本発明に係るアポモルヒネ固溶体/固体分散体を以下の手順で調製した。
この実施例では、コポビドン(Kollidone VA64)を用いないことを除いては、実施例1に記載の手順を繰り返した。この実施例の組成物は本発明に係るものではない。
実施例1の固溶体/固体分散体粉末製剤の溶出度を、模倣唾液(pH6.2±1.0)中で試験し、実施例2で調製した粉末製剤の模倣唾液(pH6.2±1.0)中での溶出度と比較した。
薬物動態試験を、ウサギを用い、各群n=6で行った。市販のAPOKYN(登録商標)組成物の皮下注射である対照製剤(1回0.5mg/匹)の投与又は本口腔投与を当該動物に(麻酔下で)行った。当該動物にはケタミン/ジラジン(zylazine)(及び/又は、必要であれば、イソフルレン/O2)で軽度麻酔を施した。薬物投与後およそ30秒まで、投薬の間、ウサギの頭を仰向けに固定した状態で優しく拘束した。粉末製剤は、動物の口腔の舌下領域に振り掛けた。実施例1で調製した粉末製剤は、1回1.0mg/匹で与えた。特定の時間点で、LC/MS/MSを用いた血漿中濃度分析のために血液を採取した。図2はアポモルヒネ血漿中濃度プロファイルを示す。実施例1で調製した粉末製剤の口腔粘膜投与の生物学的利用能は、APOKYN(登録商標)の皮下投与に対して89%である。
この実施例では、製造バッチサイズをより大きくし(実施例1のおよそ30倍大きい規模)、凍結乾燥前の溶液を凍結乾燥用の瓶ではなくステンレス製トレーに入れた点を除いては、実施例1に記載の手順を繰り返した。
(i)凍結:−55℃
(ii)濃縮器:−70℃
(iii)真空:100mTorr
(iv)追加の時間:180分
実施例5で調製した前凍結乾燥溶液をブリスター包装ベースシートに予め形成されているブリスターポケットに収めることにより、急速経口崩壊性/溶解性錠剤を調製することができる。充填したブリスター包装ベースシートは、凍結乾燥機に掛け、実施例5に記載の手順に従い凍結乾燥することができる。
(i)凍結:−60℃
(ii)濃縮器:−70℃
(iii)真空:200mTorr
(iv)追加の時間:60分
凍結乾燥前の前凍結乾燥溶液の固形分が錠剤の用量強度を調節するために変更できる点を除いては、実施例6の手順に従って急速経口崩壊性/溶解性錠剤を調製できる。例えば、3.75%、5%、7.5%、10%、又はこれ以上の固形分を有する前凍結乾燥溶液を、本発明に係る経口崩壊性/溶解性錠剤を調製するために用いることができる。用量強度の調節に加えて、前凍結乾燥溶液の固形分は、望ましい錠剤密度を得るため(即ち、固形分が低ければ錠剤密度が低くなるよう)に、又は、望ましい錠剤の崩壊速度を得るために調節することができる。
前凍結乾燥溶液がさらにTween80などの界面活性剤を含む点を除いては、実施例6の手順に従って急速経口崩壊性/溶解性錠剤を調製できる。
NaOH:アポモルヒネのモル比が以下の通りである点を除いては、実施例6の手順に従って急速経口崩壊性/溶解性錠剤を調製できる。

コポビドンの代わりに以下のポリマーを用いる点を除いては、実施例6の手順に従って急速経口崩壊性/溶解性錠剤を調製できる。
・ポリビニルアルコール/ポリエチレングリコールコポリマー(KOLLICOAT(登録商標)IR)
・ヒドロキシプロピルメチルセルロース(METHOCEL(登録商標)F50プレミアム、METHOCEL(登録商標)E3プレミアムLV、METHOCEL(登録商標)E5プレミアムLV、METHOCEL(登録商標)E6プレミアムLV、METHOCEL(登録商標)E15プレミアムLV、及びMETHOCEL(登録商標)E50プレミアムLV)
