Abstract
Description
TSAは、正常細胞によっては発現されず、がん細胞によって発現される抗原である。腫瘍特異的抗原の注目すべき例は新生抗原である。新生抗原は、正常細胞ではなくがん細胞によって発現される変異タンパク質又はペプチドであり、抗体又はT細胞のT細胞受容体(TCR)などの免疫系の分子に結合され得る。1以上のがん特異的なアミノ酸の変異を含み、免疫系の分子に直接結合されることが知られているか若しくは疑われる新生抗原の領域は、しばしばネオエピトープと呼ばれる。ネオアンチゲンなどのTSAを標的とする治療薬又はワクチンは、より効果的で安全ながん治療を提供することが期待されている。
1以上の後続の用量の貪食可能な粒子又は注射可能な医薬組成物を対象に投与すること
を含み、当該対象が、当該対象においてがん細胞に対する免疫応答を誘発するのに十分な用量の貪食可能な粒子又は注射可能な医薬組成物を以前に投与された者である、方法を提供する。

本発明で使用するための貪食可能な粒子は、コアに強固に会合した新生抗原性構築物を含む。本発明の新生抗原性構築物は、ネオエピトープペプチドを含む。

の」)治療用量の本発明の貪食可能な粒子又は治療用量の本発明の貪食可能な粒子を含む注射用組成物を投与する。貪食可能な粒子の追加的な(又は「後続の」)治療用量は、毎日、2日毎若しくは3日毎、毎週、2週間毎、3週間毎若しくは4週間毎、毎月、2ヶ月毎、3ヶ月毎若しくは4ヶ月毎、6ヶ月毎、又は毎年投与され得る。本発明の貪食可能な粒子又は本発明の貪食可能な粒子の治療用量を含む注射用組成物の追加的な治療用量の数及び頻度は、対象、並びに治療されるがんの形態及び重症度に依存する。
本発明はまた、a)APC及び抗がんT細胞を対象から採取すること、b)対象から採取した抗がんT細胞を増殖させること、及び、c)治療用量の増殖した抗がんT細胞を対象に投与することも追加の工程として含む本発明の治療を提供する。
工程a)対象への貪食可能な粒子の投与後に、対象からAPC及び抗がんT細胞を採取する工程
工程b)対象から採取した抗がんT細胞を増殖させる工程
ステップa)で対象から採取されたAPC及び抗がんT細胞は、対象におけるがんの治療又は予防に使用するのに適した抗がんT細胞の調製に使用することができる。対象への投与に適した抗がんT細胞は、対象から採取された抗がんT細胞のインビトロ活性化及び増殖によって調製される。インビトロ活性化及び増殖方法は、
i)コアと、当該コアに強固に会合した新生抗原性構築物とを含む貪食可能な粒子貪食可能な粒子を提供する工程であって、上記新生抗原性構築物が、対象においてがん細胞によって発現されることが知られているか若しくは疑われるタンパク質又はペプチドの一部のアミノ酸配列に対応するアミノ酸配列を有するネオエピトープペプチドを含み、上記タンパク質又はペプチドの一部が、少なくとも1個の体細胞変異アミノ酸を有する、工程と、
ii)APCを提供する工程と、
iii)iii)貪食可能な粒子を、APCによる貪食可能な粒子の食作用を可能にする条件下で、APCとインビトロで接触させる工程と、
iv)対象から採取された抗がんT細胞を提供する工程と、
v)インビトロで、APCによって提示されるネオエピトープに応答して抗がんT細胞の特異的活性化及び増殖を可能にする条件下で、抗がんT細胞を工程iii)からのAPCと接触させる工程と、を含む。
法は、抗がんT細胞から本発明の貪食可能な粒子を取り込んだAPCを除去する工程を含む。本発明の貪食可能な粒子が磁性コアを含む本発明の実施形態では、APCは、磁気分離を使用することによって抗がんT細胞から除去される。
c)拡大された抗がんT細胞の治療用量を対象に投与する工程
