本発明は、患者の既存の免疫応答を癌に指向させるための方法、組成物およびキットを含む、免疫学および癌治療に関する。
持続性の無症候性ウイルス感染は、通常、健康な個体では細胞性免疫および/または体液性免疫によって制御されるが、免疫力が低下した個体では再活性化され得る。一部の慢性ウイルス感染に対する細胞性免疫は年齢とともに増加し、多くの完全に機能するウイルス特異的T細胞の誘導につながる。サイトメガロウイルス(CMV)は、世界的に蔓延し(ヒト集団の50〜90%が感染しており)、かつ健康な個体ではほとんど無症状である、β−ヘルペスウイルスである。CMVは、生涯にわたる持続的な感染を確立し、発症を阻止するためには永続的な細胞性免疫を必要とする。それゆえに、CMVの再活性化は、免疫抑制において、例えば造血幹細胞移植において、脅威である。免疫適格性の個体では、CMVに対するCD4およびCD8 T細胞応答は、複数のCMV抗原に対して幅広い反応性および高い強度を示し、一般的なヒト集団で高い有病率を示し、年齢とともに増加する(M. 本発明者らは、高齢者の慢性ウイルス感染を強力に制御するために長年にわたって展開する複雑な適応性細胞介在免疫が、腫瘍増殖の制御に有効なタイプの細胞介在免疫であることを認識していた。このタイプの抗ウイルス免疫を癌処置に利用するために、本発明者らは、腫瘍指向性パピローマウイルスシュウドビリオンを用いて高機能な既存の抗ウイルスT細胞を腫瘍環境に直接標的化するか、または最小限のウイルスCD8およびCD4 T細胞サイトメガロウイルス(CMV)エピトープのインサイチュウ注入によって、インサイチュウ免疫療法への新しいアプローチを開発した。腫瘍環境におけるウイルスエピトープの提示は、インサイチュウでのウイルス抗原特異的T細胞の動員および活性化をもたらし、結果として、ウイルス陰性の腫瘍細胞を異なる方法で死滅させ、腫瘍微小環境を変化させる。このアプローチは、早期および長期的な癌細胞の死滅およびエピトープの拡散の両方を促進し、確立することにより、免疫療法を成功させるためのすべての基準を満たすため、満たされていないニーズに対応している。
本発明は、癌を処置する新規な方法に関する。具体的には、本発明は、癌細胞を攻撃するために個体自身の免疫系を利用する、該個体における癌を処置する方法に関する。本発明の方法は、癌に応答して誘導されたのではなく、環境中の微生物によって誘導された既存の免疫応答を個体が有しているという事実を利用したものである。癌細胞は、通常、既存の免疫応答を誘導した微生物抗原を発現しないため、そのような免疫応答が癌を攻撃することは期待できない。しかしながら、本発明者らは、そのような既存の免疫応答が、癌を攻撃するために動員され得ることを発見した。これを達成することができる1つの方法は、既存の免疫応答によって認識された1以上の抗原を癌に導入することによって、免疫応答の細胞が抗原を提示する癌細胞を攻撃する結果となる。したがって、これらの方法は、癌患者の処置に先立って抗原を発現する癌細胞に向けられていない。例えば、多くの膠芽腫癌細胞は、CMV抗原を発現することが判明しており、本開示の方法は、CMVに対する個人の既存の免疫を用いて、そのような神経膠芽腫を処置するためには用いられない。さらに、癌細胞の破壊は、既存の免疫応答の構成要素が癌細胞抗原に暴露される結果となり得る。これは、癌細胞抗原に対する免疫応答の誘導をもたらし得る。したがって、本発明の一般的な方法は、既存の免疫応答が癌を攻撃するように、個体における既存の免疫応答を癌部位に動員することによって実施され得る。動員は、例えば、個体の既存の免疫応答の構成要素(例えば、T細胞)によって認識される少なくとも1つの抗原を癌に導入することによって達成され得る。
i)特定の抗原を認識するB細胞;
ii)特定の抗原を認識する抗体;
