Abstract
Description
Claims (11)
- 心臓の弁を移植するためのシステムであって、
前記システムは、流入端と流出端を持つ、半径方向に自己拡張可能な筒状体を有し、該筒状体は、前記流入端と前記流出端の間の外表面に予め形成された溝を有し、該溝は、前記筒状体の外周に少なくとも部分的に伸びていて、前記筒状体の半径方向の外側に向かって開口しており、
前記システムは、さらに、前記筒状体内に設置されて取り付けられる弁と、
前記溝の周方向に伸びる環を形成し、自然弁の弁尖及び/又は腱索の部分を前記溝内に誘導するように構成された細長い外部部材とを有し、
前記筒状体の前記流出端は、搬送カテーテルから前記流入端でなく前記流出端のみが解放された時、前記溝から前記流出端に向かって半径方向の外側に傾斜する円錐台形状を有するものであるシステム。 - (i)搬送カテーテルから前記流入端でなく前記流出端のみが解放された時の方が、(ii)搬送カテーテルから前記流出端と前記流入端の両方が解放された時よりも、前記溝の開口が大きいものであることを特徴とする請求項1に記載のシステム。
- さらに、前記溝内に移動される捕捉部材を含み、前記捕捉部材は、前記筒状体が完全に展開した時に前記溝の開口を通り抜けないような寸法又は形状を有する少なくとも部分的な環を形成するものであることを特徴とする請求項1に記載のシステム。
- 前記筒状体が、ステントであることを特徴とする請求項1に記載のシステム。
- 前記円錐台形状が、前記筒状体の長手方向の中心軸に対して、約10°〜20°の角度で半径方向の外側に傾斜した形状であることを特徴とする請求項1に記載のシステム。
- 患者の心臓に人工置換弁を移植する方法であって、
第1の搬送カテーテルから細長い外部部材を推進させ、前記細長い外部部材によって、自然弁の弁尖及び/又は腱索の周りに環を形成する工程と、
半径方向に自己拡張可能な筒状体であって、流入端と、流出端と、前記筒状体の管腔内に位置する弁と、前記流入端と前記流出端の間の外表面に予め形成された溝とを有し、該溝は、前記筒状体の半径方向の外側に向かって開口している前記筒状体を、前記筒状体を部分的に展開させる時に、前記筒状体が前記環の内側に位置し、前記流入端でなく前記流出端のみが第2の搬送カテーテルから展開し、かつ前記筒状体が前記溝から前記流出端に向かって半径方向の外側に傾斜する円錐台形状を有するように、前記第2の搬送カテーテルから部分的に展開させる工程と、
前記環を前記溝の開口から前記溝内に移動させる工程とを含む方法。 - 前記環を前記溝内に前記移動させることは、前記自然弁の弁尖及び/又は腱索の周りに形成された前記環を縮め、前記自然弁の弁尖及び/又は腱索の部分を前記溝内に誘導することを含むことを特徴とする請求項6に記載の方法。
- 前記自然弁の弁尖及び/又は腱索の部分を前記溝内に誘導することは、前記筒状体の傾斜に沿って、前記流出端から前記流入端に向かって、半径方向の内向きに前記環を移動させ、前記環を前記溝内にスライドさせることを含むことを特徴とする請求項7に記載の方法。
- さらに、前記溝に合う寸法又は形状を有し、かつ前記筒状体が完全に展開した時、前記溝の開口から通り抜けない大きさを有する捕捉部材を用いて、前記自然弁の弁尖及び/又は腱索の部分を前記溝内に捕捉することを含むことを特徴とする請求項8に記載の方法。
- (i)前記第2の搬送カテーテルから前記流入端でなく前記流出端のみが解放された時の方が、(ii)前記第2の搬送カテーテルから前記流出端と前記流入端の両方が展開された時よりも、前記溝の開口が相対的に大きいものとすることを特徴とする請求項6に記載の方法。
- 前記自然弁の弁尖及び/又は腱索を、僧帽弁又は三尖弁の一部を形成するものとすることを特徴とする請求項6に記載の方法。