Dynabeads(登録商標)MyOne(商標)カルボン酸(ThermoFischer Scientific)(直径1μmの球)をコアとして使用した。Dynabeads(登録商標)MyOne(商標)カルボン酸粒子は、酸化鉄を含み、粒子の表面が遊離カルボン酸基で官能化された常磁性ポリスチレン粒子である。カップリング手順は、メーカーのプロトコル(NHS(N-ヒドロキシスクシンイミド)及びEDC(エチルカルボジイミド)を用いた2段階手順)に従って実施した。
1mg/ml、合計100μgの濃度に希釈して、ポリスチレン粒子に加え、室温で1時間インキュベートした。ポリスチレン粒子を磁石で収集し、上清を除去し、ペプチド濃度測定のために保存した。未反応の活性化カルボン酸基を50mM Tris pH7.4で15分間クエンチした。次いで、ポリスチレン粒子をPBS pH7.4で洗浄し、次いで-80℃で保存した。
実施例1に記載の方法に従って、ポリスチレン粒子を大腸菌で産生された組換え新生抗原性構築物に結合させた。カップリング後、ポリスチレン粒子を3つの異なる洗浄緩衝液(2M NaOH pH14.3、8M 尿素、又は6Mグアニジン(グアニジン-HCl)、すべて室温の滅菌水中)のうちの1つを用いて洗浄するか、又はポリスチレン粒子を95℃のPBS中でインキュベートした。ポリスチレン粒子を緩衝液に懸濁し、4分間振盪し、磁石で回収し、上清を除去した。これを3回繰り返した。熱処理したポリスチレン粒子をPBS pH7.4に入れ、95℃の加熱ブロックに5分間入れた後、磁石で回収し、上清を除去した。これを3回繰り返した。次いで、粒子を滅菌PBSで3回洗浄して、残っている洗浄緩衝液を除去した。
チミジン取込みを測定する細胞増殖アッセイを使用して、抗原特異的T細胞活性化に対する貪食性粒径の効果を試験した。Ovalbumin(OVA)免疫マウス由来の脾細胞を、異なるサイズのOVA結合ポリスチレン粒子で刺激して、抗原特異的増殖を測定した。
(i)抗原結合貪食可能な粒子の調製
異なるサイズの3種類の常磁性ポリスチレン貪食可能な粒子を使用した。
-直径1μm(Dynabeads MyOne カルボン酸、ThermoFisher)-直径2.8μm(Dynabeads M-270 カルボン酸、ThermoFisher)
-直径4.5μm(Dynabeads M-450 エポキシ、ThermoFisher)
(ii)抗原に結合した貪食可能な粒子のインキュベーション
ナー由来の末梢血単核細胞(PBMC)を、CMV構築物に結合した貪食可能な粒子(以下、本明細書において「CMV粒子」と呼ぶ)と共に48ウェルプレート中、37℃、5% CO2、500,000細胞/ウェルで18時間、1,000,000細胞/mlの濃度で培養した。CMV粒子の濃度は、総表面積(CMV粒子の表面に結合しているときのCMV量の代用マーカー)に基づいて平準化した。これは、試料中の全PMBCの数に基づいて、1μmサイズの粒子については10個のCMV粒子/PBMC、2.8μmサイズの粒子については1.4個のCMV粒子/PBMC、及び4.5μmサイズの粒子については0.5個のCMV粒子/PBMCに等しい。各粒子サイズの結果を以下の表1に示す。
インキュベーション後、共焦点顕微鏡を使用して、貪食されたCMV粒子の数を手動で計数した。8個の細胞を計数して、平均値及び標準偏差値を得た。図5Aには、細胞内に貪食されたCMV粒子を有する代表的な細胞の共焦点顕微鏡の画像が示されている。黒破線は細胞の輪郭を示す。白線は、細胞内のCMV粒子の全体の寸法を示す。
抗原結合粒子がT細胞を刺激することでその増殖を促進する能力を、FluoroSpotアッセイ(Mabtech、Sweden)を使用して、PBMCからのIFNγ、IL22及びIL17Aの放出を測定することによって評価した。CMV感受性健常ドナー(n=2)由来のPBMC(250,000個/ウェル)を、3連でCMV粒子で刺激した。抗原結合粒子の濃度は、総表面積に基づいて前述のように平準化した(10×1μm CMV粒子/細胞、1.4×2.8μm CMV粒子/PBMC、及び0.5×4.5μm CMV粒子/細胞)。FluoroSpotアッセイのウェルあたりのPBMCの数を、ウェルあたりの単球の推定数(PBMC試料の中身の20%が単球であるという推定に基づく)と共に、各粒径について以下に示す(表3)。