iii)特定の抗原を認識するT細胞;および
iv)特定の抗原に反応して開始されるT細胞活性。
その後、抗原が癌に導入されるように、既存の免疫応答を有することが確認された個体に特異的抗原を投与し、それによって癌を処置することができる。
例えばヒトサイトメガロウイルス(hCMV)のような、通常は宿主によって十分に制御されている慢性ウイルス感染症は、しばしば、加齢に伴い、完全に機能するウイルス特異的T細胞の数がますます多くなる誘導をもたらす。hCMVに対するヒト免疫応答の重要な面を模倣したマウスmCMVモデルを用いて、本発明者らは、これらの抗ウイルスT細胞を腫瘍に誘引し、それに続いて腫瘍細胞の死滅および腫瘍ネオ抗原への強力なエピトープの拡散を誘導して、腫瘍増殖の長期的な制御および同種腫瘍細胞との再暴露からの保護を与える適応免疫応答をもたらす方法および試薬を開発した。
マウスサイトメガロウイルス感染は、mCMVペプチドプールに対するサイトカイン応答を誘導する
C57Bl/6マウスに1x104 pfuのマウスサイトメガロウイルス(mCMV)を感染させた。感染後12日目に血液サンプルを採血した。白血球を、m38、m45、m57、m122、1m39、m141およびm164 mCMVタンパク質から選択された免疫原性ペプチド集団で再刺激した。CD8+ T細胞によるIFN−γ、TNF−αおよびIL−2サイトカイン産生を、細胞内サイトカイン染色によって評価し、蛍光活性化細胞選別(FACS)によって分析した(図1A)。血液サンプルを、感染から2ヶ月後に採血した。インフレーション性(inflationary)(m122)および非インフレーション性(non-inflationary)(m45)特異的CD8+ T細胞を、MHC−I四量体染色を用いたFACSにより検出した。メモリーCD8+ T細胞応答をmCMVに対してマッピングした。脾臓を感染から6ヶ月後に採取した。m38、m45、m122 MHC−I制限およびm139560-574 MHC−II制限mCMVペプチドでのインビトロ刺激後のCD8+およびCD4+ T細胞によるIFN−γ産生を、細胞内サイトカイン染色によって評価した(図1B)。
mCMV抗原を発現するHPV Psvを用いた固形腫瘍の腫瘍内導入
C57Bl/6マウスに1x104 pfuのマウスサイトメガロウイルス(mCMV)を感染させた。感染から6ヶ月後、マウスに、E6およびE7腫瘍性タンパク質を発現する2x105 TC−1腫瘍細胞を皮下注射(s.c.)した(注射プロトコール、図2A)。腫瘍増殖を電子ノギスを用いて測定した。腫瘍注入後13日目および15日目に、m122およびm45を発現するHPV16 Psv(図2B)、または赤色蛍光タンパク質(RFP)を発現するHPV Psv(図2C)を腫瘍内に注射した(Psv当たり108感染単位)。
ポリ(I:C)と組合せてmCMV抗原を用いた固形腫瘍の腫瘍内導入
C57Bl/6マウスに、1x104 pfuのマウスサイトメガロウイルス(mCMV)を感染させた。感染から4ヶ月後、マウスに、E6およびE7腫瘍性タンパク質を発現する2x105 TC−1腫瘍細胞(図3A)を皮下注射した。腫瘍に、ポリ(I:C)(30μg)(PIC)の有無下で、11日目および13日目にm122、m38およびm45を発現するHPV16、16日目および18日目にm122、m38およびm45を発現するHPV45、ならびに21日目および23日目にm122、m38およびm45を発現するHPV58を、または対照RFPを(PsV当たり108感染単位)、腫瘍内注射した。腫瘍増殖を、電子ノギスを用いて測定した(図3B−3E)。これらの腫瘍体積/増殖データは、mCMV抗原を発現するHPV Psvによる固形腫瘍の腫瘍内導入が腫瘍増殖を遅延させ、ポリ(I:C)との共投与がさらに腫瘍増殖を遅延させることを実証している(図3Bおよび3Dを比較;および、図3Cおよび3Eを比較)。E7−(図3F)、m45−、およびm122−(図3G)特異的CD8+ T細胞による腫瘍への浸潤を、MHC−I四量体染色およびFACSによって分析した。これらのデータは、これらのCMV抗原をポリ(IC)と組み合わせて投与したとき、CD8+ T細胞の腫瘍浸潤が有意に増強されたことを証明する。
