Abstract
Description
(i)抗原に対して活性化されたT細胞を有する患者に、ペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞、およびTh-1媒介応答を支持するアジュバントを投与する段階であって、該ペプチドが抗原含有タンパク質に由来しそれによって活性化T細胞が再活性化される、前記段階
を含む、抗原含有タンパク質に対する細胞性の細胞傷害性免疫応答を延長する方法における使用のための医薬であって、
段階(i)のペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞が、T細胞が抗原に対して活性化された後の0時間〜14日の時間枠において投与される、前記医薬に関する。
(i)抗原に対して活性化されたT細胞を有する患者に、ペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞、およびTh-1媒介応答を支持するアジュバントを投与する段階であって、該ペプチドが抗原含有タンパク質に由来しそれによって活性化T細胞が再活性化される、前記段階
を含む、抗原含有タンパク質に対する細胞性の細胞傷害性免疫応答を延長する方法における使用のための医薬であって、
段階(i)のペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞が、T細胞が抗原に対して活性化された後の0時間〜14日の時間枠において投与される、前記医薬に関する。
(i)抗原に対して活性化されたT細胞を有する患者に、ペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞、およびTh-1媒介応答を支持するアジュバントを投与する段階であって、該ペプチドが抗原含有タンパク質に由来しそれによって活性化T細胞が再活性化される、前記段階

段階(i)のペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞が、T細胞が抗原に対して活性化された後の0時間〜14日の時間枠において投与される、前記医薬に関する。
(i)抗原に対して活性化されたT細胞を有する患者に、ペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞、およびTh-1媒介応答を支持するアジュバントを投与する段階であって、該ペプチドが抗原含有タンパク質に由来しそれによって活性化T細胞が再活性化される、前記段階
を含む、抗原含有タンパク質に対する細胞性の細胞傷害性免疫応答を延長する方法であって、
段階(i)のペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞が、T細胞が抗原に対して活性化された後の0時間〜14日の時間枠において投与される、前記方法に関する。
・ ペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞のさらなる投与は行われないか、または
・ ペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞のさらなる投与は、T細胞が抗原に対して活性化された後の0日〜14日の時間枠内で行われない。
抗原含有タンパク質もしくはその抗原含有断片、または抗原含有タンパク質もしくはその抗原含有断片をコードする核酸を、少なくとも1つの主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞中へ導入するための、物理的、化学的または生物学的な方法を適用して、
核酸の場合、少なくとも1つの細胞が、抗原含有タンパク質もしくはその抗原含有断片を少なくとも1つの主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞中へ発現することを可能にすること、および
少なくとも1つの細胞が、該抗原を処理し、MHC-Iの環境においてペプチドを提示することを可能にすること
によって、段階(i)のペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞が入手される。
a.少なくとも1つの主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞を、抗原含有タンパク質もしくはその抗原含有断片と共にインキュベートすること、および
少なくとも1つの細胞が、該抗原を処理し、MHC-Iのコンテクストにおいてペプチドを提示することを可能にすること、
または
b.少なくとも1つの主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞を、(i)該細胞に感染しそして(ii)該細胞中において抗原含有タンパク質もしくはその抗原含有断片を発現することができるウイルスシステムへ曝露すること、および
