Abstract
Description
[発明の背景となる技術]
本発明は、内科学、薬理学、ナノ技術および医療工学分野に関する。
[技術水準]
肺高血圧症または勃起不全の特定活性成分治療は、主に強い血管拡張剤の静脈注射投与または経口投与を含む。肺高血圧症は、肉体的な負荷収容能力を著しく制限する深刻かつ生命に脅威的な疾病である。右心機能障害が後続的に表われる肺血管圧および血管抵抗の上昇は、診断を受けた後に治療がない場合、平均的にわずか2.8年間のみ生存できることになるほど、著しく低い平均余命となる。
肺に活性成分を投与する一種の選択的な経路としては、吸入が当業者にとって公知になっており、このような吸入によると、望ましくない全身性の副作用が避けられ得る。活性成分を肺に直接的に投与することは、たとえば、肺高血圧症を治療する際に、プロスタサイクリンアナログイロプロスト(Ventavis(登録商標))について、示されているように、呼吸器疾患の治療を容易にする。しかし、肺活性成分堆積後の相対的に短い薬理作用期間は、吸入療法による頻繁な活性成分投与を要求し、特に、夜間に治療ギャップ(therapeutic gaps)を引き起こす吸入治療法の一つの大きな短所である。
[課題]
本発明の課題は、肺高血圧症または勃起不全のための活性成分を含有し、長期間の、そして制御された状態の活性成分放出を可能にし、そして、人間に投与するのに適した、肺高血圧症または勃起不全を治療するための、噴霧化が可能で、かつ吸入可能な製剤を提供することである。
[課題の解決]
肺高血圧症または勃起不全を治療するための製剤を提供しようとする課題は、本発明により、生体適合性ポリマーおよび安定化剤、そしてホスホジエステラーゼ抑制剤(PDE抑制剤)グループまたはグアニル酸シクラーゼ活性化剤またはグアニル酸シクラーゼ刺激剤またはエンドセリン受容体拮抗剤またはプロスタノイドから選択された活性成分からなる生体適合性ナノポリマー粒子によって解決される。
グアニル酸シクラーゼ活性化剤は、たとえば、シナシグアトおよびアタシグアトである;グアニル酸シクラーゼ刺激剤は、リオシグアト、BAY41−2272、BAY41−8543およびCFM−1571である。エンドセリン受容体拮抗剤は、たとえば、ボセンタン、ジボセンタン、テゾセンタン、マシテンタン、シタキセンタン、アボセンタン、クラゾセンタン、アンブリセンタン、ダルセンタン、アトラセンタン、エンラセンタンである。
本発明に係る生体適合性ナノポリマー粒子の特性化
本発明に係る生体適合性ナノポリマー粒子が10nm〜10μmの幾何学的平均粒径を有することにより、結果的に、前記粒子は優れた噴霧化がなされ得、安定化剤層の厚さは、0〜50nmである。しかし、このとき、前記安定化剤層の厚さは、生体適合性ナノポリマー粒子の幾何学的平均半径よりも大きくない。好ましい一実施例において、生体適合性ナノポリマー粒子は、より長期間の有効成分放出を可能にするために、500nm〜5μmの幾何学的平均粒度を有し、または前記粒子が大食細胞内部へ吸収されることを防止するために、50nm〜250nmの幾何学的平均粒径を有する。
本発明に係る生体適合性ナノポリマー粒子の製造
本発明に係る生体適合性ナノポリマー粒子は、たとえば、エマルション法および、後続する溶媒蒸発法によって製造される。本発明に係る生体適合性ナノポリマー粒子は、生体適合性ポリマー、そして肺高血圧症または勃起不全を治療するための安定化剤および活性成分からなる。前記本発明に係る生体適合性ナノポリマー粒子の生体適合性ポリマーは、たとえば、ポリエステル(PLGA,PLA)、ポリアンハイドライド、ポリオルトエステル、ポリホスホエステル、ポリカーボネート、ポリケタール、ポリ尿素、ポリウレタン、ブロックコポリマー(PEG−PLGA)、星形ポリマーまたは櫛形ポリマーである。本発明によれば、前記安定化剤は、非イオン界面活性剤、陰イオン界面活性剤、両性界面活性剤またはポリマーグループから選択される。本発明に係る生体適合性ナノポリマー粒子の活性成分は、ホスホジエステラーゼ抑制剤(PDE抑制剤)グループまたはグアニル酸シクラーゼ活性化剤またはグアニル酸シクラーゼ刺激剤またはエンドセリン受容体拮抗剤またはプロスタノイドから選択される。
