本特許出願は、その全内容が参照により本明細書に組み入れられる、以下2018年3月5日に出願された米国特許仮出願第62/638,697号の恩典を主張する。
遺伝性難聴は、人工内耳以外に治療選択肢が少ない深刻な障害である。遺伝性聴力障害は、多くの場合、単一遺伝子異常によるものである。言語習得前難聴は乳児の1/500において診断され、そのうち約50%が遺伝的病因を有する。それぞれが多数の異なる遺伝子のいずれかの変異によって生じ得る多数の異なる臨床サブタイプと関連するアッシャー症候群が、幼児期難聴の3〜6%の原因とされている。すべての遺伝性難聴の1〜2%であると推定される、より一般的な遺伝子異常の1つがTMC1遺伝子において発生する。アッシャー症候群のもっとも重篤な形態であるUSH1は、6つの遺伝子:USH1、MYO7A(ミオシン7a)、USH1C(harmonin「ハーモニン」)、CDH23(カドヘリン23)、PCDH15(プロトカドヘリン15)、SANS(sans;USH1Gとも知られる)、およびCIB2(カルシウムおよびインテグリン結合タンパク質2)の異常と関連する。

本発明は、聴覚および/または前庭機能を含むメカノセンセーションに必要なタンパク質(たとえばTMC1、TMC2、MYO7A、USCH1C、CDH23、PCDH15、SANS、CIB2、USH2A、VLGR1、WHRN、CLRN1、PDZD7、KCNQ4、TMPRSS3、STRC、EYA4、USH1C(たとえばハーモニンa、bまたはc)、OTOF、GPR98、MYO6、MYO15A、LOXHD1、POU3F4、EYA1、WFS1、ACTG1、TMIE、PJVK、SYNE4およびFAM65B)を、当該タンパク質のレベルまたは活性の損失または低下を有する対象の内耳の細胞、たとえば蝸牛細胞(たとえば内または外有毛細胞)中に送達し、発現させるための組成物および方法を提供する。
AAV-PHP.Bベクターは、既定のCre発現性標的細胞集団(CREATEとも呼ばれる)を形質導入するカプシドを選択的に回収するためのCre組換え依存手法を使用して作製したものである。本発明の方法において有用なこの手法およびベクターは、Deverman et al., "Cre-dependent selection yields AAV variants for widespread gene transfer to the adult brain,” Nat Biotechnol. 2016 February; 34(2): 204-209および米国特許出願公開第20170166926号に記載されている。いずれも全体として参照により本明細書に組み入れられる。ランダム化配列(7量体)の7つのアミノ酸(AA)をAAV9カプシドのAA588とAA589との間(VP1位置)に挿入することにより、AAVバリアントのライブラリーを作製した。AAV-PHP.Bは7量体配列TLAVPFKをコードし、これを、蝸牛への効率的な導入遺伝子送達に関して試験すると、内および外有毛細胞中で著しく特異的かつロバストな発現が示された。
ヒトアッシャー症候群(USH)は、盲聾の原因である希な遺伝子疾患である。常染色体劣性形質として遺伝し、米国において16,000〜20,000人を冒し、幼児期難聴の3〜6%の原因である。アッシャー症候群は、症状の重度にしたがって3つの臨床亜型(USH-1、2、および3)に分類される。USH1が最重症型である。USH1に冒された患者は、先天性の両側性重度感音性聴力喪失、前庭反射消失、および思春期前の網膜色素変性症(網膜の桿体および錐体機能の進行性両側性対称性縮退)を病む。人工内耳を装着しない限り、個体は一般的に発声能力を発達させない。現在、アッシャー患者のための生物学的治療は存在しないが、欠陥遺伝子の野生型形態の早期再導入が疾患の好転を許し得る。