少なくとも1つの細胞が、該抗原含有タンパク質抗原もしくはその抗原含有断片を処理することを可能にすること、および
少なくとも1つの細胞が、MHC-Iのコンテクストにおいてペプチドを提示することを可能にすること、
または
c.少なくとも1つの主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞に、抗原含有タンパク質もしくはその抗原含有断片をコードする核酸を含むDNA、もしくはRNA、もしくはDNA-もしくはRNA-ベースの発現システムをトランスフェクトすること、および
少なくとも1つの細胞が、該抗原含有タンパク質抗原もしくはその抗原含有断片を発現および処理することを可能にすること、および
少なくとも1つの細胞が、MHC-Iのコンテクストにおいてペプチドを提示することを可能にすること、
または
d.少なくとも1つの主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞を、細胞透過性ペプチド(CPP)と抗原含有タンパク質もしくはその抗原含有断片とを含む化合物と共にインキュベートすること、および
少なくとも1つの細胞が該化合物を処理することを可能すること、および
少なくとも1つの細胞が、MHC-Iのコンテクストにおいてペプチドを提示することを可能にすること。
(a)抗原提示細胞の表面上の受容体に結合する分子、
(b)(a)の分子に結合された前記抗原含有タンパク質
を含む送達システムを用いる方法は、患者中の抗原に特異的なT細胞を活性化することに適しており、ここで、該受容体に(a)の分子が結合すると、(b)のタンパク質が内部移行して抗原提示細胞中で処理され、該タンパク質中に含有される抗原が抗原提示細胞の表面上に提示される。例えば、樹状細胞である抗原提示細胞(APC)が、特に有用であることが判明した。しかし、B細胞もまた、抗原提示細胞として成功裏に使用された。
(1)
(a)抗原提示細胞の表面上の受容体に結合する分子、
(b)(a)の分子に結合された前記抗原含有タンパク質
を含む送達システムであって、
該受容体に(a)の分子が結合すると、(b)のタンパク質が内部移行して抗原提示細胞中で処理され、該タンパク質中に含有される抗原が抗原提示細胞の表面上に提示され、それによって患者においてT細胞が活性化される、送達システム
を前記患者に投与すること;および
(2)Th-1媒介応答を支持するさらなるアジュバントを投与すること。
(1)前記患者から得られた1つまたは複数のT細胞を、抗原含有タンパク質またはその抗原含有断片を用いてインビトロで活性化する段階、および
(2)段階(1)において得られた1つまたは複数のT細胞を該患者に再移入する段階、および
(3)任意で、Th-1媒介応答を支持するさらなるアジュバントを投与する段階。
(1)以下より選択される送達システムを前記患者に投与すること:
(a)抗原含有タンパク質またはその抗原含有断片を含む薬学的組成物、好ましくは、抗原含有タンパク質またはその抗原含有断片に結合された担体、特にナノ粒子またはリポソームを含む、薬学的組成物、および
(b)該抗原含有タンパク質またはその抗原含有断片をコードする核酸配列を含み、該患者において該抗原含有タンパク質またはその抗原含有断片を発現することができる核酸であって、
好ましくは、
・ 該核酸はDNAもしくはRNAであり、かつ/または
・ 該核酸は、ウイルスシステム、例えば、弱毒化ウイルス、非複製ウイルスシステム、ターゲティンクされたウイルスワクチンシステムもしくはターゲティンクされていないウイルスワクチンシステムであるか、または
・ 該核酸は非ウイルス発現システムであり、
それによって少なくとも1つのT細胞が活性化される、前記核酸;
ならびに
(2)Th-1媒介応答を支持するさらなるアジュバントを投与すること。
(i)抗原に対して活性化されたT細胞を有する患者に、静脈内投与によってペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞、およびTh-1媒介応答を支持するアジュバントを投与する段階であって、該ペプチドが抗原含有タンパク質に由来しそれによって活性化T細胞を再活性化し、かつ、該ペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞が樹状細胞ではない、前記段階
を含む、抗原含有タンパク質に対する細胞性の細胞傷害性免疫応答を延長および/または増幅する方法における使用のための医薬に関する。
(i)抗原に対して活性化されたT細胞を有する患者に、全身投与によってペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞、およびTh-1媒介応答を支持するアジュバントを投与する段階であって、該ペプチドが抗原含有タンパク質に由来しそれによって活性化T細胞を再活性化し、かつ、該ペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞が樹状細胞ではない、前記段階

