Abstract
Description
(a)サイズが約14μm〜64μmの大型の異型倍数体核、又は単一細胞における複数の核、
(b)サイズが約20μm〜300μmの細胞サイズ、並びに、
(c)紡錘形、オタマジャクシ形、円形、楕円形(oblong)、二本足、三本足以上(more than two legs)、細い足(thin legs)、及び無定形からなる群から選択される形態学的形状、
のそれぞれを有するものとして定義され得る。
(e)CD45発現、
(f)EpCAM発現、
(g)ビメンチン発現、
(h)PD−L1発現、
(i)単球CD11Cマーカー発現、
(j)内皮CD146マーカー発現、
(k)内皮CD202bマーカー発現、及び、
(l)内皮CD31マーカー発現、
の1つ以上を有するものとして更に定義され得る。
本明細書で定義されるように、癌腫と関連するCTCは、多くのサイトケラチン(CK)を発現する。CK8、CK18及びCK19は、最も一般に発現され、診断に使用されるサイトケラチンであるが、測量をこれらのマーカーだけに限定する必要はない。固形腫瘍CTCの表面は、通常、上皮細胞接着分子(EpCAM)を発現する。しかしながら、この発現は均一すなわち一貫したものではない。CD45は白血球マーカーであるため、CTCはCD45を全く発現しない。CTC及びCAML等の腫瘍関連細胞を識別するアッセイでは、CK8、CK18及びCK19等の固形腫瘍と関連するマーカーに対する抗体、又はCD45若しくはDAPIに対する抗体を使用することで十分である。染色技術と形態学とを組み合わせることにより、病理学的に定義可能なCTC(PDCTC)、アポトーシスCTC及びCAMLが識別され得る[3]。
DAPIによって染色される「癌様」核。核は、通常ドットパターンを有し大きいが、細胞が分裂状態にある場合には例外として、核が圧縮されることもある。
CK8、CK18及びCK19の1つ以上の発現;上皮癌に由来するCTCは、通常少なくともCK8、CK18及びCK19を発現する。サイトケラチンは線維状パターンを有する。

本明細書で定義されるように、本発明の方法で使用される循環細胞は「CAML」と称される場合がある。「循環細胞」に対するそれぞれの言及はCAMLと同じ意味であり、「CAML」に対するそれぞれの言及は循環細胞と同じ意味である。CAMLと称されるか「循環細胞」と称されるかにかかわらず、これらの細胞は、以下の特徴の1つ以上を有することを特徴とする:
CAMLは、拡大した融合核小体が一般的であるものの、大型の異型倍数体核又は細胞内にしばしば点在する複数の個別の核を有する。CAML核は、一般的には直径約10μm〜約70μmのサイズ、より一般的には直径約14μm〜約64μmのサイズの範囲である。
多くの癌に対して、CAMLは、その疾患の癌マーカーを発現する。例えば、上皮癌と関連するCAMLはCK8、CK18又はCK19、ビメンチン等を発現し得る。マーカーは、典型的には拡散されているか、又は液胞及び/又は摂取された物質と結び付いている。任意のマーカーに対する染色パターンは、細胞全体の至る所でほぼ均一に広がる。肉腫、神経芽細胞腫及び黒色腫については、CK8、CK18、CK19に代えて、癌と関連するその他マーカーを使用することができる。
CAMLはCD45陽性又はCD45陰性の場合があり、本発明は両方の種類のCAMLの使用を包含する。
CAMLは、大きく、最長寸法でおよそ20ミクロン〜およそ300ミクロンのサイズである。
CAMLは、紡錘形、オタマジャクシ形、円形、楕円形、二本足、三本以上の足、細い足、及び無定形の形状を含む、多くの明らかに異なる形態学的形状で見られる。
癌腫に由来するCAMLは、典型的には拡散したサイトケラチンを有する。
CAMLがEpCAMを発現する場合、EpCAMは典型的には細胞の至る所に拡散されているか、又は液胞及び/又は摂取された物質と結び付いており、細胞全体を通してほぼ均一であるが、いくつかの腫瘍はEpCAMの発現が非常に低いか又はそれを発現しないことから、全てのCAMLがEpCAMを発現するわけではない。
