Abstract
Description
本出願は、2018年9月25日に出願された米国仮特許出願第62/736,393号及び2018年11月28日に出願された米国仮特許出願第62/772,372号の優先権を主張するものであり、両出願の開示は参照によりその全体が本明細書に組み込まれる。
本明細書で使用されるとき、「a」又は「an」は「少なくとも1」又は「1以上」を意味する。
-1)年齢≧75歳、又は
-2)年齢≧18〜74歳で、且つ強化導入化学療法に対する適合不足に関連する少なくとも1つの以下の基準を満たすこと
のいずれかで定義される。
-東海岸癌臨床試験グループ(Eastern Cooperative Oncology Group; ECOG)パフォーマンスステータス2〜3、
-駆出率≦50%の治療を要する鬱血性心不全(CHF)又は慢性安定狭心症の心臓の病歴、
-一酸化炭素肺拡散能(DLCO)≦65%又は1秒間努力呼気肺活量(1秒量)(FEV1)≦65%、
-クレアチニンクリアランス≧30mL/min〜<45mL/min、
-総ビリルビン>1.5〜≦3.0×正常上限(ULN)の中度の肝障害、又は
-医師が従来の強化化学療法と不適合であると判断するその他の併存疾患。
急性骨髄性白血病は、新しい血液細胞生成の障害を引き起こす未分化骨髄系前駆細胞のクローン性増殖である。診断のメジアン年齢は67歳である。臨床症状には発熱、疲労、骨痛、蒼白、痣のできやすさが挙げられる。診断は骨髄に≧20%の骨髄性芽細胞を有することに基づく。細胞遺伝学及び分子遺伝学の知見を含む疾患特性は主要な予後因子である。患者特性(年齢、併存疾患)もまた、治療を受ける能力に影響するので、予後において役割を果たす。現在、適合した患者に対する最適な疾患の治療は、誘導(導入)及び強化(地固め)段階に分けられる強化化学療法に基づく。鑑別診断は他の悪性血液疾患及びMDSを含む。
骨髄異形成症候群は、骨髄が十分な健常赤血球、白血球又は血小板をつくれなくなる一群の希少疾患である。これは、異常な形状、大きさ又は外見を有するたくさんの発育不全な、又は未熟な細胞を産生する骨髄によって引き起こされる。これらは芽細胞(blast cell)と呼ばれる。殆どの専門家は、MDSが血液及び骨髄がんの一形態であり、診断するのが困難であり得ることに同意する。
新たに診断されたハイリスク/二次性AMLの高齢者(年齢60〜75歳)における第3相試験で、CPX-351は全生存を標準的な7+3シタラビン/ダウノルビシンに対して顕著に延ばし(9.56対5.95月、ハザード比=0.69、片側P=0.003)、CPX-351の安全性プロファイルは全般的に7+3レジメンの公知のプロファイルと一致した(Lancet JE, et al. J Clin Oncol. 2018;36(26):2684-2692、参照により本明細書に組み込まれる)。
7+3又はCPX-351により誘発される血小板減少症の集団薬物動態学-薬力学(PK-PD)モデル化
PK-PDモデル化に以下のCPX-351研究を使用した。
●第1相、再発性/難治性AML、ALL、又はMDSの48人の成人におけるCPX-351の非盲検用量増加研究
●CPX-351投薬量:3〜134単位/m2(1単位=1mgのシタラビン及び0.44mgのダウノルビシン)、1、3、及び5日目の90分静脈内(IV)注入による
●PKデータ収集:1、3、及び5日目の集中サンプリング
●PDデータ収集:投与前、1、2、3、4、5、7、14、21、28、35、42、及び56日目、並びにフォローアップ(研究中断後30日)
●第2相、AML、ALL又はMDSの26人の成人におけるCPX-351の非盲検研究
●CPX-351投薬量
○誘導(導入):100単位/m2(100mg/m2シタラビン+44mg/m2ダウノルビシン)、1、3、及び5日目の90分IV注入による(第2の誘導:1及び3日目)
○強化(地固め):65単位/m2(65mg/m2シタラビン+29mg/m2ダウノルビシン)、1及び3日目の90分IV注入による
●PKデータ収集:1及び5日目の集中サンプリング
●PDデータ収集:投与前、1、3、5、7、10±2、及び14±2日目、並びにその後(1)42日、(2)末梢血球数回復、又は(3)研究から撤退のいずれか最後に起こるまで毎週(±2日)
●第3相、新たに診断されたハイリスク/二次性AMLの高齢者における多中心ランダム化試験、CPX-351(n=153)対7+3(n=156)を評価する
●CPX-351投薬量:研究206と同じ
●PKデータ収集:第1週を通してまばらなサンプリング
●PDデータ収集:研究206と同じ
CPX-351及び「7+3」により治療される患者における化学療法により誘発される好中球減少症(CIN)のモデル化
