Abstract
Description

出願者は、意外にも、HSCT後の癌の再発のリスクが特定の細菌群の存在及び/または不在に関連することを発見した。したがって、本発明は、対象の癌の再発の可能性を判定するための方法及び組成物、ならびにリスクにある患者における再発のリスクを減少させるための方法及び組成物に関する。ある特定の非限定的な実施形態では、本発明は、癌の再発のリスクが大きい、または低減すると判定された対象を治療する方法をさらに提供する。

4.図面の簡単な説明
本発明は、癌を有する対象が治療後、例えば、allo-HSCT後に癌の再発をきたすリスクを判定するための方法、ならびに癌の再発のリスクを低減するための方法及び組成物、ならびに対象が癌の再発を免れる可能性を増加させるための組成物及び方法にも関する。説明の明確化のためであり、制限するためではなく、本節は以下の小区分に分けられる:

方法
本実施例では、バイオマーカー発見アプローチが適用され、未修飾(T細胞豊富)同種移植片を受けた160人の成人の後向き観察分析が行われた。患者は、便生体試料収集プロトコルに予め登録された。この分析に関して、我々は、allo-HSCT後の最初の3週間の間に少なくとも1つの検体を提供した患者を選んだ。このコホートにおける原因疾患は、AML(37%)、非ホジキンリンパ腫(33%)、ALL(8%)、MDS(7%)、CLL(6%)、ホジキンリンパ腫(6%)、CML(2%)、及び骨髄増殖性腫瘍(2%)であった。患者の平均年齢は、52歳であった(21~75の範囲)。彼らは、切除(17%)、強度低減(64%)、及び非骨髄破壊(19%)レジメンで前処置された。彼らは、臍帯血(46%)、血縁関係がない成人(33%)、または血縁関係にある成人(22%)から移植片を受けた。成人移植片の中で、92%が末梢血からであり、8%が骨髄からであった。この患者群は、「患者フローラコホート」である。
Bone Marrow Transplant 21:1373-1383 2015)、16S rRNAディープシーケンシングのqPCRにより生成された。簡潔に、各便検体に関して、前述のプロトコルに基づき、機械的な破壊(ビーズ破砕)によるフェノール-クロロホルム抽出技法を使用してDNAを精製した。454 GS FLX Titaniumプラットホーム(454 Life Sciences,Branford,CT)を使用して、細菌16S rRNA遺伝子のV1~V3領域を配列決定することにより、患者フローラコホートからの試料を分析した。
再発のリスクとの細菌の関連は、コックス単変量回帰により定量化された。分類群の中で、再発のリスクと最も顕著に関連したのはStreptococcus anginosus、Parvimonas micra、Eubacterium limosum、及びActinomycesを含むヒト口腔フローラのメンバーであった(図2)。中央存在量による患者の層別化後、我々は、この細菌の存在量が高いものは移植後の再発がより少ないことを見出した(p=0.0014)。細菌Parvimonas micraの存在量が高い患者も移植後の再発がより少なかった(図2、4A、4B、及び7)。Parvimonas micraは、歯垢において一般的に見出される口腔種である、グラム陽性の嫌気性球菌の「peptostreptococci」であり、TNF-α、IL-6、及びIL8のマクロファージ産生を刺激し、NF-kBの上流であるNOD2受容体を刺激し得る(Marchesan,Molecular Oral Microbiology 2015 Volume 31,Issue 3,June 2016,Pages 243-258)。Parvimonas micraもFc結合タンパク質を作製する。Parvimonas micraは、T細胞豊富な非臍帯移植片を受けた(図5及び7)、及び骨髄破壊前処置後(図6)の患者において再発の欠如と最も良く相関した。加えて、それぞれ、細菌Acidaminociccus intestini及びStrepticoccus anginosusの存在量が高い患者(図7)も再発率が低かった。我々は、Enterococcus faeciumなどの再発のリスクの増加に関連する細菌(p=0.0103)も特定した(図2及び7)。加えて、ある特定の細菌は、コックス単変量回帰により判定される、GVHD関連の死亡のリスクの低減に関連した(例えば、Akkermansia muciniphila及びPhascolarctobacterium)か、またはGVHD関連の死亡のリスクの増加に関連した(例えば、Eubacterium biforme及びDysgonomonas)(図8)。
