Abstract
Description
神経変性障害は、ニューロンが、通常、再生されないかまたはそれ自身置換されないとき、したがって損傷したニューロンを置換することができないときに、生じる。進行性の変性および/または神経性細胞の死は、多くの場合、運動の問題(例えば運動失調)または精神機能の問題(例えば認知症)をもたらす。多くの神経変性障害は、現在、不治であると考えられている。神経変性障害の例には、パーキンソン病(「PD」)、アルツハイマー病(「AD」)およびハンチントン病(「HD」)が挙げられる。

本発明は、1日総用量のドーパミン補充剤を患者に投与する方法に関する。ドーパミン補充剤は、ドーパミン脱炭酸酵素阻害剤(DDI)およびカテコール−O−メチル基転移酵素(COMT)阻害剤と一緒に、異なる化合物が個々にまたは組み合わせて投与され得る医薬組成物中で投与され、方法は:

上記のように、多くのドーパミン関連疾患、障害または状態は、1つまたは複数の関係する組織または部位におけるドーパミンレベルの低下と関係する。推奨療法には、多くの場合、ドーパミン、または投与後にドーパミンへと代謝的に変換される前駆体化合物の投与が挙げられる。特に、ドーパミンが血管脳関門を通過しないことを考慮すると、血管脳関門を通過する代わりの薬剤は、PDのような神経変性ドーパミン関連疾患、障害または状態の治療に特に望ましい。そのような薬剤は、本明細書において「ドーパミン補充剤」と称され、例えばドーパミンの代謝前駆体(例えばレボドパ、メレボドパ、エチレボドパ等およびそれらの組合せ)、ドーパミンアゴニスト(例えばアポモルヒネ、ブロモクリプチン、カベルゴリン、ジヒドロエルゴクリスチンメシル酸塩、ペルゴリド、ピリベジルプラミペキソール、ロピニロール、ロチゴチン等およびそれらの組合せ)、ドーパミンの分解を遮断する薬剤(例えばMAO−B阻害剤、例えばセレギリン、ラサギリン等、およびそれらの組合せ)および/または他にドーパミン産生を刺激する薬剤(例えばブジピン)が挙げられる。これらの薬剤の多様な市販の製剤および調合剤は当該技術分野で周知であり、ある特定の経口(例えばカプセル剤または錠剤)、経皮(例えばパッチ)、非経口(例えば特に注入のための、皮下、静脈内、脊髄内等)、および/または他の(例えばゲル、および特に腸内用ゲル)製剤が挙げられる。
芳香族アミノ酸であるレボドパは、白色の結晶性化合物であり、水に難溶性であり、分子量は197.2である。レボドパは、化学的に、(−)−L−α−アミノ−β−(3,4−ジヒドロキシベンゼン)プロパン酸と呼ばれる。レボドパの実験式はC9H11NO4であり、その構造式は、以下のものである:
レボドパは、30から60分である体内での短い半減期を有し、レボドパ単独を取り込むと、レボドパが脳に達する前に、90%超がドーパミンに代謝される。よって、レボドパを投与するための多くのプロトコールが大用量の投与を伴い、このことは、次いで、多くの場合悪心および他の有害な副作用を伴うことのある、脳外での高いドーパミンの濃度をもたらす。レボドパのバイオアベイラビリティを増大させ、その副作用を低減するため、レボドパは、したがって通常、ドーパミン脱炭酸酵素阻害剤(DDI)、典型的にカルビドパ(L−2−ヒドラジノ−3−(3,4−ジヒドロキシフェニル)−2−メチルプロパン酸)またはベンセラジド(DL−2’−(2,3,4−トリヒドロキシベンジル)セリンヒドラジド)とともに投与され、これは脳の外でレボドパのドーパミンへの変換を阻害し、血管脳関門を通過しない。
芳香族アミノ酸の脱炭酸の阻害剤であるカルビドパは、白色の結晶性化合物であり、水に難溶性であり、分子量は244.2である。カルビドパは、化学的に、(−)−L−α−ヒドラジノ−α−メチル−β−(3,4−ジヒドロキシベンゼン)プロパン酸一水和物と呼ばれる。