CAMLがマーカーを発現する場合、マーカーは典型的には細胞の至る所に拡散されるか、又は液胞及び/又は摂取された物質と一体化されており、細胞全体を通してほぼ均一であるが、全てのCAMLが等しい強度で同じマーカーを発現するわけではない。
CAMLは、しばしば、腫瘍起源のマーカーと関連するマーカーを発現する。例えば、腫瘍が前立腺癌起源であり、PSMAを発現する場合には、かかる患者に由来するCAMLもまたPSMAを発現する。別の例として、原発腫瘍が膵臓起源であり、PDX−1を発現する場合には、かかる患者に由来するCAMLもまたPDX−1を発現する。更なる例として、原発腫瘍又は癌起源のCTCがCXCR−4を発現する場合、かかる患者に由来するCAMLもCXCR−4を発現する。
原発腫瘍又は癌を起源とするCTCが薬物標的のバイオマーカーを発現する場合、かかる患者に由来するCAMLもまた薬物標的のバイオマーカーを発現する。免疫療法のかかるバイオマーカーの一例は、PD−L1である。
CAMLは、単球マーカー(例えばCD11c、CD14)及び内皮細胞マーカー(例えばCD146、CD202b、CD31)を発現する。
CAMLは、Fcフラグメントを結合する能力を有する。
上の概要で示唆されるように、本発明は、循環細胞サイズに基づいて、癌を有する被験体の全生存期間(OS)及び無増悪生存期間(PFS)を予測する方法に関する。これらの方法は、癌を有する第1の被験体に由来する生体試料中の循環細胞のサイズを決定することと、癌を有する第2の被験体と結果を比較することとを含む。より大型の細胞を有する被験体のOS及びPFSは、より小型の細胞を有する被験体のOS及びPFSより短いと予測される。
また、本発明は、循環細胞数に基づいて癌を有する被験体のOS及びPFSを予測する方法に関する。これらの方法は、癌を有する被験体に由来する生体試料の選択された体積中の循環細胞の比を決定することを含み、該比が生体試料7.5mL当たり約4個以上の細胞と等しい場合、上記被験体のOS及びPFSは、生体試料7.5mL当たり4個未満の細胞と等しい比を持つ癌を有する被験体よりも短いと予測される。
本発明は、さらに、循環細胞において検出されるCEP17ドットの数に基づいて、癌を有する被験体のOS及びPFSを予測する方法に関する。これらの方法は、癌を有する被験体に由来する生体試料中の循環細胞におけるCEP17ドットの数を決定することを含み、上記試料中の少なくとも1個の細胞が約8以上のCEP17ドットを有する場合、上記被験体のOS及びPFSは、被験体に由来する生体試料中のどの細胞も約8以上のCEP17ドットを有しない癌を有する被験体よりも短いと予測される。
(a)サイズが約14μm〜64μmの大型の異型倍数体核、又は同一細胞における複数の核、
(b)サイズが約20μm〜300μmの細胞サイズ、並びに、
(c)紡錘形、オタマジャクシ形、円形、楕円形、二本足、三本足以上、細い足、及び無定形からなる群から選択される形態学的形状、
のそれぞれを有するものとして定義され得る。
(e)CD45発現、
(f)EpCAM発現、
(g)ビメンチン発現、
(h)PD−L1発現、
(i)単球CD11Cマーカー発現、
(j)内皮CD146マーカー発現、
(k)内皮CD202bマーカー発現、及び、
(l)内皮CD31マーカー発現、
の1つ以上を有するものとして更に定義され得る。
本明細書で提供され、以下に概説される各実施例では、末梢血をCellSaveチューブ(カリフォルニア州サンディエゴのMenarini Silicon Biosystems Inc.)に収集し、96時間以内に処理した。CellSieve(商標)精密濾過技術を使用して、血液試料中の全ての癌関連細胞(CTC、EMT、CEC及びCAML)を収集した。CellSieve(商標)マイクロフィルタは、9mmの区域内に、160000個超の、均一な配列の、孔径7μmの細孔を有する。試薬は、前固定バッファー、後固定バッファー、透過処理バッファー及び抗体カクテルを含む。濾過を行う技術は、5mL/分で引くように設定されたシリンジポンプ[6]又は真空ポンプ[4]のいずれかを使用した。