Jun 5;123(23):3664-71)、前処置強度、及び移植片源(臍帯血対成人ドナー)を調整した多変量コックスモデルにおいて、バイオマーカーとして評価され、図3は、細菌存在量がこれらの調整がなされた後の再発を予測することを示す。
方法
同種造血幹細胞移植(allo-HSCT)を受けた613人の成人癌患者の2,391の毎週の便試料を3週間にわたって収集し、実施例1によって記載されるように分析し、試料中に存在する細菌のレベルを判定した。この実施例では、細菌16S rRNA遺伝子のV4~V5領域が配列決定された。allo-HSCT後0~21日目から試料を収集した。147人の患者が本試験から除外されたため、466人の患者の最終コホートが分析された。466人の患者コホートの特徴は表2に提供される。コホートは、allo-HSCT後36ヶ月以内に26%の癌の再発率を呈した(図9)。
これらの実験からのデータは、腸管フローラの多様性がallo-HSCT後の全体的な生存を予測するが、再発を予測しないという発見をさらに支持する。経時的な細菌存在量の曲線下面積が各細菌分類群に対する各患者の累積曝露の基準として使用された。2,018の操作的分類単位(OTU)が全試料にわたって特定された。患者の>10%において、>0.01%の存在量で存在するOTUのみが考慮された(図10)。患者は、再発事象が等しい分布で発見及び検証組(n=232及びn=234)に分けられた。コックス回帰モデルを使用して、発見組における再発との関連に関して、194OTUの存在量-AUCを評価した(図11)。3.4×10-5のカットオフ閾値は、p値を最小にするために、発見組において系統的に選択され(Camp,Clin Cancer Res 2004)、これは、次いで、検証組に適用された。このカットオフにより、約85%が細菌の存在量が低く(再発と相関した)、15%が存在量が高い(再発の欠如と相関した)結果となった。Acidaminococcus intestiniの存在量は、発見及び検証組の両方においてより少ない再発に関連すると特定された(図12A及び12B)。
方法
同種造血幹細胞移植(allo-HSCT)を受けた541人の成人癌患者の便試料を収集し、16Sリボソームシーケンシングを使用して配列決定した。患者は、便生体試料収集プロトコルに予め登録された。各患者は、allo-HSCT後の最初の21日以内に収集された、配列決定された試料を有した。541人の患者コホートの特徴は表3に提供される。2年の分析期間の間に、138の再発/POD事象(発生率25.5%)があった。
(US);2014を参照されたい)。各患者のOTUの時間加重平均存在量は、前述のもの(Falcao et al.,Gynecol Oncol 2005;97:529-34、Oudard et al.,J Clin Oncol 2004;22:9579)に類似する測定基準を使用して、台形法(図18A)を使用して計算された。患者のうちの420人(77.6%)に関して、少なくとも1つの追加の試料が試料採取期間前の週及び/またはその後の週から入手可能であった(すなわち、647の隣接試料)。これらの患者に関して、隣接試料は、試料採取期間の境界に存在量ベクターを補間するために使用された(図18B)。単一試料の移植前分析では、患者当たり2つ以上の試料が入手可能であった場合、-10日目に最も近くに収集された試料が分析に選択された。研究を通して、存在量は、単一移植前試料の分析において示される場合を除き、3週間の移植後試料採取期間にわたるログ変換時間加重平均を指す。微生物叢の特徴の存在または不在は2値変数として分析され、任意の存在量のカットオフは>0であった。
