Abstract
Description
(a)天然起源のホスホジエステラーゼを阻害する天然若しくは人工の化合物、又は卵母細胞減数分裂の天然阻害剤を含む収集培地と、
(b)本明細書に記載の受精能獲得培地と、
(c)成熟培地と、
(d)接着性及び/又は非接着性の培養プレートと、
(e)上記キットの使用に関する指示書と、
を備える。
小胞状卵胞から発達上有能な成熟卵母細胞を凍結保存することが可能となり、
小胞状卵胞からCOC(該COCは、超音波ガイド下経腟回収によって穿刺された卵胞よりも小さな卵胞の切開により得られる)を培養することが可能性となる(癌治療の場合の妊孕性温存の分野に重要)。

特定の実施の形態では、上記生殖補助医療は、体外受精又はICSIを含む。
1.未熟な哺乳動物の卵丘細胞卵母細胞複合体の体外成熟に対する受精能獲得培地であって、0.1 nM〜50 nMのC型ナトリウム利尿ペプチド(CNP)と、エストラジオールと、卵胞刺激ホルモン(FSH)とを含み、特に10 nM〜50 nMのC型ナトリウム利尿ペプチド(CNP)を含み、更に25 nMのCNPを含む、受精能獲得培地。

材料及び方法
動物モデル
本研究に使用した動物は、CBAB6F1(C57Bl/6j×CBA/caのF1雑種)であった。これらの動物を、国内法令に従い、ブリュッセル自由大学(プロジェクト番号:09-216-1)の倫理委員会の同意により、収容して繁殖させた。
未熟なCOCの収集のため、卵胞発生の第一波のコンパクトなCOCを、事前の性腺刺激ホルモン投与をせずに青春期前(pre-pubertal)のマウス(19日齢〜21日齢)の小胞状卵胞から集めた。排卵前のCOC(対照)の収集のため、2.5 IUのウマ絨毛性性腺刺激ホルモン(ChorionicGonadotropin)(eCG、Folligon、オランダ国オスのIntervet)による48時間の初回刺激の後、青春期前の雌性マウス(25日齢〜27日齢)の大胞状卵胞を穿刺してコンパクトなCOCを集めた。収集培地は、10 %の熱不活性化ウシ胎児血清(FBS)と、100 IU/mlペニシリンと、100 μg/mlストレプトマイシン(いずれもベルギー国ヘントのLife Technologies製)とを含み、収集及び培養前の取り扱いの間に減数分裂再開を予防するため200 μMの3−イソブチル−1−メチルキサンチン(IBMX、ドイツ国シュネルドルフのSigma)を補充した、Leibovitz L-15からなった。
COC(前IVM相及びIVM相)の培養のための基本培養培地は、α-MEM、2.5 %FBS(いずれもベルギー国ヘントのLifeTechnologies製)及び5 ng/mLインスリン、5 ug/mLアポトランスフェリン、5 ng/mL亜セレン酸ナトリウム(いずれもドイツ国シュネルドルフのSigma製)で構成された。
規定の時間点で、口で制御するファインボア(fine bore)ガラスピペットを使用してコンパクトな又は拡大した卵丘細胞から卵母細胞を機械的に取り除いた。減数分裂再開を、ホフマンモジュレーションコントラストシステム(日本国東京のNikon)を備えた倒立顕微鏡下で核成熟ステージを判断することにより分析した。核成熟を、GV(卵核胞期)、GVBD(GVが不可視のとき)、PB(囲卵腔で第1極体が観察される)又はDEG(卵母細胞が変性されたとき)として採点した。
卵母細胞のクロマチン配置を、前IVM培養の前後に卵核胞卵母細胞で評価した。簡潔には、減数分裂再開の判断の後、GV卵母細胞を5分間の10 μg/mL Hoechst33258(Sigma、ドイツ国シュネルドルフ)で染色した。核小体のクロマチン配置を蛍光顕微鏡(IX70、Olympus)の下で分析した。クロマチン配置を、核小体のまわりのクロマチン凝集のパターンにより核小体を取り囲まない(NSN)段階、核小体を取り囲む(SN)段階、又は移行(NSN/SN)段階として分類した[27-29]。これらの卵母細胞のうちのいくつかの直径を、ヘキストで染色する前に記録した。
卵母細胞の発生能を評価する最終試験では、前IVM+IVM培養期間の後、体外受精(IVF)に続いて胚盤胞期までの胚培養を行った。この実験について、100 ng/mLのEREGをトリガーとして減数分裂再開に使用した。
