健康な被験者における安全性、耐容性及び薬物動態
この実施例は、20:1の比率のレボドパ4’-一リン酸とカルビドパ4’-一リン酸からなる医薬組成物が安全であることを実証し、そして、その薬物動態も評価する。
16時間にわたって単一の連続皮下注入(CSCI)として送達されたレボドパ4’-一リン酸及びカルビドパ4’-一リン酸の安全性、耐容性及び薬物動態を評価するために、合計で8人の健康な高齢(45~75歳)のヒト被験者が、単一盲検プラセボ対照3期間クロスオーバーデザイン臨床試験に参加した。各用量のレボドパ4’-一リン酸とカルビドパ4’-一リン酸の組成物又はプラセボが、単一のCSCIとして単一の盲検法で腹部に投与され、移動可能なポンプに連結された注入セットを介して16時間にわたって一定の速度で送達された。レジメンA、B、C及びDは、それぞれ、640/160mgのレボドパ4’-一リン酸/カルビドパ4’-一リン酸、640/64mgのレボドパ4’-一リン酸/カルビドパ4’-一リン酸CSCI、640/32mgのレボドパ4’-一リン酸/カルビドパ4’-一リン酸CSCI又はプラセボCSCIで構成され、16時間にわたって一定の速度で腹部に送達された。薬物動態学的分析のために、血液サンプルを、カテーテルのプライミングの前、注入の前(0時間)、並びに、注入開始後0.5、1、2、4、8、12、16、20、24、26及び28時間に、8人の健康な高齢(45~75歳)のヒト被験者のそれぞれから採取した。
資格のある被験者は、年齢が45歳から75歳までの健康な男性及び女性のボランティアであった。女性の場合、被験者は、閉経後少なくとも1年間経過していなければならないか、又は、外科的に不妊でなければならない。除外基準には、重大な皮膚の状態又は障害(例えば、乾癬、アトピー性皮膚炎など)の病歴、又は、最近の日焼け、にきび、瘢痕組織、入れ墨、開いた傷、ブランディング又は着色の証拠が含まれていた。
プラセボを投与された被験者と比較して、レボドパ4’-一リン酸とカルビドパ4’-一リン酸の組成物をCSCI注入又はボーラス注射された後の、治療によって発生した有害事象の性質又は頻度に関するパターンは明白ではなかった。試験された全てのレジメンに対して、被験者は充分な耐容性を示した。有害事象の重大なパターン及び検査所見は、報告されなかった。血圧や脈拍数に関する個々の測定値についても、又は、ECGからの定量的測定値からも、注目すべき所見はなかった。
図1~3に示されているように、レボドパ4’-一リン酸とカルビドパ4’-一リン酸の組成物は、それぞれレボドパ4’-一リン酸対カルビドパ4’-一リン酸の4:1(レジメンA、図1);10:1(レジメンB、図2);及び、20:1(レジメンC、図3)の、様々な重量比で健康なヒトボランティアに皮下送達された。皮下に送達されたレボドパ4’-一リン酸とカルビドパ4’-一リン酸の該比率のそれぞれに関する時間濃度血漿レベルプロフィール(図1-3)を、4:1のレボドパ:カルビドパの重量比のレボドパ及びカルビドパの同じヒトへの経口投与から得られたレボドパ及びカルビドパの血漿レベルの時間濃度プロフィール(図4)と比較した。さらに、レボドパ4’-一リン酸とカルビドパ4’-一リン酸を20:1の比率で皮下投与することによって得られたレボドパ血漿レベルの健康なヒトボランティアにおける時間濃度プロフィール(図5)を、レボドパ対カルビドパの重量比が4:1であるDuodopa(登録商標)を十二指腸内投与することによって得られたレボドパ血漿レベルの患者における時間濃度プロフィール(図6)と比較した。さらに、レボドパ4’-一リン酸とカルビドパ4’-一リン酸を20:1の重量比で皮下投与することによって得られたレボドパ血漿レベルの健康なヒトボランティアにおける時間濃度プロフィールを、レボドパ対カルビドパの重量比が4:1であるレボドパ及びカルビドパを経口投与することによって得られたレボドパ血漿レベルの患者における時間濃度プロフィール(図7)と比較した。
図5は、時間濃度プロフィールを提供しており、表1は、20:1の重量比のレボドパ4’-一リン酸とカルビドパ4’-一リン酸からなる医薬組成物が5分間にわたる初回ボーラス注入として皮下投与され、その後、72時間にわたる連続皮下注入として皮下投与された後の、健康な高齢のヒトボランティアにおけるレボドパ及びカルビドパの血漿レベルの薬物動態パラメータを提供している(960mg/48mg レボドパ4’-一リン酸/カルビドパ4’-一リン酸)。
パーキンソン病患者を患っている被験者における72時間皮下注入の安全性、耐容性及び薬物動態
この実施例は、20:1の比率のレボドパ4’-一リン酸とカルビドパ4’-一リン酸からなる医薬組成物が安全であることを実証し、そして、72時間にわたるその薬物動態も評価する。
72時間にわたって単一の連続皮下注入(CSCI)として送達されたレボドパ4’-一リン酸及びカルビドパ4’-一リン酸の安全性、耐容性及び薬物動態を評価するために、パーキンソン病を患っている約16人の被験者において、レボドパ4’-一リン酸とカルビドパ4’-一リン酸からなる医薬組成物及びプラセボの皮下注入を単一盲検単回用量漸増試験で評価した。
資格のある被験者は、レボドパ応答性があり、一般的に良好な健康状態であり、及び、1日当たり少なくとも3回の投与を受ける安定した経口レボドパレジメンに基づいている、特発性パーキンソン病と診断された、年齢45~85歳の成人男性及び女性の被験者であった.
