Abstract
Description
本願は、2011年4月29日付で出願された、米国仮特許出願第61/480,415号の35 U.S.C. §119(e)に基づく優先権の利益を主張するものであり、上記開示は全て参考のため本願明細書中に引用される。
本発明は、パーキンソン病におけるレボドパ誘導性ジスキネジー(levodopa-induced dyskinesia)(LID)、ならびにトゥレット・シンドローム(Tourette's syndrome) に関連したジスキネジー、遅発性ジスキネジー(tardive dyskinesia)およびハンチントン病(Huntington's disease)等の、ジスキネジーの治療方法に関するものである。
パーキンソン病(PD)は、アメリカ合衆国で150万人の人々及び世界中で630万人の人々が患っている2番目に多い神経変性疾患である。PDの発症率は2040年までに2倍に増えると予想される。アメリカ合衆国では、50,000件の新たな症例が1年で診断され、55歳以上の人口の1%が患っている。PDの年間の社会的費用はアメリカ合衆国だけで250億ドルを超える。
LID発症の中核は、グルタミン酸作動性、アデノシン作動性(adenosinergic)、アドレナリン作動性、ドーパミン作動性、セロトニン作動性、内在性カンナビノイド及びオピオイドを介した神経伝達によって調節される神経ネットワークの変化であると考えられ、これらはすべて病気で変化する特性を有する。これらのうち、オピオイドは大脳基底核機能を調節する共伝達物質(co-transmitter)であるため、オピオイドレセプターを介した神経伝達は特に興味深い。この作用を介して、オピオイド薬は、LIDに病原的に(pathogenically)関連するL−DOPA療法を用いたパルス刺激(pulsatile stimulation)のネガティブな効果を鈍化するのを助ける可能性がある。LIDでは、内因性オピオイドレセプターリガンドの前駆体が大量にアップレギュレートされて、プレプロエンケファリンレベルが動物モデルの線条体で増加し、さらに、患者の検視研究でも同様に観察される。加えて、エンケファリン、ダイノルフィン及びα−ネオエンドルフィンが、正常なまたはジスキネジーではないパーキンソン病状態(nondyskinetic Parkinsonian state)ではなく、ジスキネジー状態(dyskinetic state)で顕著に上昇する。したがって、オピオイドレセプター拮抗作用は有益でありうると報告された。しかしながら、基底核回路の複雑さ、シナプス前後(pre- and post-synaptically)双方、および興奮性及び抑制性双方ニューロンでのオピオイドレセプターの存在が、考慮される必要のあるオピオイドレセプターリガンドに対する応答性の複雑さを顕著に生み出している。大脳基底核での異なる分布を有し、異なる機能を有するオピオイドレセプターの主要な関連クラスが3つ存在する。
・μ−オピオイドレセプター選択的アンタゴニストであるシプロダイム(cyprodime)は、ピーク−ドーズLIDを有意に減少させる。しかしながら、選択的μ−オピオイドレセプターアンタゴニストであるADL5510はLIDを減少するが、U−形状の用量反応曲線である。
オピオイドレセプターの活性化は、モルヒネやコデイン等の多数の広範に使用され乱用されたアヘン剤によってなされる。これらの化合物が鎮痛や他の兆候に明らかな有益な効果がありうるにも関わらず、重篤な中毒作用及び鎮静作用を有しうる一方で、アンタゴニストがアヘン剤の患者の禁断症状を起こさせる可能性がある。したがって、LIDに適切な用量はこれらの副作用を考慮して考えられなければならない。μ−アンタゴニストに関連する主要な特異的な副作用は胃腸管及び違和感に関連するものであり、κ−アゴニストでは、例えば、パーキンソニズム及び違和感を悪化させる、鎮静状態である。
本発明は、PD患者に一般的である、ジスキネジー、特にL−DOPAで誘導されるジスキネジー(LID)を治療する治療剤としてのナルブフィンおよび関連化合物の新規な使用に関する。詳しくは、本発明は、LIDの治療におけるナルブフィンの使用を包含する。
a.少なくとも0.01mgの量の、遊離塩基または製薬上許容できる誘導体、プロドラッグ若しくは塩の形態のナルブフィン;および
b.製薬上許容できる担体
