Abstract
Description
a)図1に示すように、単一列の縫合糸アンカー1が回旋腱板の腱2の下に在り、腱を骨3に固定するために縫合糸が腱を通過してしっかりと結ばれる
b)図2に示すように、二重列の縫合糸アンカー1が回旋腱板の腱2の下に在り、腱を骨3に固定するために縫合糸が腱を通過してしっかりと結ばれる。
c)図3に示すように、単一列の縫合糸アンカー1が回旋腱板の腱2の下に在り、縫合糸が腱を通過してしっかりと結ばれて、結び目からの縫合糸は腱を越えて側方へ延びて骨3に固定され、結び目のない縫合糸アンカー4が腱の縁外にある。
Claims (21)
- 軟組織を骨に固定するためのアンカーシステムであって、
非配備形態と配備形態とを有するインプラントであって、当該インプラントが、前記配備形態にあるとき隣接する骨に固定可能である、インプラントと、
前記インプラントに接続された芯であって、当該芯が、収縮非配備形態と拡張配備形態との間で可動な拡張表面領域を有する第1部分と、前記第1部分を前記インプラントに接続する第2部分と、を備え、当該芯の第1部分が、細胞が前記骨内から前記骨と軟組織との間の界面へ移行するための経路を与えて前記骨及び軟組織の界面における治癒を促進するように適合された材料で構成される、芯と、
前記芯の周りに配置されて、前記芯が所望の骨及び軟組織の界面に隣接する位置へ挿入される際に前記芯を保護する遮蔽構造体と、
を備える、アンカーシステム。 - 前記遮蔽構造体がシースを備える、請求項1に記載のアンカーシステム。
- 前記シースが、前記シースの中に配置された複数の開口を含む、請求項2に記載のアンカーシステム。
- 配備前に前記インプラント及び前記芯をそれらの非配備形態において所定の位置に保持するスリーブを含むインサータを更に備える、請求項1に記載のアンカーシステム。
- 前記インプラントを配備するための近位アクチュエータを更に備える、請求項4に記載のアンカーシステム。
- 前記インサータが近位方向へ引き戻されたとき、前記芯の第1部分がその拡張配備形態へ動かされる、請求項4に記載のアンカーシステム。
- 前記芯が、その中に配置された整列ファイバを有する重合体から形成された折り畳み可能な薄い足場シートを備え、前記芯の第1部分がパドル部分を備え、前記芯の第2部分が細長部分を備え、前記芯の第2部分が、前記芯がその非配備形態にあるときに、前記シースの対応する細長部分と係合する、請求項1に記載のアンカーシステム。
- 前記芯のパドル部分がその拡張配備形態にあるとき、前記パドル部分が骨面と隣接する軟組織材との間に挟まれるように適合され、前記パドル部分の中に配置された整列ファイバが、前記骨面内の骨髄から前記軟組織への細胞の移行方向と整列するように適合される、請求項7に記載のアンカーシステム。
- 前記芯が、前記薄い足場シートにエレクトロスピンされた85/15Lラクチド/グリコリド重合体で構成される、請求項8に記載のアンカーシステム。
- 前記インプラントが全縫合糸アンカーを備える、請求項1に記載のアンカーシステム。
- 前記芯の第2部分が、前記芯の第2部分の配備前に、前記インサータのシャフトの周りに巻き付けられる、請求項4に記載のアンカーシステム。
- 前記シースがPEEKの薄いシートから製造される、請求項2に記載のアンカーシステム。
- 前記遮蔽構造体が2ピースアンカーを備える、請求項1に記載のアンカーシステム。
- 前記2ピースアンカーが、遠位アンカーピースと近位アンカーピースとを備え、前記芯が、前記芯の第1部分の配備前に前記遠位アンカーピースと近位アンカーピースとの間に配置される、請求項13に記載のアンカーシステム。
- 前記芯の第2部分が、前記芯の第1部分の配備前に前記遠位アンカーピースと近位アンカーピースとの間に配置される、請求項14に記載のアンカーシステム。
- 配備前に前記芯の第1部分をその非配備形態において所定の位置に保持するスリーブを含むインサータを更に備える、請求項15に記載のアンカーシステム。
- 軟組織アンカーシステムにおいて使用するためのインサートであって、
骨及び軟組織の界面を有する処置部位においてインプラントと一緒に埋植されるように適合された芯であって、当該芯が、収縮非配備形態と拡張配備形態との間で可動な拡張表面領域を有する第1部分と、前記第1部分に接続された第2部分と、を備え、当該芯の第1部分が、細胞が前記骨内から前記骨と軟組織との間の界面へ移行するための経路を与えて前記骨及び軟組織の界面において治癒を促進するように適合された材料で構成される、芯と、
前記芯の周りに配置されて、所望の骨及び軟組織の界面に隣接する位置へ前記芯が挿入される際に前記芯を保護する遮蔽構造体であって、当該遮蔽構造体が前記芯の第2部分に接続される、遮蔽構造体と、
を備える、インサート。 - 前記遮蔽構造体が2ピースアンカーを備える、請求項17に記載のインサート。
- 前記2ピースアンカーが遠位アンカーピースと近位アンカーピースとを備え、前記芯が、前記芯の第1部分の配備前に前記遠位アンカーピースと近位アンカーピースとの間に配置される、請求項18に記載のアンカーシステム。
- 前記芯の第2部分が、前記芯の第1部分の配備前に前記遠位アンカーピースと近位アンカーピースとの間に配置される、請求項19に記載のアンカーシステム。
- 軟組織を骨に固定する方法であって、
埋植可能なアンカーを所望の骨部位の中へ挿入することと、
前記骨部位において前記アンカーを前記骨に固定するために前記アンカーを配備することと、
拡張された芯表面を前記骨に配備するためにインサータを引き戻すことと、
前記軟組織が前記拡張された芯表面に係合し、前記拡張された芯表面が前記骨と前記軟組織との間に挟まれるように、前記軟組織を前記骨に隣接させることと、
を含む、方法。
