Abstract
Description
i)前記対象から得た生体試料中の6種の遺伝子の発現レベル及び2種の臨床的パラメータによって複合寛容スコア(cSoT)を確立する段階であって、
前記cSoTが次式:
ii)このcSoTを既定の基準値と比較する段階と、
iii)cSoTが既定の基準値よりも高い場合には対象がTOLであると結論を下し、又はcSoTが既定の基準値よりも低い場合には対象がSTAであると結論を下す段階を含む方法に関する。
i)本発明に係る移植後に免疫寛容の(TOL)対象と移植後に免疫寛容でない(STA)対象とを識別するための方法を用いて、対象がTOL対象であるか又はSTA対象であるかを決定する段階と、
ii)対象がSTAである場合には対象を免疫抑制療法で治療する段階を含む方法に関する。
i)本発明に係る移植後に免疫寛容の(TOL)対象と移植後に免疫寛容でない(STA)対象とを識別するための方法を用いて、対象がTOL対象であるか又はSTA対象であるかを決定する段階と、
ii)対象がTOLである場合には対象が免疫抑制療法の断薬又は最小化に適格であると結論を下す段階を含む方法にも関する。
本発明では、以下の用語は以下の意味を有する。
i)前記対象から得た生体試料中の少なくとも2種、少なくとも3種、少なくとも4種、少なくとも5種、少なくとも6種又はそれ以上の遺伝子の発現レベル、及び少なくとも1種、少なくとも2種又はそれ以上の臨床的パラメータによって複合寛容スコア(cSoT)を確立する段階であって、前記スコアが次式:
ii)前記cSoTを既定の基準値と比較する段階と、
iii)前記cSoTが既定の基準値よりも高い場合には対象がTOLであると結論を下し、又は前記cSoTが既定の基準値よりも低い場合には対象がSTAであると結論を下す段階を含む方法に関する。
i)前記対象から得た生体試料中のID3、AKR1C3、CD40、CTLA4、TCL1A及びMZB1から成る群から選択される、少なくとも2種、少なくとも3種、少なくとも4種、少なくとも5種、少なくとも6種又はそれ以上の遺伝子の発現レベル、及び試験時の前記対象の年齢及び移植時の対象の年齢から成る群から選択される、少なくとも1種、少なくとも2種又はそれ以上の臨床的パラメータによって複合寛容スコア(cSoT)を確立する段階であって、前記スコアが次式:
ii)前記cSoTを既定の基準値と比較する段階と、
iii)前記cSoTが既定の基準値よりも高い場合には対象がTOLであると結論を下し、又は前記cSoTが既定の基準値よりも低い場合には対象がSTAであると結論を下す段階を含む。
i)前記対象から得た生体試料中の6種の遺伝子ID3、AKR1C3、CD40、CTLA4、TCL1A、MZB1の発現レベル、及び試験時の前記対象の年齢及び移植時の対象の年齢から選択される2種の臨床的パラメータによって複合寛容スコア(cSoT)を確立する段階であって、前記スコアが次式:
ii)前記cSoTを既定の基準値と比較する段階と、
iii)前記cSoTが既定の基準値よりも高い場合には対象がTOLであると結論を下し、又は前記cSoTが既定の基準値よりも低い場合には対象がSTAであると結論を下す段階を含む。
i)対象が本発明に係る移植後に免疫寛容の(TOL)対象であるか又は移植後に免疫寛容でない(STA)対象であるかどうかを決定する段階と、
ii)対象がSTAである場合には1種以上の免疫抑制薬で対象を治療する段階を含む方法にも関する。
i)対象が本発明に係る移植後に免疫寛容の(TOL)対象であるか又は移植後に免疫寛容でない(STA)対象であるかどうかを決定する段階と、
ii)対象がTOLである場合には対象が免疫抑制療法の断薬又は最小化に適格であると結論を下す段階を含む方法にも関する。
材料&方法
メタ分析データセット
遺伝子発現データセットは、既に記述されている遺伝子発現オムニバス(GEO)データベース(受託番号GSE28456)から得た[18]。このデータセットは596種の試料を収集した5つの独立した研究からのメタ分析の結果であった[14、15、21、50、51]。
公的に入手可能な3種のマイクロアレイデータセットをGEO:GSE14630[34]、GSE22224[35]及びGSE45593[8]から収集し、Rソフトウェアのaffyパッケージ[52]を用いてロバストマルチアレイ平均法(RMA)で正規化した。データセットGSE45218[33]から収集した正規化した発現値をコンピュータによる交差検証に使用した。全てのデータセットについて、目的の6種の遺伝子の遺伝子発現を中心化/スケーリングした後、cSoTの係数を適用した。
