Abstract

本開示は、2011年10月20日に出願された米国仮特許出願第61/549,681号及び2012年2月2日に出願された米国仮特許出願第61/594,304号のパリ条約による優先権及び利益を主張する。これらの出願は、その全体が本明細書に完全に記載されているかのように、参照により本明細書に援用される。
免疫刺激量とは、これに限定されるものではないが、ELISPOTアッセイの結果を測定可能な量まで増加させる量、ICSアッセイの結果を測定可能な量まで増加させる量、テトラマーアッセイの結果を測定可能な量まで増加させる量、抗原特異的CD4+T細胞の血集団を測定可能な量まで増加させる量、抗原特異的CD8+T細胞の血集団を測定可能な量まで増加させる量であるか、又は適切な対照と比較した時に、増加量が少なくとも10%、20%、30%、40%、50%、60%、70%、80%、90%、100%、1,5倍、2.0倍、3.0倍等である量とすることができる。適切な対照とは、対照ワクチンであり、樹状細胞がメラノーマ細胞をロードしていないか、又はメラノーマ細胞由来のペプチドをロードしていない対照ワクチンとすることができる。

本開示の樹状細胞ワクチンは、皮内経路、結節内経路、粘膜経路、皮下経路、又はこれらの任意の組み合わせにより投与することができる。1回の投与量は、約10×103樹状細胞、20×103細胞、50×103細胞、100×103細胞、200×103細胞、500×103細胞、1×106細胞、2×106細胞、20×106細胞、50×106細胞、100×106細胞、200×106細胞、500×106細胞、1×109細胞、2×109細胞、5×109細胞、10×109細胞等を含むことができる。投与回数は、例えば、週に1回、週に2回、2週間に1回、3週間に1回、4週間に1回、月に1回、2ヶ月に1回、3ヶ月に1回、4ヶ月に1回、5ヶ月に1回、6ヶ月に1回等とすることができる。投与を行う日の合計日数は、1日、2日、又は3、4、5、6、7、8、9、10、11、12、13、14、15、16、17、18、19、もしくは20日等とすることができる。当然のことながら、どの投与においても同じ日に2回以上の注射を含む可能性がある。一つの態様において、本開示は、腫瘍細胞全体を樹状細胞にロードすることを含み、樹状細胞にロードされたメラノーマ細胞由来タンパク質のうちの少なくとも10%、少なくとも20%、少なくとも30%、少なくとも40%、少なくとも50%、少なくとも60%、少なくとも70%、少なくとも80%、少なくとも90%、少なくとも95%、又は少なくとも99%が、腫瘍細胞全体に帰する。限定されない実施形態において、樹状細胞ワクチンは、フラスコ、バイアル、ボトル、シリンジ、カテーテル、カニューレ等に収められる。投与に関して、投与される樹状細胞の少なくとも20%、少なくとも30%、少なくとも40%、少なくとも50%、少なくとも60%、少なくとも70%、少なくとも80%、少なくとも90%、少なくとも95%、又は少なくとも99%が、成熟樹状細胞である。
実施形態において、本開示は、in vitroでのロードに由来するメラノーマペプチドを含有する樹状細胞を含むワクチンであって、ワクチンが、樹状細胞(メラノーマペプチドを含有するDC及びメラノーマペプチドを含有しないDCの集合体)を含み、樹状細胞/メラノーマ細胞の比率が、少なくとも5/95、10/90、20/80、30/70、40/60、50/50、60/40、70/30、80/20、90/10、95/5、98/299/1等であるワクチンを提供する。また、in vitroでのロードに由来するメラノーマペプチドを含有する樹状細胞を含むワクチンであって、ワクチンが、樹状細胞(メラノーマペプチドを含有するDC及びメラノーマペプチドを含有しないDCの集合体)を含み、[樹状細胞]/[DCでもメラノーマでもない細胞]の比率が、少なくとも5/95、10/90、20/80、30/70、40/60、50/50、60/40、70/30、80/20、90/10、95/5、98/2、及び99/1等であるワクチンを提供する。本開示は、第1の区画にメラノーマ細胞を含み、第2の区画に樹状細胞を含む、区画された容器を提供する。2つの区画は、膜、フィルター、バルブ、導管、連結器により隔てることができ、これによりメラノーマ細胞が樹状細胞に接触するのを防ぐが、手動による移送、又は膜の除去もしくはバルブの開放によりメラノーマ細胞が樹状細胞に接触できるようにし、メラノーマ細胞、メラノーマ細胞片、及び/又はメラノーマペプチドを樹状細胞にロードできるようにする。

