Claims (34)
- 人体内の領域に器具をガイドするためのアクセス装置であって、前記アクセス装置は、
近位端及び遠位端を含むシャフトであって、前記近位端と前記遠位端との間で長手方向に延びるチャネルを含む内腔を画定し、器具チャネル、バルーン膨張チャネル、及びガイドワイヤチャネルを含む前記シャフトと、
前記シャフトの前記遠位端に取り付けられたヘッドであって、最上表面と、前記最上表面と反対側の最下表面と、前記器具チャネルを通して延びた器具が前記人体の前記領域と相互に作用できるように構成されたポートとを含む前記ヘッドと、
前記ヘッドに取り付けられたカメラであって、前記シャフトを通して延びるカメラケーブルに可換に結合され、前記人体の前記領域の画像を取り込むように向けられた前記カメラと、
前記ヘッドの前記最上表面に取り付けられたバルーンであって、前記バルーン膨張チャネルと流体連絡し、前記ヘッドの前記最下表面に隣接して広がることなく、前記ヘッドの前記最上表面に隣接して広がるように構成された前記バルーンと、を含む、前記アクセス装置。 - 前記ヘッドは円柱形状を有し、前記バルーンは、前記ヘッドの前記最上表面の円周に沿って170度から190度の間に広がる、請求項1に記載のアクセス装置。
- 前記ポートは前記ヘッドの前記最下表面上にあり、前記器具チャネルは真空に接続する、請求項1に記載のアクセス装置。
- 前記チャネルは、前記器具チャネルから離れた真空チャネルをさらに含み、前記真空チャネルは真空に接続し、前記ヘッドは前記ヘッドの前記最下表面上に真空ポートを画定し、前記真空ポートは前記真空チャネルに流体的に接続されている、請求項1に記載のアクセス装置。
- 前記器具チャネルは代替的に、診断プローブと、切除器具と、注射可能物質を送達するように構成された注射器具とを収容するように構成されている、請求項1に記載のアクセス装置。
- 前記チャネルは、前記人体の前記領域における組織厚さ及び密度を決定するように構成された超音波トランスデューサプローブを収容するように構成された診断プローブチャネルをさらに含む、請求項1に記載のアクセス装置。
- 前記ヘッドは、前記人体の前記領域における組織厚さ及び密度を決定するように構成された超音波トランスデューサプローブをさらに含み、前記超音波トランスデューサプローブは、前記シャフトを通して延びる診断ケーブルに可換に結合されている、請求項1に記載のアクセス装置。
- 前記超音波トランスデューサプローブは、印加電流に応答して振動する電子的に相互接続された圧電性結晶を含む、請求項7に記載のアクセス装置。
- 前記カメラは、光ファイバ照明を伴う相補型金属酸化物半導体(CMOS)チップを含む、請求項1に記載のアクセス装置。
- 前記アクセス装置は、前記カメラに対する照明を与えるように構成された光ファイバケーブルを含む、請求項9に記載のアクセス装置。
- 前記カメラは前記ヘッドの長手軸に対して1~30度の角度で向けられている、請求項1に記載のアクセス装置。
- 前記チャネルはカメラフラッシュチャネルをさらに含み、前記ヘッドは、前記カメラのカメラレンズに渡って流体を送るように構成されたカメラフラッシュポートをさらに含む、請求項1に記載のアクセス装置。
- 前記アクセス装置の前記シャフト及びヘッドのサイズは、心臓に対する外科的処置、心臓処置、またはEP心臓カテーテル法実験室処置の間に前記人体の皮膚及び心膜における2センチメートル(cm)以下の開口部を通して挿入するように取られている、請求項1に記載のアクセス装置。
- 少なくとも1つの平面における前記アクセス装置の前記ヘッドの関節運動の手動制御を可能にするように構成された制御装置を含むハンドル部をさらに含み、前記関節運動は、前記ヘッドが少なくとも1つの方向に偏向すること含む、請求項1に記載のアクセス装置。
- 人体内の領域に器具をガイドするためのアクセス装置であって、前記アクセス装置は、
近位端及び遠位端を含むシャフトであって、前記近位端と前記遠位端との間で長手方向に延びるチャネルを含む内腔を画定し、前記チャネルは器具チャネル及びガイドワイヤチャネルを含む、前記シャフトと、
前記シャフトの前記遠位端に取り付けられたヘッドであって、最上表面と、前記最上表面と反対側の最下表面と、前記器具チャネルを通して延びた器具が前記人体の前記領域と相互に作用できるように構成されたポートとを含む前記ヘッドと、
前記ヘッドに取り付けられたカメラであって、前記シャフトを通して延びるカメラケーブルに可換に結合され、前記人体の前記領域の画像を取り込むように向けられた前記カメラと、
前記ヘッドの前記最上表面に取り付けられたヒンジ留めシェルであって、前記ヘッドの前記最下表面に隣接して延びることなく、前記ヘッドの前記最上表面に隣接して上方に延びるように構成された前記ヒンジ留めシェルと、を含む、前記アクセス装置。 - 前記ヒンジ留めシェルからヒンジ留めシェル制御装置まで延びるプルワイヤをさらに含み、前記ヒンジ留めシェル制御装置は、前記プルワイヤを引いて前記ヒンジ留めシェルの遠位端を前記ヘッドから離れるように延ばすように構成されている、請求項15に記載のアクセス装置。
- 前記ヒンジ留めシェル制御装置は前記アクセス装置のハンドル部上の親指トグルである、請求項16に記載のアクセス装置。
- 前記ヒンジ留めシェルは指の爪形状を有する、請求項15に記載のアクセス装置。
- 前記ヒンジ留めシェルの先端は前記ヘッドの前記遠位端を越えて遠位に延びる、請求項15に記載のアクセス装置。