実施例1と同様にしてCPX-351及び7+3研究並びにシタラビン/ダウノルビシンモデル化を行った。
ができた。関連のある患者人口統計は解析データセットの場合と同等な分布で生じさせた。
CPX-351に対する代替の用量-レジメンのシミュレーション
より低い用量/レジメンシナリオに対する骨髄抑制プロファイルのシミュレーション:

CPX-351及びベネトクラクスはex vivo白血病モデルにおいて相乗作用を示す
ベネトクラクスと組み合わせたCPX-351の細胞毒性を評価するために、様々な薬剤:薬剤の組合せをヒト急性骨髄性白血病(AML)の15のPassage 1(P1)モデルで試験し、細胞生存率を分析した。
CPX-351及びベネトクラクスの臨床試験
CPX-351低強度療法(LIT)プラスベネトクラクスの第1b相試験が強化化学療法に適格ではないAMLの対象に対する一次(第一選択)治療として行われる。
●対象は世界保健機関(WHO)基準による組織学的確認を伴う新たに診断されたAMLを有していなければならない。
標準的なICTに適格ではない対象の定義:
●各々の対象は、研究に登録されるには、療法の最初の日の前21日以内にICTを受けるのに適格ではないと特徴付ける以下の基準を満たさなければならない:
○年齢≧75歳(≧ 75 years of age)
又は
○年齢18〜74歳(≧ 18 to 74 years of age)で、以下のICTに対する適合性の欠乏に関連する少なくとも1つの基準を満たす:
- 東海岸癌臨床試験グループ(ECOG)パフォーマンスステータス2〜3
- 治療を要する鬱血性心不全(CHF)の心臓の病歴又は左心室駆出率(LVEF)≦50%
- 一酸化炭素肺拡散能(DLCO)≦65%又は1秒間努力呼気肺活量(FEV1)≦65%
- Cockcroft-Gault式により計算されるクレアチニンクリアランス(CrCl)≧30mL/min〜<45mL/min
- 総ビリルビン>1.5〜≦3.0×正常上限(ULN)の中度の肝障害
- 研究に登録する前に研究メディカルモニターにより審査され承認されなければならない従来の強化化学療法と不適合であると医師が判断する他の併存疾患
加えて、全ての対象は以下の基準を満たさなければならない:
●対象が年齢≧75歳であれば、ECOGパフォーマンスステータスは0〜2でなければならない。
●対象は、CrCl≧30mL/min(Cockcroft Gault式により計算されるか又は24時間採尿により測定される)により示される適切な腎機能を有していなければならない。
●対象は:
○アスパラギン酸アミノトランスフェラーゼ(AST)≦3.0×ULN*、
○アラニンアミノトランスフェラーゼ(ALT)≦3.0×ULN*、
○ビリルビン≦1.5×ULN(年齢<75歳の対象はビリルビン≦3.0×ULNであってもよい)*
*白血病性臓器の障害に起因すると考えられる場合を除く
により示される適切な肝機能を有していなければならない。
●女性の対象は:
○年齢>55歳で、別の医学的原因なしに≧2年月経がない、
○又は
○年齢≦55歳で、別の医学的原因なしに≧12カ月月経がなく、且つ卵胞刺激ホルモンレベル>40IU/L
○又は
○永久不妊手術(両側卵巣摘出、両側卵管切除又は子宮摘出)
として定義されるように閉経していなければならない、
又は
●出産可能であって、研究の1日目から開始して研究治療の最後の投薬後少なくとも6カ月の間中バースコントロールの少なくとも1つのプロトコルに規定された方法を実施する女性
●出産可能な女性は:
○最初の研究治療投与に先立つ14日以内に得られる血清サンプルを用いる前処理で、そして
○血清妊娠検査結果を得てから>7日経っていたら、周期1の1日目に得られる尿サンプルを用いて投薬前に
実施される妊娠検査でネガティブな結果を有していなければならない。
○前処理時にボーダーラインの妊娠検査をした対象は、ポジティブな結果なしのままと記録するには≧3日後に血清妊娠検査を受けなければならない。
●性的に活性な男性の対象は、研究1日目から研究治療の最後の投薬後少なくとも6カ月間を通してプロトコルに規定された避妊方法を実践することに同意しなければならない。男性の対象は最初の研究治療投与から研究治療の最後の投薬後少なくとも6カ月間精子の提供を控えることに同意しなければならない。
●対象は白血球数≦25×109/Lでなければならない(注:治療上の細胞除去のためにヒドロキシウレア投与又は白血球アフェレーシスを受けた対象は適格であると考えられる)。
以下の基準のいずれかを満たす対象は研究から除外される:
●対象が年齢に関係なくECOGパフォーマンスステータス>3を有する。
●対象が治療開始/CPX-351の最初の投薬まで許されるヒドロキシウレアを除いてAMLのための前治療を受けている(注:骨髄異形成症候群のための前治療はシタラビン又はダウノルビシンの使用を除いて許される)。