本開示は、癌細胞を、例えば、照射又は核酸架橋剤により不活性化させる、組成物及び方法を提供する。一例として、架橋剤は、DNAを架橋するが、タンパク質は未修飾のままにすることができる。核酸アルキル化剤は、β−アラニン、N−(アクリジン−9−イル)、2−[ビス(2−クロロエチル)アミノ]エチルエステルであってもよい。幾つかの実施形態において、核酸標的化化合物は、UVA照射により活性化されたソラレン化合物である。例えば、核酸標的化合物は、4’−(4−アミノ−2−オキサ)ブチル−4,5’,8−トリメチルソラレン(本明細書中で「S−59」とも称する)であってもよい。細胞は、150マイクロモーラーのソラレンS−59及び3J/cm2のUVA光線(FX1019照射装置、バクスター・フェンバル社、ラウンドレーク、イリノイ州)で不活性化することができる。S−59での不活性化は光化学処理と呼ばれ、細胞を完全に不活性化する。様々な濃度の核酸架橋剤について、細胞の不活性化における有効性、例えば、細胞分裂の妨害における有効性を検証してもよい。S−59は、核酸を架橋するが、無傷タンパク質を未修飾のままにするという機能により、識別することができる。細胞は、0、1、10、100、及び1000nMのソラレンS−59を含有する5mLの生理食塩水中で浮遊させることができる。各サンプルは、以下のように照射することができる。S−59は、100nMの濃度で添加することができる。サンプルには、約2J/cm2の放射線量でUVA照射することができる(FX1019照射装置、バクスター・フェンバル、ラウンドレーク、イリノイ州)。次に、各サンプルを15mLチューブに移し、遠心分離し、上清を除去し、その後、5mLの生理食塩水で洗浄し、遠心分離し、上清を除去し、最終ペレットを0.5mLの生理食塩水に懸濁した(Dubenskyの米国特許7,833,775号明細書及びDubenskyの米国特許7,691,393号明細書)。
メラノーマ細胞集団は、例えば、非アポトーシス性自食性細胞を非自食性及びアポトーシス性である細胞から分離する手法を用いることにより、非アポトーシス性であるメラノーマ細胞を富化することができる。分離は、自食性非アポトーシス性細胞が表面に接着することにより行われ、他の細胞は浮遊している。また、非アポトーシス性メラノーマ細胞が富化された集団は、ホスファチジルセリンに特異的な抗体を用いてアポトーシス性細胞を除去することにより得ることができる。固定化抗体により細胞を除去する手法が利用できる(Onodera (1998) Ther. Apher. 2:37−42)。ホスファチジルセリンに特異的な抗体は市販されている(例えば、EDミリポア社、ビレリカ、マサチューセッツ州)。また、メラノーマ細胞のバルク集団は、蛍光抗ホスファチジルセリン抗体で標識することができる。標識されたアポトーシス性メラノーマ細胞は、フローサイトメトリー、アフィニティークロマトグラフィー、免疫磁気分離法で除去される(例えば、Hoeppener (2012) Recent Results Cancer Res. 195:43−58; Dainiak (2007) Adv. Biochem. Eng. Biotechnol. 106:1−18を参照)。
アポトーシス阻害剤Z−VAD(Z−VAD−fmk)は、例えば、エンゾライフサイエンス社(エクセター、英国)、R&Dシステムズ社(ミネアポリス、ミネソタ州)、トクリスバイオサイエンス社(ブリストル、英国)、バイオモル社(プリマスミーティング、ペンシルベニア州)、及びEMDケミカルズ社(ギブスタウン、ニュージャージー州)から購入することができる。Z−VAD−fmkは、合成ペプチドのZ−Val−Ala−Asp(OMe)−CH2Fである。カスパーゼは、プロテアーゼファミリーのシステイン−アスパラギン酸特異的メンバーである。カスパーゼは、TRAIL、FAS等の死滅受容体ライゲーション、DNA損傷、ストレス、血清飢餓、及び細胞の種類によってはインターフェロンにより活性化される。カスパーゼは、核フラグメンテーション、クロマチン凝縮、細胞質の完全性の喪失を含めた、高度に調節されたアポトーシスの過程において、重要な役割を果たす。汎カスパーゼ阻害剤Z−VAD−fmk(カルボベンゾキシ−バリル−アラニル−アスパルチル−[O−メチル]−フルオロメチルケトン)は、カスパーゼプロテアーゼの触媒部位に不可逆的に結合し、アポトーシス誘導におけるカスパーゼプロテアーゼの機能を阻害する。IFN−γに応答してアポトーシスを行う細胞の能力を阻害することは、洗浄により浮遊アポトーシス性細胞を除去する選択工程を経ずに、非アポトーシス性であるが自食性である細胞を生成する手段となり得る。
