神経保護剤は、結果としてCNS損傷をもたらす合併症を防止および処置することを目的とする。神経保護能を伴う化合物または薬剤は、脳卒中および神経系の傷害などの急性障害のほか、神経変性障害などの慢性疾患においても適用される。これらの状態では、神経組織に対する損傷の根底にある機序のうちの多くが類似しており、神経保護化合物または神経保護剤であれば、複数の障害において用いうるであろう。疾患には、脳血管障害、外傷性脳損傷、脊髄損傷、アルツハイマー病、パーキンソン病、ハンチントン病、筋萎縮性側索硬化症(ALS)、多発性硬化症、てんかん、運動ニューロン疾患(複数可)、および虚血性神経障害が含まれる。他の適用には、麻酔時および手術時における神経保護が含まれる。フリーラジカルスカベンジャー、抗興奮毒性剤、アポトーシス(プログラム細胞死)阻害剤、抗炎症剤、神経栄養因子、金属イオンキレート化剤、イオンチャネル調節剤、および遺伝子治療のカテゴリーに由来する生成物を含め、多くの生成物が神経保護効果について探索されている。NMDAアンタゴニストおよび神経伝達物質の放出を低減する薬剤を含め、複数の神経保護候補の生成物が、臨床試験に到達しているが、有意性を示すことに失敗している(Clark WMら(2000年)、Stroke、31巻(6号):1234〜9頁;Sarco RIら(2001年)、JAMA、285巻(13号):1719〜28頁;Albers, GWら(2001年)、JAMA、286巻(21号):2673〜82頁)。神経保護剤についての1つの難題は、薬剤の効果が、正常細胞に対してそれほどの活性を伴わずに、これにより、望ましくない副作用、なお潜在的には有害な副作用を伴わずに、傷害されまたは損傷した/障害されたニューロンを指向するような特異性およびターゲティングである。ここで、本発明は、血液脳関門を越え、傷害、障害、または損傷の領域を標的とし、正常ニューロンに対する望ましくない作用を最小限として保護をもたらすことが可能な、特異的で忍容可能な薬剤(複数可)を提供する。
自己反応性ヒトmAbは、ヒト天然自己抗体(NatAb)に分類される(Cohen
I.R.(2007年)、J Autoimmun、29巻:246〜249頁)。NatAbは、本発明者らのヒト免疫グロブリンレパートリーの一部であり(Cohen、I.R.およびM. Schwartz(1999年)、Journal of neuroimmunology、100巻:111〜114頁)、通常は体細胞変異なしに、ヒト固有の遺伝子から天然で作製され、細胞過程を刺激するように機能する場合もあり、細胞残屑を除去するように機能する場合もある(Lutz, HU(2007年)、J Autoimmun、29巻:287〜294頁)。NatAbが、自己抗原に反応するのに対し、従来の抗体は、外因性抗原に反応する。NatAbは、比較的低アフィニティーであり、通常は多重反応性であり、しばしばIgMである。これらのヒトIgMの作用機構は、同様であり、膜のマイクロドメインを介して作用すると考えられる。NatAbは、定義により多重反応性であり、したがって、in vivoにおける特異的機能について関与性の抗原を同定することは、難題でありうる。理論に束縛されずに述べると、現在のところ、これらのIgMは、細胞表面における膜のマイクロドメインの架橋形成分子に結合することが理解されている。通常文脈にない分子は、まとまってシグナル伝達複合体をなす(Rodriguez, M.、A. 本明細書で提示される実施例および研究は、本明細書で提示され、記載され、想定される抗体の有効性および適用を、脳卒中、虚血、および脳またはニューロン細胞に対する低酸素の例のモデルにおいて裏付ける。したがって、特定の態様では、本発明は、脳卒中、脳虚血、脳卒中が疑われるかもしくは推測される状態、一過性脳虚血発作(TIA)もしくは軽度脳卒中に関する介入、または脳卒中もしくは脳虚血性イベントもしくは脳における低酸素性イベントの危険性が高い個体における抗体の使用に関する。
本発明の抗体または断片は通常、本発明の抗体(複数可)または断片に加えて少なくとも1つの成分を含みうる、医薬組成物の形態で投与する。したがって、本発明による医薬組成物および本発明に従って用いられる医薬組成物は、有効成分に加えて、当業者には周知である、薬学的に許容される賦形剤、キャリア、緩衝剤、安定化剤、または他の物質も含みうる。このような物質は、非毒性であるべきであり、有効成分の有効性に干渉するべきではない。キャリアまたは他の物質の正確な性質は、経口経路の場合もあり、注射、例えば、静脈内注射を介する場合もあり、腫瘍部位における沈積を介する場合もある、投与経路に依存する。
本発明はまた、神経細胞の損傷、傷害または障害を検出または決定する方法を含めた多様な診断的適用にも関する。本発明の抗体および断片を用いて、in vitroまたはin vivoにおけるニューロンを評価し、定量化し、標的化し、かつ/または画像化することができる。それらの断片を含めた本抗体、ならびに特異的結合メンバー、抗体および/またはそれらのサブユニットの生成または活性を調節する薬物は、特定の診断適用を保有することが可能であり、例えば、ニューロンの障害、変性、傷害、損傷、または死滅を伴う状態または疾患を検出し、かつ/または測定する目的に用いることができる。それらの断片を含めた本抗体は、ある種の診断的適用を保有し、例えば、外傷性脳傷害を含めた脳卒中または脳傷害を含めた脳虚血を伴う状態または疾患を検出および/または測定する目的で用いることができる。放射性標識した抗体およびそれらの断片を含め、標識した抗体およびそれらの断片は、in vitroの診断法およびin vivoの放射性画像化法、ならびに放射性免疫治療において有用である。in vivoにおける画像化の例では、本発明の抗体または断片を、例として挙げると、抗体分子にキレート化基を介して多数の常磁性イオンをロードした磁気共鳴画像増強剤が含まれるがこれらに限定されない、放射性同位体(複数可)以外の造影剤にコンジュゲートすることができる。キレート化基の例には、EDTA、ポルフィリン、ポリアミン、クラウンエーテル、およびポリオキシムが含まれる。常磁性イオンの例には、ガドリニウム、鉄、マンガン、レニウム、ユーロピウム、ランタニウム、ホルミウム、およびフェルビウムが含まれる。本発明のさらなる態様では、放射性標識した抗体およびそれらの断片、特に、ラジオイムノコンジュゲートが、特に、細胞療法のための放射性標識した抗体として、放射性免疫治療において有用である。なおさらなる態様では、放射性標識した特異的結合メンバー、特に、抗体およびそれらの断片が、放射免疫ガイド下手術法において有用であり、この場合、これらにより、障害されもしくは損傷したニューロンもしくは神経傷害部位の存在および/または位置を、手術中、または手術後に同定および指示して、このような細胞を標的とするかまたは除去することもでき、これらの特定の部位に細胞を移植または投与することもできる。