図16A及び図16Bに示されているように、レボドパ4’一リン酸対カルビドパ4’一リン酸の比が20:1である医薬組成物の様々な用量からのレボドパ及びカルビドパのそれぞれの平均測定濃度は、72時間にわたって一定のままであった。
安定性
この実施例は、20:1の比率のレボドパ4’-一リン酸とカルビドパ4’-一リン酸からなる医薬組成物が経時的に安定であることを示している。
構造が式(A-1)に対応するレボドパ4’-一リン酸プロドラッグ及び構造が式(B-1)に対応するカルビドパ4’-一リン酸プロドラッグ、水並びに測定された最終pHを得るのに充分な中和剤を含んでいる様々な水性医薬組成物表を表3に示されている様に調製した。表3に示されている処方を有する医薬組成物を調製し、総液量約11mLとするために10ccバイアル(約13.5mL最大充填容量を有する)の中に充填し、そして、バイアル容量の残りを窒素でパージして、約5.5%の酸素ヘッドスペースを残した。
健康なボランティアにおけるレボドパ及びカルビドパプロドラッグの24時間の連続皮下注入: 1年間の曝露をシミュレートするための10日間にわたる安全性及び耐容性
この試験は、構造が式(A-1)に対応するレボドパ4’-一リン酸プロドラッグと構造が式(B-1)に対応するカルビドパ4’-一リン酸プロドラッグ(プロドラッグの組み合わせ)を含んでいる水性医薬組成物の24時間連続皮下注入の安全性及び局所耐容性を評価するためにデザインされた。この試験では、健康なボランティアの腹部の限られた領域に該医薬組成物を連続して10日間投与することによって、1年間の暴露をシミュレートした。
該試験は、健康なボランティアの第1相、無作為化、プラセボ対照試験であり、そのボランティアは、腹部の両側への10日間にわたる同時24時間連続皮下注入よって、600mgのレボドパを提供するのに充分な該医薬組成物及び同体積の生理食塩水を投与された。該試験は、表8に示されているように、3つの期間で構成された。
良好な臨床実践及び事例に基づくデータによって、感染又は刺激、脂肪組織の蓄積(肥大)及び瘢痕形成(線維症)のリスクを低減させるために、注射部位を定期的にローテーションすること、少なくとも2.5cm離しておくことが推奨された。この試験のために選択された注入セット(Smith Cleo 90)は、セットの無菌性を維持するために3日毎にセットを交換することを推奨していた。臨床実施においては、11箇所の代替的な部位を使用した後、同じ注入部位を使用できるようにローテーションを可能にする時計のようなローテーションスキームを患者が採用することが期待される。個々人は、へそ(中央)の周りの注入部位のローテーションスケジュールを採用する。12時の位置からローテーションが始まり時計回りに進む場合、各注入部位を3日間使用すると仮定すると、患者は約36日後に12時の位置の注入部位に戻る - (12*3=)36日間、平均して同じ注入部位を10回/年で使用する。この試験は、患者が1年間にわたって使用する反復注入部位の数に関して健康なヒトボランティアの局所耐容性を評価するための長期使用の加速シミュレーションを提供した。
この試験に関する主要な包含基準は次のとおりである:
・ 45~75歳の成人男性又は女性の健康なヒトボランティア;
・ ボディマス指数(BMI) 18.0kg/m2から32.0kg/m2まで;
・ 病歴、身体検査、及び、臨床的に重要ではない検査値、心電図(ECG)又はバイタルパラメータに基づいて、一般的に良好な健康状態;及び、
・ 試験責任医師が試験評価を妨げる可能性があると特定した重大な皮膚の状態又は障害の病歴がないこと。
全身及び局所の安全性及び耐容性について、毎日評価した。顕著な皮膚反応は、注入セットの使用から予測可能な反応とは通常関連しない事象として事前に定義した(注入部位評価尺度におけるグレード≧D又は≧6)。被験者と評価者の両方は、レボドパ4’-一リン酸とカルビドパ4’-一リン酸を含む水性医薬組成物及びプラセボが注入された腹部の側面を知らなかった。具体的には、10日間の投与期間を完了した33人の健康なヒトボランティアにおいて、腹部への10日間の当該医薬組成物の24時間連続皮下注入の局所皮膚耐容性について評価した。局所皮膚耐容性は、注入部位評価2パート尺度(表9)を使用して、盲検評価者によって評価された。