mCMV MHC−I制限ペプチドの腫瘍内注射は生存率の向上をもたらす
C57Bl/6マウスに、1x104 pfuのマウスサイトメガロウイルス(mCMV)を感染させた。感染から4ヶ月後、マウスを、E6およびE7腫瘍性タンパク質を発現する2x105 TC−1腫瘍細胞を皮下注射した(図3A)。腫瘍に、11日目、13日目、16日目、18日目、21日目および23日目に、ポリ(I:C)(30ug)の有無下、選択されたm38、m45およびm122ペプチド(各1μg)を、および対照として生理食塩水またはポリ(I:C)単独を、腫瘍内注射した。動物の死亡を記録し(図4A)、腫瘍増殖を電子ノギスを用いて測定した(図4B)。これらのデータは、mCMV MHC−I制限ペプチドの腫瘍内注射が腫瘍増殖を遅延させ、生存率の増加をもたらすことを証明している。
mCMV MHC−I制限ペプチドの腫瘍内注射は腫瘍増殖を遅延させる
C57Bl/6マウスに、1x104 pfuのマウスサイトメガロウイルス(mCMV)を感染させた。感染から4ヶ月後に、E6およびE7腫瘍性タンパク質を発現する2x105のTC−1腫瘍細胞を皮下注射した。腫瘍に、11日目、13日目、16日目、18日目、21日目および23日目に、ポリ(I:C)(30μg)の有無下、選択されたm38、m45およびm122ペプチドの漸減用量(1μg、0.1μgおよび0.01μg)を、および対照として生理食塩水またはポリ(I:C)単独を、腫瘍内注射した。腫瘍増殖を、電子ノギスを用いて測定した(図5)。これらのデータは、mCMV MHC−I制限ペプチドの腫瘍内注射が腫瘍増殖を遅延させることを示している。
mCMV MHC−IおよびMHC−II制限ペプチドの組合せは腫瘍増殖を遅延させる
C57Bl/6マウスに、2.5x105 mCMVを感染させた。感染から4ヶ月後、マウスにE6およびE7腫瘍性タンパク質を発現する2x105のTC−1腫瘍細胞を皮下注射した。腫瘍に、12日目から28日目まで、MHC−I制限された選択されたm38、m45およびm122ペプチド、および/またはMHC−II制限されたm139選択されたペプチドまたは生理食塩水を、6回、腫瘍内注射した。すべてのペプチドを、ポリ(I:C)(30μg)と共に注射した。群は、MHC−Iペプチドを6回、またはMHC−IIペプチドを6回、またはMHCIおよびMHCIIペプチドを共に6回、またはMHC−Iペプチドを3回、次いでMHC−IIペプチドを3回、またはMHC−IIペプチドを3回、次いでMHC−Iペプチドを3回を順次注射した。腫瘍増殖を、電子ノギスを用いて測定した(図6Aおよび6B)。これらのデータは、mCMV MHC−IおよびMHC−II制限ペプチドの組合せの腫瘍内注射が腫瘍増殖を遅延させることを実証している。血中のE7−、m45−、m122−特異的CD8+ T細胞応答もまた、各ペプチドのMHC−I四量体を用いたFACSによって分析した(図6C)。これらのデータは、mCMV CD4エピトープ、次いでCD8エピトープでの逐次的な腫瘍内接種が優先的に抗腫瘍免疫を誘導することを示している。
原発腫瘍の完全クリアランスは長期的な腫瘍保護(tumor protection)をもたらす