を含み、錠剤またはカプセル形態を有する非注射組成物(non-injectable composition)で必要とする患者に治療上有効な量のナルブフィンを投与することを有する。好ましくは、ナルブフィン組成物は経口投与される。好ましくは、成分a.は、少なくとも0.1mgの量で存在する。本発明のさらなる他の実施形態においては、二重作用性のμ−アンタゴニスト/κ−アゴニストは、持続注入を介して患者に投与されるナルブフィンを含む。好ましくは、持続注入量は、少なくとも約0.0001mg/kg/日である。
ナルブフィン−二重のμ−アンタゴニスト/κ−アゴニスト(dual mu-antagonist/kappa-agonist)
ナルブフィン(ヌバイン(Nubain))は、μ−オピオイドレセプターアンタゴニスト及びκ−オピオイドレセプターアゴニスト双方としての活性を有する合成オピオイドである。陣痛の女性のためなどの中程度/激しい痛みのための鎮痛剤として1979年から臨床上使用されていた。これは、安全性の表示である、規制薬物法(Controlled Substances Act)下で規制されないタイプの唯一の麻酔剤であり、この際、主要な副作用は一部の人々では鎮痛用量で軽い鎮静状態があることである。我々の分析によると、ナルブフィンは、鎮痛下(sub-analgesic)で鎮静作用の少ない(non-sedative)用量で有効に使用できる安全窓(safety window)でLIDの有効な薬剤であるのに必要な特性を備えている。それは、二重のμ−オピオイドレセプターアンタゴニスト及びκ−オピオイドレセプターアゴニストとしての活性を示す。それ自体は、これらの2つの活性を組み合わせて一つの安全な治療剤とする機会が与えられる。ゆえに、ナルブフィンにより、投与量、投与形態または特定の患者での病気の段階にかかわらず、関連する脳コンパートメント(brain compartment)での所定の割合のμ−拮抗作用/κ−作動性等の重要な薬理学的な利点が得られる。この独特な特性により、LID患者及び他の関連する病状で重篤な副作用を引き起こることが報告されている過剰なκ作動性に患者を晒す危険を伴うことなくLIDを治療することができるという医学的な利点がある。
本発明を、下記実施例によってより十分に説明するが、下記実施例は本発明を限定するものではない。これらの実施例の修飾は、当業者には明らかであろう。パーキンソン病患者にとって主に必要である、レボドパ誘導性ジスキネジー(Levodopa-induced dyskinesia)の治療を目的とするナルブフィンをターゲットとするために、その活性を臨床前モデルで確認し、それらのデータを用いて、ヒトへの臨床試験のための投与の指針を示す。
霊長類での研究は、ヒトの症状を厳密に反復するのに必要であり、ヒトへの臨床試験前に投薬および有効性を微調整するのに使用される。パーキンソン病の霊長類モデルは、ヒト疾患の運動の兆候(motor manifestations)を反復し、PD治療薬に応答性があり、さらにL−DOPA誘導ジスキネジーを発症するよく確立されたモデルである。マカクザル(Macaca FascicuJaris)を本研究に使用し、これに、中程度から重篤にまでの安定したパーキンソン病の症状が発症するまで、MPTP(1−メチル−4−フェニル−1,2,3,6−テトラヒドロピリジン(1-methyl-4-phenyl-l ,2,3,6-tetrahydropyridine;0.5〜0.8mg/kg)を静脈内(iv)投与する。次に、これらの動物に、安定したジスキネジーが確立されるまで、レボドパ/カルビドパ(carbidopa)(Sinemet 25/100)を1日2回経口投与する。次に、レボドパメチルエステル単独の皮下(SC)注射への動物の応答を評価し、中程度の再現性のあるジスキネジーが得られる最適投与量を各動物で確立する。レボドパメチルエステルを、レボドパの抹消の副作用を低減するためにデカルボキシラーゼ阻害剤である、ベンセラジドと共に定期的に投与する。次に、動物にナルブフィンを、レボドパメチルエステル及びベンセラジドと共に皮下(SC)で投与して、ジスキネジーの重篤度及び期間に関する組み合わせの効果を評価する。最小の有効投与量を見出し、また、将来の臨床試験のための実行可能な投与ストラテジーを決定する目的で、最低限の4回のナルブフィン投与及びベヒクルをランダムの順序で投与して、研究の完全性を確保する。