研究された主要な転帰は、疾患特異的基準による疾患の再発または進行(POD)までの時間であった。微小残存病変の検出は、フローサイトメトリー、X線検査、または分子結果が療法の開始、ドナーリンパ球の注入、または免疫抑制療法からの離脱により臨床的に作用したとき、再発/POD事象としてスコア化された。
et al.Biol Blood Marrow Transplant 2007;13:1469-76を参照されたい)。研究を通して、ランドマーク分析は、微生物叢の試料採取期間の終了から事象開始までの時間を考慮するために適用され、指定日の21日目前に転帰を有する患者は、その転帰の分析から除外された。統計分析はRソフトウェアを使用して行われた。(R Core Team,R:A Language and Environment for Statistical Computing.Vienna,Australia:R Foundation for Statistical Computing;2015を参照されたい)。
経験的に定義された関連する細菌(crOTU)群は、個々の種を表す操作的分類単位(OTU)よりも臨床転帰と強い関連を有した。主にEubacterium limosumから構成されたcrOTUの存在は、多変量モデルにおいてより少ない再発に関連した(HR 0.54、CI 0.38~0.78、p=0.001)。この関連は、T細胞豊富同種移植片のレシピエントの間で最も明らかであった。
逆シンプソン指数により評価される腸管微生物叢の多様性は、前の観察を踏まえて、再発/PODまでの時間に関連しなかった(p=0.16)(図19を参照されたい)。(Taur et al.,Blood 2014;124:1174-82を参照されたい)。
発見組における存在量と再発/PODまでの時間との間の関連は、208のOTU及び1,343のcrOTUに関して評価された。図21は、最低の多変量p値を有する10のcrOTUを提供する。各crOTUに関して、最存在量の種、ならびに隣接するcrOTUからcrOTUを区別する希少種を列記する。括弧付きの数字は特定のcrOTUと関連する、指名される種のOTUの数を示す。次に、多変量モデルは、RDRI、前処置強度、及び移植片源に関して調整された。単変量(図20A及び20C)または多変量(図20B及び20D)のいずれかのコックスモデルにおけるp≦0.01の基準は、再発/PODまでの時間に関連する有力な候補を特定するために使用された。発見組において再発/PODリスクに最も密接に関連する候補は、主にEubacterium limosumならびに他の関連種から構成されたクラスターであるcrOTU1614(多変量HR 0.84、CI 0.73~0.96、p=0.01)であった(図21)。
発見組において特定された最良の候補crOTUは、検証組において、転帰とのその関連の再現性に関して評価された。crOTU1614の存在量は、検証組において、再発/PODのリスクの低減に有意に関連した(HR=0.82、CI 0.71~0.95、p=0.009、図22C)。この関連は、前処置強度、移植片源、及びRDRIを多変量調整した後、有意なままであった(HR=0.82、CI=0.70~0.96、p=0.01、図22C)。
発見及び検証組は、さらなる探索的分析のために統合された。crOTU1614は、541人の患者のうち422人(78%)に存在し、平均存在量は0.16%であり、最大存在量は8%であった。再発/PODの漸進的なリスクの低下は、コホートが4つの存在量ビンに階層化されたとき、コホートにわたって観察され(p=0.001、図26A及び26B)、この関連は、多変量調整後、有意なままであった(p=0.004、図22B)。存在量は、用量依存様式でより少ない再発/PODに関連した(図26A)。
crOTU1614の腸管の存在は、全体的な生存の増加(HR=0.65、CI=0.47~0.90、p=0.008、図29)、及び再発/PODの累積発生率の減少に関連した(図29)。crOTUは、急性GVHD(グレード2~4、HR=0.81、CI=0.56~1.17、p=0.27)、または移植関連の死亡(HR=1.0、CI=0.63~1.59、p=0.99、図29)に有意に関連しなかった。研究中の集団の不均一性を考慮して、患者におけるcrOTU1614の再発/PODとの関連を、移植源、前処置強度、HLA適合の程度、RDRI、及び疾患の種類により細分する(図30)。
