（技術分野）
本開示は、電気機械的外科手術システムおよびその使用方法に関する。より具体的には、本開示は、再プログラム可能および／または再構成可能電気機械的外科手術器具に関する。
線形クランピング、切断、およびステープリング外科手術デバイスは、組織を切除するために、外科手術手技において採用され得る。従来の線形クランピング、切断、およびステープリングデバイスは、ハンドルアセンブリと、ハンドルアセンブリから延びるアダプタアセンブリと、アダプタアセンブリに取り外し可能に結合される外科手術装填ユニットとを含む。外科手術装填ユニットは、組織の周りでクランプ締めする一対の対向する把持ジョー部材を含む。このデバイスでは、アンビル部分等の２つの把持部材の一方または両方が、全体構造に対して移動もしくは枢動する。外科手術デバイスの作動は、ハンドルアセンブリ内に維持されるグリップトリガによって制御され得る。

本開示は、ハンドルアセンブリと、外科手術装填ユニットと、外科手術装填ユニットをハンドルアセンブリと相互接続するアダプタアセンブリとを含む外科手術器具を提供する。外科手術器具は、コントローラによって制御されるモータを含み、外科手術器具は、外科手術器具の動作特性を調整するために、臨床医によって再プログラムおよび／または再構成され得る。
図１Ａ−Ｃを参照すると、本開示のある実施形態による、外科手術器具１０が、動力式ハンドヘルド電気機械的外科手術器具として示される。外科手術器具１０は、いくつかのアダプタアセンブリ２００のうちの任意１つを用いたそこへの選択的取り付けのために構成されるハンドルアセンブリ１００を含み、次に、各独特なアダプタアセンブリ２００は、任意の数の外科手術装填ユニット３００との選択的接続のために構成される。装填ユニット３００およびアダプタアセンブリ２００は、ハンドルアセンブリ１００による作動および操作のために構成される。

図１Ａを参照すると、ハンドルアセンブリ１００は、アダプタアセンブリ２００の対応する駆動結合アセンブリ２１０を受け入れるように構成されるノーズまたは接続部分１１０を画定する。ハンドルアセンブリ１００の接続部分１１０は、アダプタアセンブリ２００がハンドルアセンブリ１００に嵌め合わされると、アダプタアセンブリ２００の駆動結合アセンブリ２１０を受け取る円筒形陥凹（図示せず）を有する。接続部分１１０は、アダプタアセンブリ２００の対応する回転可能コネクタスリーブとインターフェース接続する１つ以上の回転可能駆動コネクタ（図示せず）を格納する。
アダプタアセンブリ２００が、ハンドルアセンブリ１００に嵌め合わされると、ハンドルアセンブリ１００の回転可能駆動コネクタ（図示せず）の各々は、アダプタアセンブリ２００の対応する回転可能コネクタスリーブと結合する。この点においてハンドルアセンブリ１００の複数のコネクタとアダプタアセンブリ２００の複数の対応するコネクタスリーブとの間のインターフェースは、ハンドルアセンブリ１００の駆動コネクタの各々の回転がアダプタアセンブリ２００の対応するコネクタスリーブの回転を生じさせるように鍵止めされる。
ハンドルアセンブリ１００の駆動コネクタとアダプタアセンブリ２００のコネクタスリーブとの嵌め合わせは、回転力が、３つのそれぞれのコネクタインターフェースの各々を介して独立して伝達されることを可能にする。ハンドルアセンブリ１００の駆動コネクタは、ハンドルアセンブリ１００の駆動機構によって独立して回転されるように構成される。
ハンドルアセンブリ１００の駆動コネクタの各々は、アダプタアセンブリ２００のそれぞれのコネクタスリーブとの鍵止めインターフェースおよび／または実質的に非回転可能インターフェースを有するので、アダプタアセンブリ２００がハンドルアセンブリ１００に結合されると、回転力は、ハンドルアセンブリ１００の駆動機構からアダプタアセンブリ２００に選択的に転送される。