XCR1+ DC中への抗原のターゲティンク後の細胞傷害活性の誘導(図1)
モデル抗原OVAを、XCR1特異的mAb MARX10(Bachem et al. 2012)へ組換え融合したか、またはXCR1受容体へ特異的に結合するケモカインリガンドXCL1(Hartung et al. 2015)へ組換え融合した。MARX10-OVAまたはXCL1-OVAを低レベルでナイーブC57BL/6マウス中へ静脈内注射した場合、抗原はXCR1+ DC中へターゲティンクされた。この抗原プライミングがアジュバント(3μg LPS、CpG、または10μg ポリI:C)の存在下で生じた場合、実質的な細胞傷害活性が誘導された(アジュバントとしてポリI:Cを用いたデータを示す)。この細胞傷害活性を、第6日にプライミングした動物へ標的細胞(脾臓リンパ球)を静脈内注射することによって試験し、これらには、OVA由来ペプチドである、SIINFEKL(SEQ ID NO: 11)が前もってインビトロで結合されていた。標的細胞を、結合後にフルオロフォアCFSEで高程度に標識し、一方、非結合対照脾臓リンパ球をCFSEで低程度に標識した。図1Aに示されるように、OVA+アジュバントでのプライミング動物は、細胞傷害活性を生じさせ、これはほぼ全てのSIINFEKL結合標的細胞を排除した。図1Bは、XCR1+ DCへターゲティンクされた様々な量の抗原で得られた用量応答曲線を示す。同一の結果が、XCR1+ DC中への免疫原性ペプチドのターゲティンクのためにMARX10-SIINFEKLまたはXCL1-SIINFEKLを使用した後に得られた。
誘導された細胞傷害活性による感染症からのまたは癌細胞の播種からの保護(図2)
C57BL/6マウスを、MARX10-OVA(2μgのOVAを含有)およびアジュバント(10μgのポリI:C)でプライミングしたか、または未処置のままにした。5日後、全てのマウスに、1×106 CFU (= 5x LD50)の、組換え操作によりペプチド配列SIINFEKL(SEQ ID NO: 11)が入れられたL.モノサイトゲネス(L. monocytogenes)株を感染させた(Foulds et al., 2002, J. Immunol. 168, 1528-1532)。全ての未処置マウスは3〜4日以内に死滅したが、抗原性プライミングによる誘導レベルの細胞傷害性は、疾患から全ての動物を完全に保護した(図2A)。
抗原性ペプチドSIINFEKL(SEQ ID NO: 11)が結合した同系リンパ球の注射によって得られたCD8+ T細胞の細胞傷害性の増幅(図3)
C57BL/6マウスを、第-20日、第-15日、第-10日、第-7日、第-5日、または第-3日に、MARX10-OVA(2μg OVAを含有)およびアジュバント(ポリI、CpG、またはLPS;10μg ポリI:Cを用いたデータを示す)でプライミングした。第0日に、プライミングした動物に、10×106個の、注射前にインビトロでSIINFEKL(SEQ ID NO: 11)を結合させた脾細胞(「抗原依存的増幅システム」、ADAS)を静脈内注射した(図3A)。ペプチド結合細胞と一緒にアジュバントを注射し(様々な量のLPSまたはポリI:C)、50μgのポリI:Cを用いたデータを示す。第10日に動物を犠牲にし、SIINFEKL特異的CD8+ T細胞の総数を、SIINFEKL特異的四量体およびフローサイトメトリーを使用して脾臓中において測定した(図3A)。結果は、抗原特異的CD8+ T細胞の増幅のために使用したシステムが、最初の抗原刺激後5日〜9日の狭い時間枠において有効であったことを実証した(図3A)。
ペプチド結合細胞および複合体化IL-2の同時適用による細胞傷害性CD8+ T細胞の高相乗的増幅(図4)

定着腫瘍の処置における抗原ターゲティンクならびにペプチド結合細胞および複合体化IL-2の同時適用の相乗効果(図5)
C57BL/6マウスに、OVAを発現するように操作された悪性同系腫瘍株である5×105個のEG.7細胞(Moore et al. 1988)を皮下注射した。腫瘍注射後第6日に、マウスをMARX10-OVA(2μg OVAを含有)およびアジュバント(10μg ポリI:C)でプライミングし、ある群を未処置のままにした。プライミングの6日後(第11日)、ある群のマウスにSIINFEKL結合脾細胞(10×106)およびアジュバント(50μg ポリI:C)を注射した(「プライミング+ADAS」)。別のプライミングした群のマウスに、第12日、第13日、第14日、第15日、第16日、第17日、第18日、第19日、第21日、および第23日にIL-2cxのみを注射した(「プライミング+IL-2cx」)。