Abstract
Description
本発明は、治療方法に関し、本発明は、より具体的には、少なくとも一つの、(置換フェニル)プロペナール部分を有する化合物を用いた、神経変性性障害の治療のための方法、およびその使用を提供する。
特定の天然物が、治療効果を持つ可能性があることはよく知られているため、多くの文化(漢方薬や他の多くの民間伝承薬など)で、ヒトの病気の治療と予防に使用されている。
このような治療の有効性により、製薬産業は、これらの天然物から、活性化合物を探して単離し、さまざまな疾患または病状の治療および予防のための治療薬または予防薬として有効成分を開発している。
したがって、多くの一般的に使用される医薬品が、天然物から、開発されているか、生じている。

しかし、クルクミンもその類似体も、人間の病気を治療する治療薬として開発されていない。
これは、本来の形態のクルクミンが、おそらく、治療薬への開発に最適な分子ではないことを示す。
ポリQ疾患は、無関係な遺伝子の翻訳領域における、CAGトリヌクレオチドの繰り返しの病理学的伸長によって特徴付けられる。
翻訳されたポリグルタミンが伸長された(polyQ−expanded)タンパク質は、特定のニューロン亜集団の機能障害と変性につながる変性ニューロンにおいて、コンフォメーションのミスフォールドと凝集をされる(Fan H.C.、Ho L.I.、Chi C.S.、Chen S.J.、Peng G.S.、Chan T.M.、Lin S.Z.、Harn H.J.、ポリグルタミン(ポリQ)疾患:遺伝学から治療へ(genetics to treatments)、Cell Transplant.23(4−5):441−458、2014)。

担体または賦形剤には、限定ではなく例示として、緩衝剤、希釈剤、崩壊剤、結合剤、接着剤、湿潤剤、ポリマー、潤滑剤、流動促進剤、不快な味または臭気をマスクまたは打ち消すために添加される物質、香味料、色素、香料、および組成物の外観を改善するために添加される物質が含まれ得る。
許容される担体または賦形剤には、クエン酸緩衝液、リン酸緩衝液、酢酸緩衝液、重炭酸緩衝液、ステアリン酸、ステアリン酸マグネシウム、酸化マグネシウム、リン酸および硫酸のナトリウムおよびカルシウム塩、炭酸マグネシウム、タルク、ゼラチン、アカシアゴム、アルギン酸ナトリウム、ペクチン、デキストリン、マンニトール、ソルビトール、乳糖、ショ糖、デンプン、ゼラチン、セルロース系材料(アルカン酸のセルロースエステルやセルロースアルキルエステルなど)、低融点ワックスココアバター、アミノ酸、尿素、アルコール、アスコルビン酸、リン脂質、タンパク質(たとえば、血清アルブミン)、エチレンジアミン四酢酸(EDTA)、ジメチルスルホキシド(DMSO)、塩化ナトリウムまたはその他の塩、リポソーム、マンニトール、ソルビトール、グリセロールまたは粉末、ポリマー(ポリビニルピロリドン、ポリビニルアルコール、ポリエチレングリコールなど)、および他の薬学的に許容される材料が含まれる。
担体は、治療薬の薬理活性を破壊してはならず、治療量の治療薬を送達するのに十分な用量で投与される場合、無毒でなければならない。
適切な経路、製剤および投与スケジュールは、当業者によって決定され得る。
本発明において、医薬組成物は、液剤、懸濁剤、乳剤、シロップ剤、錠剤、丸剤、カプセル剤、徐放剤、粉末、顆粒、アンプル、注射、注入、キット、軟膏、ローション、リニメント、クリームまたはそれらの組み合わせなどの対応する投与経路に従って、様々な方法で製剤化することができる。
必要に応じて、それらの多くは当業者に知られている、滅菌されてもよく、または任意の薬学的に許容される担体または賦形剤と混合されてもよい。

基剤の性質に応じて使用できる増粘剤およびゲル化剤には、軟パラフィン、ステアリン酸アルミニウム、セトステアリルアルコール、ポリエチレングリコール、ウール脂肪、蜜蝋、カルボキシポリメチレンおよびセルロース誘導体、および/またはモノステアリン酸グリセリルおよび/または非イオン性乳化剤が含まれる。
滴剤は、1つまたは複数の分散剤、可溶化剤、懸濁剤または防腐剤も含む、水性または非水性基剤で製剤化することができる。
吸入に適したエアロゾル組成物は、懸濁液または溶液のいずれであってもよい。
エアロゾル組成物は、所望により、オレイン酸またはレシチンなどの界面活性剤およびエタノールなどの共溶媒などの、当技術分野で周知の追加の製剤賦形剤を含んでもよい。
接着剤リザーバーシステムにより、連続的または長期の送達を実現できる。
そのような水性製剤は、好ましくは、水性ゲルまたはヒドロゲル製剤の形態である。
ヒドロゲル製剤は、液体溶液を増粘する増粘剤を含む。
