本発明は、抗プログラム細胞死-1(PD-1)抗体および抗細胞傷害性Tリンパ球抗原-4(CTLA-4)抗体の組合せを対象に投与することを含む、該対象における肺癌の処置法に関する。
ヒト癌は、免疫系によって認識され得る可能性のある新抗原(neoantigen)を生じる、多くのジェネティック変異およびエピジェネティック変異を示す(Sjoblom et al. (2006) Science 314:268-74)。Tリンパ球およびBリンパ球からなる獲得免疫系は、多様な腫瘍抗原に応答する広範な能力および優れた特異性を有する強力な抗癌剤としての可能性を有する。さらに、免疫系はかなりの可塑性および記憶的要素を示す。獲得免疫系のこれらの全ての特性をうまく利用することは、すべての癌処置法の中で免疫療法を比類のないものとするであろう。
本発明は、肺癌に罹患している対象を処置する方法であって、該対象に以下の組合せを投与することを含む方法に関する:(a)プログラムされた細胞死-1(PD-1)受容体に特異的に結合し、PD-1活性を阻害する抗体またはその抗原結合部分(ここで、PD-1抗体またはその抗原結合部分は、約2週間毎に1回(Q2W)、体重1kgあたり約0.1mgから約5.0mgの範囲の用量で投与される);および、(b)細胞傷害性Tリンパ球抗原-4(CTLA-4)に特異的に結合し、CTLA-4活性を阻害する抗体またはその抗原結合部分(ここで、CTLA-4抗体またはその抗原結合部分は、約6週間毎に1回(Q6W)または12週間毎に1回(Q12W)、体重1kgあたり約1mgから約5.0mgの範囲の用量で投与される)。ある態様において、肺癌は、非小細胞肺癌(NSCLC)である。他の態様において、NSCLCは、扁平上皮組織像を有する。さらに他の態様において、NSCLCは、非扁平上皮組織像を有する。

高親和性でPD-1に特異的に結合するHuMAbは、米国特許第8,008,449号に記載されている。他の抗PD-1モノクローナル抗体は、例えば、米国特許第6,808,710号、同第7,488,802号、同第8,168,757号および同第8,354,509号、ならびにPCT公開番号WO2012/145493に記載されている。米国特許第8,008,449号に記載された抗PD-1 HuMAbはそれぞれ、以下の特性の1以上を示すことが実証されている:(a)Biacoreバイオセンサーシステムを用いた表面プラズモン共鳴によって決定されるように、1×10-7M以下のKDでヒトPD-1に結合する;(b)ヒトCD28、CTLA-4またはICOSに実質的に結合しない;(c)混合リンパ球反応(MLR)アッセイにおいてT細胞増殖を増加させる;(d)MLRアッセイにおいてインターフェロン-γ産生を増加させる;(e)MLRアッセイにおいてIL-2分泌を増加させる;(f)ヒトPD-1およびカニクイザルPD-1に結合する;(g)PD-L1および/またはPD-L2のPD-1への結合を阻害する;(h)抗原特異的記憶応答を刺激する;(i)抗体応答を刺激する;ならびに、(j)インビボでの腫瘍細胞増殖を阻害する。本発明において使用可能な抗PD-1抗体には、ヒトPD-1に特異的に結合し、上記の特性の、少なくとも1つ、いくつかの態様において、少なくとも5つを示すモノクローナル抗体が含まれる。いくつかの態様において、抗PD-1抗体はニボルマブである。一態様において、抗PD-1抗体はペムブロリズマブである。
本発明の抗CTLA-4抗体は、CTLA-4とヒトB7受容体との相互作用を阻止するようにヒトCTLA-4に結合する。CTLA-4とB7との相互作用は、CTLA-4受容体を有するT細胞の不活性化に繋がるシグナルを伝達するため、相互作用の阻止は、そのようなT細胞の活性化を効果的に誘導、増強または延長し、それ故に、免疫応答を誘導、増強または延長する。

標的化療法の複数の選択療法で進行した患者のための有効な薬剤、ならびに現在の標準療法を超えてより長い期間生存を延長する療法が明らかに必要とされている。免疫療法を伴う新規のアプローチ、特にCTLA-4、PD-1およびPD-L1阻害経路を含む免疫チェックポイントの阻害は、最近、効果を発揮している(Creelan et al. (2014) Cancer Control 21(1):80-89)。従って、化学療法と組み合わせたイピリムマブは、小細胞肺癌および非小細胞肺癌の両方において有望な結果が示されている。加えて、抗PD-1および抗CTLA-4を組み合わせたものなどの二重免疫チェックポイント阻害戦略(dual checkpoint blockade strategy)は、黒色腫の処置において高度に有効であることが証明されており(Wolchok et al. (2013) N Engl J Med 369(2):122-33; WO 2013/173223)、抗PD-L1、抗LAG-3または抗KIRを含む他の組合せ剤が、腫瘍応答率および持続性を増加するために試験されている。