別のプライミングした群のマウスに、SIINFEKL結合脾細胞(10×106)およびアジュバント(50μg ポリI:C)を第11日に、ならびにIL-2cxを第12日、第13日、第14日、第15日、第16日、第17日、第18日、第19日、第21日、および第23日に注射した(「プライミング+ADAS+IL-2cx」)。第25日に全てのマウスを犠牲にした(腫瘍が直径1 cmを超えたため、一部の対照動物をより早く犠牲にしなければならなかった)。全てのデータは、各処置群(n=6)における腫瘍の平均サイズ(mm2)を示す。結果は、ターゲティンクされたOVAを用いる腫瘍注射マウスのプライミング単独は、有効ではなかったことを実証している。対照的に、IL-2cxまたはADASのいずれか単独は、プライミングしたマウスにおいておよそ7〜10日間腫瘍塊を強く減少させることによって有効であったが、その後腫瘍の成長を防ぐことができなかった。ADASおよびIL-2cxの組み合わせをプライミングした動物へ適用した場合、腫瘍塊は強く減少し、腫瘍は実験の終了時まで完全に抑制された(一部のマウスは腫瘍組織を完全に退けた)。
様々な様式の免疫化は低頻度のプライミングされた抗原特異的CD8+ T細胞を生じさせる。これらはADAS手順を用いて概して強く拡大させそしてキラーCD8+ T細胞へ分化させることができる。
MAb MARX10(Bachem et al., Front Immunol 2012, EP2641915A1)は、XCR1+ DCについての系統マーカーであるXCR1を認識し、Mab DEC-205(NLDC-145, Kraal et al., 1986, Biolegendから入手)は、マウスXCR1+ DC上に発現されるCD205分子を認識し、mAb 33D1(Nussenzweig et al., 1982, ATCCから入手)は、SIRPα+ DC上のDCIR2分子を認識し、mAb 1D3はB細胞上のCD19を認識し(Krop et al., 1996, ATCCから入手)、mAb MOPC-21(Potter et al., J Natl Cancer Inst 1961, Biolegendから入手)はマウスにおけるいずれの分子も認識せず、従ってIgG1アイソタイプ対照として使用される。XCL1は、XCR1についてのケモカインリガンドであり、マウスおよびヒトにおけるXCR1+ DCへの抗原のターゲティンクのために使用され得る(Hartung et al., 2015)。
ADASについてのプライマリー細胞の細胞質画分中への抗原の送達は抗原由来ペプチドのMHC-Iへの有効な結合をもたらす
本発明者らは、様々なプライマリー細胞(例えば、B細胞、T細胞、DC、脾細胞)のMHC-Iへの抗原由来ペプチド(例えば、SIINFEKL)の外部的結合が、ADASのために使用できることを実証した。本発明者らは、同じ手順が、上述したように、タンパク質全体をプライマリー細胞中へ導入するかまたはあるタンパク質を細胞内部で発現させる様々な方法でも機能するだろうと予想した。

Th1アジュバントの適用はプライミングステップおよびADAS手順の両方において抗原の適用から時間的に分離され得る
宿主の免疫化を達成するために、抗原は抗原と一緒に適用されなければならないと、現在、一般的に仮定されている(Cohn et al. 2014)。従って、抗原は通常アジュバントと混合され、一緒に適用される。アジュバントは、最適な結果を達成するために、理想的にはさらには抗原へ物理的に連結されるべきであると、現在、一般的に仮定されている。従って、プライミングおよびADAS手順におけるアジュバント送達からの抗原投与の分離が良好かつさらにはより良い免疫応答をもたらすことを本発明者らが理解したことは、非常に驚くべきことであった。
ADASと複合体化IL-15の投与とを組み合わせることによる細胞傷害性CD8+ T細胞の高相乗的増幅
本発明者らは、第0日におけるMARX10-OVA(2μg)およびポリI:C(10μg)を用いたプライミング後の第1日、第2日、および第3日における複合体化IL-2(IL-2cx=その高親和性受容体CD25へのIL-2の結合を遮断する抗体と複合体化されたマウスIL-2)の注射が、第6日におけるプライミングされたCD8+ T細胞の数を実質的に上昇させなかったことを実証した。しかし、動物を、第0日にMARX10-OVA(2μg)およびポリI:C(10μg)でプライミングし、第5日にADAS(SIINFEKLが結合した10×106個の脾細胞および50μg ポリI:Cの注射)へ供した場合、第6日、第7日、第8日および第9日にIL-2cxを注射することによって、第10日における脾臓中のSIINFEKL特異的CD8+ T細胞の数は劇的に上昇した(実施例4および図4)。
抗原特異的様式で記憶CD8+ T細胞を再活性化および拡大するためにADAS手順を使用することができる
第0日に2μgのMARX10-OVAおよび10μgのポリI:Cでプライミングしたナイーブ動物を用いて、本発明者らは、マウスにおいて、ADASは第3日に行った場合は応答の増幅についてあまり効果がないが、その後に有効となり、少なくとも第20日まで引き続いてある程度有効であり(実施例3および図3)、第4日から第9日の間が最も有効であることを実証することができた。