上記に示すように、再発のリスクのバイオマーカーは、HSCTで治療された患者の評価及び治療に有用である。かかるバイオマーカーは、移植前に再発のリスクに対する情報を提供する場合、例えば、何の治療レジメンを患者に使用するかの決定過程の一部としての追加の有用性を有する。crOTU1614の関連が最も明らかである未修飾PBSC/BM(T細胞豊富)移植片の172人のレシピエント中、143人の患者(83%)も幹細胞注入3週間前に試料を収集した。これらの患者は、allo-HCT前の3週間の間に収集された単一便試料中のcrOTU1614の存在量に基づき、4つのビンに階層化された。2つ以上の試料がallo-HCTの3週間前に入手可能であった場合、-10日目に最も近くに収集された試料が分析に選択された。最も高い存在量ビンの患者は、統合した3つの低存在量群と比較して、再発/PODのリスクが低かった(HR=0.28、CI=0.10~0.80、p=0.02、図32)。移植前試料における再発/PODのリスクの減少とcrOTU1614との間で類似するが、あまり統計的に有意ではない関連も、移植前試料が入手可能であった全ての種類の移植源の469人のレシピエントにおいて観察された(図33、HR=0.63、CI=0.40~1.09、p=0.06)。よって、allo-HCTの前またはその後のいずれかのcrOTU1614またはcrOTU1614生物のサブセットの腸管の存在は、移植後の再発/PODリスクのバイオマーカーとして使用することができる。図34は、図26B、26C、32、及び33に描写されるように、試験全体を通して使用された存在量ビンのカテゴリーの境界を示す。
この後向き観察の単一施設研究において、現時点までのこの種類の分析のために集められた最大規模のallo-HCT患者のコホートに関して、腸管微生物叢の組成が研究された。発見-検証アプローチを使用して、腸管細菌の特定のサブセットの存在量とallo-HCT後の再発/PODのリスクの減少との間の関連を特定した(図23)。
腸管微生物叢のある特定のメンバーと同種造血幹細胞移植(例えば、T細胞が移植源に存在するとき)後の再発のリスクの減少との間の関係は、実施例1~3によって記載される。本実施例は、腸管フローラ中の該細菌が移植片対腫瘍(GVT)及び移植片対白血病(GVL)活性を増強し得ることを判定する方法を記載する。かかるモデルにおいて、GVTまたはGVLは、抗生物質によって、ならびに該細菌、例えば、Eubacterium limosum及び/またはParvimonas micraの腸管微生物叢内への再導入によっても増強される。
1.マウスモデルにおけるGVT活性に対する抗生物質治療の作用
GVTの1つのモデルにおいて、シフェラーゼ発現腫瘍細胞、及びT細胞を伴う、または伴わないT細胞除去不適合骨髄(BM)をマウスに注射した後、腫瘍負荷が生物発光撮像により系列的に監視され得る。2~4週間で動物の50%に致死である用量を定義するために、ルシフェラーゼ発現構築物を保有するマウスB細胞リンパ腫A20及びマウスT細胞リンパ腫EL4の誘導体を、限界段階希釈でマウスに静脈注射する。次に、T細胞を伴う、または伴わないT細胞除去不適合骨髄(BM)と共に、この細胞濃度でマウスに注射し、次に、2つの異なるレジメン(広域スペクトル腸内除染または規定スペクトル)のうちの1つに従い抗生物質で治療した。抗生物質での治療後、GVTを、インビボ生体発光撮像、生存分析、及び組織学により評価する。マウスの腸内フローラは2つの異なる方法で操作される。1つ目は、経口バンコマイシン及びアンピシリンでの腸内広域スペクトル除染である。2つ目は、規定スペクトルを有する薬物を用いることにより、フローラの組成を変更することである。Eubacterium limosum及びParvimonas micraの両方は嫌気性であるため、アズトレオナム(嫌気活性を欠く)の作用がイミペネル(強力な嫌気活性を有する)と比較される。
