Abstract
Description
本発明に係る組成物は、局所適用に適した医薬として製剤化され、具体的には、皮膚への投与のために製剤化できる。
本発明により使用するための好適な組成物は、医薬組成物であって、
a.不飽和遊離脂肪酸と、
b.硬化剤と、
c.9:1から18:1までの間の質量比のロピナビルおよびリトナビルと
を含み、
不飽和遊離脂肪酸が、医薬組成物の総質量の少なくとも20質量%のレベルで存在し、医薬組成物が、室温で半固体である、医薬組成物である。
不飽和遊離脂肪酸は、オレイン酸、リノール酸、α−リノール酸、パルミトレイン酸、ゴンド酸、およびリシノール酸から選択してもよい。不飽和遊離脂肪酸は、好ましくは、オレイン酸である。
硬化剤は、組成物が室温で半固体となるように組成物を硬化させるために使用される賦形剤である。好適には、硬化剤は、飽和遊離脂肪酸、例えば、C10〜C38飽和遊離脂肪酸、例えば、C16〜C22遊離脂肪酸である。飽和遊離脂肪酸は、遊離脂肪酸であり(すなわち、脂肪酸は、別の分子、例えばグリセロールに結合していない。)、脂肪酸の炭素原子間に二重結合が存在しない。一実施形態では、硬化剤は、カプリン酸、ウンデシル酸、ラウリン酸、トリデシル酸、ミリスチン酸、ペンタデシル酸、パルミチン酸、マルガリン酸、ステアリン酸、ノナデシル酸、アラキジン酸、ヘンイコシル酸、ベヘン酸、トリコシル酸、リグノセリン酸、ペンタコシル酸(pentacosylic acid)、セロチン酸、ヘプタコシル酸(heptacosylic acid)、モンタン酸、ノナコシル酸(nonacosylic acid)、メリシン酸、ヘナトリアコンチル酸(henatriacontylic acid)、ラクセロン酸(lacceroic acid)、プシル酸(psyllic acid)、ゲジン酸(geddic acid)、セロプラスチン酸(ceroplastic acid)、ヘキサトリアコンチル酸(hexatriacontylic acid)、ヘプタトリアコンタン酸、およびオクタトリアコンタン酸から選択される。硬化剤は、好ましくは、ステアリン酸である。
本発明に係る組成物は、場合により、下記に記載するような粘膜付着剤およびその他の賦形剤を含んでもよい。
一実施形態では、皮膚への局所適用のための組成物であって、
a.リトナビルと、
b.ロピナビルと、
c.オレイン酸と、
d.ステアリン酸と
を含む皮膚への局所適用のための組成物が提供される。
e.リトナビルと、
f.ロピナビルと、
g.ヒドロキシプロピルメチルセルロースと、
h.オレイン酸と、
i.ステアリン酸と、
j.ブチル化ヒドロキシトルエンと
を含む局所適用のための組成物が提供される。
a.約1.2〜約1.4質量%のリトナビルと、
b.約9〜約11質量%のロピナビルと、
c.約0.5〜1.5質量%のヒドロキシプロピルメチルセルロースと、
d.約55〜約65質量%のオレイン酸と、
e.約28〜約32質量%のステアリン酸と、
f.約0.0.5〜約0.5質量%のブチル化ヒドロキシトルエンと
を含み、ここで、%はすべて、組成物の総質量に対する質量%である。
a.約0.5〜約0.7質量%のリトナビルと、
b.約4〜約6質量%のロピナビルと、
c.約0.5〜1.5質量%のヒドロキシプロピルメチルセルロースと、
d.約55〜約65質量%のオレイン酸と、
e.約28〜約32質量%のステアリン酸と、
f.約0.05〜約0.5質量%のブチル化ヒドロキシトルエンと

必要とされるロピナビルおよびリトナビルの量は、生物学的活性およびバイオアベイラビリティにより決定されるものの、これは、一部には、正確な投与様式、使用する組成物の物理化学的特性、および組成物が単剤療法として使用されるのか、または他の経口もしくは局所薬と共に、併用療法で使用されるのかによることを理解されたい。実際、少なくとも1つの医薬品有効成分を、同じ化合物または他の医薬品有効成分の経口投与に加えて、局所投与することも可能である。投与頻度もまた、上述の要因、特に、処置を受ける対象の体内での医薬品有効成分の半減期に影響される。
<総API濃度20μMで、ロピナビル:リトナビルの8:1〜16:1w/w比が、hTERT細胞に与える効果の評価>