ベンセラジドは、芳香族アミノ酸の脱炭酸の阻害剤であり、分子量は257.2である。ベンセラジドは、化学的に、(RS)−2−アミノ−3−ヒドロキシ−N’−(2,3,4−トリヒドロキシベンジル)プロパンヒドラジドと呼ばれる。ベンセラジドの実験式はC10H15N3O5であり、その構造式は、以下のものである:
一部の実施形態では、本発明の特性は、組成物中の、および特に、任意選択で1つまたは複数の他の活性剤と(例えばレボドパとおよび/またはDDIと)組み合わせた腸内用ゲル組成物中のCOMT阻害剤の投与を通して、ある特定の有益な効果が達成され得、および/または問題が避けられ得るという認識を含む。本開示は、例えば医薬組成物中のCOMT阻害剤の腸内投与が、ドーパミン前駆体を用いた、および特にレボドパを用いた治療を受けている対象に特に利益を有することを示す。
i)多様な日常活動の延長を可能とする、レボドパの排出半減期の予期される延長;
ii)ホモシステインの血漿レベルの低減を引き起こす、不活性なレボドパ代謝産物である3−O−メチルドパ(3−OMD)の血漿レベルの低減。血管イベント、認知機能不全のような非運動性症状(NMS)および末梢性ニューロパチーについての問題となるリスク因子にも関わらず、ホモシステインのレベルの上昇が提唱されている;
iii)Lecigonを用いて1日にわたり得られるレボドパの血漿レベルの漸増は、1日の後半の間に高頻度に観察されるパーキンソン症状の悪化に適合する。
医薬組成物中のレボドパの安定性を改善するため、および/またはその送達の一貫性を改善するために、例えばレボドパ投与の1つまたは複数の副作用(例えばジスキネジア)を低減すること、ならびに/または「オフ期間」の頻度および/もしくは長さを低減することを期待して、ある特定の試みがなされている。
本発明のある特定の実施形態の1つの特性は、医薬組成物、および特に腸内用ゲル組成物の、保存安定性または貯蔵寿命に関する。
本明細書に記載されているように、本発明は、(a)ドーパミン補充剤;(b)1つまたは複数のDDIおよび(c)1つまたは複数のCOMT阻害剤を用いた組合せ療法を伴うおよび/または達成する、技術を提供する。本明細書に記載されているように、多くの実施形態では、本開示は、ある特定の治療レジメンおよび形態が既に周知である、個々の薬剤または組合せ薬剤の投与に関する。一部の実施形態では、本開示で具体化された識見は、そのように周知のレジメンおよび/または形態に対して低減された投薬(例えば1日量、選択された期間にわたる総量、および/または投薬の頻度において)を含有するまたはそれを含む、組成物および/または投薬レジメンを提供する。
「静脈内投与(3mg/kg)の後、エンタカポンの排出半減期(t1/2β)(0.8時間)は、トルカポンの排出半減期(2.9時間)よりも明らかに短かった。トルカポンの線条体/血清比は、エンタカポンのものよりも3倍高かった。単一の経口用量(10mg/kg)の後、エンタカポンおよびトルカポンの両方は、末梢組織におけるCOMT阻害について等しい最大の程度をもたらしたが、トルカポンは、エンタカポンよりも効果的に線条体内のCOMTを阻害した。7日間の治療(1日2回、10mg/kg)の後、COMT活性は、エンタカポンの最終用量後8時間以内に、対照の67から101%のレベルに回復した。トルカポンで治療された動物では、末梢組織においてなお大きなCOMT阻害が依然として存在し、阻害の程度は、単一用量後に達成されたものよりも高かった。薬物動態学的および薬力学的モデリングは、エンタカポンおよびトルカポンの両方で、最大の達成可能な阻害に近いCOMT阻害の停滞期が、2000ng/mlを下回る血漿濃度により既に達成されたことを明らかにした。エンタカポンおよびトルカポンは、それぞれ10.7および10.0nMのKi値で、インビトロでラット肝COMTを等しく阻害した」