黒色腫と同様に、NSCLC患者は、異なる免疫療法薬の組合せあるいはそのような薬剤と標的薬剤または外科手術、放射線、標準的な癌化学療法もしくはワクチンを含む他の療法との組み合わせのいずれかで利益を得ることができる。ニボルマブおよびイピリムマブの組合せ剤は、黒色腫を、副作用を管理しながら処置するのに極めて有効であることが証明されており(Wolchok et al. 本発明の範囲内にはまた、治療用途のための抗PD-1抗体および別の抗癌剤を含むキットもある。キットは、一般的に、キットの内容物の意図された使用を示すラベルおよび使用説明書を含む。ラベルという用語には、キットに添付されているまたは含まれる、あるいはそれ以外の方法でキットに包含される、文書または記録された資料が含まれる。従って、本発明は、肺癌に罹患している対象の処置のためのキットを提供し、該キットには、(a)約4mgから約500mgの範囲の量のPD-1抗体またはその抗原結合部分;(b)約40mgから約500mgの範囲の量のCTLA-4抗体またはその抗原結合部分;ならびに、(c)本明細書に記載の方法における、PD-1抗体またはその抗原結合部分およびCTLA-4抗体またはその抗原結合部分の使用のための指示書が含まれる。いくつかの態様において、キットは、PD-1抗体またはその抗原結合部分およびCTLA-4抗体またはその抗原結合部分を別個の組成物として含む。いくつかの態様において、キットは、PD-1抗体またはその抗原結合部分およびCTLA-4抗体またはその抗原結合部分を単一の組成物として含む。ある態様において、抗PD-1抗体および抗CTLA-4抗体は、単位投与量形態で共パッケージされ得る。ヒト患者を処置するための特定の態様において、キットは、本明細書に記載の抗ヒトPD-1抗体、例えばニボルマブまたはペムブロリズマブを含む。他の態様において、キットは、本明細書に記載の抗ヒトCTLA-4抗体、例えばイピリムマブまたはトレメリムマブを含む。
ニボルマブおよびイピリムマブでのNSCLCの処置
試験デザイン
ニボルマブ(“nivo”)およびイピリムマブ(“ipi”)の投与量および投薬スケジュールを試験するために、4つの異なる群を作成した。これらの群は、nivo+ipiの併用療法の初期試験とは異なる投与スケジュールを試験するように設計された。従前の併用療法の結果の概要を表1に示す。NR=達成されていない。
四捨五入により、一部が100%にならない場合がある。PD-L1発現率は、既知のPD-L1症状を有する患者に基づいている(nivo 1+ipi 1 Q3W、n=25;nivo 1 Q2W+ipi 1 Q6W、n=28;nivo 3 Q2W+ipi 1 Q12W、n=31;nivo 3 Q2W+ipi Q6W、n=30)。
NC=測定していない。処置に関連する有害事象は、初期ニボルマブ+イピリムマブコホートと比較して、新しい投薬スケジュールではより低頻度かつ低重篤度であった(表1参照)。新しい投薬スケジュールは、初期ニボルマブ+イピリムマブコホートと比較して、中止に繋がる処置に関連する有害事象が低頻度であることと関係していた(表3)。中止率は、ニボルマブ単剤療法と同程度あった(表1参照)。処置に関連する死亡はなかった。
NRは、奏効持続(ongoing response)の割合が高いか、または事象および/もしくはフォローアップの数が不十分であるため(達成されていない)。記号+は、打ち切り値を示す。これらの値には、nivo 1mg/kg Q2W + ipi 1mg/kg Q6W、nivo 3mg/kg Q2W + ipi 1mg/kg Q12W またはnivo 3mg/kg Q2W + ipi 1mg/kg Q6Wで処置された非扁平組織像を有する患者が含まれる。DCRには、確定された完全奏効(CR)、部分奏効(PR)および病勢安定(SD)を有する患者が含まれる。記号+は、打ち切り値を示す。
3週間毎に1mg/kgのニボルマブおよび3mg/kgのイピリムマブまたは3mg/kgのニボルマブおよび1mg/kgのイピリムマブを用いた初期の試験は臨床活性を示したが、その投薬スケジュールは毒性と関連があった。この試験では、新しい投薬スケジュールは、予期されない相乗的な活性および増加した臨床活性、ならびに許容される安全性を示した。ニボルマブ+イピリムマブを用いる第一選択療法は、進行NSCLC患者における深く持続的な奏効により特徴付けられる高レベルの臨床活性を示す。さらに、ニボルマブ+イピリムマブの併用処置は、良好な安全性プロファイルと関連している。中止につながる処置に関連するグレード3から4の有害事象の頻度は低く、処置に関連した死亡はなかった。
Nivo 3 Q2W + Ipi 1 Q6Wを受容した1名の患者にける病理学的CRの症例
転移性大細胞肺癌を有する54歳男性(元喫煙者、喫煙指数52パックイヤー)(PD-L1<1%;患者は、データベースロックの時点で分析時に部分奏効および未知のPD-L1発現を有するとして包含された)を、Nivo 3 Q2W + Ipi 1 Q6Wで処置した。患者は、RECISTによる53%の腫瘍サイズの縮小と、遠隔転移のない肺および縦隔リンパ節における放射線学的残存病変を有していた。図7A-7Dを参照のこと。