標準的な実験モデルの欠点の1つは、腫瘍細胞系の臨床的相関が患者におけるそれらとGVT反応との間の重要な相違により限定されることである。代替えとして、移植片対白血病(GVL)の実験モデルは、混合系列白血病(MLL)関連急性骨髄性白血病(AML)のために開発された。MLL遺伝子を伴う再配列は、それらが療法関連AMLに非常に関係があり、臨床予後不良をもち、同種移植の適応であるため、移植研究に特に関連がある。(DiMartino et al.,British Journal of Haematology 106,614-626(1999))。腸内除染抗生物質での治療後、AMLは、一般的なMLL転座を繰り返すMLL-AF9融合構築物でレトロウイルスによりトランスフェクトされたGFP発現骨髄細胞をマウスに注射し(Stubbs et al.,Leukemia 22,66-77(2007)及びKrivtsov et al.,Nature 442,818-822(2006))、続いてT細胞を伴う、または伴わないT細胞除去BMを不適合移植することによって誘導される。高リスク二次性白血病のこのモデルは、規定された遺伝的欠損により新しく発生させた白血病を利用し、インビトロ継代腫瘍細胞系よりも近い臨床疾患を反映する。腫瘍の進行は、移植の転帰、ならびに末梢血、脾臓、及び骨髄における白血病を示すGFP蛍光により監視される。

前記組成物が、前記対象におけるEnterococcus faeciumの存在を減少させる、及び/または前記対象におけるEnterococcus faecium毒素の量を減少させるのに有効な量で投与される、項目7または8のいずれか1項に記載の方法。

癌の再発のリスクにある対象を特定することをさらに含み、
対象から腸管微生物叢の試料を得、前記腸管微生物叢の試料中に存在する、項目1~3のいずれか1項に記載の1つ以上の細菌またはその芽胞のレベルを判定することと、
前記試料中の前記1つ以上の細菌またはその芽胞のレベルを参照細菌レベルと比較することと、
前記試料中の前記1つ以上の細菌またはその芽胞のレベルが前記細菌参照レベルよりも低いとき、前記対象に前記組成物を投与することと、を含む、項目7~9のいずれか1項に記載の方法。

癌の再発のリスクにある対象を特定することをさらに含み、
対象から腸管微生物叢の試料を得、前記腸管微生物叢の試料中に存在する、Enterococcus faecium細菌のレベルを判定することと、
前記試料中の前記細菌のレベルを参照細菌レベルと比較することと、
前記試料中の前記細菌のレベルが前記細菌参照レベルよりも大きいとき、前記対象に前記組成物を投与することと、を含む、項目7~9のいずれか1項に記載の方法。

癌の再発のリスクを有する対象を診断する方法であって、
対象から腸管微生物叢の試料を得、前記腸管微生物叢の試料中に存在する、項目1~3のいずれか1項に記載の1つ以上の細菌またはその芽胞のレベルを判定することと、
前記試料中の前記1つ以上の細菌またはその芽胞のレベルを参照細菌レベルと比較することと、
前記試料中の前記1つ以上の細菌またはその芽胞のレベルが前記細菌参照レベルよりも低いとき、癌の再発のリスクを有すると前記対象を診断することと、を含む、方法。

癌の再発のリスクを有する対象を診断する方法であって、
対象から腸管微生物叢の試料を得、前記腸管微生物叢の試料中に存在する、Enterococcus faecium細菌のレベルを判定することと、
前記試料中の前記細菌のレベルを参照細菌レベルと比較することと、
前記試料中の前記細菌のレベルが前記細菌参照レベルよりも大きいとき、癌の再発のリスクを有すると前記対象を診断することと、を含む、方法。
(項目14)
前記対象が、造血幹細胞移植(HSCT)を受けたことがあり、前記造血幹細胞移植が、任意選択で、同種造血幹細胞移植(allo-HSCT)であり、前記allo-HSCTが、任意選択で、T細胞豊富なallo-HSCTである、項目12または13に記載の方法。
(項目15)
癌の再発のリスクを有すると診断された前記対象に、癌療法を施すことをさらに含み、
前記癌療法が、任意選択で、手術;放射線療法;化学療法;免疫療法;幹細胞療法;細胞療法;プロバイオティック細菌、プロバイオティック酵母、もしくはこれらの組み合わせ;プレバイオティック;ポストバイオティック;抗生物質;またはこれらの組み合わせを含む、項目12~14のいずれか1項に記載の方法。
