Abstract
Description
まず第一に、骨髄ニッチにHSCを定着させる(populate)ために、インビボでのGαs伝達シグナルの必要性が言われている(Adams GB ら,(2009)骨髄生着のための造血幹細胞のGαs媒介シグナリングへの依存(Haematopoietic stem cells depend on Gas-mediated signaling to engraft bone marrow),Nature 459:103−107)。これらの発見は、Gαsの活性化が造血幹細胞の生存及び分化を促進することを示した先のインビトロ実験を裏付けるものである(Dexter TM ら,(1985)膜結合タンパク質のコレラ毒素誘導アデノシン二リン酸リボシル化の阻害薬による幹細胞分化の遮断 (Inhibitors of cholera toxin-induced adenosine diphosphate ribosylation of membrane-associated proteins block stem cell differentiation),Blood 65:1544−1548,Long MW ら,(1988)コレラ毒素とホルボールジエステルによるマウス造血前駆細胞増殖の相乗的調節 (Cholera toxin and phorbol diesters synergistically modulate murine hematopoietic progenitor cell proliferation) Exp Hematol.16:195−200)。Gαsは、膜結合型哺乳動物アデニリルシクラーゼの全9個のアイソフォームを刺激するヘテロ三量体Gタンパク質のグアニンヌクレオチド結合α−サブユニットである。Gαsは、細胞をコレラ毒素で処理することにより、エクスビボ/インビボで構成的に活性化されうる。これは、コレラ毒素が、触媒アルギニン残基(R186/187/201/202、アルギニンの正確な数はGαsのスプライスバリアントによる)をAPDリボシル化するためである。無傷のアルギニン残基は、GTP−加水分解及びその結果のGαsの失活のために必要とされる(Freissmuth M,Gilman AG(1989)細菌毒素によりタンパク質の反応性を変化させるために設計されたGsαの変異。ARG187における置換はGTPアーゼ活性の喪失をもたらす(Mutations of Gsα designed to alter the reactivity of the protein with bacterial toxins. Substitutions at ARG187 result in loss of GTPase activity),J Biol Chem 264:21907−21914)。実際、生着の増進は、コレラ毒素で造血幹細胞を前処理した後に観察できる。幹細胞調製物がコレラ毒素で処理された場合、骨髄中に約2倍の(Lin−)前駆体細胞が存在した(Adams、2009)。第二に、HSCは、全4個のプロスタグランジンE受容体(EP1〜4)を発現している。造血幹細胞を(ジメチル化)プロスタグランジンE2で前処理すると、それらの生着が増進される(North TE,ら,(2007)プロスタグランジンE2による脊椎動物の造血幹細胞の恒常性の調節 (Prostaglandin E2 regulates vertebrate haematopoieticstem cell homeostasis),Nature 447:1007−1011,8;Hoggatt J,ら,(2009)プロスタグランジンE2による造血幹細胞のホーミング、生存、及び増殖の増進 (Prostaglandin E2 enhances hematopoietic stem cell homing, survival, and proliferation)(Blood 113:5444−5455)。この効果は、標準(canonical) Gαs依存性シグナリングにより媒介される。なぜならば、プロテインキナーゼA(PKA)のcAMP誘導活性化がWnt依存性シグナルと相乗作用してβ−カテニンを安定化させるからである(Goessling W ら(2009)PGE2とWntシグナリングの遺伝的相互作用による幹細胞の発生仕様及び再生の調節 (Genetic interaction of PGE2 and Wntsignaling regulates developmental specification of stem cells and regeneration),Cell 136:1136−1147)。
SDF−1-CXCR4軸も、細胞ホーミングに関与していることが知られている。SDF−1は、正常HSCの輸送の調節ならびにそれらホーミング及び骨髄での保持に極めて重要な役割を果たしている(Kucia ら,Stem Cells,2005)。