と報告しており、これらの結果を、「トルカポンが12時間間隔で投与されるときに、末梢のCOMT阻害剤は継続的に阻害されるが、これはエンタカポンでは見られないことを示唆する」と結論づけている。
一部の実施形態では、本発明は、レボドパ、DDIおよびCOMT阻害剤の組合せを対象に投与し、ここで組合せ中の1つまたは複数の薬剤が医薬ゲルの腸内投与により投与されることは、1つまたは複数の活性剤の1つまたは複数の薬物動態学的特性(例えば曲線下面積(AUC)、バイオアベイラビリティ(例えば絶対的バイオアベイラビリティ、相対的バイオアベイラビリティ)、半減期等)の予期されない改善をもたらすという識見を包含する。
PKモデルを開発し、400mgから2000mgまでのレボドパの1日総用量についてレボドパの濃度時間曲線をシュミレーションした。シュミレーションの結果を図1に示す。レボドパの1日総用量の25%を、朝の用量として与えることを仮定する。シュミレーションは、レボドパの血漿レベルの漸増が、より高い用量でより明白であることを示す。
Claims (26)
- 医薬としての使用のための、ドーパミン補充剤、ドーパミン脱炭酸酵素阻害剤(DDI)、およびカテコール−O−メチル基転移酵素(COMT)阻害剤を含む医薬組成物であって、前記ドーパミン補充剤の1日総投与量が、
a.前記ドーパミン補充剤の前記1日総投与量の20〜35%である、前記医薬組成物の第1の投与量を投与するステップ;および
b.合計して前記ドーパミン補充剤の前記1日総投与量となる前記医薬組成物の第2の投与量を継続投与するステップ
により投与される、医薬組成物。 - 方法が、
a.前記ドーパミン補充剤の前記1日総投与量の20〜35%である、医薬組成物の第1の投与量を投与するステップ;
b.合計して前記ドーパミン補充剤の前記1日総投与量となる前記医薬組成物の第2の投与量を継続投与するステップであって、前記第2の投与量の第1の部分は最初にあらかじめ決定された時間継続投与され、その後前記第2の投与量の第2の部分が継続投与され、前記第1のおよび第2の部分の合計は、合計して前記第2の投与量となり、前記第2の部分について、より低い量の前記ドーパミン補充剤が、時間単位あたり投与される、継続投与するステップ
を含む、請求項1に記載の医薬組成物。 - 前記ドーパミン補充剤が、レボドパまたは薬学的に許容されるその塩または誘導体である、請求項1または2に記載の医薬組成物。
- 前記ドーパミン脱炭酸酵素阻害剤が、カルビドパ、ベンセラジドおよびそれらの組合せからなる群から選択される、請求項1から3のいずれか1項に記載の医薬組成物。
- 前記ドーパミン脱炭酸酵素阻害剤がカルビドパである、請求項4に記載の医薬組成物。
- 前記COMT阻害剤が、エンタカポン、トルカポン、オピカポンおよびそれらの組合せからなる群から選択される、請求項1から5のいずれか1項に記載の医薬組成物。
- 前記COMT阻害剤がエンタカポンである、請求項5に記載の医薬組成物。
- 前記組成物が、カルビドパのヒドラジンへの分解を阻害する物質をさらに含む、請求項1から7のいずれか1項に記載の医薬組成物。
- 前記物質がエンタカポンを含む、請求項8に記載の医薬組成物。