(項目16)
前記プロバイオティック細菌が、項目1~3のいずれか1項に記載の1つ以上の細菌またはその芽胞を含む、項目15に記載の方法。
(項目17)
項目1~6のいずれか1項に記載の組成物を含む、キット。
(項目18)
癌の再発のリスクを有すると対象を診断するためのキットであって、腸管微生物叢の試料中の、項目1~3のいずれか1項に記載の1つ以上の細菌またはその芽胞の存在量を検出するための、1つ以上の薬剤を含む、キット。
(項目19)
対象における癌の再発のリスクの低減、及び/または癌の再発からの生存の可能性の増加に使用するためのものであり、前記対象が、任意選択で、造血幹細胞移植(HSCT)を受けたことがあり、前記造血幹細胞移植が、任意選択で、同種造血幹細胞移植(allo-HSCT)であり、前記allo-HSCTが、任意選択で、T細胞豊富なallo-HSCTである、項目1~6のいずれか1項に記載の組成物。
(項目20)
前記組成物が、前記対象におけるEnterococcus faeciumの存在を減少させる、及び/または前記対象におけるEnterococcus faecium毒素の量を減少させるのに有効な量で投与可能である、項目19に記載の使用のための組成物。
(項目21)
前記対象が、癌の再発のリスクにあり、前記対象からの腸管微生物叢の試料が、細菌参照レベルよりも低い、項目1~3のいずれか1項に記載の1つ以上の細菌またはその芽胞のレベルを有するか、または
前記対象からの腸管微生物叢の試料が、細菌参照レベルよりも大きいEnterococcus faecium細菌のレベルを有する、項目19に記載の使用のための組成物。
(項目22)
対象からの腸管微生物叢の試料をアッセイし、前記腸管微生物叢の試料中に存在する、項目1~3のいずれか1項に記載の1つ以上の細菌またはその芽胞のレベルを判定することと、
前記試料中の前記1つ以上の細菌またはその芽胞のレベルが前記細菌参照レベルよりも低い場合、前記対象に治療有効量の前記組成物を投与することと、を含む、項目19に記載の使用のための組成物。
(項目23)
対象からの腸管微生物叢の試料をアッセイし、前記腸管微生物叢の試料中に存在する、Enterococcus faecium細菌のレベルを判定することと、
前記試料中のEnterococcus faecium細菌のレベルが前記細菌参照レベルよりも大きい場合、前記対象に治療有効量の前記組成物を投与することと、を含む、項目19に記載の使用のための組成物。
(項目24)
癌の再発のリスクを有する対象を診断するインビトロ方法であって、前記対象からの腸管微生物叢の試料において、細菌参照レベルよりも低い、項目1~3のいずれか1項に記載の1つ以上の細菌またはその芽胞のレベルを検出することを含むか、または
前記対象からの腸管微生物叢の試料において、細菌参照レベルよりも大きいEnterococcus faecium細菌のレベルを検出することを含む、インビトロ方法。
(項目25)
前記対象が、造血幹細胞移植(HSCT)を受けたことがあり、前記造血幹細胞移植が、任意選択で、同種造血幹細胞移植(allo-HSCT)であり、前記allo-HSCTが、任意選択で、T細胞豊富なallo-HSCTである、項目24に記載の方法。
(項目26)
対象における癌療法に使用するための組成物であって、前記癌療法が、手術、放射線療法、化学療法、免疫療法、幹細胞療法、及び/または細胞療法からなる群から選択され、
前記組成物が、任意選択で、プロバイオティック細菌、プロバイオティック酵母、もしくはこれらの組み合わせ;プレバイオティック;ポストバイオティック;抗生物質;またはこれらの組み合わせを含み、
前記対象が、癌の再発のリスクにあり、前記対象からの腸管微生物叢の試料が、細菌参照レベルよりも低い、項目1~3のいずれか1項に記載の1つ以上の細菌またはその芽胞のレベルを有するか、または
前記対象が、癌の再発のリスクにあり、前記対象からの腸管微生物叢の試料が、細菌参照レベルよりも大きいEnterococcus faecium細菌のレベルを有する、癌療法に使用するための組成物。