難聴を生じさせる40を超える異なる変異がTMC1において同定されている。これらは、35の劣性変異および5つの優性変異へと細分される。劣性変異の大多数は重度の先天性聴力喪失を生じさせるが(たとえばDFNB7/11)、少数は晩期発症型の中〜重度の聴力喪失を生じさせる。優性変異のすべてが、十代半ばに発症する、進行性の聴力喪失を生じさせる(たとえばDFNA36)。特に、本明細書に記載されるAAV9-PHP.Bベクターは、非変異体(たとえば野生型)TMC1配列またはTMC2配列を送達し、それによって、聴力喪失(たとえば、さらなる聴力喪失)を予防する、および/または聴覚機能を回復させるために使用することができる。
成体哺乳動物蝸牛の感覚細胞は自己修復能力を欠くため、現在の治療戦略(障害のレベルおよび正確な位置に依存する)は、増幅(補聴器)、より良い音声伝達(中耳プロテーゼ/アクティブインプラント)または直接神経刺激(人工内耳)に依存して、聴覚神経を形成し、音響情報を脳へと中継する一次感覚有毛細胞またはらせん神経節ニューロンへの永久的損傷を補償する。これらの手法は変革的であったが、現代生活にとって重要な複雑なヒト聴覚機能の回復においては依然、最適からはほど遠い。特に、主な問題はさらに、限られた周波数感度、不自然な音声知覚および騒がしい環境における限られた語音弁別を含む。

核酸を細胞に送達する方法は一般に当技術分野において公知であり、導入遺伝子を含有するウイルス(ウイルス粒子と呼ぶこともできる)をインビボで内耳細胞に送達する方法が本明細書に記載されている。本明細書に記載されるように、約108〜約1012個のウイルス粒子を対象に投与することができ、ウイルスは、適当な量(たとえば10μL、50μL、100μL、500μLまたは1000μL)の、たとえば人工外リンパ液内に懸濁させることができる。

仔マウス(P0〜P2)に対し、斜角研磨したガラスマイクロインジェクションピペットを使用して、正円窓膜(RWM)を介して、AAV-PHP.BベクターCMV GFP注入を実施した。P-2000ピペットプラー(Sutter Instrument, Novato, CA)上、ガラス毛管(WPI)からピペットを引き、マイクロピペットベベラ(Sutter Instrument, Novato, CA)を使用して斜角研磨した(先端径約20μm、角度28°)。手術部位(左乳様突起)を覆うために無菌スワブを使用して、鎮痛のためにEMLAクリーム(リドカイン2.5%およびプリロカイン2.5%)を外部から塗布した。術前、加温パッド上で体温を38℃に維持した。仔マウスを、氷/水中で2〜3分間、急速な低体温誘導によって麻酔して意識消失させ、手術中、この状態を冷却プラットフォーム上で5〜10分間維持した。べタジンでスクラブし、70%エタノールで拭くことを3回繰り返すことにより、手術部位を消毒した。耳後部の切開を実施して透明な耳胞を露出させ、マイクロピペットを手で耳胞およびその上の筋膜に通し、マイクロピペットの先端をRWMに通した。ウイルス約1μLを、1分以内に、C57BL/6動物の左耳だけに手で注入した。注入後、6-0黒色モノフィラメント縫合糸(Surgical Specialties, Wyomissing, PA)を使用して皮膚切開創を閉じた。その後、仔マウスを38℃の加温パッドに戻して5〜10分おき、次いで、飼育のために母マウスに戻した。
関心対象の遺伝子をコードする導入遺伝子を含むAAV-PHP.Bベクターの形質導入に続き、有毛細胞の電気生理を分析した。蝸牛を切除し、カバーガラスに載せ、63×水浸対物レンズおよび微分干渉コントラストオプティクスを備えたAxio Examiner.A1正立顕微鏡(Carl Zeiss, Oberkochen, Germany)で観察する。MEM(Life Technologies, Carlsbad, CA)のように、137 NaCl、5.8 KCl、10 HEPES、0.7 NaH2PO4、1.3 CaCl2、0.9 MgCl2および5.6 D-グルコース(mM単位)、ビタミン(1:100)およびアミノ酸(1:50)を含有する標準液(pH7.4;約310mOsm/kg)中、室温(22℃〜24℃)で電気生理学的記録を実施する。記録電極(3〜4MΩ)をR-6ガラス(King Precision Glass, Claremont, CA)から引き、140CsCl、5EGTA-KOH、5HEPES、2.5Na2ATP、3.5MgCl2および0.1CaCl2(mM単位)を含有する細胞内液(pH7.4;約280mOsm/kg)で満たす。