E1.以下の組合せを対象に投与することを含む、肺癌に罹患している対象を処置する方法であって、
(a)プログラムされた細胞死-1(PD-1)受容体に特異的に結合し、PD-1活性を阻害する抗体またはその抗原結合部分(ここで、抗PD-1抗体またはその抗原結合部分は、約2週間毎に1回、体重1kg当たり約0.1から約5.0mgの範囲の用量で投与される);および
(b)細胞傷害性Tリンパ球抗原-4(CTLA-4)に特異的に結合し、CTLA-4活性を阻害する抗体またはその抗原結合部分(ここで、抗CTLA-4抗体またはその抗原結合部分は、約6~12週間に1回、体重1kg当たり約1から約5.0mgの範囲の用量で投与される)
方法。
(a)約4mgから約500mgの範囲の量のPD-1抗体またはその抗原結合部分;
(b)約40mgから約500mgの範囲の量のCTLA-4抗体またはその抗原結合部分;ならびに、
(c)E1-E38のいずれかに記載の方法における、PD-1抗体またはその抗原結合部分およびCTLA-4抗体またはその抗原結合部分の使用のための指示書
を含むキット。
Claims (15)
- 肺癌に罹患している対象の処置において使用するための、プログラムされた細胞死-1(PD-1)受容体に特異的に結合し、PD-1活性を阻害する抗体またはその抗原結合部分(抗PD-1抗体またはその抗原結合部分)ならびに細胞傷害性Tリンパ球抗原-4(CTLA-4)に特異的に結合し、CTLA-4活性を阻害する抗体またはその抗原結合部分(抗CTLA-4抗体またはその抗原結合部分)を組み合せて含む組成物であって、
ここで、該抗PD-1抗体またはその抗原結合部分が、約2週間毎に1回、体重1kg当たり約0.1~5.0mgの範囲の用量で投与され;そして、
抗CTLA-4抗体またはその抗原結合部分が、約6~12週間に1回、体重1kg当たり約1~5.0mgの範囲の用量で投与される、組成物。 - 肺癌が非小細胞肺癌(NSCLC)である、請求項1に記載の組成物。
- 抗PD-1抗体またはその抗原結合部分が、ヒトPD-1への結合でニボルマブと交差競合する、請求項1または2に記載の組成物。
- 抗CTLA-4抗体またはその抗原結合部分が、ヒトCTLA-4への結合でイピリムマブと交差競合する、請求項1から3のいずれか一項に記載の組成物。
- (i)抗PD-1抗体がニボルマブであるか;
(ii)抗CTLA-4抗体がイピリムマブであるか;または
(iii)抗PD-1抗体がニボルマブであり、かつ抗CTLA-4抗体がイピリムマブである、請求項1から4のいずれか一項に記載の組成物。 - 抗PD-1抗体またはその抗原結合部分が、約2週間毎に1回、体重1kg当たり約1mgまたは約3mgの用量で投与される、請求項1から5のいずれか一項に記載の組成物。
- 抗CTLA-4抗体またはその抗原結合部分が、体重1kgあたり約1mgの用量で投与される、請求項1から6のいずれか一項に記載の組成物。
- 抗PD-1抗体またはその抗原結合部分が、約2週間毎に1回、体重1kgあたり約3mgの用量で投与され、かつ抗CTLA-4抗体またはその抗原結合部分が、約12週間毎に1回、体重1kgあたり約1mgの用量で投与される、請求項1から7のいずれか一項に記載の組成物。
- 抗PD-1抗体またはその抗原結合部分が、約2週間毎に1回、体重1kgあたり約1mgの用量で投与され、かつ抗CTLA-4抗体またはその抗原結合部分が、約6週間毎に1回、体重1kgあたり約1mgの用量で投与される、請求項1から7のいずれか一項に記載の組成物。
- 対象が、1%以上、5%以上、10%以上、25%以上、または50%以上のPD-L1発現率を有する肺癌を有する、請求項1から9のいずれか一項に記載の組成物。
- 臨床的利益が観察される限り、または病勢進行もしくは管理不可能な毒性が生じるまで投与される、請求項1から10のいずれか一項に記載の組成物。
- 抗PD-1抗体および抗CTLA-4抗体が、静脈内投与用に製剤される、請求項1から11のいずれか一項に記載の組成物。
- 抗PD-1抗体またはその抗原結合部分および抗CTLA-4抗体またはその抗原結合部分が、対象に連続して投与される、請求項1から12のいずれか一項に記載の組成物。
- 抗PD-1抗体またはその抗原結合部分および抗CTLA-4抗体またはその抗原結合部分が、別個の組成物として同時に投与されるか、または単一の組成物として同時に投与される、請求項1から12のいずれか一項に記載の組成物。
- (i)抗PD-1抗体またはその抗原結合部分が、治療用量以下で投与されるか;
(ii)抗CTLA-4抗体またはその抗原結合部分が、治療用量以下で投与されるか;または
(iii)抗PD-1抗体またはその抗原結合部分および抗CTLA-4抗体またはその抗原結合部分がそれぞれ、治療用量以下で投与される、
請求項1から14のいずれか一項に記載の組成物。
