Abstract
Description

1.深い失望、絶望
2.規則的に午前中に悪化する抑うつ
3.早朝覚醒
4.顕著な精神運動抑制または精神運動興奮
5.著しい食欲不振または体重減少
6.過度のまたは不適切な罪悪感
を特徴とする。
a)メランコリアの特徴を伴う抑うつ障害の患者を特定するステップと、
b)治療的有効量の前記κ−オピオイド受容体活性を伴うオピオイド拮抗薬、および任意選択で治療的有効量のボルチオキセチンを、ステップa)で特定された患者に投与するステップと
を含む。
本明細書全体を通して、「κ−オピオイド受容体活性を伴うオピオイド受容体拮抗薬」、「κ−オピオイド受容体活性を伴うオピオイド拮抗薬」、「κ−活性を伴うオピオイド受容体拮抗薬」および「κ−活性を伴うオピオイド拮抗薬」は互換的に使用される。全てが、以下に限定されるものではないが:ナルメフェン、ナルトレキソン、ナロキソン、ノル−BNI(ノル−ビナルトルフィミン)、ブプレノルフィン、ナルトリベン、ナロルフィン、レバロルファン、LY2456302((S)−3−フルオロ4−(4−((2−(3,5−ジメチルフェニル)ピロリジン−1−イル)メチル)フェノキシ)ベンズアミド)、ダイノルフィン、JDTic((3R)−7−ヒドロキシ−N−{(1S)−1−{[(3R,4R)−4−(3−ヒドロキシフェニル)−3,4−ジメチル−1−ピペリジニル]メチル}−2−メチルプロピル}−1,2,3,4−テトラヒドロ−3−イソキノリン−カルボキサミド)および他のκ−活性を伴うオピオイド拮抗薬から選択されるκ−活性を伴うオピオイド拮抗薬を含むものとする。一実施形態において、本発明のκ−活性を伴うオピオイド拮抗薬は、ナルメフェン、ナルトレキソン、ナロキソン、ノル−BNI(ノル−ビナルトルフィミン)、ブプレノルフィン、ナロルフィンおよびレバロルファンから選択される。特にナルメフェンが挙げられる。

以下に、本発明の実施形態を開示する。第1の実施形態をE1で表し、第2の実施形態をE2で表し、以下同様とする。

齧歯類は、先天的に、甘い餌および液体に関心がある。ストレスの多い条件に曝露されると、甘い餌または溶液への関心が低減し、このことは、報酬に対する感受性が低減することを示している。これは例えば、ストレスが誘導するショ糖溶液摂取量の低減が快感消失(報酬に対する感受性を低減する)の尺度として使用される、ショ糖嗜好試験で評価することができる。この、ストレスが誘導するラットのショ糖摂取量の低減は、快感消失の動物モデルとして確立している(Nielsen,Arnt and Sanchez.Intracranial self−stimulation and sucrose intake differ as hedonic measures following chronic mild stress:interstrain and interindividual differences.Behav.Brain.Res.(2000),107(1−2):21−33)。
動物
雄のウィスターラット(Charles River、独国)を、実験開始1カ月前に実験室に持ち込んだ。後述の場合を除いて、動物を一匹ずつ収容し、餌と水を自由に与え、12時間の明/暗サイクル(08:00に点灯)ならびに一定の温度(22±2℃)および湿度(50±5%)条件を維持した。この試験で使用した手順は全て、86/609/EEC Directiveの規則および原則に従っており、ポーランドのInstitute of Pharmacology,Polish Academy of Sciences,KrakowでLocal Bioethical Committeeの承認を受けた。
3週間の実験室および収容条件に対する順応期間をおいた後、1%ショ糖溶液を消費するように動物を訓練した;訓練は、餌および水が14時間遮断された後、ホームケージの中にショ糖が与えられる1時間のベースライン試験9回で構成した。試験の最後に、ショ糖摂取量を、予め秤量しておいたショ糖溶液を含有するびんを秤量することによって測定した。引き続き、同一条件で、全実験期間を通して週1回の間隔でショ糖消費量をモニターした。
初期2週間のストレスの後のショ糖摂取量を基に、ストレス負荷および対照の両群を、それぞれさらに対応亜群(n=8)に分け、次の5週間、ビヒクル(0.9%生理食塩水、1ml/kg)、ボルチオキセチン(10mg/kg)、ナルメフェン(1mg/kg)、ボルチオキセチン(10mg/kg)とナルメフェン(1mg/kg)との共注射または参照処置としてイミプラミンHCl(10mg/kg)を毎日IP投与した。全ての注射の体積は1ml/kgとした。薬物を大体10:00に投与し、毎週のショ糖試験を直近の薬物注射から24時間後に行った。5週間後、全ての処置を停止し、中止から1週間後にさらに1回ショ糖試験を行った。処置および中止の期間全体にわたってストレスを継続した。
この試験で得られた結果は全て、3つの対象間要因(ストレス/対照、薬物処置および連続的ショ糖試験)での分散の複合分析によって解析した(Statistica for Windows v.6.0)。フィッシャーのLSD検定を使用して平均の事後比較を行った。
慢性軽度ストレスは、1%ショ糖溶液消費量を漸減させた。最終ベースライン試験では、全ての動物がおよそ13gのショ糖溶液を飲用した。初期2週間のストレスの後で、対照では摂取量が同一レベルのままであったが、ストレス負荷動物ではおよそ7gに落ち、有意な群間影響が見られた[F(1,76)=113.835;p<0.001]。このような対照とストレス負荷とのビヒクル処置動物間の差は、実験の残りの期間も同一レベルで持続した(図1および2を参照)。

