Abstract
Description
既存の仮システム、キット、及び方法では、角度付き骨切り(例えば膝の関節線に対して平行でない骨切り)を脛骨の近位端又は大腿骨の遠位端に施す場合に、外科医は骨関節運動特性を洞察することができない。既存の仮システム、キット、及び方法の場合、さらに、永久脛骨補綴システムの適切な形態に達するために比較的数多くの仮コンポーネントの積み重ねが必要となり、或いは仮膝関節バランスをリアルタイムに示す力又は圧力感知データを提供することができない。本発明の仮システム、キット、及び方法は、外側縁とは異なる高さを有する内側縁、又は後縁とは異なる高さを有する前縁、のうちの一方又は両方を有するシム・コンポーネント、又は支承コンポーネント、支承支持コンポーネント、又はシム・コンポーネントと結合又は一体化されたセンサを含むことができる。このようなシム・コンポーネント形態は、大腿骨又は脛骨に角度付き骨切りを施す前に、この骨切りに関する膝関節運動特性を外科医が洞察するのを可能にし、そして外科的サイジング中に必要となる仮コンポーネントの個数を低減することができる。センサはリアルタイムの膝関節バランス・テストを容易にすることができる。
Claims (21)
- 外科処置中の膝関節の力バランスを判定するシステムであって、
支承コンポーネントと、ベース・コンポーネントと、シム・コンポーネントとを含む脛骨補綴物を含み、
前記支承コンポーネントは、上関節面と下面とを有し、且つ上側部分と、下側部分と、該上側部分と該下側部分との間に配置されたフレームと、該フレームと前記下側部分との間に配置された複数のセンサとを含み、前記フレームは、サイズ又は形状が該複数のセンサに対応する複数のアパーチャを有しており、
前記ベース・コンポーネントは上面と下面とを有しており、
前記シム・コンポーネントは、前記支承コンポーネントの下面と前記ベース・コンポーネントの上面との間で前/後方向にスライド可能であるように形成されている、システム。 - 前記複数のセンサが前記フレームと前記下側部分との間に配置されたプレート上に位置決めされている、請求項1に記載のシステム。
- 前記複数のセンサが前記支承コンポーネントの上関節面の大部分と整合するように、前記複数のセンサが前記プレートの大部分を占めている、請求項2に記載のシステム。
- 前記複数のセンサが、圧電センサ、力感知抵抗器、歪みゲージ、力ゲージ、ロードセル、ポテンショメータ、バロメータ、他の圧力センサ、及び他の力センサから成る群から選択される、請求項1又は2に記載のシステム。
- ディスプレイ上の提示のために前記脛骨補綴物の面積又は体積を表示するユーザーインターフェイスをさらに含み、該表示は内側/外側指示及び前/後指示を含む、請求項1又は2に記載のシステム。
- 前記複数のセンサと前記ユーザーインターフェイスとの間の有線接続部又は無線接続部をさらに含み、前記ユーザーインターフェイスは、前記複数のセンサの少なくとも第1部分集合からの力又は圧力の感知データ及びセンサ位置に基づいて前記ディスプレイ上に提示するための少なくとも1つの力中心インジケータを含み、該少なくとも1つの力中心インジケータは、膝関節上の力又は圧力レベル間のバランス又はアンバランスを指し示す、請求項5に記載のシステム。
- 前記複数のセンサからの情報を受信して処理するように形成された、前記ユーザーインターフェイスと接続されたプロセッサさらにを含む、請求項5に記載のシステム。
- 前記プロセッサが前記脛骨補綴物内に含まれている、請求項7に記載のシステム。
- 複数の個別の突起をさらに含み、該突起は前記フレームの複数のアパーチャ内部に嵌合するようにサイジングされて成形され、前記支承コンポーネントの上関節面と下面との間に位置決めされている、請求項1又は2に記載のシステム。
- 前記複数の個別の突起のそれぞれが前記複数のセンサのうちの1つと動作的に係合する、請求項9に記載のシステム。
- 前記シム・コンポーネントが、前記支承コンポーネントと前記ベース・コンポーネントとの間の近位/遠位方向の間隔に基づいて、複数のシムから選択される、請求項1又は2に記載のシステム。