ハンドルアセンブリ１００の駆動コネクタの選択的回転は、外科手術器具１０が、選択的に、エンドエフェクタ３０４の異なる機能を作動させることを可能にする。ハンドルアセンブリ１００の第１の駆動コネクタの選択的かつ独立した回転は、エンドエフェクタ３０４の選択的かつ独立した開放および閉鎖ならびにエンドエフェクタ３０４のステープリング／切断構成要素の駆動に対応する。ハンドルアセンブリ１００の第２の駆動コネクタの選択的かつ独立した回転は、縦軸「Ｘ」を横断する関節運動軸の周りでのエンドエフェクタ３０４の選択的かつ独立した関節運動に対応する。特に、エンドエフェクタ３０４は、第２のまたは個別の縦軸を画定し、第２のまたは個別の縦軸が縦軸「Ｘ」と実質的に整列させられる第１の位置から、第２の縦軸が縦軸「Ｘ」に対して非ゼロ角度で配置される少なくとも第２の位置まで移動可能である。加えて、ハンドルアセンブリ１００の第３の駆動コネクタの選択的かつ独立した回転は、外科手術器具１０のハンドルアセンブリ１００に対した縦軸「Ｘ」の周りでの装填ユニット３００の選択的および独立した回転に対応する。
図１Ａ−１Ｃを継続して参照すると、アダプタアセンブリ２００は、ノブ筐体２０２と、ノブ筐体２０２の遠位端から延びる細長い本体２０４とを含む。ノブ筐体２０２および細長い本体２０４は、アダプタアセンブリ２００の構成要素を格納するように構成され、寸法を決定される。細長い本体２０４は、内視鏡下挿入のために寸法を決定され得る。実施形態では、細長い本体２０４は、典型的トロカールポート、カニューレ等を通過可能であり得る。ノブ筐体２０２は、トロカールポート、カニューレ等に進入しないように寸法を決定され得る。細長い本体２０４は、ハンドルアセンブリ１００に取り付けられるように構成されるノブ筐体２０２に取り付けられる近位部分２０６ａを有する。細長い本体２０４はまた、装填ユニット３００の近位部分３０２ａに結合されるように構成される遠位部分２０６ｂを含む。細長い本体２０４はさらに、遠位部分２０６ｂから遠位に延びる、遠位キャップ２０８を含む。細長い本体２０４は、加えて、円筒形外側筐体２１２と、その中に配置される円筒形内側筐体２１４（図２）とを含む。
図１Ａを参照すると、ユーザインターフェース１２０を含むハンドルアセンブリ１００が示される。ユーザインターフェース１２０は、画面１２２と、複数のボタン１２４とを含む。ユーザインターフェース１２０は、器具１０内に配置されるセンサによって報告される情報に基づいて、センサを介してユーザインターフェース１２０に通信され得る「モード」（例えば、回転、関節運動、または作動）、「ステータス」（例えば、関節運動の角度、回転の速度、または作動のタイプ）、およびステープルが発射されたかどうか等の「フィードバック」等、器具１０の種々のタイプの動作パラメータを表示し得る。
画面１２２は、ＬＣＤ画面、ＯＬＥＤ画面、エレクトロルミネセント画面等の任意の好適なディスプレイデバイスであり得る。一実施形態では、画面１２２は、ボタン１２４に取って代わる、および／またはそれを補足するタッチスクリーンであり得る。タッチスクリーンは、抵抗、表面波、容量、赤外線、歪みゲージ、光学、分散信号、または音響パルス認識タッチスクリーン技術を組み込み得る。タッチスクリーンは、ユーザが、動作フィードバックを視認しながら、入力を提供することを可能にするために使用され得る。このアプローチは、密閉画面構成要素の促進を可能にし、器具１０を滅菌し、かつ粒子および／または流体汚染を防止するのに役立ち得る。ある実施形態では、画面は、使用中または準備中の画面を視認することにおける柔軟性のために、器具１０に枢動可能または回転可能に搭載され得る（例えば、ヒンジまたは玉継ぎ手搭載を介して）。