ベヒクルを含む各試験投与を3回繰り返した。L−DOPA投与前におよびその後20分間隔で3〜4時間、サルを評価した。一晩絶食させた後の朝に専門家によって試験を行い、これを薬剤ウァッシュアウト(drug washout)のため少なくとも48時間間隔をあけて繰り返す。通常の臨床評価に加えて、ジスキネジーを評価するためのMPTP処置霊長類用の標準化スケール、クリューバーボードテスト(Kluver board test)(運動課題)、および神経系を評価するスケール(特に注意力)を使用する。上記実施例により、ヒト臨床試験に変換できるナルブフィンの有効投与量が同定される。
マカクに、促進したパーキンソニズム(advanced Parkinsonism)を誘導するためにMPTPを静脈内(iv)投与し、ジスキネジーを誘導するためにL−DOPA/カルビドパ(carbidopa)を長期間経口で処置した。
Claims (28)
- ジスキネジーを治療または軽減する必要のある患者に、前記ジスキネジーを軽減するのに十分な、治療上有効な量の、二重作用性のμ−オピオイドレセプターアンタゴニスト(dual-action mu-opioid receptor antagonist)/κ−オピオイドレセプターアゴニスト(kappa-opioid receptor agonist)を投与することを有する、ジスキネジーの治療または軽減方法。
- 前記二重作用性のμ−アンタゴニスト/κ−アゴニストは、ナルブフィン、ナロルフィン、ペンタゾシン、ブトルファノールおよびこれらの2種以上の組み合わせからなる群より選択される、請求項1に記載の方法。
- 前記二重作用性のμ−アンタゴニスト/κ−アゴニストは、0.001mg/kg〜3mg/kgの投与量で患者に投与される、ナルブフィンを含む、請求項1または2に記載の方法。
- 前記二重作用性のμ−アンタゴニスト/κ−アゴニストは、
a.少なくとも0.01mgの量の、遊離塩基または製薬上許容できる誘導体、プロドラッグ若しくは塩の形態を有するナルブフィンを含む活性成分;および
b.製薬上許容できる担体
を含み、錠剤またはカプセル形態を有する非注射組成物(non-injectable composition)で患者に投与される、ナルブフィンを含む、請求項1〜3のいずれか1項に記載の方法。 - 前記二重作用性のμ−アンタゴニスト/κ−アゴニストは、パーキンソン病に伴うジスキネジー(Parkinson-associated dyskinesia)を治療するための非注射薬剤組成物(non-injectable pharmaceutical composition)で前記患者に投与される、ナルブフィンを含み、前記薬剤組成物は、錠剤、カプセル、シロップ、ゲル、坐剤、皮膚用パッチ剤、吸入可能粉末(inhalable powder)、吸入可能エアロゾル(inhalable aerosol)、舌下スプレーもしくは舌下用固形の剤型、鼻腔内スプレーおよび鼻腔内エアロゾルからなる群より選択される形態を有するまたは薬剤を連続的にデリバリー可能である医療デバイスを介する、請求項1〜4のいずれか1項に記載の方法。
- 前記ジスキネジーは、パーキンソン病と診断された患者において、レボドパ誘導される(levodopa-induced)、請求項1〜5のいずれか1項に記載の方法。
- 前記ジスキネジーは、遅発性ジスキネジー(tardive dyskinesia)と診断された患者における、請求項1〜4のいずれか1項に記載の方法。

- 前記他の抗パーキンソン病薬はCOMT阻害剤である、請求項16に記載の方法。
- 前記他の抗パーキンソン病薬はアマンタジンである、請求項16に記載の方法。
- 前記他の抗パーキンソン病薬は抗コリンアゴニスト(anti-cholinergics)である、請求項16に記載の方法。
- いずれかの薬剤の投与が0〜12時間遅れ、投与が同じまたは異なる経路による、請求項10および16〜24のいずれか1項に記載の方法。
- 前記二重作用性のμ−アンタゴニスト/κ−アゴニストが持続注入により患者に投与されるナルブフィンを含む、請求項1または2に記載の方法。
- 前記連続注入量が少なくとも約0.0001mg/kg/日である、請求項26に記載の方法。
- ナルブフィンは経口で投与される、請求項4または5に記載の方法。