別段の言及がなければ、結果を平均±SDとして示す。種々のin vitro条件間のIVF後の卵母細胞の減数分裂再開(1つの減数分裂期当たり)、クロマチン配置及び胚発生の比率の差を、分散分析に続くテューキー多重比較検定、p<0.05によって判断した。2つの条件を比較する場合、減数分裂再開の比率を比較するため、独立t検定を使用した。統計学的分析を行う前にパーセンテージデータを変換した(逆正弦)。
CNPは、効率的に減数分裂停止を維持し、EGFR依存性の減数分裂再開を遅延させる
性腺刺激ホルモン初回刺激マウス(26日齢〜27日齢)から回収された排卵前COCを、0 nM(対照)、1 nM、10 nM、100 nMのCNP-22の存在下で、4ng/mLのEGFと組み合わせて18時間に亘って培養に置いた。
上に記載される研究と同様に、未熟なCOC(高度に減数分裂上/発生上、無能力)を用いる追試を行った。かかるCOCを(材料及び方法に記載されるように)小胞状卵胞から回収し、48時間培養に置いた。結果は、CNP-22が48時間ではなく24時間に亘る減数分裂停止を維持することができることを示したことから(データは示されていない)、CNP-22への反応性がE2、FSH及びGDF9による培地補充によって支持される研究を計画した。
未熟なCOCを雌性マウス(19日齢〜20日齢)の小胞状卵胞から回収し、25 nM CNP-22及び10 nM 17-β-エストラジオールの存在下、2.5 mIU/mL FSH、又は2.5 mIU/mL FSH+50 ng/mLGDF9を添加して、48時間の培養に置いた(前IVM条件)。これらの前処理に続いて、EGFを含む培地による18時間の排卵刺激を行った(IVM条件)。
前IVMに続いてIVM培養期間を経る、卵母細胞の発生能を調べた。卵母細胞を体外受精させ、胚を5日目まで培養した。
上述の実験は、本発明による受精能獲得培地の使用が、減数分裂停止状態に卵母細胞を維持するだけでなく、IVMを経るかかるCOCの能力に影響せずにそのようにすることを示す。しかしながら、受精能獲得培養中の卵母細胞発生能が大きく改善されたことから、ホスホジエステラーゼ(PDE、減数分裂再開中の卵母細胞内のcAMPの分解を担う酵素)に対する効果によってCNPの作用を有することから、CNPの作用は排他的に考慮することができない。したがって、「低」用量のCNP(特に1 nM〜50 nMのCNPの濃度範囲において)では、卵母細胞及び卵丘細胞の間の良好な伝達の維持により、卵母細胞の発生能の改善において更なる効果(追加的効果)を有すると仮定した。
動物モデル
本研究に使用した動物は、CBAB6F1(C57Bl/6j×CBA/caのF1雑種)であった。これらの動物を、国内法令に従い、ブリュッセル自由大学(プロジェクト番号:14-216-1)の倫理委員会の同意により、収容して繁殖させた。
COC(前IVM相及びIVM相)の培養のための基本培養培地は、α-MEM、2.5 %FBS(いずれもベルギー国ヘントのLifeTechnologies製)及び5 ng/mLインスリン、5 ug/mLアポトランスフェリン、5 ng/mL亜セレン酸ナトリウム(いずれもドイツ国シュネルドルフのSigma製)で構成された。受精能獲得培地は、10 nM E2 17-β-エストラジオールと組み合わせて25 nM CNP、又は1 μMOrg9935若しくは1 μMシロスタミドのいずれかを補充した基本培地からなった。CNPをPhoenix Europe(ドイツ国カールスルーエ)から得て、シロスタミドをEnzo Life Sciences(ベルギー国アントウェルペン)から得た。実験の目標がCNPとPDE3阻害剤との間に明確な潜在的な能力の差を作ることであったため、FSHの干渉の可能性を回避することは決定的であり、したがって、後者を受精能獲得培地から除いた。FSHの存在下で試験した場合、後者は、それ自体をPDE3阻害剤の存在下でより一層明白である上記接続に寄与させ、卵母細胞及び卵丘細胞の周囲層の間の接続に対するCNPの効果をマスキングした(データは示されていない)。
透明帯を貫通する突起(TZP、卵母細胞と接続する顆粒膜細胞からの膜性の膨張)は、テキサスレッド−ファロイジン又はActin green(商標)でFアクチンを蛍光標識することにより証明され、透明帯(矢印)を通り抜けるフィラメントとして見えた。