Hussain Filzaらは、トレプロスチニル(Treprostinil)の幹細胞移植への効果を報告している(BMC Pharmacology,vol 11,no.Suppl 2,2011,p A6)
US2008/085264は、造血幹細胞の移植効率を増大させるための前処理に、DPP−IV阻害薬/CD26ペプチダーゼ阻害薬の使用を開示している。
驚くべきことに、造血幹細胞のホーミング及び生着は、エクスビボで、前記幹細胞及び/又は前駆細胞を、前記細胞の生着特性を刺激するプロスタサイクリン類似体で前処理し、さらにジペプチジルペプチダーゼIV(DPP−IV)の阻害薬を、前記造血幹細胞を移植された個人に投与することにより、成功裡に増進できることが示された。
本発明は、選ばれた個人群、すなわち、骨髄疾患又は骨髄関連疾患(具体的には、骨髄疾患は、白血病、骨髄異形成症候群、骨髄増殖性疾患、再生不良性貧血、鎌状赤血球疾患、血液細胞コンパートメントの欠陥、化学療法又は放射線照射によって誘発された骨髄疾患である)を患い、移植前にインビトロでプロスタサイクリン類似体及びcAMP阻害薬で前処理された造血幹細胞サンプルを用いて幹細胞移植を受ける個人の治療におけるDPP−IV阻害薬の使用を取り扱う。
本発明の態様に従って、DPP−IV阻害薬はビルダグリプチンであり、造血幹細胞は移植前にインビトロでトレプロスチニルとフォルスコリンで処理されている。
本発明の態様は、DPP−IV阻害薬を、骨髄移植後少なくとも1日間、少なくとも2日間、少なくとも3日間、少なくとも4日間、少なくとも5日間、少なくとも6日間、少なくとも7日間、少なくとも8日間、少なくとも9日間、好ましくは少なくとも10日間、好ましくは少なくとも14日間投与することも取り扱う。
本発明は、造血細胞の生着能力を増強するための方法も提供し、前記方法は、
a)造血幹細胞及び/又は前駆細胞のサンプルを用意し、
b)前記細胞に、生着を増進する有効量のプロスタサイクリン類似体を投与し、
c)前記混合物を、前記細胞におけるGアルファs−シグナリングを刺激するのに足る期間インキュベートし、
d)前記細胞を単離し、
e)前記細胞をそれを必要とする個人に移植し、
f)前記個人に有効量のDPP−IV阻害薬を投与する
連続工程を含む。

本発明は、移植後骨髄に生着する造血細胞の数を増加させる方法も提供し、前記方法は、造血細胞をインビトロで有効量のプロスタサイクリン類似体、具体的にはトレプロスチニルと、特にcAMPエンハンサー、具体的にはフォルスコリンも共に接触させ、プレインキュベートされた細胞をそれを必要とする個人に投与し、そしてさらにDPP−IV阻害薬、具体的にはビルダグリプチンを前記個人に造血細胞の投与の直前及び/又は後に投与する工程を含む。

トレプロスチニルは、プロスタサイクリンの合成類似体で、代謝的に安定である。トレプロスチニルは、RemodulinTMとして市販されている。トレプロスチニルは、(1R,2R,3aS,9aS)−[[2,3,3a,4,9,9a−ヘキサヒドロ−2−ヒドロキシ−1−[(3S)−3−ヒドロキシオクチル]−1H−ベンズ[f]インデン−5−イル]オキシ]酢酸・一ナトリウム塩である。

本発明の特定の態様に従って、トレプロスチニル誘導体は、トレプロスチニルの酸誘導体、トレプロスチニルのプロドラッグ、トレプロスチニルの多形体、トレプロスチニルの無水多形体、又はトレプロスチニルの異性体の群から選択される。
特定の発明的態様に従って、2種類、特に3種類以上の異なるプロスタサイクリン類似体が発明的方法に使用できる。あるいは、4、5又は6種類、又はさらに多くの異なるプロスタサイクリン類似体が使用されてもよい。
16,16−ジメチルPGE2は、15−ヒドロキシPGDHの競合的阻害薬であるが、酵素の基質ではない。15−ヒドロキシPGDHによる代謝に対して抵抗性があるため、インビボで長期の半減期を有する。16,16−ジメチルPGE2は、ほとんどのEP受容体サブタイプに対するアゴニストとして作用する。単離されたEP2受容体を活性化するためのKdは約1nMである3。16,16−ジメチルPGE2は、培養下での増殖中に造血幹細胞の自己複製能を保存しながら分化を防止するために使用される4,5。
本発明に従って、“処理”、“前処理”又は“インキュベーション”という用語は互換的に使用できる。当該用語は、プロスタサイクリン類似体及びcAMPエンハンサーと接触させられる単離された造血幹細胞に関して使用される。
代替の方法に従って、DPP−IV阻害薬は、シタグリプチン、アログリプチン、サキサグリプチン及びリナグリプチン、アナグリプチン、テネリグリプチン、ゲミグリプチン及びデュトグリプチンから選択され、造血幹細胞は移植前にインビトロでトレプロスチニルとフォルスコリンで処理されている。
プロスタサイクリン類似体の量は、刺激HSCの調製法に依存する。