- 前記ドーパミン補充剤、前記ドーパミン脱炭酸酵素阻害剤(DDI)および前記カテコール−O−メチル基転移酵素(COMT)阻害剤、ならびに任意選択で好適なアジュバントが、単一の医薬組成物中に含有される、先行する請求項のいずれか1項に記載の医薬組成物。
- 前記医薬組成物が第1、第2および第3の部を含み、前記第1の部が前記ドーパミン補充剤を含み、前記第2の部が前記ドーパミン脱炭酸酵素阻害剤(DDI)を含み、前記第3の部が前記カテコール−O−メチル基転移酵素(COMT)阻害剤を含み、各部が任意選択で好適なアジュバントを含むか、または前記医薬組成物が、前記ドーパミン補充剤および前記ドーパミン脱炭酸酵素阻害剤(DDI)を含む第1の部分ならびに前記COMT阻害剤を含む第2の部分を含み、各部が任意選択で好適なアジュバントを含む、請求項1から10のいずれか1項に記載の医薬組成物。
- 前記第1の投与量の前記ドーパミン補充剤、前記ドーパミン脱炭酸酵素阻害剤(DDI)および前記カテコール−O−メチル基転移酵素(COMT)阻害剤、ならびに任意選択で好適なアジュバントが個々に投与されるか、または前記ドーパミン補充剤および前記ドーパミン脱炭酸酵素阻害剤(DDI)が同時に投与され、前記COMT阻害剤が、前記ドーパミン補充剤および前記ドーパミン脱炭酸酵素阻害剤(DDI)と本質的に同時に投与される、請求項1から10のいずれか1項に記載の医薬組成物。
- 前記ドーパミン補充剤の前記1日総投与量が、200から3500mg、例えば400〜3000mgである、先行する請求項のいずれか1項に記載の医薬組成物。
- 患者がパーキンソン病を患っている、先行する請求項のいずれか1項に記載の医薬組成物。
- 前記患者が、運動亢進症の感受性を患っているか、または運動亢進症になる傾向を有する、請求項14に記載の医薬組成物。
- 前記継続投与が、24時間継続的に実施される、請求項1から14のいずれか1項に記載の医薬組成物。
- 前記継続投与が、12〜18時間、例えば13〜17時間または14〜16時間、継続的に実施される、請求項1から15のいずれか1項に記載の医薬組成物。
- 前記第1の投与量が、午前5時と午前10時との間のような朝に投与される、先行する請求項のいずれか1項に記載の医薬組成物。
- 前記第1の投与量の前記組成物が、経口用組成物、液体組成物、懸濁剤またはゲルである、先行する請求項のいずれか1項に記載の医薬組成物。
- 前記第1の投与量が、経口、静脈内、皮下、腸内または経皮投与される、請求項19に記載の医薬組成物。
- 前記第2の投与量の前記組成物が、液体組成物、懸濁剤またはゲルである、先行する請求項のいずれか1項に記載の医薬組成物。
- 前記第2の投与量が、静脈内、皮下、腸内または経皮投与される、請求項21に記載の医薬組成物。
- 第1の容器および少なくとも1つのさらなる容器を含むキットであって、前記第1のおよび前記少なくとも1つのさらなる容器が、それぞれ、医薬としての使用のための、請求項1から22のいずれか1項に記載の医薬組成物の第1のおよび第2の投与量を含み、
a.前記第1の投与量は、前記ドーパミン補充剤の前記1日総投与量の20〜35%を含み;
b.前記第2の投与量は、合計して前記ドーパミン補充剤の前記1日総投与量となり;
少なくとも前記第2の投与量は、液体の医薬組成物、懸濁剤またはゲルである、キット。 - 前記第1のおよび第2の投与量の両方が、液体の医薬組成物、懸濁剤またはゲルの形態である、請求項23に記載のキット。
- ドーパミン補充剤、ドーパミン脱炭酸酵素阻害剤(DDI)およびカテコール−O−メチル基転移酵素(COMT)阻害剤、ならびに任意選択で好適なアジュバントを含む医薬組成物により、それを必要とする患者に前記ドーパミン補充剤の1日総投与量を投与する方法であって、