・ヒドロキシプロピルセルロース(KLUCEL(登録商標)JF、KLUCEL(登録商標)LV、KLUCEL(登録商標)EF)
以下の組成で、実施例5及び6の手順に従って急速経口崩壊性/溶解性錠剤を調製した。
アルミニウム製ブリスターカードをポリ塩化ビニル(PVC)製ブリスターカードで置き換えた点を除いては、実施例12の手順に従って急速経口崩壊性/溶解性錠剤を調製した。凍結乾燥後、PVC製ブリスターカードを、アルミニウム製の蓋で封止し、Uline社から購入した、0.5g、1.0g(2×0.5)、又は2.43gのDRI−CRAD(商標)乾燥剤カードと共にアルミニウムポウチ内に収めた。その後、アルミニウムポウチを加熱封止した。加熱封止したポウチのサンプルを、25℃、相対湿度60%及び40℃、相対湿度75%で保存した。保存試験の結果は以下の通りである。
A:水/トリフルオロ酢酸(TFA)(100:0.1、v/v)及び
B:アセトニトリル:TFA(100:0.1、v/v)であり、
以下のように用いた。
実施例1に記載の組成物のアポモルヒネの溶解度を、米国薬局方等級のアポモルヒネ塩酸塩の溶解度と、様々なpHで、比較した。試験手順は以下のとおりである。
実施例1に記載の組成物の溶出度を、米国薬局方等級のアポモルヒネ塩酸塩の溶出度と、比較した。 溶出プロファイルを得るために以下の手順を採用した。
実施例1に記載の組成物を、プロトンNMR装置で試験した(JOEL ECX−400NMR。サンプルは、重水素化メタノール(DC3OD)に溶解させ、周囲温度とし、スペクトロメーターは400MHzで作動させた。得られたFIDをPCに転送し、NUTS NRM Inc.を用いて処理した。1Hの化学シフトはTMS、0ppmを基準とした。)。実施例1の組成物から得られた1HppmのNMRシフトを、既知量のアポモルヒネ遊離塩基及びアポモルヒネ塩酸塩を含む既知混合物から得られたシフトと比較した。実施例1から得られたデータと既知サンプル混合物から生成された較正曲線との比較に基いて、実施例1の組成物中のアポモルヒネの約70%が非プロトン化型であることが推定された。口腔粘膜への浸透性が高いため、非プロトン化型は患者の口腔への送達に望ましい。
実施例12で調製した錠剤及び実施例1で調製した粉末からX線粉末回折(XRPD)図形を得た。また、アポモルヒネ塩酸塩のサンプル、及び、実施例12の錠剤を調製するために用いた成分の物理的混合物であり、固溶体を形成していないサンプルからXRPD図形を得た。実施例12の錠剤のXRPD図形を図3aに示す。実施例1で調製した粉末のXRPD図形を図3bに示す。アポモルヒネ塩酸塩のXRPD図形を図3cに、実施例12の錠剤を調製するために用いた成分の物理的混合物のXRPD図形を図3dに示した。
実施例1の粉末製剤を、単一拠点、オープンラベル、無作為化、単回投与、無作為化、四処理、交差試験で、18から45歳の健康な人間の志願対象に投与した。20人の対象が参加した。各対象に、絶食条件で、以下の処理の単回投与を行った。
薬物/ポリマーの固溶体/固体分散体を含む薬物の口腔粘膜送達のための固体剤形であって、前記薬物/ポリマーの固溶体/固体分散体は、50重量%以下の薬物及び50重量%以上の水溶性ポリマーを含み、前記薬物/ポリマーの固溶体/固体分散体は、6.2±1.0のpHで37±0.5℃の温度の模倣唾液内に置かれたとき、5分以内に溶けることを特徴とする固体剤形。