3つの化合物が卵母細胞の減数分裂停止を維持することができたのに対し、予想外にも、CNPが、卵母細胞と卵丘細胞の周囲層との間の双方向性の伝達に必須の透明帯を貫通する突起を保持することができる因子であることが明らかになった(図6及び図7)。図7では、平均ピクセル強度を、透明帯を通り抜ける陽性アクチン染色(TZP)を定量する手段として使用した。画像分析は、ImageJを用いて行われ、卵母細胞と卵丘細胞との間に輪郭が描かれた目的の領域(Region of Interest:ROI)、すなわち透明帯が位置する領域の平均ピクセル強度の計算で構成された。画像分析の後、透明帯を貫通する突起(TZP)によって、CNPに暴露されたCOCが卵母細胞及び卵丘細胞の間の連結性をより良好に保持したことが観察された。図7(A)では、Org9935をPDE3に対する阻害剤として使用し、アクチンフィラメントをテキサスレッド結合ファロイジンで証明し、一方、図7(B)では、シロスタミドをPDE3に対する阻害剤として使用し、アクチンフィラメントをActin green(商標)で証明した。CNP群及びPDE3i群を、パネルAに対するP値0.0082及びパネルBに対するP値<0.0001により、マンホイットニー検定を使用して、統計学的に比較した。
先の結果が、CNPがCOCにおける卵母細胞と卵丘細胞との間の連結性を増強することを実証することから、以下の結果は、初期胞状卵胞から培養されたかかるCOCの発生能にCNPが影響を及ぼすかどうかを判断することを目標とした。この研究では、初期胞状卵胞に由来するマウスの卵丘細胞に囲まれた卵母細胞の発生能に対する、受精能獲得培養中に存在するCNP及びPDE3Iの差動効果を評価した。実験の目標がCNPとPDE3阻害剤との間に明確な潜在的な能力の差をつくることであったため、FSHの干渉可能性を回避することは決定的であり、したがって、この実験においても後者は受精能獲得培地から除いた。
未熟な卵丘細胞−卵母細胞複合体を、初期胞状卵胞(刺激されていない19日齢〜20日齢のマウスに由来)から単離した。実施例2と同様、CNP又はシロスタミド(PDE3阻害剤)のいずれかの存在下で48時間の間、COCを基本培養培地に置いた。
1)受精能獲得培養を行わない対照条件をIVMの前に行うもの、
2)標準的なin-vivo対照、すなわち26日齢〜27日齢のマウスに由来する完全に成長した成熟卵母細胞。これらのCOCは、23日齢〜24日齢のマウスに対する48時間のPMSGに続くhCGの投与によって得られた(IVFプロトコル)。
CNP群とシロスタミド群との間の受精及び胚盤胞形成の比率の差を、カイ2乗によって評価した。
両方の処理の受精率に対して差が観察されなかったのに対し(図8(A)を参照されたい)、1つの受精卵当たりの形成された胚盤胞の量は、CNPの存在下での受精能獲得培養の後で有意に高かった(図8(B)を参照されたい)。
PDE阻害剤とCNPを比較するこれらの2つの補足的研究における卵母細胞及び胚の質に対して観察された予期しない差は、CNPに起因する可能性がある。これらの結果は、CNPが3型ホスホジエステラーゼの調節により媒介したものを越えて延長する作用を有することを示唆する。PDE3の作用の阻害の他に、CNPは、卵母細胞と卵丘細胞との間の物理的な連結性を増加させ、卵母細胞の最終的な発生(細胞質の成熟)を完了するのに必須の因子の獲得を増強する。
実施例1では、CNP-22が減数分裂停止の維持に対して用量依存性の効果を有したことが実証された。以下の研究では、より広いCNP範囲を含む更なる実験を行った。
この更なる実施例で使用される材料及び方法は、適宜、実施例1で使用される材料及び方法と共通する。24日齢〜26日齢のマウスに由来する卵丘細胞−卵母細胞複合体を、ウマ絨毛性性腺刺激ホルモン(eCG、Folligon、オランダ国オスのIntervet)刺激の48時間後に、in vivoで成長させた胞状卵胞から単離した。22日齢〜24日齢の時、マウスに2.5IUのeCGを注射した。少なくとも2つの卵丘細胞の層を有する無傷のCOCを、以下の用量のCNPの存在下で18時間に亘り培養に置いた:

