多発性硬化症(MS)のバイオマーカーとしてのインフラマソームタンパク質の検討
多発性硬化症(MS)は、脳および脊髄に影響を与える自己免疫疾患である。MSを有する患者のケアに重要なことは、疾患の開始、疾患の悪化および処置への応答を予測することができるバイオマーカーについての必要性である1。
参加者:
この研究において、120人の正常なドナーおよびMSと診断された32人の患者からの血清試料を分析した。試料は、BioreclamationIVTから購入した。正常なドナー群は、20〜70歳の年齢範囲の、60人の男性および60人の女性のドナーから得られた試料で構成された。MS群の年齢範囲は、24〜64歳の年齢範囲の患者から得られた試料で構成された(図4)。
血清中のインフラマソームタンパク質のASC、IL−1βおよびIL−18の濃度を、Simple PlexおよびSimple Plex Explorerソフトウェアを使用して分析した。示す結果は、三反復で行われたそれぞれの試料の平均に相当する。当技術分野において公知の任意のシステム/機器を体液中のタンパク質(例えば、インフラマソームタンパク質)のレベルを測定するために使用することができることに留意すべきである。
Prism7ソフトウェア(GraphPad)を、Simple Plex Explorerソフトウェアから得られたデータを分析するために使用した。群の間の比較を、異常値の特定、続いて、受診者動作特性(ROC)曲線下面積および95%信頼区間(CI)の決定の後に行った。使用した有意性のp値は<0.05であった。それぞれのバイオマーカーの感度および特異度を、異なるカットオフポイントの範囲について得た。アッセイの検出のレベルを下回るタンパク質値を与える試料は、その検体についての分析に含めなかった。
カスパーゼ−1、ASCおよびIL−18は、MS患者の血清中で上昇する
MS患者からの血清試料を分析し、インフラマソームシグナル伝達タンパク質のカスパーゼ−1、ASC、IL−1βおよびIL−18のタンパク質発現について、Simple Plexアッセイ(Protein Simple)を使用して、健康/対照の個体からの血清と比較した(図1A〜1D)。MS患者の血清中のカスパーゼ−1、ASCおよびIL−18のタンパク質レベルは、対照群より高かった。しかしながら、IL−1βのレベルは、MSにおいて対照よりも低かった。これらの知見は、MSの病理におけるインフラマソームについての役割を示す以前の報告と一致した6、8、11。
次いで、これらのインフラマソームシグナル伝達タンパク質がMSについての病理の信頼できるバイオマーカーである可能性があるかを決定するために、カスパーゼ−1(図2A)、ASC(図2B)、IL−1ベータ(図2C)およびIL−18(図2D)についての曲線下面積(AUC)を決定した。測定された3つのタンパク質のうち、ASCは、0.9448のAUCおよび0.9032〜0.9864の間のCI(表1)を有する最良のバイオマーカー(図3)であることを示した。加えて、0.848のAUCおよび0.703〜0.9929の間のCIを有するカスパーゼ−1も、有望なMSのバイオマーカーである。
この研究において、健康な対象と比較した場合に、統計学的に有意に高いIL−18のレベルがMS患者の血清中で検出された。加えて、患者のコホートにおけるIL−18についてのAUCは、0.6052〜0.8097の間のCIおよび84%の感度で0.7075であったが、しかしながら、特異度は、カットオフポイントが190.1pg/mlであった場合にたった44%であった。カットオフポイントが104.2pg/mlであった場合、感度は100%であったが、特異度はたった6.723%であった。同様に、カットオフポイントが427.2pg/mlであった場合、特異度は100%であったが、感度はたった15.63%であった。

脳卒中のバイオマーカーとしてのインフラマソームタンパク質の検討
序論
バイオマーカーは、客観的に測定することができ、正常または病理学的な生物学的プロセスの指標として評価することができる特徴である9。このため、脳卒中の文脈において、血液または他の体液中のバイオマーカーを、脳卒中の開始の指標として使用することができる。しかしながら、これまでに、脳卒中の診断および管理において通常使用される入手可能なバイオマーカーはない。この目的で、IL−10または腫瘍壊死因子などのサイトカイン、およびC反応性タンパク質、高移動度群ボックス−1または熱ショックタンパク質などの他の炎症性タンパク質は、脳卒中の患者におけるさらなるバイオマーカー分析のための可能性のある候補として考えられている10〜12。