N−TERT細胞は、p53の野生型であり、核型が正常な分化細胞に由来する不死化非形質転換細胞株である。これらは無期限に増殖し、さまざまな処理によって形質転換できる。したがって、これらの細胞は、ロピナビルおよびリトナビルが、非形質転換異常増殖前がん性病変であるAKに与える効果を評価するための良好なモデルである。これらの実験では、Dickson et al.(2000) (Mol Cell Biol Feb 2000 p1436−1447)に記載のように、新生児包皮ケラチノサイトに由来するN−TERT細胞を使用した。
(1.1.1 細胞培養)
N−TERT細胞は、ウシ下垂体抽出物(BPE)および上皮成長因子(EGF)を補充したケラチノサイト無血清培地(KSFM)中、5% CO2および37℃で、標準法により培養した。
T75のコンフルエントなスターター培養物からT25フラスコに播種した細胞で実験を実施し、これらは、5% CO2および37℃で5日間維持した。最初の3日間は、標準培養条件に従い、その後の2日間は、BPEを含むがEGFは含まないKSFMで細胞を成長させた。このステップは、細胞の成長周期を同調させ、本発明者らは、このステップが、その後のアッセイの性能および感度を向上させることに気付いた。
5日目の後、培養物に、EGFを再び添加し、同時にDMSOコントロール、またはロピナビルのみもしくは異なる比のロピナビルおよびリトナビルからなる合計20μMのAPIを添加し、そこで結果の項に示すように、インキュベーションをさらに2日間継続した。
APIとの培養の最終日の後、標準化された手順を用いて、細胞をトリプシン処理し、ペレット化し、PBSで洗浄した。次に、製造者のDNA断片化アッセイ手順(Chemometec Ltd、デンマーク)に従って、細胞を70%エタノールで最低4時間固定し、PBSで洗浄し、計数してから、DAPIで染色した。次に、NC3000画像血球計算盤で8チャンバースライドを用いて、染色された細胞を分析した。
図1に、DMSOコントロール、ロピナビルのみ、および本発明に係る比14:1w/wのロピナビル:リトナビルで処理した、DAPI染色N−Tert細胞の代表画像を示す。画像血球計算盤は、各試料中の1細胞当たりの特徴的DAPI−DNA染色を定量化することによって、4期の細胞周期から細胞を区別した。この代表的な出力データを付随のグラフ分析に示す。M1と表示される細胞の比率は、アポトーシスDNA断片化が起きている細胞(Sub G1、N=<2N DNA)に相当し、M2〜M4は、細胞周期の正常なG1期、S期、およびG2期を示す。
図2に、1.1.1〜1.1.3項および1.2項に記載のように実施された、8:1、10:1、12:1、14:1、および16:1(w/w)比のロピナビルおよびリトナビルでの処置後の、N−Tert細胞のDNA断片化および細胞周期の分析の箱ひげ図を示す。さまざまな処理で処理した細胞について、インタクトな2N〜4N DNAを有する細胞(G1、S、およびG2)に対する<2N DNAを有するアポトーシスSub G1細胞のパーセンテージを計算し、データを、以降のすべての図面で使用する箱ひげ図として提示した。また、データは、Rプログラムを用いて、ペアごとの並べ替え検定で統計的有意性を分析した。
本発明者らは、アポトーシス活性の統計的に有意なピーク(細胞の誘導細胞死に相当する)が、10:1、12:1、14:1、および16:1(w/w)のAPIで、ロトナビルのみまたは8:1(本発明の範囲外の比)と比較して明白であることに気付いて驚いた(図2を参照)。図1は、図2のデータの起源を示すために含める(上記の1.1.3を参照)。
これらのデータは、本発明に係る比のロピナビルおよびリトナビルが、皮膚の増殖性障害、例えば、AKの処置において、潜在的有効性を有することを示している。
<総API濃度25μMで、ロピナビル:リトナビルの10:1〜14:1w/w比が、HaCaT細胞に与える効果の評価>
HaCaT細胞は、皮膚生検材料に由来する、自然に形質転換したが非悪性のヒト細胞株である(Boukamp et al, (1988) J cell Biol 106(3) p761−71)。この細胞は、異数性を示し、p53変異を発現し、BCCまたはSCCなどのがん病変に向かって進行中のAKに相当する表現型を示す。したがって、この細胞株は、報告されている、NMSC、AK、およびHaCaT細胞における抗アポトーシスタンパク質スルビビンの発現により証明されるように(Grossman et al.(1999) Lab Invest 79(9) p1121−6)、ロピナビルおよびリトナビルが皮膚の前がん状態および初期がん状態、例えば、AKに与える効果を評価するための良好なモデルである。