実施例6に記載の原発腫瘍暴露から生き残った保護C57Bl/6マウスに、第一暴露の反対側の脇腹に、E6およびE7腫瘍性タンパク質を発現する2x105 TC−1腫瘍細胞を皮下注射した。腫瘍採取の対照として、若齢(12週齢)マウスおよび年齢を合わせた(10月齢の)マウスに、TC−1腫瘍細胞を暴露した。腫瘍増殖を、電子ノギスを用いて測定した(図7)。これらのデータは、原発腫瘍の完全クリアランスが二次腫瘍暴露に対する長期的な保護をもたらすことを示している。
MCMVの腫瘍内注射は腫瘍免疫微小環境を変化させる
腫瘍免疫微小環境に対する、ポリICの有無下、mCMV MHC−IおよびMHC−II制限ペプチドの腫瘍内注射の効果を、最後の腫瘍内処置の終了の2日後に、Nanostring Cancer immunology遺伝子セット(nCounter)を用いて、RNAサンプルにおいて免疫遺伝子発現について分析した。結果を、解析した各遺伝子セットのスコア変化としてまとめた。遺伝子セットによる発現の差のグローバルスコアは、生理食塩水処理群(群ごとにn=4)に対する相対的なスコアとした。評価された微小環境特性には以下のものが含まれた。B細胞機能、インターロイキン、TNFスーパーファミリー、抗原プロセシング、MHC、適応免疫、トランスポーター機能、接着性、NK細胞機能、T細胞機能、CD分子、白血球機能、補体経路、ミクログリア機能、液性免疫、TLR、炎症、樹状細胞機能、インターフェロン、自然免疫、マクロファージ機能、ケモカインおよび受容体、老化、アポトーシス、サイトカインおよび受容体、癌の進行、基礎細胞機能、細胞周期ならびに病原体応答。
mCMV感染は、インフレーション性(Inflationary)C57BL/6マウスのCD8+ T細胞応答を誘導する
C57Bl/6マウスに、5x103 pfuのマウスサイトメガロウイルス(mCMV)を感染させた。血液サンプルを、感染の1ヶ月後または5ヶ月後に採血した。インフレーション性(IE3)および非インフレーション性(m45)特異的CD8+ T細胞を、MHC−I四量体染色を用いたFACSにより検出した。図8に示すように、mCMV感染は、異なるエフェクターおよびメモリーCD8+ T細胞応答を誘導した。
mCMV感染は、C57BL/6マウスにおいて強力なCD8+およびCD4+ T細胞応答を誘導する
C57Bl/6マウスに、5x103 pfuのマウスサイトメガロウイルス(mCMV)を感染させた。感染後12日目に血液サンプルを採血した。脾臓細胞を、示されたペプチドおよびm38、m45、m57、m122、m139、m141およびm164 mCMVタンパク質から選択された免疫原性ペプチドの集団を含む血液細胞で再刺激した。CD4+およびCD8+ T細胞によるIFN−γ、TNF−αおよびIL−2サイトカイン産生を、細胞内サイトカイン染色により評価し、FACSにより分析した(図9A、9B)。これらの結果は、マウスサイトメガロウイルス感染が大規模なサイトカイン応答を誘導することを示す。
mCMV特異的CD8+ T細胞の組織分布
腫瘍保有マウスにおけるmCMV特異的CD8+ T細胞の分布を調べた。C57Bl/6マウスに、5x103 mCMVを感染させた。実験スケジュールを図10Aに示す。感染から4ヶ月後に、マウスに、E6およびE7腫瘍性タンパク質を発現する2x105 TC−1腫瘍細胞を皮下注射した。リンパ節、脾臓、唾液腺および腫瘍組織を採取し、インフレーション性(IE3;図10B)および非インフレーション性(m45;図10C)特異的CD8+ T細胞を、MHC-I四量体染色を用いたFACSにより検出した。常在性記憶T細胞マーカーの発現を、CD69およびCD103抗体を用いて評価した。これらの結果は、TC1腫瘍がmCMV特異的CD8+ T細胞によって浸潤されていることを示した。
腫瘍微小環境の遺伝子発現解析
