本発明は全体として、NaPi2bを標的とする抗体-重合体-薬物コンジュゲートによる処置に対する応答者と非応答者を層別化するための組成物および方法に関する。非小細胞肺がんをサブタイプ分類する方法も提供される。
複数回膜貫通型ナトリウム依存性リン酸輸送体であるNaPi2b (SLC34A2、NaPiIIb、Npt2) (Xu et al. Genomics 62:281-284 (1999)(非特許文献1))は、哺乳動物小腸の刷子縁膜に通常発現され、経細胞無機リン酸(Pi)吸収に関与し、体内のリン酸ホメオスタシスの維持に寄与している。タンパク質レベルでのNaPi2bの発現は、肝臓において、乳腺および唾液腺の上皮細胞の頂端表面で、ならびに肺、精巣、唾液腺、甲状腺、小腸および子宮において検出されている。NaPi2bの変異は、肺胞および精巣の微石症の臨床症候群に関連付けられている。NaPi2bは、非扁平上皮非小細胞肺がん(NSCLC)、非粘液性卵巣がんおよび甲状腺乳頭がんにおいて高度に発現される。NaPi2b陽性組織の免疫反応性は、NSCLCの61%、および卵巣がん標本の92%に存在する。

さまざまな局面において、本発明は、患者から得られた腫瘍サンプルにおけるNaPi2b発現の状態を測定する段階、および腫瘍サンプルにおけるNaPi2b発現に基づいて処置のために患者を特定する段階により、NaiPi2b標的化療法に適しているがん患者を特定するための方法を提供する。
本発明の性能、したがって本発明の絶対的および相対的な臨床的有用性は、上記のように複数の方法で評価されうる。さまざまな性能評価のうち、本発明は、臨床診断および予後診断における精度を提供することを意図している。診断、予測、または予後診断の試験、アッセイ法、または方法の精度は、NaPi2bレベルの「有効な量」または「有意な変化」を対象が有するかどうかに基づいて、化学療法処置に応答する対象と応答しない対象とを識別する、試験、アッセイ法、または方法の能力に関係する。「有効な量」または「有意な変化」とは、NaPi2b測定値が所定のカットオフポイント(または閾値)とは異なることを意味し、ゆえに、治療に対する対象の応答性、またはNaPi2bが決定要因である無病生存率/全生存率を示す。

処置のために選択されると、患者は、治療的に有効な量のNaPi2b抗体コンジュゲート(例えば、本明細書において記述されたまたはWO 2017/160754において開示されたNaPi2b標的化重合体抗体-薬物コンジュゲート)を投与される。
本発明は、患者サンプルにおけるNaPi2b発現のレベル(例えば、細胞あたりの抗原の数)を検出するキメラNaPi2b抗体(例えば、MERS67)と、本明細書において記述されたまたはWO 2017/160754において開示されたNaPi2b標的化重合体抗体-薬物コンジュゲートの有効量を含む治療組成物とを含むキットを提供する。必要に応じて、キットはさらに、NaPi2b発現のレベルを検出するためのおよびWO 2017/160754に開示されているNaPi2b標的化重合体抗体-薬物コンジュゲートが患者に投与された場合に有効であるかどうかを決定するための指示を含む。好ましくは、キットは免疫組織化学試験キットである。任意で、キットはさらに、本明細書において記述されているまたはWO 2017/160754において開示されているNaPi2b標的化重合体抗体-薬物コンジュゲートを、そのような処置を受けるように選択された患者に投与するための説明書を含んでもよい。