下記を除き、1.1に記載の基本的方法に従った。
(2.1.1 細胞培養)
HaCaT細胞は、標準法により、5%ウシ胎児血清(FBS)を含有するダルベッコ変法イーグル培地(DMEM)を用い、5% CO2および37℃でインキュベートして培養した。N−Tert細胞と同様に、HaCaT細胞は、T75のスターター培養物からT25フラスコに播種し、5% FBSを含むDMEMで3日間増殖させた。次いで、細胞の成長周期を同調させるために、FBSを回収して、5% CO2および37℃で、細胞を培地中にさらに3日間のみ維持した。
(2.1.2:処置)
次いで、5% FCSを含有するDMEM成長培地と、DMSOコントロールまたは総API濃度25μMのロピナビルのみもしくは異なる比のロピナビルおよびリトナビルのいずれかとを同調HaCaT細胞に添加し、これを5% CO2および37℃でさらに3日間維持した。その後、これらを採取し、1.1.3項に記載のように、DNA断片化・細胞周期の分析のために、DAPIで染色した。
図3に、DMSO(コントロール)、20μMのロピナビルのみ、ならびに10:1、12:1、および14:1(w/w)のロピナビル/リトナビルでの処置後の、インタクトな2N〜4N DNAを有するHaCaT細胞(G1、S、およびG2)に対する、アポトーシスDNA断片化が起きているHaCaT細胞(Sub G1)のパーセンテージの箱ひげ図を示す。
本発明者らは、ロピナビルによって生じるアポトーシス活性が、リトナビルを10:1、12:1、および14:1の比で含めたときに増加することに気付いた。
このことは、本発明に係る比のロピナビルおよびリトナビルが、皮膚がんに向かって進行中のAK病変の特徴を示す細胞に対する活性を示すことによって、皮膚の増殖性障害、例えば、AKの処置において有効性を有するであろうこと、またがん自体の処置において有効性を有するであろうことを示している。
<総API濃度20μMで、ロピナビル:リトナビルの8:1、12:1、および18:1w/w比がA431細胞に与える効果の評価>
A431細胞は、85歳の女性から採取された皮膚生検材料に由来する完全悪性類表皮癌細胞株である(https://en.wikipedia.org/wiki/A431_cells)。この細胞は、変異p53を有し、NMSC、とりわけSCCと一致する表現型を示すと考えられている(Oleson (2017) Anticancer Drugs 28(10) p1106−1117)。したがって、この細胞株は、ロピナビルおよびリトナビルが完全悪性NMSCに与える効果を評価するための良好なモデルである。
下記を除き、2.1に記載の方法に従った。
3.1.1:A431細胞をT25フラスコに播種し、DMEM+5% FBSで3日間増殖させた後、FBSを2日間除去して、細胞を同調させた。
3.1.2:次いで、DMEM+5% FCSと、DMSOコントロールまたは総API濃度25μMのロピナビルのみもしくは結果の項に記載の比のロピナビルおよびリトナビルのいずれかとを添加し、細胞を2日間インキュベートした後、1.1.3項に記載のようなDNA断片化および細胞周期の分析のために採取した。
図4に、DMSO(コントロール)、20μMロピナビルのみ、ならびに8:1、12:1、および18:1(w/w)のロピナビル:リトナビルでの処置後の、インタクトな2N〜4N DNAを有するA431細胞(G1、S、およびG2)に対するアポトーシスDNA断片化が起きているA431細胞(Sub G1)のパーセンテージの箱ひげ図を示す。
本発明者らは、ロピナビルまたは8:1のロピナビル:リトナビルによって生じるアポトーシス活性が、APIを12:1または18:1の比で含めたときに増加することに気付いた。12:1での形質転換細胞の死滅の向上は、極めて有意であることが示された。
このことは、本発明に係る比のロピナビルおよびリトナビルが、完全悪性皮膚がん、例えば、SCCの予防または処置において有効性を有するであろうことを示している。
実施例1〜3において提示するこれらのデータは、本発明に係る比のロピナビルおよびリトナビルが、誘導性細胞死に対して、ロピナビルのみ、または本発明において規定される範囲から外れる比のロピナビルおよびリトナビルよりも有意に高い効果を与えることを示している。好適なAPI比が細胞株モデルのそれぞれにおいて有効であったということは、皮膚の良性増殖性障害(例えば、AK)の処置、および/またはその発症もしくは進行の抑制;増殖性障害の皮膚がんへの進行の予防;ならびに悪性皮膚がん自体(例えば、BCCまたはSCC)の処置における、規定される比のロピナビルおよびリトナビルの有用性を明らかに示している。