非常に具体的には、発明的用途の場合、トレプロスチニルの有効濃度は、0.1μM〜100μM、特に1μM〜50μM、特に5μM〜25μMの範囲、特に約10μMである。
本発明の特定の態様に従って、プロスタサイクリン類似体とフォルスコリンの比率は約1:3でありうる。フォルスコリンとプロスタサイクリン類似体で処理されたHSCは、再移植の前に精製できるが、これらのHSCはさらなる精製工程なしに再移植されてもよい。なぜならば、少量のフォルスコリンが存在しても、悪い副作用は何ら生じないと見られるからである。
あるいは、DPP−IV阻害薬は、患者に、術前、すなわち移植を受ける1、2、3、4、5、6、7、8、9又は10日前、及び/又は術後、すなわち移植の少なくとも5時間、特に少なくとも10時間、特に少なくとも15時間、特に少なくとも24時間後から、1、2、3、4、5、6、7、8、9、10日後、又はそれより多い日数まで投与できる。
具体的には、シタグリプチンは、50〜200mg/日、特に75〜150mg/日、特に約100mg/日の量で個人に投与できる。
具体的には、リナグリプチンは、約2.5〜10mg/日、特に約5mg/日の量で個人に投与できる。
発明的方法は、有益なことに、個人にそのまま投与できる刺激幹細胞を提供し、さらに前記細胞のホーミング及び生着を刺激する。
グリプチン及びプロスタサイクリン類似体で処理されたHSCは、再移植される前に精製されてもよいが、これらのHSCは更なる精製工程なしに再移植することもできる。
さらなる態様に従って、HSCをプロスタサイクリン類似体とcAMPエンハンサーで前処理するためのインビトロインキュベーション時間は、少なくとも10分間、特に少なくとも20分間、少なくとも30分間、少なくとも40分間、少なくとも50分間、少なくとも60分間である。
cAMP依存性経路は、造血幹細胞の生着を促進するための必須経路である。発明者らにより、プロスタサイクリン類似体は造血幹細胞中のcAMP上昇を誘発できることが示された。プロスタサイクリン類似体は、多数の受容体、すなわちIP受容体及びEP受容体を活性化してGアルファs−シグナリングの増加をもたらすことにより、これを行う。従って、トレプロスチニル、イロプロスト、シカプロスト又はベラプロストなどのプロスタサイクリン類似体は、cAMPレベルをより効果的に上昇させる。
HSCを含有するサンプルは具体的には骨髄でありうる。
DPP−IV阻害薬は、適用可能な、そして当該技術分野で知られている任意の様式により、対象に投与できる。さらに詳しくは、腸内、静脈内又は皮下投与が提供される。
しかしながら、DPP−IV阻害薬は、徐放性形態、錠剤又はカプセルの群から選択される経口用形態でもよい。
a)第一の単位剤形中に一定量の少なくとも一つのプロスタサイクリン類似体とフォルスコリン
b)グリプチンから選択される一定量の少なくとも一つのDPP−IV阻害薬
を、成分a)とb)の2個、3個、4個又はそれより多い個数の別個の単位の形態で含む部品のキットも提供する。
上記説明は、以下の実施例を参照することにより、より十分に理解されるであろう。しかしながら、そのような実施例は、単に本発明の一つ又は複数の態様を実施する代表的方法に過ぎないので、本発明の範囲を制限するものとして読まれるべきではない。
インビトロ移動アッセイ
マウスの造血前駆細胞を、インビトロで、10μMのトレプロスチニル(Trep)と30μMのフォルスコリン(Fsk)(Sigma Aldrich社、オーストリア・ウィーン)との組合せ又はビヒクルにより、37℃で1時間、PeproTech社(英国ロンドン)から購入した増殖因子含有幹細胞培地[(StemSpanTM(SFEM)(Stemcell technologies社、米国)(50ng/mLのマウス幹細胞因子(SCF)、50ng/mLのfms様チロシンキナーゼ−3リガンド(Flt3)、50ng/mLのインターロイキン11(IL11)及び150ng/mLのマウスインターロイキン3(mIL3)]中で前処理した。その後、細胞を洗浄し、培地中に2×106細胞/mLで再懸濁させた。
実施例2:
トレプロスチニルとフォルスコリンの存在下で刺激されたマウス造血前駆細胞のSDF−1/CXCL12誘導移動の、CXCR4アンタゴニストAMD3100による阻害
造血幹細胞及び前駆細胞をトレプロスチニル(10μM;T)及びフォルスコリン(30μM、;F)と共に37℃で1時間プレインキュベートし、洗浄して、細胞密度2×106細胞/mLで再懸濁させた。細胞懸濁液(0.1mL中に2×105細胞)をTranswellTMディッシュの上部チャンバーに加えた。ここには10μMのCXCR4アンタゴニストAMD3100(10μM)を含む培地と含まない培地が入っていた。下部チャンバーの培地は100ng/mlのマウスSDF−1を含有していた。インキュベーションを37℃で4時間続けた。その後、下部チャンバーに回収された細胞を図1の説明に概要を示したようにカウントした。示されたデータは、三重に実施された3つの独立した実験の平均±SEMである。統計学的有意差は、一元配置ANOVAと、それに続くダネットの多重比較により評価した(***、p<0.001)。