利点を伴うことを示唆している。
この実験では、2群の貪食可能な粒子、すなわち、1)ポリスチレン粒子コアを含み、コアに強固に会合した新生抗原性構築物M272120(配列番号1)を含む貪食可能な粒子、及び、2)ポリスチレン粒子コアと、コアに強固に会合した新生抗原性構築物M304748(配列番号2)とを含む貪食可能な粒子、を使用した。貪食可能な粒子を、実施例1及び2において本明細書に記載される方法を使用して調製した。すなわち、新生抗原性構築物を、実施例1及び2において概説されるプロトコルに従って1μmの超常磁性ビーズ(Sera-Mag SpeedBeads(親水性)カルボキシレート修飾磁性粒子、GE Healthcare)に結合させた。新生抗原性構築物は、B16-F10腫瘍モデルを使用した以前に公開された研究(Kreiter他(2015)、Nature、520:692及びCastle他(2012)、Cancer Res、72:1081)に基づいて設計した。作製後、2群の貪食可能な粒子を混合し(1:1の比)、次いで精製し、アジュバントとしてのCpGオリゴデオキシヌクレオチド(ODN 1668、Enzo)を含むPBSに当該粒子を入れてストック溶液を作製した。ストック溶液は1000万粒子/μlの濃度を有していた。
)で現像し、吸光度を370及び650nmで測定した。各新生抗原性構築物に対する各マウス血清試料の吸光度を、図7A(1回目の投与後にマウスから採取した血清、新生抗原性構築物=M272120(配列番号1))、図B(1回目の投与後にマウスから採取した血清、新生抗原性構築物=M304748(配列番号2))、図7C(2回目の投与後にマウスから採取した血清、新生抗原性構築物=M272120(配列番号1))、及び図7D(2回目の投与後にマウスから採取した血清、新生抗原性構築物=M304748(配列番号2))に示す。対照試料として、ナイーブマウス(n=3、投与された貪食可能な粒子の用量なし)から採取した血清試料を併せて分析した。図7A、図7B、図7C、及び図7Dによって示されるように、(リンパ節内又は皮下に投与された)高用量の貪食可能な粒子を受けたマウスは、同じ経路を介して低用量の貪食可能な粒子を受けたマウス及び貪食可能な粒子の投与を受けなかったマウス(すなわち、ナイーブマウス)から採取された血清試料と比較して、血清試料中の抗ネオエピトープ抗体の割合が高かった。
(i)膀胱がんにおけるネオエピトープ標的の同定
尿膀胱がんは、高い突然変異率を示し、その結果、免疫系によって非自己として認識される可能性がある多数のネオエピトープを発現する。したがって、本発明者らは、免疫療法用にT細胞を増殖させるために使用され得る潜在的なネオエピトープの大量のリポジトリを含む変異データベースを検索することによって、ネオエピトープペプチド及びT細胞の標的としてふさわしい腫瘍多型を調査した。
(iii)ネオエピトープによるT細胞の活性化及び増殖
マーカー、ここではCD28、CD57、T-bet、GATA-3、パーフォリン、グランザイムB(GZB)、Ki-67及びPD-1のセットによる発現強度の類似性に従って2次元マップ上でクラスター化されている。
6個の異なる貪食可能な粒子群を含む貪食可能な粒子組成物をこの試験で使用した(本明細書では「MC38粒子組成物」と呼ぶ)。各粒子群は、以下のタイプ、すなわち、1)配列番号13、2)配列番号14、3)配列番号15、4)配列番号16、5)配列番号17、6)配列番号18(表5参照)、のMC38新生抗原構築物のうちの1つに結合したポリスチレン粒子コア(Sera-Mag SpeedBeads(親水性)カルボキシレート修飾磁性粒子、GE Healthcare)を含んでいた。