ナントの病院からさらに21名の腎臓レシピエント(9名のTOLと12名のSTAを含む)を登録してqPCR検証を行った。地域の倫理委員会は本研究の全ての側面を承認し、全ての患者から書面によるインフォームド・コンセントを得た。
静脈血試料をEDTAバキュテナーに採取し、4時間以内に分析のために処理した。末梢血単核球(PBMC)をフィコール層(Eurobio、フランス、レ・ジュリス)上で分離し、TRIzol(受託商標)試薬(Thermo Fisher Scientific、米国マサチューセッツ州ウォルサム)にて−80℃で凍結した。TRIzol法(Thermo Fisher Scientific)を用いて末梢血からRNAを抽出した。RNAの質と量を、それぞれAgilent 2100 BioAnalyzer(米国カリフォルニア州パロアルト)とNanodrop(Labtech、フランス、パレゾー)を使用して決定した。ポリdTオリゴヌクレオチド及びモロニー白血病ウイルス逆転写(Thermo Fisher Scientific)を用いてRNAを逆転写した。6種の試験遺伝子(AKR1C3:Hs00366267_m1、CD40:Hs00374176_m1、CTLA4:Hs00175480_m1、ID3:Hs00171409_m1、MZB1(又はMGC29506):Hs00414907_m1、TCL1A:Hs00951350_m1)及び4種の参照遺伝子(ACTB:Hs99999903_m1、B2M:Hs00984230_m1、GAPDH:Hs99999905_m1、HPRT1:Hs99999909_m1(Thermo Fisher Scientific))について市販のプライマーとプローブセット(Taqman)を使用してStepOnePlus機器(Thermo Fisher Scientific)でリアルタイム定量的PCRを行った。4種の参照遺伝子の幾何平均を用いてRNA量を正規化し、2−ΔΔCq法[53]に従って各遺伝子の相対的発現を計算した。
ロジスティック単変量解析(Rでglmパッケージを使用)でSTAと比較したTOLに関連するパラメータを使用してcSoTを構築した。単変量解析において寛容に関連する24種のパラメータ(20種の遺伝子と4種の人口統計学的パラメータ)間で最も識別的な組み合わせを特定するため、Bolasso法を使用した[5]。この方法では、ブートストラップ再サンプリング(10,000回)をlasso(least absolute shrinkage and selection operator)回帰分析と組み合わせて実施した後、Rのmhtパッケージ(バージョン3.2.2)を用いて複数の試験を行ってモデルに関連する有意な変数(偽発見率(FDR)<0.05)を選択する[2]。mhtパッケージによって行ったスケーリングの後、mhtから得た係数を使用して他のデータセットからスコアを計算した。qPCRデータについては、スケーリングしたdCqを使用した。
統計解析はRソフトウェアバージョン3.2.2又はGraphPrism v.4ソフトウェアを用いて行った。パラメトリックスチューデントT検定、ANOVA検定又はKhi2検定を用いて群比較を行った。p<0.05の場合に差異が統計学的に有意であると定義した。
移植後の免疫寛容状態に関連する臨床的パラメータの選択
上述したメタデータセット[18]から、ナントのコレクションに加え、欧州の寛容指数(IoT)と米国の免疫寛容ネットワーク(ITN)のおかげで、様々な研究の中から312名の重複しない患者、即ち、96例のTOL試料のうち移植後に免疫寛容の個々の患者46名(TOL)及び311例のSTA試料のうち安定な移植片機能を有する患者266名(STA)を特定することができた。TOL及びSTA患者から入手可能な臨床的パラメータの人口統計学的説明を表1及び表2に示す。予測スコアを構築するために、患者の人口統計学的臨床パラメータの中から、内因性で非変異の患者に関連しTOLの少なくとも半分について知られているものだけを選択した(表1及び表2)。単変量ロジスティック回帰(p<0.20)を用いて寛容状態に関連する4種のパラメータ、即ち、移植時の患者の年齢(p<0.0001)、試験時の患者の年齢(p=0.176)(表1)、HLAミスマッチの数(p<0.0001)及びドナーの性別(p=0.154)(表2)を複合スコアのために選択した。
技術的反復実験(同一の血液採取時間)では、20種の遺伝子の各々についての平均発現を計算し、時間的反復実験では、最も早い時点を選択した。TOLとSTAの差異として以前に報告した20種の遺伝子の発現[1]は、この大きな312名の患者のコホート(46名のTOLと266名のSTA、p<0.0001)における単変量解析で確認した。