- 前記複数のシムが種々の異なるサイズのシムを含んでおり、少なくとも1つのシムが、外側縁とは異なる高さを有する内側縁、又は後縁とは異なる高さを有する前縁、のうちの一方又は両方を含む、請求項11に記載のシステム。
- 外科処置中の膝関節の力バランスを判定するシステムであって、
支承コンポーネントと、ベース・コンポーネントと、一連のシム・コンポーネントとを含む脛骨補綴物を含み、
前記支承コンポーネントは上関節面と下面とを有し且つ複数のセンサを含み、
前記ベース・コンポーネントは上面と下面とを有しており、
前記一連のシム・コンポーネントのうちの少なくとも1つのシムが、外側縁とは異なる高さを有する内側縁、又は後縁とは異なる高さを有する前縁、のうちの一方又は両方を含み、前記支承コンポーネントと前記ベース・コンポーネントとの間の近位/遠位方向の間隔に基づいて、前記支承コンポーネントの下面と前記ベース・コンポーネントの上面との間に前後方向で挿入するために、1つ又は2つ以上のシムが選択される、システム。 - 前記支承コンポーネントは、前記複数のセンサを保持するように構成されたプレートをさらに含む、請求項13に記載のシステム。
- 前記支承コンポーネントは、上側部分と、下側部分と、該上側部分と該下側部分との間に配置されたフレームとをさらに含み、該フレームは、サイズ又は形状が前記複数のセンサに対応する複数のアパーチャを有し、前記プレートは前記フレームと前記下側部分との間に配置されている、請求項13又は14に記載のシステム。
- ハンドリング器具をさらに含み、該ハンドリング器具は、膝関節内の前記脛骨補綴物の挿入中に該脛骨補綴物の少なくとも1つのコンポーネントと係合するように形成されている、請求項13又は14に記載のシステム。
- 外科処置中の膝関節の力バランスを判定するシステムであって、
支承コンポーネント及びベース・コンポーネントを含む脛骨補綴物であって、前記支承コンポーネントは、上関節面と下面とを有し、上側部分、下側部分、該上側部分と該下側部分との間に配置されたフレーム、及び該フレームと該下側部分との間に配置された複数のセンサを含み、前記フレームは、サイズ又は形状が該複数のセンサに対応する複数のアパーチャを有しており、前記ベース・コンポーネントは上面と下面とを有している、脛骨補綴物と、
該支承コンポーネントの下面と該ベース・コンポーネントの上面との間で前/後方向にスライド可能であるように形成された少なくとも1つのシム・コンポーネントであって、種々のサイズの複数のシム・コンポーネントから選択される少なくとも1つのシム・コンポーネントと、
ディスプレイ上の提示のために前記脛骨補綴物の面積又は体積を表示するユーザーインターフェイスと、
前記ユーザーインターフェイスと前記複数のセンサとの間の接続部であって、前記ユーザーインターフェイスが前記複数のセンサからの情報を受信して、前記ディスプレイ上の提示のために膝関節上のバランス又はアンバランスの少なくとも1つの力中心インジケータを提供するように形成されている、接続部と、を含む、システム。 - 種々のサイズの前記複数のシム・コンポーネントが、外側縁とは異なる高さを有する内側縁、又は後縁とは異なる高さを有する前縁、のうちの一方又は両方を有する複数のシム・コンポーネントを含む、請求項17に記載のシステム。
- 前記少なくとも1つのシム・コンポーネントが第1シムと第2シムとを含み、該シムのそれぞれが、前記脛骨補綴物が膝関節内に挿入された後で前記支承コンポーネントの下面と前記ベース・コンポーネントの上面との間で摺動可能である、請求項17又は18に記載のシステム。
- 前記ユーザーインターフェイスが、前記ディスプレイ上の提示のための複数のセンサ・アイコンを含み、該センサ・アイコンが、前記支承コンポーネント内部の前記センサの相対位置に対応する、該ディスプレイ上の位置を有している、請求項17又は18に記載のシステム。
- 前記ユーザーインターフェイスと前記複数のセンサとの間の接続部が、前記複数のセンサから力又は圧力データを受信して処理するように形成されたプロセッサと、該プロセッサから該力又は圧力データを受信するコンピュータ・デバイスとを含み、前記プロセッサと該コンピュータ・デバイスとが無線接続される、請求項17又は18に記載のシステム。
