によるNET可視化によって、遊離DNAの定量的測定において認めれた知見が裏付けられた。
以下の材料および方法のセクションは、下記の実施例7~11に当てはまる。

Eastern Virginia Medical School IRB承認済みのプロトコール02-06-EX 0216のもと、文書による同意を得て健常ドナーの血液を採取した。血液を反応物、すなわち精製した血小板よび正常ヒト血清の調製のために使用した。
PIC1(IALILEPICCQERAA-dPEG24)は、HPLCおよび質量スペクトル解析によって確認される純度が95%以上になるようにPolyPeptide Group(San Diego, CA)によって製造された。凍結乾燥されたPIC1を0.05Mヒスチジン緩衝液、pH6.7に可溶化した。血小板感作抗体は、免疫性血小板減少性紫斑病(ITP)患者の血清の形態で、Tissue for Research LTD(Suffolk, United Kingdom)から入手した。PrestoBlue(登録商標)細胞生死判定試薬は、Life Technologies (Eugene, Oregon)によって製造された。PKH26赤色蛍光細胞染料は、Sigma-Aldrich(St. Louis, MO)によって製造された。
ベロナール緩衝生理食塩水、0.1%ゼラチン、0.15mM CaCl2、および1mM MgCl2を含む補体許容性GVBS++緩衝液を調製した[12]。ベロナール緩衝生理食塩水ならびに0.1%ゼラチンおよび10mM EDTAを含む補体阻害性緩衝液GVBS--を調製した。
以前に説明されているようにしてプール正常ヒト血清(NHS)を調製した[12]。
ラット血清は、Innovative Research(Peary Court, Novi, MI)から購入した。
ヒトドナー間での再現性を保証するために、多数の健常ヒトドナーから血小板を精製した。健常志願者の末梢血(8mL)を、酸性クエン酸デキストロース(ACD)を含むバキュテナー(BD, Franklin Lakes, NJ)無菌採取チューブ中に直接抜き取り、採取から3時間以内に使用した。さらに250ulのACDを添加した後、22℃にて、500×g、100×g、および800×gの順で各5分間、血液を遠心分離し、各遠心後に上清を採取し、新しいチューブに移した。次いで、精製した血小板をGVBS--、次に生理食塩水で洗浄し、その後、最後にGBVS++に再懸濁した。
インビトロ実験の場合、製造業者の推奨に従って、血小板をPrestoBlue細胞生死判定試薬で染色した。インビボ実験の場合、製造業者の推奨に従って、血小板をPKH26赤色蛍光細胞染料で染色した。
ヒト抗血小板抗体で血小板を感作する実験の場合、精製した血小板を30℃で15分間、4%ITP血清で処理した後、補体が十分な2%正常ヒト血清を添加し、37℃で30分間、インキュベーションした。PIC1を含む試料については、感作した血小板と混合する前に、血清およびPIC1を5分間プレインキュベーションした。次いで、試料を遠心し、ELISAによるC5a解析のために上清を取り出した。C5a ELISAキット(R&D Systems)を製造業者の取扱い説明書に従って使用した。細胞をGVBS--で1回、水で1回洗浄して血小板を溶解した。残存する細胞ペレットを37℃で1時間、25mMメチルアミンで処理し、遠心し、ELISAによって上清をiC3bについて分析した。以前に説明されているように、捕捉用のヤギ抗ヒトC3抗体(Complement Technology, Tyler, TX)、プローブ用のマウス抗ヒトiC3b抗体(Quidel, San Diego, CA)、および検出用のヤギ抗マウスHRP(Sigma-Aldrich, St Louis, MO)を用いて、iC3bについてのELISAを実施した[30]。