TOL試料は3種の異なる起源(ナント、IoT及びITN)に由来したにも関わらず、cSoTは患者起源と関連していなかった(p=0.13)。移植片機能が安定した患者においてISを中止した主な理由の1つは、PTLD等の重篤な副作用の出現である。しかし、cSoTはPTLDの経験による影響を受けなかった(PTLD、n=4、p=0.19)。
1)CsA(n=14)又はラパマイシン(Rapa、n=23)単独療法中の患者の第1のコホート[7]、及び
2)アザチオプリンからMPAに変更した後の患者の第2のコホート(n=5対、MPA変更前又は変更3ヶ月後)[6]において、cSoT由来の6種の遺伝子の個々又はその組み合わせに及ぼすISレジメンの影響について更に試験した[34、35]。
主な問題の1つは経時的なcSoTの安定性及び結果である。本発明者らは、以前に、移植片機能喪失の幾つかの症例がこのTOLのコホートで観察され[36]、クレアチン血症(>150μmol/L)又はタンパク尿(>1g/24時間)が増加し得ることを報告した。殆どの臨床情報が入手可能であったナントのコホート由来の15名のTOLのうち、9名では経時的に機能が低下した(移植後17.09±3.46年)。更に、これらの15名のTOLのうち、8名では抗HLA抗体が新たに生じ(移植後14.67±1.13年)、そのうち、4名の患者ではドナー特異的抗体(DSA)が生じた(移植後13.41±0.21年)。また、新たな抗HLA抗体の存在は寛容の喪失と関連していた(p=0.034)。
移植後の免疫寛容プロファイル又はプロトコル誘導の寛容プロファイルに従ってcSoTの特異度を評価するため、寛容誘導の治験においてcSoTを試験し、生体ドナー同胞由来のHLAが同一の腎臓レシピエントを移植後4年間追跡した[9][8]。プロトコルは、早期のシロリムス変更及びドナー造血CD34+幹細胞の注入を伴う、タクロリムス及びMPAによるリンパ球枯渇アレムツズマブ治療で構成される。免疫抑制を移植から2年後に中止した。これらの患者は、ISを完全に中止してから少なくとも1年後には正常な生検(即ち、無症候性拒絶反応の徴候がない)と良好な腎機能を有していた。
cSoTスコアは遺伝子マイクロアレイ手段に基づいているので、本発明者らはそれを検証し、そのルーチンでの潜在的使用を裏付けるために定量的PCRを用いてその有用性を確認した。メタデータセットに含まれていない5種の独立したTOL試料及びメタデータセット由来の4種のTOLに対してqPCRを様々な時間で行った。
本出願を通して、様々な参考文献が、本発明が関係する技術の最先端を説明している。これらの参考文献の開示は参照により本開示に組み込まれる。

i)前記対象から得た生体試料中の6種の遺伝子の発現レベル及び2種の臨床的パラメータによって複合寛容スコア(cSoT)を確立する段階であって、
前記6種の遺伝子はID3、AKR1C3、CD40、CTLA4、TCL1A及びMZB1であり、
前記cSoTは次式:
ii)このcSoTを既定の基準値と比較する段階と、
iii)cSoTが既定の基準値よりも高い場合には対象がTOLであると結論を下し、又はcSoTが既定の基準値よりも低い場合には対象がSTAであると結論を下す段階を含む方法。 - 2種の臨床的パラメータは試験時の前記対象の年齢及び移植時の前記対象の年齢である、請求項1に記載の方法。
- 既定の基準値はTOL対象のcSoTである、請求項1又は2に記載の方法。
- 既定の基準値はSTA対象のcSoTである、請求項1又は2に記載の方法。
- 対象は免疫抑制治療中である、請求項1〜4のいずれか一項に記載の方法。
- 前記対象はヒトである、請求項1〜5のいずれか一項に記載の方法。
- 前記対象は腎臓レシピエントである、請求項1〜6のいずれか一項に記載の方法。
- 前記腎臓レシピエントは更に腎臓ドナーの膵臓が移植されており、任意選択で腎臓ドナーの十二指腸片が移植されている、請求項7に記載の方法。
- 移植された対象を免疫抑制療法で治療する方法であって、
i)対象が請求項1に記載の移植後に免疫寛容の(TOL)対象であるか又は移植後に免疫寛容でない(STA)対象であるかを決定する段階と、
ii)対象がSTAである場合には対象を免疫抑制療法で治療する段階を含む方法。 - 免疫抑制療法中の移植された対象を免疫抑制療法の断薬又は最小化の候補として特定するための方法であって、
i)対象が請求項1に記載の移植後に免疫寛容の(TOL)対象であるか又は移植後に免疫寛容でない(STA)対象であるかを決定する段階と、
ii)対象がTOLである場合には対象が免疫抑制療法の断薬又は最小化に適格であると結論を下す段階を含む方法。
