プログラム細胞死リガンド−1(PD−L1)および細胞傷害性Tリンパ球関連抗原4(CTLA−4)免疫チェックポイントは、抗腫瘍T細胞活性を阻害する。抗PD−L1抗体デュルバルマブ(MEDI4736)および抗CTLA−4抗体トレメリムマブは、PD−L1陰性腫瘍を有する患者において単独療法よりも大きい抗腫瘍活性を提供し得る。この試験では、過去に治療した局在進行性または転移性NSCLCを有する患者におけるCTLA−4阻害剤トレメリムマブと組み合わせたデュルバルマブの安全性および抗腫瘍活性を検討した。確認された局在進行性または転移性NSCLCを有する免疫療法ナイーブ患者を試験に含めた。特に、患者は、(免疫組織化学アッセイを用いて評価した)PD−L1の発現とは無関係に適格であった。抗腫瘍活性においては、客観的奏効は確認された完全または部分奏効(CRまたはPR)と定義し、24週目の疾病管理はCR、PR、または安定疾患(SD)の持続時間が≧24週と定義した。24週目の客観的奏効率(ORR)および疾病管理率(DCR)を評価し、正確な二項分布を用いて95%信頼区間(CI)を算出した。試験薬は、13用量のデュルバルマブ(D)を4週毎(q4w)、6用量をq4w、次いで3用量のトレメリムマブ(T)を12週毎(q12w)、静脈内投与した。デュルバルマブ3mg/kg(D3)〜20mg/kg(D20)とトレメリムマブ1mg/kg(T1)〜3mg/kg(T3)との複数の組み合わせについて探索した。特に、4週毎(q4w)の3、10、15、もしくは20mg/kgまたはq2wの10mg/kgのデュルバルマブ用量は、1、3、もしくは10mg/kgをq4wの6用量として、次いでq12wの3用量としてのトレメリムマブと組み合わせた(例えば、D15 q4w/T10の組み合わせを含む)。漸増期の間、D10 q2wについても、T1またはT3と組み合わせて試験した。
がん患者に対する免疫療法は、有望な治療オプションを拓くが、それらに応答する患者の亜群を同定することは課題である。NSCLC後期患者においては、現在用いられる最良の層別子は、病理学者により手作業で判定される、膜上にPD−L1を発現している細胞の割合である、PD−L1状態に対するカットオフである。この方法の不便性および制限は過度の単純化を含むが、特に免疫系の重要性が最小化され、またそれがマニュアル試験であり、その課題によって代表される複雑性および統計学的精度をカバーしない可能性があることが理由である。さらに、PD−L1状態による層別化は、適中率および正確度に関する要件を満たすのに十分でない場合がある(Rizvi et al.,J Clin Oncol 33,2015(suppl;abstr 8032);図1)。
応答予測のための層別化アルゴリズムは、PD−L1染色組織切片の視覚的な病理学者スコアリング;Cognition Network Languageスクリプトを用いてDefiniensのデータフィケーションチームによって計算された、PD−L1およびCD8の染色およびデジタル化組織切片の基本的読み取り;基本的読み取りの様々な組み合わせ(例えば、掛け算、加算、最小値など);ならびに分割および分類された画像の画像マイニングを通じて得られるヒートマップからの特徴に対して実行した。
データフィケーションワークフローは、CD8+リンパ球、CD8+浸潤リンパ球、CD8+細長いリンパ球、PD−L1+腫瘍細胞(さらに、染色が弱い膜、中程度の膜および強い膜に対応するM1、M2、M3亜群)、PD−L1+リンパ球、およびPD−L1+浸潤リンパ球などの細胞集団の細胞密度(細胞/mm2)および百分率を含む、組織切片における基本的特徴を同定するようにセットアップした。密度および百分率は、病理学者からの注釈付き領域内部、すなわち注釈付き領域(AA)、腫瘍コア(TC)、侵襲性辺縁部(IM)および腫瘍領域(TR)内部(画像分析により誘導された)で測定した。