データを図2に示す。
ビルダグリプチンは、トレプロスチニルとフォルスコリンと共にプレインキュベートされた造血前駆細胞のSDF−1/CXCL12誘導移動を増強する
マウス造血前駆細胞(mHPC;左側パネル)及び臍帯血由来ヒトCD34+細胞(右側パネル)をトレプロスチニル(10μM;T)及びフォルスコリン(30μM、;F)の不在下又は存在下で1時間プレインキュベートした。その後、細胞懸濁液(0.1mL中に2×105細胞)をTranswellTMディッシュの上部チャンバーに加えた。ここには30nMのビルダグリプチン(Vil)を含む培地と含まない培地が入っていた。下部チャンバーの培地は100ng/mlのマウス又はヒトSDF−1を適切に含有していた。インキュベーションを37℃で4時間続けた。その後、下部チャンバーに回収された細胞を図1の説明に概要を示したようにカウントした。示されたデータは、三重に実施された3つの独立した実験の平均±SEMである。統計学的有意差は、一元配置ANOVAと、それに続くダネットの多重比較により評価した(*、p<0.05;**、p<0.01;***、p<0.001)。
実施例4:
インビボ・ホーミングアッセイ
ビルダグリプチンとトレプロスチニルは、トレプロスチニル及びフォルスコリンと共にプレインキュベートされた造血前駆細胞のホーミングを増進するが、インビボで組み合わせると相互に拮抗的である。
(i)トレプロスチニル及びフォルスコリンとのインビトロ・プレインキュベーションはホーミングを増大した(参照:非処理及び処理と標識されているバー)。
(iv)組合せ(一番右側の標識“+インビボtrep+vil”)は、いずれかの化合物の単独投与(7及び8番目の標識“+インビボtrep”、“+インビボvil”)より効果がなかった。従って、インビボで組み合わせると、トレプロスチニルとビルダグリプチンは相互に拮抗的である。
インビボ生着アッセイ
インビトロでトレプロスチニルとフォルスコリンとの組合せで前処理された造血前駆細胞を注入されたBALB/cレシピエントマウス(致死的に放射線照射されている)へのトレプロスチニルとビルダグリプチンの併用投与は、ビルダグリプチン又はトレプロスチニルいずれかの単独インビボ投与よりも、これらのマウスの生存の増進に効果が少ない。
我々は、トレプロスチニルとフォルスコリンの作用がさらに増強される条件を探った。これは、トレプロスチニルとフォルスコリンとの組合せの作用が、少なくとも一部はCXCR4受容体(すなわち、ケモカインのストローマ細胞由来因子−1=SDF1=CXCL12の受容体)の誘導によって媒介されているとの観察に基づいていた。さらに、我々は、ジメチルPGE2及びトレプロスチニルがヒトHSPCによるcAMP蓄積に及ぼす効果も比較した。
マウス及びヒトの造血幹細胞及び前駆細胞の単離
10匹のマウス(C57BL/6又はBalb/C)を頸椎脱臼により犠死させた。後肢の長骨(すなわち大腿骨及び脛骨)を筋肉及び結合組織から外し、注射器と27 1/2G針を用いてRPMI培地でフラッシュ洗浄した。細胞懸濁液から目に見える結合組織を除き、収集して遠心管に移した。細胞を遠心分離(1,200rpm/〜100g、5分間)により採取し、3mLの赤血球溶解バッファー(0.15M NH4Cl、10mM KHCO3、0.1mM EDTA、pH7.2〜7.4に調整)中に再懸濁させた。細胞懸濁液を20℃で2分間、次いで氷上で4分間インキュベートした。その後、RPMI(10mL)を加え、細胞を遠心分離により採取し、カウントした。細胞の典型的な収量は3*107/マウスであった。
マウス及びヒト造血幹細胞及び前駆細胞(HSPC)調製物の純度をフローサイトメトリーにより評価した。細胞表面マーカーの染色に使用された抗体は下記の供給源由来であった。すなわち、マウス系統パネル抗体は、Becton Dickinson Biosciences社由来であり(BD 559971、抗CD3ε、抗CD11b、抗CD45R、抗Ly−6G/Ly−6c、抗Ter−119をビオチン化して含有)、アフィニティー精製ラット抗マウスCD16/CD32(マウスFcγII/IIIブロック、BD 553142)及び蛍光染料ストレプトアビジン−アロフィコシアニン−Cy7(ストレプトアビジンAPC−Cy7、BD554063)もBecton Dickinson Biosciences社より得た。フィコエリトリン(PE)−Cy7−標識抗マウスLy6A/E(=幹細胞抗原−1=Sca1)PE−Cy7(カタログ番号25−5981−82)及びPE−Cy5抗マウスCD117(c−Kit)(カタログ番号15−1171−81)はeBiosciences社より得た。
