Abstract
Description

μθはマイクロキュリーを意味する。
DOTMPは、1,4,7,10−テトラアザシクロドデカンテトラメチレンホスホン酸を意味する。
EDTMPは、エチレンジアミンテトラメチレンホスホン酸を意味する。
eq.は、同等を意味する。
hrは、時間を意味する。
キットは、所定の指示に従い、放射性医薬品を調製するための放射性同位元素と組み合わされた、必要な成分を含むバイアルまたはバイアルを意味する。
FDAは、規制を含む米国食品医薬品局を意味する。
哺乳動物は、ヒトを含む温血動物を意味する。
min.は、分を意味する。
リン酸緩衝液は、医薬として許容される一リン酸塩緩衝液、二リン酸塩緩衝液、または三リン酸塩緩衝液またはそれらの混合物を意味する。
QCは、品質管理を意味する。
放射性同位元素は、Sm−153、Gd−159、Ho−166、Lu−177およびY−90を意味し、それらの冷同位体、放射性核種のサブセットを含む。
RCPは、特定の化学的または生物学的形態における特定の放射性核種の全活性の割合(%で表示)を意味する。
放射性同位元素−DOTMPキレート剤は、放射性同位元素−EDTMPキレート剤よりも熱力学的に安定であるので、放射性同位元素に対するDOTMPの比率は、EDTMPから放射性同位体に必要とされる最低約300:1ではなく、約1:1(最低でも3:1の保護手段として)程度に低くすることができる。この現在の製剤は、骨痛、1つ以上の石灰化した腫瘍を有する、または骨髄抑制を必要とする哺乳動物を治療するために使用される。
バイアル1は、DOTMP(10mg、18.2マイクロモル)および経験的に決定された量のNaOH(21.9mg)およびNaHCO3(10.7mg)(酸添加後に炭酸塩緩衝液になる)から調製された凍結乾燥混合物である;
バイアル1は、約4.0mLのSm−153(Sm−152などの他のSm同位体と共に存在するSmCl3溶液として)0.1N HCl中で再構成する;10mgのDOTMPあたりの総Sm質量は、1.38mg(6マイクロモル)を超えてはならない;次に、バイアルを反転させるか、または穏やかに振とうして溶液を撹拌して、すべての固体を溶解させる。
Sm−153−DOTMP溶液の活性は用量キャリブレーターを用いて測定する。
放射性同位体は、0.1N HCl溶液としてMissouri Research Reactor大学から購入した。
以下の実施例において、文字を付された実施例は比較例または手順であり、番号が付された実施例は本発明である。
10mLのバイアルに、2.5mLの以下の溶液を分配した:0.25Lの脱イオン水中の1.00gのDOTMP、0.135gのCa(OH)2および7.959gの10N NaOH。バイアルを凍結乾燥機に入れ、凍結乾燥した。これらのバイアル中のNaOHの量は、滴定実験を用いて経験的に決定した。これは、0.1N HCl中の5.0mLの放射性同位体溶液の体積を中和し、pHが9〜10になるように設計された。次に、これらの凍結乾燥バイアルを、以下の実施例に示すように使用した。
HCl中のSmは、1.9mgの非放射性Sm(NO3)3.6 3/40を微量のSm−153と共に6mLの0.1N HClに添加することによって調製した。これは、微量の活性を使用するが、はるかに高い臨床的に関連する用量に含まれるであろうSm金属の量を模倣するために行われた。凍結乾燥したCa−DOTMPバイアル(実施例Aのように調製)に、このSm溶液4mL(むしろ5mL)を添加した。これは、錯体形成に対するCaと組み合わせた高pHの効果を評価するために行った。得られたpHは11であり、所望のSm−DOTMP錯体は72%のRCPで示されるように部分的にのみ形成された。RCPは、1mLのセファデックスSPカラムに試料の小滴を添加することによって測定した。錯体化サマリウムを1〜2mLの画分で溶出したが、遊離の(非錯体化)Smはカラムに保持した。放射活性を用量キャリブレーターにおいて測定し、RCPは、溶離液およびカラムの全活性に対する、合わせた溶出液中の活性の単純な比として表される。
