Abstract
Description

BBBは、適切な機能のために必要な栄養素を脳に供給しながら、血流中の有害物質から脳を保護する毛細血管内皮細胞の特殊なシステムである。細胞を超えて/細胞間で物質の比較的自由な交換を可能にする末梢毛細血管とは異なり、BBBは物理的(タイトジャンクション)と代謝(酵素)の両方の障壁を介して脳への輸送を厳密に制限する。したがって、BBBは、処置薬の脳への浸透を決定する際の律速要因となることがよくある。

ステント処置において、処置領域の下流にステント処置中にのみ配置される塞栓シャワー保護装置を使用することは一般的であり、保護装置は、ステント中に血管壁から落下するデブリを捕捉するという考えである。本開示の例示的な実施形態では、多孔質構造104を備えた強化されたステント装置100の使用は、塞栓シャワー保護装置の必要性を排除する。多孔質構造104の小さな開口サイズは、多孔質構造104と内腔壁404との間でステント処置中および/または後に除去される特定のサイズの動脈壁プラーク402および他のデブリを捕捉するように設計される。本開示の例示的な実施形態では、直径の開口部のサイズよりも大きいデブリがこの方法で血流に入るのを防ぐ。

本開示のいくつかの例示的な実施形態では、多孔質構造104は、支持要素102とは別に、または支持要素102なしで、内腔に移植するためにカテーテル上に配置される。カテーテルによる処置は、時間の経過とともに内腔に処置を施すように適合され、そして構築された多孔質構造104を移植するためにカテーテルを使用することによって提供される。選択肢として、医薬品または他の処置薬は、本明細書に記載されるような多孔質構造104内に埋め込まれる。本開示の例示的な実施形態では、位置決めは、中心軸106に沿って多孔質構造104の内部を通過して少なくとも部分的にカテーテルを挿入することを伴う。内腔内の処置部位への多孔質構造104の送達中に、送達中におけるカテーテルへの多孔性構造104の固定と、処置部位での多孔性構造104の移植および内腔内の多孔性構造104をそのまま残しながらカテーテルを引き抜くのを妨げるほど確実でない固定と、の間で選択肢としてバランスが取られる。例えば、多孔質構造104は、選択肢として、ロックタイト瞬間接着剤番号40340、40840、46040または3411-UV硬化性などの接着剤を使用して、選択された点でカテーテルに接着される。接着剤は、付着した多孔質構造104が送達中にカテーテルから滑り落ちるのを防ぐのに十分な強さであるが、自己拡張または他の拡張時に、多孔質構造104とカテーテルとの間の結合が切断され、多孔質構造104の内腔内の処置部位における移植が可能になる。本開示のいくつかの実施形態では、送達は6時間以下続く。選択肢として、送達は3時間以下続く。選択肢として、送達は1時間以下続く。
上記のように、自己拡張型ステントは、一般に、半径方向へのステント組立体のばねのような動きをもたらすパターンで切断された管状形状を有する。これらのステントは、小さな直径にクリンプすることによって送達システムに事前に装填され、その後、手順中にステントが展開され、血管の直径まで徐々に拡張する。ステントは、ステントの形状に基づいて足場として機能し、ステントによって血管壁に加えられる半径方向力は、半径方向力(RF)と見なされる。
図21は、捲縮およびその後の展開(例えば、ステント直径の変化)によるステント装置100の一実施形態の反力のグラフ2060を含む。ステント装置100の一実施形態は、直径20μmのPETの一本鎖から織られた外側多孔質構造104を備えた連続セルニチノールステントを含む。グラフ2060は、線2052および2054ならびに第2のゾーン2058を含む。図22は、ステント装置100の一実施形態の拡張中の慢性外向き力を詳述する表2062を含む。試験されたステント装置100の一実施形態について、半径方向力は、セグメント化ヘッド半径方向力の試験装置(ブロックワイズエンジニアリング(Blockwise Engineering)LCC、テンペ、アリゾナ、米国)を用いて決定された。ステント装置100の一実施形態は、5mmの開始直径で試験装置に直接放出された。テストは、体温を概算するために37±2°Cの温度で実施された。次に、試験ステント組立体装置の直径を、慢性的な外向きを表す半径方向力を絶えず測定しながら、0.2mm/秒の速度で直径11mmまで増加させ(ステント装置100の一実施形態の完全な拡張)。その後、ステント装置100の一実施形態の半径方向抵抗力を表す半径方向力を絶えず測定しながら、試験装置の直径を5mmまで減少させた。すべての半径方向力の値はステント装置100の一実施形態の長さによって正規化された。最小許容直径(すなわち5.5mm)での半径方向力を100%と定義すると、最大許容直径(すなわち9mm)での半径方向力は最大半径方向力の59%であった。圧縮中の半径方向の抵抗力は、慢性的な外向きの力の約2倍であり、テストした直径範囲で同様の進行が見られた。いくつかの実施形態では、慢性的半径方向力は、例えば、ステント装置100の耐用年数または設計寿命に亘ってなど、長期間にわたってステント装置100により血管壁に加えられる半径方向力である。