本発明に係る生体適合性ナノポリマー粒子の用途
本発明に係る生体適合性ナノポリマー粒子は、肺高血圧症または勃起不全を治療するための治療剤を製造するために使用されてよい。このとき、「生体適合性」という表現は、たとえば肺のような目標となる場所にある組織および細胞に対する適合性を意味する。
以下の実施例1および実施例2は、本発明に係る生体適合性ナノポリマー粒子の製造あるいは特性化を記述している。実施形態においては、PDE−5抑制剤であるシルデナフィルが活性成分として使用され、それに相応するように一種のPDE−5抑制剤に対する例としてみなされなければならない。本発明に係る生体適合性ナノポリマー粒子は、以下において簡単な形態として粒子とも称される。ポリ(D,L−ラクチド−コ−グリコライド)−コポリマー(PLGA)は、以下において簡単にポリマーとも称される。
1.実施例1
1.1 エマルション法および後続する蒸発法を利用した、請求項14による本発明に係る生体適合性ナノポリマー粒子の製造
本発明に係る生体適合性ナノポリマー粒子を、たとえば、室温において、当業者にとって公知となっているエマルション法および後続する溶媒蒸発法によって合成した。この目的のために、まず最初に、商業的に入手可能であり、かつ、たとえば、Boehringer Ingelheim(ドイツ・Ingelheim所在)のResomer(登録商標) RG502H、RG502、RG503HまたはRG504Hとして関連され得る1〜100g/lのポリ(D,L−ラクチド−コ−グリコライド)−コポリマー(PLGA)またはポリ(ビニルスルホネート−コ−ビニルアルコール)−グラフト−ポリ(D,L−ラクチド−コ−グリコライド)−コポリマー(P(VS−VA)−g−PLGA)を肺高血圧症または勃起不全を治療するための活性成分、たとえば、AK Scientific(米国・カリフォルニア州・Mountain View所在)から、遊離塩基として商業的に入手可能な、たとえば、PDE−5抑制剤であるシルデナフィルを1%〜20%添加した状態において、または添加していない状態において、たとえば、塩化メチレンのように水と混合され得ない溶媒内で溶解させた。本発明に係る生体適合性ナノ粒子の5%の活性成分の理論的な含量を得るために、使用されたポリマーを基準とした場合、活性成分を7%〜20%使用した。その次に、2mlの有機相(分散相)を、たとえば、1mMの4−(2−ヒドロキシエチル)ピペラジン−1−エタンスルホン酸によってpH−8に設定され、たとえば、ポリビニルアルコール(PVA)のような表面安定化剤を0.1〜15g/l分だけ含有する10mlの水相(定位相;constant phase)へと変換させた。PVAは、通常、Sigma−Aldrich(ドイツ・Steinheim所在)から、たとえば、Mowiol4−88(登録商標)として購買することができる。2つの相を混合した後に、乳剤を超音波で処理した。その後に続いて、前記有機相を、回転蒸発器を利用した溶媒蒸発法によって徐々に除去した。粒子を製造直後に使用した。代案的には、前記製造に続いて、本発明に係る生体適合性ナノポリマー粒子の噴霧乾燥が続く。この目的のために、本発明に係る生体適合性ナノポリマー粒子(0.2%〜2%)を濾過した後に、たとえば、ナノ−噴霧乾燥機B−90(Buchi, Flawil、スイス)のような噴霧乾燥機を利用して、製造業者の指示に応じて噴霧乾燥させた。
1.2 噴霧乾燥法を利用した、請求項15による本発明に係る生体適合性ナノポリマー粒子の製造