樹状細胞は、フィコール分離産物のプラスチック接着法(Choi, et al. (1998) Clin. Cancer Res. 4:2709−2716; Luft, et al. (1998) Exp. Hematol.26:489−500; Cornforth, et al. (2011) Cancer Immunol. Immunother. 60:123−131)により、抗生物質無添加のAIM−V培地(インビトロジェン社、グランドアイランド、ニューヨーク州)中で、IL−4(セルジェニックス社、フレイスバーグ、ドイツ)及びGM−CSF(バーレックス社、シアトル、ワシントン州)(DC培地)をそれぞれ1,000IU/mLずつ加えて生成した。次に、IFN−γ処理照射自己腫瘍細胞をロードする前に、フラスコを6日間培養した。
純粋腫瘍細胞は、Cornforthらに従って生成した(Cornforth, et al. (2011) Cancer Immunol. Immunother. 60:123−131; Dillman et al. (1993) J. Immunother. Emphasis Tumor Immunol. 14:65−69; Dillman et al. (2000) Cancer Biother. Radiopharm. 15:161−168)。次に、腫瘍細胞は、1,000U/mLのインターフェロン−γ(インターミューン社、ブリスベン、カリフォルニア州)と共に72時間インキュベートし、セシウム源から100Gy照射し、凍結保存した(Selvan, et al. (2007) Int. J. 限定を示唆するものではないが、本開示のメラノーマ細胞は、Mage、Mart−1、Mel−5、HMB45、S100、又はチロシナーゼのうちの1つ又は複数を発現する(Dillman et al. (2011) Cancer Biotherapy Radiopharmaceuticals 26:407−415)。1つの態様において、腫瘍抗原の検出にIFN−γに暴露していない細胞を使用する一方、別の態様においては、腫瘍抗原の検出をIFN−γで処理した細胞で行う(例えば、Cornforth et al. (2011) Cancer Biotherapy Radiopharmaceuticals 26:345−351を参照)。その全体が参照により本明細書に援用されるDubenskyらによる米国特許出願公開第2007/0207171号明細書に開示されているように、1つ又は複数のメラノーマ抗原又は1つ又は複数の単離したメラノーマ抗原を含む組成物を発現するメラノーマ細胞を包含する。
アポトーシスは、ヨウ化プロピジウム及び7−アミノアクチノマイシンD(7−AAD)等の色素で染色すること、ミトコンドリア内膜電位の低下を測定すること、カスパーゼの活性化又は切断を測定することにより、多くの試薬、例えば、蛍光色素標識アネキシンで検出又は測定することができる。例えば、George, et al. (2004) Cytometry Part A. 59A:237−245を参照。アポトーシスの初期事象は、細胞膜の外面においてホスファチジルセリンへ暴露することであり、蛍光色素標識アネキシンで検出することができる。利用可能な方法により、生細胞、壊死細胞、初期アポトーシス細胞、及び後期アポトーシス細胞を区別することができる。本開示では、7−AADアッセイによりアポトーシスではないとされるメラノーマ細胞、アネキシンVアッセイによりアポトーシスではないとされるメラノーマ細胞、IFN−γ処理後のアポトーシス用アッセイによりアポトーシスではないとされるメラノーマ細胞(Dillman et al. (2011) Cancer Biotherapy Radiopharmaceuticals 26:407−415)、又はBcl−2、カスパーゼ−3、P53、もしくはスルビビンのうちの1つ又は複数のバイオマーカーによりアポトーシスではないとされるメラノーマ細胞(Karam, et al. (2007) Lancet Oncol. 8:128−136)を使用する。本開示の医薬組成物、試薬、及び関連方法は、例えば、参照により本明細書に援用されるHerlynらの米国特許7,544,465号明細書、Thorpeらの米国特許7,714,109号明細書に従ってアポトーシスが判断される、アポトーシス性のIFN−γ処理メラノーマ細胞を除外する。