30人の連続した適格な患者が登録され、10.5mmの自由直径を有するステント装置100の一実施形態で処置された。すべての患者は、内頸動脈の高度の狭窄および/または症候性狭窄を有していた。症候性患者の修正ランキンスケールは1.4±0.7であった。主要な選択基準には、高悪性度の狭窄および/または症候性狭窄が含まれていた。介入前1ヶ月以内の頸動脈狭窄の臨床症状;血管径は4.7~9.0mmである。主要な除外基準には、80%未満の無症候性狭窄;頭蓋内動脈の同時急性閉塞;頭蓋内出血;および以前の同側ステント、が含まれた。患者および病変の特徴は、図27の表2090に記載されている。病変の特徴は、総頸動脈(CCA)8.4±0.6mmから内頸動脈(ICA)5.8±0.6mmまでの介入直径を示した。21本の動脈は伸長があり、そのうち6本はひどい屈曲があったが、9本の動脈は比較的まっすぐであった。すべての患者は、介入前に二重超音波(DUS)とCTまたはMRIを受けた。ステント装置100の一実施形態のそれぞれは、局所麻酔を使用する経大腿アプローチによって移植された。長い(すなわち、90cm)サポートシース(6F、Destination、Terumo Europe、Leuven、Belgium)を総頸動脈に配置した後、遠位塞栓保護装置(Boston Scientific、Natick MA、USAのFilterWire EZTM)を30人中12人(12/30)の患者に使用した。一次ステント術は、すべての患者で前拡張なしで実施された。ステント装置100の一実施形態が、患者に埋め込まれた(10x40mm(n=25)および10x30mm(n=5))。すべての患者は0.5mgのアトロピンI.V(Braun、ドイツ)を投与され、そして後拡張は、5x30 mmのバルーン(ボストンサイエンティフィック(Boston Scientific、米国マサチューセッツ州ネイティックのSterling(登録商標)モノレール)を使用して10気圧で実施した。ステント、拡張後、およびこれに続く頭蓋内デジタルサブトラクション血管造影(DSA)による遠位フィルターの除去を含むすべての症例がベースラインで記録された。穿刺部位では、クリップベースの閉鎖装置(米国カリフォルニア州サンタクララのAbbot VascularのStarClose SE)が使用された。介入後、すべての患者は脳卒中ユニットに紹介され、神経学的モニタリングを受けた。DUSは、介入後および30日後に繰り返された。サブグループでは、30日間の拡散強調磁気共鳴画像法(DW-MRI)が実施された。すべての患者は、介入の前日にアセチルサリチル酸(ASA)500mgとクロピドグレル300mg(Sanofi Aventis,ドイツ)を事前に装填された。介入中、活性化凝固時間(ACT)に応じて、5,000~10,000 IUのヘパリンが投与された。これは、250~300秒である必要があった。介入から、75mg/日のクロピドグレル(Sanofi Aventis,ドイツ)が最低6週間与えられ、ASA(100mg/日)が恒久的に与えられた。技術的な成功率は100%であった。いずれの場合も、ステント前の事前拡張は行われなかった。30例中12例(12/30)で、遠位フィルターが使用された。フィルターを回収した後、デブリは観察されなかった。関連するけいれん、遠位塞栓または解離の症例はなかった。穿刺から血管閉鎖までの処置時間の中央値は37.4±8.7分であった。介入中のACTは中央値266.3秒であった。死亡または重大な有害事象(MAE)の症例はなかった。軽度または重度の脳卒中の発生率は、手順の前後または30日間のフォローアップ期間中に発生しなかった。新しい神経学的症状を発症した患者はいなかった。死亡または重大な有害事象(MAE)の症例はなかった。軽度または重度の脳卒中の発生率は、手順の前後または30日間のフォローアップ期間中に発生しなかった。新しい神経学的症状を発症した患者はいなかった。修正されたランキンスケールは0であった。DUSの術後および30日後、すべてのステントは正規化ドップラー速度で開存した。30日で、最高収縮期速度(PSV)の中央値は75.8±9.1であった。外頸動脈はすべての患者で開存していた。DW-MRIは、30日後に30人中10人(10/30)の患者で実施され、新しい同側病変は検出されなかった。
図31aを参照して、ステント装置100は、ジャケットのない独立型管状ステント202、本明細書では裸ステント202を含む。裸ステント202は、典型的には、大きなメッシュ状の開口270を有する金属またはポリマーの管状構造を含む。裸ステント202はバルーン260を取り囲むように示されている。バルーン260が拡張すると、裸ステント202が半径方向外側に拡張する。
「実験データ」のセクションで説明されているように、上記のステントおよびジャケットの特定の構成が利点を提供するように見えることが見出された。これらの構成の特定の機能について説明する。