マウスモデルの腫瘍細胞における遺伝子の発現を、生理食塩水;ポリI:C(PIC)(50μg);mCMV m139ペプチド(MHC−II制限/CD4)(CD4)(3μg);mCMV m38、m122、m45ペプチド(MHC−I制限/CD8)(CD8)(各1μg);mCMV m139+ポリI:C(PIC CD4)(各3μg);mCMV m38、m122、m45ペプチド(MHC−I制限/CD8)+ポリI:C(PIC CD8)(各1μg)による腫瘍内処置(各群4匹)後に調べた。腫瘍を、TC1腫瘍細胞を皮下に配置後、11週、13週および16週に3回処置した。実験プロトコルのタイムラインを、図11Aに示す。処置および腫瘍採取に続いて、QIACubeを用いて腫瘍RNAを抽出した。腫瘍細胞遺伝子発現を、tumor PanCancer Immune Profiling Panelの770個の遺伝子を形成する遺伝子転写物を測定するNanostring Cancer immunology遺伝子セット(NS_MM_CANCERIMM_C3400)を用いて分析した。簡潔には、正規化されたデータは、特定の生物学的プロセス(適応免疫、抗原プロセシング、T細胞機能、樹状細胞機能、NK細胞機能、インターフェロン、TNFスーパーファミリー遺伝子)内の遺伝子セット発現のヒートマップとして表される。生理食塩水処置に対する遺伝子発現変化のボルケーノプロットを構築する(プロットは、対照処置(生理食塩水)に対する処置群の変化(倍数増加または倍数減少で表される)を統計的有意性をもって表す)。細胞浸潤定量化アルゴリズムを適用する(CD45、細胞傷害性CD8、CD4 Th1、NK細胞、樹状細胞)。結果は、MHC−I制限/CD8およびMHC−I制限/CD8+ ポリ(I:C)処置動物において、グローバルな有意差スコアの最大変化を示した。
1)mCMV m139ペプチド:MHC−II制限/CD4 − 3mg(230個の遺伝子が上方制御され、4個の遺伝子が下方制御された)。
2)mCMV m38、IE3、m45ペプチド:MHC−I制限/CD8 − 1mg(359個の遺伝子が上方制御され、43個の遺伝子が下方制御された)。
3)mCMV m38、IE3、m45ペプチド:MHC−I制限/CD8+ ポリ(I:C)(309個の遺伝子が上方制御され、49個の遺伝子が下方制御された)。
mCMV CD8エピトープの腫瘍増殖を遅延せるmCMVの腫瘍内注射
C57Bl/6マウスに、5x103 pfuのマウスサイトメガロウイルス(mCMV)を感染させた。感染から4ヶ月後に、E6およびE7腫瘍性タンパク質を発現する2x105のTC−1腫瘍細胞を皮下注射した。腫瘍増殖を電子ノギスを用いて測定した。腫瘍を、11、13、16、18、21および23日目に、ポリ(I:C)(30μg)の有無下、選択されたMHC−I制限m38、m45およびm122ペプチド(各0.01μg、0.1μgまたは1μg)を、および対照として生理食塩水またはポリ(I:C)単独を、腫瘍内注射した。図12Aおよび12Bは、mCMV MHC−I制限ペプチドの腫瘍内注射が腫瘍増殖を遅延させ、ポリ(I:C)共注射が腫瘍制御を改善することを示す。
ポリ(I:C)を用いたmCMV MHC−Iおよび/またはMHC−IIペプチドの腫瘍内注射によるTC1およびMC38 腫瘍暴露からの保護
C57Bl/6マウスに、5x103 mCMVを感染させた。感染から4ヶ月後、マウスにE6およびE7腫瘍性タンパク質を発現する2x105のTC−1腫瘍細胞を皮下注射した。腫瘍増殖および生存をモニタリングした。腫瘍を、12日目から28日目まで、ポリ(I:C)(30μg)の有無下、MHC−I制限された選択されたm38、m45およびm122ぺプチド、および/またはMHC−II制限された選択されたm139ペプチド、ならびに対照として生理食塩水またはポリ(I:C)単独を、6回、腫瘍内注射した。群は、MHC−Iペプチドで6回、またはMHC−IIペプチドで6回、またはMHC−IおよびMHC−IIペプチドを共に6回、またはMHC−Iペプチドで3回、次いでMHC−IIペプチドで3回、またはMHC−IIペプチドで3回、次いでMHC−Iペプチドで3回、を順次注射した。図13Aは、mCMV MHC−IおよびMHC−II制限ペプチドの組合せの腫瘍内注射が腫瘍増殖を遅延させることを示し、図13Bは、CD4(MHC−II)mCMVエピトープ、次いでCD8(MHC−I)mCMVエピトープでの逐次的な腫瘍内接種が長期生存を促進することを示している。