自食作用は、多くのタンパク質及びいくつかの細胞小器官の分解に用いられる自然発生的な過程である。自食作用は、タンパク質及び細胞小器官の代謝回転、飢餓応答、細胞分化、細胞死等を媒介する。微小管結合タンパク質軽鎖3(LC3)は、自食作用をモニターするものである。1つの手法において、自食作用は、LC3−IからLC3−IIへの変換を含むLC3の変換を測定することにより検出できる。LC3−IIの量は、自食胞の数と相関する。詳細には、LC3は、細胞質性及び可溶性であるが、LC3−IIは膜上に存在する。LC3−IIは、脂質と結合するため、分子量が大きい。LC3プロセッシングは、例えば、ウェスタンブロットにより測定することができ、一方、自食作用、自食性小胞、及び自食胞は、顕微鏡検査で測定することができる。自食作用は、例えば、プロセッシング後のLC3−IIの検出、初期の自食性画分と後期の自食性画分との比率、又は自食体積により定量することができる。(Mizushima and Yoshimori (2007)Autophagy 3:542−546:634−641、Tanida et al. IFN−γ(II型インターフェロン)のシグナル伝達は、多くの遺伝子、例えば、IFN−γ受容体、STAT1、STAT2、STAT1ホモ二量体、STAT1/STAT2ヘテロ二量体、IRF−1、GAS、及びIRF−Eの発現に依存する。研究により、IFN−γのシグナル伝達は、IFN−γ受容体(IFNGR1鎖、IFNGR2鎖)に依存していることが示されている。細胞表面におけるIFNGRの低発現は、IFN−γのシグナル伝達のいくつかの側面を阻害することができる(Schroder, et al. (2004) J. Leukocyte boil. 75:163−189)。1つの態様において、本開示は、IFNGRの表面発現が低い癌細胞の使用を除外する。別の態様において、本開示は、STAT1ホモ二量体を発現する癌細胞をスクリーニングし、これらの細胞を使用し、STAT1ホモ二量体を発現しない細胞を実質的に除外する。さらに別の態様において、本開示は、STAT1のリン酸化(セリン−727)が見られる細胞のスクリーニングを検討する。また、STAT1遺伝子に機能喪失突然変異を有する患者からの癌細胞の除外を検討する(例えば、Dupuis, et al. 本開示は、IFN−γに応答するメラノーマ細胞又は前腫瘍性メラノーマ細胞の使用を要する生体物質、組成物、試薬、及び方法を含む。メラノーマ細胞は、1つ又は複数のIFN−γ応答遺伝子の発現のためのアッセイにより、特定、区別、及び選択することができる。多くのIFN−γ応答遺伝子が、特定されている(例えば、Halonen, et al. (2006) J. Neuroimmunol. 175:19−30、MacMicking (2004) 11:601−609、 Boehm, et al. MHCクラスIIの発現は、例えば、MHCクラスII遺伝子産物に特異的な抗体又は核酸プローブを用いて測定することができる。このMHCクラスII遺伝子産物としては、HLA−DPA1、HLA−DPB1、HLA−DQA1、HLA−DQB1、HLA−DRA、HLA−DRB1、並びにHLA−DM及びHLA−DOが挙げられる(例えば、Apostolopoulos, et al. (2008) Human Vaccines 4:400−409を参照)。
IFN−γ処理後の自食作用の誘導は、主要組織適合遺伝子クラスII複合体の発現増加により測定されるため、IFN−γ全身処理に対する応答を判断するのに用いることができる。生検メラノーマ腫瘍細胞が、培養液中でIFN−γに曝された際に、自食作用を行うがアポトーシスは行わない場合、このことは、これらの患者が、IFN−γ全身処理に対して好ましく応答するであろうことを示す。また、生検からの細胞株の確立に成功すれば、患者も、自食性非アポトーシス性接着性細胞集団由来のIFN−γ処理精製腫瘍細胞株から調製した細胞治療薬の恩恵を受けるであろう。
メラノーマの研究により、アポトーシス性細胞の存在と臨床試験における低生存率との相関関係が実証された(Cornforth, et al. (2011) CancerImmunol. Immunother. 60:123−131; Dillman, et al. (2011) Cancer Biother. Radiopharmaceuticals 26:407−415)。以下の研究では、in vitroでメラノーマ腫瘍細胞をIFN−γ処理した後の自食作用、アポトーシス、及びMHCクラスII分子の誘導について調べた。
自己樹状細胞の生成