この評価には、数値グレード(0~7)及び文字グレード(A~G)尺度が含まれていた。顕著な皮膚反応は、注入セットの使用から予測可能な反応とは通常関連しない事象として事前に定義した(グレード≧D又は≧6)。主要評価項目は、10日間の注入の2日を超えた時点で、該医薬組成物の注入部位で顕著な皮膚反応を示した健康なヒトボランティアの数であった。10日間の注入の2日を超える時点で顕著な皮膚反応が見られる集団の割合に関する95%信頼上限は、Clopper-Pearson法によって得られた。各注入部位評価尺度に関して、最終評価で医薬組成物とプラセボとの間に差がないという仮説を医薬組成物の方がグレードが高い可能性が高いという対立仮説に対して試験するために、片側符号検定を行った。
表10に示されているように、安全性データセットには、34人の被験者が含まれていた。
該試験の1日目から10日目までの各健康なヒトボランティア被験者について報告された最高グレード(医薬組成物部位対プラセボ部位)が、図12及び図13において、それぞれの注入部位評価尺度についてグラフによって要約されている。10日間の投与を完了した被験者で、顕著な皮膚反応を報告した被験者の割合は、該医薬組成物注入部位で0%(0/33)であり、プラセボ注入部位で3.0%(1/33)であった。注入10日目の皮膚科学的評価に関する該医薬組成物とプラセボの差異は、数値グレード尺度(P=0.828)又は文字グレード尺度(P=0.363)に関して統計的に有意ではなかった。この試験で投与された該医薬組成物の10日間注入の2日以上で顕著な皮膚反応を示す集団の割合に関する95%信頼上限は、0.087(集団の8.7%)である。この試験で投与された10日間のプラセボ注入の2日以上で顕著な皮膚反応を示す集団の割合に関する95%信頼上限は、0.087(集団の8.7%)である。
臨床的に有意な検査値、バイタルサイン又はECG所見はなかった。全体として、被験者の97%が少なくとも1つの有害事象(AE)を報告した。試験完了の4日後に報告された1件の重篤なAEがあったが、これは、試験薬に潜在的に関連していないと考えられた。AEによる中止はなかった(表11)。最も頻繁に報告された有害事象は、注入部位の紅斑(91%)、注入部位の反応(44%)及び注入部位の痛み(32%)であった(表12)。注入部位の全てのAEは、軽度又は中程度の重症度であり、そして、すぐに消散した。
パーキンソン病患者におけるレボドパ及びカルビドパプロドラッグの4週間の連続皮下注入の安全性及び耐容性を評価する第1b相試験のデザイン
この試験は、構造が式(A-1)に対応するレボドパ4’-一リン酸プロドラッグと構造が式(B-1)に対応するカルビドパ4’-一リン酸プロドラッグを20:1の重量比で含む水性医薬組成物の4週間の連続皮下注入の安全性及び耐容性を評価するために設計された。さらに、該医薬組成物の連続皮下注入によって達成された定常状態血漿レボドパレベルについて評価し、そして、表18Aに記載されているエンドポイントにおけるベースラインからの変化によって探索的有効性を評価した。
28日間の24時間連続皮下注入による該医薬組成物の個別化治療用量で治療されたパーキンソン病患者の単群非盲検第1b相試験がデザインされた。患者は、米国の施設から募集された。該試験は、4つの期間で構成され、そして、図14Aにグラフで表示されている。スクリーニング期間には、適格性を確立し、患者の現在のパーキンソン病治療が30日間安定していることを確認するためのモニタリング期間を含む2回の訪問が含まれていた。スクリーニング期間には、訪問2の直後の7日間のモニタリング期間も含まれていた。患者は、被験者の投薬日記を使用してパーキンソン病の投薬を記録し、ウェアラブルデバイスを使用して運動症状をモニタリングした。

21人の患者が登録された。試験集団は、主に、男性(61.9%)及び白人(100%)であった:1人(4.8%)の被験者はヒスパニック系又はラテン系であった。平均(SD)年齢は、61.6(10.3)歳であった。診断以降の平均(SD)パーキンソン病持続期間は9.0(4.0)年であり、運動変動の平均(SD)持続期間は6.0(4.1)年であった。ベースラインにおける1日あたりの平均「オフ」時間は6.54時間であり、3.77時間から9.46時間まで変動した。7人の被験者(33%)は、時期尚早に中止された。2人の被験者は、は有害事象に起因して中止された。