ボタン１２４は、ユーザインターフェースメニューをナビゲートし、種々の設定を選択しながらプロンプトに応答すること等、画面１２２上に表示されるグラフィカルユーザインターフェースから選択肢をナビゲートし、選択するための入力デバイスとして使用され得、ユーザが異なる組織タイプならびにステープルカートリッジの種々のサイズおよび長さを入力することを可能にする。実施形態では、ボタン１２４は、器具１０の移動の開始および／または停止、ならびに枢動方向、速度、および／またはトルクの選択を含め、器具１０を制御するために使用され得る。少なくとも１つのボタン１２４は、種々の設定を書き換える緊急モードを選択するために使用されることができることも想定される。
ボタン１２４は、種々の形状およびサイズのマイクロ電子触覚もしくは非触覚膜、ポリエステル膜、エラストマー、プラスチック、または金属キーから形成され得る。加えて、スイッチは、互いから異なる高さに位置付けられ得、および／または隆起した印もしく他のテクスチャ特徴（例えば、凹面または凸面）を含み、ユーザが、ユーザインターフェース１２０を見る必要なく、適切なスイッチを押すことを可能にし得る。
図２を参照すると、ハンドルアセンブリ１００は、コントローラ４０２と、モータドライバ回路４０４と、電源４０６と、１つ以上のモータ４０８、ギヤセレクタボックス（図示せず）、伝動装置機構（図示せず）等を有する駆動機構とを含む。コントローラ４０２は、プロセッサと、読み取り専用メモリ（ＲＯＭ）、ランダムアクセスメモリ（ＲＡＭ）、電気消去可能プログラマブルＲＯＭ（ＥＥＰＲＯＭ）、不揮発性ＲＡＭ（ＮＶＲＡＭ）、またはフラッシュメモリ等の揮発性、不揮発性、磁気、光学、または電気媒体であり得るメモリとを含み得る。プロセッサは、限定ではないが、ハードウェアプロセッサ、フィールドプログラマブルゲートアレイ（ＦＰＧＡ）、デジタル信号プロセッサ（ＤＳＰ）、中央処理ユニット（ＣＰＵ）、特定用途向け集積回路（ＡＳＩＣ）、または個別の論理回路、マイクロプロセッサ、およびそれらの組み合わせを含む、本開示に説明される動作、計算、および／または一組の命令を行うように適合される任意の好適な論理ユニット（例えば、制御回路）であり得る。
モータドライバ回路４０４は、電源４０６からモータ４０８への電気エネルギーの流動を含む、モータ４０８の動作を制御する。モータドライバ回路４０４は、モータ４０８および電源４０６の動作状態を測定するように構成される複数のセンサ４０４ａ、４０４ｂ、・・・４０４ｎを含む。センサ４０４ａ−ｎは、電圧センサ、電流センサ、温度センサ、テレメトリセンサ、光学センサ、およびそれらの組み合わせを含み得る。センサ４０４ａ−４０４ｎは、電源４０６によって供給される電気エネルギーの電圧、電流、および他の電気特性を測定し得る。センサ４０４ａ−４０４ｎはまた、モータ４０８の毎分回転数（ＲＰＭ）としての回転速度、トルク、温度、電流引き込み、および他の特性を測定し得る。ＲＰＭは、モータ４０８の回転を測定することによって、決定され得る。種々のドライブシャフトの位置は、シャフト内もしくはそれに近接して配置される種々の線形センサを使用することによって決定されるか、またはＲＰＭ測定から推定され得る。実施形態では、トルクは、一定ＲＰＭにおけるモータ４０８の調整された電流引き込みに基づいて計算され得る。
コントローラ４０２は、ドライバ回路４０４とインターフェース接続するための複数の入力および出力を含む。特に、コントローラ４０２は、モータ４０８および電源４０６の動作ステータスに関して、ドライバ回路４０４から測定されるセンサ信号を受信し、次に、センサ読み取り値、特定のアルゴリズム命令、およびユーザ構成に基づいて、モータ４０６の動作を制御するための制御信号をドライバ回路４０４に出力する。
ハンドルアセンブリ１００はまた、コントローラ４０２に結合される、通信インターフェース４１０を含む。通信インターフェース４１０は、ユニバーサルシリアルバス（「ＵＳＢ」）ポート、および／または限定ではないが、Ｂｌｕｅｔｏｏｔｈ（登録商標）、Ｗｉｆｉ（登録商標）、近距離通信、誘導通信、または任意の他の好適な無線通信プロトコルを含む、無線インターフェース等の通信ポートを含み得る。