樹状細胞は、前述の通り、プラスチック接着法により生成した(Choi, (1998) Clin. Cancer Res. 4:2709−2716; Luft, (1998) Exp. Hematol.26:489−500)。簡単には、自己アフェレーシス産物を、フィコール・ハイパック(GEヘルスケア社、バッキンガムシャー、英国)密度勾配分離にかけた。得られた末梢血単核細胞は、細胞培養フラスコ(コーニング・コースター社、カミング、ニューヨーク州)において、15×106細胞/mLの割合で、IL−4(セルジェニックス社、フレイスバーグ、ドイツ)及びGM−CSF(バーレックス社、シアトル、ワシントン州)(DC培地)をそれぞれ1,000IU/mLずつ加えた抗生物質無添加のAIM−V培地(インビトロジェン社、グランドアイランド、ニューヨーク州)に撒いた。1時間インキュベートした後、非接着性集団を廃棄し、新たなDC培地をフラスコに加えた。翌朝、非接着性細胞を廃棄し、フラスコを常温のPBSで1回洗浄し、新たなDC培地を加えた。次に、フラスコを6日間培養し、その時にこの手法で生成したDC(ロード前DC)の割合及び表現型を判定するためにフローサイトメトリー評価を行った。
前述の通り生成し、特徴付けられた純粋腫瘍細胞を、2億個まで増やした後、15%FBS/ECSのRPMI(完全培地)中で1000IU/mLのIFN−γ(インターミューン社、ブリスベン、カリフォルニア州)と共に72時間インキュベートし、セシウム源から100Gy照射し、前述のように凍結保存した(Choi, (1998) Clin. Cancer Res. 4:2709−2716; Luft, (1998) Exp. Hematol.26:489−500; Dillman (1993) J. Immunother. Emphasis Tumor Immunol. 14:65−69)。IFN−γ処理照射腫瘍細胞は、凍結保存から起こし、PBSで3回洗浄した後、in vitroで培養したDCに加え、〜24時間インキュベートした。抗原ロードDCは、ラバー・ポリスマンで穏やかに剥がして採取し、同量ずつ9−11等分にして凍結保存した。ロード後DC細胞を表すフローサイトメトリー評価のため、1等分量の細胞を得た。
樹状細胞集団の表現型の特徴付けは、BDファーミンゲン社(サンディエゴ、カリフォルニア州)から購入した表面マーカーに対するモノクローナル抗体:PerCpと結合する抗MHCクラスII抗体、APCと結合する抗CD11c抗体、PEと結合する抗CD80抗体、抗CD83抗体、抗CD86抗体、を用いて行った。対照アイソタイプを用いて陽性細胞の割合を判定した。腫瘍細胞のフローサイトメトリーは、BDファーミンゲン社から購入した、FITCと結合するMHCクラスI及びIIに対する抗体、アネキシン−V−PE、及び7−アミノアクチノマイシンD(7−ADD)を用いて行った。CaliBRITEフローサイトメトリー校正(BDファーミンゲン社)を各稼動の前に使用し、フローサイトメトリーデータの収集の間中、同じ機器設定を使用した。
細胞質の細胞溶解物は、哺乳類タンパク質抽出試薬(Mammalian Protein Extraction Reagent)(サーモサイエンティフィック社、ロックフォード、イリノイ州)とプロテアーゼ阻害剤混合物(ロシュ社、インディアナポリス、インディアナ州)を用いて、10,000細胞/uLの割合で、氷上で調製した。約25uLs/レーンの細胞溶解物を12.5%トリス−グリシンゲル上で分離し、PVDF膜に転写し、以下に対する抗体でプローブした:カルレティキュリン(MBL社、ウバーン、マサチューセッツ州)、Hsp−60、Hsp−70、Hsp−90(R&Dシステムズ社、ミネアポリス、ミネソタ州)、HMBG−1(セルシグナリング社、ダンバース、マサチューセッツ州)、ICAM−1(サンタクルーズバイオテック社、サンタクルーズ、カリフォルニア州)、Mel−4、Mart−1(シグネット社、エメリービル、カリフォルニア州)、チロシナーゼ(アップステート社、レイクプラシッド、ニューヨーク州)、及びGADPH(カルバイオケム社、ダルムシュタット、ドイツ)。
メラノーマ株によるパネル抗原の発現は、免疫細胞化学的な手順を用いて測定した。細胞は、8穴チャンバースライド(サーモフィッシャー社、ロチェスター、ニューヨーク州)中で、1000IU/mLのIFN−γの存在下、又は非存在下で培養した。72時間後、細胞は、1×リン酸緩衝生理食塩水(PBS)で3回洗浄し、冷アセトン中に固定した。内因性ペルオキシダーゼを阻害した後、細胞は、列挙した抗原に対する適切な一次抗体と共にインキュベートした。免疫組織化学は、ビオチン化アンチマウス又はラビット免疫グロブリン、超高感度ストレプトアビジン標識酵素、ホースラディッシュペルオキシダーゼ色素原、及び基質キット(バイオジェネックス社、サンラモン、カリフォルニア州)を用いて行った。以下の抗ヒトポリクローナル又はモノクローナル抗体の反応性は、アイソタイプ適合対照抗体:S−100及びHMB−45(バイオジェネックス社、サンラモン、カリフォルニア州)、Mel−2、Mel−5、Mart−1(シグネット社、デッダム、マサチューセッツ州)、チロシナーゼ及びMage−1(サーモサイエンティフィック社、フリーモント、カリフォルニア州)、Melan−A、HLAクラスI及びHLAクラスII(ダコ社、デンマーク)を用いて調べた。