ADASはインビトロで活性化され養子移入されたCD8+ T細胞でも有効である
OT-Iマウスの脾細胞を、アジュバント無しで、3日間、ペプチドSIINFEKLを用いてインビトロで活性化した。しかし、CD8+ T細胞のこの活性化は、Th1アジュバントの存在下でも行うことができた。培養後、細胞(この時点で97% OT-I CD8+ T細胞から構成された)を第0日に同系C57BL/6マウス中へ移入した。ADAS(10×106個のSIIFEKL結合脾細胞および50μg ポリI:Cの静脈内注射)を第0日、第1日、第2日、第3日、第4日、第5日、または第6日のいずれかにおいて適用した。第1日、第2日または第3日におけるADASは、宿主動物中の養子移入されたCD8+ T細胞の頻度に対して有意な効果を有さなかった。しかし、移入されたCD8+ T細胞の明らかな増幅が、第4日に適用されたADASで観察され、最適な増幅が第5日におけるADASで見られた(図11)。同時に、移入されたCD8+ T細胞のエフェクター細胞への成熟を示す、表面マーカーKLRG1の発現が、第1日におけるADASでの30〜40%から第5日および第6日における80〜90%へ上昇した。これらのデータは、ADAS手順が、インビトロで活性化されそして養子移入された抗原特異的CD8+ T細胞の集団を強力に増幅することができると結論付けることを可能にする。同時に、ADASは、細胞傷害性エフェクター細胞へのこれらのCD8+ T細胞のさらなる分化を誘導する。マウスにおける実験による、ヒトにおける正確な予測は可能でないため、ADASについての最適な時点はヒトにおいて異なり得る。従って、養子移入されたCD8+ T細胞に対する最適なADAS効果のための期間は、より延長され得る。ADASは0時間〜14日という養子移入後の短い時間枠内で有効であるだけでなく、養子移入されたCD8+ T細胞が長い期間の後に記憶状態へ戻った場合も有効であることが予想され得る。
Claims (28)
- (i)抗原に対して活性化されたT細胞を有する患者に、ペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞、およびTh-1媒介応答を支持するアジュバントを投与する段階であって、該ペプチドが抗原含有タンパク質に由来しそれによって活性化T細胞が再活性化される、前記段階
を含む、抗原含有タンパク質に対する細胞性の細胞傷害性免疫応答を延長する方法における使用のための医薬であって、
段階(i)のペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞が、T細胞が抗原に対して活性化された後の0時間〜14日の時間枠において投与される、前記医薬。 - ・ 段階(i)のペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞が、前記患者に1回のみ投与され、かつ/または
・ 段階(i)のペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞が、全身に、特に静脈内に、腹腔内に、または髄腔内注射によって投与され、かつ/もしくは、好ましくは緩衝化されている生理学的に許容される水溶液中の懸濁剤として投与され、かつ/または
・ 段階(i)のペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞が樹状細胞ではなく、かつ/または
・ 1×106〜4×108個、好ましくは1×107〜4×108個のペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞が該患者に投与される、
請求項1記載の使用のための医薬。 - 段階(i)のペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞が、T細胞が抗原に対して活性化された後の0時間〜14日の時間枠において前記患者に1回のみ投与され、かつ抗原に対して活性化されたT細胞が記憶状態へ戻った後に、さらに1回または複数回、好ましくは1回のみ投与され、
より好ましくは、T細胞が抗原に対して活性化された後3週間以上後に、1回または複数回、さらにより好ましくは1回のみ投与される、
請求項1または2記載の使用のための医薬。 - 時間枠が、72時間〜12日、好ましくは5日〜12日、より好ましくは5日〜9日である、請求項1または2記載の使用のための医薬。
- (i)MHC-I提示細胞が、血液細胞、特に末梢血単核細胞(PBMC)、例えば単球もしくはリンパ球細胞であり、かつ/または