自己神経系細胞に結合する天然自己抗体(NatAb)の同定
ニューロンに結合する天然血清のヒトIgMを、モノクローナルIgGまたはモノクローナルIgMの高スパイク(血中に10mg/mlを超える)を伴う候補血清について、45年間にわたり収集された140,000を超える試料を含有するMayo Clinic血清バンクをスクリーニングし、次いで、血清を抗体の生存大脳皮質および生存小脳のスライスへの結合について調べることにより同定した(31)。次いで、このようなIgMを陽性試料から精製し、1)単離された初代ニューロンの表面への結合、2)神経突起伸長を支持するための基板として、3)ストレッサー分子に対するニューロンをアポトーシスから保護する能力についてさらに調べた。このスクリーニングプロトコールは、希突起膠細胞に結合し、MSモデル(22、およびWO0185797において記載されている)における再ミエリン化を促進するヒトIgMを同定するのに使用されるプロトコールに基づく。適切な組織または細胞の表面の認識は、治療用IgMの特徴を規定するのに重要であると考えられる。
vitroにおいて示されている(23)。
高濃度のIgGまたはIgM(>10mg/ml)を伴う血清試料を、抗体の、生存CNS組織のスライス(皮質および小脳)におけるニューロン層への結合についてスクリーニングした。152例の被験血清のうち、17例のヒト血清が、組織スライスにおいて陽性であった(23)。
ヒトIgMは皮質ニューロンを過酸化物誘導性死滅から保護する
再ミエリン化促進ヒトIgMは、培養物中の希突起膠細胞を、活性アポトーシスのマーカーであるカスパーゼ3の過酸化物誘導性活性化から保護することが示されている(33)。本明細書で示される通り、sHIgM12またはsHIgM42も、類似のプロトコールにより評価した。sHIgM12またはsHIgM42のそれぞれ、および過酸化物を、マウス初代皮質ニューロンの培養物へと併せて添加し、24時間後にカスパーゼ3の活性化度をアッセイした(図3)。
sHIgM12に由来する組換え抗体
sHIgM12の2つの組換え形態を構築した。各形態では、重鎖および軽鎖についての組換えIgM22抗体(rHIgM22)について既に用いた発現ベクターと同じ発現ベクターを用いた(22、28、WO0185797)。ベクターは、SV40プロモーターの制御下で発現させた選択用dHfR遺伝子を包含する。マウスJ鎖を伴う部分ヒト組換えIgM12抗体の形態を、まず以下の通りに構築し、その後、ヒト/マウスハイブリドーマ系であるF3B6細胞により抗体を生成させた。
rHIgM12の単回末梢投与はMSのTMEVモデルにおける神経機能を改善した
rHIgM12により、培養物中の初代ニューロンが保護され、多発性硬化症(MS)のTMEVモデルにおける身体障害が軸索喪失と相関する(2)という事実に照らして、TMEV感染マウスのrHIgM12による処置を用いて、神経欠損の進行を緩徐化する能力を評価した。
脊髄疾患を伴うマウスの脳幹におけるNAAレベル:脊髄全体における軸索保存の非侵襲的サロゲートマーカー
NAAとは、ニューロン機能と関連する代謝物質である(36、37)。NAAは、脳において2番目に豊富なアミノ酸であり、ほぼもっぱらニューロンに限定される。脳幹におけるNAAレベルの保存は、本発明者らのグループによりTMEVマウスモデルを用いて検証された脊髄軸索全体の健康の尺度である(8)。脊髄下部の軸索が損傷すると、脳幹の細胞が死滅し、NAAが低減される。MRSを介して測定されるNAAレベルは、主にニューロン密度を反映する。NAAは、他の神経細胞でも発現するが、NAAの主な発現は、ニューロンにおいてである。精製されたCNS細胞のMRSプロファイルを研究すると、NAAシグナルの振幅がニューロンにおいて優勢であるのに対し、希突起膠細胞または星状細胞のNAAシグナルの振幅は、ニューロンにおけるシグナルの、それぞれ、5%および10%であったことが示される(38)。
ヒトニューロン結合IgMがTMEV感染マウスにおけるNAAレベルまたは軸索カウントを変化させる能力を評価した。TMEV感染マウスを、脊髄軸索の脱落の開始時(感染の90日後)に、ニューロン結合IgMであるsHIgM12またはsHIgM42 100μgずつの単回投与で処置したところ、10週間後に測定したときの胸部脊髄の正常白質における有髄軸索密度がより高いことが見出された。
ニューロン結合IgMは再ミエリン化を促進することなしに軸索を保存する
希突起膠細胞に結合し、再ミエリン化を促進する複数のヒトIgM(例えば、sHIgM22およびsHIgM46)が同定されている(22、40、41)。下記で示される通り、ニューロン結合IgMは、ニューロン数を増大させ、ニューロン機能を改善するが、明白な再ミエリン化は伴わない。理論に束縛されずに述べると、作用機構は、軸索の直接的な活性化(保護、神経突起伸長)に起因し、かつ/または生得的免疫系または獲得的免疫系を活性化させてニューロンを保護する因子を分泌させることを介すると理解される。上記で提示した結果は、抗体が軸索/ニューロンに対して直接的な効果を及ぼすことを明確に裏付ける。sHIgM12およびsHIgM42が媒介する軸索保存および/または再成長が測定された同じ脊髄内では、脱髄、再ミエリン化、および炎症の全体が、処置群間で異ならなかった(図11)。
rHIgM12(ヒトJ鎖を伴う)およびsHIgM42の血清半減期
ヒトIgM、rHIgM12(ヒトJ鎖を伴う)、およびsHIgM42の半減期を決定するために、200μlの生理食塩液中に100μgのヒトJ鎖を含有するrHIgM12、または100μgのsHIgM42を、正常CD−1マウスの尾静脈へと注射した(図12)。規定された間隔(15分間、1、4、8、24、48時間)で、マウス3匹ずつの群から、心穿刺を介して血液を回収した。血清を回収し、サンドウィッチELISAを用いて、ヒトIgMミュー鎖の存在についてアッセイした。
放射性標識したヒトモノクローナルIgMは血液脳関門を越える
分子量が百万に近いIgMは、循環から血液脳関門(BBB)を越え、これにより、CNSに入るには大型に過ぎる可能性があることがしばしば受け入れられている(42、43)。しかし、一部のIgMはBBBを越えるという特定の証拠は存在する。
腹腔内送達されたヒトニューロン結合IgMは脱髄した脊髄病変に入り、ニューロフィラメント陽性の軸索に局在する