等分散の2標本を対象とする両側スチューデントt検定。有意差は、p値≦0.05で判定した。
細胞死は、自己メラノーマ腫瘍細胞株において、完全培地中でのIFN−γを用いた72時間のインキュベーションに応答して特異的に誘導された。トリパンブルー染色除外分析をIFN−γ処理又は未処理細胞に対して行ったところ、IFN−γ処理細胞の生存率の方が低くなるという有意な傾向が明らかになった(89.1±6.8%対84.9±9.3%、p=0.014、N=47)。アポトーシス誘導に関するフローサイトメトリーによる4つの自己メラノーマ細胞株の標本分析(図1A)から、メラノーマ細胞が、IFN−γ誘導アポトーシスの影響に対して特異的に感受性があり、いくつかの細胞はより後期のアポトーシスを示すか又は「死亡」した集団(7−AAD+/アネキシン−V+)であり、その他は初期アポトーシスの兆候示すか又は「瀕死」の集団(7−AAD-/アネキシン−V+)であることが明らかになった。結果としてIFN−γ処理後にアポトーシス細胞が存在することは、無増悪生存率又は全生存率における有意な減少と関連した(Cornforth (2010) Cancer Immunol. Immunother. Resistance to the proapoptotic effects of interferon−gamma on melanoma cells used in patient−specific dendritic cell immunotherapy is associated with improved overall survival(患者特異的樹状細胞免疫療法で使用したメラノーマ細胞に対するインターフェロン−γによるアポトーシス促進効果への耐性は、全生存率の改善に関連する))。ログ・ランク検定により、メラノーマ腫瘍細胞のIFN−γ処理による低い生存率と研究対象の患者の全生存率とが有意に関連することが明らかになった。
抗腫瘍免疫と関連した抗原ロード、成熟化、及び投与のプロトコール、並びに樹状細胞(DC)系免疫療法の指導は、当業者により実行される。この種の治療は、抗原源としての精製自己腫瘍細胞の使用を包含し、腫瘍関連抗原の患者特異的レパートリーを含む(Selvan (2010) Melanoma Res. 20:280−292; Dillman (2007) Cancer Biother. Radiopharm. 22:309−321)。臨床試験の中には、未精製の自己巨大腫瘍を使用しているものがある。この抗原源には、線維芽細胞及び壊死組織が混入している可能性がある(O’Rourke (2007) Melanoma Res. 17:316−322)。抗原と関連した腫瘍幹細胞が、精製細胞株中に存在している可能性がある(Dillman (2006) New Engl. J. Med. 355:1179−1181)。IFN−γ処理により、MHCクラスII分子の発現が増加する。MHCクラスII分子は、樹状細胞系療法への応答に重要である。取り込まれた物質中に存在する分子、例えば、カルレティキュリン、HMGB−1、及び熱ショックタンパク質等は、成熟シグナルに関与している可能性があり、この関与は、サイトカイン混合物による関与に加わる可能性がある。本開示のDCの調製では、成熟化の傾向が見られ、後期アポトーシス細胞の取り込みに関連する可能性がある(lp (2004) J. Immunol. 173:189−196)。アポトーシス細胞の使用は、腫瘍細胞溶解物又は壊死細胞をロードした樹状細胞と比べてリンパ球のIFN−γ分泌をより効果的に刺激した樹状細胞の生成と相関しており、アポトーシス細胞をロードした樹状細胞は、in vivoでより効果が高い可能性がある。IFN−γによるアポトーシス促進効果への耐性は、より良好な臨床転帰がもたらされる可能性がある(Cornforth (2010) Cancer Immunol. Immunother. 60:123−131)。成熟DCによるインターロイキン−12(IL−12)の分泌は、強力な細胞傷害性リンパ球(CTL、Cd+8T細胞)の活性に繋がる可能性がある。ex vivoでの成熟化が腫瘍免疫の持続に繋がるか否かという課題が取り組まれてきた。また、抗原特異的CTLを抑制することができる制御性T細胞を未成熟DCが誘導する危険が、サイトカイン成熟DCと共に生じることが示されている。DC成熟化のプロトコールを改善するために、成熟化に繋がる一連のシグナル伝達事象の再評価が検討されている。