体重250gの約16週齢の雄ウィスターラットを購入し(Hilltop Scottdale, PA)、頚静脈内カテーテルを所定の位置に留置した。実験過程の間ずっと、ケタミンおよびアセプロマジンを用いてラットを鎮静させ、バイタルサインをモニターした。PKH26染色ヒト血小板の輸血の30秒前に、PIC1注入を実施した。輸血前、次いで、輸血後0.5分目、2分目、5分目、20分目、60分目、および120分目に、動物からK2EDTAマイクロティナチューブ(BD, Franklin Lakes, NJ, USA)中に血液試料を採取した。最後の採血を完了すると、Fatal Plus(Vortech Pharmaceuticals, Dearborn, MI, USA)を用いて動物を安楽死させた。安楽死後に剖検を完了して、組織病理検査のために器官を採取した。
DXP 8 Color 488/637/407アップグレード(Cytek Development, Freemont, CA, USA)と共にFACSCaliburフローサイトメーター(Becton Dickinson, Franklin Lakes, NJ, USA)を用いて、フローサイトメトリーを実施した。Cytek FlowJo CEバージョン7.5.110.7を用いてデータを取得した。フローサイトメトリー機器による解析の前に、ラットの全血を500×gで5分間、遠心した。血漿血小板層を取り出し、次いでPBSで1:10に希釈した。血小板サイズに基づいて選択した約20,000個の事象をFlowJo機器によって測定した。FlowJo V10.4.2(FlowJo LLC, Ashland, OR, USA)を用いてデータを解析した。
安楽死後に肝臓および脾臓を取り出し、ホルマリン中で固定し、組織学的検査スライド用に薄片に切った。蛍光顕微鏡を用いてスライドを可視化した。
Excel(Microsoft, Redmond, WA)を用いて定量データを解析して、平均値、標準誤差(SEM)、およびスチューデントのt検定を明らかにした[15]。
抗体感作した血小板が補体をどの程度まで活性化できるかを明らかにするために、精製ヒト血小板およびプール正常ヒト血清(NHS)を用いたエクスビボ実験を以下のように実施した。理論に拘束されることを望むものではないが、臨床医学において、一般に血小板不応状態は抗血小板抗体の存在と関連付けられている。これらの実験では、血小板を感作するためにITP血清抗体を使用した。
本発明者らのエクスビボでの知見を血小板不応状態の動物モデルに変換するために、ヒト血小板がラット血清中の補体を活性化する程度を調査した。以前に開発された急性溶血性輸血反応(AHTR)の同種異系赤血球輸血モデルをこのアッセイのために使用した[29]。このモデルでは、ウィスターラット血漿中にもとから存在する天然抗体をヒト赤血球に仕向けて、補体媒介性溶血、および輸血されたヒト赤血球の血管外除去をもたらした。また、このモデルでは、補体阻害により、循環血中の輸血赤血球の数が一定期間にわたって増加した[29、31]。ラット血清がヒト血小板生存能力を低下させ得る程度を評価するために、ウィスターラットまたはスプラーグドーリーラットに由来する段階的に濃度を上げたラット血清中で、ヒト血小板をインキュベーションした。このデータを図7Aに示す。生体染色剤Presto Blueを用いて、細胞生存能力を調査した。ラット血清濃度が上昇するにつれ、生存可能なヒト血小板の数が劇的に減少し、10%血清中での30分間のインキュベーション後、25%またはそれ未満の血小板のみが生存可能であった(P<0.02)。
PKH26で血小板を染色するための以前に報告されている方法を最適化し、ラット血液試料中の輸血血小板を調査するために使用した[32]。PKH26は、ウィスターラットに輸血する前にヒト赤血球を染色するために使用されており、その後、循環中の赤血球の残留性をフローサイトメトリーによって測定する[31]。図8Aのデータに示すように、未染色の精製ヒト血小板は、530/30nmでのフローサイトメトリーによって、低いバックグラウンドシグナルを伴う典型的な分布を示した。図8Bにおいて、PKH26染色した精製ヒト血小板は、バックグラウンドより約104倍大きなピークシグナル強度を示した。染色血小板および未染色血小板を1:1の比率で混合した。図8Cに示すように、フローサイトメトリーのヒストグラムは、うまく分かれたピークおよび同様の曲線下面積を示した。これらの結果から、ヒト血小板のPKH26染色についての最適化した方法は、輸血後の信頼性が高いフローサイトメトリーデータをもたらすことが示される。