ヒートマップは、以下のルールに従って定義される、腫瘍細胞(赤)、リンパ球CD8−(青)およびリンパ球CD8+(緑)の密度を計算することにより作成した(図2)。
・リンパ球CD8+:データフィケーションのルールセットにより陽性と定義
・腫瘍細胞:リンパ球CD8+およびピクセル面積>120および絶対値(長さ−幅)<0.3×長さ
・リンパ球CD8−は、リンパ球CD8+およびピクセル面積<125およびピクセル面積>36および絶対値(長さ−幅)<0.3×長さと定義した
異なる細胞集団の相互作用によりスライドをスコア化するため、以下のステップに従い、空間統計学の計算を行った。1.ヒートマップをCD8画像分析(Definiens Image Miner Next Generationツール)から作成し、CD8+/−T細胞および腫瘍細胞を同定した。2.各ヒートマップのピクセルについて、その近隣を考慮することにより特徴ベクターを計算した。3.目的変数なしのクラスター分析をヒートマップのピクセルのオブジェクトに対して実施した。4.患者の特徴ベクターを相対面積/集団/患者の計算により作成した。
PD−L1−CD8関係における観察される最大についての説明モデルを作成するため(図3)、T細胞集団と腫瘍との相互作用についての単純な数値シミュレーションモデルを実行した。実行した主な機構は、腫瘍へのT細胞のフロー、T細胞によるIFNγの産生と、その後のT細胞を殺滅しかつ/またはそれらの増殖を少なくとも低減する、腫瘍細胞によるPD−L1応答であった。モデルの単純化したグラフ表示を図4に示す。
デュルバルマブ療法に対する患者の応答は、2倍交差検証を用いる浅い決定木の森により、(上記の)すべての特徴に対して予測した。森は、次のように、訓練および試験した1000CART決定(回帰)木からなる。

Claims (55)
- 免疫介在性がん治療薬を同定された腫瘍を有する患者に投与することを含む治療方法であって、ここで前記患者は、腫瘍細胞を含む前記患者の組織試料からの組織切片を特徴づけることによって同定され、ここで前記組織切片は、2つ以上の検出可能な親和性試薬と接触され、ここで前記特徴づけは、
(a)前記組織切片の、癌マーカーに特異的に結合する第1の親和性試薬を検出する第1の画像チャネル内の第1の検出可能なシグナルを測定するステップと;
(b)前記組織切片の、免疫細胞マーカーに特異的に結合する第2の親和性試薬を検出する第2の画像チャネル内の第2の検出可能なシグナルを測定するステップと;
(c)腫瘍を特徴づけるため、第1の検出可能な親和性試薬からの検出可能なシグナルを含む前記組織切片の領域内の前記第2の画像チャネル内の検出可能なシグナルの測定を用いるステップと、を含み、
ここで前記第2の画像チャネル内での前記測定により、前記患者が免疫介在性がん治療薬を含む治療に対する応答性を示すとして同定される、治療方法。 - 腫瘍を、免疫介在性がん治療薬に対する応答性を示すとして特徴づける方法であって、
(a)2つ以上の検出可能な親和性試薬と接触された、腫瘍細胞を含む組織試料からの組織切片の、癌マーカーに特異的に結合する第1の親和性試薬を検出する第1の画像チャネル内の第1の検出可能なシグナルを測定するステップと;
(b)前記組織切片の、免疫細胞マーカーに特異的に結合する第2の親和性試薬を検出する第2の画像チャネル内の第2の検出可能なシグナルを測定するステップと;
(c)腫瘍を特徴づけるため、前記第1の検出可能な親和性試薬からの検出可能なシグナルを含む組織切片の領域内の前記第2の画像チャネル内の検出可能なシグナルの測定を用いるステップと、を含み、
ここで前記第2の画像チャネル内での前記測定により、前記腫瘍が免疫介在性がん治療薬に対して応答性を示すことが示される、方法。 - 前記組織試料ががん患者に由来する、請求項2または3に記載の方法。