ホールセルタイトシール技術を使用して、Axopatch 200B(Molecular Devices, Sunnyvale, CA)を使用してメカノトランスダクション電流を記録する。有毛細胞を−84mVに維持した。ローパスBesselフィルタによって電流を5kHzでフィルタリングし、12ビット収集ボード(Digidata 1440A、Molecular Devices, Sunnyvale, CA)によって≧20kHzでデジタル化し、pCLAMP10ソフトウェア(Molecular Devices, Sunnyvale, CA)を使用して記録した。LVPZT増幅器(E-500.00、Physik Instrumente, Karlsruhe, Germany)によって駆動されるPICMAチップピエゾアクチュエータ(Physik Instrumente, Karlsruhe, Germany)に取り付けた堅いガラスプローブを使用して、IHCおよびOHCからの毛束を偏向させ、8ポールBesselフィルタ(Model 3384フィルタ、Krohn-Hite Corporation, Brokton, MA)によって40kHzでフィルタリングして、残留ピペット共振を除去した。堅いガラスプローブは、全束記録のために有毛細胞不動毛の列の凹側面に嵌まるように設計されたものである(OHCの場合には直径3〜4μm、IHCの場合には直径4〜5μm)。>P10での全細胞電気生理学記録のために、P5〜7で蝸牛組織を解体し、MEM(1×)+1%FBSを含むGlutaMAXTM-I培地中、37℃、5%CO2で30日間までインキュベートする。
聴力は、遺伝的に聴覚障害を有するマウスに形質導入したのちにアッセイする。聴性脳幹反応(ABR)および歪成分耳音響放射(DPOAE)データを収集する。DPOAEは、正しい蝸牛増幅および同調のためのアッセイであり、外有毛細胞の生存可能性の高感度な尺度である。麻酔したマウスにおいて試験した刺激は、5.6、8、11.3、16、22.6および32kHzの周波数で音圧レベル10から90dBまで変化させた。ABRを誘発するために必要な最小限の音閾値をプロットする。
ロータロッド装置上での平衡行動に関してマウスを試験する。前庭機能が損傷したマウスは、ロータロッド装置上でのパフォーマンスが良くないことが知られている。以前の研究が、一方の耳のみが障害を有する場合に平衡機能不全を検出する、このロータロッド試験の能力を強調している。マウスに対し、P1で注入を実施し、P36で試験し、非注入対照マウスをP79で試験する。以下のロータロッドプロトコルを使用してすべてのマウスを試験する。1日目、マウスを、4RPMで回転するロッド上で5分間の平衡を保つよう、訓練する。2日目、マウスを5回の試行(5分間隔)で試験する。試行ごとに、ロッドを2RPMの開始速度から1RPMずつ加速する。マウスが装置から落ちるまでの時間(秒単位)を記録する。
組織標本
電気生理学的研究のために、Ush1c c.216G>Aヘテロ接合型またはホモ接合型変異体マウスからの卵形嚢およびコルチ器を出生後0〜8日(P0〜P8)で採取する。出生後の仔マウスを迅速な断頭によって屠殺する。側頭骨を切除し、10mM HEPES(pH7.4)を添加したMEM(Invitrogen, Carlsbad, CA)に浸漬する。前記(53)のように、酵素を使用せずにコルチ器を切り出す。0.1mg/mlのプロテアーゼ(Protease XXIV, Sigma)による10分間の処理ののち、卵形嚢を取り出す。切除した器官を丸いカバーガラスに載せる。事前にカバーガラスに接着しておいた一対の細いガラス繊維を組織の縁に配置して、組織をフラットな位置で安定させる。組織は、すぐに使用するか、1%ウシ胎仔血清の存在下で培養するかのいずれかである。インビトロでのウイルスベクター感染を伴う実験のために、培養を7〜8日間維持し、培地を2〜3日ごとに交換する。