本発明に係る生体適合性ナノポリマー粒子を、たとえば、噴霧乾燥法を利用して製造した。この目的のために、商業的に入手可能であり、かつ、たとえば、Boehringer Ingelheim(ドイツ・Ingelheim所在)のResomer(登録商標) RG502Hとして関連され得る0.1%〜10%のポリ(D,L−ラクチド−コ−グリコライド)−コポリマー(PLGA)を肺高血圧症または勃起不全を治療するための活性成分、たとえば、AK Scientific(米国・カリフォルニア州・Mountain View所在)から、遊離塩基として商業的に入手可能な、たとえば、PDE−5抑制剤であるシルデナフィルを(使用されたポリマーを基準とした場合)1%〜20%添加した状態において、または添加していない状態において、たとえば、塩化メチレンのように水と混合され得ない溶媒内で溶解させた。その後に続いて、前記溶媒を濾過後に、たとえば、ナノ−噴霧乾燥機B−90(Buchi, Flawil、スイス)のような噴霧乾燥機を利用して、製造業者の指示に応じて噴霧乾燥させた。
2.実施例2
本発明に係る生体適合性ナノポリマー粒子の特性化
実施例1.1または1.2により製造された生体適合性ナノポリマー粒子を、以下における実施例2、ポイント2.1〜2.4に記載された方法および結果を利用して特徴化した。この目的のために、前記ナノポリマー粒子を製造直後に使用し、または、たとえば、Aerogen (Dangan, Galway、アイルランド)のAeroneb(登録商標) Professionalのような噴霧器を利用した噴霧直後に、製造業者の指示に従って使用した。
2.1 本発明に係る生体適合性ナノポリマー粒子の直径、サイズ分布および表面電荷
エマルション法および実施例1.1以下に記述されたところのような後続の溶媒蒸発法により製造される、製造されたばかりの生体適合性ナノポリマー粒子を、ポリマー濃度(5〜100g/l)およびPVA濃度(1〜50g/l)を多様に組み合わせた状態において、前記粒子の特性である直径、サイズ分布および表面電荷と関連して調査した。生体適合性ナノポリマー粒子の流体力学的な直径およびサイズ分布(多分散性−PDI;polydispersity)を動的光散乱法(DLS;dynamic light scattering)を利用して測定した。表面電荷のための尺度としてのゼータ電位は、たとえば、ゼータサイザーNanoZS/ZEN3600(ドイツ、Herrenberg, Malvern Instruments)を利用したレーザードップラー風力測定法(LDA;Laser Doppler Anemometry)によって決定される。すべての測定は、25℃の温度において部分標本として実施され、前記部分標本を、DLSのために濾過され再蒸留された水によって、あるいはLDAのための1.56mMのNaClによって希釈した。すべての測定を、生体適合性ナノポリマー粒子を製造した直後に、少なくとも10回反復される3回の決定として実施した。この場合、nは、以下において決定の回数を指示する。
2.2 本発明に係る生体適合性ナノポリマー粒子の安定化剤の層厚
本発明に係る生体適合性ナノポリマー粒子の安定化剤としての機能をする、吸着されたPVA層の厚さを、DLS−およびゼータ電位−測定を利用してポイント2.1以下に記述されたところのように、電解質−濃度の関数として決定した。適した決定方法は、当業者にとって公知となっている。DLS測定と関連して、前記吸着されたPVA層の厚さ(δ)は、下記一般式(1)によるコーティングされていない(d0)およびコーティングされた(dads)生体適合性ナノポリマー粒子の粒子サイズの比較から導出した。
2.3 本発明に係る生体適合性ナノポリマー粒子中に含まれる活性成分の含量