Claims (18)
- 1つの範囲の体腔サイズをステントするように適合されたステント組立体を使用する方法であって:
前記ステント組立体が配置される体腔の一部に関連する体腔直径を推定するステップと;
推定された前記体腔直径に基づいて、前記ステント組立体の標的拡張ステント直径を決定するステップと;
前記標的拡張ステント直径に基づいて前記体腔の一部をステントするためのステント組立体を選択するステップであって、
前記ステント組立体は、拡張直径の範囲内で初期直径から1つまたは複数の拡張直径に拡張し、一方で約0.20N/mmから約0.33N/mmの半径方向力を印加するように構成され、
前記拡張直径の範囲は、約5.5mmから約9mmであり、そして
前記標的拡張ステント直径は、前記拡張直径の範囲内にある、ステップと;
を実行させる命令を有する、ステント命令を提供するステップと:
そして
前記ステント命令に関連して、選択された前記ステント組立体を提供するステップと:
を有することを特徴とする方法。 - 前記ステント組立体は、前記拡張直径の範囲内のすべての直径に拡張するように構成される、ことを特徴とする請求項1に記載の方法。
- 前記ステント組立体が配置される前記体腔の一部は病変を含み、そして、前記推定された体腔直径は、前記病変から第1の方向に離れた第1の推定体腔直径と、前記病変から第2の方向に離れた第2の推定体腔直径を有し、前記第1の方向と前記第2の方向は反対向きである、ことを特徴とする請求項1に記載の方法。
- 前記体腔の一部が様々な体腔直径を画定する場合に、前記ステント組立体の第1の部分が前記拡張直径の範囲内にある第1の拡張直径に拡張することを可能にし、前記ステント組立体の第2の部分が前記拡張直径の範囲内にある第2の拡張直径に拡張することを可能にする、命令を更に含む、ことを特徴とする請求項1に記載の方法。
- 前記第1の拡張直径が前記第2の拡張直径とは異なる、ことを特徴とする請求項4に記載の方法。
- 前記ステント組立体が:
約7マイクロメートルから約40マイクロメートルの間の直径を有する繊維から形成された拡張可能なメッシュ構造を含むニットステントジャケットと;そして
前記ニットステントジャケットと動作可能に関連する拡張可能なステントと;
を有し、
ここで、前記拡張可能なメッシュ構造は、初期直径に関連する収縮状態と、1つまたは複数の拡張直径に関連する展開状態とを含み、前記拡張可能なメッシュ構造は、移植前および、前記拡張可能なメッシュ構造が展開状態にあるとき、最小中心寸法が約100マイクロメートルを超え、かつ約300マイクロメートル以下の開口部を画定し、
前記拡張可能なメッシュ構造は、約12.5マイクロメートルより大きく、約100マイクロメートル以下の厚さを有する、
ことを特徴とする請求項1-5のいずれか一項に記載の方法。 - ステントの方法であって:
ステント組立体が配置される体腔の一部に関連するベース基準直径を推定するステップと;
推定された前記ベース基準直径に基づいて、前記体腔の一部に配置される前記ステント組立体の標的拡張ステント直径を決定するステップと;
前記体腔の一部をステントするための前記ステント組立体を選択するステップと;

初期直径から拡張直径の範囲内の1つまたは複数の拡張直径に拡張するステップであって、
前記1つまたは複数の拡張直径の範囲は、約5.5mmから約9mmであり、
前記標的拡張ステント直径は、前記1つまたは複数の拡張直径の範囲内にある、ステップと;そして
前記ステント組立体が配置される前記体腔の一部を形成する壁に慢性的な半径方向力を加えるステップであって、前記慢性的な半径方向力は約0.33N/mm未満である、ステップと;
を実行するように構成される、

前記標的拡張ステント直径に基づいて前記体腔の一部をステントするためのステント組立体を選択するステップであって、前記ステント組立体は、拡張直径の範囲内で初期直径から任意の拡張直径に拡張し、一方で約0.20N/mmから約0.33N/mmの半径方向力を印加するように構成され、前記標的拡張ステント直径は、前記拡張直径の範囲内にある、ステップと;および
前記体腔の一部に前記ステント組立体を移植するステップと;
を実行する命令を含む、ことを特徴とする請求項14に記載のキット。 - 前記ステント組立体が、前記拡張直径の範囲内のすべての直径に拡張するように構成される、ことを特徴とする請求項14または15に記載のキット。
- 前記体腔の一部が様々な体腔直径を画定する場合に、前記ステント組立体の第1の部分が前記拡張直径の範囲内にある第1の拡張直径に拡張することを可能にし、前記ステント組立体の第2の部分が前記拡張直径の範囲内にある第2の拡張直径に拡張することを可能にする、命令を更に含む、ことを特徴とする請求項14または15に記載のキット。
- 前記第1の拡張直径が前記第2の拡張直径とは異なる、ことを特徴とする請求項17に記載のキット。