フィルタを通って引く前に、前固定バッファー7.5mL中で血液7.5mLを前固定することによって濾過プロセスを開始させた。次いで、フィルタ及び捕捉された細胞を、洗浄、後固定、洗浄、透過処理及び洗浄に供した。次いで、フィルタ上に捕捉された細胞を、FITC抗サイトケラチン8、18、19と、フィコエリトリン(PE)複合化EpCAMと、Cy5−抗CD45(5)とからなる抗体カクテルで染色した後、洗浄した。フィルタをスライドガラス上に置き、Fluoromount−G/DAPI(Southern Biotech)で封入した(cover-slipped:カバーガラスにて封入した)。特定の蛍光キューブ及び白黒カメラを使用して細胞を撮像するため、Carl Zeiss AxioCamを備えるオリンパス株式会社のBX54WI蛍光顕微鏡を使用した。細胞間の均等なシグナル比較のため、曝露を3秒(Cy5)、2秒(PE)、100ミリ秒〜750ミリ秒(FITC)及び10ミリ秒〜50ミリ秒(DAPI)としてプリセットした。Zen2011 Blue(Carl Zeiss)を使用して画像を処理した。
PDCTCは、癌の初期段階ではめったに見られない。PDCTCがステージIII及びステージIVの乳癌及び前立腺癌の患者でより頻繁に見られるが、その他の大半の固形腫瘍で見られる頻度は低い。CAMLはより一般的に見られる。
図3A及び図3Bは、図2からの試料に対するステージに基づいてCAMLの数及びサイズを分析した。ステージが増加するにつれて、CAMLの平均数は増加する。端から端まで25μm〜50μmのサイズを有するCAMLの平均数は、全てのステージでほぼ同じである。しかしながら、サイズが50μm〜100μm及びサイズが100μm超のCAMLの平均数は、図3Bに示されるように、ステージと共に増加する。
CAMLサイズの増加を、腫瘍細胞及び腫瘍物質のCAML貪食の結果であると仮定した。実際に、実験により、貪食された核の数の指標である染色体計数プローブ17(CEP17)のドット数が、細胞のサイズと十分相関することが実証された。図4は、サイズが25μm〜50μmのCAMLよりも、サイズが50μm超のCAMLはより多くのCEP17のドットを有することを示す。また、データは、50μmのCAMLサイズが、図3Bに提示される疾患進行に関する結果と一致する、境界線であることを示唆する。
図3及び図4は、侵攻性疾患を区別するため50μmが使用され得ることを示唆する。末梢全血7.5mLを、CellSieve(商標)精密濾過アッセイを使用して分析した。様々な癌及びステージに由来する、24カ月に亘って追跡した157名の患者の探索セットを、OS(図5中、点線の曲線)及び無増悪生存期間(図6中、点線の曲線)について分析した。OSに対するハザード比は3.4であり、p<0.001で95%CI=2.1〜5.6である。PFSに対するハザード比は3.2であり、p<0.001で95%CI=2.1〜5.0である。これは、50μm以上のCAMLサイズを有する患者は、より短いPFS及びOSを有することを示す。
図5及び図6に示されるデータに基づいて、OS及びPFSに対する多変量解析を、上に記載されるn=293患者試料に対して行った。患者を24カ月に亘って追跡した。下記表に示されるように、多変量解析を行った。50μmのCAMLサイズは、OS及びPFSを決定する最も重要なパラメーターであることを示す。p値が小さいほど、統計が正確である確率が高い。

Claims (19)
- 癌を有する被験体の全生存期間(OS)及び無増悪生存期間(PFS)を予測する方法であって、前記方法が、癌を有する第1の被験体に由来する生体試料中の循環細胞のサイズを決定することと、癌を有する第2の被験体と結果を比較することとを含み、より大型の細胞を有する被験体のOS及びPFSは、より小型の細胞を有する被験体のOS及びPSFよりも短いと予測される、方法。
- 癌を有する被験体のOS及びPFSを予測する方法であって、前記方法が、癌を有する被験体に由来する生体試料中の循環細胞のサイズを決定することを含み、前記試料中の少なくとも1個の細胞のサイズが約50μm以上である場合に、前記被験体のOS及びPFSは、どの細胞のサイズも約50μmを超えない癌を有する被験体よりも短いと予測される、方法。