増粘剤の例には、カルボマー、セルロース基材、ゴム、アルギン、寒天、ペクチン、カラギーナン、ゼラチン、ミネラルまたは改質ミネラル増粘剤、ポリエチレングリコールおよび多価アルコール、ポリアクリルアミド、その他のポリマー増粘剤が含まれ、これらに限定されない。

ミスフォールディングされたタンパク質とROSの誘導は、ポリQ疾患の神経細胞を支配的に損傷する(Bertoni A1,Giuliano P、Galgani M、Rotoli D、Ulianich L、Adornetto A、Santillo MR、Porcellini A、Avvedimento VE.、Early and late events induced by polyQ−expanded proteins:identification of a common pathogenic property of polyQ−expanded proteins(ポリQ伸長タンパク質によって誘発される初期および後期イベント:ポリQ伸長タンパク質の共通の病原性特性の特定)、J Biol Chem.、11;286(6):4727−41.2011)。
(置換フェニル)−プロペナール部分を含む化合物は、細胞内の酸化還元恒常性を調節する酸化還元レギュレーターである。
タンパク質合成中、影響を受けた遺伝子における伸長されたCAGの繰り返しは、ポリグルタミン鎖(polyglutamine tracts)として知られているものを形成する一連の連続したグルタミン残基に翻訳される。
このような伸長されたポリグルタミン鎖は、タンパク質の構造に著しい変化を引き起こし、次いで、機能の喪失、凝集の増加、および/または下流のシグナル伝達経路の異常な活性化を引き起こす可能性がある(Klockgether T、Mariotti C、Paulson HL.、脊髄小脳性運動失調(Spinocerebellar ataxia)、Nat Rev Dis Primers.2019;5(1):24)。
抑制されない場合、これらの細胞内事象は、小脳萎縮を伴う小脳プルキンエニューロンに顕著な損傷を引き起こす。
脊髄、脳幹神経節、および脳幹の橋核などの、神経系の他の部分も関与する可能性がある。
ミトコンドリアの機能障害は、この病気に罹っているすべての人の4分の1で検出されている(Bargiela D、Shanmugarajah P、Lo Cら、 Mitochondrial pathology in progressive cerebellar ataxia(進行性小脳性運動失調症におけるミトコンドリアの病理)、Cerebellum Ataxias.2015;2:16−16)。
ミトコンドリアの病理を示唆する組織化学的変化は、遺伝的に確認されたSCA14、SCA28およびSCA35を有する患者の筋肉生検では明ならかではなかったが、ウォードらによる最近の報告は、SCA7患者、マウスおよびヒト幹細胞由来の神経における、変更された代謝およびミトコンドリア機能障害を実証している(Ward JM、Stoyas CA、Switonski PMら、Metabolic and Organelle Morphology Defects in Mice and Human Patients Define Spinocerebellar Ataxia Type 7 as a Mitochondrial Disease(マウスおよびヒト患者の代謝およびオルガネラ形態学的欠陥は、脊髄小脳運動失調型7型をミトコンドリア病として定義している)、Cell reports.2019;26(5):1189−1202.e1186)。
一般に、変異タンパク質の凝集が神経変性を引き起こすメカニズムは、ほとんど知られていないが、疾患の病因はmHTTのタンパク質分解に起因する可能性があり、その結果、ニューロンに異常で毒性のあるタンパク質凝集が生じる。
これらの凝集体はシャペロンを誘導してユビキチン化されるが、それらは持続し、タンパク質のミスフォールディングとタンパク質分解の失敗を示す。
mHTT凝集体の蓄積は、シャペロン、プロテアソームタンパク質、通常のHTTおよび転写因子などの他の重要な細胞タンパク質を隔離(sequester)するため、これはタンパク質の恒常性に影響を与える。
証拠は、mHTTと転写因子との直接的な相互作用が転写機能不全を引き起こす欠陥であることを示唆する。