結果を図9に示す。図9から分かるように、腫瘍成長は、ワクチン接種していないマウス(「陰性対照」マウス)と比較して、MC38粒子を投与したマウス(「ワクチン接種」マウス)で顕著に減少した。これらの結果は、MC38粒子が、MC38結腸直腸腫瘍
マウスモデルにおいて腫瘍成長を阻害する抗がん免疫応答を誘導するのに有効であることを示している。
貪食可能な粒子の最大許容量を評価するために、ラットモデルを使用して、貪食可能な粒子のコアの毒性及び生体内分布プロファイルを評価する。
試験に使用した粒子は、Sera-Mag SpeedBeadカルボキシレート修飾磁性粒子(親水性)である。これらの粒子は、GE Healthcare Life Sciences(粒子ロット/バッチ番号 GE:16807675、濃度:ダルベッコリン酸緩衝生理食塩水中2.3%固形分(g/100 g)(10mg Fe/mLに相当)、pH7.1)によって供給される。粒子は、使用前に2~8℃で保存する。
5匹の雌ウィスター系ラットをパイロット試験に使用する。ラット#1に、実行可能な最大濃度(鉄50mg/kgに相当する濃度5mL/kg)の粒子を静脈内(IV)注射し、その後、画像を取得するためにラットをコンピュータ断層撮影(CT)カメラに入れる。静脈内注射は可能な限りゆっくりと行う。毒性応答が明らかな場合、粒子を20分間にわたってゆっくり注入することで残りのラットに送達する(最大20 mL/kg)。ラットに送達する粒子濃度を、最大許容量が特定されるまで滴定する。粒子の用量を受けていないラットを陰性対照としてバックグラウンドCTスキャンを取得するために使用する。
12匹のラットを用いてさらなる試験を行って、粒子除去速度を同定する。全てのラットは、パイロット試験で特定した用量で粒子のIV注射を受ける。粒子の投与直後に、ラットをCTカメラに入れる。その後、ラットを、投与から24時間後、3日後、7日後、14日後及び1ヶ月後にCTカメラでモニターする。各時点で、組織病理学的分析のために器官を切除するべく2匹のラットを安楽死させる。ラットを、投与から24時間後、3日後及び7日後、そして、その後は週に1回、健康状態及び体重の変化についてモニターする。
本実験は、層流空気流(LAF)安全キャビネット内で滅菌条件下で実施した。新生抗
原構築物に結合したコア(Sera-Mag SpeedBeadsカルボキシレート修飾磁性粒子、GE Healthcare)を含む貪食可能な粒子を、高濃度アルカリ溶液(2Mから5M NaOH)で4回洗浄した。最初の洗浄後、貪食可能な粒子を新しい滅菌チューブに移し、上清を除去し、2度目の量のアルカリ溶液を添加した。貪食可能な粒子を超音波処理浴中で10分間超音波処理し、次いで、同じアルカリ溶液中でくるくると回転させながら30分間インキュベートした。このプロセスをさらに2回繰り返した後、滅菌ダルベッコ修飾PBSで4回洗浄した。
洗浄処理がないとbacillus subtilissubsp.spizizeniiのコロニーが多量に増殖したのに対して、5M及び2M NaOH処理のいずれも細菌の増殖を効果的に無効にした。結論として、2M及び5M NaOH処理のいずれも、貪食可能な粒子から人為的に高くしたバイオバーデンを除去するのに非常に有効であった。
Claims (22)
- 対象のがんの治療又は予防に使用するための貪食可能な粒子であって、コアと、前記コアに強固に会合した新生抗原性構築物とを含み、前記新生抗原性構築物が、前記対象のがん細胞によって発現されることが知られているか若しくは疑われるタンパク質又はペプチドの一部のアミノ酸配列に対応するアミノ酸配列を有するネオエピトープペプチドを含み、前記タンパク質又はペプチドの前記一部が少なくとも1個の体細胞変異アミノ酸を有する、貪食可能な粒子。
- 前記新生抗原性構築物が、2個以上の共有結合したネオエピトープペプチドを含む、請求項1に記載の使用のための貪食可能な粒子。