同位体で標識した、再ミエリン化を促進するマウスIgMである、SCH94.03についてのHunterによる研究(44)は、オートラジオグラフィーを用いて、放射性標識が、in vivoにおいてTMEV感染マウスの脊髄、とりわけ、超微細構造的に希突起膠細胞と同定された細胞に局在することを裏付けた。35S rHIgM12による同様のオートラジオグラフィー研究も実施されている。本発明者らは、従来の免疫細胞化学を用いて、脊髄病変内のニューロン結合ヒトIgMを検出した(図14)。
疾患の発症時に施された場合、rHIgM12またはsHIgM42はMOGペプチド誘導性EAEを増悪させない
自己反応性のCNS結合IgMを自己免疫が活性な動物に投与することにより、疾患を増悪させうるという憂慮に対処するため、EAEを伴うマウスにおけるrHIgM12およびsHIgM42の効果を調べた。MOGペプチド(200μg)誘導性EAEを伴うC57BL6マウス10匹の群に、100μgのrHIgM12、sHIgM42、対照のヒトIgM、または生理食塩液の単回静脈内投与を施した。個々のマウスは、それらの臨床スコアが1に到達した(尾の引きずり)時点で処置した。次いで、マウスを、処置群に対して盲検として、1日おきに体重を記録し、臨床スコアを評価する試験実施者が、マウスが免疫化の28日後に到達するかまたは瀕死となるまで追跡した。
ヒトIgM抗体であるrHIgM12は軸索の形成を促進し、脂質ラフトと相互作用して微小管を標的とする
抗体sHIgM12は、初代培養小脳顆粒ニューロンにおける神経突起成長を促進した。本例では、初代海馬ニューロンを用いて、完全ヒト抗体(ヒトJ鎖を有するrHIgM12)が、軸索の形成を促進することを見出した。rHIgM12は、ニューロン膜に結合し、コレステロールおよびガングリオシドであるGM1のクラスター形成を誘導する。
IgMのニューロンの分化および根底にある分子機構に対する効果をさらに理解するために、組換えrHIgM12を用いて、培養された海馬ニューロンによる神経突起成長アッセイを実施した。海馬ニューロンは、3つの分化段階を経て軸索を形成する(Dottiら、1988年)。複数の過程が細胞体から始まる。急速に成長しつつある神経突起は、その後、そのうちの一方が軸索へと分化し、他方が樹状突起へと分化し、これは、それぞれ、Tau1およびMAP2の示差的分布を介して同定することができる(Lewisら、1989年;(KanaiおよびHirokawa、1995年)。本発明者らは、基板として存在する場合のrHIgM12が、ニューロンの分化を実質的に促進することを見出した。播種後12時間以内に、rHIgM12上で成長しつつある海馬ニューロンが複数の神経突起を発生させ、それらのうちの1つは、近傍の神経突起と比較してはるかに長かった。これに対し、ポリ−D−リシン(PDL)上に播種されたニューロンは、複数の対称的な神経突起を示した(図18A、B)。
基板としてのrHIgM12上で成長しつつある海馬ニューロンは、分化の増強を示した(図18および19)。しかし、本発明者らは、rHIgM12を培養液に適用しても、rHIgM12が神経突起成長を誘導することを観察しなかった(データは示さない)。この観察は、rHIgM12は、その機能を果たすために、細胞外マトリックス分子として提示することが必要であることを示唆した。rHIgM12−ニューロン膜間相互作用をさらに理解するために、生存ニューロンを、まず、rHIgM12で処置し、次いで、固定し、それぞれ、コレステロールまたはガングリオシドであるGM1に結合する、フィリピンまたはコレラ毒素B(CTB)で二重染色した(ShogomoriおよびFuterman、2001年)。rHIgM12、CTB、およびフィリピンは、非処置の対照ニューロンの細胞表面を均等に標識した(図20A)。これに対し、37℃のrHIgM12による処置は、30分後にニューロン膜の再組織化を誘導した(図20B、C)。第1に、rHIgM12は、非処置のニューロン(図20A)、またはニューロン膜に結合しない対照のIgM抗体で処置したニューロン(データは示さない)では観察されない、膜における「パッチ」様構造(図20B1、C1)へと凝集した。第2に、コレステロールのクラスター(図20B2)およびGM1のクラスター(図20C2)のいずれもが、細胞体、神経突起軸、および成長円錐のニューロン膜において形成された。第3に、凝集したrHIgM12は、とりわけ、成長円錐の中央ドメイン(図20B〜C、高拡大率)ではクラスター形成したコレステロール(図20B3)またはGM1(図20C3)と共局在したが、成長円錐の末梢では共局在しなかった。これらの結果は、液浴で適用したrHIgM12が、神経突起/軸索伸長は支持しないが、ニューロン膜の再構成を誘導することを示した。基板として提示された場合のrHIgM12は、膜分子を、rHIgM12−ニューロン膜間接触部位へと動員する結果として、方向付けのシグナル伝達を生成させうる可能性がある。
低温状態における非イオン性洗浄剤による抽出に基づく生化学的研究を用いてコレステロールおよびスフィンゴ糖脂質を含有する脂質ラフトの存在が裏付けられている(ChichiliおよびRodgers、2009年)。しかし、ナノスケールのサイズは光学顕微鏡の解像度を超えるため、脂質ラフトについての形態的情報は限定されたものである(LingwoodおよびSimons、2010年)。rHIgM12が膜のクラスター形成を誘導し、コレステロール含有膜ドメインおよびGM1含有膜ドメインと共局在する(図20)ことの観察により、rHIgM12が脂質ラフトドメインと会合する可能性が立ち上がった。
本発明者らのデータは、凝集したrHIgM12が、微小管が優勢である細胞体、神経突起軸、および成長円錐の中央ドメインなどの領域に局在する(図20)ことを示した(ForscherおよびSmith、1988年)。膜結合rHIgM12が、これらのいずれもがβ3−チューブリンを含有する脂質ラフト画分およびペレット画分へと分別されたという事実(図22B)と併せて述べると、これらの知見は、rHIgM12が、細胞骨格の構成要素(複数可)と会合しうることを示唆した。この仮説を検証するために、海馬ニューロンを、rHIgM12で処置して、膜の再構成を誘導し、次いで、37℃で0.1%のTriton X−100を含有する4%のパラホルムアルデヒドにより同時に固定および抽出した。抽出後、rHIgM12で標識した洗浄剤不溶性膜の点が、神経突起軸において束ねられた微小管束に沿って並んだ(図23A2)。成長円錐では、rHIgM12が、脱束化微小管により占められる中央ドメインに主に局在したが、F−アクチンに富む成長円錐末梢には局在しなかった(図23A3およびA4)。これらの結果は、rHIgM12が微小管と会合しうることを示した。