処置後の、血中E7四量体陽性CD8+ T細胞応答
C57Bl/6マウスに、5x103 mCMVを感染させた。感染から4ヶ月後、マウスにE6およびE7腫瘍性タンパク質を発現する2x105のTC−1腫瘍細胞を皮下注射した。腫瘍の大きさを電子ノギスを用いて測定した。腫瘍を、12日目から28日目まで、ポリ(I:C)(30ug)の有無下、MHC−I制限された選択されたm38、m45およびm122ペプチド、および/またはMHC−II制限された選択されたm139ペプチド、ならびに対照として生理食塩水またはポリ(I:C)単独を、6回、腫瘍内注射した。すべてのペプチドをポリ(I:C)(30ug)と共に注射した。群は、MHC−Iペプチドで6回、またはMHC−IIペプチドで6回、またはMHC−IおよびMHC−IIペプチドを共に6回、またはMHC−Iペプチドで3回、次いでMHC−IIペプチドで3回、またはMHC−IIペプチドで3回、次いでMHC−Iペプチドで3回を順次注射した。血中のE7−、m45−、m122−特異的CD8+ T細胞応答を、各ペプチドのMHC−I四量対を用いてFACSにより分析した。図14は、mCMV CD4エピトープ、次いでCD8エピトープでの逐次的な腫瘍内接種が、優先的に抗腫瘍免疫を誘導することを示す。
二次腫瘍暴露に対する長期的な保護
上記のよう一次腫瘍暴露を生き延びた保護されたC57Bl/6マウスを、一次暴露の反対側の脇腹にE6およびE7腫瘍性タンパク質を発現する2x105 TC−1腫瘍細胞を皮下注射した。腫瘍増殖を電子ノギスを用いて測定した。腫瘍摂取の対照として、若齢(12週齢)のおよび年齢を合わせた(10ヶ月齢)マウスをTC−1腫瘍細胞で暴露した。図15は、一次腫瘍の完全クリアランスが二次腫瘍暴露に対する長期的な保護をもたらすことを示す。
ポリ(I:C)と共にmCMV MHC−IペプチドおよびMHC−IIペプチドの腫瘍内注射によるMC38腫瘍暴露からの保護
C57Bl/6マウスを5x103 mCMVに感染させた。感染の4ヶ月後に、マウスに、ハイパーミュテーションおよびマイクロサテライト不安定性を示するマウス結腸腺癌由来の5x105 MCM38腫瘍細胞を皮下注射した。腫瘍増殖をモニタリングした。腫瘍を、12日目から28日目まで、ポリ(I:C)(30μg)の有無下、MHC−I制限された選択されたm38、m45およびm122ペプチド、またはMHC−II制限された選択されたm139ペプチドのみ、ならびに対照として生理食塩水のみを、6回、腫瘍内注射した。図16は、一次腫瘍の完全なクリアランスが、二次腫瘍暴露に対する長期的な保護をもたらすことを示す。図16は、mCMV MHC−IおよびMHC−III制限ペプチドの組合せの腫瘍内注射が、腫瘍増殖を遅延させ、腫瘍クリアランスをもたらすことを示す。

- 癌が血液の癌である、請求項1から26のいずれか一項に記載の方法。
- 患者において癌を軽減するための、少なくとも1つのCMVペプチド抗原または該ペプチドをコードする核酸、薬学的に許容される担体、容器ならびに該CMVペプチドの投与について記載するパッケージ添付文書またはラベルを含む、個体における癌に対する既存の免疫応答を動員するためのキット。
- 患者を検査し、患者において癌の部位に対する既存の免疫応答を動員するための、キット。