さらに、ミトコンドリア複合体II、III、およびIVの欠陥は、死後のHD脳の線条体でも観察された(Bott LC、Badders NM、Chen K−Lら、A small−molecule Nrf1 and Nrf2 activator mitigates polyglutamine toxicity in spinal and bulbar muscular atrophy(低分子Nrf1およびNrf2アクチベーターは、脊髄および延髄の筋萎縮におけるポリグルタミン毒性を緩和する).Human molecular genetics.2016;25(10):1979−1989)。
好ましくは、(置換フェニル)ープロペナール部分を有する化合物が、Nrf2の抗酸化経路を活性化し、抗酸化酵素の発現を増加する。
具体的には、(置換フェニル)ープロペナール部分を有する化合物は、抗酸化酵素HO−1、Nqo1、Gclcおよびカタラーゼの発現を増加する。
Nrf2の発現をノックダウンすると、(置換フェニル)ープロペナール部分を有する化合物に対する応答において、HO−1およびNqo1の誘導を妨げる。
筋萎縮性側索硬化症(ALS)、アルツハイマー病(AD)、パーキンソン病(PD)、多系統萎縮症(MSA)、レビー小体病、前頭側頭型認知症(Frontotemporal dementia)、皮質基底核変性(CBD)、脊髄小脳性運動失調(SCA)、脊髄性筋萎縮(SMA)、運動ニューロン疾患、プリオン病または進行性核上性麻痺(PSP)。
実施例
実施例1:脊髄小脳失調症モデル
JM17は、ニューロンSH−SY5Y細胞のポリQ凝集を抑制する
このネットワークは、複数のシグナル伝達経路によって制御され、外因性および内因性ストレッサーに応答する高分子および細胞小器官への損傷を最小限に抑える(Klockgether T、Mariotti C、Paulson HL.、Spinocerebellar ataxia(脊髄小脳性運動失調)Nat Rev Dis Primers.2019;5(1):24)。
これらの細胞を、JM17で24時間処理し、そして、フィルター処理された遅延アッセイと定量的イムノブロッティングによりポリQ凝集をアッセイした。
図1A‐1Bに示すように、ヒト集団における最も一般的な伸長を有するアタキシン−2を発現する細胞(22Qの伸長)は、もしあったとしても、ほとんど、測定可能な凝集を有しない。
しかし、アタキシン−2凝集体は、108Qの伸長を持つアタキシン−2を発現する細胞では、容易に検出できる。
さらに、図1Bに要約された、これらの実験の結果は、JM17は、用量依存的に、108Qの伸長凝集を有するアタキシン‐2の形成を減少させることを実証する。
JM17は、in vitroモデルで、SCA3における機能不全のミトコンドリアを修復する。
このシステムでは、SK−N−SH神経芽細胞腫細胞に、78のグルタミン残基を含む緑色蛍光タンパク質(GFP)タグ付き完全長ATXN3(ataxin−3−78Q−GFP)(以降、MJD78細胞と呼ぶ。)を過剰発現する構造体をトランスフェクトした。
実験では、テトラメチルローダミンエチルエステル(TMRE)染色のフローサイトメトリー分析により、JM17処理の有無において、野生型およびMJD78細胞における、ミトコンドリア活性を評価した。
JM17の細胞毒性効果は、死細胞を染色するためにヨウ化プロピジウム(PI)を使用して測定された。
野生型およびMJD78細胞の両方は、0.1、0.3、0.5、0.7、0.9および1μMのJM17で、24時間処理した。結果は、野生型もMJD78細胞も、いずれも、このような処理に続いて、細胞毒性の兆候を有していないことが示された(図2A)。
しかし、MJD78細胞には反対の効果があり、同じ用量のJM17で処理すると、DMSO処理のMJD78細胞と比較して、ミトコンドリア膜電位が2〜2.5倍増加した(図2B)。
JM17は、SCA2動物モデルの運動協調を改善する。
このモデルは十分に特徴付けられており、若い動物では運動機能は正常であるが、年齢とともに徐々に低下するという点でSCAヒト表現型を再現することが示されている。
実験マウス(グループあたり9匹と7匹のマウス)に、JM17を経口投与した。
毎日マウスの体重の10分の1の食事摂取量を想定して、6週齢から、毎日適切な量のJM17を、低用量40mg/kgと高用量120mg/kgで標準的なげっ歯類の食事に完全に混合された。
年齢と体重を実験マウスに合わせた9匹のSCA2トランスジーン動物で構成される対照群には、プラセボが投与された。
別の対照群は、野生型マウスに匹敵する年齢/体重の11匹の動物で構成された。
9週目から、ロータロッド試験を加速することにより、すべてのマウスの運動協調を、週に2回評価した。
これらの評価は、マウスが40週齢に達するまで続けられた。
図3は、回転盤上の保持時間として表されたロータロッド試験の結果を示す。