実施例1に従って製造された生体適合性ナノポリマー粒子のPDE−5抑制剤の含量を決定するために、たとえば、1mlの生体適合性ナノポリマー粒子の懸濁液を16873xgにおいて30分間、25℃で遠心分離させた。その後に続いて、上澄み液を慎重に除去し、そのうちカプセルに包まれていないPDE−5抑制剤の量を決定した。遠心分離から得られるペレットを冷凍乾燥し、計量した後に続き、たとえば、PLGAおよびシルデナフィルのための溶媒として適したクロロホルムの形態で溶解した。溶解されなかった部分(安定化剤)を遠心分離によって分離させた。その後に続いて、カプセルに包まれているPDE−5抑制剤の量を決定するために、有機相から部分標本を除去した。PDE−5抑制剤の濃度を分光光度計(たとえば、Ultrospec(登録商標)3000、Pharmacia Biotech、Freiburg、ドイツ)を利用するUV/Vis分光学によって決定した。すべての部分標本の吸着を、291nmの波長において測定した。生体適合性ナノポリマー粒子(PLGA−BNPP)内に含有されているPDE−5抑制剤(PDE5H)の含量を、検量線を参照して計算し、下記一般式(4)において規定した。
2.4 本発明に係る生体適合性ナノポリマー粒子からの活性成分の放出
活性成分PDE−5抑制剤を体外から(in vitro)放出させるための調査を、リン酸緩衝生理食塩水(phosphate−buffered saline)内において、たとえば、7.4のpH値で、500分間、37℃において実施した。本研究を、5%(w/w)の理論的なPDE−5抑制剤含量を有する生体適合性ナノポリマー粒子で実施した。前記生体適合性ナノポリマー粒子の懸濁液の部分標本をガラスチューブへ移し、リン酸緩衝生理食塩水(PBS)pH7.4+0.1%ナトリウムドデシルスルフェート(SDS)からなる媒質で希釈した。後続する培養は、37℃において、部分標本の攪拌下で行われた。実験的なアッセイ(experimental assay)に並行して、PDE−5抑制剤を同一条件下で媒質内において単独で培養した。部分量を事前に決定された時点で除去し、遠心分離した。PDE−5抑制剤の放出をKorsmeyer−Peppas方程式を参照して、下記一般式(5)

に基づいて算出し、前記式においてMt/M∞は、部分的な物質放出であり、tは放出時間であり、kは活性成分−ポリマー−システムに対して特徴的な運動定数であり、nは活性成分放出のメカニズムを特徴化する指数である。
Claims (21)
- 肺動脈高血圧症または勃起不全を治療するための製剤を製造するための生体適合性ナノポリマー粒子であって、
前記粒子は、生体適合性ポリマーおよび安定化剤および肺高血圧症または勃起不全を治療するための活性成分からなることを特徴とする、生体適合性ナノポリマー粒子。 - 前記生体適合性ポリマーは、ポリエステル、ポリアンハイドライド、ポリオルトエステル、ポリホスホエステル、ポリカーボネート、ポリケタール、ポリ尿素、ポリウレタン、ブロックコポリマー(PEG−PLGA)、星形ポリマーまたは櫛形ポリマーから選択されることを特徴とする、請求項1に記載の生体適合性ナノポリマー粒子。
- 前記ポリエステルは、ポリ(D,L−ラクチド−コ−グリコライド)−コポリマー(PLGA)であることを特徴とする、請求項2に記載の生体適合性ナノポリマー粒子。
- 前記櫛形ポリマーは、ポリ(ビニルスルホネート−コ−ビニルアルコール)−グラフト−ポリ(D,L−ラクチド−コ−グリコライド)−コポリマー(P(VS−VA)−g−PLGA)、またはスルホブチル−ポリビニルアルコール−グラフト−ポリ(ラクチド−コ−グリコライド)−コポリマー(SB−PVA−g−PLGA)であることを特徴とする、請求項2に記載の生体適合性ナノポリマー粒子。
- 前記安定化剤は、非イオン界面活性剤、陰イオン界面活性剤、両性界面活性剤またはポリマーグループから選択されることを特徴とする、請求項1に記載の生体適合性ナノポリマー粒子。
- 前記ポリマーは、ポリビニルアルコール(PVA)であることを特徴とする、請求項5に記載の生体適合性ナノポリマー粒子。
- 前記活性成分は、ホスホジエステラーゼ抑制剤(PDE抑制剤)グループまたはグアニル酸シクラーゼ活性化剤またはグアニル酸シクラーゼ刺激剤またはエンドセリン受容体拮抗剤またはプロスタノイドから選択されることを特徴とする、請求項1に記載の生体適合性ナノポリマー粒子。