Ush1c c.216G>Aノックインマウスをルイジアナ州立大学Health Science Centerから入手した。C57BL6バックグラウンド上のインポートされた株を事前にCdh23(Ahl)変異から育種して、加齢性聴力喪失を生じさせた(48、49)。トウクリップ(P8の前)またはイヤーパンチ(P8の後)を使用してマウスの遺伝子型を決定し、前記(32)のようにPCRを実施する。すべての試験に関し、雄雌マウスをほぼ等しい割合で使用した。他の点では無作為化パラダイムは適用しない。
選択したプロモーター下で関心対象の遺伝子を発現するAAV-PHP.Bベクターを作製する。ベクター0.8μl〜1ulをP0〜P1およびP10〜P12の新生仔マウスに注入する。最初に、P0〜P1マウスを低体温曝露によって麻酔し、P10〜P12マウスをイソフルランによって麻酔する。麻酔中、耳後部切開を実施して耳胞を露出させ、蝸牛を可視化する。マイクロマニピュレータ(Askew et al., 2015)によって制御されるガラスマイクロピペットを用いて、RWMを通して注入を実施する。10分間、注入物質の量をおよそ0.02μl/分に制御する。標準的な術後ケアを適用する。試料サイズを最適化し、分散を減らすために、インビボ実験のための試料サイズを連続的に測定した。
144 NaCl、0.7 NaH2PO4、5.8 KCl、1.3 CaCl2、0.9 MgCl2、5.6 D-グルコースおよび10 HEPES-NaOH(mM単位)を含有する、pH7.4および320mOsmol/kgに調節された標準人工外リンパ液中で記録を実施する。濃縮物(Invitrogen, Carlsbad, CA)からビタミン(1:50)およびアミノ酸(1:100)を添加した。63×水浸対物レンズおよび微分干渉コントラストオプティクスを備えたAxioskop FS正立顕微鏡(Zeiss, Oberkochen, Germany)を使用して、頂端面から有毛細胞を観察した。記録ピペット(3〜5MΩ)をホウケイ酸ガラス毛管(Garner Glass, Claremont, CA)から引き、135 KCl、5 EGTA-KOH、10 HEPES、2.5 K2ATP、3.5 MgCl2、0.1 CaCl2(mM単位)を含有するpH7.4の細胞内液で満たした。全細胞電圧固定下、室温で−64mVの保持電位で電流を記録した。Axopatch Multiclamp 700AまたはAxopatch 200A(Molecular Devices, Palo Alto, CA)を使用してデータを取得し、ローパスBesselフィルタによって10kHzでフィルタリングし、12ビット収集ボード(Digidata 1322)ならびにpClamp8.2および10.5(Molecular Devices, Palo Alto, CA)を用いて≧20kHzでデジタル化した。OriginLabソフトウエアによってデータをオフラインで分析した。別段の記載がない限り、平均値±標準偏差として表す。
Startle Monitor(Kinder Scientific)を使用して聴覚性驚愕反応(ASR)を計測する。マウスを、圧電/プレキシガラス感知アセンブリに固定された小型の非制限的な立方体プレキシガラス記録チャンバ(27cm×10cm×652 12.5cm)に入れ、60dB SPLバックグラウンドホワイトノイズで5分間順化させる。各セッションは35回の試行からなり、その間、10dB SPL強度が60〜120db SPLの範囲である1つのノイズパルスを平均30秒(25〜35秒範囲)の試行間隔で印加する。外部ノイズ干渉を制限するために、パルスは、一定の60dB SPLバックグラウンドノイズ上、擬似ランダムな順序で並べる。Startle Monitorシステムが、ピーク驚愕反応(ASR振幅)および刺激からピーク驚愕反応までの時間(ASR潜時)の計算のために、各パルスへの反応を第一N、最大Nおよび反応の最大時間(ms)の計測値へと換算した。ASRはすべて盲検的に実施した。