脊髄損傷に有益なヒトMAbについてのナノ細孔光学バイオセンサーアッセイ
脊髄損傷(SCI)後における軸索の保護および修復は、運動ニューロンの喪失および永続的な身体障害を防止するのに有効な戦略としての大きな可能性を保持する。ニューロンの保護は、傷害後における軸索の損傷を防止し、軸索の修復を促進するための、標的化される栄養因子を用いて達成されている。これらの分子は主に、特異的な低分子である神経栄養因子の標的化に焦点を当てるin vitro系に基づく選択戦略を用いて同定された。
多発性硬化症モデル:用量反応評価
IgM抗体を、それらがマウスの多発性硬化症モデルにおいて機能的改善を促進し、再ミエリン化過程、神経突起成長過程、またはこれらの両方の過程を増強する能力について評価する。モデルは、Warringtonら、2007年と同様の方法を用いてヒトMSにおいて見出される病変と同様の特徴を伴う、CNSの遷延性、慢性で進行性の脱髄性病変を誘導する、ピコルナウイルスのサイラーマウス脳脊髄炎ウイルス(TMEV)の使用を伴う。
多発性硬化症モデル:抗体の組合せ
この実験では、用量比を固定した組換えIgM抗体の多様な組合せを、前出の例で記載したMSのTMEVモデルにおける神経学的転帰の改善について調べる。TMEVへの曝露の6カ月後、マウスにおける組換えIgMの組合せによる処置を開始する。この一連の研究では、マウスに単独のビヒクルまたはIgMの多様な組合せを静脈内単回投与(混合剤)により施す。上記の通りに、感覚運動の評価項目をモニタリングする。髄鞘形成および神経突起成長を評価して、抗体の組合せによりもたらされる変化を調べる。評価される組合せを表4Aおよび表4Bに示す。
脊髄損傷モデル
IgM抗体を、それらがラットの脊髄損傷モデルにおいて機能的改善を促進し、再ミエリン化過程、神経突起成長過程、またはこれらの両方の過程を増強する能力について評価する。モデルは、脊髄を、幅を2.5mmとする改変カバースリップ用ピンセットのブレードの間で側方から、あらかじめ設定したブレード間隔である0.9mmまで、15秒間にわたり圧迫することを伴う。結果として得られる病変は、ヒトSCIにおいて見出される特徴と同様の特徴を示す(Grunerら、1995年)。

脊髄損傷モデルにおける個別の抗体の使用
この実験では、組換えヒト抗体を、表5Aおよび表5Bに示す通り、3つの用量レベルにおいて単独で投与する。
脊髄損傷:抗体の組合せ
この実施例では、用量比を固定した組換えIgM抗体の多様な組合せを、本明細書の実施例14で記載した脊髄損傷モデルにおける神経学的転帰の改善について調べる。したがって、IgM抗体の組合せ(混合剤として)またはビヒクル対照による静脈内処置を、損傷の10分後に施す。運動評価項目を、上記の通りにモニタリングする。さらに、髄鞘形成および神経突起成長を評価して、抗体の組合せによりもたらされる変化を調べる。評価した組合せを、表6Aおよび表6Bに示す。
ヒトニューロン結合IgMである組換えrHIgM12抗体は脊髄軸索を保護する
本発明者らは、天然ヒト血清抗体であるsHIgM12が、in vitroにおいてニューロンに結合し、神経突起成長を促進することを裏付けた。本発明者らは、同一の特性を伴う組換え形態であるrHIgM12を生成させた。サイラーマウス脳脊髄炎ウイルス(TMEV)による感受性マウス系統の脳内感染は、多発性硬化症の進行性の形態と同様の進行性の軸索喪失および神経機能不全を伴う慢性脱髄性疾患を引き起こす。このモデルを、希突起膠細胞に結合するIgMクラスの抗体で処置すると、CNSの再ミエリン化が改善される(Warringtonら(2007年)、J Neurosci Res、85巻:967〜976頁)。これに対し、ニューロンに結合する血清由来のヒトモノクローナル抗体(sHIgM12)は、ラミニンと同程度に頑健な神経突起成長を促進し、CNSミエリンの神経突起成長に対する阻害効果を低減する(Warringtonら(2004年)、J Neuropathol Exp Neurol、63巻(5号):461〜473頁)。より近年には、上記と同一の生物学的特性を伴うヒトsHIgM12の組換え形態(rHIgM12)を生成させた。rHIgM12の脊髄軸索の完全性に対する効果を研究するために、本発明者らは、解剖学的に連続であり、機能的に保存された軸索に依拠する技法である逆行標識法を実施した。
逆行標識法:マウスに麻酔をかけた後、下部胸椎(T10〜11)において背部椎弓切除を実施した。脊髄を右側において片側切断し、片側切断部位に4%Fluorogoldの滅菌溶液を充填した。手術の1週間後、マウスを屠殺し、脳を摘出した。脳幹の連続切片(40mm厚)を作製し、切片を4枚ごとに解析した。16枚の脳幹スライスから、大型で明るい蛍光ニューロンを、200倍の拡大率下でカウントした。
TMEVモデルは、神経保護を促進し、軸索喪失を防止する戦略を開発するためのプラットフォームをもたらす。疾患の早期は炎症および脱髄を包含し、後期は軸索喪失および機能欠損を提示する。前出例で詳述し、共焦点顕微鏡画像(図15)を介して確認される通り、1mgの単回腹腔内注射の後、rHIgM12は脊髄に入り、ニューロフィラメント+(NF)の軸索に局在する。rHIgM12はまた、クロスカット形としてNF+線維束にも共局在する(図15D)。動物研究では、rHIgM12が、逆行標識した脳幹ニューロン数を増大させる。感染の90日後(dpi)に、rHIgM12または生理食塩液をSJLマウスに投与した。処置の9週間後に、逆行標識法のための脊髄手術を実施した。手術の1週間後、マウスを屠殺し、脳幹の連続切片により、蛍光標識したニューロンを定量化した。rHIgM12は、生理食塩液対照と比較して逆行標識した脳幹ニューロン数を増大させる(データは示さない)。したがって、rHIgM12で処置したマウスの逆行標識した脳幹ニューロンは、生理食塩液で処置した対照と比較してより多かった。
ニューロン結合ヒトモノクローナル抗体の単回投与はマウス脱髄モデルにおける自発活動を改善する