本開示による、ハンドルアセンブリ１００は、動作パラメータおよび／またはアルゴリズム命令のうちの１つ以上のものの調節、修正、および／または再構成を提供する。実施形態では、再構成データは、構成デバイス５００上に記憶され得る。構成デバイス５００は、データを記憶し、データをコントローラ４０６に通信可能な任意の好適なデバイスであり得る。好適な構成デバイス５００として、限定ではないが、コンピュータ、モバイルコンピューティングデバイス、フラッシュドライブ、メモリカード、ＲＦＩＤ等が挙げられる。構成データは、コントローラ４０６のメモリ内に記憶される、モータ制御アルゴリズムを修正するためのパラメータを含み得る。
構成デバイス５００はまた、パラメータに加え、またはその代わりに、コントローラ４０６によって実行可能なソフトウェア命令を含み得る。さらなる実施形態では、構成データは、コントローラ４０６のメモリ内に記憶され得、コントローラ４０６は、構成デバイス５００から受信された識別子に基づいて所望の構成データを選択する。構成データパラメータはまた、ユーザインターフェース１２０を通して選択され得る。
使用中、構成デバイス５００は、通信インターフェース４１０を通して、ハンドルアセンブリ１００のコントローラ４０６に接続される。構成デバイス５００とハンドルアセンブリ１００との間の接続は、その間の通信インターフェースに依存する。したがって、ＲＦＩＤが構成データを伝送するために使用されている場合、ハンドルアセンブリ１００は、ＲＦＩＤインテロゲータを利用し得る。構成データがコントローラ４０６によって処理されると、コントローラ４０６は、ハンドルアセンブリ１００の設定を修正する。実施形態では、臨床医は、例えば、構成データが複数の構成を含む場合、利用可能な構成データに基づいて、複数の構成のうちから１つを選択し得る。構成データは、クランプ締めおよびクランプ締め解除速度、発射率、後退率、回転、ならびに関節運動速度等、種々のパラメータを含み得る。実施形態では、パラメータは、ボタン１２４および制御アセンブリ１０８のボタンのためのボタンマッピングを含み得る。これは、入力構成が、臨床医に好適であるように修正されることを可能にする。パラメータはまた、外科手術症例ごとに設定され得る。各個人化制御は、行われている外科手術症例および臨床医の個人的選好に応じて、複数の設定を有し得る。
種々の修正が、本開示のアダプタアセンブリの実施形態に行われ得ることを理解されたい。したがって、前述の説明は、限定としてではなく、単に、実施形態の例示として解釈されるべきである。当業者は、本開示の範囲および精神内における他の修正も想起するであろう。

Claims (18)

  1. 外科手術器具であって、
    少なくとも１つのエンドエフェクタを作動させるように構成されているモータと、
    構成デバイスに結合するように構成されている通信インターフェースと、
    前記モータおよび前記通信インターフェースに結合されているコントローラと
    を備え、
    前記コントローラは、前記モータを動作させるための少なくとも１つの動作パラメータを記憶するように構成されているメモリを含み、前記コントローラは、前記構成デバイスから受信される構成データに基づいて、前記少なくとも１つの動作パラメータを修正するように構成されている、外科手術器具。
  2. 前記コントローラに結合されているユーザインターフェースをさらに備えている、請求項１に記載の外科手術器具。
  3. 前記メモリは、前記ユーザインターフェースを構成するための少なくとも１つのインターフェースパラメータを記憶し、前記コントローラは、前記構成デバイスから受信される構成データに基づいて、前記少なくとも１つのユーザインターフェースパラメータを修正するようにさらに構成されている、請求項２に記載の外科手術器具。