AAV-PHP.Bベクターによって送達された導入遺伝子の発現の分布を決定するために免疫染色を実施する。そうするために、解体したばかりのコルチ器を、PBS中に希釈した4%パラホルムアルデヒドによって室温で1時間浸漬固定して、免疫染色を実施する。次いで、組織をPBS中で洗浄し、0.01〜0.1%Triton X-100で30分間透過処理し、AlexaFluor546ファロイジン(Molecular Probes、1:200希釈)で1時間対比染色して、フィラメント状アクチンを標識する。
治療ベクターを内耳に送達するための新規な注入法を開発した。以前の注入法は、正円窓膜、卵円窓または後半規管を通してベクターを送達するものであった。いくらかは効果的であるが、これらの方法はすべて、外科的にアクセスし難い標的、不均一なウイルス分散および外リンパまたは内リンパ空間を標的とするときの有意な可変性を含む有意な欠点を抱えている。これらの制限を回避するために、聴覚または前庭機能不全を生じさせることなく内耳空間への効率的な送達を可能にする新規な方法を設計した。
蝸牛および前庭ニューロンを効果的に形質導入することができるかどうかを決定するために、AAV9-PHP.Bベクターを、シナプシンプロモーターを含むように改変した。P1で、C57マウスに対し、PHP.B-Syn-eGFPの卵形嚢注入を実施した。P15で組織を採取し、横断切片およびホールマウント切片を調製した。図15を参照すると、蝸牛および前庭横断切片ならびにホールマウント切片は、AAV-PHP.B-Syn-eGFPが、らせん神経節および前庭ニューロンを形質導入する場合に高い特異性および効率を有することを示す。
治療ポリペプチドの発現を駆動するPHP.Bベクターの能力をホモ接合型Tmc1変異体マウスにおいて評価した。P1でマウスに注入を実施し、P30で聴覚機能を計測した。ABRおよびDPOAE閾値によって計測されたように、AAV9-PHP.B-Cmv-Tmc1ベクターは、変異体マウスにおいて聴覚機能を回復させた(図16Aおよび16B)。このベクターはAAV1-Cmv-Tmc1およびAnc80-Cmv-Tmc1の性能を上回り、AAV9-PHP.B-Cmv-Tmc1ベクターを投与された1匹のマウスは野生型と同様の性能を示した(図16A)。
特定の細胞または組織中で発現を駆動するプロモーターは、治療導入遺伝子の標的化送達およびオフターゲット発現の最小化にとって特に貴重である。特に前庭細胞中で発現を駆動するいくつかのプロモーターの能力を調べた。図17A〜17Cを参照すると、Pcdh15、ミオシン6およびミオシン7aプロモーターが内および外有毛細胞中で発現を駆動している。KCNQ4プロモーターは外有毛細胞中で特異的に発現を駆動する(図17D)。
本明細書において、方法および物質組成はいくつかの異なる局面に関連して説明されたが、様々な局面の前記説明は、方法および物質組成を例示することを意図したものであり、方法および物質組成の範囲を限定することを意図したものではないことが理解されよう。他の局面、利点および変形が以下の請求項の範囲内である。

- 内耳障害がアッシャー症候群である、請求項7記載の方法。
- 対象における聴覚および/または前庭機能を増強、改善、または維持する、請求項7記載の方法。
- 欠陥を有する対象に欠陥遺伝子の野生型形態を導入するための方法であって、該対象の細胞を請求項1または2記載のAAVベクターと接触させる工程を含む、方法。
- 対象におけるアッシャー症候群を治療する方法であって、該対象の細胞をAAV9-php.bベクターと接触させる工程を含み、該ベクターが、Espinプロモーター、PCDH15プロモーター、PTPRQプロモーター、Myo6プロモーター、KCNQ4プロモーター、Myo7aプロモーター、シナプシンプロモーター、GFAPプロモーター、CMVプロモーター、CAGプロモーター、CBHプロモーター、CBAプロモーター、U6プロモーター、およびTMHS(LHFPL5)プロモーターからなる群より選択されるプロモーターを含み、該プロモーターが、ミオシン7a、ハーモニン、カドヘリン23、プロトカドヘリン15、USH2A、ADGRV1/VLGR1/GPR98、WHRN、CLRN1、HARS、SANS、およびカルシウムおよびインテグリン結合タンパク質2からなる群より選択されるポリペプチドをコードするポリヌクレオチドの発現を指示する、方法。