前記薬物は、遊離塩基型、塩型、又はこれらの混合物である、付記1に記載の固体剤形。
前記薬物は、少なくとも50%がアモルファス型である、付記1に記載の固体剤形。
抗酸化剤をさらに含む、付記1に記載の固体剤形。
医薬上許容可能な酸、医薬上許容可能な塩基、緩衝剤、又はこれらの混合物をさらに含む、付記1に記載の固体剤形。
前記薬物/ポリマーの固溶体/固体分散体は、6.2±1.0のpHで37±0.5℃の温度の模倣唾液内に置かれたとき、2.5分以内に溶ける、付記1に記載の固体剤形。
前記薬物/ポリマーの固溶体/固体分散体は、6.2±1.0のpHで37±0.5℃の温度の模倣唾液内に置かれたとき、1分以内に溶ける、付記1に記載の固体剤形。
前記薬物は、アポモルヒネ又はその医薬上許容可能な塩である、付記1に記載の固体剤形。
約25℃で約60%の相対湿度で少なくとも4ヶ月間保存されたとき、及び、約40℃で約75%の相対湿度で少なくとも2ヶ月保存されたとき、どの個別分解産物量も約2.0%以下である、付記1に記載の固体剤形。
どの個別分解産物量も約1.5%以下である、付記9に記載の固体剤形。
総分解産物量は約2.5%以下である、付記9に記載の固体剤形。
総分解産物量は約2.0%以下である、付記9に記載の固体剤形。
前記固体剤形は、患者の頬部、舌下部、又は歯肉部への投与のための錠剤形状であり、充填剤、結合剤、崩壊剤、甘味料、矯味矯臭剤、pH調整剤、可溶化剤、滑剤、流動促進剤、及びこれらの混合物からなる群より選択される少なくとも1つの添加剤をさらに含む、付記1に記載の固体剤形。
患者の口腔にアポモルヒネ/ポリマーの固溶体/固体分散体を含む急速崩壊性固体剤形を投与することを含み、前記アポモルヒネ/ポリマーの固溶体/固体分散体は、50重量%以下のアポモルヒネ、水溶性ポリマー、及び抗酸化剤を含み、前記アポモルヒネ/ポリマーの固溶体/固体分散体は、6.2±1.0のpHで37±0.5℃の温度の模倣唾液内に置かれたとき、5分以内に溶け、急速崩壊性及び溶解性の前記固体剤形は有効量の吸収を可能とすることを特徴とするパーキンソン病の治療法。
アポモルヒネ/ポリマーの固溶体を含むアポモルヒネの口腔粘膜送達のための固体剤形であって、前記アポモルヒネ/ポリマーの固溶体は、50重量%以下のアポモルヒネ及び50重量%以上の水溶性ポリマーを含み、前記アポモルヒネ/ポリマーの固溶体は、6.2±1.0のpHで37±0.5℃の温度の模倣唾液内に置かれたとき、5分以内に溶け、前記アポモルヒネは、前記アポモルヒネ/ポリマーの固溶体中に、少なくとも50%が非プロトン型であるプロトン型と非プロトン型のアポモルヒネの混合物として存在し、前記アポモルヒネの少なくとも50%は、前記アポモルヒネ/ポリマーの固溶体中に、アモルファス型で存在することを特徴とする固体剤形。
抗酸化剤をさらに含む、付記15に記載の固体剤形。
可溶化剤をさらに含む、付記15に記載の固体剤形。
矯味矯臭剤をさらに含む、付記15に記載の固体剤形。
約25℃で保存された個別アポモルヒネ分解産物量は約2.0%以下である、付記15に記載の固体剤形。
3ヶ月間、相対湿度が約60%である、付記19に記載の固体剤形。
約40℃で保存され、2週間、相対湿度が約75%である、付記19に記載の固体剤形。
総アポモルヒネ分解産物量は約2.5%以下である、付記19に記載の固体剤形。
