Abstract
【解決手段】鬱血性心不全及び関連する心臓病態に罹患している患者に移植されるように適合した直接心臓接触デバイスであって、心室補助、心室サポート及び拡張期リコイルを提供する、又は心室サポート及び拡張期リコイルのみを提供する手段を有する心臓デバイスである。

Description
すなわち、本発明は、
(1)鬱血性心不全及び関連する心臓病態に罹患している患者に移植されるように適合された直接心臓接触デバイスであって、心室補助、心室サポート及び拡張期リコイルを提供する、又は心室サポート及び拡張期リコイルのみを提供する手段を有する心臓デバイス、
(2)相転移点(標的拡張終期容積(TEDV,target end diastolic volume))を判定する手段をさらに含む、上記(1)に記載のデバイス、
(3)相転移点が調整可能である、上記(2)に記載のデバイス、
(4)動作の二相モードを達成する手段をさらに含む、上記(1)に記載のデバイス、
(5)心臓容積がTEDV未満の場合は、デバイスが充満増強相で充満を増強し、心臓容積TEDVを超える場合は、デバイスが充満妨害相で充満を妨害する、上記(1)に記載のデバイス、
(6)動的調整可能サポートの手段をさらに含む、上記(1)に記載のデバイス、
(7)二相コンポーネントの充満妨害が、全治療サイクルを通じて受動的サポートを調整するのに使用される、上記(1)に記載のデバイス、
(8)心臓の心外膜面に連続してサポートを加え、これにより逆リモデリングを促す調整可能な受動的サポートコンポーネントをさらに含む、上記(1)に記載のデバイス、
(9)サポートを調整するステップをさらに含み、罹患した心臓がより小さくなることでサポートに応答し始めるにつれて、TEDVが初期治療介入と同じ量のサポートを提供するように調整され得る、上記(1)に記載のデバイス、
(10)拡張期リコイル増強をさらに含む、上記(1)に記載のデバイス、
(11)拡張期リコイル増強が、心室充満を促す陰圧を発生させて心臓圧力がTEDVより低い場合に利用される弾性記憶コンポーネントを含み、心臓圧力がTEDVを超える場合、デバイスが充満及び心臓容積を抑制するように働く、上記(10)に記載のデバイス、
(12)縫合又は直接連結方法なしで、内因的空気取り付けにより心臓周囲に配置されたままになるように適合された拡張期リコイルデバイスを含む、上記(1)に記載のデバイス、
(13)流体で充満される1又は2以上の封入物又はブラダの上に生体適合フィルムが形成された、上記(1)に記載のデバイス、
(14)1又は2以上の弾性エネルギー貯蔵要素を含む、上記(1)に記載のデバイス、
(15)弾性エネルギー貯蔵要素が、形状記憶合金又はポリマーでできているフレーム又はメッシュである、上記(1)に記載のデバイス、
(16)ポートに結合された1又は2以上の流体充満ブラダをさらに含み、前記ブラダ中の流体容積が移植後に変えられ得る、上記(1)に記載のデバイス、
(17)損傷又は罹患した心臓に対して調整可能な受動的サポート、能動的補助、又は能動的補助及び調整可能な受動的サポートの併用を提供するように設計されたコンポーネントをさらに含む、上記(1)に記載のデバイス、
(18)運動障害性及び/又は運動低下性の運動を減少させる罹患又は損傷した心臓での攣縮歪みパターンを促す拡張期リコイルデバイスを含む、上記(1)に記載のデバイス、
(19)心臓が攣縮されると心臓にねじり運動を与え、次いで心臓がリコイルするとねじれを戻す拡張期リコイルデバイスを含む、上記(1)に記載のデバイス、
(20)心臓の湾曲を反転させる又は著しく乱すことなく攣縮時に心臓を圧迫する直接心臓接触を提供して、罹患又は損傷した心臓を補助する拡張期リコイルデバイスを含む、上記(1)に記載のデバイス、
(21)心臓の表面にさまざまな又は均一な圧力を加えて心臓の収縮終期構造を変化させる拡張期リコイルデバイスを含む、上記(1)に記載のデバイス、