ヒト造血CD34+幹細胞及び前駆細胞(HSPC、4*105/mL)をまず、増殖因子を補給したX−VIVO15培地中で、100ng/mlの百日咳毒素(Sigma Aldrich社)の不在下存在下で16時間プレインキュベートし、次いで、増殖因子と10μg/mLのアデノシンデアミナーゼ(Roche社)を補給したX−VIVO15培地中の[3H]アデニン(Perkin Elmer社、1μCi/mL)中での更なる4時間のインキュベーション期間により、アデニンヌクレオチドプールの代謝標識を行った。その後、細胞を、1又は10μMのトレプロスチニル、1又は10μMのジメチルPGE2(dmPGE2)、フォルスコリン(30μM)又はフォルスコリンとプロスタノイドとの組合せに30分間暴露(challenge)した。次いで、細胞をペレット化し(100gで5分間)、培地を除去して、ペレットを、0.1mMのcAMPを含有する氷冷2.5%過塩素酸(0.9mL)中で溶解し、氷上に1時間保持して、4.2M KOH(0.1mL)で中和した。ATPとcAMPを、Dowex AG50−X8及び中性アルミナを含有するカラム上での連続クロマトグラフィーにより分離した(3)。
ヒト又はマウスHSPCを、ビヒクル又は10μMのトレプロスチニルと30μMフォルスコリンとの組合せの存在下、37℃で1時間及び24時間インキュベートした。4℃のPBSで洗浄後、細胞を、外在化ホスファチジルセリンについては製造業者のプロトコルに従ってPE Annexin−V Apoptosis Detection Kit I(登録商標)を用い、又はDNA含有量についてはPI(PBS中50μg/mL)を用いて、37℃で40分間染色した。フローサイトメトリーにより得られたデータをFacs Divaソフトウェア(登録商標)を用いて分析した。マウスHSPCのコロニー形成は下記のようにして調べた。10μMのトレプロスチニルと30μMフォルスコリンの存在下での1時間のインキュベーションの後、CFU−GM形成用としてGM−CSF及びIL−3(それぞれ10ng/mL)を含有し、CFU−E用として3U/mLのEPO及びIL−3を含有するMethoCult(登録商標)中に再懸濁させ、37℃及び5%CO2で7〜10日間培養した。コロニーの数は光学顕微鏡下でカウントした。
BALB/cドナーマウスの骨髄由来のマウス系統陰性造血幹細胞及び前駆細胞(HSPC)及び臍帯血から単離したヒトCD34+ HSPCを、マウス幹細胞培地(SFEMTM;それぞれ50ng/mLの増殖因子;SCF、Flt3、IL11及び150ng/mLのIL3)又はヒト幹細胞培地(X−VIVOTM15、それぞれ50ng/mLのTPO、FL3、及びSCF)のいずれかの中で、10μMのトレプロスチニルと30μMフォルスコリンとの組合せで刺激した。洗浄後、細胞懸濁液(0.1mL中2×106細胞)をTranswellTM(直径6.5mm、孔径3μm)の上部チャンバーに入れた。下部チャンバーは、増殖因子と100ng/mlのSDF−1を含有する300μLの培地で満たした。必要な場合、上部及び底部チャンバーの両方に30nMのビルダグリプチンを補給した。37℃での4時間のインキュベーション後、SDF−1への走化性を調べた。底部チャンバーに移動したHSPCを細胞計数器でカウントした。アッセイは三重に実施された。
ドナーマウスB6.SJL−PtrcAPep3B/Boyl(CD45.1+)の全骨髄細胞から、系統陰性HSPCをMACSマイクロビーズ(上記参照)を用いて単離した。細胞をインビトロで10μMのトレプロスチニル及び30μMフォルスコリン又はビヒクル対照で1時間37℃で処理した。洗浄後、0.2×106個の系統陰性CD45.1+ 細胞を、致死的に放射線照射された(10Gy)レシピエントC57Bl/6(CD45.2+)マウスに注入した。レシピエントマウスは、インビボで、トレプロスチニル(0.15mg/kg/8時間)、ビルダグリプチン(10mg/kg)、又は両薬物の組合せのいずれかによる皮下処置も受けた。対照マウスには同じ量のビヒクルを注入した。移植の16時間後、レシピエントマウスから全骨髄細胞を単離した。赤血球の溶解後、細胞をCD45.1及びCD45.2マーカーに対し染色した。骨髄中のCD45.1+及びCD45.2+細胞の割合はフローサイトメトリーにより測定した。
同系のレシピエントマウス(C57BL/6又はBALB/c)に致死的放射線照射を浴びせた。造血幹細胞及び前駆細胞(HSPC)の静脈内投与で救えなければ、これらのマウスは最初の2週間以内に死亡した。Lin−(Sca1+及びc−Kit+)HSPC細胞を上記に概要を示した通りに調製し、エクスビボで、10μMのトレプロスチニル+30μMのフォルスコリン(FSK)の組合せの不在下及び存在下、37℃で1時間、前処理した。その後、細胞(1〜5*105/マウス)を尾静脈から注入した。白血球数をFACSにより求め、血液サンプルを第9日目(血球数は〜1G/Lであった)から3〜5日ごとに採取した。場合によって、マウス(25〜30g)は、10日間、8時間ごとに皮下投与された(i)トレプロスチニル(0.15mg/kg/8時間)及びビルダグリプチン(10mg/kg/12時間)又は両者の組合せでも処置された。