- 癌を有する被験体のOS及びPFSを予測する方法であって、前記方法が、癌を有する被験体に由来する生体試料の選択された体積中の循環細胞の比を決定することを含み、該比が生体試料7.5mL当たり約6個以上の細胞と等しい場合、前記被験体のOS及びPFSは、生体試料7.5mL当たり6個未満の細胞と等しい比を持つ癌を有する被験体よりも短いと予測される、方法。
- 癌を有する被験体のOS及びPFSを予測する方法であって、前記方法が、癌を有する被験体に由来する生体試料中の循環細胞におけるCEP17ドットの数を決定することを含み、前記試料中の少なくとも1個の細胞が約10以上のCEP17ドットを有する場合、前記被験体のOS及びPFSは、被験体に由来する生体試料中のどの細胞も約10以上のCEP17ドットを有しない癌を有する被験体よりも短いと予測される、方法。
- 前記OS及び/又はPFSが少なくとも12カ月の期間に及ぶ、請求項1〜4のいずれか一項に記載の方法。
- 前記OS及び/又はPFSが少なくとも24カ月の期間に及ぶ、請求項1〜4のいずれか一項に記載の方法。
- 前記生体試料のサイズが5mL〜15mLである、請求項1〜4のいずれか一項に記載の方法。
- 前記循環細胞が、以下の特徴:
(a)サイズが約14μm〜64μmの大型の異型倍数体核、又は単一細胞における複数の核、
(b)サイズが約20μm〜300μmの細胞サイズ、並びに、
(c)紡錘形、オタマジャクシ形、円形、楕円形、二本足、三本足以上、細い足、及び無定形からなる群から選択される形態学的形状、
を有する、請求項1〜4のいずれか一項に記載の方法。 - 前記循環細胞が、以下の追加の特徴:
(d)CD14陽性表現型、
(e)CD45発現、
(f)EpCAM発現、
(g)ビメンチン発現、
(h)PD−L1発現、
(i)単球CD11Cマーカー発現、
(j)内皮CD146マーカー発現、
(k)内皮CD202bマーカー発現、及び、
(l)内皮CD31マーカー発現、
の1つ以上を有する、請求項8に記載の方法。 - 前記生体試料の供給源が、末梢血、血液、リンパ節、骨髄、脳脊髄液、組織、及び尿の1つ以上である、請求項1〜4のいずれか一項に記載の方法。
- 前記生体試料が、前肘静脈血、下大静脈血、大腿静脈血、門脈血又は頸静脈血である、請求項10に記載の方法。
- 前記癌が、固形腫瘍、ステージIの癌、ステージIIの癌、ステージIIIの癌、ステージIVの癌、癌腫、肉腫、神経芽細胞腫、黒色腫、上皮細胞癌、乳癌、前立腺癌、肺癌、膵臓癌、結腸直腸癌、肝臓癌、頭頸部癌、腎臓癌、卵巣癌、食道癌、又はその他の固形腫瘍癌である、請求項1〜4のいずれか一項に記載の方法。
- 循環細胞が、サイズ排除法、免疫捕獲、赤血球溶解、白血球枯渇、FICOLL、電気泳動、誘電泳動、フローサイトメトリー、磁気浮上、及び様々なマイクロ流体チップ、又はそれらの組み合わせからなる群から選択される1つ以上の手段を使用して、前記決定工程のため前記生体試料から単離される、請求項1〜4のいずれか一項に記載の方法。
- 循環細胞が、マイクロフィルタを使用することを含むサイズ排除法を用いて、前記生体試料から単離される、請求項13に記載の方法。
- 前記マイクロフィルタが約5ミクロン〜約20ミクロンの範囲の孔径を有する、請求項14に記載の方法。
- 前記マイクロフィルタの細孔が、円形、レーストラック形、楕円形、正方形、及び長方形の細孔形状を有する、請求項15に記載の方法。
- 前記マイクロフィルタが、精密な細孔形状及び均一な細孔分布を有する、請求項15に記載の方法。
- 循環細胞が、物理的なサイズに基づく選別、流体力学的なサイズに基づく選別、分類、捕捉、免疫捕獲、大きな細胞の濃縮、又はサイズに基づく小さな細胞の排除によってマイクロ流体チップを使用して単離される、請求項13に記載の方法。
- 循環細胞が、CellSieve(商標)低圧精密濾過アッセイを使用して前記決定工程のため前記生体試料から単離される、請求項1〜4のいずれか一項に記載の方法。