本発明者らの実験室は、天然ヒト血清抗体であるsHIgM12が、in vitroにおいてニューロンに結合し、神経突起成長を促進することを裏付けた。本発明者らは、同一の特性を伴う組換え形態であるrHIgM12を生成させた。サイラーマウス脳脊髄炎ウイルス(TMEV)による感受性マウス系統の脳内感染は、結果として、多発性硬化症の進行性の形態と同様の進行性の軸索喪失および神経機能不全を伴う慢性脱髄性疾患をもたらす。rHIgM12のTMEV感染マウスの運動機能に対する効果を研究するため、本発明者らは、夜間における自発活動を、何週間にもわたってモニタリングした。通常は活動的な夜間のモニタリング時間において最大限の活動変化が生じることが予測されるため、夜間挙動は、齧歯動物の神経機能についての高感度の尺度である。ベースラインの自発活動についてまとめるため、マウスを、処置前に8日間にわたり活動ボックスに入れた。処置後、各群における活動を8週間にわたり持続的に記録した。本発明者らは、以下の2つの理由で8週間にわたる長期のモニタリング期間を選択した:(1)本発明者らは既に、IgM誘導性再ミエリン化が、処置後5週間までに示されると裏付けたこと、および(2)この系統におけるTMEV誘導性脱髄性疾患の進行は極めて遅いこと。長期の観察期間および大規模なデータセットに起因して、フィルタリングされていない元の記録を研究しながら、処置群間の差異を十分に理解することは困難でありうる。高度に変動的な元のデータにおける変化を明確に詳述するために、本発明者らは、3つの異なる方法:(1)ビニング法、(2)ガウスローパスフィルター(GF)の適用、および(3)多項式フィッティングを適用した。3つの方法の各々を用いて、本発明者らは、rHIgM12による早期の処置が、水平方向の運動機能および垂直方向の運動機能のいずれにおいても、対照のIgMおよび生理食塩液と比較して改善を誘導するのに対し、後期の処置が改善するのは水平方向の活動だけであることを示した。rHIgM12は、正常な非感染マウスの活動を変化させなかった。この研究は、ニューロン結合IgMによる処置は、in vitroにおいてニューロンを保護するだけでなく、また、運動機能の改善にも影響を及ぼすという仮説を裏付ける。
齧歯動物の疾患モデルにおける、長期にわたる神経機能のモニタリングおよび解析は、依然として難題である。通常は活動的な夜間のモニタリング時間において最大限の活動変化が生じることが予測されるため、夜間挙動は、齧歯動物の神経機能についての高感度の尺度である[1]。しかし、進行が緩徐な疾患の動物モデルでは、動作のモニタリングが数週間にわたることが多い。本発明者らは既に、希突起膠細胞結合抗体(rHIgM22)が、処置後5週間までに、再ミエリン化を増強することを報告した[2]。疾患および修復の発生のいずれもが緩徐な過程であることを考え合せ、かつ、活動における任意の変動を確実にするために、本発明者らは、短期のモニタリングにおいて用いられるサンプリング密度と同じサンプリング密度で、長期にわたる活動をモニタリングした。これにより、大規模で高度に変動的なデータセットが創出された(図32A、C)。処置後の変化を明確に詳述し、長期の活動における一般的な傾向を復元するために、本発明者らは、Mathematica(Wolfram Research,Inc.)を用いることにより、データビニング、ガウスフィルタリング、および多項式フィッティングの使用を比較した。
マウス:SJL/Jマウス(Jackson Laboratories、Bar Harbor、ME)を、Mayo Clinicの動物ケア施設に収容し、飼育した。動物用のプロトコールは、Mayo ClinicのInstitutional Animal Care and Use Committeeにより承認された。
感染の90日後において施すと、rHIgM12はSJLマウスにおける水平方向の活動を改善する
感染の90日後(dpi)におけるSJLマウス5匹ずつの3つの群を活動ボックスに入れ、連続8日間にわたりベースラインの活動を測定した。2つの群のマウスを、rHIgM12またはアイソタイプの対照IgM 200μgずつの単回投与で処置した。第3群は、生理食塩液で処置した。処置後、自発活動を、8週間にわたり持続的に記録した。データは、1時間のブロックで収集したため、本発明者らは、各群について約900ずつのデータ点を得た。この元の生データ(図32A〜C)は高度に変動的であるので、処置群間の差異を十分に理解することは困難でありうる。3つの方法(ビニング法、ガウスフィルタリング、および多項式フィッティング)全てにより、rHIgM12で処置したマウスが、水平方向の活動における改善を示すのに対し、対照のIgMおよび生理食塩液で処置したマウスは、8週間にわたり活動の変化を示さないことが明らかとなった(図33A、C、およびE)。多項式フィッティングの後、本発明者らは、3つの処置についての直接的な対応のある比較を用いて、rHIgM12処置マウスにおける水平方向の運動機能の改善が、処置の7日後(対照のIgMと比較して)および11日目において(生理食塩液と比較して)統計学的に有意となることを決定した(図34)。rHIgM12処置動物の水平方向の夜間活動において観察される改善は、約30日間にわたり持続し、次いで、ベースラインレベルに戻った。生理食塩液処置群の対照IgM処置群と対比した対応のある比較は、38〜52日目にわたり統計学的な有意性を示した。しかし、rHIgM12処置群と対照との間で観察される主要な差異と比較した場合、本発明者らは、対照群間におけるこれらの差異は、生物学的に有意ではないと考える。他方、垂直方向の活動は、主要な差異を示さず、3つの群全てにおいて同様であった(図33B、D、およびF)。
以前の研究では、垂直方向の活動(後肢による立脚挙動)を主要なリードアウトとして用いた[1]。慢性TMEV感染マウスでは、軸索の脱落に起因して、後肢が徐々に脆弱となり、こわばるので、後肢による立脚が低減された。この研究の第1の実験では、rHIgM12を、脱髄が最大となり、進行性の軸索喪失が始まる時点で投与した(感染の90日後)ところ、水平方向の活動だけが改善された。後肢による立脚挙動は影響を受けず、3つの処置群全てにおいて同等であった。したがって、本発明者らは、早期の時点における処置がより有益でありうるかどうかを問うた。第2の実験では、マウス5匹の群を、感染の45日後に、200μgのrHIgM12、アイソタイプの対照IgM、または生理食塩液の単回投与で処置した。同一の実験デザインを用い、ベースラインの活動を8日間にわたり収集し、処置後の活動を8週間にわたり収集した。処置の約2週間後に始まるこの実験では、rHIgM12で処置したマウスが、水平方向および垂直方向のいずれの活動においても明らかな改善を示した。これは、3つの方法の全て:データの平均化、ガウスフィルターまたは多項式フィッティングの適用を用いた後で明らかとなった(図35C〜H)。対照のIgMおよび生理食塩液で処置したマウスは、研究の終了まで同様の活動レベルを示した。rHIgM12で処置したマウスでは、実験が終了するまで、水平方向の活動の改善が明らかであった。他方、垂直方向の活動の改善は、約4週間にわたって続き、次いで、ベースライン値へと低下した。3つの処置についての直接的な対応のある比較は、rHIgM12で処置したマウスにおける水平方向の運動機能の改善が、処置の13日後において(対照のIgMと比較して)、および処置の9日後において(生理食塩液と比較して)有意に異なることを示した(図36AおよびC)。同様に、rHIgM12で処置したマウスにおける垂直方向の運動機能も、処置の14日後において(対照のIgMと比較して)、および処置の6日後において(生理食塩液と比較して)有意に異なった(図36BおよびD)。対照のIgM処置群を生理食塩液処置群と対比する比較は、水平方向の活動または垂直方向の活動のいずれについても大きな生物学的差異を明らかにしなかった(図36EおよびF)。