<好適な製剤の調製>
下記に提示するすべての製剤について、使用する材料は、日本医薬品賦形剤グレードの白色セレシンワックスを除き、すべてが医薬品グレード(米国薬局方または欧州薬局方のいずれか)であった。
i.ミキサーに、3、4、5、6、7、8、9、1、10、11の材料を入れる。
ii.容器の内部から空気を排除する。
iii.低せん断混合下、70℃に加熱して、澄んだ透明融解物を得る。
iv.ミキサーに、2の材料を入れる。
v.容器の内部から空気を排除する。
vi.低せん断で混合して、融解物中にHPMCを微分散させる。
vii.低せん断混合下、内容物の温度を45℃まで下げる。
viii.保管容器に取り出し、保管中は空気を排除する。
ix.組成物1.0〜5.0gを調剤するのに適したアルミニウムチューブに組成物を充填する。アルミニウムチューブには、20〜50mlの容量の組成物を入れることができる。
<ロピナビル/リトナビルによる局所処置が表在型基底細胞癌(BCC)を示す対象に与える効果の臨床的検討>
試験の目的は、表在型BCCの処置として表4の軟膏を局所投与することの有効性について、概念実証パイロットデータを得ること、また30日周期でBCC病変に局所適用した場合の、0.85gの1日3回投与の忍容性を評価することであった。
軟膏は、ラテックスシールを備える75gのチューブに詰められた。1日の総用量2.5gは、約0.8gの3回適用(病変によくもみ込む)で投与されるよう意図される。
実施例5に、本発明に係る好適な組成物を投与する好適な方法を記載する。
Claims (14)
- 皮膚適用のために製剤化される医薬組成物であって、薬学的に許容されるビヒクル中に治療有効量のロピナビルおよびリトナビルを含み、ロピナビル:リトナビルの質量比(w/w)が9:1から18:1までの間である、医薬組成物。
- 皮膚がんおよび皮膚の良性増殖性障害を処置するための、および/またはその発症または進行を抑制するための医薬として使用するための、請求項1に記載の組成物。
- 前記組成物が、基底細胞癌(BCC)、扁平上皮癌(SCC)、または日光角化症(AK)を処置するための、および/またはその発症もしくは進行を抑制するための有効量のロピナビルおよびリトナビルを含む、請求項2に記載の組成物。
- 初期皮膚腫瘍を処置するための、および/またはその発症を抑制するための医薬として使用するための、請求項1〜3のいずれか1項に記載の組成物。
- 皮膚のHPV感染に関連するがんもしくは良性増殖性障害を処置するための、またはその発症を予防するための医薬として使用するための、請求項1〜4のいずれか1項に記載の組成物。
- ロピナビルおよびリトナビルが、10:1から16:1の間の質量比で含まれる、請求項1〜5のいずれか1項に記載の組成物。
- ロピナビルおよびリトナビルが、10:1から14:1までの間の質量比で含まれる、請求項6に記載の組成物。
- ロピナビルおよびリトナビルが、約12:1の質量比で含まれる、請求項6に記載の組成物。
- 軟膏、ゲル剤、ペースト剤、クリーム剤、またはローション剤として製剤化される、請求項1〜8のいずれか1項に記載の組成物。
- 前記薬学的に許容されるビヒクルが軟膏である、請求項1〜9のいずれか1項に記載の組成物。
- 前記組成物が、
(a)不飽和遊離脂肪酸と、
(b)硬化剤と、
(c)9:1から18:1の間の質量比のロピナビルおよびリトナビルと
を含み、
前記不飽和遊離脂肪酸が、前記医薬組成物の総質量の少なくとも20質量%のレベルで存在し、前記医薬組成物が、室温で半固体である、請求項9に記載の組成物。 - 皮膚がんおよび皮膚の良性増殖性障害の処置またはその発症もしくは進行の抑制が必要な対象において、皮膚がんおよび皮膚の良性増殖性障害を処置する、および/またはその発症もしくは進行を抑制する方法であって、治療有効量の請求項1〜11のいずれか1項に記載の医薬組成物を前記対象に投与するステップを含む方法。
- 前記がんまたは障害が、ヒトパピローマウイルス(HPV)によって引き起こされるか、または誘導される、請求項12に記載の方法。
- 前記がんまたは障害が、基底細胞癌(BCC)、扁平上皮癌(BCC)、または日光角化症(AK)である、請求項12に記載の方法。