- 前記新生抗原性構築物が、3個以上の共有結合したネオエピトープペプチドを含み、例えば、3個、4個又は5個の共有結合したネオエピトープペプチドを含む、請求項2に記載の使用のための貪食可能な粒子。
- 前記共有結合したネオエピトープペプチドが、スペーサー部分を介して共有結合している、請求項2又は3に記載の使用のための貪食可能な粒子。
- 前記スペーサー部分が、1から15個のアミノ酸の配列であり、好ましくは1から5個のアミノ酸の配列であり、より好ましくはアミノ酸配列VVR及び/又はアミノ酸配列GGSを含む、請求項4に記載の使用のための貪食可能な粒子。
- 前記共有結合したネオエピトープペプチドのそれぞれが、3から25アミノ酸長である、請求項2から5のいずれか一項に記載の使用のための貪食可能な粒子。
- 前記新生抗原性構築物が前記コアに共有結合的に付着している、請求項1から6のいずれか一項に記載の使用のための貪食可能な粒子。
- 前記貪食可能な粒子が、前記コアに強固に会合した2個以上の異なる新生抗原性構築物を含み、例えば、前記コアに強固に会合した2個、3個、4個又は5個の新生抗原性構築物を含む、請求項1から7のいずれか一項に記載の使用のための貪食可能な粒子。
- 前記異なる新生抗原構築物のそれぞれが、異なるネオエピトープペプチド配列又はネオエピトープペプチドの異なる組み合わせを含む、請求項8に記載の使用のための貪食可能な粒子。
- 前記貪食可能な粒子が、5.6μm未満の最大寸法、好ましくは4μm未満の最大寸法、より好ましくは3μm未満の最大寸法、さらにより好ましくは0.5から2μmの最大寸法、又は最も好ましくは約1μmの最大寸法を有する、請求項1から9のいずれか一項に記載の使用のための貪食可能な粒子。
- 前記コアが常磁性又は超常磁性である、請求項1から10のいずれか一項に記載の使用のための貪食可能な粒子。

- 前記貪食可能な粒子が、請求項2から13のいずれか一項に定義される、請求項14に記載の注射可能な医薬組成物。
- 対象のがんの治療又は予防に使用するための、請求項14又は15に記載の注射可能な医薬組成物。
- 前記がんが、固形がんであり、例えば、乳がん、結腸がん、肝臓がん、肺がん(非小細胞及び小細胞)、肺カルチノイド腫瘍、膵臓がん、前立腺がん、卵巣がん及び膀胱がんから選択されるがんである、請求項1から13に記載の使用のための貪食可能な粒子又は請求項16に記載の使用のための注射可能な医薬組成物。
- 対象においてがんを処置又は予防する方法であって、貪食可能な粒子を前記対象に投与することを含み、前記貪食可能な粒子が、コアと、前記コアに強固に会合した新生抗原性構築物とを含み、前記新生抗原性構築物が、前記対象においてがん細胞によって発現されることが知られているか若しくは疑われるタンパク質又はペプチドの一部のアミノ酸配列に対応するアミノ酸配列を有するネオエピトープペプチドを含み、前記タンパク質又はペプチドの前記一部が、少なくとも1個の体細胞変異アミノ酸を有する、方法。
- がんの前記処置又は予防が、さらに、

c)治療用量の前記増殖した抗がんT細胞を前記対象に投与する工程とを含む、請求項1から13、17若しくは19のいずれか一項に記載の使用のための貪食可能な粒子、又は請求項16、17若しくは19に記載の使用のための注射可能な医薬組成物。 - 前記APC及び抗がんT細胞を前記対象に由来するPBMC試料から採取し、例えば、前記APC及び抗がんT細胞を同じPBMC試料から採取するか、又は前記APC及び抗がんT細胞を異なるPBMC試料から採取する、請求項20に記載の使用のための貪食可能な粒子又は注射可能な医薬組成物。
- 前記抗がんT細胞の活性化及び増殖工程b)が、

v.前記APCによって提示されたネオエピトープに応答して抗がんT細胞の特異的活性化を可能にする条件下で、前記抗がんT細胞を工程iii)からのAPCとインビトロで接触させる工程とを含む、請求項20又は21に記載の使用のための貪食可能な粒子、又は注射可能な医薬組成物。