卵母細胞の減数分裂再開(1つの減数分裂期当たり)の比率の差を、分散分析に続いてテューキーの多重比較検定、p<0.01によって評価した。統計学的分析を行う前にパーセンテージデータを変換した(逆正弦)。
概して、本補足実験は、CNP-22が減数分裂停止の維持に対して用量依存性効果を有していることを確認した。卵母細胞の減数分裂停止を、倒立顕微鏡下で無傷の卵核胞(GV)の存在によって確認した。
概して、これらのデータは、1 nMから100 nMに及ぶ用量間隔で80%超の比率で排卵前の卵母細胞においてCNP-22が減数分裂停止を維持することができることを示唆し、全ての卵母細胞をGV期における停止状態に維持するために使用されるCNPの望ましい用量は、10 nM〜50 nM、より好ましくは10nM〜25 nMの用量となるという特許請求の範囲と一致する。そのため、この挙動は、用量を増加させることが卵母細胞を連続的に停止させたままにしたPDE3阻害剤の使用とは異なる。
上述のCNPの効果は、CNP-22にそれ自身制限されないことを裏付けるため、、これらの効果がCNP類縁体によって再現され得るかどうかを試験する追加実験を行った。CNP類縁体であるCNP-53の効果を試験する追加実験を行った。CNP-22と同様、CNP-53は、53個のアミノ酸配列を含むC型ナトリウム利尿ペプチドの主な内因性形態のうちの1つである。
この更なる実施例で使用される材料及び方法は、代わりにCNP-53を使用することを除いて、適宜、実施例4で使用される材料及び方法と共通する。24日齢〜25日齢のマウスに由来する卵丘細胞−卵母細胞複合体を、eCG刺激の48時間後に、in vivoで成長させた胞状卵胞から単離した。22日齢〜23日齢の時、マウスに2.5IUのeCGを注射した。無傷のCOCを、以下の用量のCNP-53の存在下で18時間に亘り培養に置いた:

CNP-22を使用する補足実験で見出された結果に類似して、CNP-53は、卵母細胞の減数分裂停止の維持に対して予想外の(最大の)用量依存性効果を有することを実証した。
全体として、これらのデータは、CNP-22に匹敵し、CNP-53は高い比率で排卵前の卵母細胞において減数分裂停止を維持することができることを示す。さらに、CNP-53が効率的な用量は、CNP-22のものに類似し、すなわち1 nM、10 nM、50 nM及び100 nMであり、全ての排卵前の卵母細胞において減数分裂停止を維持するのに最適な10nM〜50 nMの用量範囲を含む、1 nM〜100nMのCNPの範囲において異なる発現をしている。
前臨床試験の結果によれば、本発明の受精能獲得培地は、小卵胞に由来する卵母細胞の成熟に確かに予期しない効果を有する。この研究では、本出願の受精能獲得培地を使用するIVM方法が、卵母細胞の発生可能性に対する効果を有するかどうか、それらが受精可能な卵母細胞をもたらすかどうかについて、また、それらの胚盤胞形成可能性が増加するかどうかについて判断した。
研究(N=15)に含まれるIVM治療を受ける患者は、研究胚の作製のため、卵母細胞の一部を寄贈することを承諾し、次の特徴を有した:年齢37歳未満、ロッテルダム基準(RotterdamESHRE/ASRM-Sponsored PCOS consensus workshop Group, 2004)による多嚢胞性卵巣症候群(PCO又はPCOS)の病歴。
実験的治療=COC受精能獲得(本発明による受精能獲得培地を使用する)+IVM
日常的な臨床群=従来のIVM(Origio(商標)IVM方法論、VUB適応)
70mmHgの吸引圧で17ゲージ単一内腔針を用いて2 mm〜10 mmの卵胞から卵丘細胞−卵母細胞複合体(COC)を回収し、25 IU/mlのヘパリン(Heparin Leo、ベルギー国のLeo Pharma)を補充した50 μMのIBMX(Sigma)を含む「収集培地」へと収集した。
図11及び図12は、欧州ICSIデータ(正常に刺激されたサイクルに由来する)及び、UZBrussel(2014-2015)において日常的に適応されるIVMに対して、「新たな受精能獲得培養+IVM」の結果と、同胞の卵母細胞に対する「日常的なIVM」とを比較している。