予想通り、約17週齢で、SCA2マウスは運動能力を失い始めたが、運動能力はその後悪化し続けた。
重要なことに、野生型マウスに似た対照群では、運動機能の測定可能な損失はなかった。
SCA2導入遺伝子マウスのこの表現型は、SCA患者の運動協調の年齢依存性の喪失と一致しているため、SCAにおけるJM17の有効性を評価するために使用される動物モデルであることを検証した。
さらに、1日あたり40mg/kgで、JM17を投与されたSCA2マウスは、SCA2対照マウスと比較して、ロータロッド試験のパフォーマンスが大幅に改善されることを実証した。
まず、40mg/kg/日のJM17治療は、SCAの臨床症状の発現を遅らせた(約15−17週から約22−24週まで)。
第二に、低下率はJM17によって妨げられ、そして、処理された動物は40週目でも未処理の対照群よりも有意に良好に機能した(図3、p<0.001、2元配置分散分析)。
JM17を1日あたり120mg/kgで投与したグループはまた、20週から26週の間の時間枠で、SCAの進行の遅延を示し、対照群よりも優れたパフォーマンスを示したが、この利点はマウスの加齢とともに失われた(図3)。
追加の研究は、最適な治療レジメンを見つけるのに役立つが、現在入手可能なデータを総合すると、JM17治療が疾患の検証されたモデルにおける運動機能のSCA関連損失の進行を遅らせることがわかる。
まず、JM17は、ポリQトラックを持つタンパク質の毒性凝集を減少させた。
第二に、実験的なin vitroSCA3細胞モデルは、機能不全のミトコンドリア活性の改善に対するJM17の肯定的な効果を示す。
最後に、SCA動物モデルでの実験は、JM17治療が、加齢に伴う運動機能の喪失の臨床的に意味のある評価によって測定されるように、疾患の進行を減らすという説得力のある証拠を提供する。
JM17は、HDの動物モデルでの有効性を実証する。
この障害は、ハンチンチン遺伝子のCAGトリヌクレオチド反復配列の伸長によって引き起こされ、そして、変化したタンパク質は線条体の中型有棘ニューロンの損失につながると仮定されている(Frank S.Treatment of Huntington’s disease(ハンチントン病の治療).Neurotherapeutics.2014;11(1):153−160)。
HDの最初のトランスジェニックモデルとして、R6/2マウスには、HTTのN末端フラグメント(エクソン1)が含まれており、エクソン1で、約144−150のCAGが繰り返されている。
それは、進行性の均質なHDのような表現型を示し、生存期間は14〜21週間である。
最近の発見は、このモデルが、人間のHDに、より密接に対応する、進行性のHDのような行動および神経病理学的表現型を示すことを示唆しているため、R6/2マウスがHDの潜在的な治療法をテストするための適切なモデルであるというサポートを提供する(Ferrante RJ.Mouse Models of Huntington’s Disease and Methodological Considerations for Therapeutic Trials(ハンチントン疾患及び治療試験のための方法論的な考慮事項)、Biochimica et biophysica acta.2009;1792(6):506−520)。

主な調査結果は、以下のとおりである。
マウスを、a)運動機能/自発運動、およびb)脳容積について試験した。
HDでは、特に線条体および皮質領域の運動機能の喪失および脳容積の減少が、疾患の進行の顕著な特徴である(Frank S.Treatment of Huntington’s disease(ハンチントン病の治療).Neurotherapeutics.2014;11(1):153−160)。
体重、回転ロッドにとどまる能力(ロータロッド試験)、および運動活動は、5〜14週齢において、毎週記録された。
脳容積についてのMRIは、13.5週齢で分析された。
HD治療におけるJM17の有効性の追加測定として、脳容積を治療終了時(13.5週間)にMRIで測定した。
JM17治療群の線条体と皮質の平均容積は、ビヒクル対照群と比較して増加した(図6)。
そのようなすべての代替、修飾、および変形は、本発明の範囲内にあるとみなされる。
Claims (12)
- 神経変性性障害を、治療を必要とする対象において治療する方法であって、前記対象に、有効量のVIIIの化合物および所望により、薬学的に許容される担体または賦形剤を含む医薬組成物を投与することを含む:
R18およびR28は、モノ−またはジ−置換された基であり、および、メトキシ基、ヒドロキシル基、およびアルキルスルホニル基からなる群から、独立して、選択され;