  4. 前記構成デバイスは、コンピューティングデバイス、フラッシュドライブ、メモリカード、およびＲＦＩＤから成る群から選択される、請求項１に記載の外科手術器具。
  5. 前記少なくとも１つの動作パラメータは、クランプ締め速度、クランプ締め解除速度、発射率、後退率、回転速度、および関節運動速度から成る群から選択される、請求項１に記載の外科手術器具。
  6. 前記通信インターフェースは、無線送受信機または有線ポートのうちの少なくとも１つである、請求項１に記載の外科手術器具。
  7. 外科手術器具であって、
    外科手術装填ユニットと、
    前記外科手術装填ユニットに結合するように構成されている遠位端を含む細長い本体と、
    前記細長い本体の近位端に結合するように構成されている遠位端を含むハンドルアセンブリと
    を備え、
    前記ハンドルアセンブリは、
    前記外科手術装填ユニットを作動させるように構成されているモータと、
    構成デバイスに結合するように構成されている通信インターフェースと、
    前記モータおよび前記通信インターフェースに結合されているコントローラと
    を含み、
    前記コントローラは、前記モータを動作させるための少なくとも１つの動作パラメータを記憶するように構成されているメモリを含み、前記コントローラは、前記構成デバイスから受信される構成データに基づいて、前記少なくとも１つの動作パラメータを修正するように構成されている、外科手術器具。
  8. 前記ハンドルアセンブリは、前記コントローラに結合されているユーザインターフェースをさらに含む、請求項７に記載の外科手術器具。
  9. 前記ユーザインターフェースは、複数のボタンを含む、請求項８に記載の外科手術器具。
  10. 前記メモリは、前記ユーザインターフェースを構成するための少なくとも１つのインターフェースパラメータを記憶し、前記コントローラは、前記構成デバイスから受信される構成データに基づいて、前記少なくとも１つのユーザインターフェースパラメータを修正するようにさらに構成されている、請求項９に記載の外科手術器具。
  11. 前記コントローラは、前記構成デバイスから受信される構成データに基づいて、前記複数のボタンのボタン割り当てを再マップするようにさらに構成されている、請求項１０に記載の外科手術器具。
  12. 前記構成デバイスは、コンピューティングデバイス、フラッシュドライブ、メモリカード、およびＲＦＩＤから成る群から選択される、請求項７に記載の外科手術器具。
  13. 前記少なくとも１つの動作パラメータは、クランプ締め速度、クランプ締め解除速度、発射率、後退率、回転速度、および関節運動速度から成る群から選択される、請求項７に記載の外科手術器具。
  14. 前記通信インターフェースは、無線送受信機または有線ポートのうちの少なくとも１つである、請求項７に記載の外科手術器具。
  15. 外科手術デバイスを構成する方法であって、
    外科手術デバイスの通信インターフェースを構成デバイスに結合することと、
    構成データを前記構成デバイスから受信することと、
    前記構成データに基づいて、前記外科手術デバイスのメモリ内に記憶された前記外科手術デバイスを動作させるための少なくとも１つの動作パラメータを修正することと
    を含む、方法。
  16. 前記構成データに基づいて、ユーザインターフェースの少なくとも１つのユーザインターフェースパラメータを修正することをさらに含む、請求項１５に記載の方法。
  17. 前記少なくとも１つのユーザインターフェースパラメータを修正することは、前記ユーザインターフェースの複数のボタンのボタン割り当てを再マップすることを含む、請求項１６に記載の方法。
  18. 前記少なくとも１つの動作パラメータは、クランプ締め速度、クランプ締め解除速度、発射率、後退率、回転速度、および関節運動速度から成る群から選択される、請求項１６に記載の方法。