以前の2つの実験におけるrHIgM12による処置は、神経障害を来した感染マウスにおいて明らかに有益な効果を示した。この抗体が刺激性の特性を有し、したがって、運動機能の増大を誘発する可能性を除外するために、本発明者らは、非感染マウスによる同様の実験を実施した。週齢を一致させた非感染マウスの3つの群を、rHIgM12、対照のIgM、または生理食塩液で処置した。感染マウスにおける活動の増強と比較して、rHIgM12、ならびに、他の2つの処置は、正常マウスの運動機能の増大を誘導しなかった(図37)。3つの群全てが、自発活動の低下傾向を示した。この結果は、いずれの抗体も、正常マウスにおける活動の増大に影響を及ぼさないことを示す。
多発性硬化症のほか、他の脱髄性疾患および神経変性疾患のための神経保護療法を開発することが火急に必要とされている。炎症性CNS疾患における軸索損傷の軽減を間接的にもたらしうる抗炎症薬も存在するが、ニューロン/軸索のレベルで直接作用する薬物は存在しない。神経保護の主要な目標は、ニューロンの機能不全を制限し、ニューロンおよび軸索の機能的完全性を維持しようと試みることである。多年にわたり、MSの病理学的特徴である脱髄は、永続的な神経欠損の原因であると考えられた。今日では、脱髄が永続的な軸索喪失に必要ではあるが十分ではないことが明らかである[9]。脱髄だけが、露出された軸索に、T細胞の細胞傷害作用または死滅した希突起膠細胞に由来する局所性の神経栄養性の支持の喪失により引き起こされる続発的損傷に対する素因を与える[10]。
運動ニューロン疾患であるALSを処置するためのニューロン保護的ヒトMAb
筋萎縮性側索硬化症(ALS)とは、主に前角細胞および皮質脊髄路ニューロンを損なう深刻な神経疾患である。ALSとは、脳および脊髄における運動細胞の進行性の変性を特徴とする運動ニューロン疾患である。運動細胞(ニューロン)は、個体が動き回り、話し、呼吸し、嚥下することを可能とする筋肉を制御する。神経がそれらを活性化しなければ、筋肉は徐々に脆弱化し、失われる。広範な研究にもかかわらず、この障害の病因は、大部分未知であり、有効な処置は存在しない。ALSのまれな遺伝子形態を伴う少数の患者において関与する遺伝子により、この障害に対する潜在的な鍵がもたらされる[1]。同定された1つの遺伝子変異は、Cu/Zn SOD(銅/亜鉛スーパーオキシドジスムターゼ)変異であり、これは、ALSの遺伝形態を伴う患者のうちのわずかな比率において存在する[2]。SOD変異を保有する患者は、関連の遺伝子変異を伴わずにこの疾患を自発的に発症する患者と比較して、神経学的転帰が酷似することが明らかである[3]。この酵素は、スーパーオキシドから酸素および過酸化水素への触媒を行う。SOD1とは、ALSと関連する酵素形態である。軸索輸送を急速に損なうSOD1の機能獲得が存在するようである[4]。家族性ALSにおけるSOD変異の頻度は12〜23%で変化し、この遺伝子は通常、常染色体優性遺伝子として遺伝する。
本発明者らは、G86R hSOD1変異を伴うマウス(FVBTg SOD1 G86R M1Jwg、Jackson Labs)[13]を、ヒト抗体rHIgM12で処置する実験を実施した。G86Rマウスは、出生時は正常に見える。しかし、約90〜100日齢から、G86Rマウスは、顕著な体重減少を経て、著明な筋肉の萎縮を発症し、急激な体重減少から数週間以内に呼吸器不全で死滅する。LUDOLPH?無作為化「盲検」試験では、これらのマウスが55日齢のときに、GMPグレードで精製されたrHIgM12を、単回腹腔内投与(200μg)として施した。これに対し、プラセボ群には、リン酸緩衝生理食塩液(PBS)を施した。少数のマウスは未処置のまま放置して、人為的な操作を伴わない疾患の自然経過を決定した。PBS処置マウスおよび非処置マウスについてのデータは同様であったため、これらの群を統計学的な解析のために併合した。マウスは、本発明者らの実験室の1人の試験実施者による処置のために無作為化し、それらが瀕死となるまで、別の「盲検の」試験実施者に神経欠損について観察させた。処置する試験実施者と、病理学的解析を実施する他の試験実施者とは、コードが解読されるまで無作為化プロトコールについて知らされなかった。
上記で、組換えヒトモノクローナル抗体(rHIgM12)が、前角細胞および軸索の喪失を防止することにより、ヒトALSのトランスジェニックモデルにおける生存を延長しうることを示したので、ALS患者における第I相臨床試験へと向かって、薬物動態および毒性学を介する前臨床データを作成する研究が進行中である。
ALSの表現型を伴うSODマウスの2つの系統(SOD1 G86RおよびSOD1 G93A)において、組換えヒト抗体rHIgM12を、アイソタイプ対照のヒト抗体であるsHIgM39と対比して調べる、決定的な「盲検」プラセボ対照研究を実施する。組換えヒト抗体であるrHIgM12 200μgの単回投与の、疾患を軽減する有効性を、G86R SOD1およびG93A SOD1両方[14](B6 Cg−Tg SOD1 G93A 1Gur、Jackson Labs)の変異体トランスジェニックマウスモデルにおいて、アイソタイプ対照のヒト抗体であるsHIgM39 200μgの投与と比較して調べる。ENMCによる発症前処置の推奨に従い、抗体による処置を55日齢で行うと[18]、rHIgM12をPBSと比較するパイロット研究を反映する結果がもたらされる(図38〜41)。主要評価項目は、生存および体重減少の防止である。各実験群は、上記データに基づく差異を検出するのに十分な24匹のマウスからなる。加えて、屠殺後の全てのマウスにおけるCNSも調べ、NeuNについての標識を用いて胸部中央の脊髄における前角細胞の数を決定する。異常なミエリン渦により示される、変性した軸索の数をカウントする。