実施例4に記載されている試験のための最も適切な開始投与量の医師による決定を容易にするために、経口レボドパをカルビドパ4’-一リン酸とレボドパ4’-一リン酸の医薬組成物に変換するアルゴリズムを作成した。このアルゴリズムは、連続的に皮下送達された場合のレボドパ暴露のばらつき及び変動が少ないこと、24時間暴露、該組成物を用いた以前の試験から得られたレボドパの薬物動態プロフィール及び別の臨床的考慮事項を考慮に入れている。
健康なヒトボランティアにおけるパーキンソン病の24時間レボドパ及びカルビドパプロドラッグ連続皮下注入治療の薬物動態
この試験は、腹部に連続皮下注入された後の、構造が式(A-1)に対応するレボドパ4’-一リン酸プロドラッグと構造が式(B-1)に対応するカルビドパ4’-一リン酸プロドラッグの組み合わせ(プロドラッグ組合せ)を含む水性医薬組成物の薬物動態を特徴付けるようにデザインされた。
該医薬組成物プロドラッグは、非盲検試験において、8人の健康なボランティア(45~75歳)に24時間皮下投与された。投与は、レボドパリン酸100mgの負荷投与と、それに続く24時間にわたるレボドパリン酸850mgの連続安定注入で構成された。該医薬組成物の全ての注入は、腹部の皮下空間に投与された。該医薬組成物の注入中及び注入後に、レボドパ及びカルビドパについてアッセイするために連続血漿サンプルを収集した。以前のDuopa第1相試験(Nyholm, D., et al. AAPS Journal 2013; 15-2: 316-329)からのレボドパとカルビドパの薬物動態データを使用して、本発明の医薬組成物とDuopaの間の薬物動態データを比較した。皮下注入部位に関連する局所有害事象(AE)を含む安全性と耐容性を、該試験をとおして評価した。該医薬組成物の投与後、最初の16時間にわたるレボドパの平均薬物動態プロフィールは、パーキンソン病患者における以前のDuodopa第1相試験(Nyholm, D., et al. AAPS Journal 2013; 15-2: 316-329)と同様である(図15及び図16)。
該医薬組成物の投与中及び投与後、健康なヒトボランティアにおける最初の16時間にわたるカルビドパの平均薬物動態プロフィールは、パーキンソン病患者における以前のDuodopa第1相試験(Nyholm, D., et al. AAPS Journal 2013; 15-2: 316-329)よりも変動が少ない薬物動態プロフィールを提供することが分かった(図17及び図18)。該医薬組成物の注入後のレボドパ及びカルビドパの薬物動態パラメータが、表20に示されている。
異なる部位に投与された構造が式(A-1)に対応するレボドパ4’-一リン酸プロドラッグと構造が式(B-1)に対応するカルビドパ4’-一リン酸プロドラッグ(プロドラッグ組合せ)を含む水性医薬組成物の薬物動態並びに安全性及び耐容性を特徴付けるために、該医薬組成物を3つの注入部位(健康なボランティアの腹部、腕、及び、大腿部)に投与した。
該医薬組成物を、12人の健康なボランティア(45~75歳)の3つの皮下注入部位(腹部、腕、及び、大腿部)に無作為化クロスオーバーデザインで24時間皮下投与した。投与量は、960mgの構造が式(A-1)に対応するレボドパ4’-一リン酸プロドラッグで構成され、各注入部位で24時間にわたって定常の速度で送達された。該医薬組成物の注入の終了と該医薬組成物の次の注入の開始との間に、24時間の最小ウォッシュアウト期間が組み込まれた。該医薬組成物の注入後、レボドパ及びカルビドパについてアッセイするために連続血漿サンプルを収集した。皮下注入部位に関連する局所的な有害事象を含む安全性と耐容性は、試験をとおして評価された。
該医薬組成物の投与中及び投与後、レボドパ及びカルビドパの両方の平均PKプロフィールは、3つの皮下注入部位の間で類似しているように見えた(図19及び図20)。異なる皮下注入部位における該医薬組成物の注入後のレボドパ及びカルビドパのPKパラメータが、表22に示されている。

- 化合物(A-1)の濃度が、約216mg/mL~約264mg/mLである、請求項1に記載の安定な液体水性医薬組成物。
- 化合物(B-1)の濃度が、9.6mg/mL~13.2mg/mLである、請求項1に記載の安定な液体水性医薬組成物。