RNAを、2mLのTrizol(登録商標)(Invitrogen社)を用いて、マウス及びヒト造血幹細胞及び前駆細胞(3*106細胞)と、マウス大脳皮質から調製された混合(すなわちニューロン及びグリア)培養物と、ヒト前立腺がん細胞株PC3及びヒト結腸がん細胞株HCT116(陽性対照として)から単離した。ホモジナイズされたサンプルを室温で5分間インキュベートして、核タンパク複合体を完全解離させた。クロロホルム(0.4mL)を細胞溶解物(ライセート)に加え、試験管を手で15秒間激しく振盪した。室温での3分間のインキュベーション後、サンプルを12,000gで15分間、4℃で遠心分離した。遠心分離後、混合物は、下部の赤色フェノール−クロロホルム相、中間相、及び無色の上部水性相に分離する。水性相(RNA)を新しい試験管に移し、1mlのイソプロピルアルコールと混合することによりRNAを沈殿させた。室温での10分間のインキュベーション後、サンプルを12,000gで10分間、4℃で遠心分離した。上清を除去し、ゲル様RNAペレットを2mlの75%エタノールで一度洗浄した。サンプルを渦流混合し、7,500gで5分間、4℃で遠心分離した。RNAペレットを10分間風乾し、溶液をピペットの先端に数回通すことにより、80μLの(高純度)水中に溶解し、55℃で10分間インキュベートした(−20℃で貯蔵)。
トレプロスチニル及びdmPGE2に誘導されたサイクリックAMP蓄積ヒト造血幹細胞及び前駆細胞
材料及び方法で概要を示したように、[3H]アデニンで代謝標識後、ヒトHSPCを、トレプロスチニル(Trep、10μM)、dmPGE2(10μM)又はフォルスコリン(Fsk、30μM)、トレプロスチニル(10μM)又はdmPGE2(10μM)とフォルスコリン(30μM)との組合せで刺激した。必要な場合、HSPCは、刺激前に百日咳毒素(PTX)で16時間、前処理された。データは、三重に実施された3つの独立した実験から得たものである。誤差バーは標準偏差(s.e.m.)を示す。30μMのフォルスコリンの存在下では、10μMのトレプロスチニルは10μMのdmPGE2よりも有効であった(P=0.03;ウィルコクソン検定)。この差は百日咳毒素による前処理で消滅した(ns、有意差なし)。データを図7に示す。
cAMPの継続的な上昇は、幹細胞におけるアポトーシスを誘発しうる(10)。dmPGE2処理HSCの生着向上は、細胞生存、増殖及びホーミングに対する効果によるものであった(11)。トレプロスチニルとdmPGE2は、Gタンパク質を動員するそれらの能力に違いがある。dmPGE2が誘導するGiの動員は特に関係がありうる。なぜならば、Giを介したシグナリングは、脂質キナーゼのPI3キナーゼと下流のキナーゼAKTの活性化をもたらすことができ、これが細胞の増殖及び生存を刺激するからである(12,13)。そこで、我々は、30μMフォルスコリンと10μMの組合せによるヒトHSPCのインビトロ処理がどうかについて調べた。
トレプロスチニルは、細胞に対してよりアポトーシスを発生させやすくし(図8A&B)、又は細胞周期へのそれらの進入及び進行を妨げ(図8C&D)、又は特定系統を生み出すマウスHSPCの能力を変化させた(図8E&F)。原ドット図(図8A)に示され、図8Bにまとめられているように、生存細胞、早期アポトーシス細胞及び死細胞の存在は同等であり、アネキシン−V陽性細胞の数は、30μMのフォルスコリン及び10μMのトレプロスチニルでインビトロ処理しても増加しなかった。同様に、非同期的に増殖する非処理ヒトHSPC及びトレプロスチニルとフォルスコリンの存在下で維持されているHSPCの細胞周期分布は、HSPCが1時間暴露されようが又は24時間暴露されようが関わりなく同等であった(参照:代表的な原ヒストグラムについては図8C及び平均化されたデータについては図8D)。重要なことに、我々は、トレプロスチニルとフォルスコリンが骨髄コロニー及び赤血球コロニーの形成に及ぼす何の影響も検出できなかった。マウスのHSPCを骨髄から単離し、トレプロスチニル及びフォルスコリンの存在下で1時間インキュベートして、分化ならびにCFU−GM及びBFU−Eのコロニー形成単位の増殖を支持するのに必要な増殖因子入りのメチルセルロース含有培地中に再懸濁させた。10日後、コロニーの形態(図8E)及びコロニーの数(図8F)は同等であった。
前述のように、dmPGE2処理HSCの生着向上は、細胞生存、増殖及びホーミングに対する効果によるものであった(10)。我々の実験条件下で、トレプロスチニル及びフォルスコリンによるマウス及びヒトHSPCのインビトロ処理は、骨髄再構成を増進したが(以下及び図13参照)、インビトロにおける細胞生存性も細胞周期の進行も変化しなかった(参照:図8)。