(22)心臓の表面にさまざまな又は均一な圧力を加えて心臓の拡張終期構造を変化させる拡張期リコイルデバイスを含む、上記(1)に記載のデバイス、
(23)心臓の収縮終期及び拡張終期構造を別々に調節する拡張期リコイルデバイスを含む、上記(1)に記載のデバイス、
(24)心臓の自然な拍動に同期してさまざまな又は均一な圧力を加える拡張期リコイルデバイスを含む、上記(1)に記載のデバイス、
(25)心臓の拍動をペーシングし、ペーシングに同期してさまざまな又は均一な圧力を加える拡張期リコイルデバイスを含む、上記(1)に記載のデバイス、
(26)右心室流入路を塞いで右心室充満を調節する拡張期リコイルデバイスを含む、上記(1)に記載のデバイス、
(27)左胸部小切開(mini left thoracic incision)により低侵襲移植される拡張期リコイルデバイスを含む、上記(1)に記載のデバイス、
(28)拡張期リコイルデバイスが生体適合材料でできている、上記(1)に記載のデバイス、
(29)損傷又は罹患した心臓の機能を改善するための1又は2以上の薬物療法、幹細胞、又は他の心臓補助技術をさらに含む、上記(1)に記載のデバイス、
(30)温度、圧力、EKG信号、及び伝導性の1又は2以上をモニタリングすることができるデバイス内に組み込まれたセンサーをさらに含む、上記(1)に記載のデバイス、
(31)蛍光分光法を含む、分光分析の目的のため心臓環境への光アクセスを提供することができるデバイス内に埋込された光ファイバーをさらに含む、上記(1)に記載のデバイス、
(32)埋込されたセンサーから得た1又は2以上の感覚信号の結果に基づきデバイスの作動を制御する手段を含む、上記(31)に記載のデバイス、
(33)心臓に近接した分光分析により検出されたバイオマーカーの分析に基づきデバイスの作動を制御する手段を含む、上記(1)に記載のデバイス、
(34)デバイスのTEDVをモニタリングすること、マイクロプロセッサの入力に対して前記モニタリングを提供すること、及び前記マイクロプロセッサの出力による前記デバイスの制御を提供することを含む、上記(1)に記載のデバイス、
(35)相転移点(標的拡張終期容積(TEDV))を判定するステップと、
調整可能な相転移点(TEDV)に関する二相モードにおいて動作するステップと
を含む、直接心臓接触心室補助、心室サポート及び拡張期リコイルの使用方法、
(36)心臓容積がTEDV未満の場合に充満増強相で充満を増強するステップと、
心臓容積がTEDVを超える場合に充満を妨害するステップ(すなわち、「充満妨害」相)と
をさらに含む、上記(35)に記載の方法、
(37)二相性コンポーネントの充満妨害中に動的調整可能サポートを使用するステップを含む、上記(35)に記載の方法、
(38)心臓の心外膜面に連続してサポートを加えて、逆リモデリングを促すステップを含む、上記(35)に記載の方法、
(39)罹患した心臓がより小さくなることでサポートに応答し始めるにつれて、初期治療介入と同じ量のサポートを提供するようにTEDVを調整する、上記(35)に記載の方法、
(40)心臓圧力がTEDVより低い場合、心室充満を促す陰圧を発生させて弾性記憶コンポーネントが利用される場合に二相コンポーネントの充満増強が拡張期リコイルを増強し、心臓圧力がTEDVを超える場合、充満及び心臓容積が抑制される、上記(38)に記載の方法、
(41)心臓の心外膜面に接着するための第1の生体適合フィルムと、
胸腔に接着するための第2の生体適合フィルムと、
第1の生体適合フィルム及び第2の生体適合フィルムを隔てて心臓の心外膜面と胸壁の間の癒着を回避する1又は2以上の流体充満ブラダと、
前記第1の生体適合フィルム、前記第2の生体適合フィルム又は両方に接触して心臓の攣縮時に弾性エネルギーを貯蔵し、心臓の充満期にエネルギーを放出する1若しくは2以上の構造要素
とを含む、心臓の拡張期リコイルを改善し、術後の心膜癒着を減少させるための直接心臓接触拡張期リコイルデバイス、
に関する。