- 前記第2の画像チャネル内での前記測定により、前記患者が免疫介在性がん治療薬向けの治療に対して応答性を示すことが示される、請求項1〜3のいずれか一項に記載の方法。
- ステップ(a)が、デジタル画像の第1の画像チャネルを生成することを含み、ここで前記第1の画像チャネルのピクセル値が前記組織切片中の陽性染色腫瘍細胞の局所密度を示す、請求項1〜4のいずれか一項に記載の方法。

- ステップ(c)が、前記第1の画像チャネルを用いて、前記デジタル画像中の領域を分割することを含む、請求項5または6に記載の方法。
- 前記分割することにより、前記デジタル画像の総ピクセルが、前記第1の画像チャネル内のピクセル値が所定の第1のチャネルの閾値よりも大きい場合の前記領域に割り当てられる、請求項8に記載の方法。
- ステップ(c)が、前記がん患者が前記免疫介在性がん治療薬に対する応答性を有することを、前記分割された領域内の前記第2の画像チャネルのピクセル値の統計学的特性を利用することにより予測することをさらに含む、請求項7または8に記載の方法。
- 前記統計学的特性が、前記第2の画像チャネル内のピクセル値が所定の第2のチャネルの閾値より大きい場合の前記領域内のピクセルの相対数である、請求項9に記載の方法。
- 前記検出可能な親和性試薬の1つ以上が、抗体および検出可能なレポーターを含む、請求項1〜10のいずれか一項に記載の方法。
- 前記第1の親和性試薬が、PD−L1に特異的に結合する抗体を含む、請求項1〜11のいずれか一項に記載の方法。
- 前記第2の親和性試薬が、CD8、CD3、FOXP3、およびCD4からなる群から選択される免疫細胞マーカーに特異的に結合する抗体を含む、請求項1〜12のいずれか一項に記載の方法。
- 前記ピクセル値により細長い免疫細胞が同定される、請求項6〜13のいずれか一項に記載の方法。
- 細長い免疫細胞が、前記細胞の境界ボックスの長さ対幅比により測定される、請求項14に記載の方法。

(b)第2の親和性試薬を検出する、前記組織切片の第2の画像チャネル内の第2の検出可能なシグナルを測定するステップと;
(c)前記腫瘍を特徴づけるため、前記第1の検出可能な親和性試薬からの検出可能なシグナルを含む前記組織切片の領域内の前記第2の画像チャネル内での測定を用いるステップと、を含み、
ここで前記第2の画像チャネル内での前記測定により、前記NSCLCがデュルバルマブ療法を含む治療に対して応答性を示すことが示される、方法。 - デュルバルマブ、またはその抗原結合断片を非小細胞肺がん(NSCLC)患者に投与することを含む治療方法であって、ここで前記患者は、腫瘍細胞を含む前記患者の組織試料からの組織切片を特徴づけることにより同定され、ここで前記組織切片は、2つ以上の検出可能な親和性試薬と接触され、ここで前記特徴づけは、
(a)前記組織切片の、PD−L1に特異的に結合する第1の親和性試薬を検出する第1の画像チャネル内の第1の検出可能なシグナルを測定するステップと;

ここで前記第2の画像チャネル内での前記測定により、前記患者がデュルバルマブ療法を含む治療に対する応答性を示すとして同定される、方法。 - 免疫介在性がん治療薬を腫瘍を有する患者に投与することを含む治療方法であって、ここで前記患者は、腫瘍細胞を含む前記患者の組織試料からの組織切片を特徴づけることにより同定され、ここで前記特徴づけは、
(a)腫瘍細胞を含む組織試料からの第1の組織切片中のPD−L1+免疫細胞の密度を測定するステップと;
(b)前記組織試料からの第2の切片中のタンパク質または遺伝子の発現レベルを測定するステップと;
(c)ステップ(a)および(b)にて得られた前記測定値に基づくスコアを生成するステップと、を含み、
ここで閾値よりも大きいスコアにより、前記患者が免疫介在性がん治療薬を含む治療に対する応答性を示すとして同定される、方法。 - 腫瘍を免疫介在性がん治療薬に対する応答性を示すとして特徴づける方法であって、