AMD3100によるアンタゴニズムをインビボで再現した。致死的に放射線照射されたレシピエントマウスの骨髄をトレプロスチニル及びフォルスコリンで前処理されたマウスHSPCを用いて再構成し、マウスにはその後最適用量のトレプロスチニルを10日間皮下投与した。AMD3100の同時投与(3.3mg/kg/8時間)はトレプロスチニルの有益作用を鈍化した結果、すべてのレシピエントマウスは最終的に骨髄不全で死亡した(図11)。従って、総合すれば、観察から、トレプロスチニル処理HSPCにおけるトレプロスチニル誘導性のcAMP蓄積、CXCR4の発現増大、及びCXCR4によるシグナリングの増強の間には機構的な関連性が示されている。さらに、トレプロスチニルの作用はCXCR次第、すなわち、CXCR4によるシグナリングが遮断されると骨髄は生着せず、全動物とも死亡したことも文書に記されている。
いくつかのケモカインはCD26/DPPIV(ジペプチジルペプチダーゼIV)によって分解されることが知られている。これは、SDF−1/CXCL12にも該当する。トレプロスチニルの作用は−少なくとも一部は−CXCR4の誘導によって媒介され、CXCR4次第であるという発見を考慮すると。従って、増強されたSDF−1/CXCL12の作用は有益であることが予測される。その意味で、トレプロスチニル及びフォルスコリンによる処理がCD26の発現も誘導したことは考慮に値する。CD26は、非刺激HSPCでは本質的に検出できなかったが、トレプロスチニル及びフォルスコリンで刺激されたHSPCの10%を超える細胞で検出された(図9C)。いくつかのDPP−IV阻害薬は入手可能であり、それらの人体薬理学も良く理解されている。それらは長年、II型糖尿病の治療において何百万人の患者に投与されてきた。実際、大多数の患者はDPP−IV阻害薬に対する耐性があり、危険な副作用もない。
(i)インビトロでのトレプロスチニル+フォルスコリンの組合せによるHSPCの単独前処理は、統計学的に有意なホーミングの増進をもたらすのに十分でなかった(参照:図12の3番目のバー及び1番目のバー)。
致死的に放射線照射されたBALB/cマウスは、2*105個のLin−、c−Kit+、Sca1+ HSPCの静脈内注入により救われた。これらの条件下で、HSPCの数は、非処理HSPCを注入された全動物が死亡するほど限定的である(図13の実線)。これに対し、HSPCがインビトロでトレプロスチニルとフォルスコリンとの組合せで前処理され、レシピエント動物がトレプロスチニルを10日間投与された場合、60%の動物が生き残った(図13の三角/点線)。レシピエントマウスの生存は、HSPCがインビトロでトレプロスチニルとフォルスコリンとの組合せで前処理され、レシピエント動物がビルダグリプチンを10日間投与された場合、100%に向上した(図13の円/点線)。しかしながら、トレプロスチニルとビルダグリプチンの併用投与は、顕著な相互拮抗作用をもたらした。インビトロでトレプロスチニルとフォルスコリンとの組合せで前処理されたHSPCを注入され、その後トレプロスチニルとビルダグリプチンを投与された場合、大多数のレシピエントは死亡した(図13の四角/点線)。これらの観察は、図13にまとめられたホーミングアッセイの結果と一致している。言い換えれば、二つの独立した手法で、トレプロスチニルとビルダグリプチンの、インビボで同時投与された場合の相互拮抗作用と、適正な時系列で適用された場合、すなわちトレプロスチニル+フォルスコリンによるHSPCのインビトロ前処理と、それに続くビルダグリプチンのレシピエントマウスへのインビボ投与の相乗作用が報告された。
このプロジェクトの根底にある作業仮説は、フォルスコリン+トレプロスチニルの組合せで前処理されたHSPCを受けたレシピエントマウスへのビルダグリプチンの投与は、SDF−1/CXCL12の分解が阻害されるので、アップレギュレートされたCXCR4を介するシグナリングが増強されたことを事実と仮定している。もしそうであれば、ビルダグリプチンの作用は、AMD3100/プレリキサホルの同時投与によって鈍化するはずである。実験は7群のレシピエントBALB/cマウスで実施された(5匹/群)。これらの群のいくつかは内部標準で、先の発見が再現されることを確認するために含められた。全マウスともHSPCを受けたが、これらの細胞は尾静脈から注入される前にインビトロでトレプロスチニルとフォルスコリンとの組合せで前処理されていた。HSPCの数は限られていたので(BALB/cにおいて2*105/マウス)、前処理されたHSPCを注入するだけではレシピエントを救うのに不十分であった。ビルダグリプチンを注入されたレシピエントマウスが最良であった。彼らの生存は、トレプロスチニルを投与された群よりも良かった(図11のデータ、下記参照)。さらに、トレプロスチニルとビルダグリプチンとの組合せを投与された動物は思わしくなかった。従って、これらの観察は図13に示された結果を再現している。ビルダグリプチンとAMD3100/プレリキサホルの組合せを受けたレシピエントの生存は、ビルダグリプチン単独で処置された動物よりも低かった。従って、ビルダグリプチンの作用は、少なくとも一部は、CXCR4を介したSDF−1/CXCL12のシグナリングの増強によると説明できる。すべての条件は並行して試験されたので、比較は合理的であることが強調される。