- 前記PDE抑制剤は、PDE−5抑制剤シルデナフィルであることを特徴とする、請求項7に記載の生体適合性ナノポリマー粒子。
- 前記シルデナフィルは、遊離塩基として存在することを特徴とする、請求項8に記載の生体適合性ナノポリマー粒子。
- 前記生体適合性ナノポリマー粒子は、圧電、ジェット(jet)−、超音波−エアロゾル発生器、ソフト−ミスト−吸入器、定量吸入器または粉末式吸入器によって噴霧化され得ることを特徴とする、請求項1〜9のいずれか一項に記載の生体適合性ナノポリマー粒子。
- 前記生体適合性ナノポリマー粒子は、
−10nm〜10μmの直径を有し、
−0〜50nmの安定化剤層厚を有し、
−肺高血圧症または勃起不全を治療するための活性成分を0〜50%(w/w)含有し、そして
−前記活性成分を48時間までの時間にわたって放出することを特徴とする、請求項1〜10のいずれか一項に記載の生体適合性ナノポリマー粒子。 - 前記生体適合性ナノポリマー粒子は、特に好ましくは、より長期間の有効成分放出のために500nm〜5μmの直径を有し、または、前記粒子が大食細胞内部へ吸収されることを防止するために50nm〜250nmの直径を有することを特徴とする、請求項1〜11のいずれか一項に記載の生体適合性ナノポリマー粒子。
- 前記生体適合性ナノポリマー粒子は、特に好ましくは、肺高血圧症または勃起不全を治療するための活性成分を1〜20%(w/w)有することを特徴とする、請求項1〜12のいずれか一項に記載の生体適合性ナノポリマー粒子。
- 請求項1〜13のいずれか一項に記載の生体適合性ナノポリマー粒子を製造するための方法であって、
a)有機相を形成し、かつ、溶媒内において生体適合性ポリマーおよび活性成分を溶解させるステップ、
b)安定化剤を含有する水相において有機相を乳化させるステップ、
c)前記有機相および水相を混合するステップ、および
d)溶媒を除去し、懸濁液内において粒子を得るステップを含むことを特徴とする、生体適合性ナノポリマー粒子を製造するための方法。 - 請求項1〜13のいずれか一項に記載の生体適合性ナノポリマー粒子を製造するための方法であって、
a)有機相を形成し、かつ、溶媒内において生体適合性ポリマーおよび活性成分を溶解させるステップ、および
b)有機相を噴霧乾燥させるステップを含むことを特徴とする、生体適合性ナノポリマー粒子を製造するための方法。 - 請求項14または15による生体適合性ナノポリマー粒子を製造するための方法であって、
前記生体適合性ポリマーは、ポリエステル、ポリアンハイドライド、ポリオルトエステル、ポリホスホエステル、ポリカーボネート、ポリケタール、ポリ尿素、ポリウレタン、ブロックコポリマー(PEG−PLGA)、星形ポリマーまたは櫛形ポリマーであることを特徴とする、生体適合性ナノポリマー粒子を製造するための方法。 - 請求項14または15による生体適合性ナノポリマー粒子を製造するための方法であって、

前記活性成分は、ホスホジエステラーゼ抑制剤(PDE抑制剤)グループまたはグアニル酸シクラーゼ活性化剤またはグアニル酸シクラーゼ刺激剤またはエンドセリン受容体拮抗剤またはプロスタノイドであることを特徴とする、生体適合性ナノポリマー粒子を製造するための方法。 - 請求項1〜18のいずれか一項による生体適合性ナノポリマー粒子を、肺高血圧症を治療するための治療剤を製造するために使用する、生体適合性ナノポリマー粒子の利用。
- 請求項1〜18のいずれか一項による生体適合性ナノポリマー粒子を、勃起不全を治療するための治療剤を製造するために使用する、生体適合性ナノポリマー粒子の利用。
- 請求項19による生体適合性ナノポリマー粒子の利用において、
前記治療剤を、吸入法、滴下法、気管支鏡または呼吸装置を介して投与することを特徴とする、生体適合性ナノポリマー粒子の利用。