- 前記カメラは前記ヒンジ留めシェル上に取り付けられている、請求項15に記載のアクセス装置。
- 前記アクセス装置は、前記カメラに対する照明を与えるように構成された光ファイバケーブルを含む、請求項15に記載のアクセス装置。
- 前記光ファイバケーブルは、前記アクセス装置から軸方向に延びる照明を与え、一方で、前記カメラは、前記人体の前記領域の前記ヒンジ留めシェルから角度が付いた視野を与える、請求項21に記載のアクセス装置。
- 人体内の領域に器具をガイドするためのアクセス装置であって、前記アクセス装置は、
近位端及び遠位端を含むシャフトであって、前記近位端と前記遠位端との間で長手方向に延びるチャネルを含む内腔を画定し、前記チャネルは器具チャネル及びガイドワイヤチャネルを含む、前記シャフトと、
前記シャフトの前記遠位端に取り付けられたヘッドであって、最上表面と、前記最上表面と反対側の最下表面と、前記器具チャネルを通して延びた器具が前記人体の前記領域と相互に作用できるように構成されたポートとを含む前記ヘッドと、
前記ヘッドに取り付けられたカメラであって、前記シャフトを通して延びるカメラケーブルに可換に結合され、前記人体の前記領域の画像を取り込むように向けられた前記カメラと、
前記ヘッドに配置された超音波トランスデューサであって、前記人体の前記領域における組織厚さまたは密度を決定するように構成された前記超音波トランスデューサと、を含む、前記アクセス装置。 - 前記超音波トランスデューサは、前記シャフトを通して診断プローブチャネルを通して挿入されるプローブを用いて位置付けられる、請求項23に記載のアクセス装置。
- 前記超音波トランスデューサは前記ヘッドに取り付けられ、前記シャフトを通して延びる診断ケーブルに可換に結合されている、請求項23に記載のアクセス装置。
- 前記アクセス装置はさらに、前記ヘッドに取り付けられてバルーン膨張チャネルと流体連絡するバルーンを含む、請求項23に記載のアクセス装置。
- 前記アクセス装置はさらに、前記ヘッドの前記最上表面に取り付けられたヒンジ留めシェルであって、前記ヘッドの前記最下表面に隣接して延びることなく、前記ヘッドの前記最上表面に隣接して上方に延びるように構成された前記ヒンジ留めシェルを含む、請求項23に記載のアクセス装置。
- 前記チャネルはカメラフラッシュチャネルをさらに含み、前記ヘッドは、前記カメラのカメラレンズに渡って流体を送るように構成されたカメラフラッシュポートをさらに含む、請求項1に記載のアクセス装置。
- アクセス装置を用いて、人体内の心臓に隣接した心膜スペース内で処置を行うための方法であって、
前記人体の皮膚における2センチメートル(cm)以下の剣状突起下切開を通して、または経皮針を介して、前記心膜スペース内にガイドワイヤを挿入することと、
前記ガイドワイヤを用いて前記アクセス装置のヘッドを前記心膜スペース内に挿入することであって、前記アクセス装置は、近位端及び遠位端を含むシャフトを含み、前記シャフトは、前記人体の外側の前記近位端と前記心膜スペース内で前記ヘッドに取り付けられた前記遠位端との間で長手方向に延びるチャネルを含む内腔を画定する、前記挿入することと、
前記心臓の処置すべき領域を、前記ヘッドに配置されて組織厚さまたは密度を決定するように構成された超音波トランスデューサを用いて決定した前記心臓のマッピングに基づいて決定することと、
前記アクセス装置の前記ヘッドを前記心臓の処置すべき前記部分に隣接して配置することと、
前記アクセス装置の一部を前記アクセス装置の最上表面から離れるように延ばして、前記心臓の処置すべき前記部分を前記心膜の周囲部分から離すことと、
前記アクセス装置の前記ヘッドに隣接する前記心臓の処置すべき前記部分の画像を観察することであって、前記心臓の処置すべき前記部分の前記画像は、前記アクセス装置の前記ヘッドにおけるカメラによって取り込まれる、前記観察することと、
前記心臓の処置すべき前記部分における心臓組織を処置することと、を含む、前記方法。 - 前記心臓組織を処置することは、前記アクセス装置の前記シャフト内の前記チャネルの1つを通して挿入された電気生理学(EP)アブレーションカテーテルを用いて、前記心臓組織を切除することを含む、請求項29に記載の方法。
- 前記心臓組織を処置することは、前記アクセス装置の前記シャフト内の前記チャネルの1つを通して挿入された注射カテーテルを用いて、前記心臓の前記部分に隣接してまたはその部分内に物質を注射することを含む、請求項29に記載の方法。
- 前記心臓組織を処置することは、前記アクセス装置の前記シャフト内の前記チャネルの1つを通して挿入され、前記心臓組織の表面とある角度をなして位置する注射カテーテルを用いて、前記心臓の前記部分に隣接してまたはその部分内に物質を注射することを含み、前記角度は約45度である、請求項31に記載の方法。
- 前記心臓組織を処置することは、処置カテーテルを関節運動させて、前記処置カテーテルの先端を前記心臓組織に対して正確に位置決めすることを含む、請求項29に記載の方法。
- 前記心膜スペース内の前記アクセス装置を移動させることを、前記アクセス装置の前記部分を前記アクセス装置の前記最上表面から離れるように後退させることと、前記心膜スペース内の前記アクセス装置を移動させることと、前記アクセス装置の前記部分を前記アクセス装置の前記最上表面から離れるように再び延ばすことと、によって行うことをさらに含む、請求項29に記載の方法。