(1989))を参照されたい。本明細書において記述される分析化学、合成有機化学、ならびに医化学および薬化学に関連して用いられる名称、ならびにそれらの実験手順および技法は、当技術分野において周知であり、一般的に用いられる手順および技法である。化学合成、化学分析、薬剤調製、調合、ならびに患者への送達および処置に関して、標準的な技法が用いられる。
MERS67は、ヒト化抗NaPi2b抗体、XMT-1535(別名RebMab200 (Lopes dos Santos, 2013))に基づいて設計されたヒト-ウサギキメラ抗体であり、ウサギIgG1定常領域またはウサギIg Igκ-b4鎖C領域にそれぞれ結合したヒト重鎖および軽鎖可変配列からなる。標的化DNA配列は、ヒト化可変重鎖および軽鎖領域ならびにウサギ定常領域のアミノ酸配列からコドン最適化を用いて設計された。遺伝子合成、発現ベクター構築、プラスミド調製、および一過性発現は、GenScript(登録商標)で実施された。キメラ重鎖および軽鎖のDNA配列を図1に示す。産生のため、ExpiCHO-S細胞を無血清ExpiCHO(商標) Expression Medium (Thermo Fisher Scientific)中で増殖させた。細胞をオービタルシェーカー(VWR Scientific)上、8% CO2で37℃にてエルレンマイヤーフラスコ(Corning Inc., Acton, MA)中で維持した。トランスフェクションの1日前に、細胞を適切な密度でCorningエルレンマイヤーフラスコ中に播種した。トランスフェクションの日に、DNAとトランスフェクション試薬を最適な比率で混合し、その後、トランスフェクションの準備が整った細胞を有するフラスコに加えた。Mers67の重鎖および軽鎖をコードする組み換えプラスミドをExpiCHO-S細胞培養懸濁液に一過性に同時トランスフェクションした。残りの培養のためトランスフェクション後1日目に、細胞を、5% CO2を有する32℃のインキュベータに移した。エンハンサーおよびフィード(Thermo Fisher Scientific)を1日目に加え、フィードをトランスフェクション後5日目に加えた。14日目に収集された細胞培養上清を精製に用いた。
MERS67の結合を親抗体XMT-1535の結合と比較して評価した。
NaPi2bの細胞表面発現のレベルをフローサイトメトリーにより確認した。1×106個のOVCAR3またはJIMT-1細胞100 μlを5 μg/mlのフィコエリトリン結合RebMAB200とともに氷中で1時間インキュベートした。Quantibrite PE蛍光キャリブレーションキット(BD BioSciences)およびMACS Quant Instrument (Miltenyi Biotec, Bergisch Gladbach, Germany)を用い、OVCAR3およびJIMT-1それぞれの細胞表面受容体数は標準曲線に対して1.95×105および120であると計算された。
IHC用に開発された手動の方法を用いて、37例の腫瘍からなるヒト肺腫瘍組織マイクロアレイをMERS67で染色した。組織コアの21/37 (57%)は検出可能な膜免疫反応性を有していた。2つのヒト肺腺がんの免疫組織化学染色を図5に示す。
卵巣原発がん異種移植モデルは、漿液性卵巣がんまたは卵管がんに由来する一連の腫瘍モデルからのものであり、腫瘍が層別平均体積125〜250 mm3に達したら、WO 2017/160754に開示されているNaPi2b標的化重合体抗体-薬物コンジュゲート3 mg/kgで毎週1回3週間処置された免疫不全マウスにn=3の群で移植した。n=2〜4の群の未処置動物を対照として含めた。計画された試験エンドポイントは、1 cm3の腫瘍体積または45日であった。完全応答の場合、腫瘍の再成長を評価するために、より長い時間経過でマウスを追跡した。図6は、動物ごとに各時点で0日目に対して計算され、各モデルの最良応答の中央値として表された、最良応答の中央値を示す棒グラフである。y軸は最良応答の中央値を示し; x軸はモデル番号を示す。このモデルは、以前に処置を受けた患者と処置を実施しなかった患者との両方から導出された。NaPi2b標的化重合体抗体-薬物コンジュゲートの抗腫瘍効果が、両方の腫瘍クラスで見られた。バーは、処置未実施(暗色のバー)の腫瘍または処置後(明色のバー)の腫瘍に由来するように色分けされている。-50と-100の間の最良応答の中央値を示す10のモデルのうち、5つは処置未実施の腫瘍に由来し、5つは以前に処置された腫瘍に由来した。
肺がんの組織病理学的分類は形態学的特徴に基づいているが、正確な診断のために付随的な免疫組織化学染色を要することがある。非小細胞がんは、扁平上皮がん(SCC)および腺がん(ACA)を含むいくつかのサブタイプに分類することができる。特定の治療剤の使用は、異なる肺がんサブタイプにおいて適応される、または禁忌とされうる(例えば、Am J Surg Pathol., Volume 35, Number 1, January 2011を参照のこと)。現在、腫瘍を分類するために免疫組織化学染色のパネル、例えばサイトケラチン5/6、TTF-1、ナプシンA、p40およびp63が利用されている。MERS67により検出されるNaPi2bタンパク質は、肺ACAの前駆細胞でありうる細胞型(すなわちII型肺胞細胞)を含む正常肺組織における発現を有し、腺がんのマーカーでありうる。それは単一の染色マーカーとして利用されうるか、または現在の免疫組織化学パネルを改善するために使用されうる。
MERS67の免疫組織化学アッセイ法は、Leica BondRx Instrumentにて確立された。このアッセイ法は、NSCLCおよび小細胞肺がん(SCLC)細胞株アレイ、ならびにNSCLCヒト腫瘍アレイを含む、組織マイクロアレイ(TMA)にて実施された。NSCLCアレイの腫瘍はあらかじめ、形態学的特徴のみに基づいて分類されていた。全てのアレイは、Hスコア法に基づいてスコア化された。