主な発見は次のように要約できる:
(i)トレプロスチニルはdmPGE2とは異なる。なぜならば、トレプロスチニルは(マウス及びヒトの)HSPC中のGs共役プロスタノイド受容体を活性化するが、dmPGE2は、やはりHSPC上に存在するGi共役EP3受容体も活性化する。従って、トレプロスチニル(フォルスコリンと組み合わせて)による単独のインビトロ処理だけではインビトロにおけるHSPCの増殖及び生存も(図8)、インビボにおける骨髄へのホーミングも(図11)増進しない。これは、dmPGE2に関する公表データ(11)とは対照的である。ホーミングが増進されるのは、レシピエント動物がインビボでトレプロスチニルにより連続的に処置された場合だけである(図12)。
(i)トレプロスチニルとフォルスコリンとの組合せによるHSPCのインビトロ前処理は、トレプロスチニル又はビルダグリプチンのインビボ投与のいずれかと組み合わせると、骨髄再構成を増進できる。
Claims (15)
- ジペプチジルペプチダーゼIV(DPP−IV)の少なくとも一つの阻害薬を含む、造血幹細胞移植レシピエントの治療に使用するための組成物であって、前記造血幹細胞及び/又は前駆細胞は、生着を増進するために、移植前にインビトロでプロスタサイクリン類似体で処理されている、上記組成物。
- 造血幹細胞及び/又は前駆細胞が、移植前にプロスタサイクリン類似体及びcAMPエンハンサーで処理されている、請求項1に記載の使用のための組成物。
- DPP−IV阻害薬がグリプチンから選択される、請求項1又は2に記載の使用のための組成物。
- グリプチンが、シタグリプチン、ビルダグリプチン、アログリプチン、サキサグリプチン及びリナグリプチン、アナグリプチン、テネリグリプチン、ゲミグリプチン及びデュトグリプチンからなる群から選択される、請求項1〜3に記載の使用のための組成物。
- 前記プロスタサイクリン類似体が、トレプロスチニル、イロプロスト、シカプロスト及びベラプロスト又はそれらの薬学的に許容可能な塩の群から選択される、請求項1〜4のいずれか1項に記載の使用のための組成物。
- 前記プロスタサイクリン類似体が、トレプロスチニル、好ましくは、トレプロスチニルの酸誘導体、トレプロスチニルのプロドラッグ、トレプロスチニルの多形体又はトレプロスチニルの異性体の群から選択されるトレプロスチニルの誘導体である、請求項1〜5のいずれか1項に記載の使用のための組成物。
- 前記cAMPエンハンサーがフォルスコリンである、請求項1〜6のいずれか1項に記載の使用のための組成物。
- DPP−IV阻害薬がビルダグリプチンであり、造血幹細胞がインビトロでトレプロスチニル及びフォルスコリンで処理されている、請求項1に記載の使用のための組成物。
- DPP−IV阻害薬が、造血幹細胞移植の少なくとも5時間前、特に少なくとも10時間前、特に少なくとも15時間前、特に少なくとも24時間前に投与される、請求項1〜8のいずれか1項に記載の使用のための組成物。
- DPP−IV阻害薬が、造血幹細胞移植後少なくとも1日間、特に少なくとも2日間、特に少なくとも3日間、特に少なくとも4日間投与される、請求項1〜9のいずれか1項に記載の使用のための組成物。
- 骨髄疾患を患う個人の治療における、請求項1〜10のいずれか1項に記載の使用のための組成物。
- 骨髄疾患が、白血病、血液細胞コンパートメントの欠陥、化学療法又は放射線照射によって誘発された骨髄疾患である、請求項1〜11のいずれか1項に記載の使用のための組成物。
- 前記血液細胞コンパートメントの欠陥が、異常ヘモグロビン症、好中性顆粒球機能の欠陥、又は、Tリンパ球及び/又はBリンパ球の欠陥である、請求項12に記載の使用のための組成物。
- 造血細胞の生着能力を増強するための方法であって、
a)造血幹細胞又は前駆細胞のサンプルを用意し、
b)前記細胞に有効量のプロスタサイクリン類似体とcAMPエンハンサーを投与し、
c)前記混合物を、前記細胞におけるGアルファs−シグナリングを刺激するのに足る期間インキュベートし、
d)前記細胞を単離し、
e)前記細胞をそれを必要とする個人に移植し、
f)前記個人に有効量のDPP−IV阻害薬を投与する
連続工程を含む方法。 - 前記幹細胞又は前駆細胞が、臍帯血、ドナー骨髄又は胎盤由来である、請求項14に記載の方法。