PKH26染色ヒト血小板が輸血後血液試料中で測定され得るかどうかを評価するためのパイロット研究を、頚静脈内カテーテルを留置した6匹のラットにおいて実施した。3匹のラットに1×108個のPKH26染色ヒト血小板を静脈内投与した。3匹のラットに見せかけの輸血を行った。血液試料は、輸血前(T=0)、ならびに輸血から0.5分、2分、5分、20分、60分、および120分後に得た。血液を小児用K2EDTAバイアル中に採取し、次いで生理食塩水で希釈し、フローサイトメトリーによって解析した。図9Aに示すように、T=0の試料では、最小限の染色血小板シグナルしか検出されなかった。図9Bに示すように、輸血後0.5分の時点で、PKH26染色血小板が、正確な蛍光強度で検出可能であった。輸血された3匹の各ラットにおいて、0.5分の時点で測定された染色血小板は、血液試料中の血小板総数の約5%に相当した。図9Cに示すように、輸血後2分の時点で、PKH26染色血小板に由来するシグナルは減少したが、それでもなおバックグラウンドを上回り測定可能であった。以降の時点では、血液試料中の染色血小板のフローサイトメトリー検出は減少し続けた。
古典的補体経路阻害物質PA-dPEG24を用いて、古典的補体経路の阻害が血流中の輸血された不適合性血小板の数に影響を与え得るかどうかを評価した。PA-dPEG24は、AHTRラットモデルの血流中の不適合性輸血赤血球の数を増やすことができる。図6Cに示したように、PA-dPEG24は、ラット血清中のヒト血小板の生存率を高めることができる。
14歳の鎌状赤血球症患者に、血管閉塞性疾患および急性胸部症候群向けのpRBC(packed Red Blood Cell)輸血を施した。輸血後8日目に、四肢疼痛の悪化が該患者に起こり、続いて、新たな発熱、高血圧、および呼吸代償不全が起こった。該患者のヘモグロビンは一晩で7.6g/dLから5.0g/dLに減少し、これは遅発性溶血性輸血反応と合致していた。その後、該患者は、複数回のpRBC輸血を必要とし、かつメチルプレドニゾロン、IVIg(intravenous immunoglobulin)、エクリズマブ、リツキシマブ、およびトシリズマブを含む複数の免疫調節薬を投与されながら、8日間のICU滞在を生き延びた。該患者の疾患プロセスをもっと良く理解するために、患者の赤血球および血漿を、古典的補体経路阻害物質であるPIC1(PA-dPEG24)の存在下または非存在下で溶血アッセイにおいて解析した。
Claims (45)
- 対象における輸血血小板に対する免疫応答を阻害するための方法であって、
a)古典的補体経路阻害物質を、それを必要とする対象に投与する段階;および
b)該対象に血小板を輸血する段階
を含む、方法。 - 同種免疫された対象における血小板不応状態を抑制するための方法であって、
a)古典的補体経路阻害物質で血小板を処理する段階;および
b)該処理された血小板を該対象に輸血する段階
を含む、方法。 - 抗原不適合ドナーに由来する血小板を受け取る対象における血小板不応状態を予防する方法であって、該血小板が該対象に輸血される前に古典的補体経路阻害物質を該対象に投与する段階を含む、方法。
- 抗原不適合ドナーに由来する血小板を受け取る対象における血小板不応状態を予防する方法であって、
a)古典的補体経路阻害物質で血小板を処理する段階;および
b)該処理された血小板を該対象に輸血する段階
を含む、方法。 - 前記対象における輸血血小板の生存率を高めるのに有効である、請求項1~4のいずれか一項記載の方法。
- 前記対象における輸血血小板に対する補体媒介性攻撃を低減するのに有効である、請求項1~5のいずれか一項記載の方法。