よって、本明細書に示す証拠はDCが成熟過程を開始することを示しているため、この研究における照射腫瘍細胞全体の抗原源としての使用は、ex vivoでのサイトカイン成熟化の必要がなく、DC免疫療法のより好ましい方法である可能性がある。注射に際し、これらの「成熟中の」DCは、ライセンシング、抗原特異的CD40L発現CD4+T細胞を引き付けるサイトカインの分泌により成熟過程を完了してもよい。免疫補助剤GM−CSFに応答して樹状細胞で生成されるCCL17/TARCのような血清ケモカインは、無増悪生存率の向上に関連している。状況によっては、樹状細胞によるリンパ球の活性化は、CD80及びCD86のような共刺激分子の発現を要する可能性がある。成熟化のマーカーとして、CD83は、成熟樹状細胞で発現されるが、より強い免疫応答を誘導することができる樹状細胞に対応する可能性がある(Prazma (2008) Immunol. Lett. 115:1−8)。これは、医薬品中の全細胞の一部を表すものである。どの医薬療法においても、成熟DCの数が単独で良好な患者反応と相関する可能性もあれば、相関しない可能性もある。

メラノーマ細胞株
市販のメラノーマ細胞株A375、SK−Mel−5、及びSK−Mel−28を、アメリカ合衆国培養細胞系統保存機関から購入した(カタログ番号:CRL−1619、HTB−70、及びHTB−72)。A375、SK−Mel−5、及びSK−Mel−28は、5%ウシ胎仔血清入りRPMI−1640(インビトロジェン社、カタログ番号:11875−085)中で維持した。汎カスパーゼ阻害剤Z−VAD−fmk及びその対照化合物z−FAD−fmkをBDファーミンゲン社から購入した(カタログ番号:550377及び550411)。GFP−LC3のトランスフェクションをメーカーの使用説明書に従って行い(インビボゲン社、カタログ番号:psetz−gfplc3及びlyec−12)、DP72デジタルカメラを用いてオリンパス社製BX−51顕微鏡に顕微鏡写真を撮影した。腫瘍細胞株は、分析の前に、1000U/mLのIFN−γ(インターミューン社、カタログ番号)と共に72時間インキュベートした。患者特異的細胞株は、説明されているように(Hamai (2008) Cancer Res. 68:9854−9864; Tyring (1984) J. Natl. Cancer Inst.73:1067−1073)、外科的腫瘍サンプルの酵素消化、並びに1mMピルビン酸ナトリウム、1mMグルタミン、及びHEPES緩衝液を加えたウシ胎仔血清及び濃縮子牛血清(オメガ・サイエンティフィック社、サンディエゴ、カリフォルニア州)入りRPMI−1640組織培養培地での培養により生成した。ニコン社製DS−L1デジタル顕微鏡カメラを用いてオリンパス社製CK−2顕微鏡に位相差顕微鏡写真を撮影した。
樹状細胞は、フィコール分離産物のプラスチック接着法(Selvan (2007) Int. J. Cancer 122:1374−1383、Cornforth (2010) Cancer Immuno. 60:123−131)により、抗生物質無添加のAIM−V培地(インビトロジェン社、カタログ番号)中で、IL−4(セルジェニックス社、カタログ番号)及びGM−CSF(バーレックス社、シアトル、ワシントン州)(DC培地)をそれぞれ1,000IU/mLずつ加えて生成した。次に、IFN−γ処理照射自己腫瘍細胞をロードする前に、フラスコを6日間培養した。
IFN−γに応答した腫瘍細胞の細胞死及び主要組織適合遺伝子複合体クラスII発現の変化の分析は、MHCクラスII、アネキシン−V、及び7−アミノ−アクチノマイシンD(7−ADD)に対する抗体を用いて行い、ベクトン−ディケンソン社製FACSカリブル(Calibur)(登録商標)フローサイトメーターに取得した。
メラノーマ腫瘍細胞溶解物は、10−12.5%SDS−PAGEで分離し、ニトロセルロースに転写し、一次抗体で一晩プローブした後、二次抗体の結合を行い、バンドを展開するためのノベックスAP発色基質(インビトロジェン社、カールスバッド、カリフォルニア州)による展開を行った。LC3−B抗体(セルシグナリング・テクノロジーズ社、ボストン、マサチューセッツ州)及びGADPH(EMDバイオサイエンシス社、ドイツ)に対する抗体を、メーカーが推奨するようにそれぞれ1:100及び1:10,000に希釈して使用した。
in vitroでメラノーマ腫瘍細胞をIFN−γ処理した後の自食作用、アポトーシス、及びMHCクラスII分子の誘導について調べた。自己メラノーマ腫瘍細胞株モデルは、自食作用、アポトーシス、及びMHCクラスIIの発現の分析前に、1000IU/mLのIFN−γと共に72時間インキュベートした。自食作用は、LC3IIに対する抗体を用いた免疫ブロット、及びエンゾ社製CytoID(登録商標)オートファジー検出キットを用いたフローサイトメトリーにより検出した。アポトーシス及びMHCクラスIIの誘導は、それぞれ7−AAD及びアネキシン−V染色、並びにMHCクラスIIに対する抗体を用いたフローサイトメトリーによりアッセイした。
