(viii)(2R,3’S)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩;
(ix)(2R,1’R)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩;および
(x)(2R,1’S)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩
からなる群より選択される少なくとも1つの化合物、および(b)非毒性の眼科的に許容される担体を含む、近視の治療における使用のための眼科用組成物であって、近視を患う被験体の目に少なくとも週に1回局所投与する、特に、その進行を遅延させるために、近視を患う約6〜約18歳の年齢の小児に少なくとも週に1回、少なくとも1年の期間投与する場合の、眼科用組成物を提供する。
(1)その組成物を、日に1または2回、週に1〜7日間投与する;
(2)その投与を、少なくとも2年の期間実施する;
(3)その組成物中の少なくとも1つの化合物は、(2R,3’R)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩である;
(4)その化合物は、組成物中に約0.2%w/v〜約1%w/vの量で存在する。
N−Substituted Soft Anticholinergics」、Pharmaceutical Research、22巻、12号、2035〜2044頁(2005年9月26日オンラインで入手可能)、Kluwer Academic、Plenum Publishers、U.S.も参照されたい。上記特許文献は、化合物の合成および分割を記載しており、また、薬理学的テストデータも含んでいる。
目的
(2R)SGAとも称される化合物(ii)の散瞳活性を調査し、小児における可能性のある近視を治療するために、ヒトの目におけるその薬理学的効果および特徴を評価した。
同じ2人のヒト被験体(40〜50歳)において局所投与した後、代表的な双性イオン(2R)SGAとトロピカミドの散瞳効果を比較した。100μLアリコートの1%(w/v)化合物溶液を、各被験体のそれぞれの目に投与した。実験は、明かりのもと、温度制御された部屋で実施した。適切な時間間隔で、各被験体のそれぞれの目の瞳孔直径を記録した。各時間点とゼロ時間点の間の瞳孔直径の差を計算し、散瞳応答[%での(治療された対照)/対照]として報告した。散瞳応答−時間曲線下の面積(AUCeff)を台形公式で計算し、テストした化合物の活性と持続時間を比較するのに使用した。各被験体のそれぞれの目において、生理食塩水(100μL)を使用して対照研究も実施し、「瞳孔拡張がない」と確認された。
局所投与後、代表的な双性イオン(2R)SGAの効力および持続時間を、最も高い頻度で使用されている散瞳剤であるトロピカミドのそれと比較した。2人のヒト被験体それぞれのそれぞれの目での100μLの局所投与に続いて、瞳孔サイズを周期的に測定し、最大散瞳効果(Emax、瞳孔サイズの変化%)および散瞳応答−時間曲線下面積(AUCeff 0〜168h)を決定し比較した(表1)。双性イオン性(2R)SGAは、トロピカミドと類似した良好な局所散瞳活性をもたらした。活性の発現はより遅かったが[(2R)SGAの3時間対トロピカミドの0.5時間]、作用の持続時間は、トロピカミドの場合よりはるかに長かった。(2R)SGAの回復時間、すなわち、治療された目における瞳孔のサイズが対照の1mm未満の範囲内に回復するのに要した期間は、トロピカミド(被験体1および被験体2について、それぞれ8時間および3.5時間)と比較して、被験体1および被験体2について、それぞれ、およそ120時間および36時間であった。ヒトの目において、(2R)SGA[化合物(ii)]は、不快な感覚を引き起こすことはなかった。これらの結果を、(2R)SGAでのウサギ研究における以前の報告(一方の目に投与した場合、反対側の治療されていない目における瞳孔拡張がないこと;および体循環からの急速な排出)に加えて、代表的な化合物(ii)の安全な特徴が確認された。さらに、より低い用量(例えば、0.5%w/vまたはそれ未満)が、小児の近視の進行を遅延させるのに特に有利であると示唆される。
pA2によって特徴付けられるような固有の抗コリン作用活性に関して非常にそれが弱いので(以前報告されているように)、目に適用した場合、双性イオンについてそうした高くかつ長期に及ぶ散瞳活性が見られることは驚くべきことである。さらに、静脈内で投与した場合、双性イオンは非常に急速に排出されるが、それでも、目に点滴した場合、長い活性を有する。これは、「動力学的選択性」、目の中の受容体への強くかつ長期に及ぶ結合を示唆しているが、それでも、それは副作用なし(未治療の反対側の目では活性なし)で急速に排出される。したがって、驚くべきことに、「不活性代謝物」(対応するソフト型エステルと比較して、pA2における少なくとも10倍の減少)は、目において、強力なソフトまたはハード型類似物(グリコピロレート)に匹敵するかまたはより高い活性を有する。それ以上代謝されないので、不活性代謝物は、実際は理想的な「ハード型薬物」である。
臨床研究を、小児の近視の進行を遅延させるそれらの能力について1つまたは複数の選択された双性イオン[化合物(ii)または(vii)など]の眼科用製剤の効能を評価するために提起する。それぞれの選択された双性イオンの2つの濃度、1.0%または0.5%をテストした。以下で説明するような患者に、2日ごとに、それぞれの目に2滴のテスト製剤を施す。治療を2年間継続し、次いで、近視の進行に対する効果を評価するために、患者はもう2年間追跡される。開始ベースラインが得られ、評価を、4、8、12、16、20および24カ月の治療の間に行うことになる。治療が終了した後のフォローアップ期間の間、評価を6カ月の間隔で行うことになる。