(ii)抗原含有タンパク質に由来するペプチドが、アミノ酸8、9もしくは10個の長さを有し、かつ/もしくは、MHC-Iによって、好ましくは対立遺伝子HLA-A2、HLA-A1、HLA-A3、HLA-B7、HLA-B35、HLA-A24、もしくはHLA-A30によって、より好ましくは対立遺伝子HLA-A2によって提示されるペプチドであり、かつ/または
(iii)少なくとも1つのペプチドが抗原を含む、
請求項1〜4のいずれか一項記載の使用のための医薬。 - 段階(i)の主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞が前記患者から入手され、好ましくは、該患者から入手された段階(i)の主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞が、2時間〜3日間、より好ましくは2時間〜2日間インビトロで培養される、請求項1〜5のいずれか一項記載の使用のための医薬。
- (a)少なくとも1つの主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞を該ペプチドと共にインビトロでインキュベートすること、または
(b)抗原含有タンパク質もしくはその抗原含有断片、もしくは抗原含有タンパク質もしくはその抗原含有断片をコードする核酸を、少なくとも1つの主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞中へ導入するための物理的、化学的もしくは生物学的な方法を適用して、
核酸の場合、少なくとも1つの細胞が、抗原含有タンパク質もしくはその抗原含有断片を少なくとも1つの主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞中へ発現することを可能にすること、および
少なくとも1つの細胞が、該抗原を処理し、MHC-Iの環境においてペプチドを提示することを可能にすること
によって、段階(i)のペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞が入手される、請求項1〜6のいずれか一項記載の使用のための医薬。 - 前記患者がヒトである、請求項1〜7のいずれか一項記載の使用のための医薬。
- 細胞性の細胞傷害性免疫応答を延長する方法が、腫瘍および/もしくは感染症を予防的に処置するもしくは処置するためのものであり、かつ/または
前記患者が免疫無防備状態もしくは免疫抑制状態である、
請求項1〜8のいずれか一項記載の使用のための医薬。 - 前記T細胞が、
(1)
(a)抗原提示細胞の表面上の受容体に結合する分子、
(b)(a)の分子に結合された前記抗原含有タンパク質
を含む送達システムであって、
該受容体に(a)の分子が結合すると、(b)のタンパク質が内部移行して抗原提示細胞中で処理され、該タンパク質中に含有される抗原が抗原提示細胞の表面上に提示され、それによって患者においてT細胞が活性化される、送達システム
を前記患者に投与すること、および
(2)Th-1媒介応答を支持するさらなるアジュバントを投与すること
によって前記抗原に対して活性化されている、請求項1〜9のいずれか一項記載の使用のための医薬。 - 抗原提示細胞が、樹状細胞、マクロファージ、およびB細胞より選択される、請求項10記載の使用のための医薬。
- 抗原提示細胞が、XCR1+(CD141+)樹状細胞、SIRPα+樹状細胞、単球由来樹状細胞、皮膚樹状細胞、例えばランゲルハンス細胞、および形質細胞様樹状細胞(pDC)より好ましくは選択される樹状細胞(DC)である、請求項11記載の使用のための医薬。
- (i)Th-1媒介応答を支持するアジュバントが、RIG-Iアゴニスト、合成もしくは組換えTLRリガンド、特にTLR8アゴニスト、例えばレシキモド(R848)、もしくはTLR3アゴニスト、例えばポリICLCおよびポリイノシン:ポリシチジル酸(ポリI:C)、モンタニド(Montanide)、例えばISA51、ISA720、サポニン、例えばQuil-A、ISCOM、QS-21、AS02およびAS01、ポリイノシン:ポリシチジル酸(ポリI:C)、リポ多糖(LPS)、ならびにCpGオリゴデオキシヌクレオチドからなる群より選択され、
好ましくは、Th-1媒介応答を支持するアジュバントが、RIG-Iアゴニスト、TLR8アゴニスト、例えばレシキモド(R848)、ならびにTLR3アゴニスト、例えばポリICLCおよびポリイノシン:ポリシチジル酸(ポリI:C)より選択され、
かつ/または
(ii)Th-1媒介応答を支持するアジュバントが、ペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞の投与前に、投与と同時に、もしくは投与後に投与され、

より好ましくは、Th-1媒介応答を支持するアジュバントが、(a)ペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞の投与と同じ日に、もしくは(b)該投与後1日〜3日の時間枠内で投与され、