この試験の結果は、CMS手順によって1%ショ糖溶液消費量が有意に減少し、この減少分をイミプラミンでの5週間の処置によって十分に回復することができることを示す、発明者らの以前のデータと合致している。イミプラミンの作用発現は、発明者らのこの薬物での他のCMS試験で観察された作用発現と類似していた。
κ−活性を伴うオピオイド拮抗薬およびボルチオキセチンの快感消失に対する処置効果を特徴づけるための他の非臨床モデルは、例えば、実施例2〜3に概要を述べるように、齧歯類における、天然の強化子に対する比率累進応答、および/または脳内自己刺激(ICSS)試験である。これらのモデルの各々において、κ−活性を伴うオピオイド拮抗薬は、適切な量の適切なビヒクルに溶解して投与することができ、例えば、適切な用量の経口、皮下または腹腔内投与で動物に投与することができる。ナルメフェンは、例えば適切な量の適切なビヒクル(例えば生理食塩水または水)に溶解したナルメフェン塩酸塩の形態で投与することができ、例えば、適切な用量の経口、皮下または腹腔内投与で動物に投与することができる。ボルチオキセチンは、例えば、適切な量の適切なビヒクル(例えば生理食塩水、水または10%の2−ヒドロキシプロピル−β−シクロデキストリン)に溶解したボルチオキセチン臭化水素酸塩、ボルチオキセチン乳酸塩または他の塩の形態で投与することができ、例えば、適切な用量の経口、皮下または腹腔内投与で動物に投与することができる。さらに、これらの化合物は動物の餌とともに特殊な製剤で与えることができる。

Claims (15)
- メランコリアの特徴を伴う抑うつ障害の処置に使用するためのκ−活性を伴うオピオイド拮抗薬。
- メランコリアの特徴を伴う抑うつ障害の処置に使用するためのκ−活性を伴うオピオイド拮抗薬であって、前記使用が次のステップ;
a)メランコリアの特徴を伴う抑うつ障害の患者を特定するステップと、
b)治療的有効量のκ−活性を伴う前記オピオイド拮抗薬を、ステップa)で特定された患者に投与するステップと
を含む、メランコリアの特徴を伴う抑うつ障害の処置に使用するためのκ−活性を伴うオピオイド拮抗薬。 - κ−活性を伴う前記オピオイド拮抗薬が、メランコリアの特徴を伴う抑うつ障害の処置において独立型処置として使用される、請求項1または2に記載のκ−活性を伴うオピオイド拮抗薬。
- κ−活性を伴う前記オピオイド拮抗薬が、メランコリアの特徴を伴う抑うつ障害の処置においてボルチオキセチンと併用される、請求項1に記載のκ−活性を伴うオピオイド拮抗薬。
- κ−活性を伴う前記オピオイド拮抗薬が、メランコリアの特徴を伴う抑うつ障害の処置においてボルチオキセチンと併用され;ステップb)が、治療的有効量のボルチオキセチンを、ステップa)で特定された患者に投与するステップをさらに含む、請求項2に記載のκ−活性を伴うオピオイド拮抗薬。
- κ−活性を伴う前記オピオイド拮抗薬が、ナルメフェン、ナロキソン、ナルトレキソン、ノル−BNI、ブプレノルフィン、ナロルフィンおよびレバロルファンから選択される、請求項1〜5のいずれか一項に記載のκ−活性を伴うオピオイド拮抗薬。

- ボルチオキセチンの用量が1〜30mgの間である、請求項4〜8のいずれか一項に記載のボルチオキセチンと併用されるκ−活性を伴うオピオイド拮抗薬。
- κ−活性を伴う前記オピオイド拮抗薬およびボルチオキセチンが個別の単位剤形に含有される、請求項4〜9のいずれか一項に記載のボルチオキセチンと併用されるκ−活性を伴うオピオイド拮抗薬。
- κ−活性を伴う前記オピオイド拮抗薬およびボルチオキセチンが同一の単位剤形に含有される、請求項4〜9のいずれか一項に記載のボルチオキセチンと併用されるκ−活性を伴うオピオイド拮抗薬。
- 前記処置が、メランコリアの特徴を伴う抑うつ障害の処置とアルコール依存症の処置との両方を含む、請求項1〜11のいずれか一項に記載のκ−活性を伴うオピオイド拮抗薬。
- κ−活性を伴うオピオイド拮抗薬およびボルチオキセチンを、薬学的に許容される添加剤と一緒に含む医薬組成物。
- κ−活性を伴う前記オピオイド拮抗薬がナルメフェンである、請求項13に記載の医薬組成物。
- 1〜30mgのナルメフェンおよび1〜30mgのボルチオキセチンを含む、請求項14に記載の医薬組成物。