からなる群より選択される少なくとも1つの化合物の使用であって、眼科用医薬組成物を、近視を患う約6〜約18歳の年齢の小児の目に少なくとも週に1回、少なくとも1年の期間、局所的に投与することにより前記小児における近視の進行を遅延させることにおける、前記少なくとも1つの化合物を含む前記眼科用医薬組成物の調製における、使用。
(項目2)
前記組成物を、日に1または2回、週に1〜7日間投与する、項目1に記載の使用。
(項目3)
少なくとも2年の期間実施する、項目1または2に記載の使用。
(項目4)
前記少なくとも1つの化合物が、(2R,3’R)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩である、項目1から3のいずれか一項に記載の使用。
(項目5)
前記化合物が、約0.2%w/v〜約1%w/vの量で前記組成物中に存在する、項目1から4のいずれか一項に記載の使用。
(項目6)
有効量の:
(a)(2R,3’R)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩;
(b)(2R,3’S)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩;

からなる群より選択される少なくとも1つの化合物の使用であって、眼科用医薬組成物を、近視を患う被験体の目に少なくとも週に1回、局所的に投与することにより前記被験体における近視を治療することにおける、前記少なくとも1つの化合物を含む前記眼科用医薬組成物の調製における、使用。

前記組成物を、日に1または2回、週に1〜7日間投与する、項目6に記載の使用。
(項目8)
前記少なくとも1つの化合物が、(2R,3’R)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩である、項目6または7に記載の使用。

(項目10)
近視を患う約6〜約18歳の年齢の小児における近視の進行を遅延させるための方法であって、有効量の:
(a)(2R,3’R)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩;
(b)(2R,3’S)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩;
(c)(2R,1’R)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩;および
(d)(2R,1’S)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩
からなる群より選択される少なくとも1つの化合物を前記小児の目に少なくとも週に1回、少なくとも1年の期間局所投与するステップを含み、
前記少なくとも1つの化合物を、任意選択で、前記有効量の前記化合物および非毒性の眼科的に許容される担体を含む眼科用組成物の形態で投与する、方法。
(項目11)
前記化合物を、日に1または2回、週に1〜7日間投与する、項目10に記載の方法。
(項目12)

近視を患う被験体における近視を治療するための方法であって、有効量の:
(a)(2R,3’R)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩;
(b)(2R,3’S)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩;
(c)(2R,1’R)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩;および
(d)(2R,1’S)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩
からなる群より選択される少なくとも1つの化合物を前記被験体の目に少なくとも週に1回局所投与するステップを含み、
前記少なくとも1つの化合物を、任意選択で、前記有効量の前記化合物および非毒性の眼科的に許容される担体を含む眼科用組成物の形態で投与する、方法。