結果として、メラノーマ細胞株において、IFN−γにより自食性細胞集団及びアポトーシス性細胞集団の両方が誘導されることが実証された。アポトーシス性集団は、主に非接着性集団で見られたが、自食性細胞は、フラスコに接着したままであった。阻害剤3−メチルアデニン(3−MA)を用いて自食作用を阻害すると、IFN−γへの応答によるMHCクラスII陽性細胞の誘導が阻害される(39.4%IFN−γ対10.0%IFN−γ+3−MA)。汎カスパーゼ阻害剤Z−VADを用いたカスパーゼ活性の阻害により、IFN−γ処理細胞において、アポトーシスは妨害されるが、自食作用は摂動されない(2.75±0.15IFN−γ対3.04±0.27IFN−γ+Z−VAD、倍率変化)。アポトーシスの誘導は、自食作用及びMHCクラスII発現のレベル低下に関連している。インターフェロン−γ処理純粋腫瘍細胞株由来の非アポトーシス性自食性細胞である、自己腫瘍細胞ロード樹状細胞株を投与された患者は、無増悪生存率及び全生存率が改善した(それぞれp=0.003、p=0.002)。IFN−γ処理後、アポトーシス性細胞は死滅させるが、自食性生細胞は維持する手法により、この種の細胞系免疫療法の有効性が高まる可能性がある。
自己増殖性自己複製腫瘍細胞(推定腫瘍幹細胞及び/又は初期前駆細胞)は、転移性癌の新たな持効性製剤の確立に重要であり、ワクチン用の優れた抗原源である可能性がある。これらの研究は、そのような細胞由来の抗原を用いた免疫からの生存に与える影響について検討した。
データは、3つの連続したフェーズII試験から集積した。これらの試験は全て、転移性メラノーマと実証された患者を試験対象とし、患者は、自己腫瘍細胞の細胞培養液由来の抗原を使用したプロトコールで治療を受けた。皮下注射を週1回3週間行った後、月1回5ヶ月間行った。74人の患者が照射腫瘍細胞(TC)を注射され、54人の患者が照射自己腫瘍細胞(NCI−V01−1646)と共培養した自己樹状細胞(DC)を注射された。無作為フェーズII試験では、24人の患者がTCを注射され、18人の患者がDCを注射された。
表2は、各試験の全生存率(OS)を集約している。統合分析において、98人がTC患者、72人がDC患者であった。年齢(51、52)、男性(62%、62%)、治療時に疾患の兆候がないこと(46%、47%)、及び治療時にM1c内臓疾患が存在すること(13%、14%)に関しての特徴は、類似していた。OSは、DCで治療した患者でより長かった(中央値:63.1ヶ月対20.2ヶ月、5年OS:51%対26%、p=0.0002、マンテル・コックス・ログ・ランク検定)。また、無作為試験におけるOSの違いも有意であった(p=0.007)。
Claims (33)
- メラノーマを有する被験体から採取したメラノーマ細胞に由来するメラノーマ特異的ペプチドを含む哺乳類樹状細胞集団であって、
前記メラノーマ特異的ペプチドは、前記メラノーマ細胞から樹状細胞によりin vitroで得られ、
前記メラノーマ細胞は、in vitroでインターフェロン−γ(IFN−γ)又はIFN−γ模倣薬で処理されておらず、
in vitroでIFN−γ又はIFN−γ模倣薬で処理されていない前記メラノーマ細胞の60%超は、自食性及び非アポトーシス性であり、
前記樹状細胞及び前記メラノーマ細胞は、同じ被験体に由来する、
哺乳類樹状細胞集団。 - in vitroでIFN−γ又はIFN−γ模倣薬で処理されていない前記メラノーマ細胞の80%超が、自食性及び非アポトーシス性である、
請求項1に記載の樹状細胞集団。 - 請求項1に記載の樹状細胞集団を含む、請求項1に記載の被験体のためのワクチン。
- IFN−γ又はIFN−γ模倣薬で処理されていない前記メラノーマ細胞の実質的に全てが、細胞分裂することができない、

非自食性及びアポトーシス性であるメラノーマ細胞が除外されることを含み、
前記自食性及び非アポトーシス性であるメラノーマ細胞を1つ又は複数の自己樹状細胞に提供するか、又は前記自食性及び非アポトーシス性であるメラノーマ細胞に由来するペプチドを1つ又は複数の自己樹状細胞に提供する、
調製方法。 - インターフェロン−γ(IFN−γ)で処理されていない、第1の被験体に由来する少なくとも1つのメラノーマ細胞、及び同じ第1の被験体に由来する少なくとも1つの抗原提示細胞(APC)を含み、

製造方法。 - 樹状細胞ワクチンの調製方法であって、