Claims (38)
- a. 患者から得られた腫瘍サンプルを抗NaPi2b抗体と接触させることによって、腫瘍サンプルにNaPi2bが存在するかどうかを検出する段階;
b. NaPi2bと該抗体との間の結合を検出する段階; および
c. 腫瘍サンプルにおけるNaPi2bの存在が検出される場合に、患者が処置に対し応答性であることを予測する段階
を含む、NaPi2b標的化抗体薬物コンジュゲートによる処置に対するがん患者の応答性を予測する方法。 - a. 患者から得られた腫瘍サンプルを抗NaPi2b抗体と接触させることによって、腫瘍サンプルにNaPi2bが存在するかどうかを検出する段階;
b. NaPi2bと抗NaPi2b抗体との間の結合を検出する段階; および
c. 検出を病理学的にスコアリングする段階であって、病理スコアが、処置に対する応答性と相関する、段階
を含む、NaPi2b標的化抗体薬物コンジュゲートによる処置に対するがん患者の応答性を予測する方法。 - a. 患者から得られた腫瘍サンプルを抗NaPi2b抗体と接触させることによって、腫瘍サンプルにおけるNaPi2bの発現レベルを測定する段階; および
b. 腫瘍サンプルにおけるNaPi2bの発現レベルが所定のカットオフポイントよりも高い場合に、患者が処置に対し応答性であることを予測する段階
を含む、NaPi2b標的化抗体薬物コンジュゲートによる処置に対するがん患者の応答性を予測する方法。 - 応答性であると予測された対象にNaPi2b標的化抗体薬物コンジュゲートを投与する段階をさらに含む、前記請求項のいずれか一項記載の方法。
- a. 患者から得られた腫瘍サンプルを抗NaPi2b抗体と接触させることによって、腫瘍サンプルにNaPi2bが存在するかどうかを検出する段階;
b. NaPi2bと抗NaPi2b抗体との間の結合を検出する段階;
c. 腫瘍サンプルにおけるNaPi2bの存在が検出される場合に、患者がNaPi2b標的化抗体薬物コンジュゲートによる処置に対し応答性であることを予測する段階; および
d. 応答性であると予測された対象にNaPi2b標的化抗体薬物コンジュゲートを投与する段階
を含む、NaPi2b標的化抗体薬物コンジュゲートにより対象におけるがんを処置する方法。 - 予測する段階が、段階(b)の検出を病理学的にスコアリングすることによって達成され、病理スコアが、処置に対する応答性と相関する、請求項1、4または5記載の方法。
- 予測する段階が、腫瘍サンプルにおけるNaPi2bの発現レベルが所定のカットオフポイントよりも高いことを決定することによって達成される、請求項1、4または5記載の方法。
- 所定のカットオフポイントがHスコア法によって計算される、請求項3または7記載の方法。
- 病理スコアが定量的または半定量的スコアである、請求項2または6記載の方法。
- 病理スコアが光学顕微鏡または画像分析によって決定される、請求項2、6または9記載の方法。
- がんがNaPi2b発現がんである、前記請求項のいずれか一項記載の方法。
- NaPi2b発現がんが、肺がん、卵巣がん、乳がん、結腸直腸がん、腎臓がん、甲状腺がん、腎がん、唾液管腺がん、子宮内膜がん、胆管がん、甲状腺乳頭がんまたは乳頭腎がんである、請求項11記載の方法。
- 肺がんが非小細胞肺がん(NSCLC)である、請求項12記載の方法。
- NSCLCが非扁平NSCLCである、請求項13記載の方法。
- NSCLCが腺がんである、請求項13記載の方法。
- 卵巣がんが上皮性卵巣がんである、請求項12記載の方法。

を含む、前記請求項のいずれか一項記載の方法。 - 非小細胞肺がんサンプルを抗NaPi2b抗体と接触させることによって、サンプルにNaPi2bが存在するかどうかを検出する段階;
NaPi2bと該抗体との間の結合を検出する段階;および
サンプルにおけるNaPi2bの存在が検出される場合に、非小細胞肺がんを腺がんとしてサブタイプ分類する段階
を含む、非小細胞肺がんを腺がんとしてサブタイプ分類する方法。 - 非小細胞肺がんサンプルにおいてTTF-1、ナプシンA、p63、p40またはCK5/6の1つまたは複数を検出する段階をさらに含む、請求項18記載の方法。
- 検出する段階が免疫組織化学的に実施される、前記請求項のいずれか一項記載の方法。