HCl中のSmは、Sm(N03)3.6 3/40 1.4mgと微量のSm−153を5.5mLの0.1N HClに添加することによって調製した。凍結乾燥したDOTMPバイアル(実施例Aと同様に調製)に、このSm溶液5.0mLを加えた。このSm−DOTMP溶液のpHは8であり、RCPは87%であった。これは、CaとpH8との組合せが錯体形成を妨げることを示す。これはまた、この製剤がpH範囲9〜10にあるように設計されているため、正確な量の酸を送達することによるpH制御が困難であることも示している。
40.4mgのDOTMP、5.2mgのCa(OH)2および318.6mgの10N NaOHからCa−DOTMP溶液を調製し、この容量を脱イオン水で全量を10mLとした。先の滴定実験に基づいて、この溶液2.5mL中のNaOHの量を計算し、0.1N HCl中の放射性同位体溶液5.0mLを中和し、pHを9〜10にした。
DOTMP溶液は、10mLの脱イオン水中の40.2mgのDOTMPおよび202.0mgの50%NaOHを用いて、カルシウムなしで調製した。先の滴定実験に基づいて、この溶液2.5mL中のNaOHの量を計算し、0.1N HCl中の放射性同位体溶液5.0mLを中和し、pHを9〜10にした。
この溶液2.5mLにSm溶液を加えてCa−DOTMP溶液(実施例Dと同様に調製)を試験した。Sm溶液は、Sm(N03)3・6H2Oの2.6mgと、0.1N HCl 4mL中の微量のSm−153とから調製した。全4mLを次に2.5mLのDOTMP溶液に添加した。そのpHは11であったので、追加の0.1N HClをpHが9に達するまでゆっくりと添加した.pHを調整するために約0.2mLが必要であった。所望のpHに調整したにもかかわらず、RCPはわずか88%であった。これは、所望のpH範囲でさえ、Caの存在が錯体形成を妨害することを示す。これはまた、追加された酸の総量4.2mLが予想される5.0mLよりも少ないので、この目標pH範囲を達成することは困難であることを再度示している。
DOTMP(10mg)を、20mLバイアル中の10N NaOH(72.6mg)および脱イオン水(1mL)に溶解した。この溶液を0.1N HClで滴定した。これらの結果を表1および図1に示す。これらの結果は、炭酸塩緩衝液の非存在下での再構成時に所望のpH範囲を達成することが困難であることを示している。
10mLのバイアルに、99.9mgのDOTMP、726.5mgの10N NaOH、および107.3mgのNaHCO3を25mLの脱イオン水に合わせて調製した2.5mLの溶液を分配した。バイアルを凍結乾燥機に入れ、凍結乾燥した。これらのバイアル中のNaOHの量は、滴定実験を用いて経験的に決定した。それは0.1NのHCl中の4.0mLの放射性同位体溶液の体積を中和し、9〜10のpH範囲で緩衝するように設計された。
炭酸塩を含むDOTMPの凍結乾燥バイアル(実施例1と同様に調製)を0.1N HClで滴定した。炭酸塩は、関心領域(pH9−10)で緩衝能を有することが示された(表2および図2参照)。図2は、錯体の最適な形成のための所望のpH範囲を灰色で強調するこの滴定曲線を示す。
実施例1のように調製したバイアルを、実施例Bと同様に調製した0.1N HCl中の微量Sm−153溶液4.0mLを加えることによって再構成した。pHは10であり、RCPは99.7%であった。このバイアルに、0.5mLの36.5mM CaCl2を添加した。このバイアル中の得られた溶液に、0.5mLの0.5Mのリン酸塩を加えた。pHは7であり、RCPは99.8%であった。