参加者:この実施例において、80人の正常なドナーおよび脳卒中と診断された16人の患者からの血清試料を分析した。試料は、BioreclamationIVTから購入した。正常なドナー群は、46〜70歳の年齢範囲の、40人の男性および40人の女性ドナーから得られた試料で構成された。脳卒中群の年齢範囲は、46〜87歳の年齢範囲の患者から得られた試料で構成された(図11)。
血清キット(Invitrogen)からの総エキソソーム分離による:血清からの総エキソソーム分離を、製造者の使用説明書(Invitrogen)に従って使用した。簡潔には、100μlのそれぞれの試料を、2000×gで30分間遠心分離した。次いで、上清を、20μlの総エキソソーム分離試薬とともに4℃で30分間インキュベートし、続いて、10,000×gで10分間、室温で遠心分離した。上清を廃棄し、ペレットを50μlのPBSに再懸濁させた。
血清および血清由来EV中のカスパーゼ−1、ASC、IL−1βおよびIL−18のタンパク質濃度を決定するために、Simple Plexアッセイを行い、Simple Plex Explorerソフトウェアで分析した。示す結果は、三反復で行われたそれぞれの試料の平均に相当する。当技術分野において公知の任意のシステム/機器を体液中のタンパク質(例えば、インフラマソームタンパク質)のレベルを測定するために使用することができることに留意すべきである。
分離されたEV中のタンパク質濃度を定量化するために、Pierce Coomassie(Bradford)タンパク質アッセイキット(ThermoFisher Scienftific,Inc.)を、製造者の使用説明書に従って使用した。血清由来EVを、記載されているように、溶解緩衝液に溶解した(1:1希釈)6。
EVをNanoSight NS300(Malvern Instruments Company、Nanosight、およびMalvern、United Kingdom)によって分析した。分離されたエキソソームを分析のためのPBS(1:1000)に希釈し、次いで、3回の90秒のビデオを記録した。データを、最小バックグラウンドを維持しながら、可能な限り多くの粒子を追跡するために、それぞれの試料について最適化された検出閾値および10に設定されたスクリーニングゲインで、Nanosight NTA 2.3分析ソフトウェア(Malvern Instruments Company)を使用して分析した。少なくとも3つの独立した測定をそれぞれの分離された試料について行った。
分離されたEV中のインフラマソームシグナル伝達タンパク質の検出のために、EVを、タンパク質溶解緩衝液に再懸濁させ、15に記載されているように、免疫ブロット法によって分解した。簡潔には、ペレットの溶解の後、タンパク質を、NLRP3(Novus Biologicals)、カスパーゼ−1(Novus Biologicals)、ASC(Santa Cruz)、IL−1ベータ(Cell Signaling)、IL−18(Abcam)、CD81(Thermo Scientific)およびNCAM(Sigma)に対する抗体(1:1000希釈)を使用して、10〜20%のCriterion TGXステインフリープレキャストゲル(Bio−Rad)中で分解した。バンド密度の定量化を、UN−SCAN−IT gel 5.3ソフトウェア(Silk Scientific Corporation)を使用して行った。10μlの試料をロードした。膜中の化学発光基質(LumiGlo、Cell Signaling)を、ChemiDoc Touchイメージングシステム(BioRad)を使用してイメージングした。
Criterion TGXステインフリープレキャストゲル中の総タンパク質を、ChemiDoc Touchイメージングシステム(BioRad)を使用して、タンパク質の移動後に、ChemiDoc Touchのトレイにゲルを置くことによって、イメージングした。次いで、イメージをスクリーンにおいて調整して、ゲルの全体を示し、アプリケーションウィンドウにおいてステインフリーブロットの設定を実行した。
InvitrogenおよびExoQuickの分離手順の間の統計比較を、両側スチューデントのt検定を使用して行った。
EVを、formvar−炭素でコーティングされたグリッドにロードした。次いで、10μlの試料滴を、きれいなパラフィルム上に置き、グリッドを、30分間浮かせた(表を下にして)。その後のステップも、10μlの泡上にグリッドを浮かせることによって行った。次いで、EVがロードされたグリッドを、0.1MのMillonigのリン酸緩衝液(Electron Microscopy Sciences)で5分間すすいだ。過剰の液体を排出した。次いで、グリッドを、2%のグルタルアルデヒド中に5分間置いた。その後の洗浄を、7つの異なる泡上で、0.1MのMillonigのリン酸緩衝液で5分間、続いて、蒸留水で2分間を7回すすぐことによって行って、過剰のグルタルアルデヒドを除去した。次いで、グリッドを、0.4%の酢酸ウラニル溶液に5分間、移した。グリッドを、イメージングのために乾燥させた。イメージを、80kVの電圧でJoel JEM−1400透過型電子顕微鏡およびデジタルGatanカメラを用いて取得した。