前記化合物を、日に1または2回、週に1〜7日間投与する、項目15に記載の方法。

(i)有効量の:
(a)(2R,3’R)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩;
(b)(2R,3’S)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩;
(c)(2R,1’R)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩;および
(d)(2R,1’S)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩
からなる群より選択される少なくとも1つの化合物、および
(ii)非毒性の眼科的に許容される担体
を含む、近視の治療における使用のための眼科用組成物であって、近視を患う被験体の目に少なくとも週に1回局所投与される、眼科用組成物。
(項目20)
近視を患う約6〜約18歳の年齢の小児に、その進行を遅延させるために、少なくとも週に1回、少なくとも1年の期間投与する、項目19に記載の使用のための組成物。
(項目21)
日に1または2回、週に1〜7日間投与する、項目19または20に記載の使用のための組成物。
(項目22)
少なくとも2年の期間実施する、項目19から21のいずれか一項に記載の使用のための組成物。
(項目23)
前記少なくとも1つの化合物が、(2R,3’R)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩である、項目19から22のいずれか一項に記載の使用のための組成物。
(項目24)
前記化合物が、約0.2%w/v〜約1%w/vの量で前記組成物中に存在する、項目19から23のいずれか一項に記載の使用のための組成物。
Claims (14)
- 近視を患う被験体の目に少なくとも週に1回、局所的に投与することにより前記被験体における近視の進行を遅延させることにおいて使用するための、有効量の:
(a)(2R,3’R)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩;
(b)(2R,3’S)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩;
(c)(2R,1’R)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩;および
(d)(2R,1’S)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩
からなる群より選択される少なくとも1つの化合物の、前記少なくとも1つの化合物を含む眼科用医薬組成物の調製における使用。 - 前記組成物が、日に1または2回、週に1〜7日間投与される、請求項1に記載の使用。
- 前記少なくとも1つの化合物が、(2R,3’R)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩である、請求項1または2に記載の使用。
- 前記化合物が、約0.2%w/v〜約1%w/vの量で前記組成物中に存在する、請求項1から3のいずれか一項に記載の使用。
- 近視を患う被験体における近視の進行を遅延させるための、
(a)(2R,3’R)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩;
(b)(2R,3’S)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩;
(c)(2R,1’R)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩;および
(d)(2R,1’S)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩
からなる群より選択される少なくとも1つの化合物を含む組成物であって、前記組成物は、前記被験体の目に少なくとも週に1回局所投与され、
前記組成物は、任意選択で、前記少なくとも1つの化合物および非毒性の眼科的に許容される担体を含む眼科用組成物の形態で投与されることを特徴とする、組成物。 - 前記組成物が、日に1または2回、週に1〜7日間投与されることを特徴とする、請求項5に記載の組成物。
- 前記少なくとも1つの化合物が、(2R,3’R)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩である、請求項5または6に記載の組成物。
- 前記化合物が、約0.2%w/v〜約1%w/vの量で前記組成物中に存在する、請求項5から7のいずれか一項に記載の組成物。
- (i)有効量の:
(a)(2R,3’R)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩;
(b)(2R,3’S)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩;
(c)(2R,1’R)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩;および
(d)(2R,1’S)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩
からなる群より選択される少なくとも1つの化合物、および
(ii)非毒性の眼科的に許容される担体
を含む、近視の進行を遅延させることにおける使用のための眼科用組成物であって、前記組成物が近視を患う被験体の目に少なくとも週に1回局所投与される、眼科用組成物。 - 前記組成物が、近視を患う約6〜約18歳の年齢の小児に、その進行を遅延させるために、少なくとも週に1回、少なくとも1年の期間投与されることを特徴とする、請求項9に記載の使用のための組成物。
- 前記組成物が、日に1または2回、週に1〜7日間投与される、請求項9または10に記載の使用のための組成物。
- 前記投与が少なくとも2年の期間実施されることを特徴とする、請求項9から11のいずれか一項に記載の使用のための組成物。
- 前記少なくとも1つの化合物が、(2R,3’R)3−(2−シクロペンチル−2−フェニル−2−ヒドロキシアセトキシ)−1−(カルボキシメチル)−1−メチルピロリジニウム分子内塩である、請求項9から12のいずれか一項に記載の使用のための組成物。
- 前記化合物が、約0.2%w/v〜約1%w/vの量で前記組成物中に存在する、請求項9から13のいずれか一項に記載の使用のための組成物。