Claims (22)
- 薬物/ポリマーの固溶体/固体分散体を含む薬物の口腔粘膜送達のための固体剤形であって、前記薬物/ポリマーの固溶体/固体分散体は、50重量%以下の薬物及び50重量%以上の水溶性ポリマーを含み、前記薬物/ポリマーの固溶体/固体分散体は、6.2±1.0のpHで37±0.5℃の温度の模倣唾液内に置かれたとき、5分以内に溶けることを特徴とする固体剤形。
- 前記薬物は、遊離塩基型、塩型、又はこれらの混合物である、請求項1に記載の固体剤形。
- 前記薬物は、少なくとも50%がアモルファス型である、請求項1に記載の固体剤形。
- 抗酸化剤をさらに含む、請求項1に記載の固体剤形。
- 医薬上許容可能な酸、医薬上許容可能な塩基、緩衝剤、又はこれらの混合物をさらに含む、請求項1に記載の固体剤形。
- 前記薬物/ポリマーの固溶体/固体分散体は、6.2±1.0のpHで37±0.5℃の温度の模倣唾液内に置かれたとき、2.5分以内に溶ける、請求項1に記載の固体剤形。
- 前記薬物/ポリマーの固溶体/固体分散体は、6.2±1.0のpHで37±0.5℃の温度の模倣唾液内に置かれたとき、1分以内に溶ける、請求項1に記載の固体剤形。
- 前記薬物は、アポモルヒネ又はその医薬上許容可能な塩である、請求項1に記載の固体剤形。
- 約25℃で約60%の相対湿度で少なくとも4ヶ月間保存されたとき、及び、約40℃で約75%の相対湿度で少なくとも2ヶ月保存されたとき、どの個別分解産物量も約2.0%以下である、請求項1に記載の固体剤形。
- どの個別分解産物量も約1.5%以下である、請求項9に記載の固体剤形。
- 総分解産物量は約2.5%以下である、請求項9に記載の固体剤形。

- 前記固体剤形は、患者の頬部、舌下部、又は歯肉部への投与のための錠剤形状であり、充填剤、結合剤、崩壊剤、甘味料、矯味矯臭剤、pH調整剤、可溶化剤、滑剤、流動促進剤、及びこれらの混合物からなる群より選択される少なくとも1つの添加剤をさらに含む、請求項1に記載の固体剤形。
- 患者の口腔にアポモルヒネ/ポリマーの固溶体/固体分散体を含む急速崩壊性固体剤形を投与することを含み、前記アポモルヒネ/ポリマーの固溶体/固体分散体は、50重量%以下のアポモルヒネ、水溶性ポリマー、及び抗酸化剤を含み、前記アポモルヒネ/ポリマーの固溶体/固体分散体は、6.2±1.0のpHで37±0.5℃の温度の模倣唾液内に置かれたとき、5分以内に溶け、急速崩壊性及び溶解性の前記固体剤形は有効量の吸収を可能とすることを特徴とするパーキンソン病の治療法。
- アポモルヒネ/ポリマーの固溶体を含むアポモルヒネの口腔粘膜送達のための固体剤形であって、前記アポモルヒネ/ポリマーの固溶体は、50重量%以下のアポモルヒネ及び50重量%以上の水溶性ポリマーを含み、前記アポモルヒネ/ポリマーの固溶体は、6.2±1.0のpHで37±0.5℃の温度の模倣唾液内に置かれたとき、5分以内に溶け、前記アポモルヒネは、前記アポモルヒネ/ポリマーの固溶体中に、少なくとも50%が非プロトン型であるプロトン型と非プロトン型のアポモルヒネの混合物として存在し、前記アポモルヒネの少なくとも50%は、前記アポモルヒネ/ポリマーの固溶体中に、アモルファス型で存在することを特徴とする固体剤形。
- 抗酸化剤をさらに含む、請求項15に記載の固体剤形。
- 可溶化剤をさらに含む、請求項15に記載の固体剤形。
- 矯味矯臭剤をさらに含む、請求項15に記載の固体剤形。