さらにより好ましくは、Th-1媒介応答を支持するアジュバントが、ペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞の投与と同じ日に投与され、
かつ/または
(iii)Th-1媒介応答を支持するアジュバントが、単一の薬学的組成物としてペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞と共に投与されるか、もしくはペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞から空間的に分離した状態で投与される、
請求項1〜12のいずれか一項記載の使用のための医薬。 - (i)Th-1媒介応答を支持するさらなるアジュバントが、RIG-Iアゴニスト、合成もしくは組換えTLRリガンド、特にTLR8アゴニスト、例えばレシキモド(R848)、もしくはTLR3アゴニスト、例えばポリICLCおよびポリイノシン:ポリシチジル酸(ポリI:C)、モンタニド、例えばISA51、ISA720、サポニン、例えばQuil-A、ISCOM、QS-21、AS02およびAS01、ポリイノシン:ポリシチジル酸(ポリI:C)、リポ多糖(LPS)、ならびにCpGオリゴデオキシヌクレオチドからなる群より選択され、

好ましくは、Th-1媒介応答を支持するさらなるアジュバントが、送達システムの投与前1日〜投与後3日の時間枠内で投与され、
より好ましくは、Th-1媒介応答を支持するさらなるアジュバントが、(a)送達システムの投与と同じ日に、もしくは(b)該投与後1日〜3日の時間枠内で投与される、
請求項10〜13のいずれか一項記載の使用のための医薬。 - 前記方法が、複合体化インターロイキン2(IL-2cx)、複合体化インターロイキン15(IL-15cx)、複合体化インターロイキン4(IL-4cx)、複合体化インターロイキン7(IL-7cx)、またはIL-2ムテインを投与する段階をさらに含み、
好ましくは、投与が、前記ペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞を前記患者に投与した後に行われる、請求項1〜4のいずれか一項記載の使用のための医薬。 - 複合体化インターロイキン2(IL-2cx)、複合体化インターロイキン15(IL-15cx)、複合体化インターロイキン4(IL-4cx)、複合体化インターロイキン7(IL-7cx)、もしくはIL-2ムテインが、繰り返し、特に1、2、3、4、5、6、7、8、9、10、11、12、13もしくは14回投与され、さらにより好ましくは、複合体化インターロイキン2(IL-2cx)、複合体化インターロイキン15(IL-15cx)、複合体化インターロイキン4(IL-4cx)、複合体化インターロイキン7(IL-7cx)、もしくはIL-2ムテインが、1日もしくは2日毎に投与され、かつ/もしくは、5日間〜1ヵ月間、さらにより好ましくは1〜2週間、繰り返し投与され、
かつ/または
複合体化インターロイキン2(IL-2cx)、複合体化インターロイキン15(IL-15cx)、複合体化インターロイキン4(IL-4cx)、複合体化インターロイキン7(IL-7cx)、もしくはIL-2ムテインが、静脈内に、皮下に、腹腔内に、皮内に、もしくは髄腔内に投与されるか、もしくは、好ましくは注射によって、腫瘍中へ投与される、請求項15記載の使用のための医薬。 - 請求項1〜7のいずれか一項記載のペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞、ならびに
(i)複合体化インターロイキン2(IL-2cx)、複合体化インターロイキン15(IL-15cx)、複合体化インターロイキン4(IL-4cx)、複合体化インターロイキン7(IL-7cx)、およびIL-2ムテインより選択される少なくとも1つの作用物質、ならびに/または
(ii)請求項10〜12のいずれか一項記載の送達システム、
を含み、
任意で、Th-1媒介応答を支持する少なくとも1つのアジュバントをさらに含む、パーツのキット。 - (A)
(i)
(1)
(a)抗原提示細胞の表面上の受容体に結合する分子、
(b)(a)の分子に結合された抗原含有タンパク質
を含む送達システムであって、
該受容体に(a)の分子が結合すると、(b)のタンパク質が内部移行して抗原提示細胞中で処理され、該タンパク質中に含有される抗原が抗原提示細胞の表面上に提示され、それによって患者においてT細胞が活性化される、送達システム
を患者に投与する段階、および
(2)Th-1媒介応答を支持するさらなるアジュバントを投与する段階
または
(ii)
(1)該患者から得られた1つもしくは複数のT細胞を、抗原含有タンパク質もしくはその抗原含有断片を用いてインビトロで活性化する段階、
(2)段階(1)において得られた1つもしくは複数のT細胞を該患者に再移入する段階、および
(3)任意で、Th-1媒介応答を支持するさらなるアジュバントを投与する段階