実施例1のように調製したバイアルを、実施例Bと同様にして調製した0.1N HCl中の微量Sm−153溶液4.0mLを加えることにより再構成した。pHは10であり、RCPは99.9%であった。このバイアルに、0.5mLの0.5Mのリン酸塩を加えた。pHは7であり、RCPは99.9%であった。
実施例3(w/Ca)および実施例4(w/out Ca)で調製した製剤をそれぞれ、3匹の雄Sprague−Dawleyラットの尾静脈に注射した。ラットの体重はそれぞれ230〜245gであった。2時間後、ラットを屠殺し、血液、心臓、肺、大腿、筋肉、肝臓、脾臓、腎臓、小腸、大腸、胃および脳の器官および組織を採取した。また、尿、糞便、および体の残りが集められた。これらの試料の各々における放射能を測定し、それぞれにおける注入された投与量(%ID)を計算した。結果を表3に示す。
このプロセスにおけるステップの理解を助けるために、以下のプロセススキームが提供される。すべてのプロセスは、製薬プロセスのためのFDAの要件、および滅菌条件の使用、ならびに放射性薬物および同位体の安全処理の要件に従って行われる。
バイアル1を0.1N HCl中のSm−153(SmCi溶液として)4mLで再構成する;10mgのDOTMPあたりの総Sm質量は1.38mgを超えてはならない。
次いで、バイアル1を反転させる(または穏やかに振とうして)溶液を攪拌し、すべての固体を溶解させる。
バイアル3は、第2緩衝液として0.5Mリン酸ナトリウム(pH7)を含有し、バイアル3からの溶液0.5mLをバイアル1に添加し、逆にして内容物を混合する。
Sm−153−DOTMP錯体溶液の活性は、用量キャリブレーターを用いて測定する。
Claims (8)
- 放射性同位体DOTMPキレートの医薬として許容される薬物製剤を調製するための使用説明書を含む、2成分緩衝系を有する少なくとも2つの成分を含む放射性骨探索剤を調製するための医薬として許容されるキット製剤であって、ここで、キットの成分は、1)凍結乾燥DOTMP、炭酸塩およびNaOHまたはKOH;2)場合により医薬として許容される水性溶媒中のカルシウム溶液;および3)医薬として許容される防腐剤、希釈剤および賦形剤を必要に応じて含む、pHが約7の医薬として許容される水性溶媒中のリン酸塩緩衝液である、上記キット製剤。
- HClまたはHNO3中のSm−153、Gd−159、Ho−166、Lu−177およびY−90からなる群から選択される放射性同位元素のさらなる追加成分が、成分1)に加えて、pHが9〜10に達成され、次に、残りの成分2)および3)をこの順に加えて薬剤を形成させる、請求項1に記載のキット。
- 放射性同位体がSm−153またはLu−177である、請求項2に記載のキット。
- 第2の成分であるカルシウムが省略されている、請求項1、2または3に記載のキット。
- 第2の成分であるカルシウムが存在する、請求項1、2または3に記載のキット。
- 使用説明書が、キットとともにパッケージインサートの一部として提供されるか、またはキット情報とともにオンラインで入手可能である、請求項1に記載のキット。
- 請求項1、2、3、4または5に記載されるキットを使用して医薬として許容される薬物製剤を調製する方法であって、
a)HClまたはHNO3中の放射性同位体を用いて、凍結乾燥成分1)、炭酸塩、DOTMPおよびNaOHまたはKOHを再構成する工程であって、ここで、炭酸塩は、放射性同位体−DOTMP錯体が少なくとも97%の初期RCPで容易に形成されるようにpHが約9〜10で緩衝する工程;
b)工程a)の放射性同位元素−DOTMPに任意に成分2)のカルシウム溶液を添加する工程;そして
c)pHを哺乳動物への注射に適した約7〜8にするために、工程a)またはb)の先に形成された溶液に成分3)のリン酸緩衝液を添加する工程
を含む、上記方法。 - 工程a)がSm−153またはLu−177である、請求項7に記載の方法。