データを、Prism7ソフトウェア(GraphPad)を使用して分析した。タンパク質レベルについての群の間の比較を、最初に異常値を特定すること、続いて片側t検定、次いでROC曲線下面積、ならびに95%の信頼区間およびp値を決定することによって行った(使用した有意性のp値は<0.05であった)。最後に、それぞれのバイオマーカーの感度、特異度、陽性予測値(PPV)、陰性予測値(NPV)および精度を、異なるカットオフポイントの範囲について得た。アッセイの検出のレベルを下回るタンパク質値を与える試料は、特定の検体についての分析に含めなかった。
カスパーゼ−1、ASCおよびIL−18は、脳卒中患者の血清中で上昇する:脳卒中患者および対照ドナーからの血清中のインフラマソームタンパク質のタンパク質レベルを決定するために、血清試料を、Simple Plexシステムを用いて分析した。カスパーゼ−1、ASCおよびIL−18のタンパク質レベルは、対照試料と比較した場合、脳卒中患者の血清中で高かったが、IL−1のレベルは有意差がなかった(図5A〜5D)。これらの知見は、インフラマソームが脳卒中後の炎症応答に関与することを示す以前のデータを確認する4、16。
この実施例において、ASCが脳卒中の開始の信頼できるバイオマーカーであることを示した。血清中のASCについての曲線下面積(AUC)は、0.9914〜1.004の間の信頼区間で0.9975であった。このAUC値は、この研究で分析された他のインフラマソームシグナル伝達タンパク質よりも高く:カスパーゼ−1(0.75)、IL−1ベータ(0.6111)およびIL−18(0.6675)、ASCが、この研究で調べられた他のインフラマソームタンパク質に対して優れたバイオマーカーであることを示す。ASCについてのカットオフポイントは、使用された試料のコホートで、100%の感度および96%の特異度で404.8pg/mlであった。重要なことには、AUCは、患者の小サブセットからの血清由来EV試料を分析すると、1に増加した。したがって、血清由来EV中のASCについてのカットオフポイントは、97.57pg/mlであることが見出された。

参加者:

血清およびCSF中のインフラマソームタンパク質のASC、IL−1βおよびIL−18の濃度を、Simple PlexおよびSimple Plex Explorerソフトウェアを使用して分析した。示す結果は、三反復で行われたそれぞれの試料の平均に相当する。当技術分野において公知の任意のシステム/機器を体液中のタンパク質(例えば、インフラマソームタンパク質)のレベルを測定するために使用することができることに留意すべきである。試料を、患者が病院に到着してから最初の5日間、1日3回採取した。試料を、1回目、2回目の採取(1日目)、ならびに4回目および6回目の採取(2日目)について分析した。

カスパーゼ−1およびASCは、TBI後の患者の血清中で上昇する
TBI患者からの血清試料を分析し、インフラマソームシグナル伝達タンパク質のカスパーゼ−1、ASC、IL−1βおよびIL−18のタンパク質発現について、Simple Plexアッセイ(Protein Simple)を使用して、健康/対照の個体からの血清と比較した(図15A〜15D)。TBI患者の血清中のカスパーゼ−1、ASCおよびIL−18のタンパク質レベルは、対照群より高かった。しかしながら、IL−1βのレベルは、TBIにおいて対照よりも低かった。
ASCおよびカスパーゼ−1は、TBIの良好な血清バイオマーカーである
TBI患者を、その臨床転帰に従って、6〜8のスコアを有する患者が、好ましい転帰を有すると考えられ、1〜4のスコアを有する患者が、好ましくない転帰を有すると考えられる、拡張グラスゴー転帰尺度(GOSE)に基づいて、好ましい転帰または好ましくない転帰のいずれかに分けた(表12Aおよび12B)。ASCのタンパク質レベルは、好ましい転帰を有する患者から得られた試料と比較した場合に、好ましくない転帰を有するTBI患者の血清中で高かったが(図19B)、カスパーゼ−1(図19A)およびIL−18(図19C)のレベルは、2つの群の間で統計的に相違しなかったことが見出された。
ASCをTBIの予後のバイオマーカーとして使用することができるかを決定するために、本発明者らは、2回目(図20A)および4回目(図20B)の採取でASCについてAUCを決定した。ASCについてのAUCは、4回目の採取において、0.7914〜1.042の間のCIで、0.9167であった(表12A)。さらにまた、カットオフポイントは、86%の感度および100%の特異度で547.6pg/mlであった(表12Bおよび表19(4回目の採取))。このように、これらの知見は、ASCが血清中のTBIの有望な予後のバイオマーカーであることを示した。