または
(iii)
(1)抗原含有タンパク質もしくはその抗原含有断片、
もしくは
該患者において抗原含有タンパク質もしくはその抗原含有断片を発現することができる、抗原含有タンパク質もしくはその抗原含有断片をコードする核酸配列を含む核酸
を該患者に投与する段階であって、それによって少なくとも1つのT細胞が活性化される、段階、および
(2)Th-1媒介応答を支持するさらなるアジュバントを投与する段階
ならびに
(B)
(a)抗原含有タンパク質に由来しそれによって活性化T細胞が再活性化されるペプチドが結合した主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞およびTh-1媒介応答を支持するアジュバント、ならびに
(b)複合体化インターロイキン2(IL-2cx)、複合体化インターロイキン15(IL-15cx)、複合体化インターロイキン4(IL-4cx)、複合体化インターロイキン7(IL-7cx)、またはIL-2ムテイン
より選択される少なくとも1つの再賦活剤を患者に投与する段階であって、
該再賦活剤が、T細胞が抗原に対して活性化された後の0時間〜14日の時間枠において投与され、好ましくは、該再賦活剤が、T細胞が抗原に対して活性化された後の0時間〜14日の時間枠において1回のみ投与される、前記段階
を含む、細胞性の細胞傷害性免疫応答を誘導する方法における使用のための医薬。 - (B)(a)の再賦活剤が前記患者に1回のみ投与され、かつ/または
該再賦活剤、好ましくは(B)(a)の再賦活剤の再賦活剤が、T細胞が抗原に対して活性化された後の72時間〜12日、好ましくは5時間〜12日、より好ましくは5日〜9日の時間枠において投与される、請求項18記載の使用のための医薬。 - 請求項18の(A)の(i)記載の送達システムが投与され、
好ましくは、抗原提示細胞が、樹状細胞、マクロファージおよびB細胞より選択される、
請求項18記載の使用のための医薬。 - 抗原提示細胞が樹状細胞である、請求項19記載の使用のための医薬。
- 樹状細胞の表面上の受容体が、交差提示樹状細胞の表面上の受容体である、請求項21記載の使用のための医薬。
- 樹状細胞の表面上の受容体が、ケモカイン(Cモチーフ)受容体1(XCR1)、ネクチン様分子2、c型レクチン(CLEC)、例えばCLEC9Aであり、好ましくは、樹状細胞の表面上の受容体がXCR1である、請求項21または22記載の使用のための医薬。
- 送達システムの(a)の抗原提示細胞の表面上の受容体に結合する分子が、該受容体に対するリガンドまたは該受容体に対する抗体もしくは抗体断片であり、特に、該受容体がケモカイン(Cモチーフ)受容体1(XCR1)であり、かつ、(a)の分子が、抗XCR1抗体もしくはその断片またはケモカイン(Cモチーフ)リガンド1(XCL1)もしくはその機能的活性バリアントであり、特に、SEQ ID NO: 7〜10、好ましくはSEQ ID NO: 8〜10、より好ましくはSEQ ID NO: 9または10のいずれか、特にSEQ ID NO: 10の配列を含むかまたは該配列からなる、請求項20〜23のいずれか一項記載の使用のための医薬。
- 抗原含有タンパク質の抗原が、免疫原、病原体由来抗原、または腫瘍抗原である、請求項1〜24のいずれか一項記載の使用のための医薬。
- T細胞が、CD8+ T細胞またはCD4+ T細胞、好ましくはCD8+ T細胞である、請求項1〜25のいずれか一項記載の使用のための医薬。
- (i)抗原に対して活性化されたT細胞を有する患者に、全身投与によってペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞、およびTh-1媒介応答を支持するアジュバントを投与する段階であって、該ペプチドが抗原含有タンパク質に由来しそれによって活性化T細胞を再活性化し、かつ、該ペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞が樹状細胞ではない、前記段階
を含む、抗原含有タンパク質に対する細胞性の細胞傷害性免疫応答を延長および/または増幅する方法における使用のための医薬。 - ・ ペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞が、血液細胞、特に末梢血単核細胞(PBMC)であり、かつ/または
・ 段階(i)のペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞が、T細胞が抗原に対して活性化された後の0時間〜14日の時間枠において投与され、かつ/または
・ 全身投与が、静脈内、腹腔内もしくは髄腔内投与であり、かつ/または
・ 1×106〜4×108個、より好ましくは1×107〜4×108個のペプチド結合主要組織適合遺伝子複合体クラスI(MHC-I)提示細胞が前記患者に投与される、
請求項27記載の使用のための医薬。