G93A SOD1変異体モデルである(B6.Cg−Tg)SOD1 G93A)1Gur/J(型番004435;Jackson Laboratory)は、これが、ALSについての、最初の、遺伝子ベースで、最も広く用いられ、最もよく特徴付けられたモデルであり、rHIgM12についての結果を、処置の枠組みについての広範なデータベースと比較することを可能とするために包含した。G86Rマウスは、疾患の発症が遅く(7カ月後)、進行が速いのに対し、G93Aマウスは、発症が速く(3〜4カ月後)、進行が遅い。この研究は、SOD1マウスへの外来タンパク質の導入について調整し、また、作用機構の概念についても取り組むものである。rHIgM12のニューロン結合特徴は、極めて重要であり、ニューロンに結合しないIgMは、疾患を改善しないはずである。本研究における主要評価項目は、生存の増大(10%以上:P<0.05)および体重減少の軽減(10%以上:P<0.05)である。SOD1マウスのうちの少なくとも1つの系統で改善が見られれば、成功と考える。全てのマウスを、いずれかの側に仰臥させて15〜30秒間以内に自ら直立できなくなる時点で屠殺する。副次評価項目では、ミエリン渦の異常、および脊髄胸部(T6レベル)におけるNeuN陽性前角細胞の密度により示される、変性した軸索の数を測定する。1)rHIgM12で処置したマウスの体重減少が対照と比較して20%多い場合、2)rHIgM12で処置したマウスが発作を発症する場合、3)rHIgM12で処置したマウスの死亡率が対照より20%高い場合は、有害事象を考慮した。
上記の研究における肯定的な結果に続き、SOD1マウスを死滅から保護するのに必要とされる最小限の用量を決定するために、用量滴定研究(55日齢のマウス1匹当たり0、5、50、100、および250μgの単回投与を施す)を着手する。マウス脳幹についてのMR分光法を用いて、N−アセチルアスパラギン酸(NAA)を測定する。本発明者らは、他の神経疾患モデルにおいて、脳幹内のNAAが、脊髄全体における軸索健康の優れたサロゲート指標であることを示した[17]。MR分光法のデータは、rHIgM12を用いる潜在的なヒト試験における抗体有効性についてのサロゲートマーカーとしてのNAAを検証する一助となる。55日齢のSOD1マウス20〜24匹の群に、腹腔内を介して0、5、50、100、および250μgの単回投与を施す。主要評価項目および副次評価項目ならびに有害事象については上記と同じとするが、N−アセチルアスパラギン酸(NAA)を測定する、脳幹におけるMR分光法が加わる。MR分光法は、100日齢時および屠殺直前の抗体による処置日において、各マウスについてまとめる。100日目または最終走査時の任意のrHIgM12処置群の脳幹におけるNAA濃度の、生理食塩液で処置したマウスと比較した10%(P<0.05)の増大を改善と考える。
研究は、正常マウス血液免疫グロブリンのバックグラウンドにおけるヒトIgMを特異的に検出する、確立されたELISA検出システムを用いて、200μgの静脈内単回投与後の50〜70日齢のSOD1 G86RマウスおよびSOD1 G93Aマウス[18]において実施する。ヒトIgMは、投与後の多様な時点(0、15分間、30分間、1時間、4時間、8時間、12時間、18時間、24時間、2日間、3日間、5日間、および7日間)の血液中で測定する。マウスの血液量は、少量(全血液量<1.5ml)であるため、各回収時点では、3つの個別のマウスを用いる。
rHIgM12が有効性を裏付けた、SOD1系統(複数可)における200μgの用量を用いて、rHIgM12についての血中反応速度試験を実施する。研究は、静脈内注射後7日間にわたる13の回収点において、1時点当たり3匹ずつのSOD1マウス群により実施する。50〜70日齢のSOD1マウスを処置して、処置時点における抗体反応速度を理解する。rHIgM12の血清半減期、曝露の飽和、および中和抗体であるIgMの存在について決定する。同位体(35S)標識したrHIgM12を追跡する研究では、上記の最小限の有効用量の決定が必要とされる。50〜70日齢のSOD1マウスに、rHIgM12 250μgおよびrHIgM12の最小限の有効用量(少なくとも1×107cpmを含有する)[19]の単回静脈内投与を施して、rHIgM12が、治療的処置の時点でCNSに入るかどうかに取り組む。注射の4、8、24、48、および72時間後において、脳、脊髄、肝臓、心臓、肺、胃、腸、筋肉、脾臓、肝臓、膵臓を含めた主要な組織を迅速に摘出する。組織部分150mgを摘出し、秤量し、Solvable(Perkin Elmer)中で溶解させ、シンチレーション液(Ultima
Gold、Perkin Elmer)中でcpmを決定する。同位体で標識したrHIgM12は、オートラジオグラフィーを用いた脊髄切片中で局在する[19]。この実験では、主要評価項目を、rHIgM12を注射した4、8、24、48、および72時間後に対照のゼロ時点と比較した、マウスの脳または脊髄における35S同位体の蓄積とする。任意の時点の脳または脊髄の全体1mg当たりの35Sのカウントの50%(P<0.05)の増大を有意と考え、rHIgM12がCNSに入りうることの証拠であると考える。オートラジオグラフィー研究では、マウスに標識したrHIgM12を投与し、rHIgM12がCNS内で増大した時点において脊髄を摘出する。脊髄切片中のニューロン全体1mm2当たりの銀粒の50%の増大を、in vivoにおける抗体ターゲティングの有意な証拠と考える。
rHIgM12により実験を実施して抗体が正常CD−1マウスおよび正常ウサギにおいて毒性であるかどうかを決定する:両方の性別の正常CD−1マウス10匹の群に、上記で決定したrHIgM12の最小限の有効用量の1倍(1×)、10倍(10×)、および20倍(20×)、または生理食塩液を、1回または7日間にわたり毎日静脈内投与する。2週間後、「完全」剖検を介して血液および組織を回収して解析する。主要な器官全ての組織切片を、毒性学評価において熟練した獣医学の病理学者が「盲検により」調べる。血液を、肝臓酵素、心臓酵素、電解質および血液学グループに対する効果についての血液研究を含めた、通常の一連の毒性スクリーニングのための化学および血液学で特徴付ける。同一の曝露研究を齧歯動物以外の種において実施し、各性別のウサギ2羽ずつを各用量で調べる。加えて、rHIgM12を用いる組織の交差反応性研究を、抗体が他の任意の組織または器官に結合するかどうかを決定するのに用いる。マウス、ウサギ、霊長動物、およびヒト(Zymed)に由来するパラフィン包埋した一連の組織切片を、rHIgM12または対照のヒトIgMで免疫標識する。in vivoにおける状況をより緊密に模倣するので、各組織内の結合の強度および構造を画像化し、rHIgM12および対照のIgMによる組織チョッパーで切断した生存組織スライスの標識化と比較する[7]。加えて、FDAへの新薬臨床試験開始届の予備的毒性試験の項と一致して、rHIgM12および対照抗体の結合も、凍結ヒト組織および凍結霊長動物組織において調べる。組織交差反応性研究は、マウスおよびウサギにおける曝露研究の間、特に、器官をモニタリングするための鍵をもたらす。
