[00155] 肺機能不全は、重度外傷性脳損傷の合併症として見つかることが多い(1)。TBI対象の約20〜25パーセントが急性肺損傷(ALI)を発症する(2)が、TBI誘導ALIの病変を媒介する機序の定義は不十分のままである。従来の文献は、TBI後の肺機能不全が、心肺機能不全をもたらす頭蓋内圧の増加に対する交感神経応答に起因するという考えを支持している(42)。しかしながら、より近年の研究は、全身炎症応答もTBI誘導肺損傷における重要な役割を担うことを示している(43)。具体的には、HMGB1−RAGEリガンド受容体経路がTBI後の肺機能不全についての中枢伝達機序として機能する(8)。さらに、HMGB1は、AIM2インフラマソーム活性化を誘導する(37)。さらなる従来の文献は、病原体がDAMP、例えば、HMGB1を担持するEVを分泌し、炎症をトリガーすることを明らかにしている(Buzas et al.,2014)。種々の研究は、血液脳関門(BBB)が損傷後3〜6時間ほど早くTBI後に浸透性になり、脳及び血管内コンパートメント間の保護障壁への損傷をもたらし、タンパク質及び体液の漏出をもたらすことを示している(44)。損傷後のBBBの破壊は、炎症性メディエーター、例えば、DAMPの分泌をもたらし、それが脳炎症を促進し、遠位器官を損傷し得る(5)。いくつかの炎症性メディエーターは、明確な脳損傷のマーカーとして作用し得るが、それらの妥当性は広く認められているわけではない(45)。さらに現在、TBI誘導ALIについての臨床的に承認された治療もバイオマーカーも存在しない。近年、EVは、いくつかの異なるタイプの疾患、例として、肺損傷(46)及びTBI(47)についてのバイオマーカー研究の関心領域になっている。TBIを有する患者の脳脊髄液から単離されたEVにおいて、対照試料と比較してインフラマソームタンパク質が増加することが既に示されている(14)。本実施例において、TBI誘導ALIの病因におけるEV媒介インフラマソームシグナリングの寄与を試験した。
動物及び外傷性脳損傷
[00156] 全ての動物手順は、University of Miami Miller School of MedicineのInstitutional Animal Care and Use Committee(Animal Welfare Assurance A3224-01)により承認され、NIH Guide for the Care and Use of Laboratory Animalsに従って行った。本試験を実施する場合、ARRIVE指針に従った。全てのC57/BL6マウスは、8〜12週齢及び24〜32グラムであった。マウスをTBIについての実験群(疑似、4時間、24)、養子移植及び処理についての実験群(ナイーブ、疑似生理食塩水、非処理、エノキサパリン、抗ASC)にプロスペクティブにランダム化した。TBI実験群について、疑似動物は外科手術を受けたが損傷されなかった。養子移植処理試験について、疑似生理食塩水群は外科手術を受け、ビヒクル処理として生理食塩水を受容した。ナイーブ動物は、外科手術を受けなかった。検出力分析(G*検出力分析、効果量F=0.85、α設定0.05を使用)及び履歴データに基づきそれぞれの群について5〜6の試料サイズを使用した49,50。全てのマウスをUniversity of MiamiにおけるLois Pope Life Centerにおけるウイルス抗原不含(VAF)動物施設において12時間の明暗サイクルで飼育し、飼料及び水を自由摂餌として供給した。施設は、畜産的手順を週2回実施し、動物の状態を毎日確認した。動物を術後観察し、それらを加熱パッド上で保持し、体温を直腸プローブにより制御し、手術室内で体温を37℃に維持し、次いで動物飼育室に移した。

重度TBIは、マウスの脳におけるAIM2インフラマソームタンパク質及びHMGB1発現を増加させる
[00168] 炎症促進性サイトカインIL−1β及びIL−18、並びにインフラマソームタンパク質の発現レベルは、流体パーカッション脳損傷後の二次損傷に関連する(18)。重度CCIが炎症促進性サイトカインのプロセシング及びインフラマソームタンパク質レベルの変更を誘導したか否かを決定するため、皮質溶解物を分析したが、重度TBIにおけるインフラマソーム活性化に対する研究が制限された。本実施例において、重度CCIの後、皮質溶解物をカスパーゼ−1(図1A、B)(p<.001)、ASC(図1A、C)(p=.003)、IL−18(図1A、D)(p=.0042)、AIM2(図1A、F)(p=0.0197)及びIL−1β(図1A、G)(p=0.0141)のレベルについて損傷の4及び24時間後において試験した。カスパーゼ−1、ASC、AIM2、及びIL−Iβのレベルは、CCIの4時間後においてピークに達し、24時間までに減少した。炎症性サイトカインの成熟の経時変化はわずかに異なったが、TBIの24時間後までにピークに達した。他者らがAIM2インフラマソームを活性化させるインフラマソームDAMP HMGB1についての役割を示しているため、それらのタンパク質のレベルも皮質溶解物において決定した。図1A、1Eに示されるとおり、CCIは、損傷の4及び24時間後においてHMGB1(図1A、1E)(p=0.0121)のレベルの有意な増加を誘導した。これらのデータは、マウスにおける重度CCIの後、AIM2インフラマソームタンパク質のレベルが損傷後の皮質において有意に上昇したことを示す。

EAEの誘発及びIC100による治療