IVM治療の一部としての「新たな受精能獲得培養」培養工程の適用は、in-vivoで成長させた(刺激後の大きな卵胞から得られる)卵母細胞に類似する程度までの小卵胞に由来する卵母細胞の成熟を増強する。このより高い成熟の効果は、初期胚発生の間持続され、従来のIVMと比較して、2倍の量の良質な胚をもたらす。初期胚発生中のかかる持続効果の達成は期待を超えており、現在用いられているIVM法と比較すると、柔軟なIVM法を作り出すものである。

Claims (18)
- 0.1nM〜50 nMのC型ナトリウム利尿ペプチド(CNP)と、エストラジオールと、卵胞刺激ホルモン(FSH)とを含む、未熟な哺乳動物の卵丘細胞卵母細胞複合体の体外成熟に対する受精能獲得培地。
- 0.1ng/ml〜10 ng/mlのインスリンを更に含む、請求項1に記載の受精能獲得培地。
- 卵母細胞分泌因子、又は、GDF-9、BMP-15、FGF-8、それらの類縁体、若しくはそれらの任意の組み合わせからなる群から選択される卵母細胞分泌因子を含む、請求項1又は2に記載の受精能獲得培地。
- 未熟な哺乳動物の卵丘細胞卵母細胞複合体の体外成熟方法であって、未熟な卵丘細胞卵母細胞複合体を収集培地中に収集することと、哺乳動物の卵丘細胞卵母細胞複合体と、請求項1〜3のいずれか一項に記載の受精能獲得培地とを接触させることと、哺乳動物の卵丘細胞卵母細胞複合体と成熟培地とを更に接触させることとを含む、方法。
- 前記哺乳動物の卵丘細胞卵母細胞複合体を、最短2時間〜最長96時間に亘って受精能獲得培地と接触させる、請求項4に記載の方法。
- 非接着性若しくは接着性の培養プレート、又は非接着性培養プレートにおいて、前記哺乳動物の卵丘細胞卵母細胞複合体を、前記受精能獲得培地と接触させる、請求項4又は5に記載の方法。
- 体外受精又はICSIを含む生殖補助医療の一部である、請求項4〜6のいずれか一項に記載の方法。
- 請求項1〜3のいずれか一項に記載の受精能獲得培地を備える、未熟な哺乳動物の卵丘細胞卵母細胞複合体の体外成熟に対するキット。
- 未熟な哺乳動物の卵丘細胞卵母細胞複合体の体外成熟に対するキットであって、
(a)天然起源のホスホジエステラーゼを阻害する天然若しくは人工の化合物、又は卵母細胞減数分裂の天然阻害剤を含む収集培地と、
(b)請求項1〜3のいずれか一項に記載の受精能獲得培地と、
(c)成熟培地と、
(d)非接着性又は接着性の培養プレートと、
(e)前記キットの使用に関する指示書と、
を備える、キット。 - 哺乳動物の卵丘細胞卵母細胞複合体の体外成熟に対する、請求項1〜3のいずれか一項に記載の受精能獲得培地の使用、又は請求項8若しくは9に記載のキットの使用。
- 前記哺乳動物の卵丘細胞卵母細胞複合体を、最短2時間〜最長96時間に亘って前記受精能獲得培地と接触させる、請求項1〜3のいずれか一項に記載の受精能獲得培地の使用、又は請求項8若しくは9に記載のキットの使用。
- 前記哺乳動物の卵丘細胞卵母細胞複合体を、非接着性若しくは接着性の培養プレート、又は非接着性培養プレートにおいて、前記受精能獲得培地と接触させる、請求項1〜3のいずれか一項に記載の受精能獲得培地の使用、又は請求項8若しくは9に記載のキットの使用。
- 前記哺乳動物の卵丘細胞卵母細胞複合体の卵母細胞を減数分裂停止状態に維持するための、請求項1〜3のいずれか一項に記載の受精能獲得培地の使用、又は請求項8若しくは9に記載のキットの使用。
- 哺乳動物の卵丘細胞卵母細胞複合体の成熟を誘導するための、請求項1〜3のいずれか一項に記載の受精能獲得培地の使用、又は請求項8若しくは9に記載のキットの使用。
- 体外受精又はICSIを含む生殖補助医療の一部としての、請求項1〜3のいずれか一項に記載の受精能獲得培地の使用、又は請求項8若しくは9に記載のキットの使用。
- 癌治療の場合又は卵母細胞バンキングの場合又は卵母細胞バンキングの場合であって年齢バンカー(35歳〜40歳)に対しての妊孕性温存法における、請求項1〜3のいずれか一項に記載の受精能獲得培地の使用、又は請求項8若しくは9に記載のキットの使用。
- 前記妊孕性温存法が小胞状卵胞の凍結保存を含む、請求項16に記載の使用。
- 前記受精能獲得培地が、前記凍結保存された小胞状卵胞の体外成熟に使用される、請求項17に記載の使用。