TBI患者からのCSF試料を分析し、インフラマソームシグナル伝達タンパク質のASCおよびIL−18のタンパク質発現について、Simple Plexアッセイ(Protein Simple)を使用して、健康/対照の個体からのCSFと比較した(図17Aおよび17B)。TBI患者の血清中のASCおよびIL−18のタンパク質レベルは両方とも、対照群におけるより高かった。
次いで、これらのインフラマソームシグナル伝達タンパク質がTBIの病理の信頼できるバイオマーカーである可能性があるかを決定するために、CSF中のASCおよびIL−18についての曲線下面積(AUC)(図18Aおよび18B)を決定した。ASCおよびIL−18は、それぞれ、1.0のAUC(6回目の採取)および0.84(1回目の採取)で(表13Aおよび13B)、最良のバイオマーカー(図18Aおよび18B)であることを示した。
この研究において、健康な対象と比較した場合に、TBIの患者の血清中で、統計学的に有意に高いASCおよびカスパーゼ−1のレベルが検出された。この研究において、本発明者らは、ASCおよびIL−18が、それぞれ1.0および0.84のAUC値で、CSF中のTBIについての信頼できるバイオマーカーであることを示す。最も重要なことには、CSFを得ることは非常に侵襲的な手順であるので、その結果、血清における本発明者らの知見は、典型的な臨床背景に対して、さらにいっそう適用可能である。したがって、本発明者らは、ASCについてのAUC値が0.90であり、カスパーゼ−1についてのAUC値が0.93であったことを見出した。このように、カスパーゼ−1およびASCは、脳傷害を有する患者のケアにおけるバイオマーカーとして考慮されるべきである。

この実施例において、試料はBioIVTから購入した。試料のドナーは、IRB番号20170439でSeraTrials,LLC.によって支援された「Prospective Collection of Samples for Research」という研究に登録された。ここで、50〜68歳の年齢範囲の72人の正常な男性および女性のドナーからの血清試料、ならびに56〜91歳の年齢範囲のMCIと診断された32人の男性および女性の患者(表20)からの血清試料を分析した。
MCIおよび年齢がマッチした対照からの血清試料中のインフラマソームタンパク質(カスパーゼ−1、ASC、IL−1βおよびIL−18)濃度の分析を、2、3に記載されるように、Ella System(Protein System)を使用して行った。
Simple Plexアッセイによって得られたデータを、Prism7ソフトウェア(GraphPad)を用いて分析した。最初に、異常値を除き、受診者動作特性(ROC)を計算し、このようにして、95%信頼区間、標準偏差およびp値を得る。有意性のp値は、0.05未満で考慮した。次いで、カットオフポイントを、異なる特異度および感度、ならびにそれらのそれぞれの尤度比の範囲について得た2、3。
ASCおよびIL−18は、MCIを有する患者の血清中で上昇する
MCI患者および年齢がマッチした健康なドナーからの血清試料を、ASC(図21A)、カスパーゼ−1(図21B)、IL−18(図21C)およびIL−1β(図21D)のタンパク質発現レベルについて分析した。ここで、ASCおよびIL−18のタンパク質レベルは、対照群と比較した場合に、MCI群で有意に高いことを見出し、このため、MCIの病理においてASCおよびIL−18の関わりを示唆する。
インフラマソームシグナル伝達タンパク質をMCIのバイオマーカーとして使用することができるかを決定するために、曲線下面積(AUC)を、カスパーゼ−1(図22A)、ASC(図22B)、IL−1β(図22C)およびIL−18(図22D)について決定した。図23は、互いに重ね合わされた図22A〜22DからのすべてのROC曲線を示す。分析されたすべてのタンパク質のうち、ASCは、最も高い0.974(p<0.0001)のAUC、続いて、IL−18は、0.6896(p=0.0025)のAUCを示した(表21)。ASCについてのカットオフポイントは、100%の感度および74%の特異度で264.9pg/mlであったが(表22および23を参照されたい);IL−18は、74%の感度および58%の特異度で213.9pg/mlのカットオフポイントを有していた(表22および25)。表22に加えて、カスパーゼ−1およびIL−1ベータについてのカットオフポイントおよび感度/特異度のデータは、それぞれ、表24および26に見ることができる。