有効性を決定した後、これらの研究では、標的組織および標的以外の組織への結合に取り組む。市販される組織アレイ(Zymed)およびヒトおよびカニクイザル(Charles River)の切片を、10μg/mlのrHIgM12および対照のヒトIgMであるsHIgM39で免疫標識する。ディジタル画像を用いて標識の強度を比較する。rHIgM12の、パラフィン包埋して固定した組織アレイおよび凍結させたヒトおよび霊長動物の脳および脊髄への結合を、生存マウスの小脳スライスにおいて観察される抗体結合と比較する。本発明者らは、標準的な効力アッセイである生存小脳スライスへの抗体結合を用いたが、これは、rHIgM12の血清形態が同定された最初のスクリーニングである。

Claims (18)
- 脳虚血もしくは脳卒中を処置もしくは改善するか、脳卒中後もしくは脳虚血後の動物における神経機能を改善するか、または脳卒中後もしくは脳虚血後のニューロンもしくは神経細胞を損傷もしくは細胞死から保護するのに使用される、ニューロンに特異的に結合し、ニューロンを細胞死から保護し、再ミエリン化を促進しない単離ヒトIgM抗体またはその断片であって、以下の配列:
(a)図5に示される、可変重鎖アミノ酸CDRドメイン配列CDR1 GGSVSLYY(配列番号31)、CDR2 GYIYSSGST(配列番号32)、およびCDR3 ARSASIRGWFD(配列番号33)、ならびに軽鎖CDR配列CDR1 QSISSY(配列番号34)、CDR2 AAS(配列番号35)、およびCDR3 QQSYHTPW(配列番号36)、または
(b)図6に示される、可変重鎖アミノ酸CDRドメイン配列CDR1 GFTFSTYA(配列番号37)、CDR2 INVGGVTT(配列番号38)、およびCDR3 VRRSGPDRNSSPADF(配列番号39)、ならびに軽鎖CDR配列CDR1 QGIG(配列番号40)、CDR2 TTS(配列番号41)、およびCDR3 QKYNSAPRT(配列番号42)
を含む抗体またはその断片。 - 配列番号1に示される可変重鎖アミノ酸配列および配列番号11に示される可変軽鎖アミノ酸配列を含むか、または配列番号17に示される可変重鎖アミノ酸配列および配列番号27に示される可変軽鎖アミノ酸配列、もしくは相同性の高いそれらの変異体を含み、該変異体がニューロン結合活性および神経保護活性または神経再生活性を保持する、請求項1に記載の単離抗体。
- 組換え抗体rHIgM42であり、図6に示される、配列番号17に示される可変重鎖アミノ酸配列および配列番号27に示される可変軽鎖アミノ酸配列を含む、請求項1に記載の単離抗体。
- ヒトJ鎖配列をさらに含む、請求項1に記載の単離抗体。
- 前記J鎖が配列番号15に示されるアミノ酸配列を含む、請求項4に記載の抗体。
- 組換え抗体rHIgM12であり、配列番号1に示される可変重鎖アミノ酸配列および配列番号11に示される可変軽鎖アミノ酸配列を含む、請求項4または5に記載の抗体。
- 配列番号1および11のアミノ酸配列または相同性の高いそれらの変異体から選択されるアミノ酸配列を含む重鎖可変領域および軽鎖可変領域を含む完全ヒト抗体またはその抗体断片であり、該変異体がニューロン結合活性および神経保護活性または神経再生活性を保持する、請求項5に記載の単離抗体または断片。
- 脳または脳細胞または神経細胞への血流または酸素が不十分な、脳卒中、外傷性脳損傷(TBI)、脳虚血、または他の状態において使用される、請求項1から7のいずれかに記載の単離抗体またはその断片。
- 脳卒中または脳虚血を処置または改善するのに使用するために、血栓溶解剤、抗血小板薬、血圧降下薬、またはスタチンと組み合わせて製剤化される、請求項1に記載の単離抗体。
- 検出可能な標識または機能的な標識で標識した、請求項1から7のいずれかに記載の抗体。
- 前記標識が酵素、特異的結合パートナー、リガンド、色素、蛍光タグ、および/または放射性元素である、請求項10に記載の抗体。
- 請求項1から7のいずれかに記載の抗体または断片をコードする配列を含む単離核酸。
- 請求項1から7のいずれか一項に記載の抗体または断片を調製する方法であって、該抗体または断片の発現をもたらす条件下で請求項12に記載の核酸を発現させるステップと、該抗体または断片を回収するステップとを含む方法。
- 請求項1から7のいずれか一項に記載の抗体または断片と、薬学的に許容される媒体、キャリア、または希釈剤とを含む、脳卒中または脳虚血を処置または改善するための医薬組成物。
- 請求項14に記載の医薬組成物の医薬剤形と、1または複数のさらなる神経作用剤もしくは治療薬、血栓溶解剤、血圧降下剤、抗血小板剤、抗炎症剤、神経伝達物質放出調節剤、神経受容体リガンドもしくは神経受容体アゴニストもしくは神経受容体アンタゴニスト、カルシウムチャネル剤、免疫調節剤、または他のCNS反応性抗体を含む個別の医薬剤形とを含む、動物における脳卒中または脳虚血を処置または改善するためのキット。
- 動物における脳卒中または脳虚血を処置または改善するための方法であって、脳卒中または脳虚血を患うことが疑われるかまたはそれを患うことが決定された動物に、有効量の、請求項14に記載の医薬組成物を投与するステップを含む方法。
- 前記組成物を、単回投与として、脳卒中または脳虚血を患うことが疑われるかまたはそれを患うことが決定された前記動物に投与する、請求項16に記載の方法。
- 脳卒中または脳虚血を患うかまたはそれを有することが疑われる哺乳動物における神経の傷害、死滅、または損傷の存在または程度を検出する方法であって、
A.脳卒中または脳虚血が疑われる哺乳動物に由来する生物学的試料を、請求項1から7のいずれかに記載の抗体または断片と、該抗体または断片の該試料中のニューロンへの結合がおこることを可能とする条件下で接触させるステップと、
B.該試料に由来する該ニューロンと該抗体との間で結合が生じたかどうかを検出するか、または該試料に由来する該ニューロンおよび該抗体により生じた結合の量を決定するステップと
を含み、
該結合の検出により該試料中の神経細胞の傷害、死滅、または損傷の存在が示され、該結合の量により神経細胞の傷害、死滅、または損傷の相対量が示される方法。