●対象がAMLのためのNational Comprehensive Cancer Network(NCCN)ガイドラインバージョン2.2014により分類された有利なリスク細胞遺伝学((t8;21)、inv(16)、t(16;16)又はt(15;17)核型異常)を有する。
●対象が、先行する骨髄増殖性腫瘍(MPN)、例えば骨髄線維症、本態性血小板血症、真性赤血球増加症、又はBCR-ABL 1転座を伴うか伴わない慢性骨髄性白血病(CML)及びBRC-ABL 1転座を伴うAMLを有していた。
●対象が急性前骨髄球性白血病(APL)を有する。
●対象がAMLの公知の中枢神経系(CNS)合併症を有する。
●対象が公知のヒト免疫不全ウイルス(HIV)感染(抗レトロウイルス薬とベネトクラクスとの潜在的な薬物-薬物相互作用に起因する)を有する。地域的ガイドライン又は機関規格により必要とされれば前処理時にHIV試験を行う。
対象はB型肝炎ウイルス(HBV)、又はC型肝炎ウイルス(HCV)感染に陽性であることが知られている。(不活性な肝炎キャリア状態又は抗ウイルス剤[非排除医薬]に対する低いウイルス性肝炎力価は除外されない。)
CPX-351は使い捨て式バイアルに入った無菌の保存剤が入っていない紫色の凍結乾燥ケーキとして提供される。CPX-351の各々のバイアルは44mgのダウノルビシン及び100mgのシタラビンを含有する。再構成後(しかし、最終希釈前)各1mLが2.2mgのダウノルビシン及び5mgのシタラビンを含有する。CPX-351はおよそ90分にわたってIV注入として投与される。ベネトクラクスは3つの強度で入手可能であり、経口用量として毎日1回食事及び水と共に自己投与される:
●100mg錠剤は、一方の側に「V」、他方の側に「100」とデボス加工された楕円形で両凸形状の淡黄色フィルムコート錠として提供される。
●50mg錠剤は、一方の側に「V」、他方の側に「50」とデボス加工された楕円形で両凸形状のベージュ色フィルムコート錠として提供される。
●10mg錠剤は、一方の側に「V」、他方の側に「10」とデボス加工された円形で両凸形状の淡黄色フィルムコート錠として提供される。
Claims (20)
- ダウノルビシン及びシタラビンを供給する唯一の有効成分としてCPX-351を含む、標準的な強化化学療法に適格ではない対象(ISICT対象)において血液増殖性疾患を治療する治療周期における使用のための医薬組成物であって、治療周期が前記組成物を投与して治療周期にわたって300mg/m2未満のシタラビンを送達することを含む、前記組成物。
- 前記治療が、治療周期にわたって250mg/m2未満のシタラビンを送達する、請求項1に記載の組成物。
- 前記治療周期が、1日あたり135mg/m2以下の1日用量のシタラビンを送達し、2日からなる、請求項1に記載の組成物。
- 前記組成物が、1及び3日目又は1及び5日目又は1及び8日目の間隔で合計2日間以下、投与される、請求項1に記載の組成物。
- 前記組成物が1日あたり32mg/m2未満の1日投与量のシタラビンを送達し、治療周期が3日以下からなる、請求項1に記載の組成物。
- 3時間未満で静脈内に投与される、請求項1に記載の組成物。
- プロトコルが、白血病を治療するのに有効であると示されている薬剤を投与することを更に含む、請求項1〜5のいずれか一項に記載の組成物。
- 前記薬剤がBCL-2阻害剤又は低メチル化剤である、請求項7に記載の組成物。
- 前記薬剤がマイロターグ、ミドスタウリン、ベネトクラ(venetocla)又はイディファである、請求項7に記載の組成物。
- 血液増殖性疾患が、急性リンパ性白血病(ALL)、急性骨髄性白血病(AML)、慢性骨髄性白血病(CML)、慢性リンパ球性白血病(CLL)又は急性前骨髄球性白血病(APL)、骨髄異形成症候群(MDS)又は骨髄増殖性腫瘍(MPN)である、請求項1〜5のいずれか一項に記載の組成物。
- 血液増殖性疾患がAML、ALL、CLL、APL、MDS又はMPNである、請求項7に記載の組成物。
- ISICT対象において血液増殖性疾患を治療する方法であって、治療周期にわたって300mg/m2未満のシタラビンを送達することを含む治療周期においてダウノルビシン及びシタラビンを供給する唯一の有効成分としてCPX-351を含む医薬組成物を前記対象に投与することを含む、方法。
- 前記治療が治療周期にわたって250mg/m2未満のシタラビンを送達する、請求項12に記載の方法。
- 前記治療周期が、1日あたり135mg/m2以下の1日用量のシタラビンを送達し、2日からなる、請求項12に記載の方法。
- 前記組成物が、1及び3日目又は1及び5日目又は1及び8日目の間隔で合計2日間以下、投与される、請求項12に記載の方法。
